Valutazione redazionale, quattro assi separatiEditorial rating, four separate axes
- AQualità scientifica3/5
- BUtilità clinica3/5
- CValore didattico4/5
- DRilevanza per la specie3/5
Che cosa misurano i quattro assi
- A · Qualità scientifica
- La domanda è ben posta? I metodi sono dichiarati? La statistica è appropriata? La conclusione sta dentro i dati?
- B · Utilità clinica
- Serve a chi ha l’animale davanti, oggi.
- C · Valore didattico
What the four axes measure
- A · Scientific quality
- Is the question well posed? Are the methods stated? Is the statistics appropriate? Does the conclusion stay within the data?
- B · Clinical usefulness
- Useful to whoever has the animal in front of them, today.
- C · Teaching value
- Does it teach something to those studying the subject?
- D · Relevance to the species
- Is it really about camelids, or a result borrowed from another species?
Each axis is worth 1 to 5 stars and carries its written reason. The stars in the index are the rounded average of the four.

«Clinical Findings and Survival in 56 Sick Neonatal New World Camelids»

Affiliazione comune, come registrata: Department of Veterinary Clinical Sciences, College of Veterinary Medicine, Purdue University, West Lafayette, IN.
3 · Authors
Complete list, four authors, in the form and order of the Crossref record:
Bertin F.R. · Squires J.M. · Kritchevsky J.E. · Taylor S.D.
None of the four has an ORCID registered in Crossref or Europe PMC. Common affiliation, as registered: Department of Veterinary Clinical Sciences, College of Veterinary Medicine, Purdue University, West Lafayette, IN. For the first author Crossref registers a distinct current address — 3999, chemin de la Côte Sainte-Catherine, Montréal, QC — which appears in the full text as «Present address», i.e. where the author was at the time of publication, not where the work was done.
4 · Affiliazione dell'autore corrispondente
Autore corrispondente: S.D. Affiliazione: Department of Veterinary Clinical Sciences, College of Veterinary Medicine, Purdue University.

Journal of Veterinary Internal Medicine — verificato il 31/08/2026.
7 · Journal status
Journal of Veterinary Internal Medicine — verified on 31/08/2026.
ISSN 0891-6640 (print, from PubMed esummary) and 1939-1676 (electronic, from PubMed and from Crossref). PubMed: yes. DOAJ: yes — the query doaj.org/api/search/journals/issn:1939-1676 returns total: 1, «Journal of Veterinary Internal Medicine». OpenAlex: 8,554 indexed works, is_oa: true, is_in_doaj: true.
A discrepancy between records, disclosed. As of the check date OpenAlex attributes the journal to Wiley, while Crossref registers as the publisher of this same DOI Oxford University Press (OUP), and the full text in PMC carries the header Oxford University Press. Chapter 14 §8.2 requires that in such cases the publisher's own record prevail: OUP. The OpenAlex entry is probably not updated since the journal's transfer, and this card reports it only so as not to hide the disagreement. It is the journal of the American College of Veterinary Internal Medicine, i.e. the proper venue for this subject matter.
8 · Licenza dell'articolo
Non è una licenza aperta. La riga di copyright del testo pieno in PMC dice: «Copyright © 2014 by the American College of Veterinary Internal Medicine».
8 · Article licence
This is not an open licence. The copyright line of the full text in PMC states: «Copyright © 2014 by the American College of Veterinary Internal Medicine». Europe PMC's license field is empty even though the article is freely readable (isOpenAccess: Y, inPMC: Y); Crossref registers as the sole licence OUP's standard publication model, not a Creative Commons one.
This card may therefore reproduce: factual data and short quoted passages, under the right of quotation. It may not reproduce figures or tables, and does not. Read and verified on 31/08/2026.
9 · Specie e soggetti
9 · Species and subjects
56 client-owned neonatal New World camelids: 23 llamas (Lama glama) and 33 alpacas (Vicugna pacos) — the breakdown is stated, and in this issue it is the only case in which the authors give it in full.
- Age: from 0 to 20 days, median 1 day.
- Sex: 17 males (30% of 56) and 39 females (70% of 56); the authors state that the overrepresentation of females is not significant (P = 0.053).
- Setting: single university referral hospital (Purdue University Veterinary Teaching Hospital, Indiana, United States), current caseload 2000-2010. No healthy control animals: the population consists entirely of referred sick animals.
- Seasonality: presentations from January to December, with a peak in May and June (21 of 56 cases, 38%).
- Suspected prematurity: 7 of 56 cases (13%).
10 · Domanda clinica (PICO)
10 · Clinical question (PICO)
Editorial formulation; the retrospective design allows it in prognostic form, not therapeutic.
In a New World camelid under 30 days of age, presented sick to a referral hospital [P], which variables at admission and which treatments administered [I], compared to their absence [C], are associated with survival to hospital discharge [O]?
The hypothesis the authors state is narrower than this, and is the one to be assessed: «Values of variables at admission and subsequent treatment would predict survival in sick neonatal NWC».
11 · Disegno dello studio
11 · Study design
Retrospective observational clinical case series, single-centre. No randomization, no constructed comparison group, no assignment of treatment by the researchers: whoever received a drug received it because a clinician decided so at the time, for reasons the record does not document.
12 · Level of evidence
Retrospective observational study without a control group — low level, with a clarification that in this case is the whole point of the card.
Consequence for the reader: the study establishes how often something was seen in 56 sick crias and which variables travel together with the outcome. It does not establish that the treatment caused survival. The odds ratios published here are univariate associations: the design does not separate the drug from the reason the clinician chose it. Chapter 14 §5.3 names this trap explicitly, and in this article it is not a theoretical risk — the authors themselves raise it, as read in field 16.
Dichiarato dagli autori: no.
13 · Relevant reporting standard
STROBE (observational studies), applied retroactively as a lens and not as a reproach: the article is from 2014-2015 and does not declare STROBE, nor did most retrospective veterinary studies declare it at the time. Declared by the authors: no.
14 · Metodo
Selezione.
Dati raccolti: segnalamento (specie, sesso, età), anamnesi disponibile compreso il mese di presentazione, reperti dell'esame fisico all'ammissione, variabili clinico-patologiche all'ammissione, esami diagnostici, diagnosi finale, trattamenti, esito.
- SIRS: presenza di ≥ 2 fra — temperatura > 39 °C o < 37,2 °C; frequenza cardiaca > 120 battiti/min; frequenza respiratoria > 40 atti/min; leucociti > 12.000/µL o < 6.000/µL; neutrofili a banda > 10 %.
- Esito: dimissione vivo dall'ospedale (esito a breve termine).
Significatività a P < 0,05. Software dichiarato in nota: Prism 5 for Mac OS X, GraphPad Software Inc., La Jolla, CA.
14 · Method
Selection. Clinical records of New World camelids presented to the Purdue University Veterinary Teaching Hospital from 2000 to 2010. Included if: age under 30 days at presentation and availability of a final diagnosis at death or discharge. The authors also state the criterion in the Methods section in a slightly different form — «NWC less than 30 days of age with complete medical records that presented between 2000 and 2010» — and the two formulations are not identical: the first requires a final diagnosis, the second a complete record.
Data collected: signalment (species, sex, age), available history including month of presentation, physical examination findings at admission, clinicopathologic variables at admission, diagnostic tests, final diagnosis, treatments, outcome.
Definitions, which are the most important part of the method.
- SIRS: presence of ≥ 2 of — temperature > 39 °C or < 37.2 °C; heart rate > 120 beats/min; respiratory rate > 40 breaths/min; leukocytes > 12,000/µL or < 6,000/µL; band neutrophils > 10%. The authors state they drew the criteria from two human medicine sources (Rangel-Frausto et al. 1995; Bone et al. 1997), not from a camelid source: this is a point the reader must keep in mind and it returns in field 18.
- Sepsis: SIRS present plus bacterial infection confirmed by any microbial culture.
- Outcome: discharge alive from the hospital (short-term outcome). There is no follow-up after discharge.
Analysis. Normality with the Shapiro-Wilk test; normal data as mean and standard deviation compared with unpaired t-test; nonparametric data as median and range compared with the Mann-Whitney U test; categorical data with chi-square or Fisher's exact test depending on expected counts, with odds ratio calculated where relevant. Significance at P < 0.05. Software stated in a footnote: Prism 5 for Mac OS X, GraphPad Software Inc., La Jolla, CA.
Number of sites: one. No sample size calculation, and no multivariable model: all published associations are univariate.
15 · Risultati (dati)
| Segno | Frequenza |
|---|---|
| Tachipnea (> 40 atti/min) | 26 casi su 51 (51 %) |
| Tachicardia (> 120 battiti/min) | 27 casi su 54 (50 %) |
| Ipotermia (< 37,2 °C) | 9 casi su 50 (18 %) |
| Ipertermia (> 39 °C) | 5 casi su 50 (10 %) |
Nessuna associazione fra segni clinici e diagnosi finale o esito.
Laboratorio. Emocromo e chimica clinica disponibili in 46 casi su 56 (82 %). Alterazioni più frequenti dell'emocromo, su 46: neutrofilia (> 7.000/µL) 24 casi (52 %), leucocitosi (> 12.000/µL) 14 casi (30 %), leucopenia (< 6.000/µL) 13 casi (28 %), neutropenia (< 3.000/µL) 9 casi (20 %). Nessuno di questi valori iniziali era associato all'esito.
SIRS. 28 casi su 56 (50 %) soddisfacevano i criteri all'ammissione.
Nessuna associazione significativa fra sepsi confermata e sopravvivenza (P = 0,07).
Batteri isolati dalle 6 emocolture positive: Staphylococcus epidermidis 3 casi (50 % delle emocolture positive), Streptococcus alfa-emolitico 3 casi (50 %), Micrococcus sp. 2 casi (33 %), Escherichia coli e Sphingomonas paucimobilis 1 caso ciascuno (17 %).
Lesione gastrointestinale in 9 casi (16 %); lesione dello scheletro appendicolare in 6 (11 %); malattia respiratoria diversa dall'atresia coanale in 5 (9 %), tutte polmoniti; lesione neurologica in 4 (7 %).
Combinazioni: 4 farmaci in 5 casi (15 %), 3 in 10 (29 %), 2 in 12 (35 %), uno solo in 7 (21 %).
Trattamenti senza associazione con l'esito: antinfiammatori (22 casi su 44 trattati, 50 % — ketoprofene IV 14, flunixin meglumine IV 9, desametasone IV 1); trattamenti oftalmici (15 casi, 34 % — P = 1,000, OR 0,8 [0,2-4,1]); ricostruzione labiale nelle 7 malformazioni vulvari (5 su 7 dimesse vive, 71 % — P = 1,000, OR 1,2 [0,2-6,8]).
I 9 casi (20 % dei trattati) che ricevettero omeprazolo orale, da solo (8) o con cimetidina (1), sopravvissero tutti; l'articolo scrive: «there was no significant association between administration of gastroprotectant drugs and survival (P = .047, OR: 10.1 [0.5-188.5])».

All denominators are those of the article. Where the article changes the denominator from one paragraph to the next — and it does, because not all tests were available in all cases — the denominator is reported here alongside the number.
Overall outcome. 37 of 56 camelids (66%) were discharged alive. Median hospitalization duration 5 days (range 1-31). Hospitalization was significantly shorter in non-survivors (P < 0.01): 12 of the 19 non-survivors (63%) died or were euthanized within 24 hours of admission.
Clinical signs at admission — note that the denominators here are not 56:
| Sign | Frequency |
|---|---|
| Tachypnea (> 40 breaths/min) | 26 of 51 cases (51%) |
| Tachycardia (> 120 beats/min) | 27 of 54 cases (50%) |
| Hypothermia (< 37.2 °C) | 9 of 50 cases (18%) |
| Hyperthermia (> 39 °C) | 5 of 50 cases (10%) |
No association between clinical signs and final diagnosis or outcome. One species difference: at admission llamas had a higher respiratory rate than alpacas (49 ± 3 vs 37 ± 2 breaths/min, P = 0.005).
Most common reasons for presentation (of 56): weakness or failure to grow 18 cases (32%), choanal atresia 9 cases (16%), colic 6 cases (11%), neurologic signs 5 cases (9%), lameness 4 cases (7%).
Laboratory. Complete blood count and clinical chemistry available in 46 of 56 cases (82%). Most frequent CBC abnormalities, of 46: neutrophilia (> 7,000/µL) 24 cases (52%), leukocytosis (> 12,000/µL) 14 cases (30%), leukopenia (< 6,000/µL) 13 cases (28%), neutropenia (< 3,000/µL) 9 cases (20%). None of these baseline values was associated with outcome.
On clinical chemistry, again of 46, the authors list twelve abnormalities; the most frequent were hyperchloremia (Cl > 103) in 40 cases (87%), hypernatremia (Na > 144) in 34 cases (74%), hyperphosphatemia (P > 5.7 mg/dL) in 30 cases (65%), creatinine > 1.8 mg/dL in 26 cases (57%), hypocalcemia (Ca < 10.7 mg/dL) in 25 cases (54%). None of these variables was associated with survival. (The units of the sodium, chloride and globulin thresholds are printed in the article as «mmol/mL» and «mg/dL» inconsistently; they have been copied here as printed. The correct form for sodium and chloride is mmol/L → [TO BE CHECKED AGAINST THE FULL TEXT] for the editorial form, not for the value.)
Two differences appear where no one was looking for them: blood glucose at admission was higher in females than in males (135 ± 16 vs 83 ± 15 mg/dL, P = 0.049), and hematocrit was higher in animals without gastrointestinal disease (35 ± 3% vs 30 ± 5%, P = 0.024).
SIRS. 28 of 56 cases (50%) met the criteria at admission. Of these 28: 16 (57%) with 2 positive criteria, 8 (29%) with 3, 4 (14%) with 4. Absence of SIRS was not associated with survival, and there was no association between the number of positive criteria and survival. Animals with SIRS had a higher respiratory rate (48 breaths/min [30-90] vs 36 [24-60], P = 0.002), more band neutrophils (1,300/µL [0-3,270] vs 0/µL [0-560], P = 0.010) and a lower albumin/globulin ratio (1.1 ± 0.2 vs 1.2 ± 0.2, P = 0.037). Within the SIRS group, survivors had a higher monocyte count than non-survivors (640/µL [0-4,910] vs 110/µL [0-1,760], P = 0.013).
Sepsis. Aerobic blood culture performed in 11 of 56 cases (20%); 6 of 11 (55%) with significant growth. Sepsis confirmed in 9 of the 28 SIRS cases (32% of SIRS cases), i.e. 16% of the 56. No significant association between confirmed sepsis and survival (P = 0.07).
Bacteria isolated from the 6 positive blood cultures: Staphylococcus epidermidis 3 cases (50% of positive blood cultures), alpha-hemolytic Streptococcus 3 cases (50%), Micrococcus sp. 2 cases (33%), Escherichia coli and Sphingomonas paucimobilis 1 case each (17%). In 3 cases more than one bacterium. The authors calculate that 80% of isolates were Gram-positive, and note that this differs from the work of Dolente and colleagues, where the most frequent were E. coli, Enterococcus sp., Listeria monocytogenes and Citrobacter sp. Growth of bacteria, even multiple, was not negatively associated with survival: 5 of the 6 animals with a positive blood culture (83%) were discharged alive.
Final diagnoses. At least one congenital defect in 23 of 56 cases (41%) — the largest group. Of the 23: choanal atresia 8 (35% of congenital defects), bone deformity 4 (17%), ocular malformations 3 (13%), vulvar malformation 3 (13%), cardiac malformations 2 (9%, one tetralogy of Fallot, one patent ductus arteriosus, one ventricular septal defect). Ophthalmic lesion in 12 of 56 cases (21%), the most common ulcerative keratitis (8 of 12, 67%); none of the 12 had been presented for an ocular reason. Gastrointestinal lesion in 9 cases (16%); appendicular skeleton lesion in 6 (11%); respiratory disease other than choanal atresia in 5 (9%), all pneumonia; neurologic lesion in 4 (7%).
The three associations with survival — and there are three, no more.
| Variable | P | Odds ratio (95% CI) |
|---|---|---|
| Absence of choanal atresia | 0.001 | 55.9 (2.5-1,232) |
| Administration of llama plasma (IV, 25 of 44 treated cases, 57%) | 0.013 | 4.9 (1.4-17.7) |
| Oral trimethoprim-sulfamethoxazole (18 of 34 that received antimicrobials, 53%) | 0.016 | 6.5 (1.3-32.2) |
Added to these is the more generic association: having received at least one antimicrobial (34 of 44 treated cases, 77%) — P = 0.019, OR 4.6 (1.4-15.1).
Antimicrobials used, in order of frequency among the 34 that received them: potassium penicillin G IV 25 cases (74%), oral TMS 18 cases (53%), amikacin IV 15 (44%), ceftiofur sodium IV 8 (24%), cefotaxime IV 5 (15%), ampicillin IV 4 (12%), gentamicin IV 3 (9%). Combinations: 4 drugs in 5 cases (15%), 3 in 10 (29%), 2 in 12 (35%), one only in 7 (21%).
⛔ No dose is written here because the article reports none. Neither for the antimicrobials, nor for the plasma, nor for the anti-inflammatories. Drug and route are given; dose, interval and duration are not →
[TO BE CHECKED AGAINST THE FULL TEXT]. Anyone wanting to derive a protocol from this would be inventing it, and this issue's rule forbids it.
Treatments without association with outcome: anti-inflammatories (22 of 44 treated cases, 50% — ketoprofen IV 14, flunixin meglumine IV 9, dexamethasone IV 1); ophthalmic treatments (15 cases, 34% — P = 1.000, OR 0.8 [0.2-4.1]); labial reconstruction in the 7 vulvar malformations (5 of 7 discharged alive, 71% — P = 1.000, OR 1.2 [0.2-6.8]).
One case in which the authors declare «not significant» a P below 0.05. The 9 cases (20% of those treated) that received oral omeprazole, alone (8) or with cimetidine (1), all survived; the article states: «there was no significant association between administration of gastroprotectant drugs and survival (P = .047, OR: 10.1 [0.5-188.5])». The P is below the threshold the authors themselves set, but the OR's confidence interval crosses 1 (0.5-188.5). The card reports the fact as printed, without correcting it: it is an internal inconsistency of the article, and is flagged in field 18.
It is possible that the positive association between administration of TMS and survival might be because it was initially administered to crias that were not judged to be as ill as those given intravenous antimicrobials, and not because of any property of the drug itself».
Gli autori propongono che i criteri di SIRS usati siano forse troppo larghi, e che «perhaps, a more rigorous definition of SIRS such as 3 or more (rather than 2 or more)» andrebbe adottata.

The authors conclude that «the use of antibiotics, particularly TMS, and llama plasma are recommended in sick neonatal NWC», and that the results might contribute to defining a sepsis scoring system specific to New World camelids.
The authors themselves raise the decisive objection about TMS, and the card reports it in full because it is the point at which the article is more honest than its own conclusion lets on: «It is possible that the positive association between administration of TMS and survival might be because it was initially administered to crias that were not judged to be as ill as those given intravenous antimicrobials, and not because of any property of the drug itself». It is confounding by indication, stated by the authors themselves.
The authors add a pharmacological reasoning, explicitly hypothetical: the bioavailability of oral TMS was found to be below 10% in adult alpacas, but the neonate, functionally monogastric, «likely achieves therapeutic plasma concentrations similar to what has been observed in horses and calves with oral bioavailability around 60%». The «likely» is theirs. The two figures (< 10% in the adult, ~60% in horse and calf) are cited by the authors from third-party works, not measured in this study.
The authors hypothesize that the association between survival and monocyte count within the SIRS group might share the mechanism described in severe human sepsis (monocyte consumption, platelet-monocyte aggregates associated with mortality). In their own study platelet count did not differ between survivors and non-survivors, and they write so.
The authors propose that the SIRS criteria used may be too broad, and that «perhaps, a more rigorous definition of SIRS such as 3 or more (rather than 2 or more)» should be adopted. This is a proposal, not a result: it was not tested on their own data.
The authors note that none of their septic animals developed septic arthritis or umbilical infection, unlike the foal, and infer that «this, in turn, might justify aggressive initial treatment». The «might» is theirs, and the sample of septic animals is 9 animals.
The authors recommend routine ophthalmic examination in all crias, even without ocular signs, on the basis of the 21% rate of lesions in animals presented for other reasons. This recommendation, unlike the others, rests on a frequency measured in their sample and not on an association with outcome.
17 · Limiti dichiarati dagli autori
- sul TMS, il confondimento per indicazione, nelle parole riportate al campo 16;
- sulla piastrine e sulla sepsi: «likely because of the low number of cases»;
- sull'equilibrio acido-base: gli autori scrivono che disturbi acido-base sono associati negativamente alla sopravvivenza in puledro e vitello e che «it is possible that a similar finding could have been seen in a larger sample»;
- sulla definizione di sepsi: «diagnostic criteria for sepsis could have allowed exclusion of animals that did indeed have sepsis, given the insensitivity of microbial culture (especially blood culture)»;
- sulle emocolture positive: possibili falsi negativi per il basso numero di colture e possibile contaminazione, che gli autori giudicano però improbabile perché tutti gli animali con emocoltura positiva avevano SIRS;
- sulla FTPI: «in the absence of a rigorous definition and diagnostic tests for FTPI, we could not objectively assess the role of FTPI in survival in our study» — ed è un limite pesante in uno studio la cui conclusione principale riguarda il plasma.
17 · Limitations stated by the authors
The article has no «Limitations» section. The limitations the authors acknowledge are scattered through the Discussion, and should be cited where they stand, not summarized into a single formula:
- on TMS, confounding by indication, in the words reported in field 16;
- on platelets and sepsis: «likely because of the low number of cases»;
- on acid-base balance: the authors write that acid-base disturbances are negatively associated with survival in the foal and calf and that «it is possible that a similar finding could have been seen in a larger sample»;
- on the definition of sepsis: «diagnostic criteria for sepsis could have allowed exclusion of animals that did indeed have sepsis, given the insensitivity of microbial culture (especially blood culture)»;
- on the positive blood cultures: possible false negatives because of the low number of cultures and possible contamination, which the authors judge unlikely, however, because all animals with a positive blood culture had SIRS;
- on FPT: «in the absence of a rigorous definition and diagnostic tests for FTPI, we could not objectively assess the role of FTPI in survival in our study» — and this is a heavy limitation in a study whose main conclusion concerns plasma.
- Nessun follow-up dopo la dimissione.
18 · Limitations added by the editors
Field distinct from 17. None of what follows is stated by the authors.
- The three published associations are univariate, and there is no multivariable model. With a sample of 56 animals of which 19 did not survive, a multivariable model would have been fragile — but the consequence must be stated: we do not know whether plasma and TMS are independent of each other, nor of the absence of choanal atresia. It is entirely plausible that the same crias that received plasma are the ones without a major congenital defect.
- The confidence intervals are enormous, and the conclusion does not account for this. The choanal atresia OR has a 95% CI from 2.5 to 1,232: a span of three orders of magnitude. It is compatible with «choanal atresia matters a bit» and with «choanal atresia is almost everything». The direction is solid, the magnitude is not estimated.
- The P = 0.047 declared non-significant (omeprazole, field 15) is an internal inconsistency relative to the 0.05 threshold set in the Methods. The most charitable reading — and probably the correct one — is that the authors gave weight to the CI crossing 1; but the text does not say so, and a reader who read only that line would come away confused.
- SIRS is defined with criteria taken from human medicine and never validated in the camelid. KT chapter 15 §1.3 notes that a SIRS criterion validated on neonatal camelids exists elsewhere; here the authors use Rangel-Frausto 1995 and Bone 1997. The study does not test its own criteria against a reference: that SIRS so defined does not predict survival is therefore a result for that definition, not for SIRS itself.
- No follow-up after discharge. «Survival» here means leaving the hospital alive, not being alive one month later. In a population with 41% congenital defects, the difference is not academic.
- The inclusion criterion is stated twice in different ways (final diagnosis available vs complete record). We do not know which was actually applied, nor how many animals were excluded: the starting denominator is missing. How many crias under 30 days passed through that hospital over eleven years? The article does not say →
[TO BE CHECKED AGAINST THE FULL TEXT], and it is not in the full text read. - European transferability: the commercial llama plasma underpinning this work's most cited association is a product of the US market. In Italy the practical question is not «should I give plasma?» but «which plasma can I obtain by tonight?», and this article does not address it. Card H-02 does.

21 · Cosa manca rispetto allo standard di reporting
Elenco puntato rispetto a STROBE, applicata retroattivamente:
21 · What is missing relative to the reporting standard
Bulleted list against STROBE, applied retroactively:
- The participant flow diagram (STROBE 13). Missing is the number of records examined, the number excluded and the reason. Only that 56 met the criteria is known.
- Handling of missing data (STROBE 12c). The denominators change — 56, 54, 51, 50, 46, 44, 34 — and the article does not explain why a test is missing nor whether it is missing at random. In a retrospective study on records, the absence of a blood culture is itself a clinical piece of information.
- Sample size calculation (STROBE 10). Absent. Legitimate in a retrospective convenience sample, but it should be stated, and it is not stated.
- Control of confounding (STROBE 16a). No adjusted analysis, hence no way to distinguish the effect of a treatment from the indication that prompted it.
- Ethical approval. Does not appear in the full text read. For a study on records of client-owned animals it is often not required, but STROBE and current JVIM norms want it stated anyway →
[TO BE CHECKED AGAINST THE FULL TEXT]: it is not in the full text. - The distinction between planned and exploratory analyses (STROBE 12b). The glucose difference between males and females is a typically exploratory finding with P = 0.049, discussed however with the same weight as the main hypotheses.
- No gap found, on the other hand, on: description of setting and dates (STROBE 5), eligibility criteria (7), definition of outcome variables (7), statistical methods listed (12a), funding statement (22).
22-24 · Giudizio — riquadro separato
22 · Commento redazionale — riquadro dell'associazione

What this article gives an Italian veterinarian, and what it does not.
It gives, first of all, a numbered snapshot of what comes through that door: of 56 sick crias, four in ten had a congenital defect, one in five an ocular lesion that no one was looking for, and half met the SIRS criteria. Someone who works with horses arrives at a cria with neonatal sepsis on their mind; this work says that the first question, statistically, is anatomical. Passing a probe into the choanae and checking the eyes with fluorescein are two gestures that cost a minute each and that, in this case series, would have changed the framing in over half the cases.
It gives, second, a negative result that is worth as much as a positive one: in this population SIRS does not predict survival, nor does the number of positive criteria. This is not a failure of the study, it is information: cria triage is not done by counting tachypnea and leukocytes. It must be said unambiguously that this holds for the criteria used here, taken from human medicine and never calibrated on the camelid.
Where we do not follow the authors. The article's conclusion — «the use of antibiotics, particularly TMS, and llama plasma are recommended» — is stronger than the design that supports it, and the authors know it, because three pages earlier they write that TMS might have been given precisely to the less severe crias. Of the two sentences, the one to bring into the clinic is the second. Oral TMS in a cria that survives is, in this data, plausibly a marker of lesser severity at least as much as a therapy. That TMS should be used is likely for other reasons; that it be used because this study found an OR of 6.5 would be to misread an honesty that the authors had.
Where instead the article is stronger than it looks. The 80% of Gram-positive isolates in the blood cultures is an inconvenient and therefore precious datum: it contradicts the Dolente case series and contradicts the foal, where Gram-negatives are about 69%. These are six positive blood cultures, a tiny number, and no one should change empirical therapy on six cultures. But it is exactly the kind of signal that, if ignored, remains ignored for twenty years because no one resamples. If in Italy a registry of cria blood cultures were built — even just with the clinics that perform them — within three years we would know what our flora is, not Indiana's.
Our operational position, for members. Facing a depressed cria: probe into the choanae, fluorescein, glucose, and the question about colostrum before the mental antibiogram. None of the three associations in this article authorizes writing a protocol. Plasma is given for the reason it has always been given — passive immunity — and card H-02 states at what dose it was actually given and what can go wrong while giving it.
A note on this series' method. We have left in the card the
P = 0.047that the article declares non-significant, instead of silently adjusting it. An article's inconsistency is not corrected in the card that summarizes it: it is flagged. A rereader must be able to find the article again as it is printed.23 · Star rating
Axis Score Rationale A · Scientific quality ★★★☆☆ Observational study with a stated n (56), explicit selection criteria, operational definitions of SIRS and sepsis written out in full, analysis appropriate to the design, funding and conflicts declared: this is the description of the grid's third level. It does not rise to four because there is no multivariable model, no sample size calculation, no participant flow diagram or starting denominator, and missing data are not addressed. B · Clinical utility ★★★☆☆ Offers a replicable procedure — systematic framing of the sick cria, with active search for choanal atresia and ocular lesions even without signs — described in sufficient detail and to be used with the caution imposed by the level of evidence. It does not rise to four because the three associations with survival are univariate and confounded by indication, and because the article reports no dose whatsoever: the grid (§11, axis B) forbids exceeding the third star on a therapeutic result whose protocols are not reported in full. C · Educational value ★★★★☆ Course material, not only reference material: it shows a complete differential reasoning process for the neonate, and teaches with a real case what confounding by indication is — it teaches it better than a textbook, because it is the authors themselves who dismantle their own conclusion. It is not five because more complete reviews exist on general neonatal camelid framing. D · Relevance to the species ★★★☆☆ A study on camelids, with a stated llama/alpaca breakdown (23 and 33) and with an actually measured difference between the two species: this is more than the average of this literature does. It stays at three because it is single-centre, because the setting is a North American university hospital, and because the treatment that supports the conclusion — commercial llama plasma — has no equally available equivalent in European practice. No overall score, by design.
24 · Bottom line
It can be concluded that in 56 sick neonatal New World camelids referred to a university hospital between 2000 and 2010, 37 of 56 (66%) were discharged alive; that congenital defects were the most frequent final diagnosis (23 of 56, 41%) and that absence of choanal atresia was associated with survival (P = 0.001; OR 55.9; 95% CI 2.5-1,232); that an ophthalmic lesion was present in 12 of 56 (21%) and in none of these was it the reason for the visit; that, with the SIRS criteria borrowed from human medicine used in this study, neither SIRS nor the number of positive criteria was associated with survival; and that, in univariate analysis, intravenous llama plasma (OR 4.9; 1.4-17.7) and oral trimethoprim-sulfamethoxazole (OR 6.5; 1.3-32.2) were associated with discharge alive.
It cannot be concluded that plasma or TMS cause survival: the associations are univariate, unadjusted, and the authors themselves caution that TMS might have been given to the less severe crias. No dosage can be derived from this article: it contains none, for any drug →
[TO BE CHECKED AGAINST THE FULL TEXT]. Nothing can be concluded about survival beyond discharge, because there is no follow-up. It cannot be concluded that SIRS is useless in the cria: it can be concluded that these criteria, in this case series, did not predict outcome. It cannot be concluded that the flora is predominantly Gram-positive in crias: the 80% is calculated on 6 positive blood cultures out of 11 performed, and contradicts another camelid case series. And it cannot be known how many crias were excluded to arrive at 56: the starting denominator is not published.Camelid Research Studies APS — CRL editorial staff · 31/08/2026
