Che cosa misurano i quattro assi
A · Qualità scientifica
La domanda è ben posta? I metodi sono dichiarati? La statistica è appropriata? La conclusione sta dentro i dati?
B · Utilità clinica
Serve a chi ha l’animale davanti, oggi.
C · Valore didattico

What the four axes measure
A · Scientific quality
Is the question well posed? Are the methods stated? Is the statistics appropriate? Does the conclusion stay within the data?
B · Clinical usefulness
Useful to whoever has the animal in front of them, today.
C · Teaching value
Does it teach something to those studying the subject?
D · Relevance to the species
Is it really about camelids, or a result borrowed from another species?

Each axis is worth 1 to 5 stars and carries its written reason. The stars in the index are the rounded average of the four.

Titolo originale

«Infectious and non-infectious causes for pregnancy loss in South American camelids – A review»

Original title

«Infectious and non-infectious causes for pregnancy loss in South American camelids – A review»

⚠️ This is a review with a declared search method, but the data it gathers are mostly descriptions of a single animal. For nine infectious agents (equine arteritis virus, Brucella abortus, Listeria monocytogenes, Streptococcus sanguinis, Sarcocystis, Encephalitozoon cuniculi, Corynebacterium pseudotuberculosis, Yersinia enterocolitica, Leptospira in England) the text describes an abortion in a single animal or a single fetus. The review does not add up, weigh or calculate frequencies among the causes: it is a catalogue of what has been seen, by whom, in which country, not an estimate of how much each cause weighs. No number in this card is an original result of the authors; every number is reported, and where the source is someone else's work the card says so.

⚠️ Second warning: the editorial team found in the text eight points where the counts, the citations or the declared scope do not add up (denominators that do not square, percentages incompatible with each other, references whose title does not correspond to what the text attributes to them). They are listed one by one in the Limitations field, and declared as discrepancies verified by the editorial team on the text: they are not resolved with the primary works, which were not read.

Licence of the original article: Creative Commons CC BY-NC-ND 4.0, verified in the PDF (the copyright line at the foot of the first page reads «This is an open access article under the CC BY-NC-ND license» with the link to the licence text) and consistent with Crossref (publisher's version, CC BY-NC-ND licence), OpenAlex and Unpaywall («hybrid» open access, CC BY-NC-ND licence both on the publisher's page and in the deposit). The «NoDerivatives» restriction weighs on a card like this one: the card does not republish the text, the tables or the figure; it restates the factual data in our own words and reports brief quoted citations; the article's two tables (infectious agents and non-infectious causes, with species, place and citation) and the flow chart are described in words. The editorial team is a non-profit association and does not use the card for commercial purposes.

Specie e soggetti

Nessun soggetto proprio.

Species and subjects

No subjects of its own. The review concerns domestic South American camelids, in practice alpacas and llamas. Of the 49 included works, 24 concern only the alpaca, 19 alpaca and llama together and 6 only the llama. By continent of origin: 26 North America, 12 Europe, 7 South America, 4 Australia (total 49). The data reported therefore come mostly from United States, English and Swiss alpacas and llamas; a single work in the list of non-infectious causes is Italian (pregnancy toxemia in two alpacas). The text invokes the bovine and the horse as points of comparison for the definitions of abortion and for some mechanisms; the introduction also cites a work on the vicuña (see Limitations). The dromedary and the Bactrian camel are not covered.

Scopo dichiarato: «This review aims to consolidate all so far known infectious and non-infectious causes of abortions in SAC, providing veterinarians, field scientists and SAC owners with a comprehensive knowledge base.»

Study type and level of evidence

Review with a declared scoping method: five phases following the approach of Tricco et al. (2016), six databases, a PRISMA-style flow chart. The text does not cite the PRISMA-ScR checklist nor declare that it was followed. There is no quality appraisal of the included works. Declared aim: «This review aims to consolidate all so far known infectious and non-infectious causes of abortions in SAC, providing veterinarians, field scientists and SAC owners with a comprehensive knowledge base.»

Level of evidence: secondary and weak in substance. The search is described, so the reader can understand what was looked for, but the material gathered consists of single cases, small series, an experiment on a single animal (Brucella), a questionnaire survey, review articles by other authors and a toxicology manual of 1985, without the review ranking them. A cause described in one animal and a cause documented in eight fetuses appear with the same weight in the tables.

Consequence for the reader. The card summarizes what this review reports; it does not establish the frequency of any cause, because the review does not establish it. The fact that Coxiella burnetii was isolated in 5 of the 8 Swiss fetuses examined does not say that it is the cause of 62.5 % of abortions: it says that in that sample of submissions, chosen by whoever decided to send the fetus, it was present five times out of eight.

Relevant reporting standard: no reporting guideline exists for non-systematic reviews; the SANRA quality scale is used (chapter 14 §4.2). The authors declare no reporting standard. The score compiled by the editorial team, item by item, is in the ratings box: 8/12.

In breve

In brief

The authors searched six databases, up to the end of 2023, for everything published on the causes of abortion, stillbirth and perinatal mortality in South American camelids, and kept 49 works out of 5,234 initial records. The result is a reasoned list by agent. Among the infectious ones: bovine viral diarrhea virus (BVDV) and equine arteritis virus; Brucella abortus, Campylobacter fetus, Coxiella burnetii, Listeria, streptococci, Salmonella, Leptospira, Chlamydia and other bacteria; Toxoplasma, Neospora, Sarcocystis, Encephalitozoon; the fungus Coccidioides. Among the non-infectious ones: drugs (prostaglandins, dexamethasone), age and body condition of the dam, mineral deficiencies, pregnancy toxemia, dystocia, twin pregnancy, malformations, nitrate poisoning, umbilical cord torsion, stress. For most abortions, the authors say, the cause remains unidentified, and they conclude that new studies are needed and that every abortion should be investigated.

Terza ragione: dice in chiaro quanto poco è documentato.

Why this study matters

First reason: it is the only list, in one place, of what has been documented. A veterinarian who finds an aborted fetus on an alpaca farm today has before them textbooks that extrapolate from the bovine and small scattered reports. This review gathers them with a search described in its steps, and derives two consultable tables from it: one by infectious agent, with species and country, one by non-infectious cause.

Second reason: it lines up the agents that impose a legal obligation or a protection for those who work. Five diseases listed in the introduction as epizootics in Switzerland (brucellosis, BVD, chlamydiosis, salmonellosis, equine viral arteritis) are all cited for South American camelids (brucellosis only as an experimental infection in one llama), and the discussion recalls that they are included in the health code of the World Organisation for Animal Health. For a farm that lives alongside cattle or equids, and with people who assist at births, this is a fact to know.

Third reason: it says plainly how little is documented. The discussion writes that the field is under-researched, that many works are dated or rest on few sources, that some hypotheses are based on a single case or on personal communications. For trace minerals, for example, it admits that the hypothesis is not confirmed in South American camelids and that it is extrapolated from the bovine.

Fourth reason: it contains one of the few Swiss entries and an Italian one. Among the included works there is a recent Swiss one, from the same group, which examined eight fetuses (Coxiella burnetii in five of them), and an Italian one on pregnancy toxemia in two alpacas. They are the two points of the review closest to a European breeder.

Obiettivo

Domanda centrale dichiarata dagli autori: «What are the known infectious and non-infectious causes of abortions, stillbirths and perinatal mortality in SAC?»

Objective

Central question declared by the authors: «What are the known infectious and non-infectious causes of abortions, stillbirths and perinatal mortality in SAC?» («SAC» stands for South American camelids).

Clinical question (editorial formulation; the PICO format does not apply to a descriptive review): in a female alpaca or llama that aborts, gives birth to a stillborn cria or loses the cria in the first hours [P], which infectious and non-infectious causes are documented in the literature, in which species and country, and with what solidity of the data [outcome]?

Disegno dello studio

Study design

Review with a scoping method, in five phases (question, search, selection, data extraction, synthesis), built on the model of Tricco et al. (2016). Structure of the text: introduction with the background data; methods and flow chart; results with the synthesis of the search, two tables (infectious agents in alphabetical order; non-infectious causes in alphabetical order) and a description by agent (viruses, bacteria, parasites, fungi) and by non-infectious cause (iatrogenic, dam and management, fetus, miscellaneous); discussion with the biosecurity measures; a conclusion of three lines. Ten pages, 54 bibliographic entries (one is the terrestrial animal health code of the World Organisation for Animal Health).

⚠️ Not verifiable and therefore not asserted: the complete list of search strings by database (the text only says that the search was «tailored» to each), the number of reviewers who did the selection (not declared), and the existence of a protocol registered before the search (not declared).

Metodi / Protocollo

Ricerca. Termini: camelidi sudamericani, alpaca, lama, aborto, perdita di gravidanza, morte embrionale, nato morto, gravidanza patologica, perdita neonatale, mortalità perinatale, morte neonatale, e i nomi di diverse malattie (brucellosi, BVDV, herpesvirus equino, coxiellosi, salmonellosi, leptospirosi, rinotracheite infettiva bovina, neosporosi, toxoplasmosi, minerali in traccia).

Methods / Protocol

Search. Six databases (PubMed, Scopus, Embase, Ovid, Web of Science, ScienceDirect), up to the end of 2023, with no limit on date, language or study type at the search stage. Terms: South American camelids, alpaca, llama, abortion, pregnancy loss, embryonic death, stillbirth, pathologic pregnancy, neonatal loss, perinatal mortality, neonatal death, and the names of several diseases (brucellosis, BVDV, equine herpesvirus, coxiellosis, salmonellosis, leptospirosis, infectious bovine rhinotracheitis, neosporosis, toxoplasmosis, trace minerals). The citations were imported into a reference manager and duplicates removed by hand.

Exclusion criteria at selection by title and abstract. Works not in English, German or French; not on South American camelids; at molecular level; not peer reviewed; on diseases of males; conference proceedings; works in which the death of the cria occurred more than 48 [unit not stated in the text, presumably hours] after birth; works prior to 1990. Also excluded were works on embryonic loss within the sixth week, following the definitions of abortion used in other species (beyond 42 days of gestation in the bovine, beyond 40 days in the horse).

Doses. The review contains drug doses in a single section, the one on iatrogenic causes: two prostaglandins and dexamethasone. They are reported in the table that follows, each with dose, route, species and source, and with an indication of how much has been measured in the camelid. They are not the authors' experience: they are data from cited works, which the editorial team did not read. The card does not recommend them. A «high» dose of dexamethasone (the example given by the text) is not reported as a number: the text does not give its route of administration, and by the convention of the collection a number without a route does not appear.

Fluprostenol, to induce parturition40 µgintramuscularalpacaBravo et al. 1996 (number of animals not reported in the review)Yes, in the alpaca (cited work; the review reports that prostaglandins induce parturition «within 24 hours» and that the crias born this way take longer to stand than those born spontaneously, with no differences in the interval to first suckling, weight, serum IgG at 24 hours and vital parameters)
Cloprostenol, to terminate an unwanted pregnancy150 µgintramuscularnot stated by the text; the title of the cited work says llamaMemon and Stevens 1997, a single case (title: «a case report»)Yes, but in a single animal
Cloprostenol, to terminate an unwanted pregnancy250 µgintramuscularnot stated by the text; the title of the cited work says llamaSmith et al. 2000 (number of animals not reported in the review); 82.9 % of abortions occur within three or four days, in rare cases a second injection is neededYes, in the llama (cited work; denominator of the 82.9 % not reported)
Dexamethasone, at low doses0.0005 mg, 0.005 mg, 0.05 mg (animal weight not stated)intravenousalpaca (the review's table 2 says «alpaca, llama»)Bravo et al. 1996; Vaughan and Tibary 2006Yes, in the alpaca: no adverse effect on the pregnancies (number of animals not reported)

The text further reports that dexamethasone at high doses causes intrauterine death of the fetus in the alpaca: the fetuses were expelled six to nine days after administration, two dams had dystocia and had to be assisted, the stillborn crias and the fetal membranes were necrotic; it concludes that dexamethasone «cannot be used to safely induce parturition in alpacas». The review attributes this experiment to Bravo et al. 2009; in the bibliography, the 2009 work by Bravo is the one on cria body weight and perinatal survival in relation to the age of the dam (see Limitations). For the other non-infectious causes the review gives no doses.

Risultati principali

I passaggi tornano aritmeticamente (5.234 − 2.818 = 2.416; 2.416 − 2.348 = 68; 68 − 10 = 58; 58 − 9 = 49).

  • Virus dell'arterite equina (EAV).

  • Placentite necrotizzante senza agente.

  • Coccidioides posadasii e immitis.

In most examined cases, the reason of the abortion remains unidentified, and no causative agent can be found».

Main results

(everything that follows is «the review reports»; the primary sources were not read. Where the text does not give the denominator, the card says so.)

The search and the selection

  • 5,234 records identified in the databases; 2,818 duplicates removed, 2,416 assessed on title and abstract; 2,348 excluded; 68 works requested in full text, of which 10 not retrieved; 58 assessed for eligibility; 9 excluded (6 because they did not mention any abortion, 1 because not peer reviewed, 2 because prior to 1990); 49 included. The steps add up arithmetically (5,234 − 2,818 = 2,416; 2,416 − 2,348 = 68; 68 − 10 = 58; 58 − 9 = 49).
  • 49 works by continent: 26 North America, 12 Europe, 7 South America, 4 Australia. By species: 24 alpaca, 19 alpaca and llama, 6 llama. By topic the text writes that 36 publications deal with aspects of pregnancy loss, 6 specifically with perinatal mortality and stillbirths, 12 with both (see Limitations: these three figures do not add up to 49).

The starting context (introduction)

The text reports that the reproductive rates of South American camelids lie between 45.5 % and 65.9 % and that abortion rates range from 24 % to 34.1 %; neither figure has a denominator or is attributed to a species in the sentence (the sources are three works, one of which is on the vicuña: see Limitations). On the cause of abortions, the text writes that careful examination of the fetus and placenta often fails to reveal a clear cause.

Viruses

  • Bovine viral diarrhea virus (BVDV). A cited experimental work shows that acutely infected animals show few clinical signs or none; isolates from the llama and the bovine are very similar, and it is concluded that the most likely source is the bovine. The virus causes early embryonic death, abortion and stillbirth. Persistent infections with BVDV type 1b are possible in South American camelids (first described in 2005): persistently infected crias show high morbidity and mortality (stillbirth, chronic wasting, low-grade fever, atypical fleece, lethargy, inappetence, low birth weight, stunted growth, diarrhea, joint swelling, opportunistic dental, intestinal and respiratory diseases) and, because of their high viral shedding, are an important source of infection for other ruminants. No number of animals or prevalence is reported by the text; the table of agents lists eleven works for this virus, from the United Kingdom, the United States and Canada.
  • Equine arteritis virus (EAV). In a zoo in western Germany an alpaca aborted in late gestation; PCR on the organs of the fetus confirmed the virus. All 5 remaining alpacas (5 of 5) were positive for antibodies; of 5 donkeys in the same zoo, 2 were seropositive. There was no direct contact with equids, but visitors could feed both and the same caretakers looked after donkeys and alpacas. It is, according to the text, the only publication on EAV infection in South American camelids (the title of the cited work ends with a question mark: see Limitations).

Bacteria

  • Brucella abortus. No spontaneous infection described in South American camelids; an infection experimentally induced in one llama by inoculation into the conjunctival sac: after 43 days the dam aborted an eight-month fetus; the bacterium was isolated from the mammary gland, lymph nodes and placenta of the dam and from the kidney and lung of the fetus; the lesions were comparable to those of the bovine.
  • Campylobacter fetus fetus. In an alpaca herd in England: four abortions and one premature non-viable birth in six weeks, two to four weeks before term; reported abortion rate 6.25 % (herd size not stated); the bacterium was isolated from the gastric contents of the fetus and from the placenta; 9 fecal samples out of 25 from pregnant alpacas contained the bacterium. Possible source of infection: feces of carrier sheep, alpacas or wildlife; on the same farm there was a flock of sheep with no history of fetal loss.
  • Coxiella burnetii. Recently identified as an abortion agent in alpacas and llamas (Swiss work): of 8 aborted or stillborn fetuses examined, 5 had the bacterium isolated from fetal organs or placenta and 3 a corresponding placentitis. The text stresses the zoonotic potential and the high infectivity.
  • Listeria monocytogenes. A single case, in a llama, in late gestation: placenta with congestion, edema and an infiltrate of neutrophils and mononuclear cells; the bacterium was isolated in pure culture from the liver and lung of the fetus; the dam remained healthy.
  • Streptococcus sanguinis type 1. One abortion in a llama after about 270 days of pregnancy; pure culture from gastric fluid, liver, lung and spleen of the fetus, interpreted as septicemia.
  • Salmonella. The English animal and plant health agency confirmed 23 cases of salmonellosis in South American camelids between 2000 and 2015, including two abortions: one due to S. dublin and one to S. typhimurium.
  • Leptospira. In the kidney of one aborted alpaca fetus, in England; for the rest the text says that it has been described as a possible cause in North and South America, citing two reviews.
  • Opportunistic coinfections (Swiss). A fetus with severe bronchopneumonia: Escherichia coli in culture from liver, lung and placenta, with Coxiella in the placenta, so that E. coli is considered a secondary pathogen. In another abortion, a coinfection of Coxiella and Streptococcus pluranimalium; the dam had already aborted in a previous pregnancy.
  • Necrotizing placentitis without an agent. A study histologically examined 85 placentae to describe microanatomy and histopathology in abortions, stillbirths and normal births of alpacas and llamas. Placentitis is reported in 11.8 % «of the aborted placentae»; in five out of ten cases a bacterium was the sole infectious cause, in the other five the etiology remained undetermined (see Limitations, discrepancy on the denominator). Another case, Swedish, of necrotizing placentitis without an agent.
  • Other bacteria. Chlamydia (two reviews); Corynebacterium pseudotuberculosis, a single case in Sweden, with abscesses in the gravid uterine horn; Trueperella pyogenes found in biopsies of the genital tract of the llama and therefore cited as a possible abortifacient; Yersinia enterocolitica, one abortion in the United Kingdom.

Parasites and fungi

  • Toxoplasma gondii. In the United States, an alpaca delivered a stillborn fetus two days before the expected date: hydrocephalus, enlarged liver, protozoan cysts in brain and kidney; maternal antibody titer increased. In Switzerland antibodies were found in the fluids of one aborted fetus and one stillborn cria, without cysts; the text infers infection because, it writes, maternal antibodies do not cross the placenta in South American camelids (a statement without a specific source in the sentence), and the dams were positive too.
  • Neospora caninum. In a Peruvian work, the cause of 14 abortions out of 50 in mid to late gestation (28 %), plus three abortions reported in an earlier work. The text also writes that protozoal infection was found in 38 % of the fetuses examined, and that the typical lesions (multifocal non-suppurative encephalitis and meningoencephalitis) were present in only 3 fetuses out of 13 (26 %) with protozoa-associated lesions in the brain; with immunohistochemistry or PCR, 28 % of the aborted fetuses showed infection (see Limitations: these percentages are not compatible with each other).
  • Sarcocystis. One alpaca with severe sarcocystosis due to S. aucheniae: recumbency, abortion and death; cysts and hemorrhages in the skeletal muscles of the dam; no cysts or hemorrhages in the fetus and placenta.
  • Encephalitozoon cuniculi. One cria of an alpaca, at 290 days of pregnancy: placentitis, premature birth and perinatal death; DNA of the microorganism from the placenta; the dam was clinically healthy. The text proposes to consider it a possible cause of sporadic abortions and perinatal deaths.
  • Coccidioides posadasii and immitis. An abortion in the ninth month in California: dam with anti-coccidioidal IgG antibodies at a titer of 1:256 (quantitative immunodiffusion), euthanized; disseminated coccidioidomycosis in lungs and serosal membranes; multifocal pyogranulomatous nodules in placenta and fetus. A second work documents four further cases of abortion or neonatal mortality due to coccidioidomycosis and indicates that camelids in California are at higher risk than those in other states; the text infers that pregnant infected alpacas appear predisposed to abort even without other signs.

Non-infectious causes

  • Drugs. See the table of doses. Cloprostenol does not appear, according to the two cited works, to have negative effects on the future fertility of the dam.
  • Age of the dam. Birth weight and placental efficiency (weight of the cria divided by the weight of the placenta) increase with the age of the dam; dams older than 6 years give birth to heavier crias, with a peak at 9 years. Stillborn crias and crias that died shortly after birth were significantly lighter than the survivors. In a work by Bravo et al. (2009) 11.3 % of the crias of dams aged two to three years died against 5.9 % of the crias of dams aged four to twelve years (denominators and number of animals not reported by the text).
  • Vitamins and minerals. Deficiencies of copper, selenium and zinc are considered probable causes of reproductive losses, but the hypothesis, the text writes, «has not yet been confirmed in SAC» and is based on the bovine; vitamins A and E might play a role in early embryonic death, without proof. In a recent work the crias of dams receiving mineral supplementation had higher birth weight, with no differences for abortions, stillbirths or neonatal deaths.
  • Pregnancy toxemia. Described in two alpacas in late gestation (the title of the cited Italian work speaks of 6 and 10 months of gestation): in one case the fetus died in utero and was aborted with prostaglandins, after which the dam improved rapidly; in the other a premature cria was euthanized at 36 hours of life for failure of passive transfer and no response to therapy.
  • Dystocia. Anatomy and constitution of the dam, incorrect positioning or malformations of the fetus can cause it; most birth difficulties depend on abnormal presentation or position; other causes: genital abnormalities, infections, poor nutrition or obesity.
  • Nutritional status. Nutritional deficiencies, especially in maiden females, can cause abortions. A recent work finds a close correlation between sarcoptic mange in a herd and an increase in abortions and stillbirths of unknown etiology (the mange would act on the dam, not on the fetus). Obesity now more often causes problems at birth because of fat in the birth canal; obese animals are more prone to hepatic lipidosis; clinical observations suggest lower progesterone concentrations in obese animals and a higher risk of early embryonic loss (no data).
  • Plant poisoning. Nitrate-nitrite poisoning after consumption of oat hay: four adult alpacas died and a full-term fetus was aborted shortly before the death of the dam. In the aqueous humor of the fetus 10 mg of NO₃/L, in that of the adults 25 mg of NO₃/L; in the bovine values above 25 mg/L are considered diagnostic (from a toxicology manual of 1985). The text attributes the abortion to fetal hypoxia from maternal methemoglobinemia.
  • Twins. Twin conception is not rare in the alpaca, but two viable twins are almost never born; an incidence of twin pregnancies below 1 % has been suggested. Before day 45 of pregnancy a very frequent spontaneous reduction occurs; in a recent work 81 % of twin pregnancies ended in abortion (denominator not reported). A placenta with areas of villous hypoplasia is associated with abortion due to twins and leads to placental insufficiency and fetal distress from hypoxia.
  • Fetal malformations. Rare; cases of spina bifida, hydrocephalus and brachygnathia superior are reported, which can cause dystocia.
  • Miscellaneous. If placental insufficiency from villous hypoplasia is considered, the share of idiopathic abortions of the alpaca in the work of Schaefer et al. falls from 66.2 % to 47.9 % of the cases examined microscopically (number of cases not reported in the review). An umbilical cord torsion in the same work. Inbreeding is associated with embryonic loss (cited from a review); maternal stress, for example from transport, can cause abortion.

Discussion and conclusion of the authors

The authors write that the field is under-researched, that many works are dated or rest on limited sources, that some hypotheses derive from single cases and other causes are mentioned only in personal communications; and that «In most examined cases, the reason of the abortion remains unidentified, and no causative agent can be found». For biosecurity they recommend testing and isolating new animals before introducing them, isolating and treating sick ones, and preventing pregnant women from coming into contact with abortion material (the authors' recommendations, without a cited source). For management they recommend quality feeding with adequate mineral supplementation, regular assessment of body condition (remembering that abundant fleece can mask it) and monitoring of births and of pregnant animals to correct dystocia from incorrect positioning of the cria. In the conclusion they urge breeders and veterinarians to investigate every abortion thoroughly, because every case adds knowledge.

Implicazioni cliniche

  4. I farmaci contano quanto i microbi.
  7. I parti vanno sorvegliati.

Clinical implications

For the Italian veterinarian who follows an alpaca or llama farm, and for the association that has them in its care. What follows is the editorial team's reading of what the review reports; where an item is an indication of the authors, the card says so.

  1. Every abortion should be investigated, and the review explains why. The cause remains unknown in most cases, but every case adds a datum to a field in which many causes rest on a single animal. The authors ask this of veterinarians and owners; they give no sampling protocol, and the card does not invent one.
  2. Some agents are zoonoses and carry obligations of their own. Coxiella burnetii (isolated in 5 fetuses of 8 in Switzerland), Brucella, Chlamydia, Listeria, Salmonella, Leptospira, Toxoplasma are named as transmissible to other species or to humans. The authors recommend avoiding contact of pregnant women with abortion material (the authors' indication, without a source). The notification obligations for listed diseases are to be checked in the Italian and European Union legislation in force, which the card did not consult.
  3. The bovine is the most likely source of BVDV in camelids. The review places BVDV among the causes of embryonic death, abortion and stillbirth, and recalls that a persistently infected cria sheds virus towards other ruminants: on a farm that lives alongside cattle it is a hypothesis to keep in mind. No prevalence data are given by the review.
  4. Drugs count as much as microbes. Dexamethasone at high doses kills the alpaca fetus in the only experiment reported and is not to be used to induce parturition; prostaglandins induce parturition or abortion, with the doses of the table and the limits stated above. For abortion induced with cloprostenol the text reports no negative effect on subsequent fertility, from two works, one of which is a single case.
  5. Body condition and nutrition are the part of prevention that can be done on the farm. The authors recommend regularly assessing body condition (the fleece can mask it), providing quality basic feed and minerals in adequate quantities. On the side of the evidence: mineral supplementation is associated with higher birth weight but not with fewer abortions, stillbirths or neonatal deaths (a single work); the hypothesis on copper, selenium and zinc is extrapolated from the bovine.
  6. Twin pregnancy almost always loses. The text reports that two viable twins are almost never born and that in the cited work 81 % of twin pregnancies ended in abortion.
  7. Births need to be watched. The text links perinatal mortality to dystocia from incorrect positioning, which monitoring of births makes it possible to correct (the authors' indication).
  8. For Italy there is a single datum, and it is not a frequency. The only Italian work in the list describes two alpacas with pregnancy toxemia; no estimate of the causes of abortion on Italian farms can be derived from the review. For Toxoplasma infection in Italy and for antibiotic resistance and biosecurity on our farms there are dedicated cards in the collection (see Related CRL cards).

Limiti

Dichiarati dagli autori.

2005, il cui titolo nell'elenco bibliografico è «Toxoplasma gondii infection in adult llamas and vicunas in the Peruvian Andean region».
1996; nell'elenco, il lavoro del 2009 di Bravo è «Cria alpaca body weight and perinatal survival in relation to age of the dam».
2000, il cui titolo nell'elenco bibliografico è «Bovine viral diarrhea virus in New World camelids».

  • Casi singoli trattati come agenti.

Finanziamento: «This review did not receive any specific grant from funding agencies in the public, commercial, or not-for-profit sectors»; nessun finanziatore nel record Crossref (verificato l'11/10/2026).

◆ COMMENTO DELL'ASSOCIAZIONE

Camelid Research Studies APS — redazione CRL · 12/10/2026

Valutazione a stelle

SANRA compilata dalla redazione — 8/12.

Voce SANRAPunteggioMotivo

Limitations

Declared by the authors. The review has no section on the limits of its own method. The discussion does however declare the limit of the literature: few studies, many dated or on limited sources, some on hypotheses or single cases, other causes reported only in personal communications; for most abortions the cause is not found.

Discrepancies and points that do not add up, verified by the editorial team on the text and not resolved with the primary works (eight):

  1. The figures by topic do not add up to the number of works. The text writes that 36 publications deal with pregnancy loss, 6 only with perinatal mortality and stillbirths, 12 with both: 36 + 6 + 12 = 54, while the included works are 49. If the 12 were already included in the 36, the sum would be 42. Neither 54 nor 42 is 49.
  2. Three percentages for the fetuses with Neospora, not reconcilable. The text reports 14 fetuses out of 50 (28 %); a protozoal infection in 38 % of the fetuses examined (19 of 50, if the denominator is the same, which the text does not say); typical lesions in «3 of 13 (26 %)», but 3/13 is 23 %, while 13/50 is 26 %: the 26 % coincides with 13 of 50 and not with 3 of 13.
  3. The reference for Neospora in the first table does not correspond. Among the sources for Neospora caninum the table lists Chavez-Velasquez et al. 2005, whose title in the bibliography is «Toxoplasma gondii infection in adult llamas and vicunas in the Peruvian Andean region».
  4. The dexamethasone experiment is attributed to a work that, by its title, does not contain it. The text cites it as Bravo et al. 2009, the second table as Bravo et al. 1996; in the bibliography, the 2009 work by Bravo is «Cria alpaca body weight and perinatal survival in relation to age of the dam».
  5. Obesity and dystocia are attributed to Belknap et al. 2000, whose title in the bibliography is «Bovine viral diarrhea virus in New World camelids».
  6. The 11.8 % of placentitis «of the aborted placentae» is 10 out of 85, that is the share of the total placentae examined. The text says that the placentae were 85 among abortions, stillbirths and normal births and that the cases of placentitis were ten (5 + 5): 10/85 = 11.8 %. The percentage is referred to the aborted placentae only, whose number the text does not give.
  7. The methodological reference is not in the bibliography. The text bases the method on Tricco et al. (2016), but this entry does not appear in the list of 54 references.
  8. The declared scope does not coincide with the contents. The methods exclude embryonic loss within the sixth week (42 days) and works in which the death of the cria occurs beyond «48» (no unit), but the text discusses early embryonic loss from obesity and from deficiency of vitamins A and E, and one of the works cited in the introduction as a source of the abortion rates (Bravo et al. 2010) has embryonic mortality in its title.

Added by the editorial team.

  • Single cases treated as agents. Nine agents rest on a single animal or fetus (list in the warning at the top): the review places them in the table next to Coxiella (5 fetuses of 8) without distinguishing them by solidity.
  • No quality appraisal of the included works, declared or done. The text does not say how many reviewers carried out the selection.
  • Missing denominators throughout the section on causes: the 6.25 % of abortions from Campylobacter (herd size not given: the same 6.25 % would be obtained with 4 of 64 or with 5 of 80, and the text does not say which); the 82.9 % of abortions within three to four days of cloprostenol; the 81 % of twin abortions; the 11.3 % and the 5.9 % of mortality; the 66.2 % and the 47.9 % of idiopathic abortions.
  • Background figures with sources that are not on the same subject. The 45.5-65.9 % range of reproductive rate cites among its sources a work whose title speaks of ultrasonographic diagnosis of fetal mortality in the vicuña; the 24-34.1 % range of abortion cites two works whose titles speak of fetal mortality and of embryonic mortality. It is not made explicit to which event they refer.
  • Cause and coincidence. The text writes «causes» but the tables list what was found in an abortion: for Encephalitozoon, Sarcocystis, Leptospira the text reports the presence of the agent, not proof that it caused the abortion. For the arteritis virus the title of the cited work ends with a question mark («as the cause of abortion in alpacas?»), while the review writes that PCR «confirmed» the virus: the presence is confirmed, causality is posed as a question by the title.
  • Nitrate toxicity. The 25 mg of NO₃/L found in the adults do not exceed the diagnostic threshold of 25 mg/L that the text reports for the bovine (it is «greater than 25»); the text nevertheless attributes the poisoning to the oats. It is not a discrepancy, it is a value on the threshold, which the review does not comment on.
  • Pregnancy toxemia. The text speaks of late gestation; the title of the cited Italian work speaks of 6 and 10 months of gestation. In the alpaca pregnancy lasts about eleven months (a fact known to the clinic, not present in the text read): the editorial team flags the difference without resolving it.
  • Case-mix and countries. 53 % of the included works come from North America (26 of 49); the text gives no frequencies and no datum is transferable to Italy without verification.
  • Cited sources not read. Everything the card says about a cited work it says through the review.
  • Date. The search reaches the end of 2023; it was not searched, and it is therefore not excluded, whether new reports have appeared after that date.

Funding: «This review did not receive any specific grant from funding agencies in the public, commercial, or not-for-profit sectors»; no funder in the Crossref record (verified on 11/10/2026).

Conflicts of interest: the four authors declare that they have no conflicts of interest. Contributions (CRediT) declared in the text: Eggimann original draft, methodology, investigation, data curation, conceptualization; Rediger original draft and data curation; Hirsbrunner and Zanolari review, supervision, methodology, conceptualization.

What is missing with respect to the reporting standard (SANRA). Partial are: the description of the literature search (item 3: databases, terms, years, criteria and flow chart are there; the strings by database, the number of reviewers and the critical appraisal are not), the referencing (item 4: references not corresponding to the titles at three points, one methodological entry missing), the scientific reasoning (item 5: no hierarchy between single cases and series, no quality appraisal) and the presentation of data (item 6: absent denominators and irreconcilable percentages). Outside SANRA but within the rules of the collection: no section on the limits of the method.

◆ ASSOCIATION COMMENTARY

Camelid Research Studies APS — CRL editorial team · 12/10/2026

A review useful as an index of what has been seen, not to be read as a ranking of what happens.

The merit of this article is the search: six databases, listed terms, exclusion criteria, a flow chart whose steps add up. It is more than the review on the sick cria that we carded immediately before (C-07) had, which had no methods. Whoever opens a file in front of an aborted fetus has, for the first time in one place, the list of what has been described in the camelid, with the species and the country.

Now the part that makes us reflect as an editorial team: the weight of the causes. Nine infectious agents among those listed rest on a single animal or a single fetus; Coxiella rests on five fetuses out of eight, chosen by whoever decided to send them to a laboratory; Neospora on 14 abortions out of 50, with three percentages that do not reconcile with each other. In the tables of the article they all stand on the same level. A reader who scans the list, twenty entries of infectious agents and twelve of non-infectious causes, risks concluding that the alpaca aborts for thirty-two equally probable reasons. That is not what the text demonstrates: it demonstrates that from 1990 onwards someone has published those things, and that in most abortions the cause is not found. The difference between «it has been described» and «it is frequent» is the one thing this card cannot give you, because the review does not give it.

Second reflection: the eight points that do not add up. We do not flag them to wrong the authors; we flag them because a list of citations that do not correspond to the titles (Neospora, dexamethasone, obesity) is a list that whoever uses it must re-verify before citing it in turn. The dexamethasone case is the most serious, because it is the only strong practical indication of the text: not to use it to induce parturition. The indication must be found again in the primary work before being repeated in a clinical record.

The comparison with the 2006 review on reproductive losses from infection (C-06) is instructive. This review cites it several times as a source for llamas and alpacas, and it is one of the sources from which a good part of the list of agents descends. Card C-06 had found an abstract that says more than the text; here the abstract is cautious, but the promise of «a comprehensive knowledge base» is supported by a collection broad in reports and thin in evidence. The lesson the two cards give together is the same: for many causes of camelid abortion there is a single publication, and the work that gathers them cannot turn them into a frequency. Card C-10 (the manual chapter on pathologies of pregnancy) adds the other half, what a clinician does in front of an abortion, which is missing here.

A sentence for whoever has an abortion in hand. The text gives no protocol. It says, and we with it, that every abortion should be investigated: keeping the fetus and placenta and sending them to a laboratory that reads them with method makes the next review more useful than this one. On a farm with cattle or equids, and with people who assist at births, the part applicable today is caution towards zoonotic agents.

SANRA itemScoreReason
1 · Importance2/2Justified in the introduction with reproductive and abortion rates, economic losses, zoonotic risk and Swiss health obligations (with the reservations on the sources of the rates).
2 · Aims and questions2/2Declared aim and central question formulated in one sentence.
3 · Description of the literature search1/2Databases, terms, years, criteria and flow chart present; the strings by database, the number of reviewers and a scope coherent with the contents are missing.
4 · Referencing1/254 references; three attributions that do not correspond by title, one methodological entry missing, many statements without a source in the sentence.
5 · Scientific reasoning1/2It declares that many causes are hypotheses or single cases, but does not distinguish them in the tables; no quality appraisal; some unqualified inferences (maternal antibodies and placenta, nitrates).
6 · Presentation of outcome data1/2Species and country almost always present; denominators absent and percentages irreconcilable (fetuses with Neospora, placentitis, topics of the works).

AxisScoreRationale
A · Scientific quality★★★☆☆SANRA 8/12, the upper margin of the band. Ceiling of chapter 14 §11: a review does not reach five stars on this axis. It loses for the points that do not add up and for the failure to distinguish single cases from series.
B · Clinical utility★★★☆☆An index by agent and by cause that guides differential diagnosis and biosecurity, with the prostaglandin doses referred to works on the camelid. It does not rise to four because it gives no frequencies and no diagnostic protocol, and because the most practical datum (dexamethasone) has a reference to be re-verified; within the constraint B ≤ A + 2.
C · Educational value★★★★☆Clear structure (viruses, bacteria, parasites, fungi; then iatrogenic, management and fetal causes), a method that can be explained to a student, and frankness about the fact that the cause is often not found. It does not reach five because the tables treat a single case and a sample of fetuses on the same level.
D · Species relevance★★★★☆Entirely dedicated to alpacas and llamas, written in a European university clinic, with 12 works out of 49 European and a recent Swiss one on the same subject. It does not rise to five because only one of the 49 works is Italian and because the weight of North American data (26 of 49) makes the frequencies uncertain for Italy.

No overall score, by construction. A = 3, C = 4 and D = 4 is the signature of a good index review: reliable as a map of what exists, fragile as a source of numbers.

Bottom line

It can be concluded that a review with a search in six databases up to 2023 selected 49 works out of 5,234 records and derives from them a list of causes of abortion, stillbirth and perinatal loss in the alpaca and the llama (infectious agents: viruses, bacteria, parasites, fungi; non-infectious causes: drugs, dam, management, fetus); that for nine infectious agents the text describes a single animal or a single fetus; that Coxiella burnetii was isolated from 5 of 8 fetuses examined in Switzerland; that in the only experiment reported, dexamethasone at a high dose caused intrauterine death of the alpaca fetus; and that the authors themselves conclude that in most abortions the cause is not identified.

It cannot be concluded that any of these causes is frequent or rare, because the review reports no frequencies among the causes; nor that the list is free of citation errors, because the editorial team found eight among counts and references, not resolved with the primary works; nor that the dexamethasone experiment is verified, because the citation in the text and that in the table point to two different works. No frequency for Italy can be derived from here.

Messaggi da portare a casa

Take-home messages

  1. A review with a declared search is cited together with its subject. Every number in this card is «the review reports»; the primary works were not read.
  2. The review is a catalogue, not a ranking. Nine agents rest on a single animal or fetus; Coxiella on 5 fetuses of 8; no frequency among the causes.
  3. For most abortions the cause is not found, the authors write. Every abortion should be investigated.
  4. Some agents are zoonoses and listed diseases: brucellosis, BVD, chlamydiosis, salmonellosis, equine viral arteritis; Coxiella and Toxoplasma are reported in Switzerland. Pregnant women should not handle abortion material (the authors' indication).
  5. The bovine is the most likely source of BVDV in camelids, and the persistently infected cria sheds virus towards other ruminants.
  6. Dexamethasone is not used to induce parturition: at a high dose it caused the death of the alpaca fetus and dystocia in two dams in the experiment reported. The citation needs to be re-checked.
  7. Fluprostenol 40 µg intramuscularly in the alpaca induces parturition; cloprostenol 150 or 250 µg intramuscularly terminates a pregnancy in works on the llama (one is a single case). Doses to be referred to a formulary.
  8. Twins: 81 % of twin pregnancies ended in abortion in one work (denominator not reported); two viable twins are almost never born.
  9. Eight points do not add up in the text (counts, references, scope): see Limitations.
