Titolo originale

«Computed tomographic and radiographic examination of dental structures in South American camelid specimen of different ages»

Original title

«Computed tomographic and radiographic examination of dental structures in South American camelid specimen of different ages»

One word of the title must be read carefully, because it is the one that decides how the article is used: «specimen». Not «patients», not «animals». These are cadaver heads. The title says so; second-hand citations almost always lose it.

Autore corrispondente: Patrik Zanolari — Clinic for Ruminants, Vetsuisse-Faculty, University of Berne, PO Box 8466, Bremgartenstrasse 109a, 3001 Berna, Svizzera. Affiliazione come dichiarata nell'articolo.

Complete list, four authors, in the order and form of the Crossref record (queried on 31/08/2026 at api.crossref.org/works/10.1186/1746-6148-10-4). No et al..

ORCID: no ORCID is present for any of the four authors, either in the Crossref record or in the full text deposited in PMC. This is consistent with the period: the article is from January 2014, and ORCID adoption by veterinary publishers came later. This is not a gap in the article, it is a gap in the 2014 infrastructure — but it remains a fact for anyone needing to disambiguate the authors.

Corresponding author: Patrik Zanolari — Clinic for Ruminants, Vetsuisse-Faculty, University of Berne, PO Box 8466, Bremgartenstrasse 109a, 3001 Berne, Switzerland. Affiliation as stated in the article.

Affiliations of the other authors, as stated in the article: Rostami has two affiliations — Clinical Radiology, Department of Clinical Veterinary Medicine, Vetsuisse-Faculty, University of Berne, and Department of Surgery and Radiology, Faculty of Veterinary Medicine, University of Tehran, Iran; Geissbühler, the same clinical radiology department in Berne; Schellenberger is given with a private address (Sandweg 5, 79183 Waldkirch, Germany), with no institutional affiliation — in the article his stated role is having advised the first author on dental nomenclature.

Context relevant for the Italian reader: this is a Swiss group, from an alpine veterinary faculty with a ruminant clinic that sees camelids. The population the heads come from is therefore European, and this is one of the reasons the article transfers here better than an Andean case series would.

Specie e soggetti

Species and subjects

SpeciesFemalesMalesTotal
Llama (Lama glama)71017
Alpaca (Vicugna pacos)033
20

None of the animals had a history of disease or abnormality of the head.

Age, as stated by the authors: median 3 years and 3 months, range 9 months to 9 years. Age, as derived from Table 1 of the article (which publishes the age in months of each of the twenty subjects): the ordered values are 9, 9, 18, 19, 21, 30, 31, 32, 32, 32, 36, 38, 39, 41, 44, 61, 61, 67, 80, 107 months. The median of these twenty values is 34 months, i.e. 2 years and 10 months, not 39 months. The stated range (9-107 months) is instead correct.

This is not pedantry, and it is the reason this card publishes the full series. A five-month discrepancy on the median changes no conclusion of the article. It does, however, change how much trust can be placed in this work's summary figures without returning to the tables, and in an article whose main contribution is percentages on twenty subjects and mean radiodensities, that trust is the very thing at stake. Chapter 14 §5.2 requires that denominators be written out: here the denominator is present, and precisely because of that the inconsistency is visible. We prefer to state it than to inherit it.

The three alpacas, and why they count for little. They are three males, aged 67, 61 and 9 months. All percentages in this article have denominator 20 and are therefore, in substance, llama data: 17 of 20 subjects are llamas. The composition is not stated as a limitation by the authors; it becomes one in the limitations section added by the editors, and it weighs on axis D of the evaluation.

Study type and level of evidence

Design: descriptive anatomo-radiological study ex vivo on specimens (cadaver heads), with a standardized acquisition protocol and internal comparison between two modalities (radiography and CT) on the same subject. It is not a clinical study, has no patients, has no outcomes.

Level of evidence: it is outside the clinical study pyramid, and rightly so: the question is not therapeutic nor diagnostic in the strict sense, it is anatomical. The clinical evidence hierarchy does not apply to a description of normal anatomy, and applying it would be a category error.

Consequence for the reader — and it is twofold. What transfers directly to the live patient: the geometry (angles, beam centering points, focus-film distance, use of the mouth gag), the nomenclature, normal anatomy, radiodensities in Hounsfield units — because a Hounsfield unit is a property of the tissue, not of the animal's vitality. What does not transfer: the exposure parameters. 60 kV and 20 mAs on an isolated head, resting on the cassette, motionless and at room temperature, are not the right exposure for a live, sedated, breathing animal, with the soft tissues of the neck in the field. And the frequency percentages of the findings come from twenty subjects that are mostly llamas: they are orders of magnitude, not prevalences.

Relevant reporting standard: none of the grids in chapter 14 §3 is designed for a descriptive anatomical study on specimens. CONSORT and REFLECT are for trials, PRISMA for systematic reviews, STROBE for observational studies on populations, CARE for case reports, ARRIVE 2.0 for in vivo experimental research — and here there is no in vivo. What applies, then, as a lens rather than a grid, is the part of STROBE concerning subject selection, study size, and reporting of measures, stating that this is an adaptation. The authors state no standard, and in 2014 no one asked them to.

In breve

In brief

Before this work no description existed of the normal radiographic and tomographic anatomy of the teeth of the South American camelid — and dental problems are among the most frequent pathologies of the species. The authors took 20 healthy heads (17 llamas, 3 alpacas), scanned them by CT first, used the CT images to calculate the correct angles of the radiographic projections, and then radiographed them using five projections measured with the machine's goniometer.

Four usable things come out of it: the two best oblique projections, with their angles; the radiodensities in Hounsfield units of enamel, cementum, dentin and pulp, for permanent and deciduous teeth; a catalogue of normal findings that resemble pathology — starting with the caudal deviation of the distal root of the mandibular 09, present in 13 of 20 heads (65%) of healthy animals; and the observation, repeated in card CRL-F-04 eleven years later, that CT sees things that radiography does not see.

Why this study matters

Because it is the document one returns to in order to know what normal looks like, and it is cited in that role by the later literature: the study in card CRL-F-04, from 2025, lists it explicitly (reference [15] of its own bibliography) among the works that «have improved the understanding of the normal radiographic and CT anatomy of alpaca cheek teeth».

And for a reason that concerns diagnostic error more than anatomy. In a species whose normal is not known, everything looks pathological. This article lists, with their frequencies, a series of findings that a radiologist accustomed to the horse or the dog would report as lesions: deviated roots, roots touching each other, alveolar bone sclerosis, absent cementum with an exposed root canal, a common pulp chamber. They were found in the heads of healthy animals.

Obiettivo

Objective

To evaluate and optimize the technique for performing dental radiography in the South American camelid, and to describe the radiographic and tomographic anatomy of normal teeth at different ages.

It is not a PICO question, and forcing it into that form would betray it: there is no patient population, no intervention, no comparison between diagnostic strategies measured on an outcome. It is a descriptive question, and the card leaves it as such.

Metodi / Protocollo

Preparazione dei campioni. Ogni testa separata dal collo a livello dell'articolazione atlanto-assiale e congelata a −20 °C fino all'esame; scongelata tre giorni prima dell'acquisizione delle immagini. Nomenclatura: sistema Triadan modificato (Tabella 2 dell'articolo: I1-I2-I3 = 01-02-03; C = 04; PM1-PM4 = 05-08; M1-M3 = 09-11).

Gli autori contrappongono esplicitamente questo metodo a quello di un lavoro precedente che usava proiezioni a 45°, osservando che «the goniometer of the X-ray machine was used for determining the incident angle, which seems to be more reliable».

Protocollo TC. Scanner a 16 strati. Finestra: larghezza 2000 / livello 800 HU. Ricostruzioni multiplanari (MPR) e 3D.

Protocollo radiografico.

Methods / Protocol

Sample preparation. Each head separated from the neck at the atlanto-axial joint and frozen at −20 °C until examination; thawed three days before image acquisition. Nomenclature: modified Triadan system (Table 2 of the article: I1-I2-I3 = 01-02-03; C = 04; PM1-PM4 = 05-08; M1-M3 = 09-11).

Order of the two modalities — and this is a methodological point, not a detail. CT first, then radiography: the CT images were used to calculate the angles of incidence of the oblique projections, then reproduced on the radiography unit with the built-in goniometer. The authors explicitly contrast this method with that of an earlier work that used 45° projections, observing that «the goniometer of the X-ray machine was used for determining the incident angle, which seems to be more reliable».

CT protocol. 16-slice scanner. Specimens positioned with the hard palate parallel to the table plane. Transverse images with mouth open and mouth closed for each head. Dental algorithms for acquisition. 120 kVp · 200 mAs · pitch 0.563 · slice thickness 1 mm. Window: width 2000 / level 800 HU. Multiplanar (MPR) and 3D reconstructions.

Radiographic protocol. Stationary high-frequency unit with computed radiography system. Focus-film distance 60 cm · 60 kV · 20 mAs · without grid. 35.5 × 43 cm cassette for all projections except the intraoral VD, for which an 18 × 24 cm mammography cassette was used. The mouth gag: for the lateral and oblique open-mouth projections, a 20 mL radiolucent syringe, 21 mm outer diameter, placed horizontally between the maxilla and mandible caudal to the canines, to keep the arcades separated and avoid superimposition. The dorsoventral and intraoral projections were performed without a mouth gag. Positioning: heads on foam rubber; for the intraoral, head in dorsal recumbency with the cassette inserted into the mouth up to the caudal margin of the diastema; for the obliques, heads fixed in sternal recumbency on two vertical supports, cassette in contact with the surface in vertical orientation.

The five projections, with beam centering (Table 3 of the article).

Risultati principali (dati)

  • in un'alpaca di 9 mesi, deviazione della sola radice distale dell'08 deciduo;
  • con l'età compare sclerosi moderata dell'osso alveolare attorno alla radice, che oscura la lamina dura e la membrana parodontale;
  • intorno ai 30 mesi, l'apice della radice distale del 09 tocca la radice mesiale del 10, con un sottile strato di osso ancora interposto;

  • Premolari mandibolari: nessun infundibolo.

  • Incisivi permanenti dell'alpaca: rettangolari in sezione trasversa e a scalpello.

La loro conclusione finale è formulata così: «Future investigations are warranted to test the hypothesis that tooth root deformities, concurrent alveolar sclerosis and interdental tooth root contact might represent predisposing factors for dental disease in SAC.»

Gli autori dichiarano la propria cautela sul campione: «considering the rather small sample size and the lack of animals at specific age categories …, caution must be applied, as the findings might not be fully representative for the entire population of SAC.»

Main results (data)

The two best oblique projections, and why

Authors' conclusion, verbatim: the most useful projections for evaluating the cheek teeth proved to be the 30° lateral ventral-laterodorsal for the mandibular arcade and the 30° lateral dorsal-lateroventral for the maxillary arcade, with slight separation of the dental arcades. Stated reason: they give the least distortion of the reserve crown and roots and the least overlap of the arcades.

The mandibular incisors are best visualized on the intraoral VD.

How to read these two tables, and how not to. Each cell is the mean of two or three point measurements on a tissue, is not accompanied by a standard deviation in the table, and it is not stated on how many heads it was measured. These are orientation values — useful because in the species there were no others, and because the authors observe that they are similar to those described in horses, which makes them plausible — not diagnostic thresholds. The gradient visible is very clear and sensible: pulp drops from 866 HU in the L07 to −26.7 HU in the U11, that is, toward negative values, characteristic of fat or air. This is exactly the phenomenon the authors describe in words: with age secondary dentin reduces the pulp cavity, and measurement in an increasingly narrow canal is affected by partial volume of the surrounding hard tissue.

The normal findings that look like pathology — with frequencies

In 20 heads of healthy animals (17 llamas, 3 alpacas):

Findingn% of 20
Caudal deviation of the distal root of the mandibular 091365%
Caudal deviation of the distal root of the mandibular 08840%
Caudal deviation of the distal root of the mandibular 10315%

Cementum of the distal root of the 09 missing, with exposed root canal toward the surface of the adjacent root210%
Presence of the mandibular 07 (the third lower premolar may be absent)1365%
Presence of the maxillary 071785%

The chronology of the deviation, as described by the authors — and it is the piece that explains why it is seen in a healthy animal:

  • in a 9-month-old alpaca, deviation of only the distal root of the deciduous 08;
  • with age moderate sclerosis of the alveolar bone appears around the root, which obscures the lamina dura and the periodontal membrane;
  • around 30 months, the apex of the distal root of the 09 touches the mesial root of the 10, with a thin layer of bone still interposed;
  • at 60 months, when all teeth are present, the distal root of the 09 appears to push the mesial root of the 10 caudally, and no alveolar bone is detectable any longer between the two roots; at this age there is no longer any sclerosis around the roots, and the periodontal ligament appears smooth and thin, with soft-tissue opacity.
  • The deformity becomes more marked with age. In the maxillary arcade the phenomenon exists but is less frequent and less marked, and sclerosis and root deformities are not as clearly visualizable.

Normal dental anatomy — what is needed to read an image

  • The cheek teeth of the South American camelid are hypsodont, with enamel folds forming palisade-like segments («columns») in both the crown and the reserve crown; each column has one rostrocaudal pulp horn segment that, together with infundibular invagination, creates the selenodont occlusal surface.
  • Each molar column contains two pulp horns, except for the distal column of the mandibular 11, which has only one. Thus: all molars have 4 pulp horns; the mandibular 11 has 5.
  • A common pulp chamber is clearly visible in young teeth, which closes gradually through the deposition of secondary dentin. The authors are explicit about the value of this finding and its limitation: the current claim that the pulp horns of the mesial and distal roots do not communicate «requires a detailed explanation», because in young teeth a common pulp chamber exists; but the precise moment at which it closes, both in healthy and in diseased teeth, cannot be determined from their results. They add that this «may be an indicator for surgical intervention» — and this is a hypothesis, not a result.
  • Interpulpar communications between the horns of the same column: not detected with their CT technique. It should be read as written — not detected with this technique — not as demonstrated absence.
  • Two infundibula in each molar, except for the distal column of the mandibular 11, which has none. The infundibula are filled with cementum; in cross-section they have a crescent shape, with the convexity facing buccally in mandibular teeth and palatally in maxillary ones.
  • No sign of infundibular lesion, but with age occlusal exposure of the infundibula and their filling with feed debris.
  • Mandibular premolars: no infundibulum.
  • Maxillary 07: the smallest in the arcade, conical, one pulp horn, no infundibulum, root that finally divides into three short horns — best visualized on MPR reconstructions in the dorsal plane.
  • Maxillary 08: its roots lie ventral to the infraorbital foramen.
  • Maxillary 10: the largest tooth in the arcade.
  • In the maxillary arcades the buccal and palatal roots of each column overlap on radiography: this is the anatomical reason why, in that site, radiography alone is not sufficient.

A difference between the two species, concerning the incisors

  • Deciduous incisors: spatulate in both species.
  • Llama permanent incisors: still spatulate, with tapering roots.
  • Alpaca permanent incisors: rectangular in cross-section and chisel-shaped.

This is the only morphological difference between llama and alpaca that the article reports explicitly, and the authors record it as confirming what has already been published by others.

Authors' interpretation (attributed, not merged with the data)

The authors conclude that the study provides detailed information on dental imaging morphology in the clinically healthy South American camelid, and that this is basic knowledge for evaluating images and planning treatments in animals with dental disease.

The authors recommend, on the basis of the CT images, the 30° oblique angles indicated above, and explicitly disagree with the earlier recommendation to use 45°.

The authors hypothesize — and it is their most interesting thesis, formulated as a hypothesis to be tested and not as a result — that an alternative explanation to the commonly proposed cause of camelid dental disease (early removal of the deciduous caps) may be that the roots of the mandibular cheek teeth are deviated, deformed or shortened and partially destroyed with various degrees of pulp exposure already under normal conditions, and that this may explain why the disease arises across such a wide age range. Their final conclusion is formulated thus: «Future investigations are warranted to test the hypothesis that tooth root deformities, concurrent alveolar sclerosis and interdental tooth root contact might represent predisposing factors for dental disease in SAC.» It is a hypothesis stated as such by the authors.

The authors observe that the HU densities of camelid dental tissues are similar to those described in horses, and that — unlike radiographic radiopacity — the Hounsfield unit allows an objective comparison between healthy and altered tissue.

The authors state that CT gives greater detail than radiography in evaluating dental structures, because superimposition of bony and dental structures — especially the roots of the maxillary cheek teeth — does not allow accurate examination on radiography. In their study this statement is not quantified: there is no measure of how many additional findings CT identified. That comparison, with numbers, comes eleven years later and is card CRL-F-04.

The authors state their own caution about the sample: «considering the rather small sample size and the lack of animals at specific age categories …, caution must be applied, as the findings might not be fully representative for the entire population of SAC.»

Implicazioni cliniche

  • la radice distale del 09 mandibolare deviata caudalmente — 65 % delle teste sane;
  • lo stesso per l'08 (40 %) e il 10 (15 %);
  • sclerosi dell'osso alveolare attorno alla radice, che oscura lamina dura e membrana parodontale — reperto di età, transitorio: presente nell'animale giovane-adulto e assente a 60 mesi;
  • radici adiacenti a contatto, senza osso interposto, a partire dai 60 mesi;
  • cemento assente sulla radice distale del 09 con canale radicolare esposto — 10 % delle teste sane;
  • una camera pulpare comune nei denti giovani;

Clinical implications

1. The angles, and the fact that the field has not yet decided. For the camelid, this work recommends 30° in both obliques. It is the most directly usable piece of information in the article — and it must be used knowing it is not a consensus: the study in card CRL-F-04, from 2025, performs the maxillary obliques at 30° (consistent with this one) but the mandibular ones at 45° (consistent with the earlier work these authors disagreed with). Eleven years later, on the mandibular obliques the two schools are still divided. Anyone setting up a clinical protocol should know this and choose, rather than believe the matter is settled.

2. The mouth gag. A 20 mL syringe, 21 mm outer diameter, radiolucent, between the arcades caudal to the canines. It costs nothing, it is in the cabinet, and the same solution appears identically in the 2025 study.

3. What does NOT transfer: 60 kV and 20 mAs. These are the values used on an isolated head resting on the cassette. On the live animal there is the neck in the field, there are soft tissues and there is respiratory movement. Anyone copying those numbers onto a patient would get an underexposed radiograph. The article does not give the values for the live animal. (For comparison only, and without any recommendation: the 2025 study in card CRL-F-04, on live sedated alpacas, used 55 kV and 6.3 mAs with a different unit and a different geometry. Two studies, two units, two contexts: they are not transferable to one another, and this parenthesis serves only to show how much exposures vary.)

4. The catalogue of false positives, which is the real gift of this article. In a healthy camelid, on radiography or CT, all of the following can be found without it being disease:

  • the distal root of the mandibular 09 deviated caudally — 65% of healthy heads;
  • the same for the 08 (40%) and the 10 (15%);
  • sclerosis of the alveolar bone around the root, which obscures the lamina dura and periodontal membrane — an age-related finding, transient: present in the young-adult animal and absent at 60 months;
  • adjacent roots in contact, with no bone interposed, starting from 60 months;
  • cementum absent on the distal root of the 09 with exposed root canal — 10% of healthy heads;
  • a common pulp chamber in young teeth;
  • the mandibular 07 absent — 7 of 20 heads: a missing tooth is not a lost tooth.

5. The operational consequence of that catalogue. In this species, a radiographic finding alone does not make the diagnosis of a root abscess: it must be made together with oral examination, clinical signs and — where needed — CT. The chapter to which this work belongs says so implicitly; the reference surgical case series (Niehaus & Anderson 2007, 123 camelids) shows how costly it is to get this wrong: postoperative complications in 42 of 84 animals with follow-up, and extraction of two teeth significantly more complicated than that of a single one. (This last figure is cited secondhand, not read here directly: it is in chapter 06 of the KT and has been verified against the abstract of that work, not against this article.)

6. Reading age as diagnostic information. Peri-radicular sclerosis appears and then disappears. Knowing that at three years sclerosis is normal and at five it is no longer present means that the same finding has a different meaning at different ages, and that the animal's age should be written on the examination request.

7. Radiodensities serve for comparison, not for a threshold. The practical value of the Hounsfield unit here is being able to objectively compare healthy and altered dental tissue in the same animal, as the authors write, and to recognize what is dental tissue in an aggressive bony lesion of the mandible or maxilla. There is no numerical disease threshold in this article.

Limiti

Dichiarati dagli autori (citati, non riassunti)

  • Campione piccolo e mal distribuito per età. «Considering the rather small sample size and the lack of animals at specific age categories (median age of the animals was 3 years and 3 months ranging from 9 months to 9 years), caution must be applied, as the findings might not be fully representative for the entire population of SAC.»

Limitations

Stated by the authors (quoted, not summarized)

  • Small and poorly age-distributed sample. «Considering the rather small sample size and the lack of animals at specific age categories (median age of the animals was 3 years and 3 months ranging from 9 months to 9 years), caution must be applied, as the findings might not be fully representative for the entire population of SAC.»
  • The moment of closure of the common pulp chamber cannot be determined from their results, neither in healthy nor in diseased teeth, and requires further investigation.
  • The images come from specimens: the authors state this, and this is why Figure 1 of the article shows the technique as it should be performed on the live animal, derived from the results and not photographed on a patient.
  • The eruption process during maturity requires further investigation to be understood.

Added by the editors

  • Seventeen llamas versus three alpacas, and all percentages have denominator 20. This is the limitation that most affects the Italian use of this article, and the authors do not raise it. The 65% deviation rate of the 09 root is, in substance, a llama figure. In the alpaca — the species most commonly seen in Italy — that frequency is not known, and it is precisely the species in which periapical pathology is most studied (card CRL-F-04). Three subjects, one of which is nine months old, do not allow any estimate.
  • The stated median age (3 years and 3 months) does not match Table 1, from which 2 years and 10 months is derived. See the Species and subjects section.
  • Radiodensities are published without dispersion and without a case count. Table 4 gives one number per cell; the text states that number is the mean of two or three measurement points, but does not state on how many heads. The methods state that «mean and standard deviations were calculated», but the standard deviations do not appear in the tables. Without dispersion, a Hounsfield value does not allow one to establish whether an observed tissue is within or outside the normal range — that is, it does not allow the very use for which the authors propose it.
  • No measure of repeatability or of inter-observer agreement. The images were «evaluated»; it is not stated by how many readers, nor whether independently, nor with what consensus procedure in case of disagreement. For a study that publishes frequencies of subjective findings (a «deviated» root is a judgment), this is the heaviest methodological gap.
  • CT superiority is asserted, not measured. No count, no agreement statistic, no systematic comparison of findings between the two modalities. It is a qualitative conclusion, and should be cited as such. The numerical comparison exists, but it is eleven years later (card CRL-F-04), it is on another species and gives a much more nuanced picture.
  • Freezing at −20 °C and thawing may alter soft tissues and, in principle, attenuation values. The authors do not discuss whether this affects the Hounsfield units, particularly those of the pulp, which is the soft tissue in the series and the one with the lowest and most variable values. This cannot be verified from here → [TO BE CHECKED AGAINST THE FULL TEXT] — in the precise sense that the information is not in the article and would need to be sought in the literature on the effect of freezing on the CT attenuation of dental tissues, literature that this card has not queried.
  • No statement of ethical approval, which in 2014 on slaughterhouse specimens was accepted practice and would not be today.
  • No formal funding statement: the only financial information is in the acknowledgments (see below).

Usando STROBE come lente adattata, e dichiarandolo:

  • Voce 10 (dimensione dello studio): nessun calcolo, nessuna giustificazione.
  • Voce 22 (finanziamento): nessuna sezione dedicata.

What is missing relative to the reporting standard

No grid in chapter 14 §3 is designed for an ex vivo anatomical study. Using STROBE as an adapted lens, and stating this:

  • Item 1: the design is not named in the abstract. The word «specimen» in the title is the only signal that these are not patients — and it is a good signal, but it is in the title, not in the design.
  • Item 5 (setting): the period during which the heads were collected is not stated, nor from how many slaughterhouses or clinics they came, nor the geographic origin of the animals. For a study describing an anatomy with frequencies, herd structure and gene pool matter.
  • Item 6 (participants): inclusion criteria are stated («healthy», «no history of head pathology»), but it is not stated how the absence of dental disease was ascertained in animals for which only the history was available. In a work whose point is to distinguish normal from pathological, the operational definition of «normal» is the premise of everything, and here it is a history.
  • Item 10 (study size): no calculation, no justification. Twenty heads are what could be collected.
  • Item 12 (statistical methods): states mean and SD, and the SDs do not appear. No image-reading procedure or disagreement-resolution procedure is stated.
  • Item 16 (main results): frequencies are given with numerator and denominator — good, and it follows the rule of chapter 14 §5.2 — but without confidence intervals. At n = 20, the 95% confidence interval of a 65% proportion is wide, roughly from 41% to 85%: 65% is a very imprecise estimate, and the article does not flag it.
  • Item 17 (subgroup analyses): no separate analysis by species, which here would have been the one thing to do, and no analysis by age class even though age is the factor the authors describe as determinant.
  • Item 22 (funding): no dedicated section.

Not missing, and this should be stated clearly: Table 1 publishes the age, sex and species of every single subject. This is what made it possible, in this card, to recalculate the median and to measure the impact of the sample composition. An article that publishes its raw data leaves itself open to being checked, and that is the best compliment its reporting can be paid.

Finanziamento e conflitti di interesse

  • Finanziamento: nessuna dichiarazione formale di finanziamento compare nell'articolo. Nei ringraziamenti si legge: «The Iranian Ministry of Science, Research and Technology generously provided a fellowship for the first author.»
  • Conflitti di interesse: «The authors declare that they have no competing interests.»

Commento dell'associazione — Camelid Research Studies APS · redazione CRL · 31/08/2026

Valutazione a stelle

AsseVotoMotivazione

Funding and conflicts of interest

  • Funding: no formal funding statement appears in the article. The acknowledgments state: «The Iranian Ministry of Science, Research and Technology generously provided a fellowship for the first author.» The funder's role in the design, collection and analysis of data, in writing, or in any restrictions on publication is not stated — this is precisely the information ICMJE requires (chapter 14 §5.4). A ministerial fellowship to a doctoral student raises no suspicion of result orientation; its placement in the acknowledgments rather than in a dedicated section is, however, a reporting flaw, because a quick reader concludes there was no funding.
  • Conflicts of interest: «The authors declare that they have no competing interests.»
  • Authors' contributions: stated in full — PZ conceived, planned and supervised the study; AR performed the radiographic and CT examinations under UG's supervision; FS advised AR on dental nomenclature.
  • No equipment manufacturer appears either among the authors or in the acknowledgments. The equipment is named in the closing notes (Philips Brilliance, Siemens Optitop, Fuji FCR 5000 and Fuji cassettes, efilm software, Excel).

Association's comment — Camelid Research Studies APS · CRL editorial staff · 31/08/2026

This is the article in this fascicle that most changes how one looks at an image, and it does so in a counterintuitive direction: it teaches one to see less disease, not more. Its most valuable content is neither the angles nor the Hounsfield units: it is the list of normal findings that resemble lesions. That in 13 of 20 healthy heads the distal root of the first mandibular molar is bent backward, that in 2 of 20 the cementum is missing with the root canal exposed, that at three years there is peri-radicular sclerosis and at five there no longer is: these are the things a good radiologist, but one accustomed to the horse, would report as pathology. In a species whose normal is not known, expertise in another species is a risk factor, not a guarantee — and it is the same observation that, eleven years later, the authors of card CRL-F-04 identify as one of the explanations for their poor agreement between modalities.

Where we credit the authors with a merit that is not obvious. They published Table 1, that is, the age, sex and species of every single head. Thanks to that table we were able to recalculate the median — and find it different from the one stated — and to measure how imbalanced the sample is toward the llama. An article that publishes raw data exposes itself to being corrected: that is exactly what it should do. The error on the median does not make us trust this work less; it makes us trust more in the way it is written.

Where we disagree, or rather where we flag a gap. The article is titled «South American camelids» and presents percentages on twenty heads of which seventeen are llamas. In Italy the typical dental patient is an alpaca. The frequencies published here have not been verified in the alpaca, and this card cautions against treating them as such: they are the best available starting point, not a species-specific figure. Three alpacas, one of them nine months old, are a sample that supports none of the article's percentages.

Something the article says that no one cites. The authors propose that physiological root deformities — not the early removal of deciduous caps — may be among the predisposing factors of camelid dental disease. If they were right, it would change what is monitored and in which animals. They have no data to support it and they say so, closing with a call to test the hypothesis. Eleven years later that test does not appear to have been published, and card CRL-F-04, which itself studies alpaca periapical infections, does not address it either. This is one of the cards we would like to write and cannot: it would need a case series of alpacas with and without abscess, with CT, counting how often the root of the 09 is deviated in the two groups. It is a case-control study on images already on file.

Our operational position, for members. Three lines. First: on a request for camelid dental radiography one should write the age, because peri-radicular sclerosis is an age-related finding and not a disease finding. Second: the 30° obliques of this work and the 45° mandibular angle of the other school are both in use; whoever sets up a protocol should choose and be consistent, and write on the report which angle was used, because an oblique without its angle is not comparable to anything. Third: this article's exposure parameters are not to be copied onto a live animal.

Star rating

AxisScoreRationale
A · Scientific quality★★★☆☆Descriptive study with stated n, individual raw data published (Table 1), explicit selection criteria, standardized and complete acquisition protocol in both modalities, stated basic analysis: this is level three. It does not rise to four because there is no ethical approval, there is no measure of repeatability or inter-reader agreement for findings that are subjective judgments, the standard deviations announced in the methods do not appear in the tables, funding is in the acknowledgments, and the stated median age does not match the published table.
B · Clinical utility★★★☆☆Offers a replicable procedure described in sufficient detail — five projections with beam centering, angles measured with a goniometer, mouth gag, focus-film distance, complete CT protocol — to be used with the caution imposed by the level of evidence: full level three. It does not rise to four because the conditions of use are not explicit where they are most needed: the exposure parameters are cadaveric and not transferable, and the recommendation on the mandibular oblique angles is not shared by the later literature, which continues to use 45°. A procedure the field has not converged on should be applied with caution, not with confidence.
C · Educational value★★★★★It is the first-choice educational reference on the dental imaging anatomy of the South American camelid: anyone who needs to learn to read a dental radiograph or CT of this species starts here, and the later literature treats it as such — the study in card CRL-F-04, from 2025, cites it explicitly in that role. It teaches something that extends well beyond the teeth: the catalogue of normal findings that look like lesions, with their frequencies and their chronology of appearance and disappearance.
D · Species relevance★★★☆☆Study on camelids, of European origin, on the two species kept here, and it is the anatomical basis of everything done in camelid dental radiology. It does not rise to four for a reason the grid itself states: the sample is 17 llamas versus 3 alpacas, all frequencies have denominator 20, and the population is not described enough to assess transferability to the alpaca, which is the species of greatest clinical interest. It should be added that the subjects are anatomical specimens, not patients: the geometry and densities transfer, the exposure and the clinical context do not.

No overall score, by design. A = 3 and C = 5 are not a contradiction: a descriptive work with reporting gaps can remain, for years, the document from which one learns something no other document teaches.

Bottom line

It can be concluded that, in 20 South American camelid heads, healthy (17 llamas, 3 alpacas), the most useful radiographic projections for the cheek teeth are the 30° lateral ventral-laterodorsal for the mandibular arcade and the 30° lateral dorsal-lateroventral for the maxillary arcade, with slight separation of the arcades, because they give the least distortion of the reserve crown and roots and the least overlap; that the mandibular incisors are best seen on the intraoral VD; that the radiodensities in Hounsfield units of the dental tissues are those in Tables 4 and 5 and are similar to those of equids; and — this is the point that changes practice — that in healthy animal heads one regularly finds caudal deviation of the distal root of the mandibular 09 (13 of 20), of the 08 (8 of 20) and of the 10 (3 of 20), peri-radicular alveolar sclerosis that appears and then disappears with age, contact between adjacent roots with no bone interposed from 60 months, absent cementum with exposed root canal (2 of 20), a common pulp chamber in young teeth and absence of the mandibular 07 (7 of 20): none of these findings, alone, is pathology.

It cannot be concluded that the reported frequencies hold for the alpaca: the sample is 17 llamas and 3 alpacas, all percentages have denominator 20, and at n = 20 even the best estimate has a very wide confidence interval. No value in this article can be used as a diagnostic threshold: the radiodensities are means of two or three measurement points, without standard deviation and without the number of heads on which they were taken. The exposure parameters cannot be transferred — 60 kV, 20 mAs, 60 cm focus-film distance — to a live animal: these are values obtained on an isolated head resting on the cassette. It cannot be concluded that CT is superior to radiography to a quantifiable degree: here the statement is qualitative and not measured, and the numerical comparison exists only eleven years later, on another species and with a far more nuanced outcome (card CRL-F-04). It cannot be concluded that the question of the oblique angles is settled: the later literature still uses 45° for the mandibular ones. It cannot be concluded that physiological root deformities predispose to dental disease: this is the authors' final hypothesis, stated by them as needing verification, and to date not verified.

Messaggi da portare a casa

  3. La radice distale del 09 mandibolare piegata all'indietro è normale in 13 teste sane su 20.

Take-home messages

  1. 30° obliques: ventral-laterodorsal for the mandible, dorsal-lateroventral for the maxilla. And the angle used should be written on the report.
  2. The mouth gag is a 20 mL syringe (21 mm outer diameter) between the arcades, caudal to the canines.
  3. The distal root of the mandibular 09 bent backward is normal in 13 of 20 healthy heads.
  4. Peri-radicular sclerosis is an age-related finding: it is there at three years, no longer there at five. The age should be written on the examination request.
  5. The mandibular 07 is absent in 7 of 20 healthy animal heads: absent does not mean lost.
  6. In young teeth the pulp chamber is common and closes with secondary dentin: a finding that would be abnormal in the mature tooth.
  7. The exposures in this article are cadaveric. The angles can be copied, the kV cannot.
  8. 17 llamas and 3 alpacas: the percentages are, in substance, llama data.

Card CRL-F-03 · version 1.0 · 31/08/2026 · synthesizes a single source, not a body of literature. The full text was read in its entirety from the Europe PMC deposit (PMC3892000/fullTextXML), including Tables 1-5 and the closing notes on equipment; all bibliographic details come from Crossref (works/10.1186/1746-6148-10-4), the Europe PMC core record, and PubMed's esummary, queried on 31/08/2026. Journal status was verified on DOAJ and OpenAlex the same day. The recalculation of the age median was performed on the twenty values published in Table 1 of the article. The figure on the surgical case series by Niehaus & Anderson 2007 is cited secondhand from chapter 06 of the KT and was not read in the original in this session. No systematic search of forward citations was conducted: it appears, from direct verification against the bibliography of the work in card CRL-F-04, that that work cites this one as reference [15].
