Specie e soggetti
Alpaca (Vicugna pacos). Novantuno (91) sieri, analizzati in batch nello stesso giorno dopo conservazione a congelamento.
⚠️ Qui la scheda deve essere sgradevolmente precisa. L'abstract dice «Serum samples from 91 alpacas»: novantuno alpaca, non novantuno cria.
Species and subjects
Alpaca (Vicugna pacos). Ninety-one (91) sera, analyzed in batch on the same day after storage frozen.
⚠️ Here the card must be uncomfortably precise. The abstract says «Serum samples from 91 alpacas»: ninety-one alpacas, not ninety-one crias. The title and the study's aim speak of neonatal alpacas, and the PubMed record carries the MeSH term Animals, Newborn; but the age, sex, herd of origin, and clinical status of the 91 animals are not verifiable in the abstract, and the full text is not accessible → to be checked against the full text. The subgroup on which the measurement was redone with the same standard is n = 19 (IT1) and n = 17 (IT2), and even the reason for that selection is not verifiable → to be checked against the full text.
Tipo di studio e livello di evidenza
Studio comparativo di laboratorio (method comparison) su campioni prospettici, con i tre metodi applicati agli stessi sieri, nello stesso giorno, in cieco reciproco per costruzione (ogni metodo ignora il risultato dell'altro). Il record PubMed lo classifica Comparative Study e Research Support, Non-U.S. Gov't; il MeSH include Prospective Studies e Reproducibility of Results.
Dichiarati dagli autori: nessuno — nessuna griglia è nominata nell'abstract.
Study type and level of evidence
Comparative laboratory study (method comparison) on prospective samples, with the three methods applied to the same sera, on the same day, mutually blinded by design (each method is unaware of the other's result). The PubMed record classifies it as Comparative Study and Research Support, Non-U.S. Gov't; the MeSH includes Prospective Studies and Reproducibility of Results.
Level of evidence: an analytical agreement study between methods — not a diagnostic accuracy study. The difference is the key to the whole card: in this paper there is no reference standard that says which cria actually had FPT. The study establishes that the three methods do not give the same number; it does not establish which of the three is right. Consequence for the reader: what can be brought into the clinic is a warning, not a correction. Anyone concluding «then use IT2 because it is lower and finds more FPT» would have read the study backwards.
Relevant reporting standard: no EQUATOR checklist fits exactly. STARD 2015 is the checklist for diagnostic accuracy papers and does not apply here, because a reference test is lacking; for the comparison between analytical methods the proper venue is the clinical chemistry literature (Bland-Altman analysis, Passing-Bablok), not an EQUATOR checklist. STROBE covers the observational part (sample selection, description of the population). Declared by the authors: none — no checklist is named in the abstract.
In breve
In brief
Ninety-one alpaca sera were measured for IgG with three commercial methods: radial immunodiffusion (RID), an immunoturbidimetric assay from an automated analyzer (IT1) and a point-of-care immunoturbidimetric assay (IT2). The medians were found to be significantly different from each other: RID 15 g/L, IT1 16 g/L, IT2 11 g/L. At the conventional decision threshold of 10 g/L, the methods contradicted each other on the diagnosis in 1-17% of samples. Electrophoresis showed why: the calibration standard of RID and IT1 was made of over 60% albumin, that of IT2 of beta and gamma globulins. Using the same standard, the discrepancy disappeared (IT1 median 11 g/L, IT2 10 g/L). The most precise method was found to be IT1, the least precise RID. Conclusion: IgG measurement in camelids is strongly dependent on the kit standard, is not directly comparable between methods, and may lead to underdiagnosis of FPT in some crias.
Why this study matters
Because failure of passive transfer is, in the camelid, the clinical trait defined by a number, and this article demonstrates that number does not stand on its own.
The chain from which the number comes is short and worth retracing. Garmendia AE, Palmer GH, DeMartini JC, McGuire TC (1987), Am J Vet Res 48(10):1472-1476, PMID 3674556, DOI 10.2460/ajvr.1987.48.10.1472 — 82 alpaca crias, 10 (12%) dead within the first month, and 7 of those 10 had serum IgG between 0 and 9 mg/mL at 48 hours. On this basis the authors defined FPT as IgG at 48 hours < 9 mg/mL, a value that lay more than two standard deviations below the mean of clinically normal crias; with that definition the prevalence of FPT in the 82 crias was 9%. Since 1 mg/mL = 1 g/L, that 9 mg/mL is the 9 g/L that clinical practice later rounded to 10 g/L.
Pinn and colleagues arrive twenty-five years later and say the thing no one had checked: that number is not a property of the cria, it is a property of the cria-kit pair. The threshold survives; its interpretation without the method does not.
Obiettivo
Dichiarato dagli autori: «to determine whether 3 commercially available immunologic assays yielded comparable results for IgG in alpacas».
Objective
Stated by the authors: «to determine whether 3 commercially available immunologic assays yielded comparable results for IgG in alpacas».
Clinical question in PICO form (editorial formulation; the design allows it in adapted form): in a neonatal alpaca [P], does measuring serum IgG with a point-of-care or automated-analyzer immunoturbidimetric assay [I] compared to radial immunodiffusion [C] give an interchangeable value for the purpose of classification as FPT at the threshold of 10 g/L [O]?
Disegno dello studio
I tre metodi:
- RID — immunodiffusione radiale, il metodo storico di riferimento nella specie;
- IT1 — immunoturbidimetrico configurato per analizzatori automatici di chimica clinica;
Study design
Sera from 91 alpacas, stored frozen and analyzed in batch on the same day with the three methods — a choice that eliminates drift between analytical runs and storage differences, and is the reason the comparison is clean.
The three methods:
- RID — radial immunodiffusion, the historical reference method in the species;
- IT1 — immunoturbidimetric assay configured for automated clinical chemistry analyzers;
- IT2 — point-of-care immunoturbidimetric assay, i.e. the test done on the farm.
⚠️ The commercial names and manufacturers of the three kits are not in the abstract and this card does not report them → to be checked against the full text. This is an omission that matters: without the kit name, the reader cannot know whether the product on their bench is IT2 or another. Anyone wanting to apply this work must open the article for that datum, and the card explicitly tells them that is where they must go.
On the subgroup, the two immunoturbidimetric assays were remeasured using the same calibration standard — the experiment that turns an observation into an explanation.
Ethical approval / owner consent: not verifiable in the abstract → to be checked against the full text.
Metodi / Protocollo
Methods / Protocol
IgG measurement with the three methods on the same 91 sera. Protein electrophoresis on the kits' calibration standards — this is the step that explains everything else, and is the reason the paper deserves a card rather than a line. Remeasurement of the IT1 (n = 19) and IT2 (n = 17) subgroups with a common standard. Precision assessment of the three methods.
⚠️ Not verifiable in the abstract: the statistical method used for the comparison (significance test between medians, and whether a Bland-Altman or Passing-Bablok analysis was conducted); the imprecision estimates (intra- and inter-run coefficients of variation) supporting the statement «IT1 more precise, RID less precise»; the operational definition of «diagnostic discordance» that produces the 1-17% interval. All → to be checked against the full text.
Risultati principali
|---|---|---|
| RID — immunodiffusione radiale | 15 g/L | comune a IT1; > 60 % albumina |
| IT1 — immunoturbidimetrico da analizzatore | 16 g/L | comune a RID; > 60 % albumina |
| IT2 — immunoturbidimetrico point-of-care | 11 g/L | beta e gamma globuline |
Gli autori concludono che «camelid IgG measurement is highly dependent on the assay standard and is not directly comparable between assays, potentially resulting in underdiagnosis of FPT in some crias».
Main results
Serum IgG medians on the same 91 alpaca sera, by method:
| Method | IgG median | Calibration standard |
|---|---|---|
| RID — radial immunodiffusion | 15 g/L | shared with IT1; > 60% albumin |
| IT1 — automated-analyzer immunoturbidimetric assay | 16 g/L | shared with RID; > 60% albumin |
| IT2 — point-of-care immunoturbidimetric assay | 11 g/L | beta and gamma globulins |
The three medians are significantly different from each other; RID and IT1 — which share the standard — give significantly higher values than IT2.
Diagnostic discordance at the 10 g/L threshold: 1-17% of samples. That is: between 1% and 17% of the 91 sera were classified differently — FPT yes / FPT no — depending on which kit had done the measurement. The range refers to the different pairwise comparisons between methods; which pair produces the 1% and which the 17% is not specified in the abstract → to be checked against the full text.
The why, measured and not hypothesized. Electrophoresis showed that the RID and IT1 standard was mostly albumin (> 60%), while that of IT2 was made of beta and gamma globulins — the very protein class one actually wants to measure.
The counter-proof. Remeasuring with the same standard, the discrepancy between IT1 and IT2 disappeared: IT1 median 11 g/L (n = 19), IT2 median 10 g/L (n = 17). Note that IT1 goes from 16 to 11 g/L from the calibration change alone: five grams per liter, that is, half the decision threshold, produced by a choice of calibrant.
Precision: IT1 the most precise, RID the least precise — that is, the method historically considered the reference in the species is the one that repeats worst.
Authors' interpretation (attributed, not merged with the data)
The authors conclude that «camelid IgG measurement is highly dependent on the assay standard and is not directly comparable between assays, potentially resulting in underdiagnosis of FPT in some crias». The direction of the error is stated: underdiagnosis. The underlying reasoning is transparent — methods calibrated on albumin return higher values, so more crias end up above the 10 g/L threshold, so fewer crias are classified as FPT than would qualify under the method calibrated on globulins.
⚠️ But the direction of the error is an inference, not a measurement. For «underdiagnosis» to be a fact and not a reasonable hypothesis, one would need to know which of the two standards is the correct one, and this study does not establish that: it establishes that they are different and that one of the two contains predominantly a protein that is not the analyte. It is a strong argument, not a demonstration. The card keeps the two things separate because the CRL grid requires it.
Implicazioni cliniche
In cartella va scritto
Clinical implications
For the Italian veterinarian who must decide today whether to transfuse plasma to a cria.
- An IgG value without the name of the method is not a clinical datum. The record should read
IgG = x g/L, method Y. AnIgG 12without a method, re-read six months later or by a colleague, is unusable: it could be a cria that is fine or a cria below threshold. - Two measurements made with different kits are not compared. If a cria is followed over time — and a doubtful cria is followed — the second measurement must be made with the same method as the first, or it is not a trend, it is two numbers.
- The 10 g/L threshold remains the clinical threshold in use, but it must be used as a threshold of suspicion and not as a verdict. The documented historical threshold is that of Garmendia 1987 — 9 mg/mL = 9 g/L at 48 hours — and it is a threshold defined on Peruvian alpacas, with the method of the time.
- The clinical decision never rests on the number alone. In the compromised cria the determinants are composite: total protein, albumin, clinical picture, nursing history. The species' handbook literature indicates considering transfusion when total protein is < 4 g/dL or albumin < 2 g/dL, as well as when FPT is confirmed or suspected (Whitehead CE, Cebra C, «Neonatology and Neonatal Disorders», chapter of Llama and Alpaca Care, full text PMC7151849 — handbook source, not a study, and this card does not summarize it: it cites it).
- Field tools are useful and are another matter. O'Neill AK, Petzel CE, Connolly JH, Vaughan JL, Rotne R (2024), Vet Res Commun 48(3):1563-1572, PMID 38396169, PMCID PMC11147829, DOI 10.1007/s11259-024-10297-0, found strong correlation between Brix refractometry (optical and digital) and colostral IgG measured with RID, and moderate correlation between serum total protein and the cria's serum IgG. The authors state that this is a pilot study on a single farm (due to COVID-19-related travel restrictions) and that further studies are still needed to establish reference intervals. That is: Brix and total protein are good screening methods, and are not a source of thresholds.
- The most concrete practical consequence is for the laboratory, not the clinician. Whoever sends samples must ask their laboratory which kit it uses and on which standard it is calibrated, and note it once and for all. It is a phone call.
Limiti
Dichiarati dagli autori.
- I campioni erano congelati.
◆ COMMENTO DELL'ASSOCIAZIONE
Camelid Research Studies APS — redazione CRL · 31/08/2026
Valutazione a stelle
Limitations
Declared by the authors. In the abstract the authors do not state limitations explicitly: the concluding sentence is a recommendation («not directly comparable between assays»), not a list of the limitations of their own work. An explicit list of limitations is not verifiable without the full text, which is by subscription and not deposited in PMC → to be checked against the full text.
Added by the editors.
- There is no reference test, and without a reference test there is no «correct» method. This is the structural limitation, not a flaw of execution: the design allows one to say that the three methods diverge, not which one converges on the truth. A study wanting to settle the question would need to anchor the measurement to an outcome — survival, sepsis, infectious disease in the first 30 days — as Garmendia did in 1987, and no one has redone it.
- The population of 91 animals is not described in what is verifiable. Age, clinical status, number of herds of origin, inclusion criteria: none of this is in the abstract. If the 91 sera came from a single herd, or had been selected to be rich in values around the threshold, the discordance rate would change. → to be checked against the full text.
- The counter-proof subgroups are small and of different size (n = 19 and n = 17): the disappearance of the discrepancy is convincing as a mechanism, but quantitatively fragile.
- The samples were frozen. Freezing and thawing may affect different methods differently, and the effect is not investigated here. It is a known confounder in clinical chemistry and the article, as far as verifiable, does not address it.
- The medians are all above threshold. 15, 16 and 11 g/L: the population studied was, predominantly, normal. The discordance rate between methods is greatest around the threshold, and in a population of sick crias — that is, the ones actually measured in practice — it would be greater, not smaller.
- The work is thirteen years old. The 2013 kits may have been recalibrated, replaced, or withdrawn. The demonstrated mechanism (calibration determines the number) does not age; the three medians do.
Funding: the PubMed record classifies the article as «Research Support, Non-U.S. Gov't», but the funding declaration with source and sponsor role is not readable in the accessible record → to be checked against the full text.
Conflicts of interest: the PubMed record contains no conflict-of-interest declaration, and the full text is not accessible → to be checked against the full text. This must be said alongside the result, not after: this is a study that classifies three commercial kits and identifies one as the most precise and one as the least precise. It is exactly the type of work in which the reader has the right to know who supplied the reagents, and here it cannot be known.
What is missing relative to the reporting standard. Relative to STROBE for the observational part and to the established practice of method-comparison papers:
- Description of the population and selection criteria of the 91 samples: absent from the accessible record.
- Identification of the kits (manufacturer, commercial name, lot): absent from the accessible record — and it is the datum that makes the work applicable or inapplicable.
- Bland-Altman agreement analysis with bias and limits of agreement, and Passing-Bablok regression with confidence intervals for slope and intercept: these are the de facto standard for comparison between analytical methods and do not appear in the abstract.
- Numerical imprecision data (CV%) supporting the statement on precision: absent from the abstract.
- Funding and conflict-of-interest declaration: not verifiable.
This is not a matter of saying «the article does not comply with X», which would be a reproach: it is a matter of saying what this card has not seen, and where the reader will find it if they open the article.
◆ ASSOCIATION COMMENTARY
Camelid Research Studies APS — CRL editorial team · 31/08/2026
A laboratory value is the result of a convention, and the convention can shift the threshold by half: the same sample, IT1, measures 16 g/L with one method and a different number with another.
It concerns the fact that a laboratory value is the result of a convention, and that the convention can change the number by half the threshold. Look at the most telling pair in the article: IT1 measures 16 g/L, and the same IT1, on the same sera, recalibrated with another standard, measures 11 g/L. The serum did not change, the instrument did not change, the operator did not change. What changed was the vial that told the machine what «one» means.
The technical reason is, if anything, even more instructive than the figure. The standard of two out of three kits was over 60% albumin. Albumin is not an immunoglobulin. Calibrating an IgG assay on a calibrant made mostly of something else is not a marginal flaw: it is the reason why, in this species, the most widely reported number in the literature is probably systematically high. And if it is systematically high, the error has one direction only: the crias we classify as «fine» include crias that are not. The authors say it with the right word — underdiagnosis — and we repeat it with the caution it deserves: it is an inference from the mechanism, not a measurement against a clinical outcome.
Why this matters more in camelids than elsewhere. In the calf and the foal, if the IgG assay is wrong, there is a safety net made of thirty years of literature, of accuracy studies against outcomes, of reference intervals redone many times. In the alpaca the net does not exist: the threshold in use descends, in a direct line, from a 1987 study on 82 crias, and from no other. When the base is a single one, a systematic calibration error is not compensated by anything. This is why the association considers this article — a comparison between kits, apparently technical and dull — one of the most important in the entire issue.
The discomfort we must declare. This work says that one of the three kits is the most precise and another the least precise, and we cannot read either the kit names or the conflict-of-interest declaration, because the article is closed and not deposited. A work that serves field practice, published in the journal of veterinary diagnosticians, and inaccessible to those in the field. This is not a fault of the authors; it is the condition in which this discipline works, and the card writes it instead of working around it.
What we ask, concretely, of our members and the laboratories they work with. Three lines to keep stuck to the fridge: (a) the method is written next to the number, always; (b) comparison over time is made with the same method, or not at all; (c) ask your laboratory once which kit it uses and on what it is calibrated, and stop asking. They cost nothing and are the only defense available against an error that, in this species, has only one direction.
And one thing we would like to see done, if anyone is looking for a thesis topic. No new kit is needed. What is needed is redoing Garmendia: a cohort of crias followed for the first thirty days, IgG measured at 48 hours with two methods in parallel, and a hard clinical outcome (death, sepsis, infectious disease) as an anchor point. It is the only design that can say which number is right, and it is within reach of a network of coordinated European farms. It is a study that does not need a research laboratory: it needs a hundred crias and someone who does not lose the follow-ups.
Star rating
Axis Score Rationale A · Scientific quality ★★★☆☆ A study with a stated sample size (91 sera), batch analysis on the same day eliminating analytical drift, and — above all — an experimental counter-proof of the mechanism (remeasurement with a common standard) that turns an observation into an explanation: the ★★★ level is fully earned. The fourth star requires discussed limitations and stated funding and conflicts: none of the three is verifiable in the accessible record, and the population selection criteria, kit identification, and agreement analysis following standard practice (Bland-Altman, Passing-Bablok) are missing. B · Clinical utility ★★★★☆ A result applicable with reasonable confidence and with explicit conditions of use: write the method next to the value, do not compare measurements from different kits, treat the 10 g/L threshold as a threshold of suspicion. It does not reach five because it does not change recommended practice: the threshold remains the same, and the work does not provide the correct number to replace it — it says that the current number is conditional, not which one is right. The cross-constraint is respected (B does not exceed A by more than two stars). C · Educational value ★★★★★ It is the first-choice reference, in the species, on what it means to measure a cria's IgG: whoever needs to learn that starts here. And it teaches, with an experiment and not an admonition, the distinction between the value and the method that produces it — course material, not reference material. D · Species relevance ★★★☆☆ A study on alpacas, on a parameter of daily use, with a mechanism directly transferable to any context including the European one. It does not reach four because the grid requires, for that level, that the population be described enough to assess transferability, and here it is not: of the 91 animals, neither age nor clinical status nor number of herds is known from what is verifiable. No overall score, by design. A = 3 and C = 5 is the typical combination of a technically well-designed work reported incompletely: it teaches very well something it has only partly demonstrated.
Bottom line
It can be concluded that in the alpaca, serum IgG concentration measured on the same serum differs significantly among three commercial methods (medians 15, 16 and 11 g/L), that the measured cause is the composition of the calibration standard (over 60% albumin in two of three kits), that at the 10 g/L threshold 1-17% of samples change classification depending on the kit, and that the discrepancy disappears when the same standard is used.
It cannot be concluded that one of the three methods is the correct one, nor can it be quantified how many crias with true FPT escape diagnosis: in the study there is no reference test nor any clinical outcome against which to anchor the measurement. «Underdiagnosis» is the expected direction of the error, not a measured quantity.
Take-home messages
- The same alpaca serum is worth 15, 16 or 11 g/L depending on the kit. An IgG value without the name of the method is not a clinical datum: the record should read
IgG = x g/L, method Y. - The cause is the calibration vial, and it is measured: the standard of two out of three kits was over 60% albumin, which is not the analyte. With the same standard, IT1 goes from 16 to 11 g/L — five grams per liter, half the threshold.
- In 1-17% of samples the diagnosis changes by changing the kit, at the 10 g/L threshold. And the discordance is greatest right around the threshold, that is, in the crias actually measured in practice.
- The error has a direction: underdiagnosis. Methods calibrated on albumin return higher values, so fewer crias are classified as FPT. It is an inference from the mechanism, strong, not a measurement against an outcome.
- The threshold in use descends from a single 1987 study on 82 crias (Garmendia et al.), which defined it as IgG < 9 mg/mL at 48 hours, i.e. 9 g/L. In the species there is no safety net below that number: this is why calibration matters so much.
- RID, the historical reference method in the species, turned out to be the least precise of the three.
