Editorial note on the publisher, not on the article: MDPI is an open-access publisher with short review times, and is the subject of debate over the robustness of peer review. The card flags this because it is contextual, and because in this article one can see a concrete trace of it: in the «Informed Consent Statement» section the template instruction from the editorial model was left printed — «Please write this part according to the instruction.» — followed by the actual sentence. A harmless typo, but one that proofreading would have removed.

Specie e soggetti

Dichiarato dagli autori: no.

Study type and level of evidence

Retrospective case series, n = 3, with prospective renal functional investigation in two of the three animals and necropsy in all three. Europe PMC classifies the article as a research article + journal article; PubMed does not assign it the Case Reports descriptor — which is, in the editorial team's judgment, a debatable classification, because the design is exactly that.

Level of evidence: case series without a comparison group. Consequence for the reader: this work documents what a ~40× overdose does in three crias; it does not establish a toxic threshold dose, an incidence, or why three out of twelve became ill while nine did not.

Relevant reporting standard: CARE (adapted to the series). Declared by the authors: no.

In breve

In brief

In October 2016, on a German farm with 55 alpacas, the attending veterinarian decided to supplement all twelve crias of the herd with vitamin D3 — a correct practice recommended at central-European latitudes. To calculate the dose, they consulted a chapter of a scientific manual. In that chapter the units of measure were swapped: 1 µg of cholecalciferol corresponds to 40 IU, and the confusion between the two scales produced a dose about forty times the intended one. Each cria received, in a single injection, between 1,000,000 and 2,000,000 IU of vitamin D3.

Seven to twelve days later, three crias fell ill with non-specific signs — weakness, cough, inappetence, and in one case anuria. Fifteen days after the injection they arrived at the clinic with very severe azotemia (creatinine up to 2,905 µmol/L, urea up to 140.3 mmol/L in cria 3), hypercalcemia, hyperphosphatemia and 25-hydroxycholecalciferol beyond the laboratory's upper measurement limit (> 3,750 nmol/L, reference 50-200). Renal function analysis in the two crias who survive to arrival shows an endogenous creatinine clearance of 0.08 and 0.17 mL/min/kg against a (equine) reference of 1.24-2.59: the kidney no longer filters. All three die within 23 days of the injection. Necropsy finds calcification of the cardiac atria, kidneys, lungs, liver and third gastric compartment, confirmed by von Kossa staining.

The other nine crias, treated with comparable doses, did not become ill.

Il veterinario curante — gli autori lo scrivono espressamente — «followed the recommendations of the scientific literature».

Why this study matters

Because it is a description, with numbers, of an incident that is not technically difficult to repeat and whose cause is one none of us considers a risk: the conversion of a unit of measure.

Vitamin D is expressed in micrograms or in International Units, and the conversion factor is 40 (1 µg cholecalciferol = 40 IU). It is the only vitamin in current use where a unit error does not produce a discrepancy of 10 or 20 percent, but a factor of forty. For a water-soluble vitamin, a factor of forty ends up in the urine. For a fat-soluble vitamin with a long half-life, injected all at once into a fourteen-kilogram animal, a factor of forty is a death sentence with a fifteen-day latency.

And there is a second reason, the one that makes this article uncomfortable: the error was in a manual. The attending veterinarian — the authors state this explicitly — «followed the recommendations of the scientific literature». They did not improvise, did not guess: they did what all of us are taught to do, and the source was wrong.

Obiettivo

Dichiarato dagli autori: «Therefore, the present study investigates severe cases of iatrogenic vitamin D3 intoxication in three alpaca crias, with a special focus on changes in kidney function.» Gli autori motivano lo scopo con una lacuna esplicita: «there is a lack of available data concerning the impact of vitamin D3 intoxication on the functional aspects of kidney parameters in SACs».

Objective

Stated by the authors: «Therefore, the present study investigates severe cases of iatrogenic vitamin D3 intoxication in three alpaca crias, with a special focus on changes in kidney function.» The authors justify the aim with an explicit gap: «there is a lack of available data concerning the impact of vitamin D3 intoxication on the functional aspects of kidney parameters in SACs».

Clinical question in PICO form (editorial formulation; the design allows this only in part): in alpaca crias who received a parenteral overdose of vitamin D3 [P], what alterations of renal function [I], compared with available reference ranges — from adult alpacas, llamas, horses [C], occur, and with what outcome [O]?

Disegno dello studio

Study design

Retrospective history-taking of the exposure (reconstructed from the owner's statements and from the treatments given by the colleagues who had visited previously), clinical and laboratory examination of the three crias on admission, prospective renal functional investigation (endogenous creatinine clearance, fractional excretions, serum and urine electrophoresis) on the two crias who survive long enough to be sampled, necropsy with histology and histochemistry on all three. No comparison group. The nine cohabiting animals that were treated and remained healthy were not sampled.

Metodi / Protocollo

Methods / Protocol

  • Hematology and clinical chemistry on jugular blood (EDTA, lithium-heparin, serum; Sarstedt Monovette tubes), using the routine methods of the Hannover clinic's clinical laboratory.
  • Venous blood gas analysis on all three animals (Osmetech OPTI™ CCA) for pH and ionized calcium.
  • Renal function analysis based on endogenous creatinine clearance, with simultaneous collection of venous blood and urine by cystocentesis, according to methods published for horse, pig and calf; fractional excretion was calculated assuming free glomerular filtration of creatinine, no tubular secretion or reabsorption, and a constant creatinine excretion of 0.17 µmol/min/kg — the horse value, following Bickhardt's method.
  • Electrophoresis of serum and urine on cellulose acetate membrane (Elphoscan-MINI PLUS, 105 V; 40 µL serum, 25 µL urine). Cria 2's urine, low in protein, was concentrated by ultrafiltration (10,000 Dalton cut-off).
  • Necropsy at the Food and Veterinary Institute Braunschweig/Hannover (LAVES); sections stained with hematoxylin-eosin and von Kossa (calcium).

⚠️ The reference ranges used, and where they come from — because this is the weak point. Clinical chemistry: Dawson et al., evaluated in adult alpacas, not crias. Hematology: Hengrave Burri et al. Vitamin D 25-OH: Gerspach et al. Renal function and clearance: horse values, because none exist for alpaca. Fractional excretion of potassium and urinary GGT: llama values (reference B in Table 5). Four species and one wrong age class to read three alpaca crias. The authors state this and urge caution; this card repeats it in large type because it is the reason why the numbers that follow describe an evident disaster, but do not define a threshold.

⚠️ Avvertenza d'uso, obbligatoria. Il testo pieno scrive «it is not known whether it was an intramuscular or subcutaneous injection» — dunque via parenterale, IM o SC non determinata → da verificare sul testo pieno, e nel testo pieno non c'è.

  • Prodotto: vitamin D3 1.000.000 I.E. ad us. vet., MEDISTAR Arzneimittelvertrieb GmbH, Holzwickede, Germania.
  • Altre fonti possibili, indagate dagli autori.

Tre letture, e vanno tenute separate.

  3. La FE del calcio era normale in entrambi. 18,36% e 3,56%, dentro l'intervallo.

Urine (Tabella 3): cria 1 pH 6,5, peso specifico 1,037, proteine 40,5 g/L, sangue +++, leucociti ++, eritrociti ++++, cellule epiteliali rotonde (tubulari) ++, cristalli di carbonato di calcio ++. Cria 2 pH 5,5, peso specifico 1,017, proteine 6,8 g/L, sangue ++++, leucociti +++, eritrociti +++.

Elettroforesi (Tabella 6).

1,46 indica proteinuria glomerulare). Rapporto **proteine/creatinina urinarie: 0,0044 nel cria

1 e 0,0007 nel cria 2 — così come stampati nell'articolo. Ipoalbuminemia in entrambi** (19,7 e 12,2 g/L, riferimento 29-40).

A · The exposure: the four things together — dose, route, species, source

⚠️ Mandatory usage warning. The numbers in this table are not a dosage: they are the dose that killed. And they have a declared gap: the route of the injection. The full text states «it is not known whether it was an intramuscular or subcutaneous injection» — hence parenteral route, IM or SC not determined → to be checked against the full text, and in the full text it is not there. Under the fifth rule of this volume, the number is written all the same, because it is a toxicological datum and not a prescription, but it is written with the gap visible.

  • Species: huacaya alpaca, crias 3-4.5 months old. Source: Wagener et al. 2021, Table 1 and §2.1, full text.
  • Product: vitamin D3 1,000,000 I.E. ad us. vet., MEDISTAR Arzneimittelvertrieb GmbH, Holzwickede, Germany. Administered in a single injection («one shot injection») in all three.
  • The "intended" dose of 2,000 IU/kg body weight is the one the authors indicate as recommended for alpaca in the note to Table 1, citing the literature. That note does not specify the route of administration → to be checked against the full text, and in the full text it is not there. Therefore this card does not use it as a dosage, and whoever seeks a dosage with all four pieces of information will find it further on, in the association's commentary.
  • Additional vitamin D treatments, from the colleagues who had visited previously. Cria 1 and cria 3 received on day 7 an injectable multivitamin preparation (Ursovit AD3EC, Serumwerk Bernburg) containing 25,000 IU of vitamin D3, in a single administration; the text qualifies it as a preparation «pro inj.» but does not state the route → to be checked against the full text. Cria 3 also received orally, on day 14, a vitamin paste containing 200,000 IU of vitamin D3 per litre: the amount of paste administered is not reported, so the dose cannot be calculated → to be checked against the full text.
  • Other possible sources, investigated by the authors. The herd's mineral feed was not vitaminized; the females received a mash containing 4,050 IU/kg of vitamin D3, and it cannot be excluded that the crias ate some of it. A botanical survey of the pastures excluded the presence of Trisetum flavescens, a calcinogenic plant.

B · The laboratory

All three crias: 25-hydroxycholecalciferol > 3,750 nmol/L, i.e. beyond the upper limit of the laboratory's measurement range. The true value is not known: it is only known that it is above. Reference adopted (Gerspach et al.): 50-200 nmol/L. The authors add that, depending on the source, the upper reference limit for South American camelids varies between 200 and 600 nmol/L, and that in humans toxicity appears at concentrations consistently above 400 nmol/L.

Course over time (Table 2, repeated samples). Cria 1's creatinine goes from 2,340 to 2,601 µmol/L between day 15 and day 16; cria 2's remains between 1,116 and 1,054 µmol/L from day 15 to day 21; the highest value in the entire series is cria 3's, 2,905 µmol/L on day 15, a few hours before death. Urea follows suit: cria 1 93.35 → 83.71 mmol/L; cria 2 from 68.94 to 128.7 mmol/L on day 21; cria 3, 140.3 mmol/L. These last two cria-3 values are confirmed verbatim in the Discussion, and are the two numbers in this series that can be cited without column ambiguity.

⚠️ An internal discrepancy, flagged by the editorial team. For cria 1 on day 15 Table 2 reports creatinine 2,340 µmol/L and Table 4 reports 2,191 µmol/L; for calcium, 3.39 versus 3.36 mmol/L. They are the same animal on the same day. The article does not explain the difference (plausibly two separate collections or two distinct determinations, the first at admission and the second concurrent with urine sampling). The card reports both series with their table of origin and does not choose between them.

Venous blood gas: acidosis in cria 2, with ionized calcium within the reference range despite high total calcium; in cria 3 ionized calcium was decreased. This is a detail that matters: normal ionized calcium did not rule out the intoxication.

C · Renal function — the heart of the article

Table 5, cria 1 on day 15, cria 2 on day 16. Reference A = horse (because none exists for alpaca); reference B = llama.

Three readings, and they must be kept separate.

  1. The glomerulus does not filter. Clearance at 0.08 and 0.17 mL/min/kg against 1.24-2.59: from a fifteenth to a seventh of the lower limit of the equine reference.
  2. The tubule does not reabsorb. FE of phosphorus, sodium, potassium and water all off the scale on the high side. Cria 1's potassium FE, 982.49%, is not a printing error: it is the sign of tubular damage severe enough that potassium is being secreted in addition to not being reabsorbed.
  3. Calcium FE was normal in both. 18.36% and 3.56%, within range. This is the most insidious datum in the card: in a vitamin D intoxication, with plasma hypercalcemia, fractional calcium excretion could turn out entirely normal.

Urine (Table 3): cria 1 pH 6.5, specific gravity 1.037, protein 40.5 g/L, blood +++, leukocytes ++, erythrocytes ++++, round (tubular) epithelial cells ++, calcium carbonate crystals ++. Cria 2 pH 5.5, specific gravity 1.017, protein 6.8 g/L, blood ++++, leukocytes +++, erythrocytes +++.

Electrophoresis (Table 6). In cria 1 the proteinuria is massive and non-selective: total urinary protein 40.5 g/L against plasma protein 41.5 g/L — i.e. virtually equal; urinary albumin 29.5 g/L; and all fractions are found in the urine, gamma-globulins included (2.7 g/L), i.e. large molecules that an intact glomerulus does not let through. Albumin/alpha-1-globulin ratio in cria 1: 10.17 (in the dog a value

1.46 indicates glomerular proteinuria). **Urinary protein/creatinine ratio: 0.0044 in cria

1 and 0.0007 in cria 2 — as printed in the article. Hypoalbuminemia in both** (19.7 and 12.2 g/L, reference 29-40).

⚠️ Two units to view with suspicion, flagged by the editorial team. Table 5 gives urinary creatinine as «9.162 and 9.183 µmol/L»: at that concentration the calculated clearance would not add up, and the value is consistent with mmol/L. Likewise the urinary protein/creatinine ratio (0.0044) is consistent only with creatinine expressed in µmol/L and protein in g/L. The article does not clarify the units of this ratio → to be checked against the full text. Anyone wishing to replicate the calculation must work out the units themselves: the clinical result (destroyed glomerulus) does not change, arithmetic reproducibility does.

In all three: mild myocardial degeneration with lympho-histiocytic myocarditis, partly granulomatous. Von Kossa staining confirmed calcification of atria, kidneys and, in part, liver, lungs and C3.

Clinical note on cria 1. Anuria, very high creatinine and colic-like signs had raised suspicion of urolithiasis, and for this reason a diagnostic laparotomy was performed: there was no urolithiasis and nothing abnormal was seen. The fibrinous peritonitis found at necropsy is, in the authors' own view, a likely consequence of that procedure.

Interpretazione degli autori

Attribuita esplicitamente, e distinta dai dati.

  • Gli autori concludono che dosi aumentate di vitamina D3 provocano calcinosi nei cria di alpaca, che la mineralizzazione renale porta a danno glomerulare e insufficienza renale, e che «great care should be taken in the dosing of vitamin D3 and in the reporting and interpretating of units».
«Our data do not give any indication of this hypothesis» — il cria 2 aveva ricevuto una sola dose e ha sviluppato una calcinosi altrettanto grave.

Authors' interpretation

Explicitly attributed, and distinguished from the data.

  • The authors conclude that increased doses of vitamin D3 cause calcinosis in alpaca crias, that renal mineralization leads to glomerular damage and renal failure, and that «great care should be taken in the dosing of vitamin D3 and in the reporting and interpretating of units».
  • The authors reconstruct the toxicological mechanism: the liver hydroxylates cholecalciferol at position 25 with high capacity and weak regulation, while hydroxylation at position 1 is tightly controlled by PTH and calcium; at pharmacological concentrations 25-hydroxycholecalciferol directly activates the vitamin D receptor, with lower affinity but enormous quantity, and additionally competes for the binding protein, freeing more active 1,25-dihydroxycholecalciferol. It is the accumulated 25-OH that causes the damage, which is why it is the metabolite that is assayed.
  • The authors reject a hypothesis by others: Gerspach had supposed that a second administration promoted the high serum levels. «Our data do not give any indication of this hypothesis» — cria 2 had received only one dose and developed calcinosis just as severe.
  • The authors hypothesize additional undocumented routes of intake (the females' mash, colostrum, milk), but have no proof of this.
  • The authors interpret cria 1's proteinuria — urinary protein equal to plasma protein, with passage of gamma-globulins — as complete destruction of the Bowman's capsule by calcification.
  • The authors observe that similar renal changes do not necessarily imply an excess of vitamin D: lambs with nephrosis or toxic tubular necrosis show similar azotemia and proteinuria, but with hypocalcemia. It is the hypercalcemia, not the azotemia, that points to the diagnosis.
  • The authors conclude, regarding the nine survivors, that «some animals seem to react more sensitively to high vitamin D doses than others», and that the number of undiagnosed vitamin D intoxications in South American camelids is probably high.

Limiti

Dichiarati dagli autori (esiste una sezione «5. Limitations», ed è breve):

  • «Although 12 crias were treated with significantly increased doses of vitamin D3, no further data are available on nine of the animals and the findings are based only on the owners' statement that the animals were "clinically unremarkable"» — i nove non sono stati esaminati.
  • «suitable reference values for alpaca crias were not available for some laboratory diagnostic tests, so approximate reference values from other age groups or other species had to be used».

Conflitti di interesse: «The authors declare no conflict of interest.»

Dichiarazione etica: presente.

◆ COMMENTO DELL'ASSOCIAZIONE

Camelid Research Studies APS — redazione CRL · 31/08/2026

Valutazione a stelle

AsseVotoMotivazione

Limitations

Declared by the authors (there is a «5. Limitations» section, and it is short):

  • «Although 12 crias were treated with significantly increased doses of vitamin D3, no further data are available on nine of the animals and the findings are based only on the owners' statement that the animals were "clinically unremarkable"» — the nine were not examined.
  • «suitable reference values for alpaca crias were not available for some laboratory diagnostic tests, so approximate reference values from other age groups or other species had to be used».
  • In the body of the Discussion, two more: the serum protein reference values (Dawson) are from adults, and a study on dromedaries showed that albumin is lower in juveniles; and the renal function and electrophoresis results «have to be interpreted with caution» for the same reason.

Added by the editorial team.

  • The route of administration of the toxic dose is not known. In a work whose only operational message is «pay attention to how vitamin D is administered», not knowing whether the injection was IM or SC is not a small gap: the kinetics of an intramuscular oily depot and those of a subcutaneous one are not the same thing.
  • The confusion of units is described twice, in different ways. The abstract says the units swapped were IU and µg; the Discussion writes «I.U. versus mg/mL». The factor of 40 is the one between IU and µg; the second formulation, in the text, is imprecise.
  • The culpable manual is not named explicitly in the body of the text: there is only a bibliographic reference. The caution is understandable, but the reader who owns that manual is not put in a position to know that they are holding it — which would have been the most useful service this article could have rendered.
  • No dose-response. Cria 3, with the lowest overdose factor (× 34.5), died before all the others (15 days), and has the highest creatinine. With n = 3 nothing can be said about a relationship between the magnitude of the overdose and severity, and indeed the authors do not claim one; but they do not rule it out either, and a hasty reader might read Table 1 as if a larger dose gave a worse course. That is not what it shows.
  • No structured differential diagnosis, as CARE would require: the diagnosis was in fact already known from the history.

Ethical approval and consent: yes, both declared — study conducted according to the Declaration of Helsinki and approved by the Animal Welfare Officer of the University of Veterinary Medicine Hannover; written informed consent obtained from the owners. This is an important difference from card CRL-B-01, where neither was present.

Funding: Deutsche Forschungsgemeinschaft and University of Veterinary Medicine Hannover, within the open-access publication funding programme only. No research funding, no industry sponsor.

Conflicts of interest: «The authors declare no conflict of interest.» The authors also declare that part of the data had already been presented as a poster (Wagener, Kammeyer, Kleinschmidt, Lehmbecker, Helmer, Ganter — «Kalzinosen bei Alpakas»): this is a declaration of prior partial publication, and it is right that it is there.

Data availability: the raw data are not accessible, because they are clinical patient data subject to confidentiality. Declared.

What is missing relative to CARE (adapted to the case series, §4.2 of ch. 14 of the KT): dated clinical timeline present and excellent (day by day, from injection to death, for all three); detailed history present; tests and treatments present; outcome present; explicit differential diagnosis missing; owner's perspective missing; consent to publication distinct from consent to treatment missing — consent is there, but in the standard form of the editorial template. Ethical statement: present.

◆ ASSOCIATION COMMENTARY

Camelid Research Studies APS — CRL editorial team · 31/08/2026

Three crias died from a unit-of-measure conversion error, not from a wrong clinical choice: the veterinarian had recognized a real, documented risk.

The veterinarian for that herd did everything they had been taught to do. They recognized a real risk — at central-European latitudes vitamin D deficiency in crias is documented and produces hypophosphatemic rickets. They looked up the dose in a scientific manual. And they injected forty times the intended dose, because in that chapter micrograms and International Units were swapped. This article does not tell the story of a distracted colleague. It tells the story of an information supply chain that broke upstream, and that is the reason this series' method exists, with its tiresome checks on every number.

The first thing to take home is arithmetic, and it must be learned by heart: 1 µg of cholecalciferol = 40 IU. This is not a conversion between similar units: it is a factor of forty. When on a bottle, on a data sheet or in a manual chapter vitamin D appears in µg, and the dose in your head is in IU, the difference between the two readings is not a margin of safety: it is the distance between a supplementation and an autopsy. Operating rule we propose: before calculating, in front of any vitamin D number, write the unit next to the number and ask out loud: "IU or micrograms?" If you don't know, don't inject.

The second thing is the only dosage this card allows itself to write out in full, and it does not come from this article — it comes from the only prospective dosimetric study we know of in the species, and it has all four pieces of information: 1,000 IU of vitamin D3 per kg of body weight, subcutaneously, as a single dose, in alpacas — to crias in late autumn and again in mid-winter, to adult females in mid-winter. Source: Judson GJ, Feakes A, «Vitamin D doses for alpacas», Aust Vet J 1999;77(5):310-315, a study conducted in South Australia, and the latitude is part of the data: it is not automatically the right dose for the Po Valley or for Trentino. The figure of 2,000 IU/kg that appears as «recommended» in Table 1 of Wagener's article is not accompanied by a route of administration, and under the fifth rule of this volume we do not transcribe it as a prescription. Anyone wishing to use it must go back to the primary source and read the route there.

The third thing is the one that truly changes an everyday gesture, and it is the form of administration, not just the quantity. These three crias died from a single injection — one shot injection, the article writes. Vitamin D is fat-soluble, it accumulates, and a depot injection cannot be called back: by the time signs appeared, seven to twelve days later, the renal damage was already done, and no intensive therapy changed the outcome in any of the three. A simple practice follows from this: injectable vitamin D must be calculated on the animal's actual weight, weighed, not estimated — we are talking about crias of 14 to 25 kilograms, where an eyeballed estimate is off by 30 percent — and the calculation must be done twice, by two people, as is done with chemotherapy. This is not excessive caution: the margin between the dose and the poison, here, is documented at a factor of forty, and the substance remains in circulation for weeks.

The fourth thing is for those who read lab reports, and it is counterintuitive. In these two crias the fractional excretion of calcium was normal (18.36% and 3.56%, reference 1.3-33.2%), and in cria 2 even ionized calcium was within range. A clinician who had looked for confirmation of the hypercalcemia in the calcium FE would not have found it. The parameter that was screaming was phosphorus — phosphate FE 15.23% and 49.02% against a reference of 0.02-4.86% — together with urinary GGT (125.2 and 45.3 U/L against 0-29) and the urinary GGT/creatinine ratio (13.67 and 4.93 against 0.1-0.64). And the simplest sign of all, the one you can see on a dipstick, was proteinuria: in cria 1 the urine contained 40.5 g/L of protein, as much as the plasma.

The fifth thing is the most unsettling, and no one has explained it yet: nine crias out of twelve survived. Same herd, same product, comparable doses, same day. Three dead, nine apparently healthy. The authors speak of «individual sensitivity», which is a correct way of saying we don't know. We add the uncomfortable part: the nine were never examined. We do not know whether they were fine or had subclinical calcinosis; no one measured their 25-OH, no one looked at their kidneys. It is exactly the same gap as in card CRL-B-01, where the most alarming finding was in the liver of the clinically healthy pen-mate. Faced with a dosing incident in a group, the question is not just «how is the sick animal doing»: it is «what is in the blood of everyone else who received the same syringe». 25-OH vitamin D is assayed on plasma, costs little, and answers.

Where we disagree with the authors — or rather, where we ask for more. The paper concludes, rightly, that «attention must be paid to the units». But the most useful operational conclusion is not a call to caution: it is the name of the manual. The article cites it only in the bibliography. If a German-language veterinary textbook contained, in 2016, a wrong conversion table for camelid vitamin D, the service that the literature owed its readers was a visible erratum, not a footnote. We do not know whether one was published; it was not searched for, and the card declares this in the colophon.

What is missing, and what an association can do. The most elementary thing is missing: a validated prophylactic vitamin D3 dose for crias at Italian latitudes, with route, season and weight. Today the figure circulating in Europe comes from an Australian study from 1999 and from North American recommendations, and the very fact that at least three different ones circulate — the authors write that «various recommended dosages […] have been published by different authors» — is what makes the error possible. A registry of supplementations with dose, unit, product, route and actual weight, kept by Italian farms for two seasons, would be within our reach and would be worth more than ten case reports.

Star rating

AxisScoreRationale
A · Scientific quality★★☆☆☆The CRL grid (§5.1 of ch. 14) places a case series with a clear timeline at ★★, and this timeline is exemplary: day of injection, day of signs, day of death, for all three. There is ethical approval, consent, conflicts, funding, an honest «Limitations» section and the disclosure of prior partial publication — that is, everything that card CRL-B-01 lacked. But the step above requires a comparison and an analysis, and here neither is present: the nine surviving crias were the comparison group already available, treated the same day with the same product, and were not sampled. It is the article's missed opportunity. There remain, besides, reference ranges taken from four species and from a wrong age class, and an unexplained internal numerical discrepancy between Table 2 and Table 4. Two stars, but two full stars.
B · Clinical utility★★★★★The grid reserves the fifth star for what «changes a recommended practice, with evidence proportionate to the change required», and here the proportion is clear-cut: the practice to be changed — checking the unit of measure before calculating a vitamin D dose, weighing the cria instead of estimating it, having the calculation double-checked — costs nothing, and the supporting evidence is three animals dead with a documented mechanism, temporal kinetics, histochemical confirmation and the cause of the error identified. It also delivers a laboratory profile usable tomorrow morning: massive proteinuria, sky-high phosphorus FE, high urinary GGT — and the warning that calcium FE can be normal.
C · Educational value★★★★☆First-rate training material: it explains the mechanism (accumulated 25-OH directly activating the receptor), shows the renal functional reasoning step by step, distinguishes glomerulus and tubule with numbers, and teaches an error to avoid that matters well beyond camelids. It does not reach the fifth star because the explicit differential is missing — precisely the part that gets taught — and because the most interesting question the work raises, why three and not the other nine, remains uninvestigated.
D · Relevance to the species★★★★☆Huacaya alpaca, Germany, private farm, product registered in Europe, error made in a manual read in Europe. One cannot get closer to the Italian reader than this. The grid would reserve the fourth star for a large or multicenter study, and this has n = 3: the editorial team assigns it anyway, and states this, because the population is described animal by animal — weight, age, sex, dose, dates — and transferability can therefore be assessed in full, which is the reason the grid asks for breadth. Not the fifth, because it is not a consolidated, widely cited reference study, but a series of three.

No overall score, by design. A = 2 and B = 5 are not a contradiction: one can learn a life-saving rule from a study that, as evidence, is weak — provided one knows what it demonstrates (that overdose kills) and what it does not demonstrate (what the safe dose is).

Bottom line

It can be concluded that in three huacaya alpaca crias of 25.5 · 19 · 14.5 kg a single parenteral injection of vitamin D3 of 78,000 · 79,000 · 69,000 IU/kg of body weight — that is, × 39.2 · × 39.5 · × 34.5 the intended dose — produced 25-hydroxycholecalciferol beyond the measurement limit (> 3,750 nmol/L, reference 50-200), renal failure (creatinine clearance 0.08 and 0.17 mL/min/kg), non-selective proteinuria up to 40.5 g/L, i.e. as much as the plasma protein, and calcinosis confirmed by von Kossa in cardiac atria, kidneys, lungs, liver and C3, with death at 18, 23 and 15 days after the injection; and that the error at the origin was a unit conversion taken from a manual (1 µg cholecalciferol = 40 IU).

It cannot be concluded from these three animals what the toxic threshold dose is, nor what the safe dose is: there is no dose-response — the cria with the smaller overdose (× 34.5) died before all the others — and there is no comparison group, because the nine surviving crias, treated the same day with the same product, were never examined, and their normality is only reported by the owners. It cannot be said by which route the toxic dose was injected: the full text states that it is not known whether it was IM or SC → to be checked against the full text. The figure of 2,000 IU/kg, which appears as «recommended» in Table 1, cannot be used as a dosage: the route is not specified, and under the fifth rule of this volume the card does not transcribe it as a prescription. The renal deviation cannot be quantified in species-specific terms: clearance and fractional excretions are compared against horse and llama values, clinical chemistry against adult alpaca values, because none exist for alpaca crias. Nor can the entire load be attributed to the injection alone: crias 1 and 3 had received 25,000 IU of a multivitamin by an undeclared route, and for cria 3's oral paste (200,000 IU/L) the amount administered is not reported, so the dose cannot be calculated.

Take-home messages

  1. 1 µg of cholecalciferol = 40 IU. Confusing the two units does not produce a 20% error, it produces a 4,000% one. It is the error that killed three alpaca crias in Germany, and it came from a scientific manual, not from a colleague's imagination.
  2. The doses that killed, with the gap they carry: 78,000 · 79,000 · 69,000 IU/kg of body weight, vitamin D3, in a single parenteral injection (IM or SC not specified in the text), in alpaca crias of 25.5 · 19 · 14.5 kg — overdose of × 39.2 · × 39.5 · × 34.5 — death at 18 · 23 · 15 days. These are not a dosage: they are the exact opposite.
  3. The latency is one to two weeks, and the signs are non-specific. Weakness, cough, inappetence, in one case anuria; appearing 7-12 days after the injection. By the time they arrive, the kidney is already mineralized: none of the three was saved by intensive care.
  4. On the report, calcium can mislead and phosphorus does not. Fractional excretion of calcium was normal in both crias tested; that of phosphate was 15.23% and 49.02% against 0.02-4.86%. Look for proteinuria (up to 40.5 g/L, as much as the plasma), phosphorus FE, urinary GGT and urinary GGT/creatinine ratio — and remember that in vitamin D intoxication azotemia goes with hypercalcemia, while in toxic nephrosis it goes with hypocalcemia.
  5. Nine crias out of twelve, treated the same way, did not become ill — and no one examined them. «Individual sensitivity» is the name of our not knowing. After a dosing incident in a group, assay 25-OH vitamin D in everyone who received the same syringe, not only in those who are sick.
