Valutazione redazionale, quattro assi separatiEditorial rating, four separate axes
- AQualità scientifica4/5
- BUtilità clinica5/5
- CValore didattico4/5
- DRilevanza per la specie4/5
Che cosa misurano i quattro assi
- A · Qualità scientifica
- La domanda è ben posta? I metodi sono dichiarati? La statistica è appropriata? La conclusione sta dentro i dati?
- B · Utilità clinica
- Serve a chi ha l’animale davanti, oggi.
- C · Valore didattico
What the four axes measure
- A · Scientific quality
- Is the question well posed? Are the methods stated? Is the statistics appropriate? Does the conclusion stay within the data?
- B · Clinical usefulness
- Useful to whoever has the animal in front of them, today.
- C · Teaching value
- Does it teach something to those studying the subject?
- D · Relevance to the species
- Is it really about camelids, or a result borrowed from another species?
Each axis is worth 1 to 5 stars and carries its written reason. The stars in the index are the rounded average of the four.

Otto nomi su dieci compaiono in entrambi i lavori.

Complete list, ten authors, in the order and form of the Crossref record. The surname von Altrock is compound and should be written as such (chapter 14 §9.3); Großmann carries the German eszett and should not be transliterated to «Grossmann» in a citation.
⚠️ These are, with a single swap of one person, the same authors as card CRL-A-02, and in reverse order: there Kiene was first and Wagener last, here Wagener is first and Kiene third. Eight names out of ten appear in both works. This is not a criticism: it is information necessary to assess the independence of the two cards, and it is discussed below (Limits added by the editorial team).

Specie e soggetti
⚠️ Tre lama erano stati testati ma sono stati esclusi dall'analisi, perché troppo pochi: «As only data from three llamas were available, further evaluation focused on alpacas alone.»
| CMh+ (13) | CMh− (22) | |
|---|---|---|
| Sesso, secondo il testo dei risultati | 5 femmine, 8 maschi (6 interi, 2 castrati) | 6 femmine, 15 maschi (11 interi, 4 castrati) → totale 21 |
| Sesso, secondo la Tabella 2 | 5 femmine, 8 maschi | 7 femmine, 15 maschi → totale 22 |
| Cria (< 1 anno) | 1 maschio | 3 maschi |
| Età (giorni) | intervallo 43-4.056; media 1.611,2 ± 1.185,6; mediana 1.655 | intervallo 206-5.013; media 1.917,9 ± 1.504,6; mediana 1.515 |
| Peso (kg) | 12,4-77,5 (n = 12) | 10,4-102,0 (n = 22) |
Species and subjects
35 alpacas (Vicugna pacos) from 23 different farms, all tested by PCR for Candidatus Mycoplasma haemolamae at the Hannover university clinic between January 2021 and January 2024.
- 13 positive (37.1%), from 11 different farms — group CMh+.
- 22 negative — group CMh−.
⚠️ The 37.1% is not a prevalence and must not be cited as such. The authors state this: the PCR was performed only when suspicion arose — presence of other positive animals in the herd, or an anemic condition of the animal. It is a fraction of positives in a population selected for suspicion, within a population already selected (animals admitted to a referral clinic). The true prevalences reported in the literature for European South American camelids — cited by the authors in the introduction and not verified by this card against the originals — are 18.7% out of 225 alpacas and llamas in Germany and Switzerland, 18.6% in Switzerland, 17.6% in llamas and 31.7% in alpacas in Austria, 29.0% in a British herd of 131 alpacas, 0.97% (2/206) in New Zealand, 66.7% (14/20) in a first Italian finding in 2022, and 35.3% in Finland.
⚠️ Three llamas had been tested but were excluded from the analysis, being too few: «As only data from three llamas were available, further evaluation focused on alpacas alone.» This is a study on alpacas only.
Group composition — and here the editorial team must flag an internal discrepancy:
| CMh+ (13) | CMh− (22) | |
|---|---|---|
| Sex, according to the results text | 5 females, 8 males (6 intact, 2 castrated) | 6 females, 15 males (11 intact, 4 castrated) → total 21 |
| Sex, according to Table 2 | 5 females, 8 males | 7 females, 15 males → total 22 |
| Cria (< 1 year) | 1 male | 3 males |
| Age (days) | range 43-4,056; mean 1,611.2 ± 1,185.6; median 1,655 | range 206-5,013; mean 1,917.9 ± 1,504.6; median 1,515 |
| Weight (kg) | 12.4-77.5 (n = 12) | 10.4-102.0 (n = 22) |
⚠️ The text says «six females» and Table 2 says «seven»; the text's count adds up to 21 animals out of 22. Table 2 correctly closes at 22 and is internally consistent (12 females and 23 males in total out of 35). This card uses the Table 2 numbers and flags the discrepancy because whoever redoes the count will encounter it. It changes no conclusion, and indeed the comparison by sex is not significant (p = 0.7260).
Gli autori lo dicono: «The extent to which CMh influenced the course of the disease cannot be assessed due to the small number of animals.»
Standard di reporting pertinente: STROBE (studio osservazionale caso-controllo). Dichiarato dagli autori: no — l'articolo non nomina alcuna griglia.
Study type and level of evidence
Single-center retrospective observational case-control study, on clinical records, with groups defined by PCR result and comparison of all clinical and laboratory parameters between the two groups.
Level of evidence: retrospective observational study, a single clinic, 35 animals, groups of 13 and 22, population selected for suspicion. Consequence for the reader: the work describes how similar an alpaca admitted PCR-positive is to one admitted PCR-negative. Its result is negative, and negative results have an inherent limitation: with 13 and 22 animals statistical power is low, and «no significant difference» is not «no difference». The authors say so: «The extent to which CMh influenced the course of the disease cannot be assessed due to the small number of animals.»
Relevant reporting standard: STROBE (observational case-control study). Declared by the authors: no — the article names no checklist. It should be recognized, however, that the reporting of statements (ethics with code, funding, conflicts, author contributions, data availability) is complete, and that the 2×2 contingency tables are published in full — something card CRL-A-04 could not say and which made it impossible there to recheck the main result. Here every number in this card can be rechecked in two minutes.
In breve
La conclusione degli autori è di una sobrietà rara: «antibiotic treatment based only on a positive test result should be critically questioned in the light of our results.»
In brief
Candidatus Mycoplasma haemolamae is a wall-less bacterium that lives on and among the erythrocytes of South American camelids. It is transmitted by blood — vaccinations, shearing, and probably vertically — is widespread in Europe, and for decades has carried with it a conviction: that it is an important cause of anemia in the alpaca, and that a positive result should be treated with oxytetracycline.
This study takes 13 admitted PCR-positive alpacas and 22 PCR-negative ones from the same clinic, and compares them on twenty-seven clinical and hematological parameters and twenty-three categorical findings.
The result is that they cannot be distinguished. The only three significant differences all go in the direction opposite to the one expected: the negative animals were thinner (emaciation 17/22 versus 3/12; p = 0.0048), had a lower BCS (median 1.5 versus 2.75; p = 0.0145) and had more poikilocytosis (16/22 versus 4/13; p = 0.0322). Anemia was more frequent in the negatives — 14/22 versus 4/13 — and the difference does not reach significance (p = 0.0858).
And there is a second result, which applies to the slide of every clinic: the «basophilic stippling» in erythrocytes, looked at as a sign of mycoplasma, was present in 4 of the 13 positives and in 4 of the 22 negatives. Nine of thirteen positives did not have it; four of twenty-two negatives did.
The authors' conclusion is of a rare sobriety: «antibiotic treatment based only on a positive test result should be critically questioned in the light of our results.»
Why this study matters
Because it dismantles, with data and without controversy, a chain of reasoning that in Italy is still the norm: anemic alpaca → smear with dots → or positive PCR → diagnosis of mycoplasmosis → oxytetracycline.
Every link of that chain is called into question by this work:
- The blood smear is not a test. Sensitivity and specificity of basophilic stippling against PCR, calculated by this editorial team from Table 3 of the article (the authors do not calculate them): sensitivity 4/13 = 30.8%, specificity 18/22 = 81.8%, positive predictive value 4/8 = 50.0%, negative predictive value 18/27 = 66.7% — in a population with 13 positives out of 35. A test that, when positive, has a one-in-two chance of being wrong, and that when negative misses seven out of ten.
- Positivity is not disease. There was no difference in outcome (discharged 12/13 CMh+ versus 17/22 CMh−; p = 0.3771), nor in length of hospitalization, nor in almost any other parameter.
- Anemia, in this case series, had another name: Haemonchus contortus. The most frequent diagnosis after CMh was haemonchosis (10/35), and those ten animals had the lowest PCVs in the case series (0.05-0.15 l/l).
- Treatment does not sterilize. In the one animal retested, after five days of oxytetracycline the smear was clean and the PCR was still positive.
And it matters for a reason of antibiotic resistance that goes beyond the alpaca: it is a study that, on its own data, asks that an antibiotic not be given on the basis of an isolated positive test.
Obiettivo
Dichiarato dagli autori: «The aim of this study was to provide an overview of the clinical and hematological findings of hospitalized alpacas with CMh infection that was confirmed by PCR. Moreover, the findings from these animals with a group of alpacas that revealed a negative PCR result to identify existing differences of infected and non-infected animals were compared.»
Objective
Stated by the authors: «The aim of this study was to provide an overview of the clinical and hematological findings of hospitalized alpacas with CMh infection that was confirmed by PCR. Moreover, the findings from these animals with a group of alpacas that revealed a negative PCR result to identify existing differences of infected and non-infected animals were compared.» (The sentence is syntactically imperfect in the original and is reported as is.)
Question in PICO form (editorial formulation): in an alpaca admitted to a veterinary clinic [P], does PCR positivity for Candidatus M. haemolamae [I] versus negativity [C] associate with a distinguishable clinical or hematological picture, or a different outcome [O]?
Metodi / Protocollo
| PCV | microematocrito centrifugato, 10 minuti a 10.000 × g |
| Emoglobina | fotometrica, soluzione cianurica |
| MCHC | calcolato: Hb [g/l] / PCV [l/l] |
| Formula leucocitaria | microscopica su striscio colorato secondo Pappenheim, a 1000 ingrandimenti |
|---|---|
| 0 | l'alterazione non compare in nessun campo |
| 1 | compare solo occasionalmente, non in ogni campo |
| 2 | compare in 1-3 eritrociti in ogni campo |

Methods / Protocol
Data source. Clinical records of the Clinic for Swine, Small Ruminants, Forensic Medicine and Ambulatory Service of the University of Veterinary Medicine Hannover, January 2021 - January 2024.
The reference test. Real-time PCR on EDTA blood, performed by external laboratories — SYNLAB.vet GmbH (Augsburg) or LABOKLIN (Bad Kissingen) — both accredited under DIN EN ISO 17025:2018. That the laboratory is stated and accredited is, as card CRL-A-01 teaches, part of the data.
Hematology — and this is the part to be read alongside card CRL-A-01, because here there is no automated analyzer and no species setting:
| Parameter | How it was obtained |
|---|---|
| Leukocytes | microscopic count in a Neubauer chamber, after lysis of 100 µl of EDTA blood in 900 µl of 3% acetic acid for 5 minutes |
| PCV | centrifuged microhematocrit, 10 minutes at 10,000 × g |
| Hemoglobin | photometric, cyanide solution |
| MCHC | calculated: Hb [g/l] / PCV [l/l] |
| Leukocyte differential | microscopic on a smear stained by Pappenheim, at 1000× magnification |
| Normoblasts | counted and reported per 100 leukocytes |
| Reticulocytes | microscopic, per 1,000 erythrocytes, smear stained with brilliant cresyl blue — not performed routinely |
| Erythrocyte count, MCV, MCH | not available: the authors state that a manual erythrocyte count was not performed |
⚠️ It is exactly the procedure that the authors of card CRL-A-01 recommend — centrifuged PCV, measured hemoglobin, calculated indices — with the difference that here, lacking the erythrocyte count, only MCHC can be calculated, not MCV and MCH. The authors state this as a limitation, and it is a real one: without MCV an anemia cannot be classified by corpuscular volume.
Erythrocyte morphology, with a 0-to-4 score stated in full — reported because it is reusable in any laboratory:
| Grade | Authors' definition |
|---|---|
| 0 | the change does not appear in any field |
| 1 | appears only occasionally, not in every field |
| 2 | appears in 1-3 erythrocytes in every field |
| 3 | appears in more than 3 erythrocytes in every field |
| 4 | appears in more than 50% of erythrocytes |
Also recorded as yes/no: basophilic stippling (a clue to mycoplasma), Howell-Jolly bodies, Cabot rings; and abnormal erythrocyte shapes as free text.

Definition of anemia: PCV < 0.22 l/l, according to Dawson et al.; severity classified according to Franz and Wittek. ⚠️ There is no published reference interval for the lymphocyte-to-monocyte ratio (LMR) in the alpaca, and the authors state this: they report it as description, without a threshold.
Fecal examination. Not McMaster: the same clinic method described in card CRL-A-02 — Baermann-Wetzel larval migration plus two sedimentation-flotations (saturated saline for strongyle eggs, sodium silicate for trematodes), on at least 10 g of weighed feces, with classification of infestation as low, medium or high grade according to Neubert et al. ⚠️ The conversion factor is not stated in this article, but it is in the other work by the same group covered in this issue (CRL-A-02): with this method, the strongyle-type egg count comes out reduced by a factor of about 100 relative to McMaster. The eggs-per-gram values of this study are not comparable to those of a McMaster flotation, and anyone reading them as such underestimates the infestation by two orders of magnitude.
Statistics. Excel (Microsoft Office 365) and SAS Enterprise Guide 7.1. Normality with Shapiro-Wilk; comparison between groups with Student's t-test (normal data) or Mann-Whitney U (non-normal); categorical data with two-tailed Fisher's exact test — the tailedness is stated, and it is worth saying so because it is precisely what card CRL-A-04 lacks. Significance p < 0.05. No correction for multiple comparisons is stated.
Euthanasia protocol, stated in the methods. ⚠️ This is not a therapy and must not be read as such: it is the protocol under which the euthanasias in this case series were performed. Pentobarbital 83-122 mg/kg body weight, intravenously in the jugular vein, alpaca (Euthadorm® 500 mg/ml, CP-Pharma Handelsgesellschaft mbH, Burgdorf, Germany). Anemia, in cifre assolute.
| Alterazione | CMh+ presente | CMh− presente |
|---|---|---|
| Anisocitosi | 13/13 (gradi: 10 di grado 1, 1 di grado 2, 2 di grado 3) | 22/22 (11 di grado 1, 5 di grado 2, 6 di grado 3) |
| Poichilocitosi | 4/13 (2 di grado 1, 1 di grado 2, 1 di grado 3) | 16/22 (7 di grado 1, 3 di grado 2, 6 di grado 3) |
| Policromasia | 8/13 | 13/22 |
| Punti basofili | 4/13 | 4/22 |
Il testo dei risultati scrive: «with means of 14,7 ± 9,5 days (mean ± SD; CMh+) and 13,5 ± 7,1 days (mean ± SD; CMh−)».
| Voce | Dato, come dichiarato nell'articolo |
|---|---|
| Principio attivo | ossitetraciclina |

| Prodotto | Tetroxy® Vet 200 mg/ml, Dechra Veterinary Products Deutschland GmbH, Aulendorf, Germania |
| Esito molecolare | PCR ANCORA POSITIVA |

- Sul risultato centrale: «No clinical or hematological relevant differences between CMh+ and CMh− alpacas were found in our study. The clinical symptoms in the animals presented were caused by various comorbidities.»
- Sul ruolo del patogeno nell'anemia: «Even if CMh infects the erythrocytes, the pathogen does not appear to be a significant cause of anemia. Haemonchus contortus is instead considered to be the main reason for anemia in South American camelids.»
- Sul vetrino: «Diagnosis of CMh based solely on microscopic examination of a blood smear does not appear to be sufficient. Instead, PCR should be used to confirm or rule out infection.» E, sui punti basofili in entrambi i gruppi: «This suggests, on the one hand, that light microscopy alone will not detect mycoplasma in every infected animal and, on the other hand, that there is a risk of false-positive results.»
- Sulla terapia — ed è la frase che dà il titolo a questa scheda: «Although our study cannot exclude that CMh might lead to relevant clinical disease, antibiotic treatment based only on a positive test result should be critically questioned in the light of our results.»

Anemia, in absolute figures. 18 of 35 alpacas were anemic (PCV < 0.22 l/l). Two CMh+ animals had a PCV of 0.05 l/l each; five CMh− animals had PCVs from 0.04 to 0.07 l/l. The 10 animals with haemonchosis (out of 35) had the lowest PCVs in the case series: range 0.05-0.15 l/l, classifiable as severe anemia. Two of the two animals with PCV 0.05 l/l and haemonchosis were also CMh-positive, and the authors state that the two contributions cannot be separated — but they add that two other animals with haemonchosis and without CMh did not survive.
Erythrocyte morphology by grade (Table 4 of the article):
| Change | CMh+ present | CMh− present |
|---|---|---|
| Anisocytosis | 13/13 (grades: 10 grade 1, 1 grade 2, 2 grade 3) | 22/22 (11 grade 1, 5 grade 2, 6 grade 3) |
| Poikilocytosis | 4/13 (2 grade 1, 1 grade 2, 1 grade 3) | 16/22 (7 grade 1, 3 grade 2, 6 grade 3) |
| Polychromasia | 8/13 | 13/22 |
| Basophilic stippling | 4/13 | 4/22 |
| Howell-Jolly bodies | 0/13 | 2/22 |
| Cabot rings | 1/13 | 5/22 |
No animal, in either group, reached grade 4. The animals with the highest grades of erythrocyte change had PCV < 0.1 l/l. The most frequent shape changes were dacrocytes and spindloid cells.
Outcome.
- Discharged: 12/13 CMh+ and 17/22 CMh− (p = 0.3771).
- CMh+: one animal euthanized — haemonchosis, deterioration after severe anemia, PCV 0.05 l/l.
- CMh−: two euthanized (one for severe anemia with PCV 0.05 l/l, one for patellar luxation) and three deaths (pneumonia, cachexia, anemia of unknown cause).
- Length of hospitalization: ranges 1-25 days (CMh+) and 2-35 days (CMh−).
⚠️ Internal discrepancy in the hospitalization means, to be flagged for anyone citing this. The results text reads: «with means of 14.7 ± 9.5 days (mean ± SD; CMh+) and 13.5 ± 7.1 days (mean ± SD; CMh−)». Table 1 attributes 14.7 ± 9.5 to the CMh− group (n = 19, median 13) and 13.5 ± 7.1 to the CMh+ group (n = 9, median 15). The two means are swapped between text and table. The ranges, however, coincide in both places. The difference is not significant (p = 0.7098) and no conclusion changes; but a number copied from the text and a number copied from the table are attributed to opposite groups → to be checked against the full text with the authors. Note also that the hospitalization denominators (19 and 9) are not 22 and 13: the datum is missing for seven animals, and the article does not say which or why.
⚠️ A unit of measure that does not add up, in the reticulocyte row. Table 1 heads the row «Reticulocytes — % of RBC» with reference 0-2.4, and reports for the CMh+ group (n = 3) values 0, median 2, maximum 98, and for the CMh− group (n = 13) mean 120.7 ± 190.8, median 89, range 3-715. The results text gives for the same three CMh+ animals the values «0%, 0.2% and 9.8% of erythrocytes», and the methods state that reticulocytes are counted per 1,000 erythrocytes. The three numbers coincide only when the table is read as per thousand: 0 → 0%, 2 → 0.2%, 98 → 9.8%. Table 1's «% of RBC» header does not match the data the column contains, which are expressed per 1,000 erythrocytes. Anyone reading that row as percentages attributes to an alpaca 71.5% reticulocytes instead of 7.15%. → to be checked against the full text with the authors. (It is the same kind of error — a unit lost along the chain — that in card CRL-A-03 turned a cell count into a protein concentration.)
Reticulocytes, for what it is worth. Not performed routinely: available for 3 CMh+ (0%, 0.2%, 9.8% of erythrocytes) and for 13 CMh−, in all 13 of which they were present, and in 6 of the 13 above 10% of erythrocytes. With three animals on one side and thirteen on the other, the comparison (p = 0.1054) means nothing, and the card does not use it.
The treatment — the only dosage in this issue, with everything needed
⚠️ This is a single case within an observational study, not a validated protocol. The authors report it because it is the only animal retested during hospitalization.

What this single case demonstrates and what it does not. It demonstrates that in one alpaca, after that course, the smear became negative and the PCR did not. It does not demonstrate that oxytetracycline is effective, nor that it is ineffective, nor that that dose is the correct one, nor that treatment is needed at all: it is one animal, with no control, no randomization, no attributable clinical outcome — the animal was admitted for lameness. Its usefulness is negative and didactic: it shows that a smear becoming negative is not recovery, and that anyone checking the efficacy of a therapy by looking at the smear is looking at the wrong thing.
Authors' interpretation (attributed, not merged with the data)
- On the central result: «No clinical or hematological relevant differences between CMh+ and CMh− alpacas were found in our study. The clinical symptoms in the animals presented were caused by various comorbidities.»
- On the pathogen's role in anemia: «Even if CMh infects the erythrocytes, the pathogen does not appear to be a significant cause of anemia. Haemonchus contortus is instead considered to be the main reason for anemia in South American camelids.»
- On the smear: «Diagnosis of CMh based solely on microscopic examination of a blood smear does not appear to be sufficient. Instead, PCR should be used to confirm or rule out infection.» And, on basophilic stippling in both groups: «This suggests, on the one hand, that light microscopy alone will not detect mycoplasma in every infected animal and, on the other hand, that there is a risk of false-positive results.»
- On therapy — and it is the sentence that gives this card its title: «Although our study cannot exclude that CMh might lead to relevant clinical disease, antibiotic treatment based only on a positive test result should be critically questioned in the light of our results.»
- On the higher BCS in positives, the authors give the explanation that defuses their own result: among the CMh+ there were clinically healthy animals, brought to the clinic as companions of another admitted animal from the same herd, in which positivity was an incidental finding. Three of the five CMh+ whose only diagnosis was mycoplasmosis were exactly these.
- On morphological differentiation: erythrocyte inclusion bodies must be distinguished carefully — basophilic stippling (ribosome aggregates) and Howell-Jolly bodies (nuclear remnants) are located only inside the erythrocyte, whereas hemotropic mycoplasmas are found inside, on and among erythrocytes. They recommend fresh smears and fresh stains, because mycoplasmas detach and stain precipitates mimic the pathogen.
Implicazioni cliniche
Clinical implications
For the Italian veterinarian facing an anemic alpaca or a positive PCR:
- A positive PCR for CMh in a sick alpaca does not explain the illness. In this case series the positives did not differ from the negatives on any clinical or hematological parameter, except three — and in all three the negatives fared worse.
- Before attributing anemia to mycoplasma, look for Haemonchus. In the case series haemonchosis was the most frequent diagnosis after CMh (10/35) and concentrated the lowest PCVs (0.05-0.15 l/l). Feces, FAMACHA©, protein: card CRL-A-02 shows, at the same hospital, how much hypoproteinemia from nematodes weighs in this species.
- The «dots» on the smear do not make a diagnosis, in either direction. Calculated from this case series by this editorial team: sensitivity 30.8% (4/13), specificity 81.8% (18/22), positive predictive value 50% (4/8), negative predictive value 66.7% (18/27). Seven of ten positives have no dots; half the smears with dots come from PCR-negative animals.
- If knowing matters, do the PCR on EDTA blood, at an accredited laboratory. This is what the authors did, and it is what they recommend.
- But «knowing» is not the same as «treating». The authors' recommendation is to critically question an antibiotic decided on positivity alone. In a country with a national plan against antibiotic resistance, this is the part of the article that weighs the most.
- If treating, efficacy is not checked with the smear. In the one animal retested, after oxytetracycline 20 mg/kg subcutaneous for five days the smear was clean and the PCR was still positive. A negative smear measures visible load, not eradication.
- Watch for erythrocyte inclusions in an alpaca: basophilic stippling and Howell-Jolly bodies sit inside the cell, mycoplasmas also on and among cells; stain precipitates mimic them. Fresh smear and fresh stain are needed, along with a reader trained on this species.
- No alpaca in this case series, positive or negative, had eosinophilia or basophilia (0/13 and 0/22). It is a useful background datum for anyone reading alpaca blood counts.
- Anisocytosis is not a discriminating finding in the hospitalized alpaca: it was present in 35 of 35 animals. Poikilocytosis, on the other hand, tracks severe anemia — the high grades were in animals with PCV < 0.1 l/l — not mycoplasma.
- PCV is measured in a centrifuged capillary, as in this study and as card CRL-A-01 recommends. Here no automated analyzer was used: leukocytes in a Neubauer chamber, PCV in a microhematocrit, hemoglobin by photometry, MCHC calculated. It is a path feasible in any equipped clinic, and it does not have the species-setting problem.
Limiti
Dichiarati dagli autori (citati, non riassunti):
- Durata dell'infezione ignota: «As the animals presented to the clinic had often been ill for a long time, it was not known how long the CMh infections had been present at the time of presentation. This has to be considered a limitation of this retrospective study. It cannot be excluded that the animals had already passed through an acute phase associated with other clinical signs.»
- Numerosità piccola: «The relatively small number of animals considered in this study represents a further limitation of the results.»

«The authors declare no competing interests.»
Approvazione etica: dichiarata, con codice. Limitations
Stated by the authors (quoted, not summarized):
- Unknown duration of infection: «As the animals presented to the clinic had often been ill for a long time, it was not known how long the CMh infections had been present at the time of presentation. This has to be considered a limitation of this retrospective study. It cannot be excluded that the animals had already passed through an acute phase associated with other clinical signs.» This is the heaviest limitation: if the acute phase had already passed, a study on chronic animals could not have seen it.
- Small sample size: «The relatively small number of animals considered in this study represents a further limitation of the results.»
- Upstream selection: PCR was performed only on suspicion, so the proportion of anemic animals is necessarily high and the fraction of positives is not a prevalence.
- Contribution of mycoplasma not separable from haemonchosis: only two animals had both conditions.
- Plasma iron not measured, so the observed hypochromia remains of uncertain origin.
- Manual erythrocyte count not performed, so MCV and MCH are not assessable.
- Diagnosis of haemonchosis largely presumptive: larval differentiation according to Colditz was performed only on the feces of two animals; in the others the diagnosis was made on elevated egg count plus anemia, «which is a common practice in small ruminant and camelid medicine».
Added by the editorial team.
- This is a negative study with low power, and it must be written alongside every «not significant». With 13 and 22 animals, even clinically important differences would remain invisible. No power calculation is stated, and no confidence interval appears anywhere in the article — neither for means nor for proportions. A proportion of 4/13 has a confidence interval covering half the space of possible values.
- A very high number of tests without correction for multiple comparisons. Between Table 1 (27 continuous-variable comparisons), Table 2 (11 contingency tables) and Table 3 (23 contingency tables) the tests stated are more than sixty, at a threshold of p < 0.05 with no adjustment. Expected from chance alone: about three significant results. The observed significant results are three (emaciation, BCS, poikilocytosis). The card is not saying they are chance findings — emaciation has p = 0.0048 and is consistent with BCS, which measures the same thing, and poikilocytosis is consistent with severe anemia — but a reader must know this, and the article does not say so.
- The two «strong» significant results measure the same phenomenon twice. Emaciation (yes/no) and BCS (numeric) are not independent findings: they are the same body-condition assessment expressed on two scales. The work presents them as two results.
- The positive group contains healthy animals. The authors state this and use it as an explanation, but the methodological consequence must be spelled out in full: the CMh+ group is not a group of animals sick with mycoplasmosis, it is a group of carriers, some of whom arrived at the clinic as companions. The «positive versus negative» comparison is therefore partly a comparison between less sick and more sick animals, and it points in the direction that alone explains the three observed significant results. It is the study's central confounder, and it is not addressed by any adjusted analysis.
- Internal discrepancies verified by this editorial team, all flagged above: the hospitalization means swapped between text and Table 1; the sex count in the text (21) differing from Table 2 (22); the unit in the reticulocyte row («% of RBC») incompatible with the data in the column and with the stated method (per 1,000 erythrocytes). None changes a conclusion; all change a copied number.
- Variable and unexplained denominators. Length of stay exists for 19 + 9 = 28 of 35 animals; weight for 34; temperature, BCS and emaciation for 34; heart rate for 31; FAMACHA© for 30; conjunctivae for 30; reticulocytes for 16. The article does not say which animals are missing or why, nor does it state how missing values were handled in the tests.
- This card and CRL-A-02 are not two independent sources. Same clinic (Hannover), eight authors in common out of ten, overlapping periods (here January 2021 - January 2024, there January 2022 - January 2023), same reference intervals (Dawson et al. 2011), same fecal method and — notably — the same ethics approval code:
TiHo-REC_14_04-24here,TiHo-REC_14_04_24in CRL-A-02 (the punctuation differs, the code is the same). It is plausible that some of the animals appear in both studies, and this card cannot say how many → to be checked against the full text. Anyone citing the two cards as mutual confirmation is counting the same alpacas twice. - A German hospital population, not an Italian farm. The same caveat as CRL-A-02 applies: the proportions describe those who reach Hannover.
- PCR is a qualitative test here. No load is reported (Ct value or copies/ml), so it cannot be known whether the «weakly positive» and the «strongly positive» behaved differently — which is precisely the hypothesis that would explain why some animals fall ill and others do not. This is the most obvious follow-up study, and it has not been done.
- Only one animal retested after therapy. With one animal nothing can be concluded about efficacy: only what was seen in that animal can be concluded.
Funding: stated. «Open Access funding enabled and organized by Projekt DEAL. We acknowledge financial support from the Open Access Publication Fund of the University of Veterinary Medicine Hannover, Foundation.» Crossref records two funders: Open Access Publication Fund of the University of Veterinary Medicine Hannover, Foundation and Stiftung Tierärztliche Hochschule Hannover (TiHo). This is coverage of open-access publication costs and institutional funding: no industry sponsor declared, and no test or antibiotic manufacturer among the funders — which, for an article recommending treating less, is pertinent information.
Conflicts of interest: stated. «The authors declare no competing interests.»
Ethics approval: stated, with code. Research Ethics Committee of the University of Veterinary Medicine Hannover, approval code TiHo-REC_14_04-24, with the clarification that only retrospective data from samples taken for clinical-therapeutic reasons in the clinic's patients were used.
Author contributions and data availability: both stated. Contributions are attributed by name (M.G.W. design and drafting; F.K. and J.B.S. statistical support; T.G. and A.P. laboratory diagnostics; M.G. supervision as laboratory head). Data: «The original contributions presented in the study are included in the article» — that is, there is no archive of individual data, and verification is possible only within the limits of the published tables.

Camelid Research Studies APS — redazione CRL · 31/08/2026
Valutazione a stelle
What is missing relative to the reporting standard (STROBE)
- No confidence interval, anywhere in the article, for any proportion or any difference between means. With groups of 13 and 22, this is the gap that most affects the reading.
- No power calculation nor discussion of power, in a study whose main result is negative. A negative result without a stated power does not distinguish «no difference» from «we did not see it».
- No correction for multiple comparisons, and no statement of the choice not to make one, across more than sixty tests.
- No adjusted analysis for the confounder the authors themselves identify (the presence of healthy animals among the positives).
- No flow diagram: it is not known how many camelids were admitted over the three years, nor how many had a CMh suspicion, nor by what criterion the PCR was decided. 35 is the number tested, not a number over a denominator — unlike card CRL-A-04, where the hospital denominator is present.
- Missing data not described: variable denominators with no explanation or stated strategy.
- No quantitative PCR data (Ct or load), even though the test allows for it.
- No follow-up after discharge: it is not known what happened to the 29 discharged animals.
- Three internal discrepancies between text and tables, listed above.
◆ ASSOCIATION COMMENTARY
Camelid Research Studies APS — CRL editorial team · 31/08/2026
Across twenty-seven continuous variables and twenty-three categorical findings, between positive and negative alpacas for the pathogen no difference attributable to the pathogen emerged.
The authors searched across twenty-seven continuous variables and twenty-three categorical findings for a difference between alpacas positive and negative for a pathogen everyone calls a cause of anemia. They did not find one. The three differences they did find say that it was the negatives who fared worse: thinner, with lower BCS, with more poikilocytosis — and, without reaching significance, more anemic (14 of 22 versus 4 of 13). Then they went to see what the more anemic animals had, and found Haemonchus contortus: ten animals out of thirty-five, with the lowest PCVs in the case series.
A published negative result is a rare thing and it should be said. In a tiny field, where every group needs to publish and where a pathogen with an exotic name makes a better title than a nematode everyone knows, these authors published that the exotic pathogen makes no difference and that the nematode is at fault. And they closed by asking for less antibiotic use. We find no evidence that anyone pays them to say it: the declared funders are their university's open-access fund and the same university's foundation.
The thing that struck us most, however, is the pair of numbers on basophilic stippling: 4 of 13 and 4 of 22. The authors comment on it in one sentence and move on. We turned it into the four numbers needed in the clinic — sensitivity 30.8%, specificity 81.8%, positive predictive value 50%, negative predictive value 66.7% — and they must be read together: looking at the dots on an alpaca smear is, as a test, barely better than a coin toss when it is positive and nearly useless when it is negative. Nine of thirteen positives had no dots. Four of twenty-two negatives had them. It is a gesture performed in every clinic, free, in two minutes, and on which therapeutic decisions have been made for thirty years. (The four numbers are a calculation of ours on their Table 3, not a result of the article, and we mark them as such: they apply to this population, with 13 positives out of 35.)
Where we are stricter than the authors. The positive group contains clinically healthy alpacas, brought to the clinic as travel companions of another animal from the herd. The authors write this and use it to explain why the positives had a better BCS. We say the more uncomfortable thing: then the comparison is not between infected and uninfected, it is partly between less sick and more sick animals, and the three observed significant results follow that gradient. It does not invalidate the conclusion — if anything it reinforces it in the direction the authors take it — but it means this study never really measured the effect of mycoplasma. It measured that, in a hospital, a positive swab does not identify a sick animal.
And we are stricter on a second point: power. Thirteen animals against twenty-two, more than sixty tests, no confidence interval anywhere in the article, no power calculation. Chapter 14 §5.2 of this series asks for the denominator alongside every proportion, and here the denominators are all present — it is an honest article. But «no significant difference» with thirteen animals is not «no difference», and it is the sentence that will be cited. We cite it too, with its attached condition.
The treatment figure, and why it is the only one in this issue. Oxytetracycline 20 mg/kg subcutaneous for five consecutive days in one two-year-old male alpaca: clean smear, PCR still positive, discharged after nine days. We wrote it with dose, route, species, product, duration, source and outcome, because that is how this series' rule wants a dose to be written — and because the outcome is the part usually lost. One animal does not found a protocol. What this animal teaches is one thing, and it must be learned: a smear that clears is not an infection that ends. Anyone checking a therapy by looking at the smear is giving themselves a reassuring answer to a question they did not ask.
A methodological point we must raise against ourselves, not the authors. This card and CRL-A-02 come from the same clinic, share eight of ten authors, overlapping periods and the same ethics approval code. They are not two independent confirmations: they are two views of the same hospital, and perhaps in part of the same animals. We write this because a series that summarizes one source per card runs exactly this risk — that two cards look like two proofs — and because chapter 14 obliges us to state what weakens our own work.
Where we ask for more, and this time the question is precise. This PCR was used as a qualitative test. No Ct value, no load. Yet the hypothesis that would explain everything — why most carriers are fine and some are not — is that how much pathogen there is matters, not whether it is there. A study reporting load together with PCV, egg count and BCS, on the same case series these authors have already collected, could be done without touching a single additional animal. It is the same kind of request we wrote in CRL-A-01, in CRL-A-03 and in CRL-A-04: the data exist, what is missing is the step that makes them decisive.
Our operational position, for members. Facing an anemic alpaca: feces, FAMACHA© and protein first; mycoplasma after. If one wants to know about mycoplasma, PCR on EDTA at an accredited laboratory, not the dots on the smear. A positive result is not, on its own, an indication to treat: the authors say so and we share it, in a country with a national plan on antibiotic resistance. If treatment is decided upon anyway, the record should state why it was decided, and the outcome should not be checked with the smear.
Star rating
Axis Score Rationale A · Scientific quality ★★★★☆ Observational study with groups defined by an objective reference test performed at ISO 17025-accredited laboratories, hematological methods described down to the reagent and the spin count, morphological scale published in full, all 2×2 contingency tables published, statistical tests named with the tailedness stated, ethics approval with code, funding and conflicts declared, author contributions attributed by name, and limitations discussed extensively, including the one that undermines the study's own case group (clinically healthy positives). This is the ★★★★☆ level of the grid: limitations discussed, funding and conflicts declared. It does not reach five because the fifth star requires a sample size justified by a calculation and a bias-risk assessment: neither is here, all confidence intervals are missing, the flow diagram and the handling of missing data are missing, there is no correction across more than sixty tests, and the editorial team has verified three internal discrepancies between text and tables. B · Clinical utility ★★★★★ Five stars, and we justify it because it is the only five on this axis in the issue. The grid's criterion is «changes a recommended practice, with evidence proportionate to the change required». The practice is: treating an alpaca with oxytetracycline because it is positive. The required change is to not do something — stopping an antibiotic decided on an isolated test — and the evidence needed to stop treating is, by construction, less than that needed to start. Here the evidence is: no clinical or hematological difference across more than fifty comparisons, identical outcome, and a microscopic test shown to be unreliable in both directions. The recommendation is the authors', it is explicit, and it goes in the direction of reducing antimicrobial use. The cross-constraint of chapter 14 §11 is respected (B = 5 with A = 4), as is the dose constraint: the one therapeutic figure is reported in full with dose, route, species, product, duration and outcome. C · Educational value ★★★★☆ Teaches three things not found together elsewhere: that an endemic pathogen can be present without being causal; that an established microscopic examination can have a sensitivity of 30% against its own reference test; and that a smear becoming negative is not eradication of an infection. The 0-4 morphological scale and the description of manual hematological methods are course material, reusable as they are. It does not reach five because the first-choice educational reference on South American camelid anemia is not this article: it is the review the same authors devoted to the topic, which this issue does not cover. D · Species relevance ★★★★☆ A study on alpacas only — the three llamas were excluded and the exclusion is stated — at a European clinic, with a population described by age, sex, weight, body condition, parasite load, diagnosis and outcome, and with species reference intervals spelled out row by row: transferability to Italy is assessable and high, which is the level-four criterion. It does not reach five because the fifth star requires a reference study taken up by subsequent literature: the article is from August 2024 and registers three citations on Europe PMC as of 31/08/2026 — too soon. No overall score, by design. B = 5 with A = 4 is the only case in the issue where clinical utility exceeds scientific quality, and it is consistent: convincing someone to stop giving an antibiotic requires less proof than convincing them to start.
Bottom line
It can be concluded that between 13 hospitalized PCR-positive alpacas and 22 PCR-negative ones for Candidatus Mycoplasma haemolamae, at the same clinic, no relevant clinical or hematological difference emerged — PCV, hemoglobin, MCHC, leukocytes, normoblasts, NLR, outcome (discharged 12/13 versus 17/22; p = 0.3771) — and that the only three significant differences go in the opposite direction to the one expected, with the negatives more emaciated (17/22 versus 3/12; p = 0.0048), with lower BCS (median 1.5 versus 2.75; p = 0.0145) and with more poikilocytosis (16/22 versus 4/13; p = 0.0322). It can be concluded that basophilic stippling on the smear does not make a diagnosis: present in 4/13 positives and 4/22 negatives, i.e. sensitivity 30.8%, specificity 81.8%, positive predictive value 50% (calculations by this editorial team on Table 3). And it can be concluded what was seen in a single animal: after oxytetracycline 20 mg/kg subcutaneous for five days the smear was clean and the PCR still positive.
It cannot be concluded that CMh is harmless: this is a low-power negative study, with 13 and 22 animals, without a power calculation and without a single confidence interval, and «not significant» is not «no difference» — the authors write that the effect on disease course «cannot be assessed due to the small number of animals» and that the acute phase could already have passed. It cannot be concluded that the two groups are comparable: among the positives are clinically healthy alpacas brought as companions, so the comparison is partly between less sick and more sick animals, and no adjusted analysis corrects for this. The 37.1% cannot be read as a prevalence: PCR was performed only on suspicion, there is no flow diagram, and 35 is the number tested, not a number over a denominator. The contribution of mycoplasma cannot be separated from that of Haemonchus: only two animals had both conditions. Nothing can be concluded about the efficacy of oxytetracycline from one animal with no control. With more than sixty tests without correction — and about three significant results expected by chance alone — the three observed must be read as such, and two of them (emaciation and BCS) measure the same phenomenon twice. Finally: this card and CRL-A-02 are not independent sources — same clinic, eight of ten authors in common, overlapping periods, same ethics code — and it is plausible that some of the animals appear in both studies → to be checked against the full text. Also left open are three verified internal discrepancies (swapped hospitalization means, sex count, unit of the reticulocyte row) and the absence of any quantitative load (Ct or copies/ml) from the PCR.
Messaggi da portare a casa
- Fra 13 alpaca PCR-positivi e 22 PCR-negativi ricoverati non c'era differenza clinica né ematologica, su oltre cinquanta confronti. Le uniche tre differenze significative — emaciazione (3/12 contro 17/22; p = 0,0048), BCS (mediana 2,75 contro 1,5; p = 0,0145) e poichilocitosi (4/13 contro 16/22; p = 0,0322) — vedevano i negativi stare peggio.
Key takeaways
- Between 13 hospitalized PCR-positive alpacas and 22 PCR-negative ones there was no clinical nor hematological difference, across more than fifty comparisons. The only three significant differences — emaciation (3/12 versus 17/22; p = 0.0048), BCS (median 2.75 versus 1.5; p = 0.0145) and poikilocytosis (4/13 versus 16/22; p = 0.0322) — showed the negatives faring worse.
- Anemia was more frequent in the negatives (14/22 versus 4/13; p = 0.0858), and its most frequent cause in this case series was Haemonchus contortus (10/35, with PCV 0.05-0.15 l/l), not mycoplasma.
- Basophilic stippling on the smear is not a test. In this case series: sensitivity 30.8% (4/13), specificity 81.8% (18/22), PPV 50% (4/8), NPV 66.7% (18/27) — calculation by the editorial team on Table 3 of the article. To confirm or rule out, PCR on EDTA is needed.
- Oxytetracycline 20 mg/kg subcutaneous for five consecutive days (Tetroxy® Vet 200 mg/ml) in one two-year-old male alpaca made the smear negative but not the PCR; the animal was discharged after nine days. A single animal: not a protocol. The transferable lesson is that a clean smear is not recovery.
- The authors recommend critically questioning an antibiotic decided on test positivity alone. This is their closing sentence, and it is the main reason this card exists.
- The 37.1% positive rate (13/35) is not a prevalence: PCR was performed only on suspicion. European prevalences reported in the literature run around 18-32%.
- No alpaca in the case series, positive or negative, had eosinophilia or basophilia; anisocytosis was present in 35 of 35 and does not discriminate anything.
- All hematology was done by hand — Neubauer chamber, centrifuged microhematocrit, photometric hemoglobin, calculated MCHC — without an automated analyzer and without species setting. It is the path card CRL-A-01 points to when the analyzer is not reliable; the price, stated by the authors, is that without an erythrocyte count there is no MCV or MCH.

CRL-A-02 (same clinic, same research group, same ethics code and overlapping periods: the biochemical picture of the same population, and the same caveat on nematode-related hypoproteinemia — not an independent source from this one) · CRL-A-01 (because here the hematology was done by hand, and because without an erythrocyte count MCV and MCH do not exist) · CRL-A-04 (the other study in which a positive laboratory result is not enough to make a diagnosis, and the other case in which the published counts need redoing) · CRL-A-03 (the other unit of measure lost along the chain).
*Card CRL-A-05 · version 1.0 · 31/08/2026 · summarizes a single source, not a body of literature, and is drafted from the full text (PMC11364540), tables and statements included. No search was performed, and it is therefore not excluded that studies subsequent to August 2024 on Candidatus Mycoplasma haemolamae in South American camelids exist, nor studies relating pathogen load to the clinical picture, nor controlled studies on the efficacy of oxytetracycline in this infection, nor Italian prevalence data subsequent to the first finding of 2022 cited by the authors. The internal bibliographic references cited in this card (Dawson et al. 2011, Hajduk, Whitehead, Fowler and Zinkl, Franz and Wittek, Neubert et al., Crosse et al., Viesselmann et al. 2022) are copied from the article's bibliography and tables and not opened by this editorial team. The accuracy measures for basophilic stippling (sensitivity, specificity, predictive values) are a calculation by the editorial team on Table 3 of the article and not a result of the authors. The bibliographic metadata are copied from the Crossref responses (this article's Crossref record), Europe PMC (the full Europe PMC record and the full text on Europe PMC) and OpenAlex (this journal's OpenAlex record) queried on 31/08/2026.*
