Titolo originale

«A Comparative Study of Radiography and Computed Tomography in Detecting Periapical Infections in Alpacas (Vicugna pacos)»

Titolo originale

«A Comparative Study of Radiography and Computed Tomography in Detecting Periapical Infections in Alpacas (Vicugna pacos)»

The title says «in Detecting», that is «in identifying». It must be read carefully, because identifying is not the same thing as correctly diagnosing: without a reference standard, knowing that a technique detects more does not tell you whether it detects better. This is the distinction that underlies this whole card, and the authors expressly acknowledge it among their own limitations.

Corresponding author: Katrien Vanderperren ([email protected], as stated in the article) — Department of Morphology, Imaging, Orthopedics, Rehabilitation and Nutrition, Faculty of Veterinary Medicine, Ghent University, 9820 Merelbeke, Belgium. Affiliation as stated in the article. Corresponding author's ORCID: not stated.

Affiliations of the other authors, as stated in the article: same department at Ghent for Rutigliano and Raes; Department of Surgery and Anesthesiology of Domestic Animals, same faculty, for Vlaminck; Proost is affiliated with «Wooly Veterinary Dentist», 9810 Nazareth-De Pinte, Belgium — that is, a private veterinary practice specialized in dentistry, not a university.

This latter fact is not a conflict and is not declared as one, but it is context that chapter 14 §5.4 requires to be given together with the result: one of the authors practices veterinary dentistry professionally, and the study concludes that CT — the more expensive examination — detects more abnormalities than radiography. Nothing in the data suggests bias; the combination must nonetheless be disclosed so the reader can weigh it independently.

Specie e soggetti

  • 14 alpaca riferite all'ospedale universitario di Gand con sospetta patologia dentale avanzata, arruolate retrospettivamente;

Sesso: 9 femmine, 2 maschi interi, 8 maschi castrati (= 19).

«In total, 158 teeth were selected as potentially diseased either upon oral examination or upon radiographic screening and were subsequently assessed on CT.» Non sono 158 denti consecutivi. Sono 158 denti già sospetti, selezionati all'esame orale oppure allo screening radiografico.

Species and subjects

19 Huacaya alpacas (Vicugna pacos) — a single species, and stated as such: a rare strength in this field. From them: 76 dental arcades (19 × 4) and 158 cheek teeth evaluated with both modalities.

Source, in two groups:

  • 14 alpacas referred to the Ghent university hospital with suspected advanced dental pathology, enrolled retrospectively;
  • 5 alpacas enrolled prospectively as cadaver heads via the pathology department: 2 euthanized following a diagnosis of advanced dental disease, 3 euthanized for reasons unrelated to the teeth.
  • No animal was euthanized for the study.

Sex: 9 females, 2 intact males, 8 castrated males (= 19). Age: from 1 year to 11 years and 5 months; mean 5 years and 8 months ± 3 years and 2 months. Table 1 of the article reports age, sex and number of included teeth for each of the 19 subjects, from 2 to 12 teeth per animal — and the sum of the nineteen rows comes to exactly 158.

How the 158 teeth were selected, and it is the premise that explains everything that follows. «In total, 158 teeth were selected as potentially diseased either upon oral examination or upon radiographic screening and were subsequently assessed on CT.» These are not 158 consecutive teeth. They are 158 already suspect teeth, selected on oral examination or on radiographic screening. Two consequences follow, which the article acknowledges and which this card does not leave implicit: there is no healthy control, and some of the teeth entered the study precisely because radiography had flagged them. The comparison between the two modalities therefore starts from a set that one of the two contributed to defining.

Non c'è verifica istologica, e gli autori lo scrivono come limite principale: «The authors acknowledge that the absence of histological confirmation represents a significant limitation of this study.»

Study type and level of evidence

Design: an inter-modality agreement study between two imaging modalities, partly retrospective and partly prospective on cadavers, single-centre, without a reference standard. Unit of analysis: the tooth (n = 158), not the animal (n = 19).

What this study is not. It is not a diagnostic accuracy study. There is no histological verification, and the authors state this as the main limitation: «The authors acknowledge that the absence of histological confirmation represents a significant limitation of this study.» There is therefore no sensitivity, specificity, or predictive values here. There is thus no "right" technique and a "wrong" one: there is a measure of how much the two diverge.

Level of evidence: observational study on a selected clinical series. Consequence for the reader: one can conclude where the two techniques disagree and in which direction; one cannot conclude which of the two is correct at the points of disagreement — although it is biologically very plausible that it is CT, and the authors argue in that direction. Plausible and demonstrated are two different things, and this card keeps them separate.

Relevant reporting standard: STROBE for the observational structure. For the agreement component, none of the checklists listed in chapter 14 §3 is the right one: STARD is for diagnostic accuracy and presupposes a reference standard that is absent here. The authors declare no checklist.

In breve

In brief

Nineteen alpacas, one hundred fifty-eight already-suspect cheek teeth, sixteen parameters evaluated twice — once on radiography and once on CT — by two ECVDI-certified radiologists, first independently and then in consensus. For each parameter, the Cohen's kappa between the two modalities, with 95% confidence interval.

Result: agreement ranges from none to moderate, never strong. None of the sixteen parameters reaches the threshold for strong agreement. CT identified more diseased teeth than radiography in twelve of sixteen parameters, and 16 more teeth were judged potentially affected by periapical infection on CT compared with radiography. For four parameters, however, it is radiography that identifies more.

And then the methodological detail that this card considers the most important to flag for the reader: in Table 2 of the article, kappa is multiplied by one hundred and presented in a column titled «Percentage of Agreement Across Modalities». A kappa of 0.57 is not 57% agreement.

Why this study matters

Because it does with numbers what the species literature had so far only asserted. The study in card CRL-F-03, from 2014, concluded that CT gives more detail than radiography: that was a qualitative observation. Here there is a parameter-by-parameter comparison, with kappa and confidence intervals, on a single stated species, in a European university hospital.

And because the result is more uncomfortable than expected. The authors compare their own agreement on suspicion of apical infection with that reported in the horse — «as high as 97%» — and find in the alpaca a fraction of that value. They offer two explanations, both unpleasant and both plausible: readers are unfamiliar with the radiographic anatomy of the South American camelid, and the alpaca skull is compact and structures overlap. The first is the same observation that card CRL-F-03 draws from the opposite side: in this species, experience gained on another species is a risk factor.

Obiettivo

Objective

To compare the detection of dental abnormalities by radiography and CT in the alpaca, highlighting the strengths and limitations of each in the diagnosis of periapical infections and sinusitis.

Working hypothesis as stated by the authors: that radiography would reveal moderate to severe dental pathology, while CT would offer a more detailed assessment and reveal early or subtle changes not visible on radiography.

In PICO form, insofar as an agreement design allows it: P — clinically suspect alpaca cheek teeth, in a European university hospital; I — high-resolution CT (0.5 mm); C — six-view radiography; O — agreement between the two modalities on sixteen parameters (not accuracy: the reference is missing).

Metodi / Protocollo

Esame orale (nelle 14 pazienti vive). Nomenclatura: Triadan modificato.

Radiografia — sei proiezioni per animale, sotto la stessa sedazione (ketamina 5 mg/kg, medetomidina 30 µg/kg):

  • dorsoventrale, in decubito sternale;
  • una laterale;
  • due oblique latero-30°-dorsal-lateroventrali (R30°Do-LVeO e L30°Do-RVeO) per le arcate mascellari;

TC. Animale in decubito sternale. Scanner a 4 strati.

Statistica. R versione 4.3.2.

Etica.

Methods / Protocol

Oral examination (in the 14 live patients). Under deep sedation, with ketamine 5 mg/kg IM and medetomidine 30 µg/kg IM or dexmedetomidine 15 µg/kg IM — dose, route and species are stated together here by the article, and this is the only reason these numbers appear in this card; they are what was administered in this study, not a dosing recommendation, and this series does not treat them as such. Oral cavity thoroughly flushed to remove food debris before each assessment; small-pony mouth gag with dedicated light source; teeth and soft tissues assessed with an oral endoscope. In the cadavers, after image acquisition, the heads were disarticulated at the temporomandibular joint and photographed tooth by tooth, to simulate the oral examination. Nomenclature: modified Triadan.

Radiography — six views per animal, under the same sedation (ketamine 5 mg/kg, medetomidine 30 µg/kg):

  • dorsoventral, in sternal recumbency;
  • one lateral;
  • two latero-30°-dorsal-lateroventral obliques (R30°Do-LVeO and L30°Do-RVeO) for the maxillary arcades;
  • two latero-45°-ventral-laterodorsal obliques for the mandibular arcades. Animal in lateral recumbency, with the arcade of interest closer to the cassette. Built-in goniometer of the unit used to determine the angles. Gag: a 20 cc syringe, for all views except the dorsoventral. Parameters: 55 kV and 6.3 mAs.

⚠️ The two schools of thought on the oblique angles, set side by side.

Maxillary arcadeMandibular arcade
CRL-F-03 (Rostami 2014, recommendation)latero-30°-dorsal-lateroventrallatero-30°-ventral-laterodorsal
CRL-F-04 (this study, technique used)latero-30°-dorsal-lateroventrallatero-45°-ventral-laterodorsal

On the maxilla the two schools agree; on the mandible they do not. Rostami and colleagues had explicitly dissented from 45°; eleven years later, 45° is the technique in use at Ghent. This study does not discuss the choice, and there is no way to know from here whether, or how much, the angle contributed to the poor mandibular agreement → [TO BE CHECKED AGAINST THE FULL TEXT] in the sense that the information is not in the article: no one has compared the two angles in the same population. This is one of the cards this series cannot yet write.

CT. Animal in sternal recumbency. 4-slice scanner. 150 kVp · 250 mA · FOV 400 mm · pitch 0.6 · slice thickness 0.5 mm. An edge enhancement filter (window level 1000, width 3500, matrix 512 × 512) applied to the raw data for better assessment of bone detail.

Worth noting, because it matters to anyone with a scanner: four slices. It is not a modern unit, and it is slower than a 16- or 64-slice one. But the slice thickness is 0.5 mm, that is half that used in the study of card CRL-F-03 and that of card CRL-F-05. In dentistry, spatial resolution matters more than the number of slices, and the authors argue this by citing the literature on high-resolution CT (thickness ≤ 1 mm) to reduce partial volume artefacts.

Reading. Two ECVDI-certified veterinary radiologists (denoted by the initials ER and KVDP). First independent reading, then consensus on every parameter of every tooth. It is not stated whether the readers were blinded to the other modality or to the clinical data → [TO BE CHECKED AGAINST THE FULL TEXT]: the information does not appear in the article, and for an agreement study it is central.

The sixteen parameters, evaluated as present/absent on each tooth and for each modality: periodontal ligament space widening (> 1 mm) · presence of sequestra · pulp changes · periosteal reaction · periapical sclerosis · periapical halo · mandibular expansion · root lysis or fragmentation · detectable lamina dura · clinical crown fracture · cortical destruction · root changes (other than clubbing) · alveolar bone changes (increased thickness) · cementomas/hypercementosis · apical root clubbing · overall suspicion of periapical infection.

Statistics. R version 4.3.2. Unweighted Cohen's kappa for each parameter, with 95% confidence interval. Interpretation scale as stated by the authors: < −0.10 disagreement · −0.10-0.19 no agreement · 0.20-0.59 weak agreement · 0.60-0.79 moderate agreement · 0.80-1 strong agreement. And then the sentence from which the reading problem arises: «Statistics were converted into percentages for easier interpretation.»

Ethics. For the live animals, prior ethical approval was not legally required (EU directive 2010/63/EU), since the imaging data were obtained «below threshold», that is, within the patient's diagnostic pathway. For the cadavers, approval was waived by the ethics committee, on the same basis, as the images were obtained post mortem. No animal was euthanized for this study.

Nessun parametro raggiunge l'accordo forte (kappa ≥ 0,80).

  • la TC vede di più dove pesa la sovrapposizione (allargamento dello spazio parodontale, fratture della corona, alterazioni radicolari e pulpari, cementomi) e dove serve la ricostruzione multiplanare (sequestri, reazione periostale, espansione mandibolare, distruzione corticale);
  • le alterazioni pulpari individuate erano soprattutto calcoli pulpari, la cui dimensione li rende non rilevabili in radiografia — e osservano che la stessa marcata sovra-rappresentazione alla TC è stata descritta nel cavallo;

  1. la scarsa familiarità degli interpreti con l'anatomia radiografica dei camelidi sudamericani, che può aver portato a sotto-rilevare anomalie, perché alterazioni periapicali sottili sfuggono facilmente a chi è abituato alle strutture dentali equine;

No parameter reaches strong agreement (kappa ≥ 0.80).

⚠️ The «Percentage of Agreement Across Modalities» column must be read with a caveat, and it is the most useful flag in this card.

In the article that column contains kappa multiplied by one hundred — 0.026 becomes «3%», 0.57 becomes «57%», 0.71 becomes «71%» — and the methods sentence says so: «Statistics were converted into percentages for easier interpretation.»

A kappa and a percentage agreement are not the same quantity. Kappa corrects for the agreement expected by chance; raw percentage agreement does not, and it is always higher. The two diverge the more the finding is rare: for pulp changes, a kappa of 0.026 is compatible with an extremely high raw agreement percentage, because radiography and CT agree on very many teeth in saying «absent» and almost never agree in saying «present». A reader of «3% agreement on pulp changes» understands that the two techniques disagree almost always. That is not what the data say: it says that, once chance agreement is removed, almost nothing is left.

The conversion does not alter any result of the article — the conclusions are drawn correctly from the kappa scale — but it produces a table that lends itself to being cited wrongly, and it is exactly the kind of figure that then travels on its own. Here the kappa column is reproduced alongside to make correct reading possible. The raw percentage agreements are not published in the article → [TO BE CHECKED AGAINST THE FULL TEXT], in the sense that the figure does not exist in the text and would have to be requested from the authors.

In which direction the two techniques diverge

CT identified more diseased teeth than radiography for twelve parameters: periodontal space widening · sequestrum formation · pulp changes · periosteal reaction · periapical halo · mandibular expansion · root lysis or fragmentation · clinical crown fracture · cortical destruction · root changes · cementoma / hypercementosis · overall suspicion of apical infection.

Radiography identified more diseased teeth than CT for four parameters: periapical sclerosis · detectable lamina dura · alveolar bone changes · apical root clubbing.

The number that sums it all up: 16 more teeth were considered potentially affected by periapical infection on CT compared with radiography.

What this section lacks, and it must be said. The article states the direction of divergence for each parameter, but does not publish the counts: we do not know how many teeth were judged positive on radiography and how many on CT for each of the sixteen parameters. The 2 × 2 tables from which each kappa is calculated are not published. Without them it is impossible to reconstruct the raw agreement percentages, or to assess how much each kappa is affected by the rarity of the finding. The only published count is the 16 additional teeth.

Authors' interpretation (attributed, not merged with the data)

The authors conclude that agreement between CT and radiography in identifying the typical signs of periapical infection ranges from none to moderate depending on the parameter, likely due to the effect of interpreting unfamiliar images and the limitation imposed by overlap in this region; and that overall CT identified more dental abnormalities than radiography, which makes it a valuable ancillary diagnostic tool for periapical infections of the alpaca's cheek teeth.

The authors explain — with arguments, not with measurements — why each group of parameters behaves as it does:

  • CT sees more where overlap weighs heavily (periodontal space widening, crown fractures, root and pulp changes, cementomas) and where multiplanar reconstruction is needed (sequestra, periosteal reaction, mandibular expansion, cortical destruction);
  • the pulp changes identified were mostly pulp stones, whose size makes them undetectable on radiography — and they note that the same marked over-representation on CT has been described in the horse;
  • radiography sees more periapical sclerosis and more alveolar bone changes, and the authors propose that overlap itself may increase the apparent density of some structures and make a subtle sclerosis more evident on radiography than on CT.

The authors report from other literature — a figure not measured here — that «mineral content must decrease by 30 to 60% before it becomes visible on a radiograph», and that CT and CBCT detect bone lysis earlier.

The authors explain the discrepancy with the horse with two stated reasons:

  1. the readers' limited familiarity with the radiographic anatomy of South American camelids, which may have led to under-detection of abnormalities, because subtle periapical changes easily escape those accustomed to equine dental structures;
  2. the compact, densely packed cranial anatomy of the alpaca, which produces significant overlap of adjacent structures on radiography.

The authors recommend that, in cases where a detailed assessment of dental and periapical structures is critical — for example in animals with suspected complex or multifocal disease — CT be considered the modality of choice; and they note that CT can be performed with the same sedation protocol as radiography, in a single anaesthetic event and without additional preparation.

The authors also argue on cost, and it must be flagged as an argument, not a result: they contend that the higher initial cost of CT may translate into lower overall expenses for the owner, by avoiding misdiagnosis, repeated procedures and complications. This study did not measure costs, outcomes, or complications. It is the textbook example of what chapter 14 §5.1 requires to be kept out of the results.

Implicazioni cliniche

3. Il pezzo operativo che vale il prezzo dell'articolo: «the possibility of performing CT under the same sedation protocol used for radiographic examination allows both procedures to be conducted sequentially during a single anesthetic event, with only minor adjustments if required», e nessuna preparazione anestesiologica aggiuntiva è necessaria per la TC di questa regione.

Clinical implications

1. CT sees more — in twelve of sixteen parameters, and with sixteen more teeth. If the clinical question is «how many teeth are involved», radiography alone underestimates.

2. But not in everything: in four parameters radiography sees more. Periapical sclerosis, lamina dura, alveolar bone changes and apical clubbing were identified more often on radiography. These are not two exams on a scale, they are two different exams, and radiography is not the poor version of CT.

3. The operational point that is worth the price of the article: «the possibility of performing CT under the same sedation protocol used for radiographic examination allows both procedures to be conducted sequentially during a single anesthetic event, with only minor adjustments if required», and no additional anaesthetic preparation is needed for CT of this region. In a species where anaesthesia is the main risk, the additional cost of CT is not a second anaesthetic risk: it is scanner money and time. This is information that changes the conversation with the owner.

4. The lesson about the reader, and it is the most uncomfortable one. The authors attribute part of their poor agreement to limited familiarity with camelid radiographic anatomy — of two ECVDI-certified radiologists. If it happens to them, it happens to anyone. The countermeasure is not one more exam: it is keeping the work of card CRL-F-03 open next to the viewing box, because that is where the normally deviated roots, the age-related sclerosis and the normally missing teeth are found.

5. Apical clubbing is the sign on which the two techniques agree most (kappa 0.71). Pulp changes and cementomas are the ones on which they do not agree at all (kappa 0.026 and 0.14): if clinical reasoning rests on one of these two, radiography is not the right exam.

6. The radiographic threshold of bone lysis. The 30-60% loss of mineral content before lysis becomes visible on radiography is a figure reported by the authors from other literature, not measured here, and not specific to the alpaca. It should be used as an order of magnitude for a sentence to tell the owner: a normal radiograph does not rule out early lysis.

7. What this study does NOT authorize one to say. It does not authorize saying that CT is more accurate, because no one verified histologically who was right. It does authorize saying that CT detects more, and considering it plausible — for the anatomical reasons the authors argue — that some of those sixteen extra teeth are true. Plausible, not demonstrated.

Limiti

Dichiarati dagli autori (citati, non riassunti)

  • Assenza di conferma istologica, indicata dagli autori come limite significativo: «Histology remains the definitive standard for diagnosing dental pathology… Without histological validation, the ability to conclusively interpret imaging findings is inherently constrained, and subtle or early-stage pathological changes may be overlooked or mischaracterized.»
«Furthermore, the inclusion of control specimens could have given interesting insights into potential pathological findings in dental elements that were not clinically or radiographically targeted as pathological.»

Limitations

Stated by the authors (quoted, not summarized)

  • Absence of histological confirmation, indicated by the authors as a significant limitation: «Histology remains the definitive standard for diagnosing dental pathology… Without histological validation, the ability to conclusively interpret imaging findings is inherently constrained, and subtle or early-stage pathological changes may be overlooked or mischaracterized.» Reason for the lack: the partly retrospective nature of the study.
  • Absence of controls. «Furthermore, the inclusion of control specimens could have given interesting insights into potential pathological findings in dental elements that were not clinically or radiographically targeted as pathological.»
  • Readers' limited familiarity with the radiographic anatomy of South American camelids, with possible under-detection.
  • Anatomical overlap of the alpaca's compact skull as an intrinsic limitation of radiography in this region.
  • Operational limitations of CT: high initial and maintenance costs, need for more staff, expertise and time for interpretation.

Added by the editors

  • The «Percentage of Agreement Across Modalities» column contains kappa × 100, not a percentage agreement. It does not alter any conclusion of the article, but it is the table row most likely to be misquoted. See the box in the results.
  • The 2 × 2 tables are not published. For each of the sixteen parameters, the number of positive teeth in each modality is missing: the reader cannot reconstruct anything, neither the raw agreement percentage, nor the prevalence of the finding, nor whether a low kappa depends on the rarity of the finding (which is almost certainly the case for pulp changes and cementomas).
  • It is not stated whether the readers were blinded to the other modality, to the oral examination, and to the clinical data. In a study where the same two radiologists read both modalities, the risk that reading the first orients reading of the second is structural, and the only possible effect is to increase the observed agreement. If this occurred, the kappa values reported here are optimistic → [TO BE CHECKED AGAINST THE FULL TEXT].
  • The unit of analysis is the tooth, but the teeth are clustered by animal (from 2 to 12 teeth per subject, according to Table 1). Unweighted Cohen's kappa treats the 158 observations as independent, and they are not: two teeth from the same alpaca share anatomy, image quality, positioning and reader. No correction for clustering was used, and the consequence is that the published confidence intervals are narrower than the true ones.
  • The sample is selected for suspicion, and partly selected by radiography itself: 158 teeth chosen because suspect on oral examination or on radiographic screening. A tooth positive on CT alone and never suspected on radiography or in the mouth could not enter the study. This design structurally underestimates the advantage of CT — and should be recognized as a caveat that favours the conclusion, not one against it.
  • No separate analysis between the 14 live patients and the 5 cadaver heads. Acquisition conditions differ (respiratory motion, positioning, soft tissues): they are two populations, treated as one.
  • No separate analysis between maxillary and mandibular arcade, although they were radiographed with different oblique angles (30° versus 45°) and although the authors themselves cite maxillary root overlap as a specific problem. This would have been the most informative subgroup analysis in the article, and it is absent.
  • The economic defence of CT is not supported by any data in this study: no costs, no outcomes, no complications were measured. It is an argument, and should be cited as such.
  • The bibliographic reference supporting the 97% agreement in equids points, in the article's bibliography, to a work whose title concerns the anatomical relationships of the maxillary cheek teeth and the volumes of the paranasal sinuses, not a comparison between modalities. The 97% figure cannot be verified from here → [TO BE CHECKED AGAINST THE FULL TEXT] of the cited equine article, which this card did not query. It is the comparison on which the authors base their main discussion, and it deserves to be checked before being repeated.

  • Voce 9 (bias): nessuna sezione dedicata.
Bene.

What is missing relative to the reporting standard (STROBE, as a retroactive lens)

  • Item 1: the design is not named in the title or abstract. «Comparative study» does not identify a design.
  • Item 5 (setting): the period over which cases were collected is not stated. For a partly retrospective series this is the item most needed, and it is entirely missing: we do not know whether the 14 patients span two years or ten.
  • Item 6 (participants): the inclusion criteria for the animals are clear; the inclusion criteria for the teeth are stated but not operationalized: «potentially diseased upon oral examination or upon radiographic screening» does not say who decided, with what threshold, nor how many teeth were examined and excluded. The true denominator — how many cheek teeth those 19 alpacas had in total — is not published, and without it one cannot know what fraction of the mouth entered the analysis.
  • Item 9 (bias): no dedicated section. Blinding is not mentioned, in any direction.
  • Item 10 (study size): no calculation, no justification either for the 19 animals or for the 158 teeth.
  • Item 12 (statistical methods): unweighted kappa is stated and the interpretation scale is made explicit with its source — a rare merit, worth noting. What is missing, however, is handling of animal-level clustering, and justification of the conversion to percentages.
  • Item 13 (participants): no flow diagram; no count of candidate and excluded teeth.
  • Item 16 (main results): kappa and 95% CI are published for all sixteen parameters — the best-done thing in the article. What is missing, however, is the raw counts (2 × 2 tables), which are what makes an agreement result reusable.
  • Item 17: no subgroup analysis (live/cadaver, maxilla/mandible, age class).
  • Items 19-22: limitations discussed with notable honesty; generalizability addressed; funding and conflicts declared. Good.

Commento dell'associazione — Camelid Research Studies APS · redazione CRL · 31/08/2026

Dove dissentiamo dagli autori. Sulla chiusura economica: «the upfront cost of CT may ultimately result in lower overall expenses and improved outcomes for both the animal and the owner.»

Valutazione a stelle

AsseVotoMotivazione

Association's comment — Camelid Research Studies APS · CRL editorial team · 31/08/2026

This is the methodologically most mature article in issue F, and at the same time the one whose result must be handled with the most caution. Mature because it does three things that the other four lack: it uses two certified readers with independent reading followed by consensus, publishes kappa and confidence intervals for all sixteen parameters, and states the interpretation scale with its source instead of applying it silently. Cautious because it has no reference standard, and without a reference standard a comparison between modalities tells you how much two techniques diverge, never which of the two is correct.

The flag we consider this card's main contribution. In Table 2 of the article, the column titled «Percentage of Agreement Across Modalities» contains kappa multiplied by one hundred. The article's conclusions are drawn correctly from the kappa scale and are not undermined by it. But that column, detached from its table, produces false statements: «on pulp changes radiography and CT agree only 3% of the time» is a sentence no data in this study supports, and it can be drawn from that column's title in a second. On rare findings — and pulp stones are rare — the raw agreement percentage is extremely high exactly where kappa is near zero, because the two techniques agree on a mountain of «absent» cases. We reproduce the kappa column here alongside the article's so that readers cannot go wrong.

Where the result surprises us, and must be looked at twice. In four of sixteen parameters radiography detects more: periapical sclerosis, lamina dura, alveolar bone changes, apical clubbing. The authors' explanation — overlap can increase apparent density and make a subtle sclerosis more evident — is biologically sensible and has an alternative reading the article does not consider: that some of those sclerosis findings «seen only on radiography» are false positives generated precisely by that overlap. Without histology, the two explanations are indistinguishable. And that is exactly the point: the two techniques are not on a scale, and no one knows, in this species, how often each one is wrong.

One thing that makes us trust the authors. They wrote, of two ECVDI-certified radiologists, that their limited familiarity with camelid radiographic anatomy may have contributed to under-detecting abnormalities. It is an admission few would put in writing, and it is the same observation that card CRL-F-03 reaches from the anatomical side: in this species, experience gained on another species is a risk factor. The two cards, eleven years apart, support each other.

Where we disagree with the authors. On the economic conclusion: «the upfront cost of CT may ultimately result in lower overall expenses and improved outcomes for both the animal and the owner.» This study measured neither costs, nor outcomes, nor complications, nor treatment decisions. It is a reasonable prediction, and it belongs in the discussion, where predictions can belong; but it is the kind of sentence that ends up in an estimate, and at that point it has stopped being a discussion. We would gladly support it if someone had counted how many more or fewer extractions are performed after a CT, and with what outcome at one year. No one has.

What is missing from the literature, and whose job it is. What is missing is the study with histology: the heads already exist — five cadavers were used here and disarticulated — and what is needed is a tooth-by-tooth histological examination on a series of cadavers, with blinded reading of the two modalities. Also missing is the comparison between 30° and 45° on the mandibular obliques in the same population: two European studies use different angles and no one has compared them. And missing are control teeth: alpacas without clinical suspicion, to know how often each modality says «pathological» in a healthy mouth. These are three cards that cannot yet be written, and all three can be done on material already archived.

Our operational position, for members. Facing an alpaca with suspected root abscess: radiography remains the first exam — it is available, inexpensive, and in four of sixteen parameters it sees more than CT. If the plan is surgical, or if multiple involvement is suspected, CT is added under the same sedation: there is no second anaesthetic risk, and on average teeth will be found that radiography had not flagged — in this study, sixteen out of one hundred fifty-eight. And the owner should be told, in the right words: a normal radiograph does not rule out early lysis, because a 30-60% mineral loss is needed before lysis becomes visible — a figure reported by the authors from other literature, not measured in the alpaca.

Star rating

AxisScoreRationale
A · Scientific quality★★★☆☆An observational study with n stated at two levels (19 animals, 158 teeth), explicit selection criteria, individual per-subject data published (Table 1), two certified readers with independent reading and consensus, and kappa with 95% CI for all sixteen parameters with a stated and referenced interpretation scale: solidly level three, and at the high end of it. It does not rise to four because the reference standard is missing (the authors acknowledge this), blinding is not stated, clustering of teeth by animal is not modelled — so the published CIs are narrower than the true ones — and the 2 × 2 tables are not published.
B · Clinical utility★★★☆☆It offers a replicable procedure described in sufficient detail (six views with angles and parameters, complete CT protocol, sedation with dose and route, sixteen listed reading parameters) to be used with the caution imposed by the level of evidence: level three. It does not rise to four because without a reference standard the conditions of use cannot be made explicit: one knows where the two techniques diverge, not which to follow when they diverge. The operational point of greatest value — CT and radiography under the same sedation, without additional preparation — is, however, directly applicable.
C · Educational value★★★★☆It teaches, with a concrete and verifiable case, the difference between agreement and accuracy, that is, why a study without a reference standard cannot say who is right. It teaches that kappa and percentage agreement are not the same quantity, and it does so despite itself. And it documents that two certified radiologists under-detect on a species that is not their own: it is a lesson about reading error that holds well beyond dentistry. It is not five because the didactic reference on camelid dental anatomy remains card CRL-F-03.
D · Species relevance★★★☆☆A study on a single, stated species (Vicugna pacos, Huacaya type), in a European university hospital, with a population described subject by subject: it is the clinical context closest to the Italian one in the whole issue. It stays at three because it is single-centre on 19 animals, with selection for suspicion and without healthy controls: generalizability is moderate by construction, and the grid reserves the fourth star for large or multicentre studies.

No overall score, by design. A = 3 and D = 3 do not diminish the article: it is the best-analysed study in the issue, on a sample larger than the field currently allows.

Bottom line

It can be concluded that, on 158 already-suspect cheek teeth of 19 Huacaya alpacas, the agreement between radiography and CT ranges from none to moderate and never reaches the strong level: the best is apical root clubbing (kappa 0.71), the worst are pulp changes (kappa 0.026) and cementomas/hypercementosis (kappa 0.14); that on the overall suspicion of periapical infection agreement is weak (kappa 0.57; 95% CI 0.43-0.71); that CT identified more diseased teeth than radiography in twelve of sixteen parameters, with 16 more teeth judged potentially infected; and that in four parameters — periapical sclerosis, lamina dura, alveolar bone changes, apical clubbing — it is radiography that identifies more. It can also be concluded, as an operational fact, that CT of this region is performed with the same sedation protocol as radiography, in a single anaesthetic event and without additional preparation.

It cannot be concluded that CT is more accurate than radiography. There is no reference standard in this study: no histological verification, no healthy control, and the authors state the absence of histology as a significant limitation. Detecting more is not detecting better: the sixteen extra teeth could contain both true positives and false positives, and no one has established which. The «Percentage of Agreement» column cannot be read as a percentage agreement: it contains kappa multiplied by one hundred, and the raw percentage agreements are not published. The counts cannot be reconstructed: the 2 × 2 tables are not there, so one cannot know how many teeth each modality judged positive for each parameter. Nothing can be concluded about the non-suspect teeth: the 158 were selected because suspect on oral examination or radiographic screening, and there is no control group. Nothing can be concluded about costs and outcomes: the authors' prediction that CT reduces overall expense and improves outcomes is not supported by any data in this study. It cannot be concluded that the oblique angle used is the optimal one: the mandibular views were performed at 45°, the species' reference anatomical work recommends 30°, and no one has compared the two angles.

Messaggi da portare a casa

Take-home messages

  1. CT detects more abnormalities in twelve of sixteen parameters, and 16 more teeth out of 158. Radiography alone underestimates how many teeth are involved.
  2. In four parameters radiography sees more: periapical sclerosis, lamina dura, alveolar bone changes, apical clubbing. They are not two exams on a scale.
  3. CT and radiography under the same sedation. No second anaesthetic risk, no additional preparation.
  4. On pulp changes and cementomas radiography is not the right exam (kappa 0.026 and 0.14).
  5. Kappa ≠ percentage agreement. The article's «%» column is kappa × 100.
  6. Detecting more is not detecting better: histology is missing, and no one knows who is right where the two techniques disagree.
  7. A normal radiograph does not rule out early lysis — a 30-60% mineral loss is needed before it becomes visible (figure reported from other literature, not measured in the alpaca).
  8. Two certified radiologists under-detected because the anatomy was not their own. Keep card CRL-F-03 open alongside the images.
