Titolo originale

«Neonatal diseases in llamas and alpacas»

Original title

«Neonatal diseases in llamas and alpacas»

Italian title (editorial translation): «Malattie neonatali nei lama e negli alpaca».

⚠️ This is a narrative review of clinical orientation, written by a single author, and a substantial part of what it contains is declared experience («in the author's experience», «the author prefers»): the word «author» or «authors» appears 22 times in the body of the text. No number in this card is a result of the review; every number is reported, and where the source is someone else's work the card says so. The second warning concerns the doses: the review contains many, and almost all of them are the author's experience or extrapolation from other species. They are reported in a dedicated table, with an indication of how much has been measured in the camelid.

It allows the text to be read in order to write a card of our own; it does not authorize this card to republish tables or figures. This card may reproduce: factual data restated in our own words and brief quoted citations. No tables, no figures: the article's table (infectious agents of diarrhea by age of the cria) and the drawing of jugular catheterization are described in words.

Specie e soggetti

Nessun soggetto proprio.

Species and subjects

No subjects of its own. The review concerns llama and alpaca neonates, defined as cria of less than 2 weeks, and extends the section on diarrhea to unweaned cria (up to about 6 months). The data reported come mostly from United States llamas and alpacas (the table of diarrhea agents is titled for the United States), from guanacos of Argentine Patagonia, from Argentine llamas (serology), from alpacas of Peru (an outbreak of coccidiosis) and from six camels of a Spanish zoo (species not specified by the text, which writes «camels»). The text often invokes the foal as a point of comparison («not unlike that in foals») and borrows practices from it; the card flags this wherever the author does so.

Tipo di studio e livello di evidenza

Revisione narrativa clinica, senza strategia di ricerca dichiarata. Scopo dichiarato: «to cover the most common problems likely to present in neonatal crias and to give guidance on how to approach these cases, particularly how to stabilize the acute presentation of a sick neonate».

Study type and level of evidence

Clinical narrative review, with no declared search strategy. Stated aim: «to cover the most common problems likely to present in neonatal crias and to give guidance on how to approach these cases, particularly how to stabilize the acute presentation of a sick neonate».

Level of evidence: narrative review, hence a structured opinion of a clinician on a literature that the reader cannot reconstruct. The body is made of three materials that the card keeps distinct: (a) the published case material on the camelid (mostly small series and retrospective studies); (b) extrapolated data from foal, calf and lamb; (c) the author's experience, partly unpublished (three references to unpublished observations or data).

Consequence for the reader. The selection of the works discussed is not reproducible: this card summarizes one author's reading, not the state of the literature. Clinical statements and doses are cited as «the author reports». Where the text names the primary work, the card names it and refers the reader to it.

Relevant reporting standard: no reporting guideline exists for narrative reviews; the SANRA quality scale is used (chapter 14 §4.2). The author declares no standard. The score compiled by the editorial team, item by item, is in the ratings box: 6/12.

In breve

In brief

The review reconstructs the approach to the sick cria in the first two weeks of life: a rapid examination, an early intravenous catheter, oxygen, correction of hypoglycemia, fluids and plasma. The conditions covered are hypothermia and hypoglycemia, cerebral hypoxia, sepsis (a frequent consequence of failure of passive transfer), meningitis, brain and vertebral abscesses, umbilical infections, septic arthritis and diarrhea. For diarrhea the review goes through E. coli, rotavirus and coronavirus, Cryptosporidium, Giardia, coccidia and Salmonella, with the age at which each agent tends to appear and the relevant diagnostics. The author reports that crias respond to intensive care better than foals, and that those that respond in the first 24 hours often recover fully; the exceptions are untreatable congenital defects. The constant recommendation is admission to a facility with 24-hour care.

Il tessuto adiposo bruno «has not been specifically documented in camelids to date»; l'accuratezza degli analizzatori portatili dei gas ematici nei camelidi è «so far untested»; i vaccini monoclonali orali per vitelli e agnelli sono «of unknown efficacy in these species».

Why this study matters

First reason: it is the overall clinical text on the sick cria in llamas and alpacas. For a veterinarian seeing the first collapsed cria of a career, it gathers in a single reading the order of things to do, the laboratory data that guide the choice, the conditions not to be forgotten and the traps (hypernatremia from isotonic fluids, stress hyperglycemia, intraperitoneal transfusion).

Second reason: it says plainly what is not documented in the camelid. Brown adipose tissue «has not been specifically documented in camelids to date»; the accuracy of portable blood gas analyzers in camelids is «so far untested»; oral monoclonal vaccines for calves and lambs are «of unknown efficacy in these species». These are three sentences in which an author gives up an assertion, which is not common in a clinical review.

Third reason: it scales down the list of «possible» agents with percentages. For cria diarrhea it gives the frequencies of a 45-case study (see Main results) and says, for example, that enterotoxigenic E. coli «have not been specifically identified in llamas and alpacas» and that Salmonella is not a frequent cause. This is a useful correction for anyone who reads lists of agents without numbers.

Fourth reason: it is the piece where foal physiology is put to the test on the cria. The text states that crias «do seem to be remarkably responsive to intensive care in contrast to foals», that hypernatremia from isotonic fluids is a tendency of theirs, and that they are subject to a hyperosmolar hyperglycemic syndrome. These are clinical observations, not measurements, but they indicate where extrapolation from the foal is risky.

Obiettivo

Dichiarato dall'autrice: «This article attempts to cover the most common problems likely to present in neonatal crias and to give guidance on how to approach these cases, particularly how to stabilize the acute presentation of a sick neonate.»

Objective

Declared by the author: «This article attempts to cover the most common problems likely to present in neonatal crias and to give guidance on how to approach these cases, particularly how to stabilize the acute presentation of a sick neonate.»

Clinical question (editorial formulation; the PICO format does not apply to a descriptive review): in a llama or alpaca cria of less than two weeks that presents collapsed or sick [P], which conditions are to be considered, in what order are they assessed and stabilized, and which interventions does the author report as effective [outcome]?

Disegno dello studio

Study design

Narrative review of 18 pages, 51 references, one table and one figure. Structure: emergency management (assessment, catheterization, emergency therapy); then individual conditions (hypothermia and hypoglycemia, cerebral hypoxia, sepsis, meningitis, abscesses, umbilical infections, septic arthritis); then neonatal diarrhea, with one paragraph per agent.

⚠️ Not verifiable and therefore not asserted: the existence of a criterion for choosing the works (absent, as far as the text says), the age of some of the data (the article cites works from 1967 to 2008), and the date and location of the unpublished data.

Metodi / Protocollo

Methods / Protocol

The review has no methods. No database, no search string, no range of years, no language, no inclusion criteria. The sources include articles, chapters of textbooks (one of large animal medicine, one of South American camelid medicine, one of bovine therapy), a veterinary formulary, three references to unpublished data or observations and two cases cited by name without a bibliographic entry (an outbreak of coccidiosis in Peru and an epidemic of septic salmonellosis in two llamas).

Doses. The review contains doses of fluids, plasma, an anti-inflammatory, an aminoglycoside, insulin and antiparasitics. They are reported in the table that follows, each with dose, route, species and source, and with an indication of how much has been measured in the camelid. They are all numbers from an author who declares that she is reporting her own practice or citing a work: none comes from a clinical trial in the cria. The card does not recommend them.

The text further indicates that ionophores (monensin, salinomycin) «should not be used» in camelids because of toxicity, and that the use of milk replacer or goat milk in quantities above 15 % of body weight per day is associated with diarrhea in bottle-fed crias (observation of the authors). For antibiotics in neonatal infections, for antiparasitics beyond those in the table and for painkillers the review gives no doses.

Risultati principali

  • Fallimento del trasferimento passivo (FPT): sotto i 7 giorni, in emergenza, il plasma è «almost certainly going to be required».
  • Trasfusione intraperitoneale: «not recommended».

Ipotermia e ipoglicemia

Sepsi

Meningite, ascessi, ombelico, articolazioni

Le E. coli enterotossigene e le enteropatogene «have not been reported» in lama e alpaca.

«crias do seem to be remarkably responsive to intensive care in contrast to foals, and those that respond well in the first 24 hours of treatment will often proceed to make a full recovery.»

«Intensive management is frequently required, so referral to a hospital facility that is able to offer 24-hour care and in-house diagnostics should be carefully considered and offered to the owner.» — È una raccomandazione, non un risultato.

«it is important to maintain a clear head when evaluating these cases and to be thorough while acting promptly to deal with life-threatening conditions.»

Main results

(everything that follows is «the author reports»; the primary sources were not read)

Assessment and stabilization

  • Normal parameters of the cria (no source cited): rectal temperature 37.8-38.9 °C, heart rate 70-100/minute, respiratory rate 20-30/minute. The text warns that a heart murmur may depend on hypovolemia, sepsis or electrolyte changes, usually no higher than grade III/VI.
  • Hypopyon (pus in the anterior chamber): to be looked for as a sign of sepsis and treated with mydriatics (atropine drops) to prevent synechiae and glaucoma, up to enucleation.
  • Venous access: jugular, 18-gauge 2-inch catheter for up to 5 days, or 16-gauge polyurethane catheter over a guidewire for longer periods; peripheral veins are «hard to visualize» in crias and «not suitable for plasma administration»; in hypovolemia, intraosseous route in the proximal femur with an 18-gauge spinal needle. The drawing of the placement in sternal recumbency is, according to the text, the author's preference.
  • Oxygen for every «collapsing cria» until blood gas analysis is available; endotracheal tube size 4-6 if the cria is not breathing on its own.
  • Failure of passive transfer (FPT): under 7 days, in an emergency, plasma is «almost certainly going to be required». For immediate assessment the author indicates the refractometer: a total solids value below 4.5 g/dL is «consistent with FPT»; radial immunodiffusion, more accurate, takes 24 hours. The text cites a work that found no significant difference in the total proteins of septic crias with positive blood culture, with or without FPT (the title of the reference mentions 21 cases): total proteins are not a reliable indicator of IgG in sepsis.
  • Intraperitoneal transfusion: «not recommended». It lists abdominal pain if rapid, peritonitis, adhesions (with colic «up to 2 years later»), accidental puncture of organs, and the contraindication if the cria already has signs of sepsis.

Hypothermia and hypoglycemia

The text explains non-shivering thermogenesis in brown adipose tissue and states that in camelids the presence of that tissue «has not been specifically documented», so it is not certain that it plays a role. It cites a work in which llama cria had very high thyroxine concentrations at birth, declining gradually over the first 90 days. The hypothermic cria is depressed, has reduced reflexes, hypoventilates, and has less time to take colostrum before the gut stops absorbing it. Rewarming should be done by insulating from the heat source and rotating the cria in recumbency, because excessive heat causes skin vasodilation and lowers core temperature; the same is true of the hot bath.

Cerebral hypoxia

Cria born after a prolonged or difficult delivery may show the signs of the «dummy cria syndrome». The text gives no specific therapy: support is nutritional, with fluids if needed, and avoiding FPT; «usually after a few days they will tend to get going for themselves» (no denominator).

Sepsis

  • Definition and agents: sepsis as a systemic inflammatory response with documented or suspected infection. A retrospective study of crias with culture-positive sepsis (the title of the reference mentions 21 cases) found E. coli, Enterococcus, Listeria monocytogenes and Citrobacter as the most frequent isolates, with Gram-negatives in 54 % of diagnoses. ⚠️ The text does not give the denominator of that 54 %, and 54 % does not correspond to a whole number of animals out of 21 (11/21 is 52 %, 12/21 is 57 %): the share was probably calculated on isolates or on diagnoses and not on crias. The median age at presentation was 2 days; hypothermia, tachypnea and tachycardia were frequent, fever was not.
  • Hematology that guides: leukopenia and neutropenia more frequent in Gram-negative infections, leukocytosis and neutrophilia in Gram-positive ones (for example Listeria). Blood culture gives no answer before 48 hours: hence, for the author, the initial choice of an antibiotic active against Gram-negatives when there is leukopenia; «in the author's experience» the combination of a penicillin and an aminoglycoside gives the best clinical response (gentamicin dose in the table).
  • A cria with weight loss or failure to grow within 24 hours may have a normal physical examination and already a hematology compatible with sepsis: for the author, treating at this stage «may often avert a crisis presentation 12 to 24 hours later» (personal experience, no data).
  • Fluids and glucose: crias tend toward hypernatremia even with isotonic fluids such as lactated Ringer's; the author prefers a fluid with lower sodium content (0.45 % sodium chloride with 2.5 % dextrose if there is no hyperglycemia). Camelids are described as prone to stress hyperglycemia because of a poor pancreatic response and slow glucose clearance; healthy crias seem to cope better, but «this may not be true in sick crias».
  • Gram-negative infections in the neonate (a 1992 work, six cases): five llamas and one alpaca, age 1 to 5 days, positive blood culture; three had diarrhea and all died shortly after admission; E. coli was cultured in 3 cases out of 6, and it is not clear whether they were the ones with diarrhea.
  • Feeding: check whether the dam has milk. Llama milk is reported with 6.5 % sugars and 2.7 % fat, richer in sugar and poorer in fat than that of other domestic ruminants; goat or cow milk are, for the author, the most suitable alternatives (a 1995 work, denominator not given).

Meningitis, abscesses, umbilicus, joints

  • Meningitis and meningoencephalitis: agents reported in crias Listeria monocytogenes, E. coli, Salmonella Newport, Streptococcus bovis. Vague and variable signs (weakness, depression, inability to stand, tremors, ataxia, opisthotonos, seizures); in one case seen by the author the only initial sign was abdominal discomfort. A case of a 3.5-month-old llama (not a neonate) had 10,886 cells/µL in the cerebrospinal fluid, with a pure culture of Listeria. «None of the animals reported in the literature survived»: for the author the prognosis is guarded even with intensive care.
  • Abscesses: brain abscess in a 2-week-old alpaca due to E. coli (died) and in a 1-month-old alpaca due to Fusiformes (recovered after surgical removal by craniotomy); the author says she knows of at least two cases of brain abscesses cured by medical therapy alone (unpublished observations). Thoracolumbar vertebral abscesses in «several» crias under 6 months of age with paresis or paralysis of the hind limbs (no number). Discospondylitis in a 2-month-old llama with ataxia from the first week: treatment attempted with florfenicol and anti-inflammatories, then ceftiofur, without success; the cria died.
  • Umbilical infections: in the author's experience they are less frequent than in the calf, perhaps because of differences in management and cleanliness of the birthing environment. Abdominal ultrasound is always indicated in a cria with abdominal pain or umbilical swelling. Surgical resection by celiotomy without entering the cavity of the abscess, or drainage and marsupialization for large abscesses.
  • Septic arthritis: a possible outcome of FPT, with an unfavorable prognosis especially when several joints are involved (no number); therapy with systemic and intra-articular antibiotics and repeated joint lavage.

Neonatal diarrhea

  • Frequency: diarrhea is the most common cause of morbidity before weaning, with about 23 % of crias affected in a study of 250 crias on four farms over five years ⚠️ (source: unpublished data cited by the author, year 2000). The high density of commercial farms is given as the reason for the increased exposure.
  • Agents by age (the article's table, referring to the United States, described in words): E. coli under 7 days; coronavirus from 10 days; Cryptosporidium and Giardia from 7 days; coccidia from 21 days; Salmonella rare at any age.
  • A study of 45 unweaned llama and alpaca crias with diarrhea (2003 work): coronavirus in 42 % of cases by electron microscopy, in 64 % of the farms studied, age from 10 to 150 days, and it is the only agent in outbreaks that also affected adults; rotavirus in 2 %; Cryptosporidium in 9 %; Giardia in 18 %; Eimeria in 13 % (6 crias, aged 21-60 days); Salmonella in none of the 45.
  • E. coli: typical at 3-7 days, often with septicemia, secondary to FPT. Enterotoxigenic and enteropathogenic E. coli «have not been reported» in llamas and alpacas. In the author's experience crias are often leukopenic with a degenerative left shift.
  • Viruses: in Patagonian guanacos, two farms with outbreaks affecting 100 % of the herds and fatal in 83 % of cases (denominators not given), animals aged 1 day to 4 months kept in small yards and fed with bovine milk powder; rotavirus in the feces of two neonates (2 and 7 days). A serology on 390 Argentine llamas: antibodies against rotavirus in 88 %, in all nine farms, without clinical cases. Coronavirus by electron microscopy in two young llamas (12 days and 9 months). Therapy is supportive. For oral monoclonal vaccines for calves and lambs: «unknown efficacy in these species».
  • Cryptosporidium: the author writes that C. parvum is the most studied and is held responsible for most of the disease in mammals. Cases reported in llamas and alpacas in crias of 7 days-6 months. In a series of 20 alpaca crias (2008 work): in 13 of 20 no other gastrointestinal pathogen was present, signs generally lasted at least 7 days, the infection was mostly self-limiting and with good supportive care the outcome was favorable in 16 of 20; partial parenteral nutrition in 19 of 20. The author has diagnosed renal failure in several crias with cryptosporidiosis (no number), which she attributes to prolonged subclinical dehydration. For diagnosis: fecal smear with modified acid-fast stain, immunofluorescence, ELISA, PCR. «Effective specific treatment remains elusive». The text reports that 10 oocysts are enough to make a susceptible individual ill (source: a general review on the biology of the parasite, not a measurement in the camelid), and that oocysts resist in the environment and most disinfectants. In 354 Californian llamas from 3 weeks to 23 years of age no oocyst was found in the feces; in six camels of a zoo, asymptomatic, all shed oocysts over the course of a year, mostly through reinfection.
  • Giardia: 18 % of the 45 crias with diarrhea in the 2003 study, and 33 % of 58 crias in a retrospective hospital study (Whitehead and Anderson 2006). In the 354 asymptomatic llamas Giardia duodenalis was shed by 3.4 %, more often by crias; risk factors: more than 10 yearlings on the farm, small pens, units of more than 20 animals. Antiparasitic in the table.
  • Coccidia: six species of Eimeria in South American camelids (E. lamae, E. alpacae, E. macusaniensis, E. punoensis, E. peruviana, E. ivitaensis); the oocyst of E. macusaniensis measures 81-107 µm against 17-40 µm for the others; the prepatent period ranges from 10 days (E. punoensis) to 33-34 days (E. macusaniensis), that of E. alpacae and E. punoensis from experimental data on four llamas. The author has seen clinical coccidiosis in alpacas as early as 21 days. In an outbreak in southern Peru 12 alpacas aged 25-35 days died, eight with diarrhea and four of sudden death (source cited by name, without a bibliographic entry). A flotation study indicates that solutions with a specific gravity of 1.20 or less (saturated sodium chloride) may fail to detect the oocysts of E. macusaniensis, and that a sugar solution at 1.28-1.30 gives more reliable results. Prevalence of oocysts in healthy animals: three studies on 239, 144 and 443 animals, with more oocysts under one year of age in two of the three. Ionophores are not recommended. Therapy and prophylaxis in the table.
  • Salmonella: none of the 45 cultures; 76 healthy Californian llamas and 99 asymptomatic llamas and alpacas from Ohio were negative. One case of septic salmonellosis in a 6-year-old llama and in a 6-day-old cria (S. typhimurium in blood culture), both died, without diarrhea. The authors have cultured Salmonella (S. ohio, S. newport, S. choleraesuis) from animals under 6 months with severe diarrhea.
  • Other causes: a single published case of portosystemic shunt in an alpaca cria, with recurrent diarrhea until diagnosis at 5 months; dietary causes (rich spring pasture, grain overload, excess milk in bottle-fed crias); six cases of forestomach acidosis, including a 2-month-old llama; antibiotic-associated diarrhea, especially with the oral route.

Author's interpretation (attributed, not merged with the data)

«crias do seem to be remarkably responsive to intensive care in contrast to foals, and those that respond well in the first 24 hours of treatment will often proceed to make a full recovery.» — This is a clinical impression without a denominator: there is no survival number in the text.

«Intensive management is frequently required, so referral to a hospital facility that is able to offer 24-hour care and in-house diagnostics should be carefully considered and offered to the owner.» — This is a recommendation, not a result.

«it is important to maintain a clear head when evaluating these cases and to be thorough while acting promptly to deal with life-threatening conditions.» — Like the rest of the tone of the review, it is exhortation.

Implicazioni cliniche

  5. La diarrea del cria ha un'età. Sotto i 7 giorni: E. coli e FPT. Dai 7 giorni: Cryptosporidium e Giardia. Dai 10: coronavirus. Dai 21: coccidi.
  7. I cria collassati non sono tutti recuperabili. I difetti congeniti non trattabili (atresia delle coane, gravi anomalie cardiache) vanno cercati all'ingresso, perché un trattamento destinato a fallire è, per l'autrice, «further distress to the cria, its dam, and the owner».

Clinical implications

For the Italian veterinarian who follows an alpaca or llama farm, and for the association that has them in its care.

  1. The first act is measuring the temperature, not the drug. A hypothermic cria does not get up to suckle and becomes hypoglycemic and without colostrum: the chain is described by the author. Rewarming should be done with care (insulated heat, rotation of the cria, no overheating).
  2. Colostrum and FPT are the beginning of almost everything. Sepsis, umbilical infections, septic arthritis and E. coli diarrhea appear, according to the text, as a consequence of failed passive transfer. The rapid check with the refractometer (below 4.5 g/dL is compatible with FPT) has however a limit that the author herself declares: in sepsis, total proteins do not follow IgG.
  3. A cria that loses weight or does not grow in 24 hours should not be waited on. For the author a hematology may already show leukocytosis or leukopenia with a left shift, and early treatment avoids the crisis 12-24 hours later. It is experience, not data.
  4. The doses in the table are not a prescription. They are the practice of an author from a British veterinary school, with only two partially measured exceptions in the camelid (gentamicin in healthy adult llamas, insulin in a work on llama cria) and none proven in the cria by a dose study. The veterinarian who uses them must check them against the formulary and the indications in force in Italy, which this card has not verified.
  5. Cria diarrhea has an age. Under 7 days: E. coli and FPT. From 7 days: Cryptosporidium and Giardia. From 10: coronavirus. From 21: coccidia. It is the article's table, referring to the United States: it has not been verified on European farms.
  6. A cria with diarrhea and a suspected zoonotic agent is a risk for those who look after it. Cryptosporidium and Giardia are defined as zoonotic by the text; card C-05 gives the data on the alpaca. For those who run assisted interventions, a cria with diarrhea does not come into contact with the public (indication of the editorial team, not of the review).
  7. Collapsed crias are not all recoverable. Untreatable congenital defects (choanal atresia, severe cardiac anomalies) should be looked for on admission, because a treatment destined to fail is, for the author, «further distress to the cria, its dam, and the owner».
  8. The facility that the text requires is not available everywhere. The author recommends admission with 24-hour care; in Italy most crias are managed on the farm. The review does not say how to adapt the sequence to the field, except for mentioning the possible usefulness of portable analyzers, whose accuracy in camelids is still unproven.

Limiti

Dichiarati dall'autrice.

◆ COMMENTO DELL'ASSOCIAZIONE

Camelid Research Studies APS — redazione CRL · 11/10/2026

Valutazione a stelle

Voce SANRAPunteggioMotivo

Limitations

Declared by the author. The review has no limitations section; however, it declares in several places that data are lacking: «It is impossible to cover every eventuality»; brown adipose tissue is not documented in camelids; the accuracy of portable blood gas analyzers in camelids is «so far untested»; the efficacy of oral monoclonal vaccines is «unknown»; the data on meningitis come from isolated cases and partly from unpublished observations.

Added by the editorial team.

  • No declared literature search: the selection of the works is not reproducible.
  • Dosing is experience, not measurement. The dose table says, row by row, how much it is so; only two have a counterpart in works on the camelid: gentamicin (pharmacokinetics in healthy adult llamas) and insulin (a work on llama cria).
  • The comparison with the foal guides many choices (dextrose, plasma, premedication) and is not proof in the cria; the text declares it rarely and cites a foal textbook only for 50 % dextrose.
  • Unpublished and non-reconstructible data: the 23 % frequency of diarrhea in 250 crias; observations on brain abscesses and cerebrospinal fluid; cases cited by name without a bibliographic entry.
  • A percentage without a denominator: the 54 % of Gram-negatives does not correspond to a fraction of 21 crias; the 83 % mortality and the 100 % morbidity of the guanaco outbreaks have no denominator.
  • Hospital and United States case-mix: the table of agents is titled for the United States; the hospital data are of crias brought to a clinic, hence more severe than average.
  • A terminology slip: the text writes «oocysts» even for Giardia, which forms cysts. The card reports the numbers and not that word.
  • Sixteen years of distance: the review dates from 2009; it was not searched, and it is therefore not excluded, whether after 2009 the literature on the cria has changed the indications (including those on the measurement of passive transfer, card C-02).
  • Cited sources not read: everything the card says of a cited work it says through the review.

Funding: not declared in the text. No funder in the Crossref record or in that of Europe PMC (verified on 11/10/2026).

Conflicts of interest: not declared in the text (no statement found in the deposited text). The text names two commercial brands (a sulfonamide and a catheter); the card does not report them.

What is missing with respect to the reporting standard (SANRA). The description of the literature search (item 3) is missing altogether. Partial are: the justification of importance (item 1, without numbers on neonatal mortality), the formulation of aims (item 2, generic), the referencing (item 4, for unpublished data and sources cited by name) and the presentation of data (item 6, for the missing denominators). Outside SANRA but within the rules of the series: funding and conflicts not declared, no limitations section.

◆ ASSOCIATION COMMENTARY

Camelid Research Studies APS — CRL editorial team · 11/10/2026

A review useful above all for the order in which it puts things, and a table of doses to be read for what it is.

The merit of this article is not a result, it is a sequence: temperature, venous access, glycemia, plasma, then you think. For whoever sees the first collapsed cria, this is the part that is needed, and the author writes it so that it can be followed step by step. In our series it earns one extra point because it states clearly what in the camelid is not documented: brown adipose tissue, portable analyzers, monoclonal vaccines.

Now the part that makes us reflect as an editorial team: the doses. The table we built shows something that the review does not hide but does not underline: eleven doses, only two with a counterpart in works on the camelid: gentamicin, measured in healthy adult llamas, and insulin, in a work on llama cria. All the rest is one author's practice or numbers whose source is not indicated or is a textbook of bovine therapy. It is not a reproach: this is how one works when evidence is lacking. But a dose number, when it passes from a review to a card to a stable notebook, loses along the way the phrase «in my experience», and then the table becomes a prescription. This is why we wrote, row by row, what is measured and what is not. Whoever uses those numbers should check them against an up-to-date formulary: we have not done so.

The comparison with the review on reproductive losses (C-06) is instructive. That one lists the possible agents of cria diarrhea without percentages; this one gives the percentages of a 45-case study and scales down some names (no Salmonella in the 45; enterotoxigenic E. coli never identified in the llama and the alpaca). Read together, the two texts correct each other. The comparison with card C-05 adds one thing: the review reports that Cryptosporidium was not found in any of the 354 healthy Californian llamas, and that in the 45 crias with diarrhea it appears in 9 %; the Peruvian study of C-05 finds it in 159 of 1,312 neonatal cria. They are different populations and they do not add up, but they say that in the camelid the frequency depends on where one looks.

A sentence for whoever has a cria in hand. The text has no survival numbers, but it says that crias «do seem to be remarkably responsive to intensive care» and that those that respond in the first 24 hours often recover fully. It is not a promise, it is the clinical impression of a hospital that recommends 24-hour care. On a farm without that hospital, the part that remains applicable is the first: warm, measure glycemia, give colostrum and recognize early the cria that does not grow. Card C-01 (the vest that brought mortality to zero in the Andes) and C-02 (IgG measured with three methods) are the two readings that complete this one.

Star rating

SANRA compiled by the editorial team — 6/12.

SANRA itemScoreReason
1 · Importance1/2The urgency is well described («the emergency of the crashing cria»), but it is not quantified: no neonatal mortality data in the text.
2 · Aims and questions1/2Aim declared but generic: to cover the most common problems and indicate how to stabilize the cria. No defined question.
3 · Description of the literature search0/2Absent altogether.
4 · Referencing1/251 references, partly specific and partly textbook chapters; three references to unpublished data, two cases cited by name without an entry, and doses without a source.
5 · Scientific reasoning2/2It consistently distinguishes the published from personal experience and declares what is not documented in the camelid; it also reports a result that limits a commonly used test (total proteins in sepsis).
6 · Presentation of outcome data1/2Numbers with species almost always; denominators often absent (54 % of Gram-negatives, 83 % mortality in the guanaco).

Conversion SANRA → stars according to chapter 14 §11: 6-8 → ★★★☆☆.

AxisScoreRationale
A · Scientific quality★★★☆☆SANRA 6/12, with a zero on the literature search. Ceiling of chapter 14 §11: a narrative review does not reach five stars on this axis.
B · Clinical utility★★★☆☆It offers procedures that are replicable and described in detail (venous access, fluids, plasma, order of assessment, tests that guide the antibiotic, flotation for coccidia), to be used with the caution imposed by the level of evidence. It does not rise to four because the doses are almost all the author's experience or extrapolated, and the only measured counterpart is in healthy adult llamas; within the constraint B ≤ A + 2.
C · Educational value★★★★☆A training text of the first rank for order, clarity and frankness about its limits: a clinician who must learn camelid neonatal emergencies starts here. It does not reach five because it dates from 2009, because it uses unpublished data and because the later literature has not been searched.
D · Species relevance★★★★☆A review entirely dedicated to llamas and alpacas, written in a European university clinic, with case material largely North and South American. Transferability to Italy is good for the emergency sequence and more uncertain for the frequencies of the agents. It does not rise to five because the epidemiological data are not European and because in places it uses data from the foal or from six zoo camels.

No overall score, by construction. A = 3, C = 4 and D = 4 is the signature of a good clinical review: reliable as a guide to the order of things, fragile as a source of numbers.

Bottom line

It can be concluded that a clinical narrative review of 2009, written by a sole author from a British university hospital, reports for the llama and alpaca cria of less than two weeks a stabilization sequence (examination, venous access, oxygen, correction of hypoglycemia, fluids, plasma); that in a study of 45 unweaned crias with diarrhea cited by the review coronavirus was the most frequent pathogen (42 % of cases) and none of the 45 cultures was positive for Salmonella; and that the author herself declares as not documented in the camelid brown adipose tissue, the accuracy of portable analyzers and the efficacy of oral monoclonal vaccines.

It cannot be concluded that the doses of the review are proven in the cria: the table shows that only gentamicin has pharmacokinetic support, in healthy adult llamas, and that insulin cites a work on llama cria without saying whether healthy or sick, while the other nine doses are experience or extrapolation; nor that the frequencies of the diarrhea agents hold for Europe, because they come from a United States table and from a study of 45 cases; nor that crias respond better than foals to intensive care, because the author gives no survival data. No data on Italy can be derived from here.

Messaggi da portare a casa

Take-home messages

  1. A narrative review is cited together with its subject. Every number in this card is «the author reports»; the primary works were not read.
  2. The collapsed cria is stabilized in an order: temperature and cautious rewarming, jugular venous access, glycemia, plasma under 7 days, then the cause is sought.
  3. The doses of the review are largely experience: only gentamicin (pharmacokinetics in healthy adult llamas) and insulin (a work on llama cria) have a counterpart in the camelid. They are read in the table, row by row, with the species and the source.
  4. FPT is at the beginning of the chain: sepsis, umbilicus, joints and E. coli depend on it. The refractometer below 4.5 g/dL is compatible with FPT, but in sepsis total proteins do not follow IgG.
  5. Diarrhea has an age: E. coli under 7 days, Cryptosporidium and Giardia from 7, coronavirus from 10, coccidia from 21 (United States table).
  6. In the study of 45 crias with diarrhea cited by the review: coronavirus 42 %, Giardia 18 %, Eimeria 13 %, Cryptosporidium 9 %, rotavirus 2 %, Salmonella none.
  7. Intraperitoneal plasma transfusion: not recommended by the author, because of peritonitis, adhesions and puncture of organs.
  8. The author declares as not documented in the camelid brown adipose tissue, portable blood gas analyzers and the efficacy of oral monoclonal vaccines.
