  • BUtilità clinica4/5
  • CValore didattico4/5
  • DRilevanza per la specie4/5
Che cosa misurano i quattro assi
A · Qualità scientifica
La domanda è ben posta? I metodi sono dichiarati? La statistica è appropriata? La conclusione sta dentro i dati?
B · Utilità clinica
Serve a chi ha l’animale davanti, oggi.
C · Valore didattico

What the four axes measure
A · Scientific quality
Is the question well posed? Are the methods stated? Is the statistics appropriate? Does the conclusion stay within the data?
B · Clinical usefulness
Useful to whoever has the animal in front of them, today.
C · Teaching value
Does it teach something to those studying the subject?
D · Relevance to the species
Is it really about camelids, or a result borrowed from another species?

Each axis is worth 1 to 5 stars and carries its written reason. The stars in the index are the rounded average of the four.

Titolo originale

«Disorders and Diseases of Pregnancy»

Original title

«Disorders and Diseases of Pregnancy»

Italian title (editorial translation): «Disturbi e malattie della gravidanza».

⚠️ This is a textbook chapter, written by three authors who often speak in the first person, and a substantial part of what it contains is declared clinical practice («in our practice», «in our experience», «in our hands», «our recommendation»): the first person plural («we», «our») appears 27 times in the body of the text, plus two references to «personal observation» and «personal communication» and one clinical observation attributed by name to one of the authors. No number in this card is an original result of the chapter; every number is reported, and where the source is someone else's work the card says so. A second warning concerns the numbers themselves: four percentages in the chapter do not add up with their denominators or with their parts (see Limitations and the box of discrepancies), and the editorial team flags them without correcting them. The third concerns the doses: there are few, and they are reported in a dedicated table with an indication of how much has been measured in the camelid.

Autori

Rodriguez: nessun ORCID in nessun record.

Authors

Pearson Lisa K., Rodriguez Jacobo S., Tibary Ahmed — three authors; the Crossref record (queried on 11/10/2026) lists them in this order, and the text deposited in Europe PMC reports the same three names. ORCID: OpenAlex attributes 0000-0002-0746-871X to Pearson and 0000-0002-6274-4019 to Tibary; neither appears in the Crossref record, in the Europe PMC record or in the deposited text, and the editorial team has not verified that the two ORCID profiles are those of the authors. Rodriguez: no ORCID in any record.

Volume editors: Cebra Christopher, Anderson David E., Tibary Ahmed, Van Saun Robert J., Johnson LaRue W. (as reported by the deposited text; the Crossref record of the chapter does not list them). Tibary is therefore both an author of the chapter and an editor of the book.

Affiliazione dell'autore corrispondente

Corresponding author affiliation

Not stated. The deposited text does not designate a corresponding author and does not assign to each author his or her own affiliation: it lists five affiliations (Oregon State University, University of Tennessee, Washington State University, Pennsylvania State University, Colorado State University) and links them to all contributors without distinction, authors and editors together. By order and by titles the list appears to be that of the five editors, so the card does not attribute it to the authors of the chapter. When the text says «our clinic» or «our service» it does not name the institution; the editorial team does not infer it.

Libro, anno, capitolo, pagine, identificatori

Libro: Cebra C, Anderson DE, Tibary A, Van Saun RJ, Johnson LW (curatori), Llama and Alpaca Care, Elsevier. Capitolo 23, Disorders and Diseases of Pregnancy, pagine 256-273 (18 pagine).

PubMed: no (nessun PMID). Nessun ISSN.

Impatto misurabile del singolo capitolo: 9 citazioni secondo OpenAlex, 5 secondo Crossref, 0 secondo Europe PMC, tutte all'11/10/2026.

Licenza dell'articolo originale: Copyright © 2014 Elsevier Inc., tutti i diritti riservati, con il permesso aggiuntivo che accompagna il testo depositato: Elsevier concede che la ricerca legata al proprio centro risorse COVID-19 sia disponibile in PubMed Central «with rights for unrestricted research re-use and analyses in any form or by any means with acknowledgement of the original source», «for as long as the COVID-19 resource centre remains active».

Book: Cebra C, Anderson DE, Tibary A, Van Saun RJ, Johnson LW (editors), Llama and Alpaca Care, Elsevier. Chapter 23, Disorders and Diseases of Pregnancy, pages 256-273 (18 pages). Record type: book chapter (Crossref: book-chapter).

Year: 2014 according to Crossref and to the copyright notice of the deposited text; 6 December 2013 according to Europe PMC, OpenAlex and the publication date of the PubMed Central record. The card cites the chapter as 2014 and records the difference between the two dates among the discrepancies.

  • ISBN (print) 9781437723526, from the Crossref record; the deposited text does not report it.
  • DOI 10.1016/B978-1-4377-2352-6.00023-7
  • PMID: none. The Europe PMC record has no PMID: the chapter is not indexed in PubMed as an article.
  • PMCID: PMC7158342 — free full text: yes, with the licence caveat that follows.
  • Full text read in its entirety: yes. Read on 11/10/2026 from the Europe PMC full-text service (version deposited in PubMed Central, about 11,800 words): all sections, the captions of the 13 figures and table 23-1 (the figures themselves are not visible in the text). The bibliographic entries are missing from the deposited text: the final list of references is empty in the Europe PMC version, and the references appear in the body as numbers from 1 to 66 without the corresponding work. Crossref declares 66 references and OpenAlex 69 cited works. No cited source was read, and for the same reason the card cannot name the primary works: where it writes «a cited work» it refers to the sequence number in the chapter.

Status of the venue (verified on 11/10/2026): it is not a journal. The chapter is in an Elsevier book; Crossref gives it with publisher Elsevier, OpenAlex with source Elsevier eBooks (publisher Elsevier BV, 1,208,620 works indexed, type «ebook platform», not open access). PubMed: no (no PMID). DOAJ: no (query by the title of the book, zero records; DOAJ lists journals, not books). No ISSN.

Measurable impact of the single chapter: 9 citations according to OpenAlex, 5 according to Crossref, 0 according to Europe PMC, all as of 11/10/2026. The three counts do not coincide and the card reports all three.

Licence of the original article: Copyright © 2014 Elsevier Inc., all rights reserved, with the additional permission that accompanies the deposited text: Elsevier grants that research linked to its COVID-19 resource centre is available in PubMed Central «with rights for unrestricted research re-use and analyses in any form or by any means with acknowledgement of the original source», «for as long as the COVID-19 resource centre remains active». It is not a Creative Commons licence: it is a publisher's permission, conditional on the continued existence of that resource centre. Read on the deposited text on 11/10/2026. It allows the text to be read in order to write a card of our own; it does not authorize this card to republish tables or figures. This card may reproduce: factual data rephrased and short quotations in quotation marks. No table, no figure: table 23-1 (formulas to estimate gestational age) and the figures (ultrasonograms, diagrams of the suture, the emergency protocol for colic in term females) are described in words.

Specie e soggetti

Nessun soggetto proprio.

Species and subjects

No subjects of its own. The chapter concerns pregnant llamas and alpacas. The data reported come from very different sources: New Zealand alpacas (158 pregnancies), Peruvian alpacas and llamas (serology, abortions), Argentine llamas (serology), United Kingdom alpacas (one outbreak of abortions), North American llamas and alpacas (single cases, the authors' clinical caseload), and the dromedary for some agents (Brucella, Coxiella, equine herpesvirus 1), flagged as such by the text. The text also invokes the mare, the bovine and the sheep as points of comparison and extrapolates from them the treatment of placentitis and the vaccination threshold; the card says so where the extrapolation is explicit. The authors' clinical caseload («our practice», «our clinic») has no declared number of animals.

Tipo di studio e livello di evidenza

Capitolo di manuale clinico, a impianto di rassegna narrativa, senza strategia di ricerca dichiarata. Lo scopo, dichiarato nell'apertura: «This chapter discusses pregnancy loss in alpacas and llamas, as well as other diseases related to pregnancy.»

Study type and level of evidence

Clinical textbook chapter, built as a narrative review, with no declared search strategy. The aim, declared in the opening: «This chapter discusses pregnancy loss in alpacas and llamas, as well as other diseases related to pregnancy.»

Level of evidence: narrative review, hence a structured opinion of a group of clinicians on a literature that the reader cannot reconstruct (all the more so as the list of references is not in the deposited text). The body is made of three materials that the card keeps distinct: (a) the published case material on the camelid, mostly single cases, small series and serological surveys; (b) extrapolated data from bovine, sheep and mare; (c) the authors' experience, partly declared as such and partly expressed in assertive form without a source.

Consequence for the reader. The selection of the works discussed is not reproducible: this card summarizes the reading of three authors, not the state of the literature. Clinical statements and doses are cited as «the authors report».

Relevant reporting standard: no reporting guideline exists for narrative reviews; the SANRA quality scale is used (chapter 14 §4.2). The authors declare no standard. The score compiled by the editorial team, item by item, is in the ratings box: 6/12.

In breve

In brief

The chapter deals with pregnancy loss and the other diseases of gestation in the alpaca and the llama. It reports that, with good management, the annual pregnancy rate is estimated at 78 % and that between 7 % and 12 % of pregnancies are lost between conception and parturition; it proposes a schedule of ultrasound examinations (14, 25-30 and 45-60 days) and a diagnostic pathway that rests on submitting the whole fetus and placenta to the laboratory. It goes through infectious causes (viral, with bovine viral diarrhea virus in the foreground; bacterial; protozoal) and noninfectious causes (iatrogenic, nutritional, maternal, fetal and placental), then prevention, placentitis, ketosis, vaginal prolapse, uterine torsion, prolonged pregnancy and other rare conditions. The authors declare that the cause of abortion is rarely found («the diagnosis rate rarely reaches 30 % of all submissions») and that uterine torsion is «by far» the most common emergency of the pregnant female. The most practical stretch of the chapter is the step-by-step description of rolling the female to correct torsion and of the Buhner suture for prolapse.

Dice che «no epidemiologic data on the incidence of pregnancy loss in alpacas and llamas exist», che l'ipoluteoidismo è «likely overdiagnosed and overtreated», che di herpesvirus equino 1, arterite virale equina e virus della bluetongue «the exact role» nell'aborto di alpaca e lama «is unknown».

Why this study matters

First reason: it is the overall clinical chapter on the pregnant alpaca and llama, written by those who see her in the clinic. For a veterinarian who has to decide in one night whether a term female with abdominal pain has a torsion, it gathers in one reading the diagnosis by palpation of the broad ligaments, the ultrasonographic threshold of fetal heart rate, the rolling technique, and the moment at which one moves on to cesarean section.

Second reason: it declares how little is known. It says that «no epidemiologic data on the incidence of pregnancy loss in alpacas and llamas exist», that hypoluteoidism is «likely overdiagnosed and overtreated», and that of equine herpesvirus 1, equine arteritis virus and bluetongue virus «the exact role» in alpaca and llama abortion «is unknown». In a clinical review it is not common for the most convenient hypothesis (progesterone deficiency) to be presented as overdiagnosed.

Third reason: it gives the denominators half of the time and lets one see where they are missing. The chapter cites numbered caseloads (158 New Zealand pregnancies; 15 Peruvian abortions with three positive for Neospora; 50 Peruvian aborted fetuses without Toxoplasma) and others without any denominator. Whoever reads it with the calculation at hand also finds the percentages that do not add up, and this is the reason why a card like this one is useful.

Fourth reason: it touches an Italian question. For Neospora and Toxoplasma the chapter reports South American seroprevalences; card CRL-I-03 gives the Italian ones (506 camelids), and the two sources can be read together without confusing them.

Obiettivo

Dichiarato dagli autori: «This chapter discusses pregnancy loss in alpacas and llamas, as well as other diseases related to pregnancy.» L'apertura precisa che la torsione uterina è «a common complication of late gestation, for which the producer and practitioner should have an emergency plan in place».

Objective

Declared by the authors: «This chapter discusses pregnancy loss in alpacas and llamas, as well as other diseases related to pregnancy.» The opening specifies that uterine torsion is «a common complication of late gestation, for which the producer and practitioner should have an emergency plan in place».

Clinical question (editorial formulation; the PICO format does not apply to a descriptive review): in a pregnant female alpaca or llama, which causes of loss or disease of pregnancy are to be considered, how is gestation monitored, how is an abortion worked up, and which interventions do the authors report for torsion, prolapse and prolonged pregnancy?

Disegno dello studio

Study design

An 18-page chapter, 13 figures, one table, 66 reference numbers in the body (list not present in the deposited text). Structure: pregnancy loss and ultrasound monitoring; diagnostic approach to abortion; etiology (infectious: viral, bacterial, protozoal; noninfectious: iatrogenic, nutritional, maternal, fetal and placental); prevention; placentitis; ketosis; vaginal prolapse; uterine torsion; prolonged pregnancy; other conditions.

⚠️ Not verifiable and therefore not asserted: the selection of the cited works (absent), the content of the numbered references, the number of animals in the authors' clinical caseload, and the date of the personal observations.

Metodi / Protocollo

Per l'altrenogest dice che «is not active orally in camelids» (affermazione senza fonte).

Methods / Protocol

The chapter has no methods. No database, no search string, no range of years, no language, no inclusion criteria. The sources are of four types: numbered primary works (mostly single cases and serological surveys), cross-references to other parts of the same book («elsewhere in this text», for the examination of the placenta and for the assessment of the fetus and placenta), personal communications and clinical observations of the authors, and a review by the same group on infectious reproductive losses (Tibary et al., Theriogenology 2006, cited as the source of table 23-1).

Doses. The chapter contains few doses. They are reported in the table that follows, each with dose, route, species and source, and with an indication of how much has been measured in the camelid. They are not prescriptions: the card does not recommend them, and has not verified that the products are authorized for the species in Italy.

Risultati principali

  • Definizione.

  • Resa diagnostica.

Di questi tre ultimi virus: «The exact role of these viruses in alpaca and llama abortion is unknown».

Cause batteriche

  • Forme lievi (mucosa rosea, lucida, visibile solo a femmina in decubito sternale): nessuna terapia; sorveglianza dell'evoluzione, pomate antibiotiche oleose o lanolina con antibiotico senza corticosteroidi, coda pulita e ambiente asciutto.

«Torsion of the gravid uterus is, by far, the most common emergency in pregnant alpacas and llamas.»

«Hypoluteoidism is likely overdiagnosed and overtreated in female alpacas and llamas that experience recurrent pregnancy loss.»

«The producer should be counseled that any abortion occurring in any female should be taken very seriously.» — Raccomandazione, non risultato.

Main results

(everything that follows is «the authors report»; the numbered primary works were not read and their list is not in the deposited text)

How much is lost, and how it is monitored

  • Frequency. Opening of the chapter: with good management and veterinary supervision the annual pregnancy rate is estimated at 78 %, with 7-12 % of pregnancies lost between conception and parturition (references 1 and 2); in South America, with traditional management, the birthing rate may fall as low as 45 %. Further on the text repeats the same 7-12 % estimate as an estimate from «our practice» and states that epidemiologic data on incidence do not exist (see the discrepancies).
  • New Zealand alpacas (reference 3, 158 pregnancies): loss of 25.7 % after day 30 of gestation and of 9.6 %-16.7 % after day 120; in females bred in autumn 17.3 % of the losses occurred before day 81, against 2.8 % in those bred in spring (no denominator for the two groups); seasonal effect absent in other groups on a neighboring farm; most of the animals had been imported from Chile and some had recurrent losses over several seasons. In 8 animals serial progesterone returned to baseline in coincidence with the loss.
  • Definition. Early loss is the loss of the embryo or fetus before 50 days; as a rule there are no outward signs. In pregnancies beyond 35 days small amounts of tissue or fluid may appear around the dung pile. After the first trimester the expulsion is visible.
  • The authors' ultrasound schedule (their own practice): examinations at 14, 25-30 and 45-60 days; a fourth between 80 and 90 days if the female has a history of recurrent loss or infertility, in particular from endometritis. The authors recommend not moving the female from the breeding farm during this period. Ultrasonographic signs of impending loss in the early phase: embryonic vesicle smaller than normal, echogenic speckling, absence of heartbeat after day 25, poorly defined fetal structures, more echogenic fluid, slow growth, echogenic ring in the vesicle, disorganized membranes with collapsed amnion, more marked uterine edema and endometrial folds.
  • After the first trimester: possible warning signs are mammary development, restlessness and colic; the aborted fetus is often found in the pasture without knowing which female lost it, and in that case the authors recommend re-examining all the females at risk by ultrasound.

How an abortion is worked up

  • History and examination. One animal only or more than one? At what stage? Breeding records, vaccinations, method of pregnancy diagnosis, diet, travel, medications administered. The authors write that females bred before reaching 63 % of adult weight and height have a higher risk of losing the pregnancy because of immaturity (source not stated). Indicated: serum trace mineral assay, ultrasonography of the uterus, uterine culture with a double-guarded swab, vaginal examination for cervical lesions, endometrial biopsy in females with recurrent loss, blood count and biochemistry, fecal examination for parasites in females in poor condition.
  • Samples. The chances of diagnosis are best when the whole fetus and the whole placenta are submitted, at once and never frozen, cooled on ice. If the placenta cannot be submitted whole: samples from the cervical region, the body of the uterus, the base, the middle and the tip of each horn and any abnormal area, in duplicate, one fresh on ice and one in 10 % formalin. From the fetus: crown-rump length and weight to estimate gestational age and rule out intrauterine growth retardation; fluid from pleura, abdomen and stomach for microbiology; organs for histology and immunohistochemistry; cardiac blood and frozen sections of liver and brain for toxicology. Hormonal assay of the female who has already aborted is not useful.
  • Diagnostic yield. «The diagnosis rate rarely reaches 30 % of all submissions» (reference 5), which the authors attribute to insufficient clinical and laboratory data and to a lack of expertise in camelid pathology. The chapter admits that the cause is often not found even with all the samples, especially in the isolated case.
  • Table 23-1 (not reproduced). It contains 13 formulas to estimate gestational age in days from fetal measurements: 7 for the llama (Lama glama) and 6 for the alpaca (Vicugna pacos), with correlation coefficients r from 0.86 to 0.99; the text warns that the formulas «are not always accurate». The table is «adapted» from a 2006 review by Tibary et al. on the infectious causes of reproductive loss. In the deposited version some alpaca formulas repeat the same term twice with different coefficients (for example weight with weight): the exponents have probably been lost in the deposit, and the card does not interpret them.

Viral causes

  • Bovine viral diarrhea virus (BVDV). The authors indicate it as the dominant cause of viral losses; the most frequent serotype in alpacas and llamas is noncytopathic BVDV-1b (reference 6). In camelids diarrhea does not seem to be part of the picture. No congenital malformation due to BVDV has been reported in alpacas or llamas (reference 9); the birth of persistently infected (PI) crias is documented. In an outbreak on a farm (reference 12) the transplacental infection rate was 82 % and led to the birth of a PI cria in 70 % of cases (n = 10), when the pregnant females had been exposed between day 64 and day 114 of pregnancy; the authors hypothesize, «on the basis of available information», a window of susceptibility between 45 and 120 days (the exact duration is not known and the variability of gestation makes it costly to determine). Chronically or persistently infected crias: poor growth, anemia, monocytosis, wasting, diarrhea, respiratory disease; about 50 % die in the first 6 months (reference 12; denominator not reported). In cattle the PI animal develops mucosal disease, which is not reported in camelids.
  • Management. Segregate pregnant females from PI animals and from cattle, sheep and goats; precautions for females that travel elsewhere for breeding with the suckling cria. The authors recommend a screening program on no less than 10 % or 15 animals of the group (reference 9). Diagnosis on an aborted fetus is by virus isolation (blood, lymph nodes, placenta) or immunohistochemistry; on the neonatal cria PCR on whole blood is the most common screening method; serology does not distinguish active infection from prior exposure and crias may have antibodies from the dam's milk or from bovine colostrum. For the PI animal the authors recommend euthanasia.
  • Vaccine. No BVDV vaccine is licensed for alpacas and llamas. In a cited work (reference 16) nonpregnant female alpacas vaccinated with a bovine modified-live vaccine (types 1 and 2) and then challenged with BVDV all seroconverted and none was PCR positive; the nonvaccinated controls were PCR positive and developed a self-limiting respiratory disease (numbers of animals not reported). The authors do not recommend vaccination, because of the low prevalence, the off-label use and the unknown efficacy in pregnant animals.
  • Other viruses. Abortions reported by owners during an outbreak of an acute respiratory syndrome of alpacas of suspected coronavirus origin (reference 17); the hypothesis for the abortion is severe fetal hypoxia; two pregnant alpacas (at 10 and 11 months) were successfully managed with oxygen, nonsteroidal antiinflammatory drugs and broad-spectrum antimicrobials. Equine herpesvirus 1 has not been reported as a cause of abortion in alpacas and llamas, but has been isolated from abortions of the dromedary and causes neurologic disease in the alpaca and the llama. Equine arteritis virus was identified by RT-PCR in one late-term alpaca abortus, with five seropositive adults in the herd. For bluetongue the text reports a severe syndrome with death in the alpaca (reference 20). Of these last three viruses: «The exact role of these viruses in alpaca and llama abortion is unknown».

Bacterial causes

The authors write that bacterial causes are the main infectious causes of pregnancy loss in alpacas and llamas, with several organisms isolated (reference 5).

  • Chlamydophila (reference 21): in a herd of llamas two aborted fetuses and one weak cria, which subsequently died, were positive; the affected females were seropositive and the unaffected pregnant females, treated with long-acting oxytetracycline, did not abort again. In a study of the normal vaginal flora of the alpaca (reference 22) Chlamydophila was not identified, which the authors take to support a pathogenic and not a commensal role. Vaccines licensed for sheep are used off-label; their influence on the incidence of abortion is not known. Diagnosis by PCR on swabs of placenta and fetal tissues; maternal serology is not always helpful.
  • Brucella: brucellosis causes abortion in camel herds, whereas the role in the llama and the alpaca has not been fully investigated. B. melitensis has been reported as a cause of abortion in alpacas of Peru (reference 23). Llamas exposed to B. abortus had bacteremia, colonization of various tissues and an antibody response (reference 24); in an experimental inoculation (reference 25; number of animals not reported) one animal aborted in late gestation, with subacute placentitis as in cattle and isolation from the mammary gland and lymph nodes after euthanasia.
  • Leptospira: in Argentine llamas positive titers in 47.3 %-96.2 % (reference 26), in guanacos 0 %-13 %, in vicuñas 9 %-62.8 %; in a Peruvian survey 79.24 % of alpacas were seropositive for serogroup Pomona, with an association with the rainy season (reference 27). In a report from Oregon L. Pomona and L. grippotyphosa were indicated as possible causes of abortion on the basis of the dam's serology (reference 5). Denominators not reported in the text. The authors write that the evidence for a role in camelid abortion is very scarce; commercial vaccines used off-label gave highly individual titers and short immunity (references 28, 29), should be reserved for endemic areas, and field observations advise against using them in the first trimester and in the last 2 months.
  • Listeria monocytogenes: only one Listeria abortion reported (reference 30): a multiparous female aborted at 8 months after a short period of lethargy, with Listeria cultured from fetal liver and lung and diffuse placental lesions; the uterine culture could not be processed; the female was subsequently found to be infected with Mycoplasma hemolamae; she became pregnant again 30 days later. Neonatal cases of Listeria sepsis are mostly attributed to failure of passive transfer.
  • Campylobacter fetus (reference 34, United Kingdom, alpaca): four abortions and one premature nonviable cria in females in late pregnancy over 6 weeks; two fetuses examined, with focal placental necrosis in one placenta and generalized necrotic placentitis in the other; the organism was isolated from placenta and from fetal stomach contents; fecal cultures of the other pregnant females showed it in 36 % of the animals (denominator not reported). Four of the five affected females were rebred and became pregnant within 31-150 days of the abortion. Sheep were present on the farm, geographically separated and without abortions that year. The authors recommend including C. fetus in the differential diagnosis of late-term abortion with placental lesions and exposure to sheep, and distinguishing it from Campylobacter-like organisms such as Arcobacter, isolated from an alpaca abortus (reference 35).
  • Coxiella burnetii: a well-established cause of abortion in camels, with a risk to humans; no case reported in alpacas and llamas.
  • Mycoplasma hemolamae: abortion is not demonstrated, but in utero transmission is reported from a nonparasitemic dam to a 4-day-old alpaca cria with almost 100 % of erythrocytes affected, which died despite stabilization; cria and dam were PCR positive (reference 36). Card CRL-A-05 deals with the clinical picture of the same infection.
  • Other isolates from alpaca and llama abortions: Escherichia coli, Streptococcus sanguis, Arcanobacterium pyogenes (now Trueperella pyogenes), Pseudomonas (references 5, 37).

Protozoal causes

  • Neospora caninum, indicated as the most common protozoal cause. Seroprevalence 4.6 % in Argentine llamas (reference 38); 36 % in alpacas and 31.5 % in Peruvian llamas (reference 39; denominators not reported). In 15 abortions of Peruvian alpacas and llamas, N. caninum was definitively diagnosed (parasite DNA in mid- to late-gestation fetal tissues) in 3 (reference 40). Another work by the same group associated the infection with 28 % of aborted fetuses, with abortions mostly in mid-gestation (reference 41; denominator not reported in the chapter). Definitive hosts are canids, domestic and wild.
  • Toxoplasma gondii: seroprevalence 35 %-44.2 % in llamas, 16.3 % in alpacas and 5.5 % in vicuñas of South America (references 38, 39, 42), 33.5 % in llamas of the United States Pacific Northwest (reference 43); tissue cysts not found, effect on pregnancy uncharacterized. Two infected pregnant llamas (one by experimental inoculation at 82 days, one by natural infection between day 26 and day 119) seroconverted, remained healthy and delivered viable crias without antibodies before colostrum (reference 44); another two llamas with rising titers aborted, and the fetal fluids of one of the aborted crias had antibodies (reference 45). In 50 aborted fetuses from Peru no T. gondii DNA was found (reference 41). The authors conclude that toxoplasmosis does not appear to be a common cause of abortion and recommend limiting contact with cats and covering the feed.
  • Sarcocystis: infection of the alpaca and the llama as intermediate hosts is common and causes condemnation of meat in South America. In Argentina (reference 38) for S. aucheniae and S. cruzi the seroprevalence is reported as 96 % of samples positive for only one genus and 75 % positive for both (the two figures cannot be reconciled: see the discrepancies). One report of abortion in an alpaca with signs of acute and chronic infection (reference 46): hypothesis of abortion from maternal anemia and endogenous release of PGF2α, with no fetal lesions attributable to the parasite.

Noninfectious causes

  • Iatrogenic. The corpus luteum is necessary for the maintenance of pregnancy: any PGF2α or analogue causes luteolysis and loss. Dinoprost tromethamine and cloprostenol induce abortion at any stage (references 47, 48); the interval from treatment to abortion is generally 3-7 days and does not depend on the stage; beyond 150 days the female may have more discomfort. In the cited work (reference 48) cloprostenol (dose in the table) caused abortion in 92.5 % of the animals at 3 days (45.2 %) or at 4 days (37.7 %) after the series of injections (the three percentages are not compatible: see the discrepancies); a small proportion required a second series; fertility after induced abortion was similar to that of untreated controls. In the authors' experience the interval in the alpaca is slightly shorter (2-4 days). The same drugs induce parturition in term females (reference 49). Corticosteroids in the last trimester cause loss (reference 50), with premature placental separation and release of PGF2α; the authors report 4 pregnant females (5-8 months) that aborted after a topical steroid eye solution, a clinical observation by Tibary; induction of parturition with dexamethasone causes in utero fetal death (reference 49). Multivalent vaccines in pregnant females have been associated with loss in early gestation, mostly the 8-way vaccines against Clostridium (reference 50); the authors do not recommend vaccinating in the first 90 days of gestation.
  • Nutritional. Trace mineral homeostasis is essential; derangements can cause infertility, subfertility or abortion (references 51-54). On plant poisoning little information is available; a recent report indicates nitrate-nitrite poisoning in alpacas fed oaten hay (Avena sativa) with abortions and deaths (reference 55), with a mechanism hypothesized as in cattle: fetal hypoxia from maternal methemoglobinemia.
  • Maternal. Hypoluteoidism: hypothesized in the alpaca and the llama, suspected in females with recurrent loss, mostly between 2 and 8 months. The authors write that it is «likely overdiagnosed and overtreated» (reference 56), that the incidence is not known, that the size and character of the corpus luteum on ultrasonography have not been correlated with circulating progesterone, and that only the serum assay measures it. The threshold cited by most sources is > 2 ng/mL; the authors have seen females with 1.5 ng/mL deliver viable crias, and hypothesize that body weight and metabolic status influence steroid metabolism. A female treated with exogenous progesterone after repeated early losses carried the pregnancy to term with a measured endogenous progesterone that was very low; it is not known whether she had hypoluteoidism or was downregulating endogenous production in the face of supplementation. The authors recommend evaluating by frequent ultrasonography every female on therapy, stopping at 300 days (risk of failure of cervical dilation with prolonged treatment) and discouraging the therapy in maiden females; the syndrome may be associated with other metabolic or hormonal disturbances (hypothyroidism), and most of the cases in their clinic involve obese females. Stress: cortisol interrupts placental function; trailer travel of as little as 30 minutes raised cortisol for several hours in the alpaca (reference 57); systemic disease, stabling in a clinic, and production of very fine fiber in females who cannot sustain both fiber and fetus are other hypothesized causes. Placentation: areas of the uterus with scar tissue do not develop placental attachments; placental insufficiency and space limitations are the main cause of abortion of twins in late gestation. Cervix: lacerations from obstetric manipulations, dystocia or abortion may compromise its competence, which will be known only once pregnancy is established or lost; importance of the vaginal examination after parturition.
  • Fetal and placental. Twins: nearly all derive from double ovulation, estimated at 8-10 % of cycles, more frequent in females in good body condition; induction of ovulation with GnRH or hCG is suspected of increasing it, but this has not been confirmed in controlled studies. In the authors' clinic 95 % of twin pregnancies diagnosed at 14 days reduce to a single one or are lost altogether by day 45 (denominator not reported); if one fetus is not eliminated by 45 days, most end with abortion in mid- to late gestation (mostly between 4 and 7 months); less than 1 % of conceived twins are born alive, generally after a gestation shorter than 320 days. A twin pregnancy in the same horn undergoes total loss; in a spontaneously reduced pregnancy in which the retained fetus was in the right horn (usually smaller), the authors flag a possible increased risk of late abortion and ask that these pregnancies be treated as high risk. The ultrasound at 14 days must examine both ovaries, and if two corpora lutea are seen the double heartbeat is sought by day 25. The authors recommend eliminating both twins if bilateral; if unilateral, a new examination between 30 and 45 days (dose in the table). No easy techniques exist for manual reduction. Severe malformations (hydrocephalus, conjoined twins, cyclopia and others) cause abortion or stillbirth, although many crias with severe malformations are carried to term; chromosomal abnormalities are suspected and poorly studied. Umbilical cord torsion: documented as a cause of abortion in mid- to late gestation (references 2, 5). Other placental abnormalities: poorly developed mineralized villi, avillous areas.

Prevention, placentitis, ketosis

  • Prevention. Biosecurity and segregation by sex, age and pregnancy status; quarantine of at least 3 weeks for females that have aborted, for newly introduced animals and for those returning from a show (reference 4), with separate equipment, designated clothing and daily monitoring of fever, lethargy, inappetence and diarrhea; reproductive examination of every new breeding animal, especially females with a history of infertility or loss, and of males. Emergency plan in case of abortion: isolation of the female, collection of the material, examination of the female and of the abortus.
  • Placentitis. Placentation is epitheliochorial, microcotyledonary and diffuse as in the mare, but the camelid cervix has cartilaginous rings that close tightly under progesterone, whereas the mare's has dilatable longitudinal folds; the authors trace to this difference the rarity of ascending placentitis («extremely rare»), which when it occurs is often seen only after the abortion. They observed it (personal observation by Tibary) in females with recurrent vaginal prolapse in late pregnancy; nonspecific hematogenous placentitis is suspected with severe dental disease, gastric ulcers or metabolic disorders, without demonstration. One case of fungal placentitis with a diagnosis of Encephalitozoon cuniculi (histology and PCR) in a primiparous alpaca that aborted at 290 days (reference 58). Diagnosis in the live animal is ultrasonographic, on the combined thickness of uterus and placenta (CTUP), with thickening or separation; treatment is extrapolated from the mare and usually fails in the camelid, because by the time of diagnosis the fetoplacental unit is already severely compromised; the diagnosis serves to adjust the care of the female.
  • Ketosis. Primary ketosis (pregnancy toxemia) is rare in camelids compared with small ruminants; it is often secondary to other illnesses and to uterine torsion. One primary case described in a 3-year-old alpaca in the 11th month of pregnancy (reference 59): after 2 weeks of anorexia, depression and weight loss, ketonuria, hyperglycemia, hypokalemia, metabolic acidosis, and increased liver enzymes, β-hydroxybutyrate and nonesterified fatty acids (values not reported in the chapter). In severely debilitated females the authors consider induction of abortion or parturition.

Vaginal prolapse

  • Timing and factors. It occurs mostly in the last 2 months of gestation. Suspected factors: genetics, advanced age, parity, poor or excess body condition, straining from gastrointestinal parasitism, an excessively large fetus; the rise in estrogens in the last weeks relaxes vulva and perineum.
  • Mild forms (pink, shiny mucosa, visible only when the female is in sternal recumbency): no treatment; monitoring of progression, oil-based antibiotic ointments or antibiotic lanolin without corticosteroids, clean tail and dry environment.
  • Severe forms (vagina, cervix and sometimes bladder): the female does not resolve it on her own. Cleaning with a very dilute chlorhexidine and saline solution, ultrasonography of the prolapsed tissue to assess bladder and fetoplacental unit, replacement without direct pressure of the fingertips (risk of rupture), with a tube speculum or a smooth round rigid object in chronic cases with necrotic tissue; epidural anesthesia is often unnecessary. Retention with a Buhner suture («postmortem» or «ski» curved needle, size 4 or 5, 6 cm long, quarter-inch umbilical tape), infiltration of lidocaine 2 % at 1-1.5 cm from the vaginal opening, passage through the full thickness from about 1 cm above the ventral commissure to about 0.5 cm above the dorsal one; alternatively surgical vulvuloplasty or «shoe-string» stitches with two to three loops per side. Sutures must be removed before parturition. The devices used in sheep (vaginal retainers, harnesses) are not useful in the alpaca and the llama.
  • After replacement. Damaged tissue sloughs or remodels; if only the mucosa was affected the convalescence is shorter but the risk of vaginal adhesions is high; lanolin with antibiotic and antiinflammatory in the vagina; topicals containing corticosteroids are contraindicated in all camelids and will probably lead to abortion. Complications are rare; females with severe prolapse should be treated with broad-spectrum antibiotics and monitored for abortion; the risk of recurrence in the subsequent pregnancy is considered high and the authors advise against rebreeding the female after parturition; a genetic predisposition is suspected but not proven.

Uterine torsion

  • Significance. «By far» the most common emergency of the pregnant alpaca and llama and, according to the authors, the probable leading cause of dystocia (references 61-63). It presents from 8 months to term, when the fetus is heavy enough to sustain the position. Contributing factors considered mostly speculative: the rolling behavior of the female and the size of the fetus; a summer tendency is suspected in relation to dusting behavior. The text observes that torsion is rare in the South American literature compared with North America and Australia and proposes as explanations the lower birth weight and smaller size of South American alpacas, besides the great variability of gestation length outside South America.
  • Clinical picture. The most common presenting complaint is a mild to moderate change in behavior; acute unrelenting pain is rare, except in severe torsions. Signs: reduced appetite, frequent visits to the manure pile, looking at the abdomen, lateral recumbency; in advanced cases only depression, diarrhea and anorexia. In severe torsions vascular occlusion compromises uterus and fetus, and tachycardia, tachypnea and vocalization are added. About 30 % of the cases referred to their service for discomfort in late pregnancy are not torsions (denominator not reported); noticeable discomfort is common in maiden females at the end of gestation. Time to referral is critical, and the prognosis is worse in females with prolonged anorexia, depressed, recumbent or in shock; the longer the blood supply is reduced, the more friable the uterus, and only surgical options remain because of the risk of rupture during rolling (reference 64).
  • Diagnosis. Transrectal palpation with great caution and plenty of lidocaine-infused lubricant, with sedation or epidural if needed; most torsions are precervical, so vaginal examination shows no spiraling; the most reliable method is palpation of the broad ligaments (tension, position of the fetus, usually more cranial than normal). In clockwise torsion the left broad ligament stretches diagonally over the uterus and the right, shorter one is trapped under the uterine body. If the direction remains uncertain, exploratory laparotomy is done. Camelids ovulate from both ovaries but carry the fetus only in the left horn, and this is proposed as the reason for the higher incidence of clockwise torsion (reference 65); counterclockwise torsion is nonetheless frequent and cannot be explained by pregnancies in the right horn, and the authors have seen an abnormal fetal position in several term cases. Torsions are classified by degree (90, 180, 270, 360); those of 90° may be underrepresented because the female is not brought to the veterinarian. At term, torsion must be distinguished from other causes of dystocia (fetal malposition, failure of cervical dilation).
  • Transabdominal ultrasonography. It assesses fetal viability (heart, heart rate): > 130 beats/minute may indicate fetal stress and < 50 beats/minute leads to considering cesarean section at once without attempting rolling (reference 61); a severe or chronic torsion thickens the uterine wall (and may cause placental separation in labor). In mild to moderate cases with normal fetal heart rate and normal wall, rolling is the first choice. Color Doppler is useful for uterine perfusion and for fetal viability, also on the fetal aorta, liver, kidney or umbilical cord if the heart is not accessible.
  • Rolling. Rolling of the animal while fetus and uterus are held still in the abdomen: 3-4 people (one controls head and neck, one the front limbs and shoulder, one the hindlimbs and hips, and the person guiding holds fetus and uterus with the hands); the female is rolled in the same direction as the torsion (in clockwise torsion, starting from right lateral recumbency, then dorsal, left lateral, sternal), then repositioned and rolled again, three to four times; correction is recognized on transrectal palpation when both broad ligaments and the fetus are easily reached. The authors write that it is not possible to overroll and cause a torsion in the opposite direction, that in large llamas heavy sedation and a plank to hold the fetus are needed, and that the correction should not take more than four or five attempts. Intramuscular butorphanol if the abdomen is not relaxed enough (dose not reported). The direction must be diagnosed with certainty because it determines that of the rolling.
  • Surgery. If rolling fails or the female worsens; if the fetus is at term or severely compromised or the female is debilitated, cesarean section. In the llama the approaches are ventral midline celiotomy and left flank laparotomy (reference 63), the latter more advantageous in large debilitated animals or in the field; in the alpaca the preferred approach is midline celiotomy, with a risk of respiratory failure in debilitated animals in dorsal recumbency because of the small lung field. At term the fetus must be delivered before attempting the correction. Before surgery: packed cell volume and total protein at the very least, and in debilitated animals complete blood count and biochemistry, correction of electrolytes, dehydration and shock with intravenous fluids (catheter in the right jugular vein), with a cautious infusion rate because of the risk of pulmonary edema.
  • Outcome. The main factors are early diagnosis and proper management. «In our hands» rolling has a better prognosis in torsions far from term, cesarean delivery at term for the survival of the cria (reference 64), but the choice does not depend on the length of gestation alone: the best prognosis is when the mammary glands are well developed and colostrum is present. Crias delivered by cesarean section often need resuscitation, and surgery should be contemplated only if intensive care is immediately available. Complications of a neglected torsion: uterine rupture, peritonitis, hemorrhage; one case of concurrent splenic torsion (reference 66). Recurrence in the same pregnancy or in subsequent ones is possible. The authors' emergency protocol is in figure 23-13 (not reproduced).

Prolonged pregnancy

Owners calculate the due date on 345 days and become concerned beyond 355-360; pregnancies of up to 380 days are not rare; the longest the authors documented with a single breeding date was 396 days, and they consulted on a case of 400 days in a llama. Suspected causes: fescue toxicosis, abnormal placentation with intrauterine growth retardation, fetal abnormalities, nutritional deficiencies, very harsh winters. The approach: verification of dates and of pregnancy diagnosis, assessment of fetus and placenta; prolonged pregnancy alone does not justify induction; many females turn out not to be pregnant; the most likely cause is intrauterine growth retardation, so induction often results in loss of the cria because of immaturity. «In our experience» beyond 370 days there is a risk to the cria, and if the fetus is alive and the female comfortable the best approach is to wait for spontaneous parturition, prepared to intervene on the cria; placentas are often smaller or have avillous areas; crias born after a very long gestation are mostly smaller than average, not larger.

Other conditions

Uterine rupture and body wall herniation are described in a few cases, usually complications of torsion or of abdominal surgery. Mummification is called «extremely rare» because the species depends on the corpus luteum and fetal death is followed by immediate expulsion, but in the same paragraph it is stated that maceration and mummification are more common in females with fetal death (see the discrepancies). The authors are not aware of any case of fetal hydrops.

Authors' interpretation (attributed, not merged with the data)

«Torsion of the gravid uterus is, by far, the most common emergency in pregnant alpacas and llamas.» — It is a clinical judgment without a denominator: the chapter does not give the frequency of torsion among pregnant females.

«Hypoluteoidism is likely overdiagnosed and overtreated in female alpacas and llamas that experience recurrent pregnancy loss.» — It is a judgment, supported by one reference and by the authors' own cases, not by a series.

«The producer should be counseled that any abortion occurring in any female should be taken very seriously.» — A recommendation, not a result.

Implicazioni cliniche

Clinical implications

For the Italian veterinarian who follows an alpaca or llama farm, and for the association that has them in its care.

  1. An abortion is worked up as if it were the first of many. Isolate the female, collect the whole fetus and placenta, do not freeze, cool on ice and send early: this is the part of the chapter that does not depend on any doubtful datum. The authors themselves recall that even so the cause is rarely found (the diagnostic yield «rarely reaches 30 %»), and this is a reason not to be discouraged if the laboratory answer is negative.
  2. Early ultrasound monitoring has a practical sense: two corpora lutea at 14 days are an alarm for twins. The authors' schedule (14, 25-30, 45-60 days) is their practice and is not compared with others; its value lies in the fact that twinning resolves by itself in the great majority of cases by 45 days (95 % in their clinic, denominator not declared) and that the cases that do not reduce mostly end with an abortion at 4-7 months.
  3. Uterine torsion is thought of in every late-pregnancy female with vague pain. About 30 % of the cases referred to their service for discomfort were not torsions: palpation of the broad ligaments decides. Fetal heart rate on ultrasonography (> 130 or < 50 beats/minute as the authors' thresholds) and the thickness of the uterine wall guide the choice between rolling and cesarean section; these are thresholds of clinical practice and have not been validated in a study.
  4. Rolling is done only as a team, in the right direction and slowly. If the direction is uncertain, the authors move to exploratory laparotomy. In the field, whoever cannot perform a cesarean section must have a referral plan before it is needed.
  5. Vaginal prolapse: no topical corticosteroids, no fingers, no rebreeding. These are three clear indications of the chapter; the card reports them as the authors' practice. The Buhner suture with umbilical tape is described in enough detail to be replicated, and it must be removed before parturition.
  6. Hypoluteoidism must not be taken for granted. The chapter says it is overdiagnosed and overtreated; it offers a progesterone threshold (> 2 ng/mL) and contradicts it with its own cases at 1.5 ng/mL. Whoever treats with hydroxyprogesterone caproate must know that the dose in the table has no specific source and that prolonged treatment is associated, according to the authors, with failure of cervical dilation.
  7. Do not vaccinate pregnant females in the first 90 days, and use multivalent vaccines with caution. The recommendation is the authors' and rests on the association between 8-way vaccines and early loss (reference 50, not read).
  8. Drugs that seem harmless can cause abortion. Prostaglandins, systemic corticosteroids and even a steroid eye solution (4 females, the authors' clinical observation) are flagged. For an association that administers drugs to pregnant females in its care this is the point to remember.
  9. For Neospora and Toxoplasma the chapter gives South American and United States numbers, not Italian ones. Card CRL-I-03 reports the Italian ones (506 camelids; seropositivity for T. gondii was 34.8 %, for N. caninum 5.7 %); the two series do not add up and they say that encountering the parasite is not abortion. For dogs (Neospora) and cats (Toxoplasma) the chapter recommends limiting contact with camelids and their feces or feed.
  10. Many agents on the list are zoonoses. The authors ask that the owner be warned of the zoonotic risk of «many of the abortion-causing agents» and expressly name Q fever (Coxiella) as a serious risk to human health in the camel. Whoever handles an abortus does so with gloves and designated clothing; this indication is the editorial team's, not the chapter's.
  11. The numbers of the chapter must not be copied without care. Four percentages do not add up (see Limitations), and the epidemiologic data are North American, New Zealand, Peruvian and Argentine.

Limiti

Dichiarati dagli autori.

  • Pratica clinica presentata in forma assertiva. Molte indicazioni cliniche («the best approach is to wait», «should be discouraged») sono esperienza degli autori; alcune sono marcate come tali (27 «we/our»), altre no.
  • Fonti geograficamente disomogenee: alpaca neozelandesi, peruviani e inglesi, lama argentini, casistica nordamericana, dromedario; nessuna fonte italiana o dell'Europa continentale.

  3. BVDV, 82 % contro n = 10. L'infezione transplacentare è data all'82 % e la nascita di un cria PI al 70 % dei casi (n = 10): 70 % di 10 è 7, ma 82 % non corrisponde a nessuna frazione di 10 animali (8/10 = 80 %, 9/10 = 90 %). Il denominatore dell'82 % è diverso o mancante.
  4. Alpaca neozelandesi, 25,7 % di 158. La perdita del 25,7 % dopo il 30° giorno non corrisponde a un numero intero di 158 gravidanze (25,7 % di 158 è 40,6; 40/158 = 25,3 %, 41/158 = 25,9 %), e le perdite del 9,6 %-16,7 % dopo il 120° giorno non corrispondono a frazioni intere di 158 (15/158 = 9,5 %; 26/158 = 16,5 %): o sono calcolate su sottogruppi non dichiarati, o i numeri sono approssimati.

◆ COMMENTO DELL'ASSOCIAZIONE

Camelid Research Studies APS — redazione CRL · 12/10/2026

Valutazione a stelle

SANRA compilata dalla redazione — 6/12.

Voce SANRAPunteggioMotivo

Limitations

Declared by the authors. The chapter has no section on limitations; it declares however at several points that the datum is missing: «No epidemiologic data on the incidence of pregnancy loss in alpacas and llamas exist»; the role of equine herpesvirus 1, equine arteritis virus and bluetongue in abortion is «unknown»; for hypoluteoidism the incidence is unknown; for toxoplasmosis «the effects on pregnancy remain uncharacterized»; the efficacy of BVDV vaccines in pregnant females is unknown; for coxiellosis in the llama and the alpaca «more investigation» is needed, and for brucellosis the role «has not been fully investigated».

Added by the editorial team.

  • No literature search declared: the selection of the works is not reproducible.
  • References absent from the deposited text: the 66 numbers in the body have no corresponding work in the Europe PMC version. No statement attributed to a reference is verifiable by this card.
  • Clinical practice presented in assertive form. Many clinical indications («the best approach is to wait», «should be discouraged») are the authors' experience; some are marked as such (27 «we/our»), others are not.
  • Thresholds without validation: fetal heart rate (> 130 or < 50 beats/minute), uterine thickness, 370 days of gestation as a risk limit, 3-week quarantine, screening of 10 % or 15 animals. None has a measure of accuracy in the text.
  • Missing denominators: seroprevalences of Leptospira, Neospora and Toxoplasma (except 15 and 50 abortions), the 36 % of fecal cultures positive for Campylobacter, the 95 % of reduced twins, the 50 % mortality of crias with BVDV, the 30 % of non-torsions.
  • Geographically heterogeneous sources: New Zealand, Peruvian and English alpacas, Argentine llamas, North American caseload, dromedary; no Italian or continental European source.
  • Extrapolations from other species in placentitis (mare), in the vaccination threshold, in mucosal disease (bovine) and in obstetric practice.
  • Table 23-1 not reproduced for licensing reasons; in the deposited version the exponents seem to be lost, so some formulas of the deposit cannot be interpreted with certainty.
  • Twelve years of distance: the chapter is from 2014 (or from 2013 according to the electronic date). It was not searched, and it is therefore not excluded, whether after 2014 the literature on pregnancy loss, on BVDV in the camelid and on uterine torsion has changed the indications.
  • Cited sources not read: everything this card says about a cited work it says through the chapter.

Discrepancies verified by the editorial team on the text (none was resolved with the primary works, which were not read):

  1. Cloprostenol, 92.5 % versus 82.9 %. The text writes that the majority (92.5 %) of the animals aborted at 3 days (45.2 %) or at 4 days (37.7 %) after the series of injections. 45.2 + 37.7 = 82.9, not 92.5: 9.6 points remain unexplained, and the chapter does not say to which day they belong.
  2. Sarcocystis in Argentina, 96 % plus 75 %. The text writes that 96 % of the samples were positive for only one genus and 75 % for both: two shares that should be mutually exclusive and that add up to 171 %. The denominator is not reported; the card does not know which of the two numbers is wrong.
  3. BVDV, 82 % versus n = 10. Transplacental infection is given as 82 % and the birth of a PI cria as 70 % of cases (n = 10): 70 % of 10 is 7, but 82 % does not correspond to any fraction of 10 animals (8/10 = 80 %, 9/10 = 90 %). The denominator of the 82 % is different or missing.
  4. New Zealand alpacas, 25.7 % of 158. The loss of 25.7 % after day 30 does not correspond to a whole number of 158 pregnancies (25.7 % of 158 is 40.6; 40/158 = 25.3 %, 41/158 = 25.9 %), and the losses of 9.6 %-16.7 % after day 120 do not correspond to whole fractions of 158 (15/158 = 9.5 %; 26/158 = 16.5 %): either they are calculated on undeclared subgroups, or the numbers are approximated.
  5. «No epidemiologic data exist» and then the data. The chapter states that epidemiologic data on the incidence of pregnancy loss in alpacas and llamas do not exist, and a few lines later (and in the opening) reports 7-12 % of losses with two references and 25.7 % from a survey of 158 pregnancies. The statement and the data coexist in the text without reconciliation; the 25.7 % is more than double the upper limit of the 7-12 %.
  6. The left horn and the fetuses in the right horn. The text writes that camelids «only carry their fetus in the left uterine horn»; further on it describes reduced twins with the retained fetus in the right horn and writes that counterclockwise torsion «cannot be explained by pregnancies in the right horn», as if those pregnancies were a hypothesis. The absolute statement and the cases described cannot be reconciled.
  7. Mummification. Mummification is «extremely rare» because fetal death is followed by immediate expulsion, but in the next sentence maceration and mummification are «more common» in females with fetal death. The comparison is not declared (more common than what?).
  8. Bibliographic record. Year 2013 (Europe PMC, OpenAlex, PubMed Central) versus 2014 (Crossref, copyright); citations 9 (OpenAlex), 5 (Crossref), 0 (Europe PMC); references 66 (Crossref) or 69 (OpenAlex) versus no entry in the deposited text; ORCIDs attributed by OpenAlex and absent from the other records. The card reports all the versions and does not choose.

Funding: not declared in the text. No funder in the Crossref record or in the Europe PMC one (verified on 11/10/2026).

Conflicts of interest: not declared in the text (no declaration found in the deposited text).

What is missing with respect to the reporting standard (SANRA). The description of the literature search (item 3) is missing altogether. Partial: the justification of importance (item 1), the formulation of aims (item 2, generic), referencing (item 4: the references are not in the deposited text and many indications are practice without a source) and the presentation of data (item 6, for the missing denominators and the percentages that do not add up). Outside SANRA but within the rules of the series: funding and conflicts not declared, no section on limitations.

◆ ASSOCIATION COMMENTARY

Camelid Research Studies APS — CRL editorial team · 12/10/2026

A chapter of obstetric practice to keep on the table for its procedures, and to read with a pencil for its percentages.

The merit of this chapter is that it is written by those who have the pregnant females in front of them. Uterine torsion is explained with palpation of the broad ligaments, the ultrasonographic threshold of fetal heart rate, the number of people needed for rolling, and the moment at which one stops rolling and opens. Vaginal prolapse has the Buhner suture with the needle and the tape. Abortion has a map of where to sample the placenta. These are pages that a field veterinarian can follow with the chapter in hand, and for this reason the card gives this axis one star more than it would for a theoretical chapter.

Now the part that makes us reflect as an editorial team: the numbers. We found four percentages that do not add up with their parts or with their denominators: cloprostenol (92.5 % versus 45.2 + 37.7 = 82.9), Argentine Sarcocystis (96 % and 75 % as shares that should be mutually exclusive), transplacental BVDV infection (82 % in a group of 10 animals) and the New Zealand losses (25.7 % of 158). None changes what the chapter recommends; but they are the ones that anyone cites from a review, because they are the easiest to copy. A number that passes from a chapter to a card to a stable notebook loses its denominator along the way, and if the denominator was not even there at the origin no one will find it again. For this reason the card reports next to every percentage what the text says about the denominator, even when it says nothing. The two background discrepancies, the one between «no epidemiologic data exist» and the data reported on the same page and the one between the fetus «only in the left horn» and the fetuses in the right horn, are instead inconsistencies of the text that do not touch the procedures.

The comparison with the review on infectious reproductive losses (C-06) is instructive. That one is largely about camels and gives few denominators; this one is about the llama and the alpaca and gives them more often, but remains second-hand. The Neospora ratio of 3 out of 15, which card C-06 flags as the only denominator of its source, appears here too (reference 40): it is probable, but the editorial team has not verified it, that the two reviews draw on the same work, and in that case they would not be two confirmations. For Italy the most useful comparison is with card I-03: seropositivity for T. gondii of 34.8 % and for N. caninum of 5.7 % in 506 Italian camelids, against the South American and United States numbers of this chapter. The chapter recalls that seropositivity is an encounter with the parasite, not abortion, and the 50 Peruvian fetuses without Toxoplasma DNA confirm it.

A sentence for whoever has a term female in hand. The authors say that in about 30 % of the cases referred for discomfort in late pregnancy it was not a torsion, that rolling is done slowly while holding the fetus still, that at term cesarean section has, in their hands, the better prognosis for the cria, but the choice does not rest on the length of gestation alone, and that the cria born this way needs resuscitation at once. These are indications from a clinic that has intensive care available; on a farm without that possibility, the part that remains applicable is early diagnosis and a referral plan decided beforehand. For the cria born after the cesarean, card C-07 (neonatal diseases) and C-08 (the neonatology chapter of the same manual) are the readings that follow.

Star rating

SANRA compiled by the editorial team — 6/12.

SANRA itemScoreReason
1 · Importance1/2It opens with the rates of pregnancy and loss (78 %, 7-12 %, 45 %) and with torsion as an emergency, but does not argue why a review is needed now.
2 · Aims and questions1/2Aim declared but generic: to discuss pregnancy loss and the other diseases of gestation. No defined question.
3 · Description of the literature search0/2Absent altogether.
4 · Referencing1/266 reference numbers in the body, but the list is not in the deposited text; many indications are the authors' practice without a source; one source (table 23-1) is «adapted» from a review by the group.
5 · Scientific reasoning2/2It consistently distinguishes its own practice from the literature, treats opposing positions (the progesterone threshold, hypoluteoidism as overdiagnosis, the counterclockwise torsion that the left horn does not explain) and declares what is not known. The two internal contradictions do not change the judgment.
6 · Presentation of outcome data1/2Numbers with species almost always; denominators often absent and four percentages that do not add up.

Conversion SANRA → stars according to chapter 14 §11: 6-8 → ★★★☆☆.

AxisScoreRationale
A · Scientific quality★★★☆☆SANRA 6/12, with a zero on the literature search. Cap of chapter 14 §11: a narrative review does not reach five stars on this axis.
B · Clinical utility★★★★☆It describes replicable procedures with explicit conditions of use (palpation of the broad ligaments, ultrasonographic thresholds, rolling, Buhner suture, map of placental sampling, ultrasound schedule). It does not reach five because the thresholds are unvalidated practice and the doses have no specific source; within the constraint B ≤ A + 2.
C · Educational value★★★★☆A chapter ordered by reproductive event, with differential, described cases and figures that lend themselves to teaching. It does not reach five because the list of references is not in the deposit, because there are percentages that do not add up and because it dates from 2014.
D · Relevance to the species★★★★☆A chapter entirely devoted to llamas and alpacas. The procedures are transferable to Italy; the epidemiologic data (New Zealand, Peru, Argentina, North America, a single outbreak in the United Kingdom) are not continental European. It does not rise to five for this reason and because in some sections it uses data from the dromedary and other species.

No overall score, by construction. A = 3, B = 4, C = 4 and D = 4 is the signature of a good review of practical clinical work: reliable as a guide to the procedures, fragile as a source of numbers.

Bottom line

It can be concluded that a textbook chapter of 2014, written by three authors, describes for the pregnant alpaca and llama a pathway for abortion (submission of the whole fetus and placenta, not frozen), an ultrasound schedule (14, 25-30, 45-60 days), a rolling technique and a Buhner suture; that the authors declare undocumented or unknown the incidence of pregnancy loss, the role of various viruses in abortion and the efficacy of the BVDV vaccine in pregnant females; and that for hypoluteoidism they write that it is probably overdiagnosed and overtreated.

It cannot be concluded that the percentages of the chapter are reliable: four do not add up with their parts or with their denominator; nor that the doses are proven in the camelid, because only the dose of cloprostenol for abortion at 4-7 months has a cited work (not read), and the others are the authors' practice or have no source; nor that the ultrasonographic and time thresholds are validated; nor that the epidemiologic data hold for Europe. No datum on Italy can be drawn from here.

Messaggi da portare a casa

Take-home messages

  1. A narrative review is cited with its subject. Every number in this card is «the authors report»; the primary works were not read and their list is missing from the deposited text.
  2. An abortion is worked up with the whole fetus and placenta, not frozen and on ice; the diagnostic yield remains low («rarely reaches 30 % of all submissions») even so.
  3. Two corpora lutea at 14 days are an alarm for twins. In the authors' clinic 95 % of twin pregnancies reduce to one or are lost within 45 days; if one fetus is not eliminated by 45 days, most end with an abortion in mid- to late gestation.
  4. Uterine torsion is the most common emergency of the pregnant female, according to the authors: it is diagnosed by palpating the broad ligaments; about 30 % of the cases referred for discomfort were not torsions; the female is rolled slowly, by 3-4 people, in the direction of the torsion, and for no more than 4-5 attempts.
  5. Vaginal prolapse: no topical corticosteroids, no pressure with the fingers, Buhner suture removed before parturition, rebreeding advised against.
  6. Hypoluteoidism is probably overdiagnosed; the threshold of > 2 ng/mL has exceptions at 1.5 ng/mL observed by the authors.
  7. Prostaglandins, corticosteroids (even as eye drops) and multivalent vaccines in the first 90 days can cause loss of the pregnancy.
  8. Four percentages of the chapter do not add up (cloprostenol, Sarcocystis, BVDV, New Zealand losses): they are cited with their problem, not alone.
