Specie e soggetti

  • 55 maschi interi, 19 maschi castrati, 51 femmine;
  • età da 3 giorni a 18 anni (6.657 giorni), mediana 3,5 anni (1.288 giorni);
  • peso corporeo da 4,3 a 110,0 kg, mediana 56,0 kg (n = 106);
  • motivi di visita più frequenti: debolezza e anoressia aspecifiche in vari gradi (26 casi), dolore addominale acuto (16), dispnea (12) e diarrea (12);

Species and subjects

Alpacas (Vicugna pacos), 125 individuals, presented as patients at the university clinic of Hannover between January 2022 and January 2023:

  • 55 intact males, 19 castrated males, 51 females;
  • age from 3 days to 18 years (6,657 days), median 3.5 years (1,288 days);
  • body weight from 4.3 to 110.0 kg, median 56.0 kg (n = 106);
  • most frequent reasons for presentation: nonspecific weakness and anorexia of various degrees (26 cases), acute abdominal pain (16), dyspnea (12) and diarrhea (12);
  • with few exceptions, all animals were hospitalized.

No llamas, no Old World camelids. It is a study on the alpaca and should be read as such. The authors themselves state that in a previous work they found similar fructosamine concentrations in the llama (Lama glama): this is their statement, reported, not a finding of this study.

The population is a hospital population, i.e. sick. The percentages that follow describe who arrives at the clinic, not alpacas farmed in Europe.

Dichiarato dagli autori: no — gli autori dichiarano invece ARRIVE: «All methods were carried out in accordance with relevant guidelines and regulations and are reported in accordance with the ARRIVE guidelines.»

Study type and level of evidence

Cross-sectional observational study on a clinical population, with correlation analysis between parameters and without a concurrent healthy control group.

Level of evidence: prospective observational study on a clinical case series, 125 alpacas, a single clinic, no control group. Consequence for the reader: the results describe observed associations in a patient population; they establish that glucose and fructosamine behave differently with respect to stress, not that fructosamine correctly identifies diabetes in the alpaca — something this study was not able to verify, and which the authors do not claim.

Relevant reporting standard: STROBE (cross-sectional observational study). Declared by the authors: no — the authors instead declare ARRIVE: «All methods were carried out in accordance with relevant guidelines and regulations and are reported in accordance with the ARRIVE guidelines.» This is an unusual declaration for a study on clinical patients rather than laboratory animals, but it is what is stated, and the card reports it as it is.

In breve

In brief

In the South American camelid, insulin resistance is physiological: less insulin is secreted than in most mammals and gluconeogenesis is more efficient. The practical consequence is that an alpaca with a disturbance of lipid metabolism tends to be hyperglycemic, while a ruminant in the same situation tends to be hypoglycemic. Added to this is the fact that the alpaca is an animal whose glycemia rises with stress. The result is that the blood glucose measured on admission to the clinic is almost always high, and says almost nothing.

This study measures, in the same 125 alpacas, glucose and fructosamine — a ketoamine formed by non-enzymatic binding of glucose to proteins that reflects the mean glycemia of the preceding 2-3 weeks — and compares them with a behavioral stress score and with serum cortisol.

The result is clean: glucose correlates with stress and with cortisol; fructosamine does not. And the inconvenient result is equally clean: fructosamine correlates even more strongly with albumin than with glucose, and in a hypoproteinemic alpaca it drops because of protein, not glycemia.

Why this study matters

Because it removes an error that in Italy is made with the first alpaca and continues to be made: reading a high blood glucose as information about the animal's disease.

In the sample, 89 of 125 alpacas (71.2%) were hyperglycemic on admission, with values up to 37.3 mmol/l. If the blood glucose of an alpaca just unloaded from the van were a signal, it would be a signal that sounds for seven animals out of ten — that is, it is not a signal.

And it matters because it provides the second number, the one that makes the first readable, together with the rule for not being fooled by that one too. Fructosamine is not a new parameter: it has been in use for decades in dogs and cats to distinguish stress hyperglycemia from diabetic hyperglycemia. This work demonstrates that in the alpaca it behaves as expected with respect to stress, and warns that it also behaves as expected with respect to protein — which, in a species in which hypoproteinemia from gastrointestinal nematodes is an everyday occurrence, is no small detail.

Obiettivo

Ipotesi dichiarata a priori: «While fructosamine should not be affected by acute stress, a significant increase of the glucose plasma concentration is anticipated.»

Objective

Declared by the authors: to evaluate fructosamine «for the application in alpacas», comparing the plasma concentrations of glucose and fructosamine with demographic data, clinical examination, laboratory parameters and a behavioral assessment of stress, with particular attention to the comparative effects of stress on the two parameters. Hypothesis stated a priori: «While fructosamine should not be affected by acute stress, a significant increase of the glucose plasma concentration is anticipated.»

Clinical question in PICO form (editorial formulation): in an alpaca presented as a patient at a veterinary clinic [P], does the measurement of plasma fructosamine [I] compared to blood glucose alone [C] allow distinguishing acute stress hyperglycemia from persistent hyperglycemia [O]?

Disegno dello studio

Study design

Observational study of all alpacas presented as patients over a thirteen-month period. For each animal, in a standardized manner: clinical examination and weighing on admission; blood sampling from the jugular vein and fecal sampling within 20 minutes of the clinical examination; body condition score (BCS) from 1 (emaciated) to 5 (obese) in increments of 0.5, by palpation of the lumbar spine.

Behavioral stress score, applied to 119 of the 125 animals (10 of the 119 excluded because they were unable to stand for pathological reasons): a leading score from 0 («cooperative») to 3 («the animal must be carried by force») over the route from the transport vehicle to the examination room, plus semiquantitative recording (0 = never, 1 = 1-3 times, 2 = more than 3 times) of vocalizations (humming, screaming, snorting, moaning), defensive behavior (rising, spitting, stamping, kicking) and signs of stress (freezing, collapse), in the first 20 minutes of the clinical examination.

Metodi / Protocollo

Fruttosamina. ⚠️ Dichiarazione degli autori da non perdere: «The assay was not separately evaluated for use in alpacas». Lo stesso vale per il cortisolo (chemiluminescenza, Immulite 1000, Siemens): «The assay was not separately evaluated for use in alpacas.»

Ematologia e biochimica.

Methods / Protocol

Fructosamine. Plasma; Cobas Mira Plus® analyzer (Roche); commercial colorimetric kit (reduction of nitroblue tetrazolium to formazan under alkaline conditions); quality control with two manufacturer control sera one to three times per working day. ⚠️ Statement by the authors not to be missed: «The assay was not separately evaluated for use in alpacas». The same applies to cortisol (chemiluminescence, Immulite 1000, Siemens): «The assay was not separately evaluated for use in alpacas.»

Hematology and biochemistry. PCV, leukocytes and neutrophil/lymphocyte ratio on EDTA with standard manual methods of the clinic; total protein, albumin and glucose in plasma; globulins calculated as the difference between total protein and albumin.

Reference intervals adopted — all from Dawson et al. 2011 (74 and 65 healthy adult alpacas, Cornell University), not established by this study:

ParameterAdult alpaca reference intervalUnit
Plasma glucose5.5 – 7.9mmol/l
Plasma fructosamine252 – 425µmol/l
Total plasma protein58 – 73g/l
Plasma albumin28 – 43g/l
PCV0.22 – 0.45l/l

Feces. Not McMaster: a clinic method combining the Baermann-Wetzel larval migration technique with two sedimentation-flotation approaches (saturated saline solution for strongyle eggs, sodium silicate for trematode eggs), on at least 10 g of weighed feces. ⚠️ The authors state that the strongyle-type egg count obtained with this method is reduced by a factor of approximately 100 compared to McMaster: the GIN score values of this study are not comparable with eggs per gram from a McMaster flotation.

Statistics. Principal component analysis (PCA, principal() command of the psych package in R) to condense the behavioral data. Two components with eigenvalue > 1.5 were retained: PC1 (25% of the variance), interpreted as «defensive behavior» (loadings: spitting 0.81, kicking 0.77, screaming 0.55); PC2 (23%), interpreted as «stress» (collapse 0.77, serum cortisol 0.71, freezing 0.60). Correlations using Pearson's method; sex effects with Mann-Whitney U; categorical variables with more than two levels with Kruskal-Wallis and pairwise Wilcoxon comparisons. All tests were repeated on adults only (from the second year of life, n = 102 in the methods; n = 105 in Tables 3 and 4 — the discrepancy is in the article, see Limitations added by the editorial team).

Risultati principali (dati)

Glucosio — disponibile per tutti e 125 gli animali.

  • 89/125 (71,2 %) sopra il limite superiore di 7,9 mmol/l; 2/125 (1,6 %) sotto il limite inferiore di 5,5 mmol/l (entrambi maschi: uno di sei mesi, uno di dieci anni).
  • Intervallo osservato 4,2 – 37,3 mmol/l, mediana 9,3 mmol/l.
  • Correlazioni positive significative con: fruttosamina (R = 0,440; p < 0,001), stress PC2 (R = 0,343; p = 0,0004), massa corporea (R = 0,300; p = 0,0018), PCV (R = 0,284; p = 0,0014), cortisolo sierico (R = 0,251; p = 0,0053), proteine totali (R = 0,199; p = 0,0274), albumina (R = 0,195; p = 0,0319).

  • Intervallo osservato 143,1 – 1.037,7 µmol/l, mediana 326,0 µmol/l.

Gli autori concludono che lo stress è l'innesco più probabile dell'iperglicemia osservata, e che la misura della fruttosamina «allows to identify single individuals with possibly permanently high glucose plasma concentration levels from the vast majority of animals with acute hyperglycemia caused by the stressful situation of transport and clinical examination».

Aggiungono un'avvertenza che è la vera consegna operativa dell'articolo: «In particular and to prevent false negative results, total protein or albumin plasma concentration should be taken into account when evaluating laboratory results.»

E aggiungono un limite di ambito che nessuno dovrebbe saltare: «the parameter alone is not capable of differentiating the type of hyperglycemia» — una fruttosamina alta è compatibile con una sindrome simil-diabetica e con uno stress cronico, e questo studio non le separa.

Osservazione comportamentale degli autori, utile e insolita: gli animali acutamente stressati «can therefore particularly be recognized by their stuporous behavior — collapsed and completely motionless».

Main results (data)

Glucose — available for all 125 animals.

  • 89/125 (71.2%) above the upper limit of 7.9 mmol/l; 2/125 (1.6%) below the lower limit of 5.5 mmol/l (both males: one six months old, one ten years old).
  • Observed range 4.2 – 37.3 mmol/l, median 9.3 mmol/l.
  • Significant positive correlations with: fructosamine (R = 0.440; p < 0.001), stress PC2 (R = 0.343; p = 0.0004), body mass (R = 0.300; p = 0.0018), PCV (R = 0.284; p = 0.0014), serum cortisol (R = 0.251; p = 0.0053), total protein (R = 0.199; p = 0.0274), albumin (R = 0.195; p = 0.0319). Significant negative correlation with body temperature (R = −0.243; p = 0.0068).
  • In adults only, the correlations with total protein (p = 0.070) and body temperature (p = 0.072) lose significance.

Fructosamine — available for all 125 animals.

  • 93/125 (74.4%) within the reference interval 252-425 µmol/l; 13/125 (10.4%) above; 19/125 (15.2%) below.
  • Read against the other denominator, which is the one that matters clinically: of the 89 hyperglycemic alpacas, only 13 (14.6%) also had fructosamine above the reference range. The other 76 were hyperglycemic without a long-term correlate.
  • Observed range 143.1 – 1,037.7 µmol/l, median 326.0 µmol/l.
  • Significant positive correlations with: albumin (R = 0.479; p < 0.001), glucose (R = 0.440; p < 0.001), total protein (R = 0.416; p < 0.001), PCV (R = 0.379; p < 0.001), body mass (R = 0.242; p = 0.0123), globulins (R = 0.180; p = 0.0475 — significance that is lost in adults only, p = 0.517, and that is strengthened in animals under one year only, R = 0.652; p = 0.0025).
  • Males have significantly higher fructosamine than females (p = 0.0207; in adults only p = 0.0349).
  • No correlation with stress (PC2: R = 0.120; p = 0.2281) and no correlation with serum cortisol (R = 0.033; p = 0.7189). This is the central finding of the study, and it is a negative result.

What lies behind the two extremes.

GroupnWhat they had
Fructosamine below 252 µmol/l19/12517/19 hypoproteinemic, 7/19 also hypoalbuminemic. On the glycemic side: 1 hypoglycemic, 8 within range, 10 hyperglycemic
Fructosamine above 425 µmol/l13/12513/13 hyperglycemic; 3/13 (23%) hyperproteinemic, mostly with hyperalbuminemia — a proportion similar to that of the whole population (18/123, 14.6%)

Serum cortisol (n = 122). Range 2.8 – 430.0 nmol/l, median 60.8 nmol/l. Distribution: 22 animals below 20 nmol/l, 55 between 20 and 50, 26 between 50 and 100, 19 above 100 nmol/l. The authors compare these values with those reported by Anderson and colleagues (baseline mean 20.7 nmol/l in 12 alpacas, 49.3 nmol/l after short transport) — comparison reported by the authors of this study, not verified against the original by this card.

Other parameters, for completeness of the denominators. Total protein available for 123 animals: 26/123 (21.1%) below 58 g/l, 18/123 (14.6%) above 73 g/l. Albumin for 122: 11/122 (9.0%) below 28 g/l, 14/122 (11.5%) above 43 g/l. PCV for 125: 17/125 (13.6%) below 0.22 l/l; no animal above 0.45 l/l. Alpacas without shedding of gastrointestinal nematode eggs had significantly higher PCV than those with high-grade shedding (p = 0.005).

Authors' interpretation (attributed, not merged with the data)

The authors conclude that stress is the most likely trigger of the observed hyperglycemia, and that the measurement of fructosamine «allows to identify single individuals with possibly permanently high glucose plasma concentration levels from the vast majority of animals with acute hyperglycemia caused by the stressful situation of transport and clinical examination».

They add a caveat that is the article's true operational message: «In particular and to prevent false negative results, total protein or albumin plasma concentration should be taken into account when evaluating laboratory results.»

And they add a scope limitation that no one should skip: «the parameter alone is not capable of differentiating the type of hyperglycemia» — a high fructosamine is compatible with a diabetes-like syndrome and with chronic stress, and this study does not separate them.

Behavioral observation by the authors, useful and unusual: acutely stressed animals «can therefore particularly be recognized by their stuporous behavior — collapsed and completely motionless». In the PCA, collapse (0.77) and freezing (0.60) load on the same component as cortisol (0.71).

Implicazioni cliniche

Clinical implications

For the Italian veterinarian facing an alpaca that has just arrived at the practice:

  1. A high blood glucose on admission is the expected finding, not information. In the sample it was present in 89 of 125 alpacas. Before building anything on it, ask: how long did the transport last, how was the animal while being unloaded, what is it doing now.
  2. The motionless, collapsed animal is not the calm animal: it is the stressed one. In the principal component analysis, «collapse» and freezing load with cortisol, not with calmness. This is the same sign that makes pain assessment difficult in this species (see the D file).
  3. If it is necessary to know whether the hyperglycemia has lasted for weeks, request fructosamine — on the same plasma sample, without a second collection. Reference interval for the adult alpaca: 252-425 µmol/l (Dawson et al. 2011).
  4. Fructosamine is never read alone. It is read together with albumin and total protein, and in the same glance. In this study the fructosamine-albumin correlation (R = 0.479) is stronger than the fructosamine-glucose correlation (R = 0.440). An alpaca with gastrointestinal nematodes, hypoproteinemic, can have low fructosamine with high blood glucose: in the group with fructosamine below reference, 10 of 19 animals were hyperglycemic.
  5. The false negative is the real risk, not the false positive. A «normal» fructosamine in a hypoproteinemic animal may conceal persistent hyperglycemia.
  6. A high fructosamine does not make a diagnosis of diabetes. It signals that blood glucose has been high for a long time; the reason why — diabetes-like syndrome, lipidosis, chronic stress — remains open and must be investigated with other tools.
  7. The finding does not transfer to the llama without saying so. This is a study on 125 alpacas. The authors report similar concentrations in the llama in a previous work: that is their statement, not a finding here.

Limiti

Dichiarati dagli autori (citati, non riassunti):

  • Manca il valore basale individuale di cortisolo. «an individual basal value is necessary to evaluate a cortisol concentration, which was not available here»; il confronto è fatto con valori medi pubblicati da un altro gruppo.
Whether these proteins play a role in the development of fructosamine in alpacas unfortunately cannot be clarified here.»

Limitations

Declared by the authors (quoted, not summarized):

  • The individual baseline cortisol value is missing. «an individual basal value is necessary to evaluate a cortisol concentration, which was not available here»; the comparison is made with mean values published by another group.
  • Comparable follow-up examinations were not possible, because subsequent treatment and sampling varied greatly from individual to individual.
  • Acute phase proteins were not measured individually, and may contribute to fructosamine formation: «Whether these proteins play a role in the development of fructosamine in alpacas unfortunately cannot be clarified here.» The authors add that in general little is known about acute phase proteins in the alpaca.
  • It is unclear whether chronic stress could itself raise fructosamine — which, if it occurred, would precisely reduce the discriminating capacity of the parameter. The authors state this explicitly and leave it open.
  • The globulin fractions were not examined individually, which leaves the interpretation of the sex difference unresolved.

Added by the editorial team.

  • Hospital population, a single clinic, a single country. The 71.2% hyperglycemia describes the alpacas brought to Hannover, not alpacas in general. An Italian farm does not have this distribution, and no one knows which one it has.
  • No concurrent healthy control group. The reference intervals come from another population (Cornell, USA, 2011), measured with different instruments. The authors state explicitly that the fructosamine assay method used by Dawson et al. is not known: «Information on the method for fructosamine determination in alpacas in the two previously published studies … is not available.» Comparing a Cobas Mira Plus measurement with an interval obtained with an unknown method is an assumption, and this study makes it without being able to verify it.
  • The assay was not validated in the species. This applies to fructosamine and to cortisol, and the authors state so. It does not invalidate internal comparisons between animals of the same study; it makes comparison with values measured elsewhere uncertain.
  • Correlations, not diagnostic performance. There is no sensitivity, no specificity, no ROC curve, no decision threshold derived from the data. The reader looking for «above what value should I worry» will not find the answer here: they will find the upper limit of someone else's reference interval.
  • Tiny subgroup sizes where the conclusions are strongest. The two groups on which the argument rests have 19 and 13 animals. One case more or less shifts a lot. Animals under one year number 20, and correlations appear in that subgroup that are not present in adults.
  • A small internal discrepancy, which does not change the conclusions but should be noted by anyone redoing the calculations: the statistics section states that the tests were repeated on adults only with n = 102, while Tables 3, 4 and the results text refer to n = 105 individuals over one year of age (and 20 under one year; 105 + 20 = 125). The two figures do not match, and the article does not explain the difference.
  • Correlation ≠ mechanism. That glucose and stress correlate does not prove that stress causes the hyperglycemia in these animals: many were sick, and disease produces both stress and hyperglycemia.

◆ COMMENTO DELL'ASSOCIAZIONE

Camelid Research Studies APS — redazione CRL · 31/08/2026

Valutazione a stelle

AsseVotoMotivazione

What is missing relative to the reporting standard (STROBE)

  • The participant flow diagram is missing. It is not known how many alpacas were presented to the clinic during the period and how many ended up in the study: 125 is the number included, not a number over a denominator. If inclusion required a complete set of examinations, those investigated the most were selected, and this is a selection bias that the card cannot quantify.
  • The inclusion and exclusion criteria are not stated as such.
  • Confounders are not controlled for in a multivariate model: the analysis is bivariate, parameter by parameter, and the authors call it so. With eleven significant correlations interwoven with each other (glucose, fructosamine, albumin, protein, PCV, body mass), a multivariate model would say different things. No correction for multiple testing is declared, and the tests number in the dozens.
  • The origin of the animals (how many farms? how many regions?) is not reported, except as transport distance (15.2 – 385 km, median 130 km).
  • STROBE is not declared by the authors, who declare ARRIVE. This is not a reproach — it is information on the type of reading the article expects.

◆ ASSOCIATION COMMENTARY

Camelid Research Studies APS — CRL editorial team · 31/08/2026

In the alpaca fructosamine does not move with stress while glucose does: this is what separates a twenty-minute hyperglycemia from a three-week one.

The first point is the one in the title: in the alpaca fructosamine does not move with stress, while glucose does, and it therefore separates a twenty-minute hyperglycemia from a three-week one. It is a useful result, replicated in design from what is known in dogs and cats, and obtained on a sample size that for this species is large.

The second point is that the parameter that should solve the problem carries a problem of its own, and it is a problem that in this species arises continually. Fructosamine is glucose bound to protein. In a population in which hypoproteinemia from gastrointestinal nematodes is among the most frequent conditions — and in this very study 26 of 123 alpacas were hypoproteinemic — the denominator of the parameter is precisely what is missing. Seventeen of the nineteen animals with low fructosamine were hypoproteinemic, and ten of those nineteen were simultaneously hyperglycemic. A clinician who had ordered fructosamine alone in those ten animals would have read «no persistent hyperglycemia» in hyperglycemic animals.

The operational consequence is a rule for ordering tests, not for interpreting them. Fructosamine should not be ordered alone. It should be ordered as a triad: glucose, fructosamine, albumin — and total protein if the laboratory provides it anyway. They cost little, they come from the same plasma sample, and without the third the second cannot be read. This is our recommendation, not the authors': the authors say proteins «should be taken into account», we say they should be ordered together, because what is not on the form is not taken into account by anyone.

Where we would ask for more. The step that would turn this work into a tool is missing: a threshold. The study produces no sensitivity, specificity or predictive value, because it has no reference standard against which to measure them — there is no independent way of knowing which of these 125 alpacas were truly chronic hyperglycemics. The design that would fill the gap is known and within reach of a European clinic: continuous glucose monitoring with devices from human medicine on about twenty animals, and fructosamine in parallel. The authors themselves cite that technique as the «elaborate» alternative that fructosamine should replace; it is also the only way to verify that it actually does. This is a card that cannot be written today.

A note of caution on other people's numbers. This study interprets its own results against reference intervals from 2011, obtained on 74 American alpacas with an assay method that — by the authors' own admission — is not known. This is not a criticism of the authors, who state it: it is a snapshot of the state of the field. Alpaca medicine in Europe runs, for its most promising parameter, on a reference interval whose chemistry is unknown. A European reference interval for alpaca fructosamine, with a stated method, is a modest, inexpensive piece of work with no risk of a null result. It is worth more than many more ambitious projects, and no one is doing it.

Star rating

AxisScoreRationale
A · Scientific quality★★★★☆Free full text open access; n = 125 with all partial denominators declared parameter by parameter; ethical approval with code (TiHo-REC_14_04_24); funding and conflicts declared; ARRIVE declared; limitations of the article discussed explicitly and not perfunctorily, including one that weakens the authors' own thesis (chronic stress). The fifth star requires a sample size justified by a calculation and risk-of-bias assessment: these are absent here, the analysis is bivariate without correction for multiple testing, and there is no participant flow diagram.
B · Clinical utility★★★★☆The result is applicable tomorrow with an assay that many Italian laboratories already perform, has explicit conditions of use (read it together with albumin and protein) and a stated limitation (does not distinguish diabetes from chronic stress). Not five because it does not change a recommended practice: the decision threshold is missing, and no dose or protocol derives from here.
C · Educational value★★★★★It is the best existing text for understanding why in the South American camelid blood glucose cannot be read as in a ruminant, and for understanding — with data, not with a statement — that a time-integrated marker depends on its substrate as much as on the analyte. The table of the nineteen animals with low fructosamine teaches in twenty lines what a textbook chapter does not teach.
D · Relevance to the species★★★★☆Alpaca, 125 individuals, European clinic, population described by sex, age, weight, reason for presentation and transport distance: transferability to Italy is assessable and high. The fifth star requires a study that is a reference in the species taken up by subsequent literature: the work is from September 2024 and this uptake is not yet verifiable.

Bottom line

It can be concluded that in 125 alpacas presented as patients at a university clinic hyperglycemia on admission is the rule (89/125, 71.2%, up to 37.3 mmol/l) and that glucose moves with stress (PC2: R = 0.343; p = 0.0004) and with cortisol (R = 0.251; p = 0.0053) while fructosamine moves with neither (R = 0.120; p = 0.2281 and R = 0.033; p = 0.7189); that of the 89 hyperglycemics only 13 (14.6%) also had fructosamine above the reference range; and that fructosamine correlates with albumin (R = 0.479) more than with glucose (R = 0.440) — 17 of the 19 animals with low fructosamine were hypoproteinemic and 10 of the 19 were at the same time hyperglycemic.

It cannot be concluded that fructosamine identifies diabetes or distinguishes the type of hyperglycemia: the authors themselves deny it, and the study produces no sensitivity, specificity, ROC curve or decision threshold, because there is no reference standard against which to measure them. It cannot be concluded how much the comparison with the 252-425 µmol/l interval is worth: it comes from Dawson et al. 2011, an American population, with an assay method that is not known, and the assay used here, like that for cortisol, «was not separately evaluated for use in alpacas». It cannot be concluded that stress causes hyperglycemia: the analysis is bivariate, without correction for multiple testing, on animals largely sick — and disease produces both stress and hyperglycemia. It cannot be concluded that the 71.2% describes farmed alpacas: it is a hospital population from a single clinic, without a participant flow diagram or stated inclusion criteria. It remains open, by the authors' own admission, whether chronic stress itself raises fructosamine; the two groups on which the argument rests have 19 and 13 animals; and none of this is measured in the llama.

Messaggi da portare a casa

Take-home messages

  1. In the alpaca brought to the clinic, hyperglycemia is the rule: 89 of 125 (71.2%) above the reference range, up to 37.3 mmol/l. A high blood glucose on admission is not, by itself, clinical information.
  2. Glucose correlates with stress (R = 0.343; p = 0.0004) and with cortisol (R = 0.251; p = 0.0053); fructosamine with neither (p = 0.2281 and p = 0.7189). This is what makes fructosamine useful.
  3. Fructosamine correlates with albumin (R = 0.479) more than with glucose (R = 0.440). In a hypoproteinemic alpaca it falls because of protein: it should always be requested together with albumin and total protein, otherwise it produces false negatives.
  4. The acutely stressed alpaca is the motionless, collapsed one, not the agitated one: freezing and collapse load on the same component as cortisol.
  5. A high fructosamine is not a diagnosis of diabetes: the authors state that the parameter alone does not distinguish the type of hyperglycemia, and chronic stress remains an open alternative.
  6. The reference interval in use (252-425 µmol/l plasma) is American, from 2011, with an assay method that is not known. Anyone using it in Italy is importing it, and should note this on the report.
