Che cosa misurano i quattro assi
- A · Qualità scientifica
- La domanda è ben posta? I metodi sono dichiarati? La statistica è appropriata? La conclusione sta dentro i dati?
- B · Utilità clinica
- Serve a chi ha l’animale davanti, oggi.
- C · Valore didattico
What the four axes measure
- A · Scientific quality
- Is the question well posed? Are the methods stated? Is the statistics appropriate? Does the conclusion stay within the data?
- B · Clinical usefulness
- Useful to whoever has the animal in front of them, today.
- C · Teaching value
- Does it teach something to those studying the subject?
- D · Relevance to the species
- Is it really about camelids, or a result borrowed from another species?
Each axis is worth 1 to 5 stars and carries its written reason. The stars in the index are the rounded average of the four.

Un'alpaca. Nessun controllo.

◆ ASSOCIATION COMMENTARY
Camelid Research Studies APS — CRL editorial team · 31/08/2026
One alpaca. No control. A therapeutic bundle in which fluralaner is the last of four interventions — and the authors write the exact opposite of "it works." One alpaca. No control. A therapeutic bundle in which fluralaner is the last of four interventions. Anyone citing this as "fluralaner works in alpacas" is citing something the article does not say — the authors write the exact opposite: field studies at the herd level are needed.
That said, this is the record an Italian veterinarian will reach for first, and for a regulatory, not scientific, reason. The article contains two sentences that, in Italy, are worth more than any dose-response curve: "in Italy products containing amitraz are no longer available", and "medications registered specifically for camelids are not available in Italy". The most effective combination documented in the literature for alpaca sarcoptic mange — topical amitraz plus repeated ivermectin — cannot be carried out in full in Italy. Not because it is wrong: because one of the two molecules is not there. This case originates there, and the authors state it: "amitraz was not an option because the compound was not available in Italy."
The value of the case lies in what is documented before the fluralaner, not after. The scraping still positive fifteen days after the second ivermectin is proof, not assertion, that that protocol was failing in that animal. This is a rare piece of data: almost all case reports reporting success with the new molecule assert the failure of the old one without measuring it. Here it is measured. And that is the reason this record exists, more than the final success.
Watch out for the time error, which the reference chapter has already made. "Within one week" the pruritus had disappeared, the eye had reopened, appetite had returned. The skin lesions healed at one month, and the first negative scraping is at one month: at fifteen days it was still positive. In a zoonotic disease, where the operational decision is when do I lift isolation, the difference between seven and thirty days is not an editorial nuance: it is the period during which whoever handles the animal can become infected. The rapid clinical improvement is real and should not be diminished — but it is not negativization.
The most useful epidemiological fact is in the first line of the story. The alpaca started healthy, went to breeding at a facility "whose health status was unknown" — six alpacas — and returned with mange. No other animal in the herd of origin showed signs of mange (the article does not say whether they were examined). External breeding is a health-relevant movement, and in Italy it is a very frequent movement in small amateur herds that do not keep a male. It applies, on a smaller scale, everything that is said about purchasing: health status of the host facility, quarantine on return, skin examination before reintroducing into the group. It costs nothing, and in this case would have saved an abortion and three months.
What changes for a veterinarian in Italy, today. Three things. (a) External breeding is treated as an introduction: health status is requested, isolation occurs on return, the skin is examined. The risk, in this case, did not enter with a purchased animal: it came back with the owner's own animal. (b) If an isoxazoline is used, it is used knowing what is not known. There are no pharmacokinetic data in South American camelids; the dose comes from a wild sheep; the product is off-label. This does not make it unusable — it makes owner informed consent, documentation of the choice, and surveillance mandatory. The oral dose works and the topical one does not, in the species from which the dose is taken: if someone uses it, use it orally. (c) Parasitological control is done at one month, and repeated. Seven days of well-being are not a negativization, and in this case they were not even at fifteen.
What this record cannot tell you. Whether fluralaner works in alpacas. One case does not demonstrate it, and the authors do not claim it does. It cannot tell you the correct posology for the species, because no one has titrated it; it cannot tell you anything about safety in pregnancy — the only animal treated had already aborted beforehand; it cannot tell you the withdrawal period. And it cannot tell you what happened to the other sixteen animals in the herd (15 females and 2 crias, the case alpaca included: 17 - 1), because the article does not report it.

9 · Specie e soggetti
9 · Species and subjects
One alpaca (Vicugna pacos), female, 2 years old, pregnant at the start of the case.
- Herd of origin: 15 female alpacas and 2 cria, kept by a private owner for fiber, in an outdoor paddock with access to covered shelter. Northern Italy.
- Exposure: in June 2022 the alpaca was temporarily transferred to another facility for breeding, and returned in September 2022. The breeding facility's herd numbered 6 alpacas.
- On admission (5 November 2022): alert, normal cardiac and respiratory parameters, preserved organ function; weight 65 kg, BCS 2.5 out of 5 (Van Saun 2003 scale).
- No other animal in the herd of origin showed clinical signs of mange.
- ⚠️ It is not stated whether the rest of the herd — nor the 6 alpacas at the breeding facility — were examined or treated → to be checked against the full text (the article does not report it). In a disease that "spreads rapidly within herds," as the authors themselves write, this is missing information.
No llama, no Old World camelid. n = 1.
10 · Domanda clinica (PICO)
10 · Clinical question (PICO)
In an alpaca with confirmed sarcoptic mange, non-responsive to ivermectin [P], does the administration of fluralaner 5 mg/kg per os as a single dose [I], compared with continuing therapy with macrocyclic lactones [C], produce clinical resolution and negativization of skin scrapings [O]?
⚠️ The comparator [C] does not exist as a study arm: it is the therapy that had already failed in the same animal. The design is pre-post in a single subject, which makes the question formulable but not answerable with this material (field 12).
11 · Disegno dello studio
11 · Study design
Single case report, with morphological and molecular parasitological confirmation, dated timeline, and 3-month follow-up with serial scrapings — extended to 7 months for the fiber outcome alone.
12 · Level of evidence
Single case report: the lowest tier of the hierarchy among designs reporting original data. For the reader this means, without qualification: n = 1; no control; no blinding; no way to distinguish the drug's effect from natural progression, nutritional support, resolution of bacterial superinfection, or removal from the source of infestation; no generalizable safety data.
Three elements, however, place it in the upper range of its kind and should be credited: (a) the timeline is dated to the day, not narrated; (b) the agent is molecularly confirmed with a sequence deposited in a public database; (c) the failure of the previous treatment is documented by a test, not merely asserted — a positive scraping fifteen days after the second ivermectin dose.
13 · Relevant reporting standard
CARE (CAse REport guidelines) is the relevant standard. The authors do not declare it, and field 21 lists item by item what is missing. It should be said at once that, without declaring it, the article satisfies an unusually large part of it: timeline, interventions with named products and doses, outcomes measured with multiple tools, owner's informed consent and ethical approval.
14 · Metodo
Parassitologia. Acari identificati morfologicamente come Sarcoptes scabiei.
Molecolare.
| Farmaco e prodotto | Dose e via | Specie | Marcatura | Fonte della scelta, dichiarata dagli autori |
|---|---|---|---|---|
14 · Method
Parasitology. Deep skin scrapings performed at the first hospital examination, at the margin of each lesion on the head, thigh, and perineal region. Material transferred to a Petri dish with 10% KOH solution, digestion for at least 2-3 hours at room temperature, inspection under stereomicroscope. Mites identified morphologically as Sarcoptes scabiei.
Molecular. DNA extraction from a portion of the scrapings (DNeasy Blood and Tissue, Qiagen); PCR amplification of a ~400 bp fragment of the cytochrome c oxidase I gene (coxI); bidirectional Sanger sequencing; sequence deposited as OR668927 and subjected to BLAST, with 99-100% match to S. scabiei sequences in GenBank.
Therapy — the three molecules, with dose, route, species and source.
| Drug and product | Dose and route | Species | Labeling | Source of the choice, as declared by the authors |
|---|---|---|---|---|
| Ivermectin (Ivomec, Boehringer Ingelheim Animal Health, Italia) — first administration, by the attending field veterinarian | 0.2 mg/kg; ⚠️ route not stated by the article for this first administration → to be checked against the full text | alpaca | [EXTRAPOLATED DOSE] | not stated for the first administration |
| Ivermectin (same product) — second administration, in hospital | 0.2 mg/kg, subcutaneously | alpaca | [EXTRAPOLATED DOSE] | Twomey et al. 2009 |
| Procaine penicillin (Prontocill, ATI, Italia) | 20,000 IU/kg, subcutaneously, twice daily for 7 days | alpaca | [EXTRAPOLATED DOSE] | D'Alterio 2006 |
| Fluralaner (Exzolt, MSD Animal Health S.r.l., Italia), oral solution | 5 mg/kg, per os, single dose | alpaca | [EXTRAPOLATED DOSE — from another species] | Hering 2020, in bighorn sheep (Ovis canadensis), for psoroptic mange. ⚠️ Not a camelid. |
Plus: weekly washes with chlorhexidine shampoo, and supportive care during hospitalization.
⚠️ The fluralaner dose comes from a different species, and the authors state this. The rationale is textual: in bighorn sheep (Hering 2020) fluralaner had been trialed at 5 and 25 mg/kg per os and at 5 and 10 mg/kg topically; both oral doses had given satisfactory results, while topical use had had no effect on the mites. Hence the choice of the oral route and the lowest effective dose. No titration in the camelid exists, and the authors write so in the discussion: "no pharmacodynamic/pharmacokinetic studies exist for the use of isoxazolines in South American camelids".
⚠️ On the product: the article names Exzolt (MSD Animal Health S.r.l., Italia) and describes it as "another drug registered in Italy for the treatment of sarcoptic mange in other species". The article does not state the product's registered target species, nor its concentration, nor the volume administered → to be checked against the full text (the article does not report it). Anyone attempting to replicate this must read the package insert, not this record.

I numeri, riassunti.

| +7 months from fluralaner (maggio 2023) | shearing and fiber analysis at an external laboratory | fiber quality corresponding to that of the previous year, when there was no infestation |
The numbers, summarized. Weight: 65 kg on admission → 56 kg at fluralaner administration → 62 kg at one month → 70 kg at three months. BCS: 2.5 → 1.5 → 2.5 → 3 out of 5. Scrapings after fluralaner: positive at 15 days, negative at 1 month, and negative in all fortnightly checks over the following two months.
What had failed before, according to the literature reported by the authors — useful because it frames the case, and each item has its own source:
- Repeated ivermectin 0.2 mg/kg s.c.: reported as effective in the alpaca, "but requiring at least eight repeated administrations" (Twomey et al. 2009).
- Doramectin, ivermectin, amitraz or diazinon in three doses: in one study three alpacas were euthanized, one died, and the others did not respond (Beck 2020).
- Topical treatments with doramectin, eprinomectin or moxidectin: limited success in the alpaca (Hunter et al. 2004; D'Alterio et al. 2005; Pollock et al. 2017).
- Topical amitraz: effective in three alpacas in the United Kingdom infested with S. scabiei and treated unsuccessfully with repeated topical eprinomectin (Lau et al. 2007).
- Topical amitraz + ivermectin 0.5 mg/kg s.c. for nine consecutive weeks: curative in one alpaca with mixed sarcoptic and chorioptic infestation (Castilla-Castaño et al. 2021).
The two physical explanations for the failure of topical treatments, reported by the authors: (a) the absence of lanolin in camelid fleece, which limits the efficacy of topical acaricides formulated for sheep and cattle; (b) hyperkeratosis, a lesion typical of alpacas with sarcoptic mange, which "may prevent the penetration and absorption of topical compounds."
And the Italian regulatory constraint, stated twice by the authors: "in Italy products containing amitraz are no longer available" and "medications registered specifically for camelids are not available in Italy and are rare elsewhere; treatment in alpacas is therefore often performed off-label."
16 · Authors' interpretation
The authors conclude that "a single oral administration proved effective in treating the mange, leading to complete recovery of the animal during the 3-month observation period," and that fluralaner at 5 mg/kg is "effective without the need for repeated treatments, highlighting how treatment with this molecule is more economically sustainable and less time-consuming compared with other protocols."
The authors hypothesize that the infestation occurred "with the animal's move to another herd, whose health status was unknown," and that "transfer stress and dietary changes could exacerbate ectoparasite infestation."
The authors state the limitation of their own work, and do so at length: "however, this is a single clinical case, without existing pharmacodynamic/pharmacokinetic studies for the use of isoxazolines in South American camelids. Further field studies, especially at the herd level, are needed to confirm the efficacy of the therapeutic protocol employed."
The authors note that sarcoptic mange "is a significant zoonotic disease" — which is the reason the animal was hospitalized in isolation.
17 · Limiti dichiarati dagli autori
17 · Limits declared by the authors
Quoted verbatim in field 16, and there are three: n = 1; absence of pharmacokinetic and pharmacodynamic data of isoxazolines in South American camelids; need for field studies at the herd level to confirm the protocol.
18 · Limits added by the editorial team
- During hospitalization the animal aborted and lost 9 kg out of 65. The article reports this — correctly — but does not discuss any link between the abortion and the treatments administered, nor attributes it to the disease. The abortion occurred before the fluralaner, during ivermectin and penicillin treatment, and this must be stated so as never to attribute it to fluralaner. But it must also be said that the link was not investigated in any direction → to be checked against the full text: the article does not report fetal examination nor investigation into the causes of the abortion.
- Fluralaner was not administered alone. At the time of administration, the bacterial superinfection had already been resolved (with penicillin) and weekly cleansing with chlorhexidine was ongoing; the animal had been hospitalized, fed, and removed from the source of infestation for three weeks according to the text of the article (but admission was on 5 November and the pre-treatment photograph is dated 5 December, 30 days later: the article does not reconcile the two statements). The effect attributed to the molecule is the effect of a bundle.
- The first negative check comes at one month, not one week. The scraping at 15 days from fluralaner was still positive. The improvement at one week is clinical (pruritus, appetite, eye opening), not parasitological. The distinction matters for anyone who must decide when to lift isolation in a zoonotic disease.
- No plasma concentration measurement, consistent with the absence of pharmacokinetic studies that the authors themselves lament: it is unknown what exposure produced that effect in a camelid.
- No data on the rest of the herd (field 9), in a disease the authors themselves describe as rapidly spreading and zoonotic.
- No formal safety evaluation: the absence of observed adverse effects is not absence of adverse effects, with n = 1 and in an animal that was already unwell.
- The withdrawal period is not discussed. The alpaca is raised for fiber and does not enter the food chain, which makes the point less pressing; but in a camelid intended otherwise, an off-label isoxazoline poses a problem that this article does not address.
⛔ Two corrections to chapter §6.2. The chapter summarizes as follows: "treated by the practitioner with ivermectin 0.2 mg/kg s.c. with no improvement; then fluralaner 5 mg/kg per os as a single dose: within one week, resolution of pruritus, healing of skin lesions and recovery of appetite." Reading the full text shows that: (a) the ivermectin administrations were two, and the subcutaneous route is stated only for the second; for the first the article does not state the route; (b) at one week from fluralaner the skin lesions had not healed — the lip lesions had "improved," and healing of the skin lesions is documented at one month; the chapter anticipates an outcome by three weeks; (c) the chapter omits that the scraping at 15 days from fluralaner was still positive, and omits the abortion and the concurrent procaine penicillin. None of this is an error in the article: these are compressions of the summary. But compressing "at one month" into "within one week" is precisely what turns a case report into a promise. The chapter must be corrected.
19 · Finanziamento
Dichiarato testualmente: «Open access funding provided by Università degli Studi di Milano within the CRUI-CARE Agreement. The authors declare that no funds, grants, or other support were received during the preparation of this manuscript.»
Ruolo dello sponsor: nessuno.
19 · Funding
Stated verbatim: «Open access funding provided by Università degli Studi di Milano within the CRUI-CARE Agreement. The authors declare that no funds, grants, or other support were received during the preparation of this manuscript.»
Sponsor's role: none. The only support is coverage of open access publication costs through the CRUI-CARE agreement. No industry funding, which, in an article reporting the favorable outcome of a commercial product named by brand, is an element to record and not take for granted.
20 · Conflitti di interesse
«The authors declare no competing interests» — dichiarazione testuale nelle Declarations.
20 · Conflicts of interest
«The authors declare no competing interests» — verbatim statement in the Declarations. Both affiliations are academic (Pisa and Milan); no author is affiliated with MSD Animal Health nor any other manufacturer.
Ethics statement, reported in full because it is unusually complete for a case report: "the animal involved in the study was treated in accordance with professional ethics for veterinarians, and with the owner's written informed consent, who also authorized the use of material obtained during hospitalization for research purposes. Furthermore, the authors obtained approval from the ethics committee of the University of Milan for the publication of data resulting from clinical activities (approval number 47/2017, 28 November 2017)."
21 · What is missing relative to the reporting standard (CARE)
Bulleted list of what CARE would require and is not present:
- No statement of adherence to CARE (the article does not name the standard).
- Remote history and patient information (item 4): incomplete — vaccination history, antiparasitic treatments prior to ivermectin, reproductive history, and pregnancy status at admission are not reported, except implicitly.
- Differential diagnosis (item 7): not discussed. With Chorioptes and Psoroptes named in the introduction as the two other genera infesting camelids, one would expect a line on how they were excluded; the answer is implicit in the morphology and PCR, but is not written.
- Patient/owner perspective (item 12): absent (CARE requires it; in veterinary medicine it is a frequently omitted item).
- Timeline diagram: present (Figure 4), and this is to its credit.
- Adverse effects and unanticipated interventions (item 10): not reported as either present or absent, except for the abortion, which is described as a clinical event without attribution.
- Therapeutic rationale and changes (item 9): present and well written — the reason for the change of molecule, the reason for the oral route and the reason for the dose are all made explicit with a source.
23 · Valutazione a stelle
| Asse | Voto | Motivazione, con un fatto dentro |
|---|---|---|
23 · Star rating
| Axis | Score | Rationale, with one fact inside |
|---|---|---|
| A — Scientific quality | ★★☆☆☆ | Single case report with clear timeline dated to the day, agent confirmed morphologically and molecularly with a deposited sequence (OR668927), failure of the previous treatment documented by a test and not merely asserted, ethics statement with approval number, funding and conflicts declared. But without a comparator and without statistical analysis, and with the differential diagnosis undiscussed: this is exactly the two-star definition of chapter 14 §11, and it cannot be higher with n = 1. |
| B — Clinical utility | ★★★☆☆ | Offers a replicable procedure described in sufficient detail — molecule, dose, route, product, check-up timing, measured outcomes — to be used with the caution imposed by the level of evidence, which here is minimal. It does not reach four because "applicable with reasonable confidence in similar cases" requires more than one case, and because the dose is taken from another species. (Constraint of chapter 14 §11 respected: B exceeds A by one star only. And the score is not based on doses the article fails to report: it reports all of them.) |
| C — Educational value | ★★★★☆ | Teaches three things through a real case: how to document a therapeutic failure instead of merely asserting it; the difference between clinical response and parasitological negativization, with dates to prove it; and how a national regulatory constraint enters a clinical decision (the amitraz that is not available in Italy). It does not reach five because the differential diagnosis — the reasoning that leads from "pruritus and crusts" to Sarcoptes, excluding Chorioptes and Psoroptes — is not written, and this is precisely the step a student should see. |
| D — Species relevance | ★★★★★ | Case in an alpaca, in Italy, in an amateur fiber herd of 15 females and 2 cria — the numerically dominant form of husbandry in the country —, with drugs available on the Italian market and with the Italian regulatory constraints explicitly discussed. There is no record in the collection with more direct transferability to the national context. |
24 · Bottom line
24 · Bottom line
It can be concluded that: in one alpaca with sarcoptic mange confirmed morphologically and by PCR (coxI, GenBank OR668927), non-responsive to two administrations of ivermectin 0.2 mg/kg — with the scraping still positive 15 days after the second —, administration of fluralaner 5 mg/kg per os as a single dose was followed by resolution of pruritus and recovery of appetite at one week, first negative scraping at one month, negative scrapings at all fortnightly checks over the following two months, complete healing of lesions at three months and weight recovery from 56 to 70 kg (BCS from 1.5 to 3 out of 5); and that at seven months fiber quality was comparable to that of the year prior to the infestation.
It cannot be concluded that: fluralaner is effective in alpaca sarcoptic mange. This is a single case, the drug was administered after three weeks of hospitalization (according to the text; the dates in the article, 5 November and 5 December, say one month), antibiotic therapy, weekly cleansing and removal from the source, the dose comes from bighorn sheep and has never been titrated in a camelid, and no pharmacokinetic studies of isoxazolines exist in South American camelids — the authors say so. - CRL-L-01 (questo fascicolo) — Hinney e coll. 2024.
Treatment of sarcoptic and chorioptic mange in an alpaca (Vicugna pacos) herd with a combination of topical amitraz and subcutaneous ivermectin.

- CRL-L-01 (this issue) — Hinney et al. 2024. The link is an operational warning that neither record contains on its own. In L-01 ivermectin administered for a Psoroptes infestation selected for resistant nematodes, and the authors recommend a fecal egg count after every ectoparasitic treatment with an endectocide. In L-04 an alpaca receives two ivermectin doses for mange. The two administrations in L-04 are also two anthelmintic treatments that no one decided on, and no one counted eggs afterward. Anyone reading the two records together finds the recommendation and the case in which to apply it.
- CRL-L-03 (this issue) — Dubey 2018: transfer as a moment of risk, in two different parasitoses.
- Verified context but not recorded, and it is the mandatory comparison for anyone who must choose a protocol: Castilla-Castaño E, Herman N, Martinelli E, Lecru LA, Pressanti C, Schelcher F, Cadiergues MC. Treatment of sarcoptic and chorioptic mange in an alpaca (Vicugna pacos) herd with a combination of topical amitraz and subcutaneous ivermectin. N Z Vet J 2021;69(2):121-126, DOI 10.1080/00480169.2020.1808544 (record from Crossref, 31/08/2026; full text not accessible). It is the protocol that works and that cannot be carried out in full in Italy, because amitraz is not available (field 15). Sala et al. cite it as "the combination with which the best results were obtained."
