Specie e soggetti

Nessun camelide.

Species and subjects

No camelid. The unit of analysis is not animals but instruments: twelve grimace scales developed for nine species — mouse, rat, rabbit, horse, piglet, sheep/lamb, ferret, cat, donkey.

This is the point of the card and must be stated at once: alpaca, llama, guanaco, vicugna, dromedary and Bactrian camel do not appear among the nine species, and they do not appear in the discussion either. Not because they were excluded: because no scale exists to be evaluated.

Tipo di studio e livello di evidenza

Revisione sistematica con protocollo registrato (SyRF #21-November-2019), riportata secondo PRISMA, con valutazione delle proprietà di misura secondo COSMIN (COnsensus-based Standards for the selection of health Measurement INstruments).

Dichiarati dagli autori: sì, entrambi, nell'abstract.

Study type and level of evidence

Systematic review with a registered protocol (SyRF #21-November-2019), reported according to PRISMA, with assessment of measurement properties according to COSMIN (COnsensus-based Standards for the selection of health Measurement INstruments).

Level of evidence: systematic review of validation studies — the highest rung of the hierarchy among the designs present in this issue. Consequence for the reader: the high level concerns the synthesis, not the instruments synthesized; the review establishes with good certainty how little is known about the measurement properties of existing scales, and for camelids it establishes an absence, not a data point.

Relevant reporting standard: PRISMA 2020 for the review; COSMIN as the assessment tool for the included instruments. Declared by the authors: yes, both, in the abstract.

In breve

In brief

Facial expressions of pain have been described in several animal species, and a grimace scale has been built for each. This systematic review collected all studies reporting the development, validation and measurement properties of such scales, and judged them with the COSMIN grid across six categories: internal consistency, reliability, measurement error, criterion validity, construct validity, responsiveness. Twelve scales for nine species emerged, with considerable variability both in how they were constructed and in the properties documented. Only four scales — mouse, rat, horse, cat — have a high level of evidence. Construct validity is the most studied property; reliability and other forms of validity are understudied. Camelids do not appear, for the elementary reason that no scale exists to be evaluated: not for alpaca, not for llama, not for the dromedary.

Why this study matters

Because it is the document that allows the association to state, with a source in hand rather than by impression, one sentence and its opposite.

The sentence: for South American camelids there is no validated instrument for facial expression-based pain assessment. Not a weak scale: no scale. Nor does even the prior rung exist, i.e. the anatomical study of facial musculature on which a coding system is built.

The opposite: this does not mean the path is impracticable. It means that it is known, has a measurable cost, and — this is the uncomfortable part — has already been walked on a species very similar in condition, the donkey, with a mediocre result. Whoever designs a pain scale for the alpaca must know in advance what awaits them, and this review tells them better than any enthusiasm could.

Obiettivo

Dichiarato dagli autori: «to provide evidence on the measurement properties of grimace scales for pain assessment».

Objective

Declared by the authors: «to provide evidence on the measurement properties of grimace scales for pain assessment».

Clinical question in PICO form (editorial formulation, the design allows it only in part): in a nonhuman mammal [P], a grimace scale [I] compared with another measure of pain or a known painful state [C], possesses documented and sufficient measurement properties — reliability, validity, responsiveness [O]?

Disegno dello studio

Study design

Systematic review with a protocol registered before conduct (SyRF #21-November-2019 — the register is that of the Systematic Review Facility for preclinical animal studies). Inclusion criteria: studies reporting development, validation or evaluation of the measurement properties of a grimace scale. Data extraction and assessment conducted by two independent researchers.

Metodi / Protocollo

  1. coerenza interna (internal consistency);
  2. affidabilità (reliability);
  3. errore di misura (measurement error);
  4. validità di criterio (criterion validity);
  5. validità di costrutto (construct validity);

Methods / Protocol

The six categories of measurement properties assessed, as listed by the authors:

  1. internal consistency;
  2. reliability;
  3. measurement error;
  4. criterion validity;
  5. construct validity;
  6. responsiveness.

The overall strength of evidence for each instrument — high, moderate, low — was assigned based on three joint elements: methodological quality of the studies, number of studies, and study results. It is not a judgment on the instrument itself: it is a judgment on how much it has been put to the test.

Risultati principali

Dodici scale, nove specie. Le specie: topo, ratto, coniglio, cavallo, suinetto, pecora/agnello, furetto, gatto, asino.

Livello di evidenza complessivoStrumenti
AltoMouse Grimace Scale (topo) · Rat Grimace Scale (ratto) · Horse Grimace Scale (cavallo) · Feline Grimace Scale (gatto)
ModeratoRabbit (coniglio) · Lamb (agnello) · Piglet (suinetto) · Ferret (furetto) Grimace Scale · Sheep Pain Facial Expression Scale (pecora)
BassoSheep Grimace Scale (pecora) · EQUUS-FAP (cavallo) · EQUUS-Donkey-FAP (asino)

Camelidi: zero strumenti, zero studi, zero righe.

Tomacheuski RM, Monteiro BP, Evangelista MC, Luna SPL, Steagall PV (2023). «Measurement properties of pain scoring instruments in farm animals: A systematic review using the COSMIN checklist». PLoS ONE 18(1):e0280830. DOI 10.1371/journal.pone.0280830 · PMID 36662813 · PMCID PMC9858734 (testo completo libero: sì, CC BY 4.0).

Nell'abstract: «Instruments with reported validation are urgently required for pain assessment of buffalos, goats, camelids and avian species.»

Main results

Twelve scales, nine species. The species: mouse, rat, rabbit, horse, piglet, sheep/lamb, ferret, cat, donkey.

Overall level of evidenceInstruments
HighMouse Grimace Scale (mouse) · Rat Grimace Scale (rat) · Horse Grimace Scale (horse) · Feline Grimace Scale (cat)
ModerateRabbit (rabbit) · Lamb (lamb) · Piglet (piglet) · Ferret (ferret) Grimace Scale · Sheep Pain Facial Expression Scale (sheep)
LowSheep Grimace Scale (sheep) · EQUUS-FAP (horse) · EQUUS-Donkey-FAP (donkey)

Four scales out of twelve reach a high level. Three out of twelve remain at a low level, and among these are both instruments of the EQUUS-FAP family, i.e. those of the equids that this issue indicates as the model.

Considerable variability among the scales both in the development process and in the properties documented. Construct validity is the most frequently reported property; reliability and the other forms of validity are understudied («Reliability and other forms of validity have been understudied»).

Camelids: zero instruments, zero studies, zero lines.

Authors' interpretation (attributed, not merged with the data)

The authors conclude that the review «identified gaps in knowledge on the measurement properties of grimace scales» and that future studies should focus on three things: improving psychometric testing, refining existing instruments, and using grimace scales for pain assessment in nonhuman mammals. This is a recommendation by the authors, not a result of the data.

The data point to be sought in a second article, and the citation that resolves only in the full text

The gap regarding camelids is not stated by this review — which simply does not name them — but by the twin review of the same group on farm animals:

Tomacheuski RM, Monteiro BP, Evangelista MC, Luna SPL, Steagall PV (2023). «Measurement properties of pain scoring instruments in farm animals: A systematic review using the COSMIN checklist». PLoS ONE 18(1):e0280830. DOI 10.1371/journal.pone.0280830 · PMID 36662813 · PMCID PMC9858734 (free full text: yes, CC BY 4.0). Twenty instruments for three species only: cattle, sheep, pig; thirteen instruments out of twenty with overall low or very low evidence.

In that article the sentence concerning camelids appears twice, with two different words. In the abstract: «Instruments with reported validation are urgently required for pain assessment of buffalos, goats, camelids and avian species.» In the discussion, the same sentence says camels. The peer review file, included in the PMC record, explains why: a reviewer wrote «Line 47. Llama, Alpaca?», and the authors replied «Corrected to camelids».

What can be drawn from this, in full. The authors deliberately broadened the term to camelids, at the explicit request of a reviewer who asked about llama and alpaca: the stated gap therefore also includes South American camelids, and this is not an extrapolation on the reader's part. But what the document does not say must be added: that review did not include any study on South American camelids — the species with instruments assessed are three, and the search term used was camel*. The sentence is a stated gap, not a result: it says that instruments are lacking, not that they were sought and assessed.

Implicazioni cliniche

Clinical implications

For the veterinarian facing a painful alpaca, today:

  1. No facial pain scale exists to use. None, for any camelid. Whoever proposes one is proposing it, not citing it.
  2. The scales with high evidence exist for mouse, rat, horse and cat — none of which is transferable by resemblance. The Horse Grimace Scale is the closest in size and in prey animal condition, and it is a scale for the horse: applying it to the alpaca means making an unverified assumption, and it must be recorded in the chart as such.
  3. Pain assessment in the camelid remains, to date, clinical and composite: posture, appetite, rumination, attitude within the group, response to handling, recumbency. These are signs, not a score; they have no inter-observer reliability measured in the species.
  4. Freezing is the trap. In the South American camelid the dominant behavioral sign under restraint and examination is immobility (Windschnurer et al. 2020, 116 alpacas and llamas in 20 herds): an immobile animal resembles a calm animal. None of the twelve scales in this review is built on a species that responds this way.

Limiti

Dichiarati dagli autori.

◆ COMMENTO DELL'ASSOCIAZIONE

Camelid Research Studies APS — redazione CRL · 31/08/2026

Valutazione a stelle

AsseVotoMotivazione

Limitations

Declared by the authors. The authors state that the review identified gaps in knowledge on measurement properties, and indicate psychometric testing and refinement of instruments as the two open fronts. This is not a list of limitations of their own work: it is a judgment on the field. An explicit list of the limitations of the review itself cannot be verified without the full text, which is subscription-access → to be checked against the full text.

Added by the editors.

  • The review assesses the instruments that exist, and therefore can say nothing about species that have none. The absence of camelids from this article is weak information — it is an absence from a list, not a finding — and it becomes strong only when read together with the stated gap in Tomacheuski 2023 and the exclusion search on Europe PMC reported in chapter 02 of the KT.
  • COSMIN originates in human medicine for instruments measuring patient-reported outcomes. Applying it to veterinary instruments is methodologically reasonable and by now customary, but it remains an adaptation: in human medicine criterion validity anchors to what the patient reports, which does not exist in veterinary medicine. This is the structural reason why criterion validity is the least documented property.
  • The count of «nine species» is a count of scales, not of literature. Two of the twelve scales concern the same species (sheep), and two the same family (equids).

Funding: the Europe PMC record classifies the article as «Research Support, Non-U.S. Gov't», but the funding statement is not readable in the accessible record, because the article is subscription-access and not deposited in PMC → to be checked against the full text.

Conflicts of interest: not verifiable for the same reason → to be checked against the full text. It should be noted that the Montréal group is the same one that developed the Feline Grimace Scale, one of the four scales to which the review attributes high evidence. This is not an accusation: it is information the reader has the right to have together with the result, not afterward.

What is missing relative to the reporting standard (PRISMA 2020). From the abstract and the record, the following cannot be verified: the flow diagram with the numbers of records found, excluded and included; the databases searched with strings and dates; the languages included and excluded. They are in all likelihood present in the full text — the review states that it follows PRISMA — but this card has not seen them and does not assert them.

◆ ASSOCIATION COMMENTARY

Camelid Research Studies APS — CRL editorial staff · 31/08/2026

The sequence that leads to a pain scale with high evidence has four rungs and is known: one does not arrive at a scale for the alpaca by collecting photographs.

It spares us from announcing, as others are doing, that «we are working on a pain scale for the alpaca» as if it were a matter of collecting photographs. The sequence that produced the high-evidence scales of this review is known and has four rungs, and the first is not photographic.

First rung: anatomy. EquiFACS — Wathan J, Burrows AM, Waller BM, McComb K (2015), PLoS ONE 10(8):e0131738, PMID 26244573 (an erratum exists, PLoS ONE 10(9):e0137818, PMID 26336096: whoever cites EquiFACS also cites the correction) — does not arise from observation, but from investigation of the underlying facial musculature, and only afterward from video-based behavioral analysis. It is the rung that makes a coding system verifiable rather than descriptive. For alpaca and llama this rung has never been climbed by anyone. It requires a comparative anatomist and dissection material, and by itself is worth months of work.

Second rung: a known and controlled painful context. The Horse Grimace Scale (Dalla Costa E, Minero M, Lebelt D, Stucke D, Canali E, Leach MC, 2014, PLoS ONE 9(3):e92281 — forty stallions surgically castrated, two analgesia groups, n = 19 and n = 21, plus six control horses for anesthesia) and the Donkey Grimace Scale both use surgical castration: a schedulable event, with a before and an after, with a manipulable level of analgesia.

Third rung: blind coding, with reliability calculated and diagnostic performance against a known state. Here lies the warning that this card exists to deliver, and it comes from the donkey. Orth EK, Navas González FJ, Iglesias Pastrana C, Berger JM, Le Jeune SS, Davis EW, McLean AK (2020), Animals 10(8):1411, PMID 32823676: nine adult male donkeys castrated, 54 pre- and post-operative photographs, observers with experience ranging from minimal to extensive. Reliability is excellent — Cronbach's α > 0.70 intra-observer, > 0.90 for the panel, panel inter-observer reliability 0.97. Diagnostic performance is just above chance: sensitivity 63.18%, specificity 62.07%, accuracy 62.60% — and the authors attribute these to the most significant indicators, which turn out to be general appearance and abnormal posture, not the facial action units alone. Observers' level of experience, knowledge and gender significantly influence (p < 0.001) the ability to score correctly.

This is the lesson, and it must be learned before spending a single euro: high inter-observer reliability is not a validation. It means that more people see the same thing; it does not mean that thing is pain. The same design that on the horse produced a high-evidence instrument, applied to a less expressive species, produced a low-evidence instrument (EQUUS-Donkey-FAP, in the table above).

Fourth rung: external evaluation of the measurement properties — which is exactly this article. It is the rung that separates «we have a scale» from «we have an instrument», and it always arrives, COSMIN in hand and without mercy.

What we would expect, if we were to do it. The South American camelid is a prey animal, with the face covered in fiber, with an expressive repertoire that no one has yet described, and with freezing as the dominant response to restraint. There is no reason to expect it to perform better than the donkey. There are several reasons to expect it to perform worse, and whoever funds the project must know this beforehand, not afterward.

Our position, therefore. A facial scale for the alpaca is not the first thing to do. The first thing to do is the anatomical study, which is cheaper, carries no risk of a null result, and is publishable work in itself. The second is multi-sign behavioral coding in already available contexts (shearing, examination, brief isolation) with three independent blind observers and calculated reliability — the rung that, to date, no study on camelids has ever completed. The face comes after, or does not come at all.

Star rating

AxisScoreRationale
A · Scientific quality★★★★☆Protocol registered before conduct (SyRF #21-November-2019), declared PRISMA reporting, COSMIN assessment across six categories, extraction by two independent researchers, explicit grading of the strength of evidence. The fifth star requires that «no conflict be undeclared»: funding and conflicts are not verifiable in the accessible record, and the author group developed one of the scales judged.
B · Clinical utility★★☆☆☆For those caring for camelids the actionable result is an absence: no instrument to apply. It suggests which extrapolation is least bad (horse) and marks it as a hypothesis to be verified, not applied. No dose, route or protocol.
C · Educational value★★★★☆It teaches, with twelve documented cases, the distinction between reliability and validity, and shows that criterion validity is the property missing almost everywhere. It is course material, not just reference material.
D · Relevance to the species★☆☆☆☆Camelids do not appear among the nine species nor in the discussion. Everything this card says about camelids comes from an absence, from a stated gap in a second article, and from context — not from this study. The CRL grid mandates the single star, and the card declares it.

No overall score, by design: A = 4 and D = 1 are not a contradiction, they are the information. An excellent article that says nothing about our species.

Bottom line

It can be concluded that there exist twelve grimace scales developed for nine species — mouse, rat, rabbit, horse, piglet, sheep/lamb, ferret, cat, donkey — and that none of them concerns a camelid; that four scales out of twelve reach an overall high level of evidence (mouse, rat, horse, cat) and three out of twelve remain low, among these both instruments of the EQUUS-FAP family, i.e. those of the equids that this issue indicates as the model; and that, among the six properties assessed with COSMIN, construct validity is the most documented while reliability and the other forms of validity remain understudied, with considerable variability among the scales both in development and in the properties reported.

It cannot be concluded from this article anything about camelids: they do not appear among the nine species nor in the discussion, and an absence from a list of instruments is not a finding. It cannot be concluded that the gap has been measured: that it exists is stated by a second article (Tomacheuski 2023, twenty instruments for three species only — cattle, sheep, pig), where it is a stated gap and not a result, because the search term was camel* and no study on South American camelids was included. It cannot be concluded that the Horse Grimace Scale is usable on the alpaca: the closeness in size and in prey animal condition is an editorial hypothesis to be recorded in the chart as such, not a data point of this review. It cannot be verified, from what this card has read, the list of limitations declared by the authors, the funding, the conflicts of interest, the PRISMA flow diagram, the databases searched and the languages included: the article is subscription-access and not deposited in PMC, and the card marks them to be checked against the full text — with the caveat that the author group developed one of the four high-evidence scales. And neither the primacy of freezing in the restrained camelid, nor the four-rung sequence, nor the 62.60% accuracy of the instrument on the donkey come from this study: they are material brought in from other sources within the association's commentary, which moreover treats as a single object the «Donkey Grimace Scale» of Orth 2020 and the EQUUS-Donkey-FAP of the table, without the read text declaring their identity.

Messaggi da portare a casa

Take-home messages

  1. For alpaca and llama no validated pain scale based on facial expression exists, and the anatomical rung that would make one possible does not exist either. Whoever offers one is offering a hypothesis.
  2. The four high-evidence scales are mouse, rat, horse and cat. The Horse Grimace Scale is the closest in size and condition; it remains a scale for the horse, and applying it to a camelid is an assumption to be recorded in the chart.
  3. High reliability is not validation. The donkey has a panel with reliability 0.97 and a diagnostic accuracy of 62.60%: more people see the same thing, but that thing is not pain.
  4. The gap concerning camelids is stated by the authors themselves, in the twin review of 2023, and at the explicit request of a reviewer who asked about llama and alpaca. It is not an extrapolation of ours: it is written.
  5. Freezing makes the camelid a difficult case from the start. In the species the dominant sign under examination is immobility, and none of the existing instruments was built on a response of that kind.
