«Without specific CLSI breakpoints for SACs, those defined for cattle and humans were used».

◆ ASSOCIATION'S COMMENTARY
Camelid Research Studies APS — CRL editorial staff · 31/08/2026
Of 125 camelids on twelve farms in northern Italy, four shed ESBL-producing E. coli and only one of those four isolates was multidrug-resistant.
The first piece of news is good, and it is the one the article set out to find. Of 125 camelids on twelve farms in Northern Italy, four were shedding ESBL-producing E. coli in their feces, and only one of those four isolates was multidrug-resistant. This is a low frequency, lower than that of other European studies, and the authors' explanation is simple: on these farms antibiotics are almost never used — six of twelve farms used none at all, and had had no infectious diseases to report. Anyone who fears that hobby camelids are a hidden reservoir of resistance is, on this data, mistaken.
The second piece of news is in the questionnaire, and it is less comforting. On twelve Italian farms raising camelids — eleven of which use them for activities with people and five receive visitors from outside the owner's family — dedicated clothing and footwear are required of the veterinarian on two of twelve farms, and of no external visitor, on any farm. On eight of twelve farms camelids can come into contact with dogs, cats, poultry and sometimes wildlife, due to the absence of physical barriers. The authors also record among the shortcomings the absence of quarantine before introducing new animals — and ten of twelve farms purchase animals frequently.
This last juxtaposition is the one to keep. Ten of twelve farms purchase frequently, and quarantine is among the missing practices. Fact sheet CRL-I-01, in this same fascicle, tells what can travel through that gap: alpacas infected with Mycobacterium bovis, with paperwork in order and a negative intradermal test, arrived at a farm doing rehabilitation work with children. It is not a prediction: it happened, and it is published.
Be careful not to make the inference the article does not make. "Little biosecurity and little resistance" does not mean "biosecurity is not needed." First of all because the authors expressly declare that they did not statistically relate the questionnaire to the bacteriological results: the two measures coexist in the article without being linked. And also because biosecurity is not needed only against resistance: it is needed against tuberculosis, against chlamydiae, against Cryptosporidium in crias, against Coxiella at parturition. The risk it protects against is not the one this study measured. The authors themselves close by saying that resistance is present despite limited antimicrobial use, and that precisely for this reason surveillance and biosecurity are needed even on low-consumption farms.
The technical detail nobody says out loud, and that alone is worth reading the article for. «Without specific CLSI breakpoints for SACs, those defined for cattle and humans were used». In other words: no CLSI clinical breakpoints exist for South American camelids. When a laboratory hands you back an antibiogram on an alpaca with an "S" or an "R" written next to a molecule, that letter was decided against a breakpoint built for cattle or for humans, with their pharmacokinetics and their achievable concentrations, which in the camelid are not necessarily the same. This is not a reason to ignore the antibiogram — it remains the best tool we have. It is a reason not to read that letter as a certainty, and to prefer, where it exists, a pharmacokinetic value obtained in the species (for meloxicam in the llama, for example, it exists, and it is catalogued in CRL-B-03).
What changes for a veterinarian in Italy, today. (a) The three most common shortcomings are known, measured and fixable at almost no cost: quarantine on entry, a physical barrier against other species, dedicated clothing and footwear for those entering. None requires an investment; all three require a decision. (b) On a farm that does activities with people, hand gel at the exit from the animal area is not decoration: it is the measure with the highest cost/benefit ratio in this whole fascicle, and it applies to most of the oro-fecal agents in fact sheet CRL-I-02. (c) The treatment log should be kept even on a hobby farm. Half of these farms used no antibiotics, and this is a merit that becomes a fact only if it is written down somewhere.
What this fact sheet cannot tell you. Whether the sample's only farm doing animal-assisted therapy was among the three positive ones: the data is in the supplementary materials, which have not been opened. And nothing about Central and Southern Italy: this study is from the North, and the title says so honestly.

9 · Specie e soggetti
9 · Species and subjects
125 South American camelids: 106 alpacas (Vicugna pacos) and 19 llamas (Lama glama), aged between 2 months and 12 years, on 12 farms in Northern Italy.
Distribution of the farms, from Table 1 of the full text: Lombardy 6 (Bergamo ×3, Cremona, Lodi, Milan), Piedmont 3 (Turin ×2, Novara), Veneto 2 (Treviso ×2), Emilia-Romagna 1 (Piacenza). Number of animals sampled per farm from 1 to 21. Composition: 9 farms with alpacas only, 1 with llamas only, 2 with both species.
A clinical examination was performed on each animal, and species, breed, sex, age, health status and prior antimicrobial treatments were recorded. On each farm all camelids present were enrolled, not a subset: this is a choice that eliminates within-farm selection (but not between-farm selection — field 18).
Intended use of the animals, from the questionnaire (totals exceed 12 because a farm can have multiple uses): fiber production 5/12, landscape management 4/12, trekking 4/12, hobby 2/12, animal-assisted therapy 1/12. The authors summarize as follows: 11 of 12 farms raise camelids for activities with people, and 5 of 12 receive visitors from outside the owner's family.
10 · Domanda clinica (PICO)
10 · Clinical question (PICO)
- P — South American camelids raised on farms in Northern Italy.
- E — farm management and biosecurity practices, recorded by questionnaire.
- C — internal comparison across farms; no external control group.
- O — fecal carriage of ESBL-producing E. coli, with phenotypic characterization (double-disk synergy, antibiogram) and genotypic characterization (*bla*CTX-M, *bla*TEM, *bla*SHV).
As a question: how many camelids raised in Italy shed ESBL-producing E. coli in their feces, which genes do they carry, and what is the state of biosecurity on the farms that house them?
11 · Disegno dello studio
2008) e CROSS — Consensus-Based Checklist for Reporting of Survey Studies — per il questionario (citando Sharma e coll. 2021).
11 · Study design
Cross-sectional observational study, with a single fecal sample per animal and a structured questionnaire administered on site. The authors explicitly declare the reporting guidelines followed: STROBE for the observational part (citing von Elm et al. 2008) and CROSS — Consensus-Based Checklist for Reporting of Survey Studies — for the questionnaire (citing Sharma et al. 2021).
Each farm was visited only once.
12 · Level of evidence
Evidence from a cross-sectional observational study on a convenience sample, with sample size calculated a priori, a microbiological outcome confirmed by phenotypic and genotypic methods, and declared reporting guidelines.
Consequence for the reader: the carriers' frequency is well measured and can be used as an Italian benchmark. Associations between farm practices and carriage were not statistically analyzed — a limitation the authors declare — so everything that links biosecurity and resistance in this article is side-by-side description, not inference. Anyone who reads "little biosecurity and little resistance" and concludes "biosecurity is not needed" is drawing an inference the study does not contain, and that field 22 dismantles.
13 · Relevant reporting standard
STROBE (observational studies), declared by the authors in the text, citing von Elm et al.
- In addition, CROSS for the questionnaire part. Chapter 14 §3 does not list CROSS among the guidelines of its closed set: this fact sheet notes it as an author declaration, not an addition to the house list — if needed, it is added to the chapter 14 grid for all fact sheets (§11), not here.
That a camelid article declares two reporting guidelines is a rare event, and it should be noted.
14 · Metodo
Letto nel testo pieno.
Numerosità calcolata a priori.
Campioni. Feci prelevate dall'ampolla rettale di ogni animale arruolato, trasportate in laboratorio, conservate a 5 °C e processate entro 24 ore.
Microbiologia.
Antibiogramma. Metodo della diffusione da disco, pannello di nove molecole di sei classi: amoxicillina/acido clavulanico 30 µg · ampicillina 10 µg · ceftiofur 30 µg · enrofloxacina 5 µg · tetraciclina 30 µg · gentamicina 10 µg · cefazolina 30 µg · florfenicolo 30 µg · trimetoprim/sulfametossazolo 25 µg. Ceppi resistenti a tre o più classi classificati come multiresistenti (MDR) secondo Magiorakos e coll. (2012).
Testualmente: «Without specific CLSI breakpoints for SACs, those defined for cattle and humans were used».

Sample size calculated a priori. Starting from an expected prevalence of 31 % of ESBL E. coli (data taken from Gonzalez-Santamarina et al. 2022b), with a 95 % confidence level and absolute precision of 10 %, 82 camelids were needed; the formula used is that of Daniel (1999) for prevalence studies. 125 were enrolled.
Recruitment. Farms chosen among breeders registered with SNAEL (Società Nazionale Allevatori Alpaca e Lama): 34 registered members in total. For convenience, farms within 300 km of the University of Milan were chosen that responded to the request sent by email between 10 and 20 February 2024. All the farms contacted took part. Sampling from late February to 15 March 2024.
Questionnaire. Administered on site by the first author, in Italian, with an interview of the owner, after signature of a declaration of consent to sampling, to analysis for scientific purposes, and to confidentiality. 45 multiple-choice questions, in seven sections: (I) farm organization; (II) interactions with other animals and with people; (III) transport and quarantine; (IV) management of adults and reproduction; (V) health management; (VI) management of parturition and cria rearing; (VII) antimicrobial treatments. Built on a previous questionnaire (Gonzalez-Santamarina et al. 2022a) and for this reason not piloted — the authors declare this. Farm names replaced by numbers. Response rate calculated according to Dillman et al. (2014); missing data according to the Rubin (1987) framework.
Samples. Feces collected from the rectal ampulla of each enrolled animal, transported to the laboratory, stored at 5 °C and processed within 24 hours.
Microbiology. Culture on CHROMagar™ ESBL plates; at least three suspect colonies per sample identified with MALDI-TOF mass spectrometry (MBT Microflex LT/SH, Bruker); subculture on blood agar; phenotypic confirmation of ESBL production with the double-disk synergy test (DDST) according to EUCAST, using disks of ceftazidime 30 µg, cefotaxime 30 µg and amoxicillin/clavulanic acid 30 µg.
Antibiogram. Disk diffusion method, panel of nine molecules from six classes: amoxicillin/clavulanic acid 30 µg · ampicillin 10 µg · ceftiofur 30 µg · enrofloxacin 5 µg · tetracycline 30 µg · gentamicin 10 µg · cefazolin 30 µg · florfenicol 30 µg · trimethoprim/sulfamethoxazole 25 µg. Strains resistant to three or more classes classified as multidrug-resistant (MDR) according to Magiorakos et al. (2012).
⚠️ The methodological detail that alone justifies this whole fact sheet, and that is news to anyone who doesn't know it. Verbatim: «Without specific CLSI breakpoints for SACs, those defined for cattle and humans were used». No CLSI clinical breakpoints exist for South American camelids. Every antibiogram performed on an alpaca in Italy — including the one your laboratory will hand back to you tomorrow morning — is read against breakpoints derived from cattle or from humans. See field 22.
Genotyping. DNA extracted with the DNeasy Blood & Tissue kit (QIAGEN) with modifications from the second step onward (Cremonesi et al. 2006), quantified on the NanoDrop ND-1000; search for bla*CTX-M and bla*TEM by multiplex PCR and for *bla*SHV by standard PCR, according to Monstein et al. (2007); electrophoresis on 2 % agarose gel.
Ethics. Protocol reviewed and approved by the Institutional Animal Welfare/Care Committee of the Università degli Studi di Milano, protocol no. 51_2024; the study declares that it was conducted in accordance with the Declaration of Helsinki.
15 · Risultati (dati)
| Classe | Molecola | Esito sui 4 isolati |
|---|---|---|
| β-lattamici | ceftiofur | resistenti 4/4 (100 %) |
| β-lattamici | ampicillina | resistenti 3/4 (75 %) |
| β-lattamici | cefazolina | resistenti 3/4 (75 %) |
| β-lattamici | carbapenemi | sensibilità osservata in tutti gli isolati ESBL |
| chinoloni | enrofloxacina | sensibili 3/4 (75 %) |
| anfenicoli | florfenicolo | sensibili 4/4 (100 %) |
| aminoglicosidi | gentamicina | resistenti 2/4 (50 %) |
| tetracicline | tetraciclina | sensibili 3/4 (75 %) |
| sulfamidici | trimetoprim/sulfametossazolo | sensibili 4/4 (100 %) |
- 1 isolato (Lombardia, quello MDR): *bla*CTX-M e *bla*TEM.
| Ambito | Dato |
|---|---|
15 · Results (data)
Numbers only, from the full text, with explicit denominators.
Carriage
- 4 of 125 camelids (3.2 %) positive for ESBL-producing E. coli.
- Breakdown of the four: one adult alpaca and one cria from a farm in Piedmont; one adult alpaca from a farm in Lombardy; one cria from a farm in Emilia-Romagna. That is, 3 of 12 farms had at least one positive.
- The other isolates identified by MALDI-TOF belonged to other genera, most frequently Pseudomonas spp., Acinetobacter spp. and Achromobacter spp.
- ⚠️ No confidence interval is reported by the authors for the 3.2 % → field 22 for the editorial calculation.
Resistance phenotype of the four isolates
The denominator is four. In this field percentages are written only alongside the absolute number, because with n = 4 a percentage alone is misleading.
| Class | Molecule | Outcome across the 4 isolates |
|---|---|---|
| β-lactams | ceftiofur | resistant 4/4 (100 %) |
| β-lactams | ampicillin | resistant 3/4 (75 %) |
| β-lactams | cefazolin | resistant 3/4 (75 %) |
| β-lactams | carbapenems | susceptibility observed in all ESBL isolates |
| quinolones | enrofloxacin | susceptible 3/4 (75 %) |
| amphenicols | florfenicol | susceptible 4/4 (100 %) |
| aminoglycosides | gentamicin | resistant 2/4 (50 %) |
| tetracyclines | tetracycline | susceptible 3/4 (75 %) |
| sulfonamides | trimethoprim/sulfamethoxazole | susceptible 4/4 (100 %) |
- Three distinct resistance profiles among the four isolates. Only one is multidrug-resistant (MDR): the adult alpaca from Lombardy, resistant to ampicillin, cefazolin, ceftiofur, enrofloxacin and tetracycline.
- All four isolates suspect on culture were positive on DDST, confirming ESBL production.
Genotype
- 2 isolates (the two from Piedmont): only *bla*CTX-M.
- 1 isolate (Lombardy, the MDR one): *bla*CTX-M and *bla*TEM.
- 1 isolate (Emilia-Romagna): negative for all three genes tested, despite being phenotypically an ESBL producer.
- All four negative for *bla*SHV.
The questionnaire — 12 of 12 farms, 100 % response rate, no missing data
| Area | Data |
|---|---|
| Purchases and grouping | 10 of 12 farms purchase animals frequently and keep camelids in groups |
| Other species on the farm | 11 of 12 host other species; only 3 of 12 prevent their direct contact with camelids |
| Physical barriers | 8 of 12 allow interaction with dogs, cats, poultry and occasionally wildlife, due to the absence of physical barriers |
| Who handles the animals | family members 12/12 · visitors 5/12 · farm staff 2/12 · veterinarians 2/12 |
| ⚠️ Dedicated clothing and footwear | required of the veterinarian on 2 of 12 farms; of no external visitor, on any farm |
| Quarantine | the authors record among the shortcomings the absence of quarantine measures before introducing new animals |
| Reproduction | 3 of 12 do not breed the animals; 7 have in-house breeding programs; on 2 the program may involve bringing in a male or mating with other farms |
| Parturition | on the 9 farms that breed, parturition always takes place in a group; 6 of 9 supervise parturition; of these 6, only 5 check for malformations and disinfect the umbilical structures outside of birth; on the other 4 farms neither procedure is performed |
| Antimicrobials | 6 of 12 reported no cases of infectious disease and did not use antimicrobials; 6 of 12 used them, 3 in the previous year |
| Reasons for use | Candidatus Mycoplasma haemolamae on 2 of 3 farms (oxytetracycline) · diarrhea on 1 farm (third-generation cephalosporins) · dental or dermatological problems in 3 herds (penicillin, but not in the last year) |
⛔ No dosage number is written in this fact sheet for the drugs above. The article names oxytetracycline, third-generation cephalosporins and penicillin without dose, without route of administration and without duration: house rule is that a dosage is written only with dose + route + species + source on the same line. Three of four elements are missing here, so the number is not written. Anyone looking for camelid dosing will find verified ones in fascicle B.
Two juxtapositions the authors report, which remain juxtapositions
- 2 of the 3 farms with confirmed ESBL E. coli were dealing with or had recently dealt with cases of Candidatus M. haemolamae and had used oxytetracycline.
- The MDR strain came from a farm where penicillin had been used for severe pyoderma linked to sarcoptic mange.
⚠️ These are descriptive observations: no statistical test supports them, and the authors declare among the limitations precisely the absence of statistical analysis of the questionnaire responses and their correlation with the bacteriological results (field 17).
16 · Interpretazione degli autori
Attribuita esplicitamente e tenuta separata dal campo 15.
Gli autori inquadrano il lavoro nella crescita della popolazione di camelidi sudamericani in Europa e in Italia e nel loro nuovo impiego «for animal therapy, outdoor activities, and as companion animals», a fronte del quale «there is limited research on the transmission of microbes or antimicrobial resistance genes from SACs».
Sulla biosecurezza: «Several data from our questionnaire revealed a lack of some external biosecurity protocols, such as the absence of quarantine measures before introducing new animals and the frequent avoidance of dedicated clothing and footwear for visitors»; e ancora, sulla mancanza di barriere fisiche verso cani, gatti, pollame e fauna selvatica in 8 aziende su 12: «These gaps in biosecurity measures in the enrolled herds are a concern, as these inadequacies could facilitate the spread of AMR bacteria and genes».
Conclusione degli autori, testuale e da riportare per intero: «Despite the low biosecurity levels observed on the SAC farms, our findings showed a low occurrence of ESBL-E. coli and a low carriage rate of multidrug-resistant (MDR) ESBL-E. coli»; e, subito dopo, «the findings emphasize the presence of antimicrobial resistance regardless of limited antimicrobial use, underscoring the need for surveillance and biosecurity measures in low antimicrobial consumption farming».
16 · Authors' interpretation
Explicitly attributed and kept separate from field 15.
The authors frame the work within the growth of the South American camelid population in Europe and Italy and their new use «for animal therapy, outdoor activities, and as companion animals», against which «there is limited research on the transmission of microbes or antimicrobial resistance genes from SACs». They declare that, to their knowledge, this is the first study to assess the presence of ESBL E. coli from single fecal samples of South American camelids in Northern Italy.
On biosecurity: «Several data from our questionnaire revealed a lack of some external biosecurity protocols, such as the absence of quarantine measures before introducing new animals and the frequent avoidance of dedicated clothing and footwear for visitors»; and further, on the lack of physical barriers against dogs, cats, poultry and wildlife on 8 of 12 farms: «These gaps in biosecurity measures in the enrolled herds are a concern, as these inadequacies could facilitate the spread of AMR bacteria and genes».
On the low frequency of carriers: they attribute it to the low use of antimicrobials on these farms, particularly of β-lactams, and observe that the prevalence found is lower than that of other European studies. They note that, unlike what happens in cattle — where ESBL E. coli is more frequent in the feces of calves than of adults — here it was found in both adult alpacas and crias.
On the isolate negative for all three genes despite being phenotypically ESBL: they call it an unusual observation, and propose as an explanation other resistance mechanisms, such as porin mutations or efflux pumps, which can alter outer membrane permeability selectively against larger molecules such as third-generation cephalosporins, while allowing smaller ones through.
Authors' conclusion, verbatim and to be reported in full: «Despite the low biosecurity levels observed on the SAC farms, our findings showed a low occurrence of ESBL-E. coli and a low carriage rate of multidrug-resistant (MDR) ESBL-E. coli»; and immediately after, «the findings emphasize the presence of antimicrobial resistance regardless of limited antimicrobial use, underscoring the need for surveillance and biosecurity measures in low antimicrobial consumption farming».
The two sentences must be read together, because taken separately they appear to contradict each other: resistance is present even where antibiotics are almost never used, and precisely for this reason surveillance and biosecurity are needed. The authors are not saying "everything is fine."
17 · Limiti dichiarati dagli autori
17 · Limitations declared by the authors
Quoted, not summarized.
- Convenience sample, with numbers of herds and animals varying from region to region.
- Only the antimicrobial resistance of ESBL E. coli was tested, without genomic sequencing techniques.
- Absence of statistical analysis of the questionnaire responses and their correlation with the bacteriological results.
- The questionnaire was not piloted, because it was derived from a previous one (declared in the methods section).
18 · Limiti aggiunti dalla redazione
- Quattro isolati.
18 · Limitations added by the editorial team
- Four isolates. The entire phenotype and genotype of this study rest on four bacteria. "100 % resistant to ceftiofur" is four isolates out of four, and "50 % resistant to gentamicin" is two. The fact sheet always writes both numbers for this reason. Editorial calculation — Wilson, z = 1.96, not an authors' result: the 3.2 % carriage rate (4/125) has a 95 % confidence interval ranging from 1.3 % to 7.9 %.
- The selection of farms is restricted three times, and each restriction pulls in the same direction: (a) registered with SNAEL — i.e. organized breeders, presumably more attentive than average; (b) within 300 km of Milan; (c) that responded to an email within ten days. None of this is hidden — the authors write it — but the combined effect is that the 3.2 % is likely the lower bound of what would be found in a less selected population, not a central estimate.
- 34 SNAEL members in total, 12 enrolled. The reference population's denominator is small, and this is a fact about the sector before it is a fact about the study: organized camelid farming in Italy is still tiny.
- A single visit per farm, a single sample per animal. Shedding of ESBL E. coli is intermittent: a single collection underestimates carriers. The German study in fact sheet CRL-I-02, which sampled four times over two years, shows exactly this for other agents — and for STEC the rate per animal (42.3 %) is visibly higher than the rate per sample (32.4 %).
- The questionnaire describes, and is not linked to the outcome. This is the authors' limitation 3, and it bears repeating because it is the first thing that will be forgotten: this article does not demonstrate any relationship between biosecurity shortcomings and the four carriers. The two things sit in the same article and do not explain each other.
- The breakpoints are not species-specific (field 14). A camelid antibiogram read against cattle and human breakpoints is what there is, not what one would wish for: the authors declare this, and the fact sheet brings it to the fore because it has consequences for every Italian report, not just for this study.
- The sample size was calculated on an expected prevalence of 31 %, and an observed prevalence of 3.2 % was found. The a priori calculation was correct and should be commended; a posteriori, with a prevalence ten times lower, 125 animals give much less precision than what was designed. This is the fate of every sample size calculation that starts from a wrong hypothesis, and it is not the authors' fault: it is a reason not to use this 3.2 % as an estimate of known precision.

21 · What is missing relative to the reporting standard (STROBE, declared by the authors)
Assessment conducted on the full text as read: the absences below are real.
- The design is not named in the title or abstract (STROBE 1a): the title says fecal carriage and biosecurity practices, not cross-sectional study.
- No estimate with a confidence interval (STROBE 16a): the 3.2 % is a bare figure, as are all the antibiogram percentages.
- No analysis of potential confounders and no multivariable analysis (STROBE 12b, 16b) — consistent with the authors' limitation 3, but it is still what is missing.
- No quantification of selection bias (STROBE 9): the selection is described honestly and is not discussed as to the direction in which it acts.
- The participant flow diagram (STROBE 13a) is absent: how many farms were contacted, how many responded, how many excluded, can be worked out from the text with some effort (34 registered, those within 300 km contacted, all contacted participating) but it is not presented as such.
- Intended use is not related to the outcome: the only farm doing animal-assisted therapy is in the sample, and it is not known whether it is among the three positive ones. This is the most interesting piece of information for a reader of this fascicle, and it cannot be retrieved even from the full text →
[TO BE CHECKED AGAINST THE SUPPLEMENTARY MATERIALS](the text refers to Supplementary File 3 and 4, which this fact sheet has not opened). - Not missing, and this should be acknowledged with emphasis: two declared reporting guidelines, a priori sample size calculation with the formula cited, ethical approval with a protocol number, item-by-item author contributions, declared conflicts and funding, questionnaire response rate reported with the calculation method, limitations declared specifically and not as a formality. On these points the article stands above almost all the camelid literature that CRL has catalogued so far.
23 · Valutazione a stelle
| Asse | Voto | Motivazione, con un fatto dentro |
|---|---|---|
23 · Star rating
| Axis | Score | Rationale, with one fact inside |
|---|---|---|
| A — Scientific quality | ★★★☆☆ | Observational study with a declared and a priori calculated sample size (82 required, 125 enrolled), explicit selection criteria down to the mileage and the email dates, baseline analysis, two declared reporting guidelines (STROBE and CROSS), ethical approval with a protocol number, CRediT contributions, declared funding and conflicts, specific limitations. This is the top of its class: the chapter 14 §11 grid reserves four stars for prospective controlled studies, RCTs and experimental studies compliant with ARRIVE, and a cross-sectional survey — however well done — is none of those things. The score is not a criticism of the article: it is the category's ceiling, and the article sits comfortably within it. (§11: if the criterion is not enough to distinguish an excellent survey from a mediocre one, the grid is corrected for all fact sheets, not this one.) |
| B — Clinical usefulness | ★★★☆☆ | Offers a replicable procedure and three immediate, near-zero-cost management interventions (quarantine, barriers, dedicated clothing), plus an operational warning about interpreting any camelid antibiogram. It does not rise to four because the study does not link practices to outcome and because four isolates do not support a prediction. (Constraint §11 respected: B does not exceed A.) |
| C — Educational value | ★★★★☆ | Useful teaching material, on two levels: it teaches how to read an antibiogram knowing which breakpoints it is built on, and it provides a numbered snapshot of the real biosecurity of Italian farms that can be projected in a classroom without comment. It does not reach five because it does not explain a mechanism — the only mechanistic hint, on porins and efflux pumps, is a discussion hypothesis. |
| D — Relevance to the species | ★★★★☆ | Italian, multicenter study (12 farms in 4 regions), on both species, with the population described enough (region, province, species, number of head per farm, age from 2 months to 12 years, intended use) to assess transferability — which for an Italian reader is maximal. It does not reach five because it is recent (January 2025) and still little cited (2 citations recorded in Crossref, 1 in Europe PMC as of 31/08/2026), and because it covers only Northern Italy. |
24 · Bottom line
24 · Bottom line
It can be concluded that: on twelve farms in Northern Italy, 4 of 125 camelids (3.2 %) were shedding ESBL-producing E. coli in their feces, with only one multidrug-resistant isolate out of four and with *bla*CTX-M as the prevailing gene (3 of 4 isolates); that on these farms antimicrobial use is low or nil (6 of 12 farms used none); and that external biosecurity practices are documented as deficient — dedicated clothing and footwear required of the veterinarian on 2 of 12 farms and of no visitor, absence of barriers against other species on 8 of 12, quarantine absent from current practices while 10 of 12 purchase animals frequently.
It cannot be concluded that: poor biosecurity is irrelevant because resistance is low — the authors did not statistically relate the two measures, and they declare this, and the risks biosecurity protects against are not the ones measured here. Nor can the resistance profile be used as a therapeutic guide: these are four isolates, read against breakpoints that for camelids do not exist.
