Journal of Veterinary Internal Medicine published by Wiley Periodicals LLC on behalf of American College of Veterinary Internal Medicine».

Licenza dell'articolo originale: Creative Commons Attribution-NonCommercial-NoDerivatives 4.0 International (CC BY-NC-ND 4.0), letta dentro il file del testo pieno (blocco blocco dei permessi: «This is an open access article under the terms of the http://creativecommons.org/licenses/by-nc-nd/4.0/ License, which permits use and distribution in any medium, provided the original work is properly cited, the use is non-commercial and no modifications or adaptations are made») e confermata su Crossref (due voci licenza, entrambe licenza CC BY-NC-ND 4.0) e su Europe PMC (licenza CC BY-NC-ND) il 31/08/2026.

OpenAlex (the OpenAlex record for this journal, id S69093139): 8,554 indexed works, open access, in the DOAJ directory. It is the journal of the American College of Veterinary Internal Medicine, i.e. the proper venue for veterinary internal medicine.

⚠️ A publisher discrepancy across the records, worth flagging to anyone citing the article. Crossref today lists the publisher as Oxford University Press (OUP); OpenAlex lists Wiley; the copyright statement inside the 2022 article reads: «© 2022 The Authors. Journal of Veterinary Internal Medicine published by Wiley Periodicals LLC on behalf of American College of Veterinary Internal Medicine». This is not an error by any of the three: it is a journal that changed publisher after this article was published, and the records were updated at different times. The publisher at the time of publication was Wiley on behalf of the ACVIM, and that is what should be written in a citation.

Licence of the original article: Creative Commons Attribution-NonCommercial-NoDerivatives 4.0 International (CC BY-NC-ND 4.0), read inside the full-text file (permissions block: «This is an open access article under the terms of the http://creativecommons.org/licenses/by-nc-nd/4.0/ License, which permits use and distribution in any medium, provided the original work is properly cited, the use is non-commercial and no modifications or adaptations are made») and confirmed on Crossref (two licence entries, both CC BY-NC-ND 4.0) and on Europe PMC (CC BY-NC-ND licence) on 31/08/2026.

⚠️ This licence is more restrictive than that of card CRL-A-02, and the difference matters. CC BY-NC-ND prohibits derivative works: figures and tables may be reproduced in full with attribution, but not modified, not cropped, not recomposed. This card therefore does not reproduce the article's tables: it extracts the data and reorganizes them into its own tables, which is citation of facts — data are not covered by copyright — and not a modified reproduction of someone else's table. This is a formal distinction and we state it so that anyone reusing this material knows where the boundary lies.

Specie e soggetti

Species and subjects

21 llamas and 33 alpacas, for 54 cases collected at the Veterinary Health Center of the University of Missouri between 1 January 2005 and 30 September 2021.

The denominator is present, and it is the best feature of this article's reporting. During the period examined, 1,314 camelids presented to the hospital: 919 alpacas (70%) and 395 llamas (30%). The 54 included are 4.1% (54/1,314); the 1,260 excluded (95.9%) did not have CSF data. Read by species: 21 of 395 llamas (5.3%) and 33 of 919 alpacas (3.6%) entered the study. A retrospective work that publishes its own flow diagram in numbers is rare, and should be acknowledged.

(data extracted from Table 1 of the article and reorganized; median [range] or n (%) as in the original)

Why the tap was performed. All 54 had CSF collected to rule out a differential diagnosis of neurologic disease. At presentation: 27/54 (50%) recumbent, 25/54 (46.3%) ataxic, 1/54 (1.85%) with head tilt, 1/54 (1.85%) with head tremors.

⚠️ A sample detail to keep in mind throughout this card: these are not 54 animals, they are 54 samples. The authors write that one alpaca had two CSF collections (March 2005 and November 2005) and that both samples were included separately, being distinct visits. However, the article continues to speak of «54 camelids included» and calculates all percentages on 54. The distinct animals are 53. The difference is small on the denominators and not trivial for inference: two observations from the same animal are not independent, and all tests used assume that they are.

Study type and level of evidence

Single-centre retrospective observational study on medical records, comparing two groups defined by outcome (survivors to discharge versus non-survivors). Stated by the authors in the Methods field of the abstract: «Retrospective study.» Europe PMC classifies it as a journal article.

Outcome definition, stated and clear-cut: survivor = discharged from the hospital; non-survivor = died or was euthanized during hospitalization. This is a binary definition, verifiable in the record and with no grey areas — and it is also, as will be discussed, the largest limitation of the work, because in a livestock species euthanasia is a decision of the owner, not only a clinical outcome.

Level of evidence: retrospective observational study, single site, 54 samples from 53 animals, short-term surrogate outcome, no randomization and no intervention. Consequence for the reader: the work establishes an association between a laboratory finding and an outcome observed at a Missouri hospital between 2005 and 2021. It does not establish that this finding causes the outcome, it does not produce a diagnostic test with sensitivity and specificity, and it does not say that modifying the finding changes the prognosis. It is an observed prognostic indicator, not a validated decision-making criterion.

Relevant reporting standard: STROBE, which covers retrospective observational studies. Stated by the authors: no. The article does not name any reporting checklist. It must however be acknowledged that it effectively follows the STROBE item most often neglected, the one on participant flow: 1,314 presented → 1,260 excluded for lack of CSF data → 54 included, with the breakdown by species on both denominators.

In breve

Parole loro: «We could find no research that specifically documented normal CSF results in alpacas.»

In brief

In the South American camelid, neurologic disease has, in North America, one protagonist: cerebrospinal nematodiasis due to Parelaphostrongylus tenuis, the «meningeal worm» that the camelid picks up from pasture shared with deer. It is a disease that progresses quickly and puts the owner in front of a rapid decision. CSF is easily collected from the lumbosacral space, often without sedation: the question of this study is whether what is read in the CSF helps to predict the outcome, not just to make a diagnosis.

The published answer is a single line: a total nucleated cell count (TNCC) below 3 cells/µl is associated with 3.9 times greater odds of survival (P = 0.04); total protein and eosinophil percentage are not associated with the outcome.

But this card was drafted from the full text and the tables, and must report two things that do not appear in the abstract and that change how that number should be used.

The first: the association is not found in either species taken separately. In llamas it goes in the expected direction but does not reach significance (odds 6.25; P = 0.07); in alpacas it is flat: the proportion with TNCC ≥ 3/µl is 72.7% among survivors (16/22) and 72.7% among non-survivors (8/11), with P = 1.0. Identical.

The second, which the authors state and which is the most useful finding of the work for an Italian reader: the CSF reference interval used in this species — TNCC < 3 cells/µl, protein < 45 mg/dl, eosinophils absent — comes from studies on llamas, and has been extended to the alpaca without verification. Their own words: «We could find no research that specifically documented normal CSF results in alpacas.»

To our knowledge, no studies have compared TNCC and survival status in camelids with neurologic disease.»

Why this study matters

Because it is, as far as this card has been able to verify, the first work relating a CSF finding to outcome in South American camelids — the authors write it: «To our knowledge, no studies have compared TNCC and survival status in camelids with neurologic disease.»

And it matters for a reason that goes beyond the CSF, one concerning the entire issue: it is documented proof that an alpaca reference interval may not exist at all, despite having been in use for thirty years. The reference cited by the authors for the normal composition of CSF is Welles EG, Pugh DG, Wenzel JG, Sorjonen DC, «Composition of cerebrospinal fluid in healthy adult llamas», Am J Vet Res 1994;55(8):1075-1079 — the title says llamas, and subsequent literature has applied it to alpacas. (Reference copied from the article's bibliography, entry 11; this card has not opened the 1994 original.)

It is exactly the same mechanism as card CRL-A-02, where the alpaca fructosamine reference interval is American, from 2011, with an unknown assay method. Two different cards, two different parameters, the same gap: the number measured on the species and on the continent where the work is done is missing.

Obiettivo

Dichiarato dagli autori: «Our objective was to determine CSF findings associated with survival or non-survival in South American camelids presented to a veterinary teaching hospital and that had neurologic disease included as a differential diagnosis based on history and physical examination findings.»

Ipotesi dichiarata a priori — e va citata per intero, perché contiene la soglia che poi verrà usata: «we hypothesized that camelids with a CSF TNCC of < 3 cells/μL, a CSF TPC of < 50 mg/dL, or < 10 % eosinophils noted on CSF or some combination of these findings would have higher odds of short-term survival than camelids with CSF results higher than these thresholds.»

Objective

Stated by the authors: «Our objective was to determine CSF findings associated with survival or non-survival in South American camelids presented to a veterinary teaching hospital and that had neurologic disease included as a differential diagnosis based on history and physical examination findings.»

A priori stated hypothesis — and it must be quoted in full, because it contains the threshold that will later be used: «we hypothesized that camelids with a CSF TNCC of < 3 cells/μL, a CSF TPC of < 50 mg/dL, or < 10% eosinophils noted on CSF or some combination of these findings would have higher odds of short-term survival than camelids with CSF results higher than these thresholds.»

⚠️ The thresholds do not match the references the authors themselves have just cited. Two paragraphs above the hypothesis, the introduction reports that previous studies give a normal TPC < 45 mg/dl; the hypothesis instead works on < 50 mg/dl. The discrepancy is not discussed and not justified → to be checked against the full text is not needed here, because the full text has been read: the justification simply is not there. On the eosinophil threshold, 10% is a choice of the authors: the value that the introduction cites from the literature as diagnostic of P. tenuis is > 17%.

Question in PICO form (editorial formulation): in a South American camelid hospitalized with neurologic disease in the differential diagnosis [P], does a CSF finding within thresholds (TNCC < 3/µl, TPC < 50 mg/dl, eosinophils < 10%) [I] compared to a finding beyond thresholds [C], associate with a higher probability of survival to discharge [O]?

Metodi / Protocollo

⚠️ Tutti i campioni sono stati comunque inclusi, contaminati o no, e gli autori dichiarano perché: «because of the common occurrence of blood contamination in CSF collection and the small sample size».

Gli autori dichiarano due programmi diversi: «Statistical analysis was performed using Fisher's exact tests using commercial software (Stata/IC 13.1; StataCorp LLC…)» e, subito dopo, «Data were analyzed using commercial software (Sigma Plot 14.0; Systat Software…) to determine P values».

Methods / Protocol

Data source. Retrospective review of electronic medical records from the Veterinary Health Center of the University of Missouri, from 1 January 2005 to 30 September 2021.

Inclusion criterion, one only and explicit: being a llama or alpaca and having had a CSF collection and analysis during hospitalization. No exclusion criterion stated beyond absence of the data.

Collection and analysis. CSF collected aseptically from the lumbosacral intervertebral space and placed in an EDTA tube for cytology and clinicopathological analysis. The analysis was performed by a board-certified clinical pathologist at the veterinary diagnostic laboratory of the same university. For each animal the following were recorded: TNCC, TPC, eosinophil percentage, and the presence or absence of blood contamination noted on the report.

Management of blood contamination — this is a piece of methodology worth copying, because it is the practical problem of every lumbosacral tap. Samples with reported contamination were re-examined for erythrocyte concentration: above 13,200 erythrocytes/µl it was noted that TNCC and TPC could be falsely increased (threshold taken from a clinical pathology textbook, bibliography entry 12: Rakich PM, Latimer KS, «Cytology», in Duncan and Prasse's Veterinary Laboratory Medicine Clinical Pathology, 5th ed., Wiley-Blackwell, 2011:331-364 — reference copied from the article's bibliography, not opened by this card). ⚠️ All samples were nonetheless included, contaminated or not, and the authors state why: «because of the common occurrence of blood contamination in CSF collection and the small sample size». This is a stated choice, not hidden, and it is debatable: it is discussed further below.

Statistics. Fisher's exact test, with significance threshold P ≤ 0.05. The authors state two different programs: «Statistical analysis was performed using Fisher's exact tests using commercial software (Stata/IC 13.1; StataCorp LLC…)» and, immediately after, «Data were analyzed using commercial software (Sigma Plot 14.0; Systat Software…) to determine P values». ⚠️ It is not clear which program produced which p-values, and the wording suggests that the tests and their p-values come from different software. It is not stated whether the tests were one-tailed or two-tailed, and no correction for multiple comparisons is stated — and there are at least eighteen in the work.

È l'unica inversione della tabella, e gli autori la segnalano esplicitamente: «non-surviving camelids had higher median values for TNCC, TPC, and percentage of eosinophils compared to surviving camelids, with the exception of percentage of eosinophils in llamas».

Il risultato dichiarato come principale.

  • Associazione significativa (P = 0,04) fra sopravvivenza a breve termine e TNCC < 3 cellule/µl in tutti i camelidi inclusi; odds di sopravvivenza 3,9 volte maggiori.
  • Nessuna associazione fra sopravvivenza e TPC < 50 mg/dl (P = 0,15) né eosinofili < 10 % (P = 1,0).

Contaminazione ematica.

  • Segnalata in 28/54 campioni (51,9 %): 9/28 lama (32,1 %) e 19/28 alpaca (67,9 %).

  • Sulla conclusione principale: «Our results suggest that a TNCC ≥ 3 cells/μL on CSF analysis is significantly associated with decreased odds of short-term survival in camelids.»
  • Sull'intervallo di riferimento, ed è la dichiarazione più importante dell'articolo: «Previous studies on normal CSF results in South American camelids have focused on llamas, with the results extrapolated to alpacas. We could find no research that specifically documented normal CSF results in alpacas.» E, di conseguenza: «It is possible that a higher association between CSF findings and survival was seen in llamas than alpacas because the previously established CSF reference ranges for these species were based on studies of llamas. Further research is needed to determine if and how these ranges differ in alpacas.»
  • Sul vantaggio di sopravvivenza dell'alpaca, gli autori non lo attribuiscono alla biologia: «these observations may reflect demographics and value perception of South American camelids favoring alpacas over llamas», e aggiungono che non è stato possibile distinguere le eutanasie decise per prognosi da quelle decise per mancanza di risorse economiche del proprietario.
  • Sull'uso del liquor normale: «obtaining normal results on CSF analysis may be helpful in localizing the source of clinical signs in camelids to systems other than the nervous system», con l'avvertenza che i falsi negativi non si possono escludere e che esistono malattie neurologiche del camelide — tossicosi, malattie metaboliche, lesioni traumatiche — in cui il liquor non cambia e serve l'imaging.
  • Chiusura, ed è misurata: «many diseases only can be diagnosed presumptively based on CSF analysis, and the animal's history, environment, and clinical presentation also must be considered when assessing prognosis.»

Main results (data)

(All the following numbers are extracted from the text and Tables 1-5 of the full text and reorganized into tables in this card. Denominators always explicit.)

Overall outcome.

  • 31/54 (57.4%) survived to discharge; 23/54 (42.6%) did not survive, of whom 20 were euthanized and 3 died during hospitalization.
  • Among survivors: 9/31 llamas (29%) and 22/31 alpacas (71%). Among non-survivors: 12/23 llamas (52.2%) and 11/23 alpacas (47.8%).
  • Odds of survival 2.66 times greater in alpaca compared to llama, P = 0.09: not significant at the stated threshold.
  • Length of hospitalization: survivors median 2.5 days (range 1-16); non-survivors median 4 days (1-11); all animals median 4 days (1-16).

Diagnosis.

  • 29/54 (53.7%) with presumptive diagnosis of P. tenuis infestation: 13/29 llamas (44.8%) and 16/29 alpacas (55.2%).

Two immediate readings of this table, which the abstract does not contain.

  1. The eosinophil median separates the two species drastically: 38% in llamas versus 0% in alpacas. This is not a detail: it means that in half of the alpacas in this case series eosinophils in the CSF were not present at all, even though 48.5% of them (16/33) had a presumptive diagnosis of P. tenuis.
  2. In surviving llamas the median eosinophil percentage (42.5%) is higher than in non- survivors (38%). This is the only reversal in the table, and the authors flag it explicitly: «non-surviving camelids had higher median values for TNCC, TPC, and percentage of eosinophils compared to surviving camelids, with the exception of percentage of eosinophils in llamas».

The result stated as the main one.

  • Significant association (P = 0.04) between short-term survival and TNCC < 3 cells/µl in all included camelids; odds of survival 3.9 times greater.
  • No association between survival and TPC < 50 mg/dl (P = 0.15) nor eosinophils < 10% (P = 1.0).
  • No correlation between species and exceeding the thresholds: TNCC (P = 0.63), TPC (P = 0.55), eosinophils (P = 0.30).

The same result, species by species — and this is where the picture changes:

SubgroupComparisonOdds reported by authorsP reported

Blood contamination.

  • Reported in 28/54 samples (51.9%): 9/28 llamas (32.1%) and 19/28 alpacas (67.9%).
  • 8/54 (14.81%) had erythrocytes > 13,200/µl, i.e. above the threshold beyond which TNCC and TPC can be falsely increased.
  • No significant correlation between species and contamination (P = 0.29), nor between species and erythrocytes > 13,200/µl (P = 0.39 and P = 0.55), nor between erythrocytes > 13,200/µl and survival status (P = 0.14).
  • Among camelids with erythrocytes ≤ 13,200/µl, the odds of survival were found to be 32.1 times greater in alpacas compared to llamas (P = 0.03). (On this number, see the critical note further below: it is arithmetically explicable and is not what it appears.)
  • Necropsy available for 10/28 (35.7%) of contaminated cases. Of these, 2/10 (20%) had spinal cord hemorrhage: one alpaca with hemosiderin in macrophages on histology, another with gross hemorrhage of the spinal cord. In those two animals the erythrocytes in the CSF were true hemorrhage, not collection-related contamination.
  • Necropsy available, for the whole case series, only for 9/54 (16.7%).

Authors' interpretation (attributed, not merged with the data)

  • On the main conclusion: «Our results suggest that a TNCC ≥ 3 cells/μL on CSF analysis is significantly associated with decreased odds of short-term survival in camelids.»
  • On the reference interval, and this is the article's most important statement: «Previous studies on normal CSF results in South American camelids have focused on llamas, with the results extrapolated to alpacas. We could find no research that specifically documented normal CSF results in alpacas.» And, as a consequence: «It is possible that a higher association between CSF findings and survival was seen in llamas than alpacas because the previously established CSF reference ranges for these species were based on studies of llamas. Further research is needed to determine if and how these ranges differ in alpacas.»
  • On the alpaca's survival advantage, the authors do not attribute it to biology: «these observations may reflect demographics and value perception of South American camelids favoring alpacas over llamas», and add that it was not possible to distinguish euthanasias decided for prognosis from those decided for lack of the owner's financial resources.
  • On the use of a normal CSF: «obtaining normal results on CSF analysis may be helpful in localizing the source of clinical signs in camelids to systems other than the nervous system», with the caveat that false negatives cannot be ruled out and that there are camelid neurologic diseases — toxicosis, metabolic diseases, traumatic lesions — in which the CSF does not change and imaging is needed.
  • Closing, and it is measured: «many diseases only can be diagnosed presumptively based on CSF analysis, and the animal's history, environment, and clinical presentation also must be considered when assessing prognosis.»

⚠️ Editorial arithmetic verification, and what does not add up

Chapter 14 §5.2 requires that the numbers of a card be verifiable. The editorial team recalculated the 2×2 tables and Fisher's exact tests from the published tables. The recalculation method validates itself, because it exactly reproduces several values from the article:

Quantity reported by the articleEditorial recalculation from the published tablesOutcome
Odds of survival alpaca vs llama = 2.66; P = 0.09OR 2.67; two-tailed Fisher P = 0.099reproduced
Llama, TNCC < 3/µl: odds 6.25OR 6.25reproduced
Llama, eosinophils < 10%: odds 7OR 7.00reproduced
Alpaca, TNCC < 3/µl: P = 1.0OR 1.00 (16/22 and 8/11: identical)reproduced
TPC < 50 mg/dl, all: P = 0.15Two-tailed Fisher P = 0.151reproduced
Erythrocytes ≤ 13,200/µl, alpaca vs llama: odds 32.1OR 32.14 — but only by applying a continuity correction of 0.5 to each cell, made necessary by an empty cell (no non-surviving alpaca in that stratum: 12 alive and 0 dead versus 3 alive and 4 dead)reproduced, and explained
TNCC < 3/µl, all: odds 3.9; P = 0.04from Table 3 (survivors 11 below threshold and 20 above; non-survivors 5 below and 18 above): OR 1.98, Fisher P = 0.37 two-tailed and P = 0.22 one-tailedNOT reproduced
P. tenuis, alpaca vs llama: odds 4.95; P = 0.05from Table 5 (alpaca 13 alive and 3 dead; llama 4 alive and 9 dead): OR 9.75, two-tailed Fisher P = 0.0095NOT reproduced
Eosinophils < 10%, all: P = 1.0Two-tailed Fisher P = 0.58NOT reproduced

What this card asserts and what it does not assert. It asserts that the article's main result — odds 3.9 and P = 0.04 — is not reproducible from the numbers of the article's own Table 3, whereas five other quantities are, some to the third figure. It does not assert that the result is wrong: the authors state they used two different statistical programs, one for the exact tests and one «to determine p-values», and do not state whether the tests were one-tailed or two-tailed. The discrepancy may arise from that, from a working table different from the published one, or from something else. It remains a fact that anyone wishing to reuse the number 3.9 cannot derive it from the published data → to be checked against the full text with the authors, not against the full text, which has been read in full.

And it remains a fact that does not depend on the recalculation, because it is written in Table 3 of the article: in alpacas the proportion with TNCC ≥ 3 cells/µl is 72.7% among survivors (16/22) and 72.7% among non-survivors (8/11). Whatever the correct overall odds ratio value may be, in the alpaca — the species Italy raises — that parameter, in this case series, distinguishes nothing.

Implicazioni cliniche

Clinical implications

For the Italian veterinarian facing a camelid with neurologic signs:

  1. CSF is collected, and it is collected from the lumbosacral space. In this case series the procedure was performed on 54 samples over sixteen years with no complication reported; the article's introduction notes that in ruminants field collection with local anesthesia has not produced sequelae. However, the complication rate is not reported in this study → to be checked against the full text: absence of mention is not absence of events.
  2. EDTA tube, and the sample must be read by someone who knows how to read it. Here the analysis was performed by a board-certified clinical pathologist: a CSF cell count and a differential formula are not an examination to be performed in an outpatient clinic.
  3. A high TNCC is a negative prognostic signal, but it is a population-level signal, not an individual verdict. In this case series 38 of 54 camelids (70.4%) had TNCC ≥ 3/µl, and 31 of those 54 survived. A parameter present in seven of ten animals does not, by definition, discriminate well.
  4. In the alpaca, in this case series, TNCC did not separate survivors from non- survivors. Anyone working with alpacas needs to know this before using the number in a prognostic conversation with the owner.
  5. The CSF reference interval used for the alpaca is that of the llama, and no one has ever measured it in the alpaca. This is a statement by the authors, not an inference. Anyone reporting «CSF within normal limits» in an alpaca is applying a reference from another species, and the report should say so.
  6. Eosinophils in the CSF behave very differently in the two species, at least in this North American case series dominated by P. tenuis: median 38% in llamas and 0% in alpacas. A CSF without eosinophils in an alpaca does not rule out cerebrospinal nematodiasis.
  7. Beware of transferring this study's epidemiology to Italy. More than half of the cases (29/54) are P. tenuis nematodiasis, a disease linked to the North American white-tailed deer and to pasture shared with it. Italian prevalence is not the subject of this study and this card does not know it. What transfers is the method (collect, count, interpret with caution) and the gap (the alpaca reference interval); not the list of diagnoses.
  8. Blood contamination is the rule, not the exception: 28 of 54 samples (51.9%). Above 13,200 erythrocytes/µl cell count and protein can be falsely increased, and in this study 8 of 54 samples (14.81%) were above it. The report must be read with this in mind.
  9. But erythrocytes in the CSF may also not be contamination: they may be spinal cord hemorrhage. In 2 of the 10 contaminated animals subjected to necropsy this was the case, and in one case the sign was hemosiderin inside macrophages — i.e. a finding that distinguishes old blood (pathology) from freshly aspirated blood (artifact). It is a question to ask the cytologist, and the authors state that in their case series that information was not systematically available.
  10. A normal CSF is information, not a failure. In the 7/54 (13%) with normal CSF the final diagnosis was non-neurologic: the CSF helped to shift the search elsewhere.

Limiti

Dichiarati dagli autori (citati, non riassunti):

  • Numerosità piccola, indicata come «a major limitation of our study», che ha limitato l'analisi statistica.
  • Natura retrospettiva, che ha impedito di analizzare altri fattori prognostici: «duration of recumbency, and presence and severity of comorbidities such as dehydration, electrolyte abnormalities, anemia, and secondary infections».
  • Impossibilità di distinguere le ragioni dell'eutanasia: «Our study did not take into account the rationale given by owners who elected euthanasia, and cases in which camelids were euthanized because of a lack of financial resources to continue treatment could not be differentiated from cases in which euthanasia was elected because of poor prognosis or quality of life concerns.»
  • Contaminazione ematica frequente, che «could have masked the effect of TNCC, TPC, and percentage of eosinophils on survival to discharge», e che è difficile da distinguere dall'emorragia da migrazione larvale.

«No funding was received for this study.»

«Authors declare no conflict of interest.» L'articolo contiene anche una dichiarazione, propria del JVIM, sull'uso extra-etichetta di antimicrobici: «Authors declare no off-label use of antimicrobials.»

Approvazione etica: dichiarata, nella forma propria degli studi su cartelle: «Approval for use of data in medical records was granted by the University of Missouri Veterinary Health Center.» Dichiarato inoltre: «Authors declare human ethics approval was not needed for this study.»

Limitations

Stated by the authors (quoted, not summarized):

  • Small sample size, described as «a major limitation of our study», which limited statistical analysis.
  • Retrospective nature, which prevented analysis of other prognostic factors: «duration of recumbency, and presence and severity of comorbidities such as dehydration, electrolyte abnormalities, anemia, and secondary infections». The authors explicitly call for a multicentre study on a larger population.
  • Impossibility of distinguishing the reasons for euthanasia: «Our study did not take into account the rationale given by owners who elected euthanasia, and cases in which camelids were euthanized because of a lack of financial resources to continue treatment could not be differentiated from cases in which euthanasia was elected because of poor prognosis or quality of life concerns.»
  • No long-term survival data, deemed impractical to collect years later: it is not known how long discharged animals lived, nor whether they retained persistent neurologic deficits — which in livestock species increase the risk of culling.
  • Presumptive diagnosis of P. tenuis only: the only definitive method is finding larvae at necropsy, and false negatives are possible even at necropsy. Necropsy was available only for 9/54 (16.7%).
  • Frequent blood contamination, which «could have masked the effect of TNCC, TPC, and percentage of eosinophils on survival to discharge», and which is difficult to distinguish from hemorrhage due to larval migration.
  • Absence of normal data in the alpaca, discussed above.

Added by the editorial team.

  • The main result is not reproducible from the published data (see the arithmetic verification). This is the most serious limitation, and it is not stated.
  • The aggregate result does not hold in either species separately, and in the alpaca it is exactly null. An association that appears only when aggregating two species with different baseline outcomes (57.4% overall survival, but 22/33 in alpacas versus 9/21 in llamas) should be viewed with suspicion, because species is simultaneously associated with the outcome and distributed differently across the parameter's levels. The article performs no stratified or adjusted analysis: it uses only 2×2 Fisher tables.
  • The «32.1 times» number should not be cited. It is an odds ratio calculated on a table with one empty cell and made finite by a continuity correction: if in one stratum no alpaca dies, the true odds ratio is infinite and the estimate is governed by the correction, not by the data. The editorial team verified this arithmetically (0.5 per cell reproduces 32.14). With 15 alpacas and 7 llamas in that stratum, that number does not measure a difference between species: it measures an empty cell.
  • 54 samples, 53 animals. One alpaca appears twice and the tests assume independent observations.
  • At least eighteen Fisher tests with no correction whatsoever for multiple comparisons, and without stating whether they are one-tailed or two-tailed. With this number of tests, a P around 0.04-0.05 is not proof: it is a candidate.
  • No count of tap complications. The work implicitly supports the feasibility of the procedure, but does not report a denominator of adverse events.
  • The outcome is discharge, and in a livestock species discharge is as much an economic decision as a clinical one. The authors state this, and it is, in our view, the limitation that most prevents reading «survival» as «biological outcome»: 20 of the 23 non-survivors were euthanized. The parameter might be associated not with death, but with the owner's decision — which the clinician makes based partly on reading, among other things, that very report. It is a closed loop and no retrospective design can open it.
  • Sixteen years of case series at a single site. In 2005 and in 2021 cells are not counted with the same instruments, nematodiasis is not treated the same way, and euthanasia is not decided with the same criteria. No period analysis is presented.
  • Non-transferable epidemiological context. P. tenuis dominates the case series and is a North American disease.

Funding: stated, and it is absent. «No funding was received for this study.» Crossref records no funder and the funding list field of Europe PMC is empty, consistently. The authors thank Justin Cermak of the University of Missouri for record collection.

Conflicts of interest: stated. «Authors declare no conflict of interest.» The article also contains a statement, standard for JVIM, on off-label use of antimicrobials: «Authors declare no off-label use of antimicrobials.»

Ethical approval: stated, in the form proper to record-based studies: «Approval for use of data in medical records was granted by the University of Missouri Veterinary Health Center.» No IACUC protocol number, which is consistent with a retrospective study on clinical data with no procedures on animals. Also stated: «Authors declare human ethics approval was not needed for this study.»

◆ COMMENTO DELL'ASSOCIAZIONE

Camelid Research Studies APS — redazione CRL · 31/08/2026

We could find no research that specifically documented normal CSF results in alpacas.»

Valutazione a stelle

AsseVotoMotivazione
«We could find no research that specifically documented normal CSF results in alpacas.»

What is missing relative to the reporting standard (STROBE)

  • It is not stated whether the Fisher tests are one-tailed or two-tailed, nor which of the two programs cited produced which p-values. Without this, the p-values are not reproducible — and indeed they were not.
  • There is no confidence interval for any odds ratio. With groups of 9, 11, 12 and 22 animals, the intervals would be very wide, and their absence makes estimates appear point-precise when they are not.
  • No correction for multiple comparisons, nor any statement that a choice was made not to do so.
  • No adjusted or stratified analysis accounting for species, even though species is the variable the authors themselves point to as a possible explanation for everything.
  • The 2×2 tables are not published: percentages and odds ratios are published, and the tables must be reconstructed. This is the reason the discrepancy on the main result only emerged from recalculation.
  • No attempt to estimate the size of the exclusion effect: 1,260 of 1,314 camelids did not have a CSF collection. Those who had a CSF collection are, by construction, those investigated more.
  • The operational definition of «presumptive diagnosis» is missing: who formulated it, with what criteria, blinded to the outcome or not.
  • The count of procedural complications is missing.
  • The collection date relative to hospitalization is missing: CSF collected on admission and one collected on day four do not carry the same prognostic significance.

◆ ASSOCIATION COMMENTARY

The result is TNCC below 3 cells/µl, with odds of survival 3.9 times greater (P = 0.04). There is a sentence in the discussion that carries just as much weight.

The result is the line everyone will cite: TNCC below 3 cells/µl, odds of survival 3.9 times greater, P = 0.04. The valuable thing is a sentence in the discussion: «We could find no research that specifically documented normal CSF results in alpacas.» The authors, trying to explain why their association worked in llamas and not in alpacas, went to look for where the number «fewer than 3 cells per microliter» comes from — and found that it comes from a 1994 study titled on llamas. For thirty years alpaca medicine has been counting CSF cells against a yardstick that no one has ever calibrated on the alpaca.

This is the third time in four cards that we have encountered the same shape of gap, and at this point it is not a coincidence: in card CRL-A-01 the alpaca erythrogram is read against an equine species setting; in CRL-A-02 alpaca fructosamine is read against an American interval from 2011 with an unknown assay method; in CRL-A-03 the peritoneal fluid of the alpaca rests on five animals within a case series of twenty-two. Now the alpaca's CSF is the llama's CSF. Alpaca laboratory medicine is, to a large extent, someone else's laboratory medicine with a relabeled tag. It is no one's fault: it is what happens to a species that arrived in the clinic before it arrived in the laboratory. But it must be said out loud, because it is the single fact that most changes the way an Italian veterinarian should read an alpaca report.

On the main result, however, we must be blunt, because our rule obliges us to count. We reconstructed the 2×2 tables from Table 3 of the article and redid the tests. The method works: it reproduces the 2.66 odds ratio and its P = 0.09, the llamas' odds of 6.25, the eosinophils' odds of 7, the alpacas' P = 1.0, the protein's P = 0.15 and — with the continuity correction — even the famous 32.1. But it does not reproduce the 3.9 and the P = 0.04. From Table 3 we get an odds ratio of 1.98 and a P of 0.37. We are not saying that the authors made a mistake: the article states two different statistical programs, one for the tests and one «to determine p-values», and does not state whether the tests are one-tailed or two-tailed — there is room for the difference to have an explanation we do not know. We are saying something simpler and more uncomfortable: the number that will be cited cannot be derived from the published numbers. In a field where thirty articles cite one another, this matters.

And there is a fact that does not depend on any recalculation, because it is printed in their Table 3. In alpacas, the proportion of animals with TNCC ≥ 3 cells/µl is 72.7% among survivors and 72.7% among non-survivors. Identical. Whatever the overall odds ratio says, in the species Italy raises, that parameter, in this case series, separated nothing. A colleague who used it in a prognostic conversation with an alpaca's owner would be using a number built, largely, on llamas.

One last observation on the number 32.1, because it is the kind of figure that enters presentations and never leaves. In that stratum, no alpaca died: twelve alive and zero dead, against three llamas alive and four dead. With a zero in the table the odds ratio is infinite, and the 32.1 emerges only by adding half an animal to each cell — we verified it, the 0.5 correction returns exactly 32.14. It is not a measure of the difference between alpaca and llama: it is the measure of an empty cell out of twenty-two animals. We ask that it not be cited, and we write it here because this is the kind of thing a card must do and an abstract never will.

Where we dissent from the design, and not only from the arithmetic. The authors chose discharge as the outcome. It is verifiable, and it is the forced choice of a retrospective study. But twenty of the twenty-three non-survivors were euthanized, and the authors honestly state that they cannot distinguish euthanasia decided for prognosis from that decided for costs. The CSF report is one of the elements on which the clinician builds the prognosis communicated to the owner, and the owner decides. The study's result is therefore in part a measure of how reports influence human decisions, not only of how cells predict biology. This is not a defect that more animals can fix: it is a defect fixable only by changing the outcome — survival at thirty or ninety days, or residual neurologic deficits.

Where we ask for more, and this time the list is short and precise. First: the reference interval for CSF in the alpaca, measured on healthy European alpacas, with collection technique and counting method stated. It is the same study we called for in CRL-A-02 for fructosamine and in CRL-A-03 for peritoneal fluid, and it is beginning to look to us like the missing work of the whole of European alpaca laboratory medicine: it costs little, carries no risk of a null result, and every card we write calls for it again. Second: **the 2×2 tables and

confidence intervals**, in this and in any similar study. It is not pedantry — it is the difference between a number that can be checked and a number that must be trusted.

Our operational position, for members. CSF in the camelid with neurologic signs is collected: it is simple, it is from the lumbosacral notch, it goes into EDTA and must be read by a clinical pathologist. The report is read with four caveats attached: (1) the reference interval is the llama's; (2) blood contamination is expected (in this case series one sample in two) and above 13,200 erythrocytes/µl it inflates cells and protein; (3) erythrocytes in the CSF can be true spinal cord hemorrhage — ask the cytologist whether there is hemosiderin or erythrophagocytosis; (4) in the alpaca, eosinophils in the CSF can be zero even in the presence of parasitic disease of the nervous system. And a normal CSF is not a wasted examination: in seven of fifty-four animals it shifted the diagnosis away from the nervous system, which is precisely what it is done for.

Star rating

AxisScoreRationale
A · Scientific quality★★★☆☆Observational study with a stated n and published population denominator (54 of 1,314 presented, broken down by species), explicit inclusion criterion, verifiably defined outcome, stated approval, stated conflicts, stated (absent) funding, extensively and self-critically discussed limitations — including one that undermines its own thesis (euthanasia for economic reasons). It is fully at the ★★★☆☆ level of the grid. It does not rise to four, and the reason is precise: the analysis is a series of at least eighteen 2×2 tables with no confidence intervals, no correction for multiple tests, no stated test sidedness, and no stratification, even though species is associated with both the parameter and the outcome; and the main result is not reproducible from the published tables. A work with exemplary declaration reporting and incomplete statistical reporting remains at three.
B · Clinical utility★★★☆☆Offers a replicable procedure described in sufficient detail — lumbosacral tap, EDTA, reading by a clinical pathologist, contamination threshold at 13,200 erythrocytes/µl — to be used «with the caution imposed by the level of evidence», which is the definition of level three. It does not reach four because the decisive condition of use is missing and not stated by the authors: the prognostic number does not hold in the species of greatest interest here, and the case series is dominated by a disease that is not documented in Italy by this card. What one takes home is the procedure and the caveats, not the threshold.
C · Educational value★★★★★Five stars for one reason alone, and it is worth more than the content: it is the article in which the camelid literature admits in writing that one of its reference intervals does not exist. «We could find no research that specifically documented normal CSF results in alpacas.» It teaches, with a concrete and verifiable case, what it means for a «normal» value to be an inherited convention; it teaches, with Table 3, that an aggregate association can vanish within every subgroup; and it teaches — anyone who redoes the arithmetic — why 2×2 tables should be published. Anyone who needs to learn to read a veterinary prognostic study critically can do the entire exercise on this article, which is moreover freely accessible.
D · Species relevance★★★★☆A study on camelids, with the two species kept distinct in every table — something CRL-A-01 could not do — population described by species, age, sex, reason for presentation, length of hospitalization, and hospital denominator: transferability is assessable, which is the criterion for level four. It does not reach five because the epidemiological context is not ours: half the case series is P. tenuis nematodiasis, a disease linked to the North American deer, and because the work is from 2022 with two citations recorded on Europe PMC as of 31/08/2026 — it is not (yet) a reference study taken up by subsequent literature.

No overall score, by design. A = 3 and C = 5 are not a contradiction, and this is the second time in this issue: an article whose result does not survive recalculation can contain the most important observation we have read on alpaca laboratory medicine.

Bottom line

It can be concluded that in 54 CSF samples from 53 camelids (21 llamas and 33 alpacas) at a single Missouri hospital, 2005-2021, the authors report an association between TNCC < 3 cells/µl and survival to discharge (odds 3.9; P = 0.04), while total protein (P = 0.15) and eosinophils (P = 1.0) are not associated with the outcome; that blood contamination is the rule (28/54, 51.9%, with 8/54 above 13,200 erythrocytes/µl) and that in 2 of the 10 contaminated animals subjected to necropsy the erythrocytes were true spinal cord hemorrhage; that eosinophils in the CSF separate the two species (median 38% in llamas, 0% in alpacas) and that a normal CSF shifted the diagnosis away from the nervous system in 7/54 (13%). And it can be concluded, because the authors state it, that the alpaca's CSF reference interval does not exist: «We could find no research that specifically documented normal CSF results in alpacas» — the one in use comes from a 1994 study on llamas.

It cannot be concluded that the 3 cells/µl threshold is usable in the alpaca: in Table 3 of the article the proportion with TNCC ≥ 3/µl is 72.7% among survivors (16/22) and 72.7% among non-survivors (8/11), P = 1.0, and in llamas the effect does not reach significance (odds 6.25; P = 0.07): the association appears only when aggregating two species with different baseline outcomes, with no stratified analysis. Nor can the number 3.9 with P = 0.04 be considered verified: this editorial team, which from the same tables exactly reproduces five other values of the article, derives from them OR 1.98 and P = 0.37 → to be checked against the full text with the authors. It cannot be concluded that «survival» means biological outcome: the outcome is discharge and 20 of the 23 non-survivors were euthanized, with no way to distinguish euthanasia for prognosis from that for costs — and no long-term data exists. The «32.1 times» figure cannot be cited in favor of the alpaca: it arises from an empty cell and a continuity correction of 0.5. Left out: confidence intervals (none), correction for at least eighteen multiple tests, test sidedness, tap complications, and the transferability of a case series dominated 53.7% by P. tenuis, a North American deer disease.

Messaggi da portare a casa

Gli autori lo dichiarano: «We could find no research that specifically documented normal CSF results in alpacas.»

Take-home messages

  1. The CSF reference interval used for the alpaca is that of the llama. The authors state it: «We could find no research that specifically documented normal CSF results in alpacas.» The original reference is a 1994 study on healthy llamas.
  2. The stated result is: TNCC < 3 cells/µl, odds of survival to discharge 3.9 times greater (P = 0.04) on 54 samples. This card was unable to reproduce it from the published tables, which give an odds ratio of 1.98 and P = 0.37 — while the same recalculation exactly reproduces five other values from the article.
  3. In alpacas the parameter is flat: TNCC ≥ 3/µl in 72.7% of survivors (16/22) and in 72.7% of non-survivors (8/11). In llamas it goes in the expected direction but without significance (odds 6.25; P = 0.07).
  4. Eosinophils in the CSF behave differently in the two species: median 38% in llamas, 0% in alpacas, in a case series in which half the animals had a presumptive diagnosis of cerebrospinal nematodiasis. Zero eosinophils in an alpaca does not rule out the disease.
  5. Blood contamination is to be expected: 28 of 54 samples (51.9%); above 13,200 erythrocytes/µl cell count and protein can be falsely increased (8/54, 14.81%). But in 2 of 10 contaminated animals with necropsy the erythrocytes were true spinal cord hemorrhage: ask the cytologist whether there is hemosiderin or erythrophagocytosis.
  6. A normal CSF is a useful result: in 7/54 (13%) it directed the diagnosis away from the nervous system.
  7. The «32.1 times» in favor of the alpaca should not be cited: it arises from a table with an empty cell and a continuity correction of 0.5 per cell, arithmetically verified by this editorial team.
  8. The measured outcome is discharge, and 20 of the 23 non-survivors were euthanized, with no way to distinguish euthanasia for prognosis from that for costs. «Survival» here is in part a human decision.
