Autori

Mignacca Sebastian Alessandro · Sánchez-Miguel Cosme · Salgado Sara · Brady Colm · Fagan Seamus · Murphy Denise · Sheahan Maresa · Kennedy Aideen · Casey Micheal · Johnson Alan · McGettrick Shane · McElroy Maire · Moriarty John.

Authors

Mignacca Sebastian Alessandro · Sánchez-Miguel Cosme · Salgado Sara · Brady Colm · Fagan Seamus · Murphy Denise · Sheahan Maresa · Kennedy Aideen · Casey Micheal · Johnson Alan · McGettrick Shane · McElroy Maire · Moriarty John.

Full list, thirteen authors, in the order of the Crossref record (queried on 31/08/2026) and identical to the author list on Europe PMC. No ORCID is present in the article's Crossref record, for any of the thirteen. No affiliation is reported at the individual-author level in the Crossref record (affiliation field empty for all thirteen).

The size of the group is itself a piece of information, and must be read as such. Thirteen authors for a case series is not an editorial anomaly: it is the signature of a national diagnostic service, in which every regional laboratory that performed part of the necropsies appears among the authors. It is not a research group that designed a study: it is a network of pathologists who searched through their own archive. The consequence for the data is in the Limitations field, and it is not a small one.

Affiliazione dell'autore corrispondente

Mignacca S.A. — Department of Agriculture, Food and the Marine — Pathology Division, Celbridge, Ireland; indirizzo di corrispondenza [email protected].

Corresponding author's affiliation

Mignacca S.A. — Department of Agriculture, Food and the Marine — Pathology Division, Celbridge, Ireland; correspondence address [email protected]. As reported in the affiliation field of the Europe PMC/PubMed record, which explicitly marks the address as the corresponding author's «Electronic address».

⚠️ The affiliations of the other twelve authors are not in the accessible records → to be checked against the full text. That the first and corresponding author is in the Pathology Division of the Irish Ministry of Agriculture, Food and the Marine is, however, the fact that matters, and it is certain: this is not a university study, it is a product of the competent veterinary authority. Whoever evaluates the source must know this, because it changes both the access regime to the material (no one else could have written this work) and the limits of the design (no one chose which animals entered the case series).

  • PMCID: nessuno.

Licenza dell'articolo originale: CC BY 4.0.

Journal, year, volume, issue, pages, identifiers

Preventive Veterinary Medicine, 2025, volume 239, no issue (a journal with continuous numbering by volume), article 106510 (the article does not have a page range: the article number is the pagination).

First publication date recorded by Europe PMC: 12 March 2025; issue collection date according to Crossref (print publication date): June 2025. The card cites the year 2025, which is the volume's year.

  • DOI 10.1016/j.prevetmed.2025.106510
  • PMID 40086179
  • PMCID: none. Europe PMC reports no PubMed Central code, not in PubMed Central, not in Europe PMC, not open access.
  • Corresponding author ORCID: not in the article record (see above).
  • Open access of the article: yes. Unpaywall, queried on 03/09/2026: hybrid open access, published version at the publisher, CC BY license.

⚠️ The full text was not read, and the reason must be spelled out because it is instructive. This article is open access under a CC BY 4.0 license — the most permissive license in circulation, the one that allows reproduction and reuse with attribution alone. Three independent sources queried on 31/08/2026 confirm it: the Crossref record (license: CC BY 4.0 on the version of record), the Europe PMC record (CC BY license) and Unpaywall (open access, hybrid open access, single publisher host, cc-by license, no PDF URL). Nonetheless the text cannot be extracted. The article is not deposited in PubMed Central; Unpaywall knows of no alternative deposit; and the publisher's page, queried directly, returned HTTP 403 with the marker <meta name="tdm-reservation" content="1"> in the HTML header, i.e. an explicit reservation on automated text and data mining. An article freely reusable under license and unreachable due to infrastructure. Every field that depends on the full text is marked accordingly, and there are many.

Journal status (verified on 31/08/2026): Preventive Veterinary Medicine, ISSN 0167-5877 (print) / 1873-1716 (electronic), publisher Elsevier BV. PubMed: yes. DOAJ: no (querying this journal's DOAJ record, zero records: it is a hybrid journal, not entirely open access). OpenAlex: 6,787 indexed works, source marked not open access and not in the DOAJ directory.

Contextual note. Preventive Veterinary Medicine is the proper venue for this work and not a fallback choice: it is the leading international journal for veterinary epidemiology and population medicine. The issue does not have a second article of this weight — it is, with C-01, one of the two Group C papers to appear in a top-choice journal in its own domain. It is worth noting because the literature on South American camelids is for the most part relegated to lesser venues, and this is not.

License of the original article: CC BY 4.0. This card could reproduce tables and figures with attribution — and reproduces none, for the reason just stated: it could not read them. It is the only card in the issue in which permission exists and access does not.

Specie e soggetti

Alpaca (Vicugna pacos): 168. Lama (Lama glama): 7.

Species and subjects

Alpaca (Vicugna pacos): 168. Llama (Lama glama): 7. These are the two numbers the abstract states verbatim — «New World Camelid (168 alpacas; 7 llamas)» — and from which follows the only reconstructible total of the case series: 175 submissions, from 2010 to 2023, i.e. thirteen years and on average about thirteen submissions a year for the whole of Ireland.

⚠️ The two numbers cannot be summed with impunity, and this is the card's main trap. The title of the article speaks of bovine tuberculosis in the alpaca; the abstract reports the share of M. bovis among submissions, without stating whether the denominator is the 175 camelids or only the 168 alpacas, and without stating whether any of the 7 llamas were among the positives → to be checked against the full text. The arithmetic reconstruction proposed further below (Main results field) suggests 175, but it remains a reconstruction and the card marks it as such.

What these 175 animals are, and what they are not. They are dead camelids that someone decided to send to a public laboratory. They are not a sample of the Irish camelid population; they are not all the camelids that died in Ireland in thirteen years; they are not selected by a stated criterion. They are the product of a chain of private decisions — an owner who grew suspicious, a veterinarian who proposed submission, a budget that allowed it. Every percentage in this article inherits that chain, and it cannot be read as a prevalence.

Breakdown by age class (percentages from the abstract, denominator not stated in the text read):

Age classShare of submissionsEditorial reconstruction on 175
Adults58.8 %103
Juveniles20.6 %36
Cria16.6 %29
Aborted fetuses4 %7

⚠️ The right-hand column is not a datum from the article: it is an arithmetic reconstruction by the editorial team, obtained by searching for the integers that reproduce the four percentages on a total of 175. The reconstruction closes exactly (103 + 36 + 29 + 7 = 175, and each ratio reproduces the published percentage to the first decimal place), which makes it very likely — but likely is not verified, and the true counts must be read in the article's tables → to be checked against the full text. ⚠️ The age criteria separating cria, juvenile and adult are not in the abstract → to be checked against the full text: it is an operational definition without which the three rows are not comparable to any other case series.

Study type and level of evidence

Retrospective case series built on national passive surveillance, with descriptive analysis. The PubMed record classifies it as a plain Journal Article; no design descriptor appears.

Level of evidence: descriptive observational study without a comparison group and without a known at-risk population. In the classical hierarchy it sits above the single case and below any analytical study. Consequence for the reader, and it must be stated immediately: from this design neither a prevalence, nor an incidence, nor a relative risk can be derived. Not because the authors made a mistake, but because the population denominator is missing: no one knows how many camelids lived in Ireland during those thirteen years, and no one knows what fraction of the dead ended up in a laboratory. What the design delivers is the composition of what arrived, and that is a different and nonetheless valuable piece of information: it tells the pathologist what to expect on the table.

Relevant reporting standard: STROBE for the observational portion — in particular items 5 (setting and recruitment period), 6 (eligibility criteria and source of participants), 12 (statistical methods and handling of missing data), 13 (numbers of participants at each stage, with a flow diagram), 14 (characteristics of participants), 16 (estimates with their precision) and 19 (limitations). Declared by the authors: not verifiable → to be checked against the full text. Item 16 is the one on which the abstract visibly stumbles, and it is addressed in the Main results field.

In breve

In brief

The causes of mortality of New World camelids in Ireland — 168 alpacas and 7 llamas — were analysed on the basis of a review of passive surveillance findings of the Irish Veterinary Laboratory Service from 2010 to 2023. The age class most frequently submitted was adults (58.8 %), followed by juveniles (20.6 %), cria (16.6 %) and aborted fetuses (4 %). Irrespective of age, 96 % of submissions were suitable for investigation, with an etiology identified in 77.4 % of cases. The pathology was systemic (30.9 %) or involved the digestive (39.3 %), respiratory (8.9 %), urogenital (4.9 %), musculoskeletal and lymphatic (2.2 % each), integumentary (1.8 %), cardiovascular (1.3 %) and nervous (0.9 %) systems; 7.6 % were non-conclusive. The etiology was bacterial (44.4 %), parasitic (24.3 %), nutritional/metabolic/toxic (6.4 %), traumatic/mechanical (3.2 %), congenital (1.6 %) and neoplastic (0.5 %), while 19.6 % had unknown etiology. The diagnostic rate was highest in cria (79.3 %) and lowest in the abortion group (0 %). Cria mostly had systemic (43.3 %) and digestive tract (26.6 %) diseases; juveniles mostly digestive (51.3 %) and systemic (30.7 %) diseases; and similarly adults (40.2 % and 25.5 %). The primary etiology found was bacterial in all three classes (45.5 % cria, 35.1 % juveniles, 48.7 % adults). Mycobacterium bovis* was identified in 27.4 % of submissions; of these, 25 % had lesions localized to a single organ (mainly the lung), while 75 % had generalized tuberculosis, with the lung as the organ most affected and the liver second. Infection with Fasciola hepatica* was the second most frequent cause or co-cause of mortality. To the authors' knowledge, this is the most extensive published case series of M. bovis pathology in alpacas.

Why this study matters

For three distinct reasons, and it is worth keeping them separate because they do not carry the same weight.

First, and this is why the card exists: it is the most extensive published description of how bovine tuberculosis presents in the alpaca. Not of how frequent it is — of what it looks like. The figure to memorize is the pair 25 % / 75 %: a quarter of alpacas with M. bovis had lesions in a single organ, three quarters had generalized tuberculosis. In cattle, a species in which the disease has been studied for a century, the typical picture is the opposite — contained lesions, often confined to the tracheobronchial and mediastinal lymph nodes, found at slaughter in an animal in good body condition. In the alpaca the disease disseminates. This is not a detail of pathology: it is the anatomical explanation of what chapter 09 of the KT documents from the diagnostic side, namely that this species has a high susceptibility and rapid progression (Krajewska-Wędzina M, Didkowska A, Sridhara AA, Elahi R, Johnathan-Lee A, Radulski Ł, Lipiec M, Anusz K, Lyashchenko KP, Miller MA, Waters WR. Transboundary Emerg Dis 2020;67(3):1306-1314, DOI 10.1111/tbed.13471).

Second: it is the European work closest to Italy in the whole issue, and it is not Italian. In this series geography is not editorial courtesy. The other four Group C cards rest on Peruvian (C-01, C-05), US (C-02) and Chilean-Canadian (C-03) works. This is the only one conducted within the European Union, on animals raised in a temperate-oceanic climate, with a public veterinary service organized like our IZS. It is not Italy, and the epidemiological difference is enormous (see Limitations), but it is the closest term of comparison that exists.

Third: it tells the pathologist what to look for, and it is the only card in the issue that does so. A veterinarian opening up an alpaca in Italy today has no reference case series. Now there is one: one starts from the digestive tract, which is the most involved in all three age classes; one looks at the liver, for generalized tuberculosis and fascioliasis together; and one factors in systemic disease, which in cria is the top category.

Obiettivo

Objective

Not stated in the form of an objective in the abstract, which opens directly with the descriptive sentence «Causes of New World Camelid (168 alpacas; 7 llamas) mortality in Ireland based on review of passive surveillance findings … were analysed» → to be checked against the full text for the explicit wording that the full text certainly contains.

Question in PICO form — and here, as in C-03, the grid must be used honestly: the design does not allow for a PICO, because there is no intervention and no comparison. The closest editorial reformulation, stated as an approximation: in New World camelids submitted to Irish passive surveillance between 2010 and 2023 [P], what is the distribution of causes of death by age class, system and etiology [O], with particular attention to the frequency and morphology of Mycobacterium bovis lesions [O2].

Disegno dello studio

Study design

Retrospective archival review of necropsy and laboratory findings of the Irish Veterinary Laboratory Service — the Irish public diagnostic service — over a period of thirteen consecutive years, from 2010 to 2023.

The study population is defined by an administrative act, not by a scientific criterion: the animals under study are those submitted. This is the trait that qualifies every passive surveillance, and it must be stated in the positive before being stated in the negative. In the positive: no selection by the researchers, no center that chose its own best cases, coverage of an entire country for thirteen years, and reasonable certainty that no severe and suspicious case was discarded — public laboratories accept whatever arrives. In the negative: the probability that a dead animal ends up in that case series is not constant, and depends on the owner's suspicion, on cost, on distance from the laboratory and — this is the point that matters for the main result — on the existence of a national bovine tuberculosis eradication program that makes suspicion of TB a strong reason to submit.

⚠️ Not verifiable from the abstract and therefore not asserted by this card: the inclusion and exclusion criteria for submissions; the operational definition of the age classes; the necropsy protocol applied; the laboratory tests used to attribute each etiology (culture, PCR, histology, serology) and their positivity criteria; how cases with more than one cause were handled; the existence and nature of a statistical analysis beyond description; any analysis of temporal trend over the thirteen years. All → to be checked against the full text.

Metodi / Protocollo

  5. Terza classificazione — eziologia. Attribuzione a una categoria eziologica (batterica, parassitaria, nutrizionale/metabolica/tossica, traumatica/meccanica, congenita, neoplastica) oppure a sconosciuta.
  6. Quarta classificazione — età. Adulti, giovani, cria, feti abortiti.

Methods / Protocol

Reconstructed from the abstract, which is descriptive and rich in numbers but poor in procedure.

  1. Data source. Archive of passive surveillance findings of the Irish Veterinary Laboratory Service, 2010-2023.
  2. Population. 168 alpacas and 7 llamas submitted for necropsy.
  3. First classification — suitability. Each submission was judged suitable or unsuitable for investigation: 96 % suitable. ⚠️ The suitability criterion is not stated → to be checked against the full text; in necropsy practice it normally corresponds to the state of preservation of the carcass, but this card does not assert this.
  4. Second classification — system. Attribution of the pathology to a system (systemic, digestive, respiratory, urogenital, musculoskeletal, lymphatic, integumentary, cardiovascular, nervous) or to the non-conclusive category.
  5. Third classification — etiology. Attribution to an etiological category (bacterial, parasitic, nutritional/metabolic/toxic, traumatic/mechanical, congenital, neoplastic) or to unknown.
  6. Fourth classification — age. Adults, juveniles, cria, aborted fetuses.
  7. Deep dive on M. bovis. For positive cases, distinction between lesion localized to a single organ and generalized tuberculosis, with indication of the organs involved. ⚠️ The method of identifying M. bovis — culture, PCR, spoligotyping, histology with Ziehl-Neelsen, or a combination — is not in the abstract, and the card does not assert it → to be checked against the full text. It is a piece of information that is anything but accessory: the distinction between M. bovis and M. microti, which in the alpaca is clinically relevant and documented, depends entirely on the identification method used.

No dosing appears in this article, and the card reports none: it is a study of pathology, not of therapy.

Risultati principali

«To the authors' knowledge, this is the most extensive published case series of M. bovis pathology in alpacas.»

Main results

The number that gives the card its title, with all the caveats it requires

Mycobacterium bovis identified in 27.4 % of submissions.

What this number is: the share of camelids dead and submitted to a public Irish laboratory in which M. bovis was found, in a country with a national bovine tuberculosis eradication program active for decades, a wildlife reservoir and high bovine infection pressure.

What this number is not, and the third convention of this series exists for exactly this: it is not a prevalence of tuberculosis in Irish alpacas. The denominator is not the population: it is passive surveillance, i.e. a funnel that selects in one direction only. A breeder who suspects tuberculosis has a very strong reason to submit the animal — legal obligations, protection of the rest of the herd, contact with the health authority; a breeder whose alpaca died of an intestinal torsion does not. The 27.4 % is therefore an overestimate of the true prevalence, of unknown magnitude and in one direction only. Whoever cites it as «more than one alpaca in four in Ireland has tuberculosis» is off by an order of magnitude, and predictably so.

⚠️ The exact denominator is not in the abstract → to be checked against the full text. Editorial arithmetic reconstruction, stated as such: 48 of 175 submissions gives 27.43 % → 27.4 %, and — this is the element that makes it convincing — 48 is the only nearby integer for which the 25 % and 75 % of the next step also fall on integers, i.e. 12 cases with lesion in a single organ and 36 with generalized tuberculosis. The alternative 46/168 (27.38 %) would give 11.5 and 34.5, which are not numbers of animals. If the reconstruction is correct, the denominator also includes the 7 llamas, and the article's title — which speaks of alpacas — must be read with that reservation. The card proposes the reconstruction and does not pass it off as a datum.

The morphology, which is the transferable result

PresentationShare of M. bovis casesOrgans
Lesions localized to a single organ25 %mainly the lung
Generalized tuberculosis75 %lung the most affected, liver second

Three quarters of alpacas with M. bovis had disseminated disease. This is the result that one takes away from this article even without knowing Ireland, because it concerns the behavior of the agent in this species and not its frequency in that country — and it is therefore the only part of the main result that transfers to Italy without correction.

The rest of the case series, with denominators where reconstructible

Suitability and diagnostic yield. 96 % of submissions suitable for investigation (editorial reconstruction: 168/175); etiology identified in 77.4 % (reconstruction: 130/168, i.e. the denominator is plausibly the suitable subset, not the total — a difference that shifts the numerator by five cases and that no one can resolve without the full text) → to be checked against the full text.

Diagnostic yield by age class — and this is the article's second practical result:

  • Cria: 79.3 % — the highest yield. Editorial reconstruction: 23/29, a ratio that exactly reproduces the published percentage.
  • Aborted fetuses: 0 %. Zero. Out of seven fetuses submitted in thirteen years, no etiological diagnosis.

The contrast between 79.3 % and 0 % is the single most useful piece of data in the entire article for an Italian holding, and it is addressed in Clinical implications, point 4.

Systems involved (percentages from the abstract; the sum of the ten items comes to 100.0 %, which indicates a single-category-per-case classification):

SystemShare
Digestive39.3 %
Systemic30.9 %
Respiratory8.9 %
Non-conclusive7.6 %
Urogenital4.9 %
Musculoskeletal2.2 %
Lymphatic2.2 %
Integumentary1.8 %
Cardiovascular1.3 %
Nervous0.9 %

Etiologies (sum of the seven items: 100.0 %):

EtiologyShare
Bacterial44.4 %
Parasitic24.3 %
Unknown19.6 %
Nutritional / metabolic / toxic6.4 %
Traumatic / mechanical3.2 %
Congenital1.6 %
Neoplastic0.5 %

By age class (the two most frequent system categories, and bacterial etiology):

ClassMost frequent systemSecondBacterial etiology
Criasystemic 43.3 %digestive 26.6 %45.5 %
Juvenilesdigestive 51.3 %systemic 30.7 %35.1 %
Adultsdigestive 40.2 %systemic 25.5 %48.7 %

Cria is the only class in which systemic disease beats digestive disease, and it must be read together with card C-01 of this issue: neonatal sepsis is the common outcome of hypothermia, delayed colostrum intake and failure of passive transfer, exactly what C-01 and C-02 discuss.

Fasciola hepatica: second most frequent cause or co-cause of mortality. ⚠️ The abstract does not give its numerical share → to be checked against the full text, and the card does not write a number it has not read. The phrase «cause or co-cause» is nonetheless informative in itself, and has a methodological consequence addressed immediately below.

⚠️ A denominator problem the card has verified and must disclose

The editorial team attempted, for each percentage in the abstract, to reconstruct the integer ratio that generates it. The reconstruction closes perfectly for some rows and does not close for others, and the distinction is systematic:

  • Close: the four age classes over 175 (103/36/29/7); suitability 96 % = 168/175; etiology identified 77.4 % = 130/168; cria diagnostic yield 79.3 % = 23/29; the pair 25 %/75 % = 12/36 out of 48.
  • Do not close: the system percentages within the age classes and the bacterial etiology percentages within the age classes. For cria, for example, the 43.3 % of systemic disease requires a denominator of 30 and the 79.3 % diagnostic yield requires one of 29; for juveniles, the 51.3 % of digestive pathology requires 39, the 30.7 % of systemic disease also requires 39, but the 35.1 % of bacterial etiology requires 37. No single integer satisfies all the rows of the same age class.

What this means, as far as the card can tell. It means that the denominators change from row to row, and the most economical hypothesis is the one suggested by the abstract itself: the phrase «cause or co-cause» implies that a case may be counted in more than one etiological category, inflating the denominator of those rows. This is an editorial hypothesis, not a statement by the authors → to be checked against the full text. But the operational consequence is certain and does not depend on the hypothesis: the age-class percentages in this abstract cannot be summed or compared with each other without the article's tables. Whoever puts them in a slide is putting them in without knowing what fractions they are.

Authors' interpretation (attributed, not merged with the data)

«To the authors' knowledge, this is the most extensive published case series of M. bovis pathology in alpacas.» — This is a claim of bibliographic primacy, not a result, and it is plausible: chapter 09 of the KT Camelidi, which surveys the literature on tuberculosis in camelids, knows of no larger case series. It must, however, be read for what it says: «the most extensive case series of M. bovis pathology», not «the largest study on alpaca tuberculosis». On diagnostic tests the reference case series are others and more informative — Infantes-Lorenzo 2018 and Bezos 2013, cited in Clinical implications.

No other authors' interpretation is in the abstract → to be checked against the full text. In particular, it is not verifiable whether the authors discuss the passive surveillance bias, which is the decisive issue for using their headline number.

Implicazioni cliniche

Clinical implications

For the Italian veterinarian caring for an alpaca or llama holding, or performing a necropsy on one.

  1. The 27.4 % does not travel to Italy, the morphology does — and the distinction must be held firm. The frequency depends on the Irish context: national eradication program, bovine infection pressure, wildlife reservoir, and a passive surveillance funnel that selects suspects. None of this transfers. What transfers is what the disease looks like when present: three quarters generalized, lung first organ, liver second. Whoever opens an alpaca with chronic weight loss should have that picture in mind, regardless of where they are.
  2. A negative tuberculosis test in a camelid is not proof of absence, and this article explains why the stakes are high. The intradermal tuberculin test, which is the official test, has an estimated sensitivity of 14-20 % in the alpaca and llama — four infected animals out of five test negative (Infantes-Lorenzo JA, Whitehead CE, Moreno I, Bezos J, Roy A, Domínguez L, Domínguez M, Salguero FJ. Front Vet Sci 2018;5:189, DOI 10.3389/fvets.2018.00189, PMID 30151368). The scheme that works is the parallel: single intradermal test at the prescapular site read at 72 hours, plus serology on a sample taken 15-30 days after PPD injection; applied to a naturally infected alpaca herd in Spain, the parallel identified from 90 % to 100 % of culture-positive animals, against 53.8 %-80 % with the intradermal test alone (Bezos J, Casal C, Álvarez J, Díez-Guerrier A, Rodríguez-Bertos A, Romero B, Rueda P, López L, Domínguez L, de Juan L. Prev Vet Med 2013;111(3-4):304-313, DOI 10.1016/j.prevetmed.2013.05.013, PMID 23809774). This card has not profiled those two works: it reports their results as an indication of where to read them, with the source alongside.
  3. The person performing the necropsy is the one at risk, not the visitor. The only published case of M. bovis transmission from a camelid to a human is cutaneous tuberculosis in a veterinarian after exposure to a tuberculous alpaca (Twomey DF, Higgins RJ, Worth DR, Okker M, Gover K, Nabb EJ, Speirs G. Vet Rec 2010;166(6):175-177, DOI 10.1136/vr.b4780, PMID 20139381). The route is broken skin plus tissue handling. In practice: gloves, eye protection and a filtering mask in every necropsy of a camelid with chronic weight loss or lung lesions, and no improvised outdoor necropsy on the farm when that is the suspicion. Tuberculosis is notifiable; the suspect animal is not opened up on one's own, it is submitted.
  4. The aborted fetus alone is useless: zero diagnoses out of seven submissions in thirteen years. This is the finding of this article that changes a practice tomorrow morning. Whoever submits a camelid abortion must also submit the placenta, and send along the dam's serum — abortion diagnostics in camelids is largely done serologically on the dam (Toxoplasma, Neospora, Chlamydia, Coxiella), not on the fetus. An autolytic fetus sent alone, in this case series, never produced an answer. Chapter 09 of the KT derives from this the serological panel; this card derives from it the submission rule.
  5. The highest diagnostic yield is in cria — and cria is the class that can be saved. 79.3 % of etiologies identified means that the laboratory, for cria, answers almost always. A holding that loses cria and does not submit them is giving up the most obtainable information it could have. And the dominant category is systemic disease (43.3 %), i.e. mainly sepsis: which points directly back to the two measures documented in cards C-01 (thermal protection in the first hours) and C-02 (verification of passive transfer).
  6. Fasciola hepatica is the second cause or co-cause, and it is a pasture disease that Italy has. It is not an exotic Irish problem: fascioliasis requires wet pastures and the intermediate snail host, conditions present in numerous Italian areas. Practical consequence: on a wet-pasture holding, the liver must be examined at every necropsy and fascioliasis belongs in the differential diagnosis of chronic weight loss and anemia — alongside tuberculosis, not in its place, because in this article both affect the liver. ⚠️ The card reports no treatment protocol: doses, molecules and withdrawal times for fascioliasis in camelids are not in this article, and must be sought where they belong.
  7. The greatest risk is not the disease: it is the purchase. The documented case that proves it is the importation of M. bovis-infected alpacas from the United Kingdom to Poland, with negative skin tests, passage through an international exhibition and final destination a farm running disabled-children assisted activities (Krajewska-Wędzina 2020, above, and Krajewska-Wędzina M, Miller MA, Didkowska A, Kycko A, Radulski Ł, Lipiec M, Weiner M. J Vet Res 2022;66(1):53-59, DOI 10.2478/jvetres-2022-0012). Long quarantine and prolonged clinical observation do what the test does not.

Limiti

Dichiarati dagli autori.

◆ COMMENTO DELL'ASSOCIAZIONE

Camelid Research Studies APS — redazione CRL · 31/08/2026

Zero diagnosi su sette conferimenti in tredici anni. Zero.

Valutazione a stelle

AsseVotoMotivazione

Limitations

Stated by the authors. Not verifiable: the abstract does not contain a limitations section, and the full text was not read → to be checked against the full text. This is a gap of this card before it is one of the article, and must be read alongside the fact that STROBE prescribes discussion of limitations at item 19: that it exists is likely; that this card has read it, no.

Added by the editorial team.

  • The passive surveillance bias is the limitation that governs everything else, and it is structural. No number in this article is a population measure. The direction of the bias is known and constant: it overestimates diseases that prompt submission — tuberculosis first and foremost, in a country with an eradication program — and underestimates those that do not: sudden deaths attributed to obvious causes, elderly animals, deaths on farms far from the laboratory.
  • There is no population denominator. It is not stated how many camelids lived in Ireland during the period, and therefore 175 submissions in thirteen years are not comparable to anything: not to the number of Irish camelids, not to expected mortality, not to other national case series.
  • The abstract's percentages cannot be resolved into consistent denominators, as documented in the Main results field. For the CRL grid this is a serious defect — axis A explicitly provides for dropping to one star for studies that «report percentages without denominators» — and the card records it at the level at which it was observed: the abstract. That the article's tables contain the counts is likely and even expected in Preventive Veterinary Medicine; this card could not verify it, and the distinction must be written out rather than left implied.
  • Thirteen years is a long period, and the card does not know whether it is homogeneous. Between 2010 and 2023, mycobacterial identification methods, submission policies, the size of the Irish camelid population and bovine tuberculosis infection pressure may have changed. No temporal trend analysis is reported in the abstract → to be checked against the full text. A 27.4 % average over thirteen years could conceal a decline, a rise or an episode concentrated in a few years.
  • Seven llamas are not a case series on llamas. Group C states in its index table the composition «alpacas (168) and llamas (7)» for exactly this reason: the 4 % of llamas allows no species-specific statement, and every result in this article must be attributed to the alpaca, with the llamas as a stated but unanalyzed presence.
  • The method for identifying M. bovis is not known to this card, and with it the fate of M. microti is not known, which is documented in the alpaca and which diagnostics based on histology alone would not distinguish. Chapter 09 of the KT reports a clinical outbreak due to M. microti in alpacas (7 positives out of 25 on the P22 ELISA, Infantes-Lorenzo 2018): the mycobacterial species is not a taxonomic detail, and this card cannot say how it was established.
  • The geography is not Italy. Ireland has a bovine tuberculosis epidemiological situation that Italy does not share, and which itself varies within Italy between officially free regions and non-free regions. Transferring the number is illegitimate; transferring the morphology and the submission rules is legitimate.

Funding: no body listed — the funder field of the Crossref record is empty and the funding list field of Europe PMC is null (verified on 31/08/2026). ⚠️ This is not equivalent to a statement of absence of funding → to be checked against the full text. It is moreover consistent with the nature of the work: a public service analyzing its own archive does not, as a rule, have dedicated funding to declare.

Conflicts of interest: ⚠️ the conflict-of-interest statement field of the Europe PMC record is empty → to be checked against the full text. Preventive Veterinary Medicine requires the statement, which will be in the full text; this card has not read it and does not assert its content.

What is missing relative to the reporting standard (STROBE). Relative to the items relevant to observational studies, not verifiable from the accessible record: the eligibility criteria and sources of participants (item 6); the variables with their operational definitions — age, system, etiology (item 7); the data sources and measurement methods, i.e. which laboratory tests established each etiology (item 8); the biases and attempts to address them (item 9); the statistical methods and handling of missing data (item 12); the number of participants at each stage with the flow diagram (item 13); the estimates with their precision, i.e. the counts and confidence intervals behind the percentages (item 16); the limitations (item 19); the funding (item 22). Nine items out of twenty-two not verifiable, and item 16 is the one this card actively tried to reconstruct, succeeding only in part — the reconstruction is in the Main results field and is stated as editorial.

◆ ASSOCIATION COMMENT

Camelid Research Studies APS — CRL editorial team · 31/08/2026

This is the article in the issue with the most numbers and the fewest denominators: this is where the false statement now circulating was born — «in Ireland more than one alpaca in four has tuberculosis».

The false statement is already in circulation, and it is heard phrased like this: «in Ireland more than one alpaca in four has tuberculosis». Every figure in that sentence is taken from the article; the sentence is wrong. The 27.4 % is not a prevalence: it is the composition of a funnel. The denominator is dead animals that someone decided to send to a public laboratory in a country engaged for decades in bovine tuberculosis eradication — i.e. precisely the country in which suspicion of tuberculosis is the strongest reason there is to submit a carcass. The bias is not random, it has a direction, and the direction is upward. By how much, no one knows, and not even the authors can know with this design.

Having said that — and it must be said first — the article is excellent, and our criticism is not of it. It is a thirteen-year passive surveillance collection, conducted by the public veterinary service of a European country, published in the right journal and under a CC BY license. No one designed it to prove a thesis; no one selected the best cases. It is the kind of work we wish existed for Italy and does not. If something does not add up, it does not add up in the abstract, which is what this card was able to read.

And here comes the thing that truly irritated us, and we write it because it concerns everyone. This article is CC BY 4.0: anyone can reproduce it, translate it, remix it, provided they cite it. Crossref, Europe PMC and Unpaywall all three say so. And we could not read it. It is not deposited in PubMed Central; no copy exists in any archive; the publisher's page returns 403 and carries a tdm-reservation header reserving automated extraction. The permission exists and the door is shut. The practical result is that a small volunteer association, wishing to bring a work useful to its members into Italian, stops at the abstract — and the resulting card has fourteen to be checked against the full text it should not have. An open license without an accessible deposit is a promise that keeps nothing. We say this here once and will not repeat it, but it holds for a growing part of the veterinary literature.

What we would make this article say, if we had to choose a single sentence to hang in the necropsy room. Not the 27.4 %. This: in the alpaca tuberculosis disseminates. Three quarters of cases with generalized lesions, lung and liver leading. It is a fact about the species and not about the country, it transfers without corrections, and it has an immediate and unpleasant consequence: the animal that arrives on your table with chronic weight loss and multi-organ lesions may have tuberculosis even if its intradermal test was negative, because that test in this species finds one in five. Whoever opens it up without protection is relying on a test that does not work. There is only one published case of transmission from camelid to human, and it is a veterinarian.

The second sentence, the one that changes a practice next week: the aborted fetus alone is useless. Zero diagnoses out of seven submissions in thirteen years. Zero. It is not a low rate, it is a null rate, and it contrasts with the 79.3 % response rate the same laboratory gives for cria. Whoever in Italy sends an autolytic fetus to the IZS and then complains that «it never comes back with anything anyway» is describing exactly this row. Abortion diagnostics in camelids goes through the placenta and the dam's serum, and this article — without saying so, because it is not its subject — demonstrates it with the force of the negative result.

A note on transferability, because it is the question we will be asked. No, Italy is not Ireland for bovine tuberculosis, and whoever carries over the 27.4 % is wrong. But the opposite error is equally wrong: saying «here we don't have it» because one's own region is officially free is a conclusion about the cattle population, not about the camelid bought at a fair. The guiding case we always tell — alpacas imported with negative tests, passed through an international exhibition, ending up on a farm doing rehabilitation with disabled children, dead of multi-organ tuberculosis — is not Irish: it is Polish and British, and no step in the chain was unlawful. The risk does not come in with regional epidemiology, it comes in on the truck.

What we will not do. We will not use this article to fuel alarm about tuberculosis in Italian camelids, on which we have no data. Nor will we use it for the opposite. We will use it for two things, both verifiable: teaching how to perform a camelid necropsy safely and what to expect to find, and explaining to a member why a negative test is not a certificate.

Star rating

AxisScoreRationale
A · Scientific quality★★★☆☆Observational study with stated n (168 alpacas, 7 llamas), defined period (2010-2023), explicit source of cases (national passive surveillance) and systematic classifications by age, system and etiology: this is the three-star band, largely consistent with STROBE. The fourth star is not attainable by construction — it is neither a prospective controlled study nor an experimental one with ARRIVE's Essential 10. It does not drop to the lower band, which the grid reserves for studies «that report percentages without denominators», only because the defect is documented at the level of the abstract and not of the article: Preventive Veterinary Medicine's tables will contain the counts, and this card could not open them. The card states that if the defect turned out to be the article's, the score would be one star.
B · Clinical utility★★★☆☆Offers two replicable and immediately applicable procedures: what to look for in a camelid necropsy (digestive and systemic first; lung and liver for generalized tuberculosis) and how to submit an abortion (never the fetus alone). It does not reach four because no result is applicable with confidence to similar cases in Italy without context correction: the quantitative part is Irish, and no dose, molecule or treatment protocol appears in the article.
C · Educational value★★★★☆First-quality training material: a thirteen-year national case series, with the breakdown by age and system that no Italian manual possesses, and it is the most extensive published description of M. bovis pathology in the alpaca. It also teaches, by example, the denominator lesson better than any theoretical treatment: it is the article on which we had to write out in full the issue's third convention. The fifth star is withheld by access: a first-choice educational reference must be readable by those studying it, and this one's figures and tables were not, for us.
D · Relevance to the species★★★★☆Study on camelids that is large, national and multicenter, with a population described enough to assess its transferability — and it is the only work in the entire issue conducted within the European Union. It does not reach five because the quantitative data are not directly transferable to the Italian context (Irish bovine tuberculosis epidemiology, passive surveillance funnel) and because the 7 llamas allow no statement about the second species.

No overall score, by design. A = 3 and C = 4 is the signature of good descriptive surveillance: a work that proves nothing and teaches much.

Bottom line

It can be concluded that in the 175 New World camelids submitted to Irish passive surveillance between 2010 and 2023 the most represented pathology was digestive (39.3 %) followed by systemic (30.9 %), with bacterial (44.4 %) and parasitic (24.3 %) etiology leading; that M. bovis* was identified in 27.4 % of submissions and that, among positives, 75 % had generalized tuberculosis with lung and liver as the organs most affected; that the diagnostic rate was 79.3 % in cria and 0 % in the seven aborted fetuses; and that Fasciola hepatica* was the second most frequent cause or co-cause of mortality.

It cannot be concluded that 27.4 % is the prevalence of tuberculosis in Irish alpacas — the denominator is passive surveillance and the bias has a single direction, upward — nor that any of these frequencies hold for Italy, nor that they hold for the llama, which is present with seven animals out of 175 and is not analyzed separately. Nor can one reconstruct, from what this card has read, a coherent set of denominators for the percentages referring to the individual age classes: those figures are not comparable to each other without the article's tables.

Messaggi da portare a casa

Take-home messages

  1. In the alpaca tuberculosis disseminates: three quarters of cases with generalized lesions, lung first organ and liver second. It is a fact about the species, not about the country, and it transfers to Italy without corrections.
  2. The 27.4 % is not a prevalence. It is the share of M. bovis among camelids dead and submitted to a public Irish laboratory: a funnel that selects upward. Whoever cites it as prevalence is off by an order of magnitude, and always in the same direction.
  3. An aborted fetus submitted alone yields no diagnosis: zero out of seven in thirteen years. Submit fetus plus placenta, and accompany it with the dam's serum. On cria, by contrast, the laboratory answers in 79.3 % of cases: it is the class in which submitting pays off most.
  4. Cria is the only age class in which systemic disease exceeds digestive disease (43.3 % against 26.6 %), with bacterial etiology in 45.5 %. It points directly back to the two measures in cards C-01 and C-02: thermal protection in the first hours and verification of passive transfer.
  5. Whoever performs the necropsy is the exposed person. The only published case of transmission of M. bovis from camelid to human is cutaneous tuberculosis in a veterinarian (Twomey 2010): broken skin plus tissue handling. Gloves, eye protection and a mask in every necropsy of an emaciated camelid. Tuberculosis is notifiable: the suspect animal is submitted, not opened up on one's own.
  6. A negative intradermal test in a camelid is not a certificate of absence: estimated sensitivity 14-20 % in the species (Infantes-Lorenzo 2018). The scheme that reaches 90-100 % is the parallel prescapular intradermal test at 72 h plus serology 15-30 days after PPD (Bezos 2013).
  7. Fasciola hepatica is the second cause or co-cause of death, and it is not an exotic problem: on wet Italian pastures fascioliasis is present. The liver must be examined at every necropsy, and fascioliasis belongs in the differential of chronic weight loss alongside tuberculosis.

Card CRL-C-04 · version 1.0 · 31/08/2026 · synthesizes a single source, read only in the abstract and metadata: the article is open access under a CC BY 4.0 license but is not deposited in PubMed Central, has no copy in any archive known to Unpaywall, and the publisher's page returned HTTP 403 with an explicit reservation on automated extraction (tdm-reservation) on 31/08/2026 — reuse permission without access. All tables, counts, microbiological identification methods, stated limitations and conflict declarations remain to be read on the publisher's site. The arithmetic reconstructions of the denominators contained in this card are editorial and stated as such: they are not data from the article. It has not been searched for, and is therefore not excluded, whether similar national case series exist in other European countries; and no Italian source on tuberculosis in camelids has been searched for, which chapter 09 of the KT Camelidi does not know.
