Specie e soggetti

Species and subjects

New World camelids — and the breakdown between llamas and alpacas is not stated in the abstract → to be checked against the full text. The sample size is also absent from the abstract: the abstract describes samples («camelid and canine blood», «a camelid sample») and does not count animals.

A number exists, but it is reported by a third party: the review by Wagener MG, Marahrens H, Ganter M, Veterinary Research Communications 2024;48(2):633-647 (DOI 10.1007/s11259-023-10274-z, CC BY 4.0, full text read), writes that «Viesselmann et al. (2019a) studied RBC parameters in 25 SACs in an analyser (ADVIA 2120)». This record reports it as that source's reading, not as data read in the original, following the rule in chapter 14 §4.2 on second-hand citation.

It should be noted that canine blood also appears in the design, used as a method comparator for spherization: it is not an animal under study, it is a reagent control.

Study type and level of evidence

Analytical method comparison and validation study. PubMed classifies it as Comparative Study · Journal Article. The unit of analysis is not the animal, it is the measurement: the work does not describe diseases, does not follow outcomes, does not compare treatments.

Level of evidence: analytical validation study on samples, without a clinical control group and without a clinical outcome. Consequence for the reader: it establishes how to measure a camelid erythrogram on an ADVIA 2120; it does not establish what the value obtained means clinically, nor whether the proposed remedy improves a clinical decision — that is a different question, and no one has studied it yet.

Relevant reporting standard: no EQUATOR checklist covers analytical validation. CARE, STROBE, CONSORT, REFLECT and PRISMA are all out of scope; ARRIVE 2.0 would be relevant only if the work involved sampling performed for animal research purposes, and this cannot be verified without the full text → to be checked against the full text. Declared by the authors: not verifiable.

In breve

In brief

ADVIA hematology analyzers measure erythrocytes in two reactions: in one the cells must be made spherical isovolumetrically in order to be optically sized, in the other they must be lysed to measure hemoglobin. Camelid erythrocytes are elliptical and resist both spherization and osmotic lysis, and — the authors write — no published evaluation of ADVIA methods for camelids existed before this work.

The result is clear-cut: camelid erythrocytes do not spherize in the RBC/PLT reagent, but they do lyse regularly in the hemoglobin reagent. From this follows the consequence: hematocrit, MCV and MCHC returned by the instrument are significantly different from the reference methods, with all three species settings tested (equine, bovine, caprine). Only the RBC count with the equine setting turned out to be acceptably close to the reference method, and the MCV recalculated by hand from centrifuged PCV and equine count came back acceptable.

The authors do not stop at diagnosing the problem: they close with a line-by-line reporting procedure.

Why this study matters

Because it is the only published work that answers a question every Italian veterinarian asks the day they send the first alpaca EDTA sample to the lab: can I trust the sheet that comes back? The verified answer is: two of five values yes, three no, and the remedy does not require a new instrument — it requires a microhematocrit centrifuge and a calculator.

And it matters for a second, less obvious reason. In all the species the Italian clinician has trained on, a high MCHC is a signal of hemolysis — in vitro or in vivo. In the South American camelid the same value can be the reagent that did not do its job. Whoever does not know this chases a hemolysis that is not there, and — this is the costly part — chases it precisely in the most anemic animals, where the deviation is greatest.

Obiettivo

Dichiarato dagli autori: «to demonstrate whether camelid erythrocytes sphere in the ADVIA red blood cell/platelet (RBC/PLT) reagent and lyse in the ADVIA cyanide HGB reagent, and to determine optimal ADVIA settings for camelids».

Objective

Declared by the authors: «to demonstrate whether camelid erythrocytes sphere in the ADVIA red blood cell/platelet (RBC/PLT) reagent and lyse in the ADVIA cyanide HGB reagent, and to determine optimal ADVIA settings for camelids».

Question in PICO form (editorial formulation; the design allows it only by analogy): in a New World camelid blood sample [P], does the erythrocyte measurement of the ADVIA 2120 with the equine, bovine, or caprine species setting [I], compared with the reference methods — impedance counter, centrifuged PCV, calculated indices [C], return acceptably concordant values [O]?

Metodi / Protocollo

RBCcontatore a impedenza Z2 Coulter
HCTPCV centrifugato (packed cell volume)
MCVcalcolato: PCV × 10 / RBC Coulter
MCHCcalcolato: HGB × 100 / PCV

Methods / Protocol

From the structured abstract, which in this case is unusually detailed:

  1. Spherization test. Camelid blood and canine blood diluted 1:625 in the RBC/PLT reagent and evaluated under the microscope for erythrocyte spherization.
  2. Lysis test. A camelid sample incubated with the hemoglobin reagent at variable dilutions, to evaluate hemolysis.
  3. Method comparison. RBC, hematocrit (HCT), MCV and MCHC obtained with three ADVIA species settings — equine, bovine, caprine — compared with the respective reference methods:
ParameterReference method used by the authors
RBCCoulter Z2 impedance counter
HCTcentrifuged PCV (packed cell volume)
MCVcalculated: PCV × 10 / Coulter RBC
MCHCcalculated: HGB × 100 / PCV
  1. The reference MCV was also compared with the MCV calculated using the RBC count from the ADVIA's equine setting — and it is this comparison that produces the final recommendation.
  2. Statistics: Passing-Bablok regression and Bland-Altman difference plots. These are the right tools for a method comparison (neither assumes that one method is the truth).

Risultati principali (dati)

«Camelid ADVIA erythrogram results should be reported as follows: RBC counts and HGB concentrations using the equine setting, spun PCVs, MCVs calculated using the PCV and equine setting RBC, and MCHCs calculated using the PCV and equine setting HGB.»

In italiano, quattro righe:

Cosa si refertaCome
Conta eritrocitaria (RBC)ADVIA, impostazione equina
Emoglobina (HGB)ADVIA, impostazione equina

La rassegna di Wagener, Marahrens e Ganter (2024) riferisce che gli autori di questo lavoro osservarono «significant deviations in MCV and MCHC from the reference method in two animals with fatal anaemia (PCV: 0.07 l/l and 0.08 l/l)».

Main results (data)

  • Camelid erythrocytes did NOT spherize in the RBC/PLT reagent; they did, however, lyse in the hemoglobin reagent. The defect is therefore in only one of the two reactions, and it is the one on which optical sizing is based.
  • The RBC count with the equine setting was «acceptably close» to the count from the Coulter counter.
  • Hematocrit, MCV and MCHC from all three settings (equine, bovine, caprine) were found to be significantly different from the respective reference methods.
  • The recalculated MCV using the RBC count from the equine setting was «acceptably close» to the reference MCV.

What the abstract does NOT report, and which this record therefore does not assert: Passing-Bablok regression coefficients, mean bias and Bland-Altman limits of agreement, p-values, sample sizes per comparison, the numerical criteria used to define «acceptably close» → to be checked against the full text. Without those numbers, «significantly different» remains a direction, not a magnitude: the record cannot say by how much the instrument is off.

Authors' interpretation and recommendation (attributed, not merged with the data)

The authors close with a reporting procedure, which is the operational part of the work and must be quoted in full because it is the one thing that applies on Monday morning:

«Camelid ADVIA erythrogram results should be reported as follows: RBC counts and HGB concentrations using the equine setting, spun PCVs, MCVs calculated using the PCV and equine setting RBC, and MCHCs calculated using the PCV and equine setting HGB.»

In four lines:

What is reportedHow
Erythrocyte count (RBC)ADVIA, equine setting
Hemoglobin (HGB)ADVIA, equine setting
Hematocritcentrifuged PCV, not the HCT calculated by the instrument
MCVcalculated: PCV and RBC count with equine setting
MCHCcalculated: PCV and hemoglobin with equine setting

This is a recommendation by the authors, not a result from the data. The data demonstrate a discordance; the procedure is what the authors propose to do about it.

A datum reported by others, and how it should be treated

The review by Wagener, Marahrens and Ganter (2024) reports that the authors of this work observed «significant deviations in MCV and MCHC from the reference method in two animals with fatal anaemia (PCV: 0.07 l/l and 0.08 l/l)». This record has not read that passage in the original. It reports it as the review's reading, and it is useful because it indicates where the deviation is greatest: in severely anemic animals, i.e. precisely those in which a transfusion decision depends on the number.

Implicazioni cliniche

Clinical implications

For the Italian veterinarian who sends an alpaca or llama EDTA sample to the lab, today:

  1. Ask the lab which analyzer it uses and with which species setting it processed the sample. If the answer is «bovine setting» or «caprine setting», the three calculated values should be discarded, not discussed: on the ADVIA 2120 all three settings were significantly different from the reference for HCT, MCV and MCHC.
  2. Always ask for the centrifuged PCV, and use that one. It is the only hematocrit that this work supports for the camelid, and it is obtained with a capillary and a microcentrifuge.
  3. A high MCHC in a camelid is not, by itself, hemolysis. Before repeating the blood draw or hypothesizing a hemolytic anemia, one checks how the number was obtained. In the 2024 review cited above, the same authors recommend considering a microscopic erythrocyte count when the MCHC is markedly elevated — that is a recommendation from that review, not from this article.
  4. This work does not authorize anything for other analyzers. ADVIA 2120 is a specific instrument, with specific reagents. On a Sysmex, a Mindray, an IDEXX or an impedance counter the behavior is plausible but not measured here: whoever takes it for granted is extrapolating, and must write so in the report.
  5. There is no «camelid» setting. The authors tried the three that exist and had to build a hybrid pathway — one piece from the instrument, one piece from the centrifuge, two pieces from the calculator.

Limiti

Limitations

Declared by the authors: not verifiable. The structured abstract does not contain a limitations section, and the article is subscription-only with no deposit in PubMed Central → to be checked against the full text.

Added by the editors.

  • The result is tied to one instrument and two reagents. It holds for the ADVIA 2120; any extension to other platforms is a hypothesis.
  • The sample size and species breakdown are not in the abstract. If the camelids were, for example, almost all llamas, the work would say little about the alpaca — which is the prevalent species on Italian farms. This is not known, and cannot be inferred.
  • The reference method for the count is itself an instrument (Coulter Z2 impedance counter), not a microscopic chamber count. The agreement demonstrated is therefore between two automated methods, not between automated and manual count.
  • The calculated MCV and MCHC share the PCV in the denominator. This is an arithmetic dependency that makes the two indices not independent of each other: an error in the centrifuged PCV propagates to both. The work does not discuss this in the abstract.
  • No clinical outcome. The work does not demonstrate that reporting according to the proposed procedure changes a decision or an outcome. It is plausible, not measured.

Funding: not declared in the accessible record. Crossref registers no funder for this DOI; the Europe PMC funding list field is empty. This is not equivalent to «no funding» → to be checked against the full text.

Conflicts of interest: not verifiable for the same reason → to be checked against the full text. It must be said, as information and not as an accusation, that the work evaluates a commercial analyzer and concludes by indicating how to use it: it is precisely the type of article for which a conflicts declaration is decisive, and it is precisely the one that the accessible record does not allow one to read.

  • numerosità dei campioni e degli animali, e ripartizione per specie;
  • origine dei campioni (clinica? allevamento? animali sani o malati?) e criteri di inclusione;
  • coefficienti di Passing-Bablok (pendenza, intercetta, intervalli di confidenza) e bias e limiti di concordanza di Bland-Altman;
  • criterio numerico con cui è stato definito «acceptably close»;
  • approvazione etica e numero di protocollo, se i prelievi sono stati eseguiti a fini di ricerca;

◆ COMMENTO DELL'ASSOCIAZIONE

Camelid Research Studies APS — redazione CRL · 31/08/2026

Valutazione a stelle

AsseVotoMotivazione

What is missing relative to the reporting standard

Since no EQUATOR checklist exists for analytical validation, the list is built on the items that any reader of a method comparison looks for, and that are not in the accessible record:

  • sample size of samples and animals, and species breakdown;
  • origin of the samples (clinic? farm? healthy or diseased animals?) and inclusion criteria;
  • Passing-Bablok coefficients (slope, intercept, confidence intervals) and bias and limits of agreement from Bland-Altman;
  • the numerical criterion used to define «acceptably close»;
  • ethical approval and protocol number, if the sampling was performed for research purposes;
  • funding and conflicts of interest.

All of these items are, in all likelihood, present in the full text. This record has not seen them and does not assert them.

◆ ASSOCIATION COMMENTARY

Camelid Research Studies APS — CRL editorial staff · 31/08/2026

Of the five red-cell numbers, two are measured, one depends on the instrument, and two are always calculated.

The habit is to look at the CBC sheet and read the five red-cell numbers as if they were five measurements. They are not, in any species: two are measured (RBC, HGB), one is measured or calculated depending on the instrument (HCT), two are always calculated (MCV, MCHC). In the South American camelid this distinction, which elsewhere is a clinical pathologist's curiosity, becomes the difference between a useful report and a report that sends the clinician in the wrong direction.

What struck us is not the result, it is where the defect lies. Camelid erythrocytes lyse regularly in the hemoglobin reagent — so hemoglobin can be measured — but they do not spherize in the count reagent. It is only one of the two chemistries that fails, and it is the one on which the entire optical sizing rests. This is why the authors' remedy works: they do not discard the instrument, they keep the two measurements that hold up and reconstruct the other three. It is laboratory craftsmanship, and it is more solid than any «camelid setting» that no manufacturer has written.

Where we disagree, or rather: where we ask for more. The article stops short of the question that interests a breeder. No one has ever measured whether an erythrogram reported according to this procedure leads to different decisions — transfuse or not, deworm or not, hospitalize or not — compared with one reported by the instrument without corrections. It is a study that could be done in a single clinic, on current caseload, without touching one more animal: keep both reports and see where they diverge. This is a record that cannot yet be written, and it is among those we would most like to write.

Our operational position, for members. Ask the lab three things and put them in writing in the request: which analyzer, which species setting, centrifuged PCV. If the lab cannot answer the first two, the third is still enough to save the clinical decision, because capillary PCV does not depend on any reagent. And a high MCHC in an alpaca is treated as a hypothesis about the instrument until the instrument has been ruled out — not as a hypothesis about the animal.

One last note, concerning the method of this collection, not the article. The number «25 camelids» that circulates in connection with this work does not appear in the abstract: it comes from the 2024 review by the Hannover group, who read the full text. We have written it as such, reported and not read. It is a small discipline, and in a field where thirty articles cite one another it is the only thing that stops a number from becoming a fact through repetition.

Star rating

AxisScoreRationale
A · Scientific quality★★★☆☆Well-posed question, reference methods declared one by one, statistics appropriate for a method comparison (Passing-Bablok and Bland-Altman, which do not presuppose a «true» method), conclusion proportionate to the data. It does not rise to four because the accessible record lacks sample sizes, sample selection criteria, ethical statement, funding and conflicts — and because no agreement figure is verifiable: «significantly different» without bias and limits of agreement is a direction, not a measure.
B · Clinical utility★★★★☆The result is actionable on Monday morning and costs nothing: four lines of reporting, a capillary and a calculator. The conditions of use are explicit (ADVIA 2120, equine setting). It is not five because it holds for a specific analyzer, and because no one has measured whether it changes a clinical decision.
C · Educational value★★★★☆It teaches, with a reagent test and not with an assertion, the difference between a measured value and a calculated value, and shows that an «abnormal» index can be a property of the instrument's chemistry. It is coursework material for clinical pathology, not just a reference.
D · Species relevance★★★☆☆The study is done on camelids and arises from a real biological peculiarity of theirs — the elliptical erythrocyte resistant to spherization. But the breakdown between llama and alpaca is not stated in the accessible record, and transferability to the Italian laboratory depends on an analyzer that is not the most widespread. The CRL grid does not allow a fourth star to a study whose population is not described enough to assess its transferability.

No overall score, by construction. A = 3 and B = 4 are not a contradiction: an article whose reporting we cannot verify by half still hands us the most useful procedure in this whole issue.

Bottom line

One can conclude that camelid erythrocytes do not spherize in the ADVIA RBC/PLT reagent but do lyse regularly in the hemoglobin reagent, and that consequently on ADVIA 2120 hematocrit, MCV and MCHC are significantly different from the reference methods with all three species settings tested (equine, bovine, caprine); that only the RBC count with the equine setting and the recalculated MCV from centrifuged PCV and equine count came out «acceptably close» to the reference.

One cannot conclude from what this record has read by how much the instrument is off: the abstract does not report Passing-Bablok coefficients, Bland-Altman bias and limits of agreement, p-values, sample sizes per comparison, or the numerical criterion with which «acceptably close» was defined → to be checked against the full text. One cannot conclude that the result holds for the alpaca in particular: sample size and llama/alpaca breakdown are not stated in the accessible record, and the «25 camelids» that circulates is reported by the 2024 review, not read in the original. One cannot conclude anything about analyzers other than the ADVIA 2120, nor that reporting according to the four-line procedure changes a decision or a clinical outcome: that procedure is the authors' recommendation, not a result from the data, and the work has no clinical outcomes.

Messaggi da portare a casa

  4. Un MCHC alto in un alpaca va trattato prima come un sospetto sullo strumento, poi come un sospetto sull'animale.

Take-home messages

  1. On the ADVIA 2120, camelid hematocrit, MCV and MCHC are significantly different from the reference methods with all three available species settings (equine, bovine, caprine). It is not an imprecision: it is a chemistry that does not occur.
  2. The cause is precise and localized: camelid erythrocytes do not spherize in the RBC/PLT reagent, even though they lyse regularly in the hemoglobin reagent.
  3. The remedy is the authors' and is complete: RBC and hemoglobin with the equine setting, centrifuged PCV, MCV and MCHC calculated from that PCV and the equine values.
  4. A high MCHC in an alpaca should be treated first as a suspicion about the instrument, then as a suspicion about the animal.
  5. None of this is demonstrated for analyzers other than the ADVIA 2120, and whoever extends it must declare it as an extension.
