Valutazione redazionale, quattro assi separatiEditorial rating, four separate axes
- AQualità scientifica3/5
- BUtilità clinica4/5
- CValore didattico4/5
- DRilevanza per la specie4/5
Che cosa misurano i quattro assi
- A · Qualità scientifica
- La domanda è ben posta? I metodi sono dichiarati? La statistica è appropriata? La conclusione sta dentro i dati?
- B · Utilità clinica
- Serve a chi ha l’animale davanti, oggi.
- C · Valore didattico
What the four axes measure
- A · Scientific quality
- Is the question well posed? Are the methods stated? Is the statistics appropriate? Does the conclusion stay within the data?
- B · Clinical usefulness
- Useful to whoever has the animal in front of them, today.
- C · Teaching value
- Does it teach something to those studying the subject?
- D · Relevance to the species
- Is it really about camelids, or a result borrowed from another species?
Each axis is worth 1 to 5 stars and carries its written reason. The stars in the index are the rounded average of the four.

9 · Specie e soggetti
- 204 trasfusioni in 174 ricoveri in ospedale.
- Alpaca 141 (81 %) · lama 23 (13 %) · guanachi 5 (3 %) · cammelli 5 (3 %).
- Sesso: 100 femmine (57,5 %), 72 maschi interi (41,4 %), 2 maschi castrati (1,1 %).
9 · Species and subjects
174 admissions, referring to 169 unique camelids — the article is explicit about this difference and it must be reported as it stands, because the denominators of the work are not all the same number:
- 204 transfusions across 174 hospital admissions.
- The breakdown by species is given on 174 (the full text writes "Among all unique camelids (n = 174)", while the title and Methods speak of 169 unique camelids; five animals had two admissions each — 3 at Kansas and 2 at Colorado — which is plausibly the origin of the discrepancy, but the article does not resolve it →
[TO BE CHECKED AGAINST THE FULL TEXT]: in the full text as read, the contradiction remains). - Alpacas 141 (81%) · llamas 23 (13%) · guanacos 5 (3%) · camels 5 (3%).
- Sex: 100 females (57.5%), 72 intact males (41.4%), 2 castrated males (1.1%).
- Median age 3 days, range from neonate to 24 years. Split into the four groups used throughout the analysis: group 1 (≤ 48 hours) 74 animals (42.5%); group 2 (> 48 hours and ≤ 1 month) 56 (32.2%); group 3 (> 1 month and < 1 year) 9 (5.2%); group 4 (≥ 1 year) 35 (20.1%).
- Setting: two US referral university hospitals — Kansas State University Veterinary Health Center and Colorado State University Veterinary Teaching Hospital — from 1 January 2010 to 31 July 2024. Median length of hospitalization 5 days (1-38).
Three quarters of the population is under one month old. This is a study on transfused camelids that is, in fact, largely a study on crias.
10 · Domanda clinica (PICO)
L'obiettivo dichiarato dagli autori è duplice e va riportato com'è: descrivere «the indications for, methods of, and outcomes associated with blood product transfusions in camelids», e come obiettivo secondario identificare i fattori di rischio per reazioni e non sopravvivenza.
10 · Clinical question (PICO)
In a camelid receiving a blood product at a referral hospital [P], which characteristics of the animal and of the transfusion — age, species, sex, prior transfusions, product type, premedication, dose, rate [I/C] — are associated with the occurrence of a transfusion reaction and with discharge alive from the hospital [O]?
The authors' stated aim is twofold and must be reported as it stands: to describe «the indications for, methods of, and outcomes associated with blood product transfusions in camelids», and as a secondary aim to identify risk factors for reactions and non-survival.
11 · Disegno dello studio
11 · Study design
Retrospective observational study on medical records, dual-center. PubMed formally classifies it as a Multicenter Study. No control arm, no assignment by the researchers: premedication, rate and dose were decided at the time, case by case, by the clinician.
Gli autori lo scrivono in una frase che vale la pena tenere: «it is important to note that this was not a controlled study designed to assess causative associations».
12 · Level of evidence
Retrospective observational study, dual-center, without a control group. Low level for causal questions, but the highest available in the species on this topic: the authors write that «evidence-based guidelines for transfusions in camelids are not currently available», and this is the largest case series ever published.
Consequence for the reader: what holds up are the frequencies with their denominators — how many reactions out of how many transfusions, of what type, with what outcome — and the doses actually administered. What does not hold up are cause-effect relationships: that premedication did not reduce reactions in this case series does not prove that it is not useful, because those who premedicated chose whom to premedicate. The authors write it in a sentence worth keeping: «it is important to note that this was not a controlled study designed to assess causative associations».
13 · Standard di reporting pertinente
STROBE, applicata retroattivamente.
13 · Relevant reporting standard
STROBE, applied retroactively. Declared by the authors: no. The article does, however, carry on its own account four formal statements that STROBE does not require and that JVIM does — conflicts, funding, off-label antimicrobial use, institutional approval — and it is the best-declared work of the five in this issue.
14 · Metodo
- Reazione trasfusionale (TR): «any change in clinical exam or pathologic data identified in the medical record that was potentially attributable to administration of a blood product».
- Sopravvivenza a breve termine: dimissione vivo dall'ospedale.
Calcoli. Dose dell'emoderivato in mL/kg di peso corporeo e velocità complessiva di somministrazione prima di qualsiasi reazione in mL/kg/h. Storia di emoderivati ricevuti entro 1 settimana, 3 settimane, o in qualunque momento precedente.
Analisi. Significatività a P ≤ 0,05.
14 · Method
Case search. Records of the two hospitals queried for billing codes relating to the purchase of blood products or the administration of a transfusion in a camelid species (llama, alpaca, guanaco, vicuña, camel), from 1 January 2010 to 31 July 2024. Included if: species of the Camelidae family, documented proof of having received any amount of blood product, and known outcome.
The operational definitions, which here are the heart of the method and which no prior camelid work had written:
- Hospital visit: a single hospital access, inpatient or outpatient.
- Transfusion: administration of a blood product from a single source in a single continuous period. Two different products administered in sequence (for example whole blood followed by plasma, or two different lots of commercial plasma) count as two transfusions — and the reason is explicit: «given the opportunity for multiple antigenic sources to elicit a TR».
- Transfusion event: a single period of administration of one or more transfusions.
- Transfusion reaction (TR): «any change in clinical exam or pathologic data identified in the medical record that was potentially attributable to administration of a blood product». It is a broad, retrospective definition, and the authors declare it as such.
- Premedication: any NSAID, corticosteroid or antihistamine administered within 24 hours before the transfusion.
- Short-term survival: discharge alive from the hospital.
Calculations. Blood product dose in mL/kg body weight and overall rate of administration before any reaction in mL/kg/h. History of blood products received within 1 week, 3 weeks, or at any prior time.
Analysis. Generalized linear model with binary distribution and logit link function (SAS 9.4 GLIMMIX procedure). Probability of reaction and probability of survival modelled with univariate logistic regression models, one per variable of interest. Descriptive statistics with UNIVARIATE, means and confidence intervals with LSMEANS. Significance at P ≤ 0.05.
15 · Risultati (dati)
Prodotti somministrati, su 204 trasfusioni: plasma 172 (84,3 %), sangue intero 29 (14,2 %), emazie concentrate 3 (1,5 %).
| Indicazione | Plasma (n = 172) | Sangue intero (n = 29) | Emazie (n = 3) | Totale |
|---|---|---|---|---|
| FTPI + setticemia | 136 | 0 | 0 | 136 |
| Ipoproteinemia | 31 | 0 | 0 | 31 |
| Perdita ematica esterna | 1 | 19 | 3 | 23 |
| Anemia di causa ignota | 0 | 9 | 0 | 9 |
| Emolisi | 0 | 1 | 0 | 1 |
| Coagulopatia | 1 | 0 | 0 | 1 |
| Specie ricevente | Donatore lama | Donatore alpaca | Donatore cammello | Non specificato | Totale |
|---|---|---|---|---|---|
| Alpaca | 102 (65 %) | 8 (5 %) | 0 | 48 (30 %) | 158 |
| Lama | 18 (55 %) | 9 (27 %) | 0 | 6 (18 %) | 33 |
| Guanaco | 3 (43 %) | 3 (43 %) | 0 | 1 (14 %) | 7 |
| Cammello | 4 (66 %) | 0 | 1 (17 %) | 1 (17 %) | 6 |
| Totale | 127 | 20 | 1 | 56 | 204 |
J Vet Intern Med 2026;40(1):aalag008. DOI 10.1093/jvimsj/aalag008):
Nessun antistaminico in nessuna cartella.
Sopravvivenza. Su 174 ricoveri, dimissione vivo in 128 (73,6 %); 20 morti e 26 eutanasie. Per gruppo di età:
| Gruppo di età | Sopravvissuti | Non sopravvissuti | Totale |
|---|---|---|---|

Products administered, out of 204 transfusions: plasma 172 (84.3%), whole blood 29 (14.2%), packed red blood cells 3 (1.5%). Origin of records: 119 from Colorado (117 camelids, 119 admissions, 136 transfusions) and 55 from Kansas (52 camelids, 55 admissions, 68 transfusions). No stored blood product: all blood was collected from the donor at the time of the transfusion.
Indications (table 1 of the article, reproduced in full — CC BY-NC licence):

FTPI = failure of transfer of passive immunity. All animals with an FTPI or sepsis indication were two months of age or younger; 136 of the 152 transfusions performed in crias under two months fall under this entry. Nine camelids — all crias under 7 days old — had confirmed or suspected neonatal sepsis as their sole indication. One case out of 204 was transfused solely at owner's request: a 2-day-old alpaca.
Xenotransfusion (table 4, reproduced in full). The donor species is documented in 148 of 204 transfusions; in the other 56 (27%) it is not specified.
| Recipient species | Llama donor | Alpaca donor | Camel donor | Not specified | Total |
|---|---|---|---|---|---|
| Alpaca | 102 (65%) | 8 (5%) | 0 | 48 (30%) | 158 |
| Llama | 18 (55%) | 9 (27%) | 0 | 6 (18%) | 33 |
| Guanaco | 3 (43%) | 3 (43%) | 0 | 1 (14%) | 7 |
| Camel | 4 (66%) | 0 | 1 (17%) | 1 (17%) | 6 |
| Total | 127 | 20 | 1 | 56 | 204 |
(Percentages are by row, as in the article; on the camel row 66 + 17 + 17 = 100 due to rounding.) In the large majority of cases the blood was not of the recipient's species, and by far the most common case is llama plasma to an alpaca.
The doses — with dose, route, species and source, as chapter 14 requires
These are the doses that were administered, not the doses recommended by the authors. The distinction is the article's, not ours: the authors later compare these doses with those in the manuals, and find them different (field 16).
Plasma, intravenous, in camelids (Mullins CR, Oman RE, Gebhardt JT, Reppert EJ. J Vet Intern Med 2026;40(1):aalag008. DOI 10.1093/jvimsj/aalag008):

Intraperitoneal route: 4 of 204 transfusions, all in alpacas under 48 hours old with an FTPI indication (two of the four also dysmature). Where recorded, the volume was 35-40 mL/kg BW. No reaction recorded and all four discharged alive. The authors explicitly state that they were not able to statistically analyze route of administration because of the too small number of intraperitoneal cases. All other transfusions were intravenous.
⛔ No dose is given for premedication, because the article reports none: which drug and how many times is known, not at what dose →
[TO BE CHECKED AGAINST THE FULL TEXT]. Premedication administered before 118 of 204 transfusions (58%): an NSAID in 115 of 118 (97%) — flunixin meglumine in 112, meloxicam in 3 —, a corticosteroid in 2 of 118 (2%), both in 1 of 118 (1%); the corticosteroid was dexamethasone in all 3 cases. No antihistamine in any record. The authors caution that it was often not possible to determine whether the drug had been given for the transfusion or for the underlying disease.
Transfusion reactions. 26 reactions in 24 of 204 transfusions (12%); two camelids had two each. The 24 transfusions concern 23 camelids, of whom 11 of 23 (48%) were not discharged alive. Only 3 of the 24 transfusions with a reaction (13%) had been preceded by a prior transfusion.
By product type: whole blood 5 of 29 transfusions (17%); plasma 19 of 172 (11%); packed red blood cells 0 of 3. The difference between whole blood and plasma is not statistically significant (P = 0.51).
Reaction types and survival (table 5, reproduced in full):
| Reaction type | Reactions (n = 26) | Survivors | Non-survivors |
|---|---|---|---|
| Increased rectal temperature | 7 | 6 | 1 |
| Increased heart rate | 6 | 3 | 3 |
| Change in respiratory rate, effort or auscultation | 6 | 1 | 5 |
| Tremor / shivering | 3 | 1 | 1 |
| Change in mentation or neurologic signs | 3 | 2 | 1 |
| Systemic anaphylaxis | 1 | 0 | 1 |
(One camelid had tremor in two distinct transfusion events of the same admission: this is the reason why the "tremor" row does not sum to 3.)
Changes in vital parameters were the most common finding: 20 of 26 reactions (77%). Crias under one month accounted for 83% of all respiratory reactions (5 of 6). All tremors resolved on their own, without intervention or change of rate.
The only death directly attributed to the transfusion: a 24-hour-old cria manifested suspected systemic anaphylaxis within 40 minutes of the start of a plasma transfusion — signs reported: «panting and frothy epistaxis that quickly led to death» —, diagnosis «strongly supported on post-mortem examination». Fatal anaphylaxis rate: 0.5% (1 of 204 transfusions).
Survival. Out of 174 admissions, discharge alive in 128 (73.6%); 20 deaths and 26 euthanasias. By age group:
| Age group | Survivors | Non-survivors | Total |
|---|---|---|---|
| 1 (≤ 48 hours) | 59 (79.7%) | 15 (20.3%) | 74 |
| 2 (> 48 hours, ≤ 1 month) | 43 (76.8%) | 13 (23.2%) | 56 |
| 3 (> 1 month, < 1 year) | 5 (55.6%) | 4 (44.4%) | 9 |
| 4 (≥ 1 year) | 21 (60.0%) | 14 (40.0%) | 35 |
Difference between age groups significant (P = 0.028): group 1 has a survival probability of 80.5% (95% CI 70.4-87.7) versus 57.5% (95% CI 41.9-71.8) for group 4 (P = 0.046).
The only two significant variables in the entire study, in a list otherwise entirely negative:
| Factor | P for reaction | P for survival |
|---|---|---|
| Age | 0.93 | 0.028 |
| Species | 0.56 | 0.42 |
| Sex | 0.36 | 0.12 |
| Prior transfusion within 1 week | 0.92 | 0.58 |
| Prior transfusion within 3 weeks | 0.64 | 0.74 |
| Prior transfusion at any time | 0.85 | 0.78 |
| Blood product type | 0.51 | 0.95 |

| Blood product dose (mL/kg) | 0.82 | 0.069 |
| Rate of administration (mL/kg/h) | 0.063 | 0.60 |
| Occurrence of a reaction | not applicable | 0.033 |
Having had a reaction is associated with a lower probability of being discharged alive: 54.5% (95% CI 34.0-73.5) versus 76.5% (95% CI 69.5-82.3), P = 0.033.
(A small inconsistency in the article, reported as it stands: the abstract prints this probability as 54.6% [95% CI 34.0-73.6], the Results text as 54.5% [34.0-73.5]. The discrepancy is one decimal point and changes nothing, but a number that appears twice differently in the same article should be flagged, not silently harmonized.)
Repeat transfusions: 29 camelids received more than one. Interval shortest between two transfusion events: 16 hours; longest within the same admission: 4 days; longest between different admissions: 14 months.
Crossmatch: performed before only 1 of 204 transfusions, in an animal never transfused before, and never before any subsequent transfusion.
Source documentation: the commercial brand or the identity of the donor animal is recorded only in 55% of transfusions. Biochemical characteristics and hematocrit of the transfused blood rarely recorded.
16 · Interpretazione degli autori
Gli autori concludono che gli emoderivati «can fulfill many therapeutic needs in the treatment of camelid species with a reasonable expectation of safety» e che «a history of prior transfusions does not appear to significantly affect the future risk of a TR».
Scrivono che la dose mediana di plasma somministrata ai crias di meno di un mese — 35 mL/kg — è «within the range that has been recommended for treatment of FTPI (20-40 mL/kg) in neonatal ruminants» ma «greater than that recommended in camelid crias (15-25 mL/kg)».
La loro raccomandazione — «administration of plasma to an otherwise healthy cria with inadequate passive transfer of immunity should be undertaken with care and include monitoring for signs of TACO» — è una raccomandazione, non un risultato.
Sulla xenotrasfusione, gli autori argomentano — citando studi precedenti secondo cui lama e guanachi non avrebbero isoagglutinine né isoemolisine, e secondo cui lama e alpaca hanno molteplici fattori eritrocitari — che «administration of blood products between camelid species is generally safe and feasible, and this will help expand the potential donor pool».
Sulla mortalità associata alle reazioni, gli autori scrivono che «death during or shortly after a transfusion might have been more influenced by underlying disease condition than by the transfusion».
Sulla premedicazione, osservano che un FANS potrebbe essere preferibile a un corticosteroide nei camelidi «due to their propensity for glucose dysregulation with the latter» — affermazione appoggiata a due citazioni, non a un dato di questo studio.
16 · Authors' interpretation
The authors conclude that blood products «can fulfill many therapeutic needs in the treatment of camelid species with a reasonable expectation of safety» and that «a history of prior transfusions does not appear to significantly affect the future risk of a TR».
On dose, and this is the point where the authors tell the reader something the reader does not know. They write that the median plasma dose administered to crias under one month — 35 mL/kg — is «within the range that has been recommended for treatment of FTPI (20-40 mL/kg) in neonatal ruminants» but «greater than that recommended in camelid crias (15-25 mL/kg)». The two ranges are cited by the authors from third-party sources (respectively a source on neonatal ruminants and a camelid source), not measured in this study: the card reports them as second-degree citations, per chapter 14 §4.2.
On the resulting risk. The authors name TACO — transfusion-associated circulatory overload — as the respiratory reaction that concerns them most, precisely because the cria receives large volumes. They cite a study on healthy crias that received 30 mL/kg of plasma intravenously over 90 minutes, in which «an increase in central venous pressure and decrease in functional residual capacity» was documented: this too is a finding from another work, reported by the authors. Their recommendation — «administration of plasma to an otherwise healthy cria with inadequate passive transfer of immunity should be undertaken with care and include monitoring for signs of TACO» — is a recommendation, not a result.
The authors suggest choosing a product with a high IgG concentration precisely to reduce the volume needed, and — where commercial plasma of known titer is not available — indicate the ideal donor: «a well-vaccinated castrated male or maiden female herd mate with no history of prior transfusion», because it will have an antibody profile reflecting the agents present on that farm. This is expert advice, not a data result.
On xenotransfusion, the authors argue — citing previous studies according to which llamas and guanacos would not have isoagglutinins or isohemolysins, and according to which llamas and alpacas have multiple erythrocyte factors — that «administration of blood products between camelid species is generally safe and feasible, and this will help expand the potential donor pool». The empirical basis in their own study is the 12% reaction rate, not a comparison between homologous and heterologous transfusion, which no one has made.
On crossmatching, the authors question its routine usefulness: costly, slow, not universally available; agglutination «is unable to assess hemolytic reactions»; no type of crossmatch identifies every reaction. The authors hypothesize that the absence of association between prior transfusion and reaction might reflect a wide range of erythrocyte groups, lower alloimmunization in the species, minor unrecognized reactions, or simply too small a sample. The four explanations are listed by them on the same footing.
On rate, the trend — higher rates, fewer reactions (P = 0.063) — is commented on by the authors with a caveat worth citing: «these results should not be construed as fact that a more rapid rate of fluid administration is protective against TRs», and the explanation they propose is the reverse: the clinician speeds up transfusions that are going well.
On mortality associated with reactions, the authors write that «death during or shortly after a transfusion might have been more influenced by underlying disease condition than by the transfusion».
On premedication, they note that an NSAID might be preferable to a corticosteroid in camelids «due to their propensity for glucose dysregulation with the latter» — a statement supported by two citations, not by a finding of this study.
17 · Limiti dichiarati dagli autori
- «As for other retrospective studies, the availability, legibility, and completeness of medical records was imperfect.»
- «There was inherent bias in differentiating TRs from underlying disease processes based on review of the medical record alone.» Gli autori dichiarano di aver adottato per questo un approccio conservativo nella definizione di reazione, e che alcune anomalie «could have been incorrectly ascribed to a transfusion complication».
- «There were relatively few red blood cell transfusions which limited our ability to draw conclusions about relative transfusion risk by blood product.»
- Sulle eutanasie: «reasons for euthanasia were frequently omitted, but financial constraints could have influenced survival».
17 · Limitations stated by the authors
The article has a dedicated paragraph, and the limitations must be cited, not summarized:
- «As for other retrospective studies, the availability, legibility, and completeness of medical records was imperfect.»
- «There was inherent bias in differentiating TRs from underlying disease processes based on review of the medical record alone.» The authors state that they adopted for this a conservative approach in the definition of reaction, and that some abnormalities «could have been incorrectly ascribed to a transfusion complication».
- «There were relatively few red blood cell transfusions which limited our ability to draw conclusions about relative transfusion risk by blood product.»
- Uncontrolled variability in the characteristics of the transfused products.
- The inability to evaluate antihistamines and corticosteroids as premedication, because of their near-nil use in this case series.
- On hemolytic reactions: the authors caution that the absence of recorded hemolysis does not mean that alloimmunization does not occur, and that monitoring after discharge — sometimes within 24-48 hours of the transfusion — did not allow delayed reactions to be identified.
- On euthanasias: «reasons for euthanasia were frequently omitted, but financial constraints could have influenced survival».
La conclusione «a history of prior transfusions does not appear to significantly affect the future risk» è formulata con prudenza — «does not appear» — ma verrà citata come se dicesse di più.
18 · Limitations added by the editors
- All models are univariate, one variable at a time. With 24 reactions out of 204 transfusions a multivariable model would have been unstable, but the consequence must be stated: the P = 0.62 for premedication and P = 0.85 for prior transfusion are not adjusted for anything, and in particular not for the severity of the underlying disease — which is precisely what drove the choice of whom to premedicate.
- The study's power is not calculated, and the main results are negative. A P = 0.85 on 24 events does not say "there is no effect": it says "with 24 events we would not have seen it anyway". The conclusion «a history of prior transfusions does not appear to significantly affect the future risk» is phrased with caution — "does not appear" — but will be cited as if it said more. No confidence interval is published for the null estimates, which makes it impossible for the reader to judge how large an effect the study might have missed →
[TO BE CHECKED AGAINST THE FULL TEXT]: in the full text as read, those CIs are not there. - 169 in the title, 174 in the results. The number of unique camelids changes between the title and the survival table, and the article does not explain it. Anyone citing this work must state which of the two numbers they are using and for what: 204 transfusions and 174 admissions are the denominators on which the analysis is done.
- 27% of transfusions have no documented donor species. The conclusion on the safety of xenotransfusion rests on a table in which 56 of 204 cells are "not specified": it is a large gap in what is perhaps the most citable message of the work.
- The definition of reaction is a double-edged sword. Broad ("any change potentially attributable") and retrospective, in a population that is three quarters critical neonates. The 12% is therefore an estimate of how many times someone wrote something in the record during a transfusion, not a measure of biological incidence. The authors partly acknowledge this; we state it in full, because it is the number the reader will take away.
- The study's hardest signal is not in the abstract. Of the 6 respiratory reactions, 5 were not discharged alive; and 5 of the 6 respiratory reactions occurred in crias under one month. These are very small numbers and no test was performed on that subgroup — but the direction is the same as the mechanism the authors fear (TACO), and it sits in a table, not in the conclusions.
- European transferability, and it must be stated explicitly here. The entire case series rests on an availability that is not the norm in Italy: commercial llama plasma, university donor herds, two referral hospitals. The Italian veterinarian's question — «tonight, for this cria, what do I have?» — finds in this article a partial but real answer: the authors' advice on the farm donor (well-vaccinated castrated male or never-transfused female of the same herd) and the finding that xenotransfusion between camelid species was well tolerated. Both, however, are respectively an opinion and an observation without comparison.

21 · Cosa manca rispetto allo standard di reporting
Rispetto a STROBE:

- The flow diagram (STROBE 13a). It is known that the search started from billing codes and that 119 + 55 records were identified, but not how many records were excluded or why.
- Estimates with their confidence interval for negative results (STROBE 16a). Only P values are published. For the two significant variables the CIs are there; for the nine non-significant ones they are not.
- The sample size calculation (STROBE 10). Absent, and in a study whose conclusions are largely negative it is the gap that weighs most.
- Subgroup analyses (STROBE 12b). The finding of respiratory reactions in crias is described but not analyzed, and it is not stated whether it was a question posed beforehand or found afterward.
- Handling of missing data (STROBE 12c). Variable denominators — 204, 174, 148, 134, 118 — without an analysis of what distinguishes complete records from incomplete ones.
- No gap found, instead, on: setting and dates (STROBE 5), eligibility criteria (6a), variable definitions (7), statistical methods (12a), limitations (19), funding (22). On point 7 this article is better than the standard: it defines "transfusion", "transfusion event" and "reaction" so that another group could replicate it.

Why this is the most useful card in the issue.
Because it is the only one that delivers a number that can be used tonight and a risk that can be named to the owner before signing. An Italian veterinarian who has to transfuse a cria today has, after this work, three things that yesterday they did not have: the dose that was actually used in 172 plasma transfusions (median 35-36 mL/kg IV in the cria under one month), the reaction rate with its denominator (24 of 204, 12%), and the fatal anaphylaxis rate (1 of 204, 0.5%). These are the three figures of an informed consent.
The two genuine pieces of news, for those working in Europe.
The first is xenotransfusion. In 127 of 204 transfusions the donor was a llama and the recipient often an alpaca; guanacos received alpaca blood; camels received llama plasma. This widens the donor pool exactly where in Italy it is narrowest: in a mixed herd, the big, docile llama in the group is a donor for the alpacas. It must be stated precisely what this study demonstrates and what it does not: it demonstrates that, with those combinations, the overall reaction rate was 12%; it did not compare homologous and heterologous transfusion, and in 27 cases out of 100 the donor species was not even written in the record.
The second is that a prior transfusion is not a prohibition. It is the question that blocks the second intervention — «but I already transfused it three weeks ago» — and in this case series having a prior transfusion, at any time, was not associated with the risk of reaction (P = 0.85). With caveat number two of our limitations in mind: with 24 reactions in total, this study would not have seen even a doubling of the risk. "It is not a prohibition" is the correct reading; "it is safe" is not.
Where we disagree with the authors — or rather, where their caution should be moved. The work concludes on safety and puts TACO in the Discussion. We would put TACO in the first line, and for a reason found in their own data: the 6 respiratory reactions are the type of reaction with the worst outcome (5 of 6 non-survivors), they are concentrated in crias under one month (5 of 6), and they occur in animals receiving 35 mL/kg of plasma, i.e. more than the 15-25 mL/kg that the camelid source cited by the authors recommends. Six cases prove nothing on their own. But the chain — high volume, neonate, respiratory sign, death — is coherent, and it costs nothing to take it into account: auscultate before, auscultate midway, and if crackles or open-mouth breathing appear, stop the transfusion instead of attributing the sign to the underlying disease. It is the only operational recommendation we feel comfortable adding, and we state it for what it is: ours, not the authors', and based on a subgroup of six animals.
On crossmatching. A single crossmatch out of 204 transfusions, over fourteen years, at two university hospitals. Anyone in Italy who has felt guilty for not doing it can stop: no one does it, and those who published the numbers say why. But "it isn't done" is not "it isn't needed": no one has ever put it to the test in camelids, and this study is not the design that could do so.
Our operational position, for members. If plasma is needed for a cria and no commercial product is available, the donor the authors indicate — well-vaccinated castrated male or never-pregnant, never-transfused female of the same herd — is reasonable advice from people who have seen 204 transfusions, and should be used as such: advice, not a result. Always record in the chart the donor species, the volume in mL/kg and the start and end time: the three things missing in 27-45% of this study's records, and without which in ten years no one will be able to write the Italian version of this work.
A note on method. This is the first article in the Camelid Research Library without page numbers, because the journal has moved to the article number. The citation at the end writes
40(1):aalag008and not a made-up page range. It looks like a librarian's detail: it is the same gesture that, applied to clinical numbers, holds up everything else.23 · Star rating
Axis Score Rationale A · Scientific quality ★★★☆☆ Dual-center observational study with stated n (204 transfusions, 174 admissions), explicit selection criteria, operational definitions written out in full and replicable, a stated statistical model (GLIMMIX, logit), limitations discussed in a dedicated paragraph, funding, conflicts and institutional approval all declared. It does not rise to four because all analyses are univariate, sample size calculation is missing in a study with predominantly negative conclusions, confidence intervals for the null estimates are not published, and donor species is missing in 27% of transfusions. B · Clinical utility ★★★★☆ Result applicable with reasonable confidence to similar cases, with explicit conditions of use: dose in mL/kg for four age groups, route, rate, reaction rate and fatal anaphylaxis rate, all with denominator. This is the case where the grid allows B = A + 1: the doses are reported in full, as axis B requires, and they are doses administered to camelids. It is not five because it does not change a recommended practice — it confirms one — and because the availability of the products it rests on is not that of Europe. C · Educational value ★★★★☆ Teaches to define before counting: the definitions of transfusion, transfusion event and reaction are a small course in method, and are the reason this work will be replicable where its predecessors were not. It also teaches how to distinguish a reaction from the disease that brought the animal to the clinic. It is not five because the mechanism (TACO, alloimmunization) is cited from other literature, not demonstrated here. D · Relevance to the species ★★★★☆ Study on camelids, dual-center, with four distinct species and their stated breakdown (141 alpacas, 23 llamas, 5 guanacos, 5 camels), age groups, sex and length of hospitalization: the population is described enough to allow its transferability to be judged — which is what the fourth level of axis D requires. It is not five because the setting remains that of two US university hospitals with their own donor herds and commercial llama plasma, conditions that are not the norm in Europe. A = 3 and B = 4 are not a contradiction: a study that cannot demonstrate causal links still delivers the only doses and the only frequencies that exist on this topic in the species.
24 · Bottom line
It can be concluded that, in 204 transfusions performed at two US university hospitals between 2010 and 2024 in camelids of four species, plasma was the most used blood product (172 of 204, 84.3%) and failure of transfer of passive immunity with or without sepsis the most frequent indication (136 of 204); that the median intravenous plasma dose was 36 mL/kg in camelids ≤ 48 hours old and 35 mL/kg in those between 48 hours and one month, with median rate 10 mL/kg/h (mean 15); that 24 of 204 transfusions (12%) were followed by a reaction, that changes in vital parameters were its most common form (20 of 26 reactions, 77%) and that fatal anaphylaxis occurred in 1 of 204 transfusions (0.5%); that having a reaction is associated with a lower probability of being discharged alive (54.5% versus 76.5%, P = 0.033); and that no statistical association emerged between reaction risk and prior transfusion (P = 0.85), premedication (P = 0.62), product type (P = 0.51), age, species, sex or dose.
It cannot be concluded that premedication or crossmatching are not useful: the study is univariate, not adjusted for the severity of the underlying disease, does not calculate power and does not publish confidence intervals for the null estimates →
[TO BE CHECKED AGAINST THE FULL TEXT]; with 24 events in total, even a relevant effect would have gone unnoticed. It cannot be concluded that xenotransfusion is safer than, or as safe as, homologous transfusion: the comparison was not made, and in 56 of 204 transfusions (27%) the donor species is not documented. It cannot be concluded that the reported doses are the recommended doses: they are the doses administered, and the authors themselves note that in crias they are higher than the 15-25 mL/kg recommended by the camelid source they cite. No premedication dose can be derived: the article states which drug and how many times, never how much →[TO BE CHECKED AGAINST THE FULL TEXT]. Nothing can be concluded about stored blood products, because none was used in this case series, nor about delayed hemolytic reactions, which the study's monitoring was not able to detect.Camelid Research Studies APS — CRL editorial staff · 31/08/2026
