◆ COMMENTO DELL'ASSOCIAZIONE
Camelid Research Studies APS — redazione CRL · 31/08/2026

◆ ASSOCIATION'S COMMENT
Camelid Research Studies APS — CRL editorial team · 31/08/2026
In 300 camelids at a European university clinic, the correlation between FAMACHA© and BCS holds in the llama and frays in the alpaca. And the answer, for the first part, is no, with a sharpness that surprises: r = −0.73 in the llama, r = −0.34 in the alpaca. The same tool, the same examiners, the same clinic, the same seven years. In the llama, conjunctival colour explains a good part of the variation in haematocrit; in the alpaca it explains little. And it is not an isolated case confined to FAMACHA© alone: four associations present in the llama do not exist at all in the alpaca — score and eosinophils, score and weight, BCS and lymphocytes, BCS and neutrophils; the one between BCS and weight is present in both, weaker in the alpaca. Anyone who treats «South American camelids» as a single clinical category is averaging two animals that respond differently to the same tools.
Why the FAMACHA© calibrated on sheep does not transfer: three reasons, and none of them is an opinion. First, the chart is not calibrated on this species: the authors write that «a colour chart specific to South American camelids is lacking», and indeed in this study the chart was not even used in routine practice. Second, the validation context is another world: Storey et al. validated the system where H. contortus was the predominant nematode in 17 of 21 farms, in the south-eastern United States. In Europe the counts are low and the species mixed (→ CRL-L-01: medians between 5 and 60 eggs per gram). A triage tool for haemonchosis-related anaemia is all the more useful the more the anaemia is due to haemonchosis. Third, the conjunctiva lies in two directions: it reddens from local inflammation — and in this study the score was not assigned to those animals, which in the field nobody does — and it pales from circulatory centralisation in an animal that is not anaemic.
And here is the Storey datum that must be looked at squarely, because it is the one nobody cites. The US validation, reported by these authors, gives for a score ≥ 4 with PCV ≤ 0.17 L/L a sensitivity of 50 %. Half. Half of the anaemic animals, in a population where haemonchosis dominates, are not caught by a score ≥ 4. Specificity is excellent (94 %), which means that when the score is high it can be trusted; but when it is low, nothing has been ruled out. It is the profile of a test to be used for finding, never for excluding — and the error to avoid, on a farm, is exactly the second one.
The good news is that there is a better tool, and it is as free as the other one: hands. Two sentences from the article together are worth a protocol: all animals with PCV < 0.10 L/L had BCS ≤ 3, and none with BCS > 3 had severe anaemia. In a clinical population of 300 camelids, no exception. It is not a sensitivity and it is not a specificity — it is an observation without a counterfactual — but it says that an animal in good body condition was not, in this case series, an animal with severe anaemia. BCS is palpated on the lumbar spine, does not require daylight, is not confounded by conjunctivitis, and in the alpaca correlates with PCV about as well as the conjunctival score (r = +0.29 versus −0.34). In the alpaca, the two tools are equivalent and should be used together; in the llama the conjunctival score is markedly more informative.
The most useful thing this article says about owners. Fewer than 39 % of 255 German farms reported emaciation in their own animals, and only 13.3 % reported anaemia. In the clinic, 60-70 % had BCS < 3 and about half were anaemic. The two populations are not comparable and the authors say so — but the reason for the gap is physical and holds everywhere: the fleece covers everything, and whoever does not palpate does not see. An emaciated alpaca has the same profile as an alpaca in shape. This is the sentence to say to the owner, and it is the reason why the 4-6 week cadence of BCS and conjunctival score (→ CRL-L-02) is not bureaucracy: it is the only moment when someone puts hands on the animal.
What changes for a veterinarian in Italy, today. Four things. (a) The conjunctival score is used to find, never to exclude. High score: investigate further. Low score: nothing has been ruled out, with a sensitivity of 50 % at the ≥ 4 threshold in the population where it works best. (b) In the alpaca, BCS is always palpated, and it is not replaced by eye colour. In the alpaca the correlation with PCV is weak; BCS adds independent information, and in this case series no animal in good condition had severe anaemia. (c) If the conjunctiva is reddened, the score is not assigned. This is what the authors did in the clinic and what almost nobody does in the field: conjunctivitis makes an animal at 0.08 haematocrit score 1. (d) The raw score does not decide a deworming on its own. At minimum, an egg count is needed — which in this study is missing, and whose absence the authors acknowledge as the central limitation. And it is necessary to know whether the molecule still works (→ CRL-L-01).
What this record cannot tell you. The diagnostic performance of FAMACHA© used correctly, with the chart, in a European farm population. It does not exist: this study measures a subjective impression in a clinical population, and the only validation with the chart is US, in a context of dominant haemonchosis, and it is not accessible in full text. A European field study with the chart, with egg counts and with larval culture — that is, with the parasitological variable that is missing here — is the second fillable gap that this issue flags, after the one in CRL-L-01. And it cannot tell you anything about llamas with the same solidity it has for alpacas: there are forty-one of them.

⛔ A documented trap, and it must be read because it is the exact mechanism by which invented authors are manufactured. The
contrib-groupof the JATS XML deposited in PubMed Central for this article contains, after the five authors, two further names: García María-Luz and Argente María-José. They are not authors: they are the MDPI academic editors of the article, and the XML places them in the same block with no affiliation and no cross-references. Crossref correctly lists five. Anyone building the citation from the PMC XML alone would write a list of seven authors — two of whom did not write a line of this article. The house directive («for authors' NAMES use Crossref») is not a style preference: here it prevents a concrete error, and this record states it so that it may serve the next ones.

9 · Specie e soggetti
300 camelidi sudamericani presentati alla clinica: 259 alpaca (Vicugna pacos) e 41 lama (Lama glama). Nessun dromedario, nessun battriano.
| Gruppo | n |
|---|---|
| Alpaca, tutti | 259 |
| — maschi cria | 26 |
| — maschi adulti | 87 |
| — femmine cria | 40 |
| — femmine adulte | 106 |
| Lama, tutti | 41 |
| — maschi cria | 2 |
| — maschi adulti | 21 |
| — femmine cria | 2 |
| — femmine adulte | 16 |
9 · Species and subjects
300 South American camelids presented to the clinic: 259 alpacas (Vicugna pacos) and 41 llamas (Lama glama). No dromedaries, no Bactrians.
Full breakdown across the ten groups declared by the authors (cria = under 1 year; adult = over 1 year):
| Group | n |
|---|---|
| Alpacas, all | 259 |
| — cria males | 26 |
| — adult males | 87 |
| — cria females | 40 |
| — adult females | 106 |
| Llamas, all | 41 |
| — cria males | 2 |
| — adult males | 21 |
| — cria females | 2 |
| — adult females | 16 |
⚠️ Two groups have n = 2. The authors draw the correct conclusion and state it: «no data on the influence of age is available for llamas, since the young llama group contained only two animals». Anyone reading the tables must know that all conclusions on llamas rest on 41 animals, and all those on adult llamas on 37.
Context and period. Case records of the Clinic for Swine, Small Ruminants, Forensic Medicine and Ambulatory Service of the University of Veterinary Medicine Hannover, Foundation, Germany, from July 2014 to March 2021. 2014 is the date on which BCS and FAMACHA© score began to be recorded routinely.
⚠️ This is not a farm population: it is a clinical population. These are sick animals, brought to a tertiary-level university clinic. The authors state this repeatedly and use it to explain the differences from field studies. It is the single piece of context that governs the reading of every percentage in this record.
Inclusion criteria, as stated: only animals with information on species, sex, age, BCS, PCV, haemoglobin, total leukocytes and smear with differential count. Absolute body weight and FAMACHA© score were not recorded for all animals, and were assessed only in a subset — which is why the denominators in field 15 change from one parameter to another.
10 · Domanda clinica (PICO)
10 · Clinical question (PICO)
In alpacas and llamas presented at a European veterinary clinic [P], how strongly is the assessment of body condition score and FAMACHA© score [I], compared with haematological parameters measured in the laboratory [C: reference standard], associated with anaemia and alterations of the leukocyte differential [O]?
The question is not «does FAMACHA© work?», and it is important not to make it say that: it is how strongly the two clinical scores move together with laboratory parameters, in a European clinical population — which is a different context from the one in which the camelid FAMACHA© was validated (field 26).
11 · Disegno dello studio
Nessun intervento, nessun controllo, nessuna randomizzazione, nessun follow-up.
11 · Study design

12 · Level of evidence
Retrospective observational study on a clinical case series. For the reader: (a) the measured associations are not causal links, and the authors write this explicitly («it is not possible to draw a conclusion whether a lower BCS was the cause of the haematological alterations or the consequence»); (b) the observed prevalences do not transfer to the farm population, because the sample is a population of sick animals; (c) the correlation coefficients are the main outcome, and must be read with the interpretation scale that the authors state before using it — which is a merit and should be said.
13 · Relevant reporting standard
STROBE, for the observational study. The authors do not state adherence to it. It must be credited, however, that the article has an explicit, separate «Limitations» section (field 17) and a pre-stated scale for interpreting correlations — two items that STROBE calls for and that most veterinary observational studies lack.
14 · Metodo
Estrazione dati.
Significatività a p < 0,05. Scala di interpretazione dichiarata preventivamente: R = 0,2-0,3 correlazione molto debole · 0,3-0,5 debole · 0,5-0,8 moderata · > 0,8 forte.
14 · Method
Data extraction. Paper records up to August 2016, digital (easyVET) from September 2016, transferred into a spreadsheet. All data collected during the course of veterinary diagnostic procedures, with the owners' written consent.
BCS. Scale from 1 (emaciated) to 5 (obese) in steps of 0.5, by palpation of the lumbar spine, by experienced examiners; optimal BCS defined as 3. Recorded at the first clinical visit, immediately after presentation.
FAMACHA© score (FS). Colour of the lower palpebral conjunctivae, from 1 (physiological red) to 5 (pale, almost white); FS > 2 defined as an indication of anaemia. In animals with reddened conjunctiva, for example from conjunctivitis, the score was not assigned.
⛔ The method line that decides how everything else is to be read, and must be quoted verbatim: «The official FAMACHA© chart was used to learn the technique, but in most cases, in routine practice, the score was assessed as a subjective impression of conjunctival colour, without using the chart's colour standard as a direct comparison.»
That is: this study does not, strictly speaking, measure FAMACHA©. It measures a subjective impression of conjunctival colour on a five-point scale, collected by several different examiners over seven years. The authors state this and treat it as a limitation (field 17). Every conclusion of this record on «FAMACHA©» must be read with this restriction applied.
Haematology. Venous blood in EDTA from the jugular, at the general clinical visit; samples processed immediately or stored at 4 °C if taken at night or on weekends. PCV: capillary microhaematocrit centrifugation, 10 minutes at 10,000 × g. Haemoglobin: photometric with cyanide solution (10 µL of EDTA blood in 2.5 mL of solution containing 18 mmol/L sodium bicarbonate, 0.768 mmol/L potassium cyanide and 0.608 mmol/L potassium ferricyanide; 5-minute incubation, reading at 546 nm, extinction multiplied by the factor 368). Leukocytes: microscopic count in a Neubauer chamber after lysis of 100 µL of EDTA blood in 900 µL of 3 % acetic acid for 5 minutes. If normoblasts were present on the smear, the count was corrected with the formula:
Leukocytes (G/L) = nuclei counted in the Neubauer chamber (G/L) × 100 / (100 + number of normoblasts per 100 leukocytes)
Differential count: 200 leukocytes per smear, Pappenheim staining, assigned to lymphocytes, segmented neutrophils, band neutrophils, eosinophils, basophils and monocytes; normoblasts were recorded in addition to the 200 leukocytes.
Definition of anaemia. Reference values of Hengrave Burri et al. 2005, reported in full because they are the threshold on which every percentage in this record rests: alpacas — cria 0.29 L/L, adult males 0.29 L/L, adult females 0.26 L/L; llamas — cria 0.28 L/L, adult males 0.29 L/L, adult females 0.27 L/L. Animals below the respective lower limit: anaemic.
Statistics. SAS Enterprise Guide 7.1. Normality with Shapiro-Wilk (failed in most groups, hence non-parametric tests). Two-sample Wilcoxon for species, sex, age and presence of anaemia; Kruskal-Wallis for the different BCS and FS values; Spearman correlation. Significance at p < 0.05. Interpretation scale stated in advance: R = 0.2-0.3 very weak correlation · 0.3-0.5 weak · 0.5-0.8 moderate · > 0.8 strong.

| Reperto | Alpaca | Lama |
|---|---|---|
| BCS < 3 | 154/259 (60 %) | 29/41 (70 %) |
| FS > 2 | 24/214 (11,2 %) | 8/39 (21 %) |
| Anemici (sotto il riferimento di Hengrave Burri) | 55 % | 49 % |
| Normoblasti presenti nello striscio | 85/259 (33 %) | 14/41 (34 %) |
*** = p < 0,001 · ** = p < 0,01 · * = p < 0,05 · n.s. = non significativo.
| Coppia di parametri | Alpaca (n = 259) | Lettura | Lama (n = 41) | Lettura |
|---|---|---|---|---|
| FS ↔ PCV | r = −0,34 \\\* | debole | r = −0,73 \\\* | moderata |
| FS ↔ emoglobina | r = −0,32 \\\* | debole | r = −0,72 \\\* | moderata |
| FS ↔ BCS | r = −0,32 \\\* | debole | r = −0,55 \\\* | moderata |
| FS ↔ normoblasti | r = +0,26 \\\* | molto debole | r = +0,54 \\\* | moderata |
| FS ↔ eosinofili | r = −0,11 n.s. | nessuna | r = −0,54 \\\* | moderata |
| FS ↔ peso corporeo | r = +0,03 n.s. | nessuna | r = −0,36 \* | debole |
| BCS ↔ PCV | r = +0,29 \\\* | molto debole | r = +0,59 \\\* | moderata |
| BCS ↔ peso corporeo | r = +0,41 \\\* | debole | r = +0,66 \\\* | moderata |
| BCS ↔ eosinofili | r = +0,28 \\\* | molto debole | r = +0,61 \\\* | moderata |
| BCS ↔ linfociti | r = +0,09 n.s. | nessuna | r = +0,54 \\\* | moderata |
| BCS ↔ neutrofili segmentati | r = −0,08 n.s. | nessuna | r = −0,46 \\ | debole |
| PCV ↔ emoglobina | r = +0,94 \\\* | forte | r = +0,96 \\\* | forte |
La quinta coppia, BCS con peso, c'è in entrambe le specie, ma più debole nell'alpaca (r = +0,41 contro +0,66 nel lama; Tabelle 2 e 3 dell'articolo, e Discussione: «a good correlation of bodyweight and BCS was found in both species»).

(a) Body condition and conjunctival score — with the denominators, which change here.
| Finding | Alpacas | Llamas |
|---|---|---|
| BCS < 3 | 154/259 (60 %) | 29/41 (70 %) |
| FS > 2 | 24/214 (11.2 %) | 8/39 (21 %) |
| Anaemic (below the Hengrave Burri reference) | 55 % | 49 % |
| Normoblasts present on smear | 85/259 (33 %) | 14/41 (34 %) |
⚠️ The denominators of the conjunctival score are 214 and 39, not 259 and 41. The score was not recorded for all animals (field 9) and was not assigned to those with reddened conjunctiva. 45 alpacas and 2 llamas have no score. Whoever writes «11.2 % of the alpacas in the case series» is changing the denominator. (The arithmetic: 24 / 214 = 0.112, i.e. 11.2 %; the article writes «About 12% (n = 24/214)», Results §3.2.2, and the 12 % recurs in the abstract.)
(b) Haematocrit and haemoglobin. PCV minimum 0.04 L/L — in one alpaca and one llama; maximum 0.43 L/L in the alpaca and 0.47 L/L in the llama. Haemoglobin: 20-198 g/L in the alpaca, 15-218 g/L in the llama. No statistically significant difference in PCV by species, sex or age.
(c) The central result: the correlations, which are not the same in the two species. Spearman coefficients, with the authors' interpretation scale applied. *** = p < 0.001 · ** = p < 0.01 · * = p < 0.05 · n.s. = not significant.

| BCS ↔ segmented neutrophils | r = −0.08 n.s. | none | r = −0.46 \\ | weak |
| PCV ↔ haemoglobin | r = +0.94 \\\* | strong | r = +0.96 \\\* | strong |
In one line: in the alpaca the conjunctival score explains little of PCV (r = −0.34, weak correlation); in the llama much more (r = −0.73, moderate correlation, close to the strong threshold). The same holds for BCS. And four associations present in the llama do not exist at all in the alpaca: FS with eosinophils, FS with weight, BCS with lymphocytes, BCS with segmented neutrophils. The fifth pair, BCS with weight, is present in both species, but weaker in the alpaca (r = +0.41 against +0.66 in the llama; Tables 2 and 3 of the article, and Discussion: «a good correlation of bodyweight and BCS was found in both species»).
(d) The finding worth a practical rule. Verbatim: «all animals with severe anaemia (PCV < 0.10 L/L) had a BCS ≤ 3» and «none of the animals with BCS > 3 had severe anaemia (< 0.10 L/L)». This holds in both species.
(e) Anaemic versus non-anaemic (Wilcoxon test, Table 1 of the article). Significant differences in both species for body weight, BCS, FS, PCV and haemoglobin. In alpacas additionally: band neutrophils, monocytes and normoblasts differed — but with a counter-intuitive detail stated by the authors: «the numerical proportion of band neutrophils was lower in anaemic alpacas than in those without anaemia, while in llamas it was the opposite». In llamas, on the other hand, eosinophils differed, lower in the anaemic animals.
(f) What did not emerge. Total leukocytes showed no difference and no significant correlation with BCS, FS or PCV in either species — despite an enormous range (0.3-75.0 G/L in alpacas; 2.8-40.4 G/L in llamas). This is a negative result, and it is useful: the leukocyte count does not help identify the anaemic animal.
(g) The comparison with the US validation, reported by the authors. Storey et al. 2017 studied the FAMACHA© score in 347 alpacas and 502 llamas (total 849) from different farms; in 17 of the 21 farms in the study Haemonchus contortus was the predominant nematode. From that validation the authors of this record report: to detect anaemia defined as PCV ≤ 0.17 L/L with a FAMACHA© score ≥ 4, sensitivity 50 % and specificity 94 %.
⚠️ Discrepancy between two articles by the same authors on Storey's sample size. Here one reads 347 + 502 = 849; in CRL-L-02 (Wagener et al. 2024) one reads 859. Storey's abstract does not report the total. →
[TO BE CHECKED AGAINST THE FULL TEXT]of Storey et al. 2017, not accessible (no PMCID). The record reports both figures and does not choose one.
(h) The contrast with what owners see. In a survey among German alpaca and llama owners, fewer than half of the 255 participating farms (38.9 %) reported emaciation in their own animals, and only 13.3 % reported anaemia — against 60-70 % with BCS < 3 and about 50 % anaemic in this case series. The authors correctly warn that the numbers are not directly comparable, because the animals brought to the clinic are sick; but the direction of the gap is that one, and the reason they indicate is physical: the dense fleece, and the fact that owners do not regularly palpate their own animals.
16 · Authors' interpretation
The authors conclude that «more than half of the alpacas and llamas presented at the clinic had a BCS < 3» and that «half of the animals suffered from anaemia»; that «a low BCS was predominantly associated with an increased FS and reduced PCV and haemoglobin»; and that «there was also evidence that a low BCS was associated with an increase in segmented neutrophils, which may indicate that animals with a lower BCS are more likely to be affected by inflammatory diseases».
The authors explain the width of the PCV range for each conjunctival score — and this is their explanation, not a datum: the score «does not specifically assess PCV, since this can also be elevated or depressed due to local inflammatory processes in the conjunctivae or circulatory centralisation»; added to this is that «a colour chart specific to South American camelids is lacking» and that the score was collected by multiple examiners.
The authors propose, and it must be reported as a proposal: «for daily clinical practice, a less precise subdivision — for example "physiological red", "pale" and "white" — might be sufficient to obtain a first impression of an individual animal».
The authors recommend educating owners on the fact that poor nutritional status is closely associated with pathological haematological findings, and encouraging them to check nutritional status regularly so that emaciation is detected in time.
The authors state that they cannot explain two findings. The association between low BCS and anaemia: «remains unclear; one could speculate about bone marrow atrophy or chronic inflammation». And the positive correlation between BCS and eosinophils: «remains unclear… a negative correlation would have been expected» given the link between anaemia, poor nutritional status and endoparasites; they hypothesise that animals with a lower BCS are «in an immunologically more inactive state», but add verbatim that «evidence for this assumption is lacking».
17 · Limiti dichiarati dagli autori
17 · Limitations stated by the authors
The article has a separate «Limitations» section, and the limitations are quoted, not summarised:
- «The focus of this study was to compare clinical scores with haematological findings. It therefore does not allow a general statement regarding individual clinical pictures.»
- «No conclusion can be drawn as to whether a lower BCS was the cause of the haematological alterations, or whether the BCS was altered as a consequence of visible alterations in the blood count.»
- «Faecal egg count results were not considered. Several animals presented at the clinic had already been dewormed by other veterinarians or by the owners themselves shortly before presentation. This made it impossible to determine the overall contribution of endoparasites to the clinical picture.»
- «The animals' clinical diagnoses were not considered in the assessment. It was not possible to define a main diagnosis for each animal, since some had more than one diagnosis. Common diagnoses besides emaciation or anaemia included decubitus and gastric ulcers.»
- «Data on pregnancy stage or lactation were not available for all animals. Therefore these parameters were not considered.»
- In the discussion, two additional limitations stated: the subjective assessment without a colour chart, by different examiners; and the fact that samples were taken after transport, with possible stress-induced haemoconcentration and alterations of the leukocyte fractions.

- The heaviest limitation is in the method, not in the limitations section. If the score was collected without the colour chart in most cases, then the variable measured is not FAMACHA©: it is a five-level subjective impression. It follows that the coefficient r = −0.34 in alpacas is not a measure of the performance of FAMACHA© used correctly, and might underestimate it. This cuts both ways and must be stated in full: the study does not demonstrate that FAMACHA© performs poorly in alpacas; it demonstrates that a subjective conjunctival impression performs poorly in alpacas. Which is nonetheless the way it is used in the field by most people.
- The comparison between species is not on equal footing. 259 alpacas against 41 llamas. A correlation estimated on 41 subjects has a much wider confidence interval than one estimated on 259, and the article does not report confidence intervals for any coefficient. The difference between r = −0.34 and r = −0.73 is marked and consistent across several variable pairs — so it is plausibly real — but it was not formally tested as a difference between two correlations.
- No measure of diagnostic performance. The article does not calculate sensitivity, specificity, predictive values or area under the curve for any score threshold. It does not, that is, produce the datum needed to decide whether to treat; it produces association coefficients. The only sensitivity and specificity figures found in the record come from Storey 2017 (field 15g) and are second-hand.
- The absence of egg counts is not a minor limitation: it is the limitation. In a study on a score created for selecting animals to be dewormed, the parasitological variable is missing, and it is missing for a reason that aggravates the problem — many animals had been dewormed shortly before.
- Single-centre and tertiary-level. A single university clinic, for seven years: the case series also reflects the referral criteria of the region's veterinarians.
- Seven years of data collection, with a change of information system halfway through (paper up to August 2016, easyVET from September 2016) and an unspecified number of examiners: the classic conditions for measurement drift, which the article does not quantify.
- The anaemic/non-anaemic classification depends entirely on an external 2005 reference (Hengrave Burri), established with a method that may not coincide with that of the Hannover laboratory — and this is exactly the caveat that the same authors give in CRL-L-02 about the use of reference values.

- Variabili quantitative e confondenti (voci 11, 16): nessuna analisi multivariabile.
21 · What is missing relative to the reporting standard (STROBE)
- No statement of adherence to STROBE.
- Data sources and measurement (item 8): the measure of the main exposure — the conjunctival score — is stated to be non-standardised (no chart, multiple examiners). The standard would call for an assessment of inter-examiner agreement, which was not done and whose absence the authors acknowledge («no data on this are available for South American camelids»).
- Bias (item 9): no description of efforts to contain potential sources of bias, beyond listing them in the limitations.
- Study size (item 10): no calculation; the sample is the one available.
- Quantitative variables and confounders (items 11, 16): no multivariable analysis. All associations are bivariate; age, sex, diagnosis, pregnancy and prior treatments are not adjusted for — and the last three are not even available.
- Missing data (item 12c): partially addressed (the variable denominators are stated), but the reason why the score was missing in 45 alpacas and 2 llamas is not reported beyond conjunctivitis.
- Generalisability (item 21): discussed, and well — the authors explain why their case series differs from the field series of Storey et al.
What is not missing, and should be credited above the average for the genre: an explicit «Limitations» section, the correlation interpretation scale stated before use, the exact denominators for every proportion, thirteen supplementary tables with full descriptive statistics, and the statement of laboratory methods down to the molarity of the reagents.
23 · Valutazione a stelle
| Asse | Voto | Motivazione, con un fatto dentro |
|---|---|---|
23 · Star rating
| Axis | Score | Rationale, with one fact inside |
|---|---|---|
| A — Scientific quality | ★★★☆☆ | Observational study with n stated and broken down into ten groups, explicit inclusion criteria, appropriate non-parametric analysis with the interpretation scale stated before use, separate «Limitations» section, thirteen supplementary tables, ethical approval with a registration number, funding and conflicts stated: this is the definition of three stars in chapter 14 §11. It does not rise to four because the measure of the main exposure is admittedly non-standardised (no colour chart, multiple examiners, no agreement assessment), there is no multivariable analysis, and the confidence intervals of the coefficients are missing. |
| B — Clinical utility | ★★★☆☆ | Offers a replicable procedure — palpatory BCS plus conjunctival assessment, with the rule of not assigning the score in case of reddened conjunctiva — to be used with the caution imposed by the level of evidence. It also provides two solid operational pieces of information: the difference in performance between llama and alpaca, and the fact that no animal with BCS > 3 had severe anaemia. It does not reach four for a precise reason: the tool measured is not the standard FAMACHA©, and the article produces no measure of diagnostic performance (sensitivity, specificity, predictive values) on which to base a treatment decision. |
| C — Educational value | ★★★★★ | This is the text that teaches, on real data, why a tool validated elsewhere does not transfer: same tool, same examiners, same clinic, and two species that respond differently. It also shows a textbook case of discordance between what the owner sees and what the animal has (13.3 % versus ~50 %), and contains a real bibliographic trap — the two academic editors in the authors' block of the XML (field 3) — that is worth a lesson on its own. |
| D — Relevance to the species | ★★★★★ | Study on alpacas and llamas, in a European clinic, with European reference values, on a population of animals kept by private owners and small breeders — the same dominant form of husbandry in Italy. The distinction between the two species is maintained in every table and every conclusion. No data from another species is passed off as camelid. |
24 · Bottom line
24 · Bottom line
It can be concluded that: in 300 South American camelids (259 alpacas, 41 llamas) presented to a German university clinic between 2014 and 2021, the conjunctival score correlates with haematocrit to very different degrees in the two species — r = −0.34 (p < 0.001) in the alpaca, a "weak" correlation according to the authors' stated scale, versus r = −0.73 (p < 0.001) in the llama, a "moderate" correlation —; that 60 % of alpacas (154/259) and 70 % of llamas (29/41) had BCS < 3, with 55 % of alpacas and 49 % of llamas below the haematocrit reference; and that all animals with PCV < 0.10 L/L had BCS ≤ 3, while no animal with BCS > 3 had severe anaemia.
It cannot be concluded that: FAMACHA© performs poorly in alpacas. The tool measured in this study is not the standard FAMACHA©: the authors state that in routine practice the score was assigned «as a subjective impression of conjunctival colour, without using the chart's colour standard». Nothing can be concluded about diagnostic performance — the article calculates neither sensitivity nor specificity —, nothing about the contribution of parasites, because egg counts were not considered and many animals had already been dewormed, and nothing about the European farm population, because this is a clinical population of sick animals.

26 · Schede CRL correlate
- CRL-L-01 (questo fascicolo) — Hinney e coll. 2024.
BE, Williamson LH, Howell SB, Terrill TH, Berghaus R, Vidyashankar AN, Kaplan RM. Validation of the FAMACHA© system in South American camelids.
26 · Related CRL records
- CRL-L-01 (this issue) — Hinney et al. 2024. The two records partly contradict each other, and it is useful that this is visible. Hinney et al. recommend FAMACHA© for selecting animals to be treated on farms with Haemonchus predominance; this record measures the performance of that score in Europe and finds it weak in the alpaca. This is not a real conflict: Hinney refers to herds where Haemonchus accounts for 53 to 97 % of cultures, i.e. where the tool has its precondition; L-05 refers to a mixed clinical population. But anyone who reads only the first applies the score where it does not hold.
- CRL-L-02 (this issue) — Wagener et al. 2024, by the same first and last authors. This record is the primary datum of that review for the anaemia frequencies in the clinic (55 % and 49 %), for the minimum PCV of 0.04 L/L, and for normoblasts in one camelid in three. Anyone citing those numbers must cite L-05. In the opposite direction, the seven-step operational algorithm — BCS and score every 4-6 weeks, further investigation below BCS 2 and above score 2 — is in L-02, and this record measures its most fragile part.
- CRL-A-01 — the record from chapter A on automated haematology in camelids (Viesselmann et al. 2019a, ADVIA 2120): it is the laboratory complement to this one. L-05 measures PCV by capillary centrifugation at 10,000 × g for 10 minutes, and this is precisely the choice that A-01 and L-02 explain as mandatory in camelid anaemia.
- Context verified but not recorded as a card, and it is the validation against which this record measures itself: Storey BE, Williamson LH, Howell SB, Terrill TH, Berghaus R, Vidyashankar AN, Kaplan RM. Validation of the FAMACHA© system in South American camelids. Vet Parasitol 2017;243:85-91, DOI 10.1016/j.vetpar.2017.06.004, PMID 28807317 (Crossref and Europe PMC, 31/08/2026; full text not accessible — no PMCID). From the abstract: 21 alpaca and llama farms followed for 2 years; H. contortus predominant in 17 of 21 (10 alpaca, 7 llama); PCV was significantly associated with FAMACHA© score, faecal egg count and BCS (p < 0.01 in all cases); best sensitivity 96.4 % with a score ≥ 3 (versus 92.9 % with ≥ 4) and best specificity 94.2 % with a score ≥ 4 and PCV ≤ 20 % (versus 69.1 % with ≥ 3); the authors conclude that «the parameters for treatment decisions based on the FAMACHA© score in camelids should mirror those established for small ruminants». ⚠️ Two caveats on citing this abstract. First: the published sentence reads «FAMACHA© scores ≥3 and PCV ≥ 15% indicating anemia», with an inequality sign that is plausibly a typo (anaemia is defined by a PCV below the threshold, as elsewhere in the same abstract); this record reproduces the published text and flags the suspected typo rather than silently correcting it. Second: the total sample size does not appear in the abstract, and is reported discordantly by the two articles that cite it (849 in L-05, 859 in L-02) →
