Commento dell'associazione

Riquadro redazionale — CRS/CRL, 31/08/2026.

Association's commentary

Editorial box — CRS/CRL, 31/08/2026.

The prevalence of cheek teeth disorders rises from 50.0 % in the 0-3 year group to 100 % in the two groups over nine years old. The 82 % is impressive and is cited badly; the line that should change a farm's calendar is a different one, the age stratification: the prevalence of cheek teeth disorders rises from 50.0 % in the 0-3 year group to 100 % in the two groups above nine years, and diastemata from 23.9 % to 86.4 %. One young alpaca in two already has something; an old alpaca simply has it. This is not an argument for «checking the old ones»: it is an argument for looking at the young ones, because that is where the window still exists in which a diastema is a diastema and not yet a periodontitis with osteomyelitis (the chain that record CRL-N-03 picks up from the other end, the surgical one).

The real contribution of this article, however, is not a percentage: it is Table 1. Before this work there was no list of operational definitions of dental disorders in the alpaca. The prevalences will age; the definitions will not, and they are the reason these numbers can be compared with those to come. Anyone replicating the study elsewhere must use those definitions, otherwise they are comparing words.

On sedation, a clarification that saves arguments. The protocol in field 14 is the only published protocol with doses for oral examination of the alpaca that this fascicle has found, applied to 228 animals in the field. It is not, however, a validated protocol: no comparison with alternatives, no systematic adverse-event count, no recovery time measured beyond the statement about fifteen minutes. It is documented practice at scale, which is a great deal, and it is not the same thing as a recommendation based on comparative evidence. The reference camelid anaesthesiology is elsewhere (Abrahamsen EJ. «Chemical Restraint, Anesthesia, and Analgesia for Camelids», Vet Clin North Am Food Anim Pract 2009;25(2):455-494, DOI 10.1016/j.cvfa.2009.02.001 — metadata verified in Crossref on 31/08/2026, text not read in this fascicle) and in chapter 04 §5 of the KT.

One remark that concerns us, not the authors. The two arithmetic discrepancies in field 15 were found by redoing the divisions, not by reading the abstract. This is why this fascicle redoes every printed division: if an excellent article, in a peer-reviewed journal, reaches print with a 52.4 % that should be 50.4 % and with percentages on a denominator different from the numerator beside them, then every number copied without recalculation is an unverified number, and our first draft learned this at its own expense.

What canNOT be concluded from here, however tempting. One cannot say how many alpacas have a tooth root abscess: this study did not measure them and says so. One cannot say that Italian alpacas are like Belgian ones. One cannot say that diastemata cause periodontal disease. And one cannot say «only 3.9 % have canine problems, so fighting teeth are not a problem»: the design did not specifically record injuries from fighting between animals, and the only gingival lesion attributed to a canine in the whole series came from an overgrown maxillary canine in one animal, not from a fight.

5 · Rivista · anno · volume · fascicolo · pagine

Journal of Veterinary Internal Medicine, 2020, volume 34, fascicolo 2, pagine 1028-1038.

5 · Journal · year · volume · issue · pages

Journal of Veterinary Internal Medicine, 2020, volume 34, issue 2, pages 1028-1038. Electronic publication: 27 February 2020 (Europe PMC firstPublicationDate); Crossref records both published-print and published-online as 1 March 2020; PubMed esummary gives pubdate: 2020 Mar and epubdate: 2020 Feb 27. Copied from the records, not reconstructed.

⚠️ A record detail that must be stated because it confuses. Crossref today attributes the article to the publisher Oxford University Press and records two licence dates for the same version of record: 27/02/2020 and 03/02/2026. In 2020 the journal was published by Wiley; OpenAlex still records host_organization_name: Wiley. This is the signature of a journal transfer that happened after publication. It changes nothing about the article, but it explains why two databases give two different publishers for the same work — and why the publisher is not reconstructed from memory.

8 · Article licence

Creative Commons Attribution – NonCommercial – NoDerivatives 4.0 (CC BY-NC-ND 4.0). Crossref records the licence for the version of record with the two start dates cited in field 5; Europe PMC reports license: cc by-nc-nd. Read in Crossref and Europe PMC on 31/08/2026.

This record may reproduce: factual data (which are not covered by copyright) and short quoted extracts for synthesis and commentary. ⛔ No figure and no table of the article is reproduced: the ND clause forbids derivative works, and a reformatted table is a derivative work. Figures 1-4 of the article — the per-farm prevalence map and the three photographic plates of lesions — remain to be looked at in the article.

9 · Specie e soggetti

Alpaca (Vicugna pacos), e solo alpaca.

  • Sesso: 115 femmine, 51 maschi interi, 62 maschi castrati.
  • Età: da 1 a 17 anni, media 5,60 ± 3,17 anni.
  • Razza: Huacaya 223/228 (97,8 %); Suri 5/228.

«The number of animals to be examined at each farm was selected by the owner depending on their economic value and the gestational status of the female animals»: il proprietario sceglieva quali animali far visitare, in base al loro valore economico.

9 · Species and subjects

Alpaca (Vicugna pacos), and only alpaca. No llamas, no guanacos, no vicuñas, no Old World camelids.

  • 228 animals from 25 farms, in the northern part of Belgium and the southern part of the Netherlands.
  • Period: June-October 2018, chosen by the authors «to avoid interference with gestation».
  • Sex: 115 females, 51 intact males, 62 castrated males.
  • Age: from 1 to 17 years, mean 5.60 ± 3.17 years.
  • Breed: Huacaya 223/228 (97.8 %); Suri 5/228.
  • Animals per farm: mean 9 ± 7, range 1-28. Mean sampling rate per farm 40.6 % (range 2.9-93.3 %).

⚠️ Editorial arithmetic check, first. The three sex groups sum correctly to 228 (115 + 51 + 62), and the printed percentages for intact males (22.4 %) and castrated males (27.2 %) match 51/228 and 62/228. The printed percentage for females is 52.4 %, but 115/228 = 50.4 %; with 52.4 % the three shares would sum to 102.0 %, whereas with 50.4 % they sum to exactly 100.0 %. The correct figure is 50.4 %, and 52.4 % is almost certainly a typesetting slip. This record reports the count (115 of 228) and flags the discrepancy; it does not rewrite the article's text.

⚠️ How animals were chosen within the farm. «The number of animals to be examined at each farm was selected by the owner depending on their economic value and the gestational status of the female animals»: the owner chose which animals were examined, based on their economic value. It is written in the methods, not in the limitations, and it has consequences that field 18 takes up.

10 · Domanda clinica (PICO)

  • I — esame orale completo in sedazione profonda, con specolo, fonte luminosa e specchio dentale e/o oroscopio rigido portatile, e cartella dentale su identificazione Triadan modificata.

10 · Clinical question (PICO)

  • P — farmed alpacas in Belgium and the Netherlands, not selected for dental signs, 1-17 years old.
  • I — full oral examination under deep sedation, with speculum, light source and dental mirror and/or portable rigid oroscope, and a dental chart using the modified Triadan system.
  • C — no comparison: this is a prevalence study, not a comparison of methods.
  • O — prevalence of individual dental disorders at animal and herd level, and associations between them.

As a question: how many alpacas have a dental disorder when you look at all of them, and not only at the sick ones — and which disorders travel together?

11 · Disegno dello studio

11 · Study design

Prospective cross-sectional multicentre observational study (25 farms), on a population not selected for signs. It is not a trial, it is not randomised, it has no control group. PubMed also indexes it as a Multicenter Study.

Sampling was convenience-based and voluntary: farms were selected among the 215 members of the Alpaca Association Benelux invited in May 2018; 33 expressed interest (response rate 15.3 %), and 25 were chosen among these on the basis of travelling distance.

12 · Level of evidence

Evidence from a multicentre cross-sectional observational study on an unselected population, with declared convenience sampling.

Consequence for the reader: these figures are, to date, the best existing prevalence estimate for dental disease in alpacas, and should be used as such. They are not an incidence estimate (a cross-sectional study photographs, it does not follow), they do not establish causation between the associated disorders (a cross-sectional odds ratio says two things occur together, not that one produces the other — and the authors say so), and they do not transfer outside north-western Europe without verification. Anyone who reads «diastemata and periodontal disease have OR 13.1» and concludes «diastemata cause periodontal disease» is extrapolating beyond the design, however plausible the hypothesis.

13 · Standard di reporting pertinente

13 · Relevant reporting standard

STROBE (observational studies), from the closed list of chapter 14 §3.5. ⚠️ The authors declare no reporting guideline, neither in the abstract nor in the full text read in its entirety.

14 · Metodo

In alpaca, sedazione profonda con ketamina 5 mg/kg IM (Ketamidor, Richter Pharma, Austria) + medetomidina 30 µg/kg IM (Sedator, Eurovet, Bladel) oppure dexmedetomidina 15 µg/kg IM (Dexdomitor, Orion). Fonte: Proost 2020 Parte 1, metodi, testo pieno.
  • Chi esaminava: «All dental examinations were performed by the same 2 investigators (K. Proost and L. Vlaminck)».

Read in the full text (ft-PMC7096633.xml), not in the abstract.

  • Selection: invitation to the 215 member farms of the Alpaca Association Benelux (May 2018); 33 interested; 25 chosen by travelling distance, the authors stating «95 % confidence to obtain a prevalence estimate with an accepted error of 10 %».
  • Sedation — the Ghent protocol, with dose, route and species. In alpacas, deep sedation with ketamine 5 mg/kg IM (Ketamidor, Richter Pharma, Austria) + medetomidine 30 µg/kg IM (Sedator, Eurovet, Bladel) or dexmedetomidine 15 µg/kg IM (Dexdomitor, Orion). Source: Proost 2020 Part 1, methods, full text. The authors state in the discussion that sedation must be deep and that most animals become recumbent, unlike the horse which is examined standing; recovery is «rapid and smooth, with most animals standing within 15 minutes after finishing the examination».
  • Examination: thorough irrigation of the oral cavity to evacuate food; miniature pony speculum with dedicated light source; dental mirror, portable rigid oroscope (Karl Storz, Tuttlingen) or both; custom dental chart; teeth identified with the modified Triadan system.
  • Who examined: «All dental examinations were performed by the same 2 investigators (K. Proost and L. Vlaminck)». Two fixed operators for all 228 animals.
  • Definitions: the article publishes in Table 1 an operational definition for each of the disorders recorded (diastema, wear abnormality, worn tooth, wave mouth, shear mouth, step mouth, enamel overgrowths, focal overgrowths, accentuated transverse ridges, periodontal disease, malpositioned teeth, occlusal pulp exposure, persisting deciduous teeth). It is the most reusable part of the work and the reason its prevalences are comparable with themselves.
  • Statistics: unit of analysis the animal; univariate logistic regression with herd as a random factor (glmer) to account for clustering; threshold P < .05; R v3.5.2.
  • Five age classes: A 0-3 years; B 3.1-6; C 6.1-9; D 9.1-12; E 12.1-17.
  • ⚠️ What the study could not diagnose, and says so: periapical infections («tooth root abscesses») are excluded from the prevalences, because they require radiography or CT, which were not performed here. The 82 % prevalence does not include tooth root abscesses: a point anyone citing this work must hold firm.

Incisivi e canini.

Esposizione pulpare occlusale: 2,2 % (1/46) a 0-3 anni → 46,2 % (6/13) a 12-17 (P = 0,03).

⚠️ Editorial arithmetic check, on the cheek teeth. The percentages are those of the full text (§3.3 and Table 3), but recalculated over 228 six of them do not hold: 97/228 = 42.5 % (printed 43.1 %), 89/228 = 39.0 % (39.6 %), 75/228 = 32.9 % (33.3 %), 44/228 = 19.3 % (19.6 %), 23/228 = 10.1 % (10.2 %), 8/228 = 3.5 % (3.6 %). They do hold over 225, the total of Table 3 by age: 97/225 = 43.1 %, 89/225 = 39.6 %, 75/225 = 33.3 %, 44/225 = 19.6 %, 23/225 = 10.2 %, 8/225 = 3.6 %. Two items do the opposite and hold over 228: 22/228 = 9.6 % and 21/228 = 9.2 % (over 225 they would be 9.8 % and 9.3 %). The overall 74.6 % holds with neither: 169/228 = 74.1 %, 169/225 = 75.1 %. The article does not explain the use of two denominators; this record reports the printed percentage and the count, and does not decide which of the two is the right one.

Incisors and canines. The article prints these values in the form n/95; %: diastemata 36/95; 15.8 % · occlusal pulp exposure 25/95; 11.0 % · malpositioned teeth 23/95; 10.1 % · wear abnormalities 21/95; 9.2 % · mandibular overbite 8/95; 3.5 % · periodontal disease 8/95; 3.5 % · crown fracture 6/95; 2.6 % · persisting deciduous incisors 2 · mandibular underbite 2 · missing incisors 1 · peripheral cemental caries 1.

⚠️ Editorial arithmetic check, second — and it is not a quibble. The printed percentages are not calculated on 95: 36/95 = 37.9 %, not 15.8 %. 15.8 % is 36/228. The same holds for every other item in the row (25/228 = 11.0 %; 23/228 = 10.1 %; 8/228 = 3.5 %). The article's notation therefore places a numerator counted on the 95 animals with incisor disorders next to a percentage calculated on the 228 total animals. Furthermore, Table 2 of the article — which distributes the same lesions between maxilla and mandible — reports 35 and not 36 for incisor diastemata. This record uses denominator 228 because that is the one on which the percentages work out, reports the count as the article prints it, and does not decide whether the right number is 35 or 36 → [TO BE CHECKED AGAINST THE FULL TEXT] with the authors. The practical point: the frequency of mandibular overbite in this population is 8 alpacas out of 228, i.e. 3.5 %, and anyone citing it as «3.5 % of the animals with incisor disorders» is inflating it fourfold.

Canines (denominator 228). Calculus 1.3 % (3/228); occlusal pulp exposure, periodontal disease and dental-related soft tissue trauma attributable to the canines each 0.9 % (2/228); uncomplicated crown fracture, bilateral malpositioning and canine overgrowth each 0.4 % (1/228).

Oral soft tissue lesions. 4.8 % (11/228) in all; of these, 5/228 (2.2 %) attributable to a dental disorder: 2 buccal lesions from increased molar mobility due to advanced periodontal disease, 2 buccal lesions from persisting deciduous teeth, 1 gingival lesion caudal to the mandibular incisors caused by overgrown maxillary canines. In the other 6 no cause was identified. ⚠️ None of the oral soft tissue lesions was attributable to enamel overgrowth — which is the opposite of what happens in the horse, and the authors stress it.

In animals with at least one diastema, concurrent periodontal disease in 60/97 (61.9 %). Number of diastemata per affected animal: from 1 to 7, mean 2 ± 1.

Topographic distribution. Periodontal disease predominates in the mandible: the article writes 61/75 (81.3 %) in the text, while Table 4 reports 60 mandibular and 34 maxillary cases (an animal may have both sites). In both arcades periodontal disease concentrates between the caudal teeth (Triadan positions 09-10 and 10-11). Mandibular diastemata are more frequent than maxillary ones (39.0 % vs 12.7 % of animals).

Age (denominator 225, not 228). Prevalence of cheek teeth disorders: 50.0 % in the 0-3 year group → 100 % in the two oldest groups. Diastemata: 23.9 % (11/46) at 0-3 years → 34.0 % (35/103) at 3-6 (Table 3 prints 36.1 %, but 35/103 = 34.0 %; the count 35 is confirmed by the total 11 + 35 + 21 + 19 + 11 = 97) → 51.2 % (21/41) at 6-9 → 86.4 % (19/22) at 9-12 → 84.6 % (11/13) at 12-17 (P < .001). Wear abnormalities: 26.1 % → 76.9 %. Occlusal pulp exposure: 2.2 % (1/46) at 0-3 years → 46.2 % (6/13) at 12-17 (P = .03). ⚠️ Editorial arithmetic check, third. The five age groups sum to 225 (46 + 103 + 41 + 22 + 13), not 228. Three animals do not appear in the age stratification and the article does not say which or why → [TO BE CHECKED AGAINST THE FULL TEXT]. It partly explains the percentages in the cheek-teeth check, but not periodontal disease: in the general text it is 33.3 % with 75 cases (holds over 225), in Table 3 by age it is 32.0 % with 73 cases (holds over 228: 73/228 = 32.0 %; over 225 it would be 32.4 %). The numerators differ, 75 and 73, as well as the denominators, and the article does not say why.

At herd level. Mean within-herd prevalence of dental disorders 85.1 % ± 18.5 %, range 22.2-100 %. By individual disorder: diastemata 43.0 % ± 22.8 %; periodontal disease 32.9 % ± 24.6 %; pulp exposure 13.9 % ± 21.8 %; persisting deciduous teeth 10.9 % ± 20.0 %; malpositioned teeth 21.2 % ± 25.3 %. Each of these disorders had farms at 0 % and farms at 100 %.

16 · Authors' interpretation

Explicitly attributed, from the full text, and kept separate from field 15.

The authors write that theirs is «the largest prevalence study on dental disorders in alpacas currently available» and that the findings «highlight the importance of routine dental examinations in sedated alpacas to allow early detection of dental disorders and thereby improve general welfare».

On the origin of diastemata, they advance a hypothesis and label it as such: the already high prevalence in the young groups (23.9 % at 0-3 years, 36.1 % at 3-6 as printed; the count over the 103 animals gives 34.0 %) «suggests diastemata to be in part a congenital problem in the current alpaca population», while the rise in older groups would be due to senile diastemata, as described in the horse.

On the pathogenic chain, they propose — conditionally — that food impaction and fermentation initiate periodontal inflammation, that deep pocketing may lead to apical infection, and that further wear leads to pulpitis and occlusal pulp exposure. This is a reconstruction, not a result of this study, which is cross-sectional.

On wear, they note that in alpaca cheek teeth wear concentrates in the mandible at positions 08 and 09, unlike the donkey where it predominates in the maxilla at 10-11, and hypothesise that «the largest masticatory forces possibly could be centered at the "09" position» — adding, however, that «this hypothesis warrants further research to measure masticatory forces and make a definite conclusion».

⚠️ Note what the authors do not claim. They propose no interval for dental checks. They do not state that diastemata cause periodontal disease. They do not say routine floating is useful: on the contrary, they recall that «routine floating of sharp enamel points as advised and performed in equine dentistry has not been encouraged in alpacas».

17 · Limiti dichiarati dagli autori

Citati, non riassunti.

  1. Bias di selezione, ammesso per esteso: «For practical reasons, our study was prone to selection bias. Given the absence of a national registration system for camelids in both countries, farms had to be selected from volunteering members of the Alpaca Association Benelux. Some owners might have volunteered because they already suspected some of their animals to have dental pathology.»
  2. Trasferibilità geografica: «differences between the current results and those of comparable studies conducted in another geographical region might exist».
  3. Confrontabilità fra specie e studi: «a cautious approach is necessary when comparing the prevalence of dental disorders among animals, species, and studies given the differences in age, study design and definitions used to describe specific pathologies».

17 · Limitations declared by the authors

Quoted, not summarised.

  1. Selection bias, admitted at length: «For practical reasons, our study was prone to selection bias. Given the absence of a national registration system for camelids in both countries, farms had to be selected from volunteering members of the Alpaca Association Benelux. Some owners might have volunteered because they already suspected some of their animals to have dental pathology.»
  2. Geographic transferability: «differences between the current results and those of comparable studies conducted in another geographical region might exist».
  3. Comparability across species and studies: «a cautious approach is necessary when comparing the prevalence of dental disorders among animals, species, and studies given the differences in age, study design and definitions used to describe specific pathologies».
  4. Power in the older groups: the non-significant differences among the older age groups are attributed by the authors to the small sample sizes of those groups (22 and 13 animals).
  5. Periapical infections were not measured, because they require imaging.

18 · Limiti aggiunti dalla redazione

18 · Limitations added by the editorial team

  1. ⚠️ The owner chose the animals, and chose them by economic value. The bias the authors declare is the one between farms (who volunteers). A second one exists within the farm, written in the methods and not taken up in the limitations: the number and identity of the animals examined were decided by the owner on the basis of economic value and gestational status. Sampling driven by commercial value plausibly selects younger animals in better condition, i.e. it underestimates pathology.
  2. The response rate is 15.3 % (33 of 215 farms). The 182 that did not respond are not described in any way.
  3. No confidence interval on any prevalence. CIs appear only on odds ratios. With 228 animals a binomial CI on 82.0 % would be narrow, but on the 3.9 % of canines — 9 animals — it would be wide enough to change the meaning of the sentence, and it is absent.
  4. Two operators, no inter-observer agreement. That all examinations were done by the same two people is a strength for internal consistency and a limitation for transferability: how well two examiners agree on these definitions has not been measured. Without that figure, one cannot know how much of the between-farm variability (22.2 % to 100 %) is biology and how much is reading.
  5. No blinding. The examiners knew the farm, the age and the history.
  6. The three animals missing from the age stratification are not explained (field 15).
  7. The arithmetic discrepancy on females and the one on incisor denominators (field 15) are typesetting errors that do not touch the conclusions, but do touch anyone who copies the numbers without redoing the division.
  8. No correlated clinical data. Weight, body condition score and owner-reported signs do not appear in this article: they are in Part 2 by the same group (10.1111/jvim.15740), whose full text has not been read in this fascicle → [TO BE CHECKED AGAINST THE FULL TEXT]. Anyone wanting the sentence «the alpaca hides oral discomfort» with a number behind it must go there, not here.
  9. No measure of severity. An animal with one diastema and an animal with seven both count as «1» in the prevalence. The per-animal distribution is given only for diastemata (1-7, mean 2 ± 1).

19 · Finanziamento

19 · Funding

⚠️ Not declared. The full text contains no funding section: after the discussion come the conflict of interest declaration, the off-label antimicrobial declaration, the ethics declarations and the acknowledgments, and nothing else. This record does not state that the study was unfunded: it states that the source does not declare it. These are two different statements and only the second is verifiable.

«A call for volunteering alpaca farms was spread through the Alpaca Association Benelux (AAB). One of the coauthors (T. Flahou) is member of the AAB and participated in this study with animals present at his alpaca farm. The AAB played no role in the final selection of participating farms neither in the collection, analysis and interpretation of data, nor in the decision to submit the manuscript for publication. B. Pardon acted as an unpaid consultant for the AAB. None of the authors has any financial or personal relationships that could inappropriately influence or bias the content of the paper.»

20 · Conflicts of interest

Declared by the authors, and it is a serious declaration, worth reading in full:

«A call for volunteering alpaca farms was spread through the Alpaca Association Benelux (AAB). One of the coauthors (T. Flahou) is member of the AAB and participated in this study with animals present at his alpaca farm. The AAB played no role in the final selection of participating farms neither in the collection, analysis and interpretation of data, nor in the decision to submit the manuscript for publication. B. Pardon acted as an unpaid consultant for the AAB. None of the authors has any financial or personal relationships that could inappropriately influence or bias the content of the paper.»

In plain terms: recruitment went through the breeders' association, one co-author is a member of that association and contributed his own animals, and a second co-author was an unpaid consultant to it. The authors write this and delimit the association's role. It must be read together with limitation 1 in field 18: this is not a suspicion of manipulation, it is a precise description of how one gets to examine 228 alpacas in a country with no national camelid register. Without the association, this study would not exist.

The authors further declare no off-label use of antimicrobials, no need for IACUC approval and no need for human ethics approval.

21 · Cosa manca rispetto allo standard di reporting

Rispetto a STROBE (studi trasversali):

21 · What is missing against the reporting standard

Against STROBE (cross-sectional studies):

  • ⛔ No participant flow diagram: 215 invited → 33 interested → 25 included is written in prose, but the step from 25 farms to 228 animals (how many animals were there in total? how many excluded and why?) is not traced.
  • ⛔ Sample size justification only gestured at: the phrase «95 % confidence to obtain a prevalence estimate with an accepted error of 10 %» is not a reproducible calculation (what expected prevalence? what reference population?).
  • ⛔ No measure of precision on the prevalence estimates (STROBE 16a): no 95 % CI.
  • ⛔ Missing data not reported (STROBE 12c): the three animals absent from the age stratification are not declared as missing data.
  • ⛔ No sensitivity analysis on the possible effect of the declared selection bias.
  • ⛔ No statement of adherence to a reporting guideline, and no protocol registration number.
  • ✅ What is not missing, and it must be said: an explicit operational definition of every outcome (Table 1), description of the recruitment source, statistical handling of clustering by farm (random factor), and a conflict of interest declaration detailed at individual author level.

23 · Valutazione a stelle

Quattro assi indipendenti, scala 1-5, criteri nel capitolo 14 §11. Nessun voto complessivo.

23 · Star rating

Four independent axes, 1-5 scale, criteria in chapter 14 §11. No overall score.

AxisScoreOne-line justification
A — Scientific quality★★★★☆Well-conducted cross-sectional design, published operational definitions, clustering by farm handled statistically; falls short of five stars because of convenience sampling with within-farm selection made by the owner, absence of CIs on prevalences, and two arithmetic slips in print.
B — Clinical usefulness★★★★★Gives the clinician what they did not have: a prior probability by age, a sedation protocol with doses applied to 228 animals, and the certainty that a quick look is not enough.
C — Teaching value★★★★★Table 1 is reusable teaching material as it stands; the diastema → periodontitis → pulp exposure chain is the thread on which the whole subject is taught.
D — Species relevance★★★★★Entirely and only alpaca, on an unselected population: there is no extrapolation from horse or small ruminant anywhere in the results.

24 · Bottom line

24 · Bottom line

One can conclude that in the alpaca population of northern Belgium and the southern Netherlands, examined under deep sedation in 2018, 187 animals out of 228 (82.0 %) had at least one dental disorder, that cheek teeth disorders rise from 50.0 % below three years to 100 % above nine, and that diastemata, periodontal disease and occlusal pulp exposure occur together far more often than chance (ORs from 2.9 to 13.1, all with 95 % CIs excluding unity).

One cannot conclude either the frequency of tooth root abscesses — which this study did not measure and declares it did not measure — or a causal relationship between the associated disorders, or that these figures hold for a population other than the one examined; nor can one derive from it a recommended dental check interval, which the article does not propose.
