Specie e soggetti
Species and subjects
Alpacas (Vicugna pacos) and llamas (Lama glama) — both, explicitly, and that is rare.
But the unit of analysis is not animals: it is 35 instructions for BCS assessment, collected from textbooks, scientific publications and theses, breeder association and government body documents, and veterinary service materials.
Study type and level of evidence
Scoping review, declared as such in the title, conducted according to PRISMA-ScR.
Level of evidence: synthesis review with a stated and reproducible search strategy — not a systematic review in the strict sense, because a scoping review by definition does not assess the risk of bias of the included sources. Consequence for the reader: the review establishes with certainty what has been written on the BCS of South American camelids and by whom; it does not establish which instruction is correct, because the data to decide that does not exist.
Relevant reporting standard: PRISMA-ScR — PRISMA Extension for Scoping Reviews — Tricco AC, Lillie E, Zarin W, O'Brien KK, Colquhoun H, Levac D, Moher D, et al. It is the only article in this issue in which the authors name their own reporting grid themselves.
In breve
An original study in which the BCS of SACs was the subject could not be found at the time of the literature search.»
In brief
The body condition of alpacas and llamas cannot be seen: under a dense fleece an emaciated animal can go unnoticed for months, and that is why all sources agree on one thing only — the BCS in South American camelids is palpated, not looked at. On everything else the sources do not agree. This scoping review compared thirty-five of them: ten textbooks, eleven scientific publications or theses, seven documents from breeding, welfare or government associations, seven materials from veterinary services. The two body sites on which the majority converge are the spine, in particular the lumbar vertebrae, and the ribs. The area between the forelimb and hindlimb and the pelvis are frequently cited but discussed in a contradictory way — on the pelvis there is one source whose most recent version advises against what the previous version recommended. The most used scale is the 1-5, with optimum 3, but the 35 instructions also include 0-5, 1-5, 1-9 and 1-10 scales, with different optima. And the finding that supports the whole card is in the full text, in a single line: «An original study in which the BCS of SACs was the subject could not be found at the time of the literature search.»
Why this study matters
Because it dismantles an illusion of measurement that the entire industry uses every day.
The BCS is the most widely used clinical score in the world in alpacas and llamas: it is in medical records, in breeding protocols, in management targets, in reports to owners, and in published correlations with hematological parameters and with behavior. It is the one number a camelid veterinarian writes almost every time.
This review shows that this number, taken without stating the scale and the site, means nothing. A "BCS 3" on a 1-10 scale is a thin animal; on a 1-5 scale it is an animal in optimal condition. A "BCS 3" palpated on the pelvis and a "BCS 3" palpated on the lumbar region are not the same assessment, and there are sources that contradict each other on which of the two is permissible.
Operational consequence, without qualification: a management target "BCS 2.5-3.5" is inapplicable until it is stated which of the 35 instructions it is calibrated on.
Obiettivo
Objective
Declared by the authors: to systematically compare the existing instructions for BCS assessment in South American camelids, for a consistent approach («For a consistent approach…»), and to check what they are based on.
Question in PICOS form — reported by the authors themselves in the text, and it is rare to find it written out in a veterinary review: Population alpacas and llamas; Intervention none; Comparison none; Outcome description of a BCS for alpaca or llama with a numeric scale; Study design all available document types.
Disegno dello studio
Study design
Scoping review — a choice justified by the authors on the grounds that the literature is continuously evolving and that a classic systematic review would not give an overall picture of it.
Eligibility criteria, declared: the source (a) describes a BCS in alpacas, llamas or both; (b) names at least one specific body site for the assessment; (c) uses a numeric score with specified upper and lower limits; (d) is published in English.
Admitted source categories: veterinary medicine or breeding textbooks (only the most recent edition, with earlier ones treated in a separate chapter); scientific publications (peer-reviewed or non-peer-reviewed journals, and theses); publications from breeding, welfare or government organizations; publications from veterinary services.
Metodi / Protocollo
`` ((body condition score) OR (body condition scoring) OR (BCS)) AND ((alpaca) OR (vicugna pacos) OR (llama) OR (lama glama) OR (South American camelid) OR (New World camelid)) ``
Deduplicazione automatica in EndNote X9 e controllo manuale in seconda battuta.
Methods / Protocol
Literature search: 29 and 30 August 2022, in four databases or platforms — Google Scholar, PubMed, Web of Science (Core Collection), CAB Abstracts (queried in both of its partitions, 1910-1989 and from 1990).
Search string, reported by the authors verbatim:
`` ((body condition score) OR (body condition scoring) OR (BCS)) AND ((alpaca) OR (vicugna pacos) OR (llama) OR (lama glama) OR (South American camelid) OR (New World camelid)) ``
Additional Google search with the terms "alpaca BCS" and "llama BCS", first 100 results examined for each, on 30 August 2022. Snowball search on the bibliographies of the sources found. Automatic deduplication in EndNote X9 and manual check as a second pass. Selection according to the PRISMA 2020 flow diagram, included as Figure 1.
Extraction: for each source, in a spreadsheet: source type, edition, authors, year, country, species, body sites assessed, examination order, scale used, optimal BCS, recommended examination frequency, presence of illustrations, and position taken on weighing versus scoring.
Declared exclusions, with names: sources in languages other than English — and among these appear Italian and German sources (Frezzato and Stelletta; Wagener and Ganter 2020; Gauly 2018; Franz 2017; Trah and Wittek 2013), i.e. part of the continental European literature; descriptions without numeric classification; scores published by companies or by individual breeders.
Risultati principali
Trentacinque fonti incluse, così ripartite:
| Categoria | n | % |
|---|---|---|
| Libri di testo (T) | 10 | 28,6% |
| Pubblicazioni scientifiche in rivista o tesi (S) | 11 | 31,4% |
| Associazioni di allevamento, welfare o enti governativi (A) | 7 | 20,0% |
| Servizi veterinari (V) | 7 | 20,0% |
«An original study in which the BCS of SACs was the subject could not be found at the time of the literature search.»
Frequenza raccomandata: a ogni manipolazione dell'animale, e comunque almeno mensile.
Wagener MG, Schregel J, Ossowski N, Trojakowska A, Ganter M, Kiene F (2023). «The influence of different examiners on the Body Condition Score (BCS) in South American camelids — Experiences from a mixed llama and alpaca herd». Frontiers in Veterinary Science 10:1126399. DOI 10.3389/fvets.2023.1126399 · PMID 36816196 · PMCID PMC9936059 (testo completo libero: sì, CC BY).
«It is unknown how exactly single examiners can reproduce their results when assessing the BCS.»
Main results
Thirty-five sources included, distributed as follows:
| Categoria | n | % |
|---|---|---|
| Textbooks (T) | 10 | 28.6% |
| Scientific publications in journals or theses (S) | 11 | 31.4% |
| Breeding, welfare or government associations (A) | 7 | 20.0% |
| Veterinary services (V) | 7 | 20.0% |
The result that should be read first, and which is in the Results section, not the discussion:
«An original study in which the BCS of SACs was the subject could not be found at the time of the literature search.»
Of the eleven "scientific" sources, eight are review articles (on nutrition, routine procedures, herd health, welfare) and three are original articles on an altogether different topic — body weight estimation in the llama, llama milk composition, and a thesis on alpaca feeding in Sweden. None of the 35 documents is an original study on the BCS. The seven association sources and the seven veterinary service sources were not found in any scientific database: only through the Google search and through snowballing.
Body sites. The spine (in particular the lumbar vertebrae) and the ribs are the two main sites, recommended by the majority. Area between the forelimbs: 24 of 35 sources (68.6%). Area between the hindlimbs: 21 of 35 (60.0%). Both: 20 of 35 (57.1%). The paralumbar fossa, shoulder and udder are cited only sporadically. The pelvis is frequently cited but treated in a contradictory way: for the same association, an older description recommended palpation of the pelvis and the more recent one advises against it.
Precision of the instructions. Fewer than half (17 of 35, 48.6%) indicate an examination order; 16 of 35 (45.7%) do not indicate one. 20 of 35 (57.1%) provide a description for each score, usually with schematic illustrations; 15 of 35 (42.9%) do not give an exact description for every score — and this group includes, the authors say, precisely the scientific sources.
Scales. The 1-5 with optimum 3 is the most cited, but the corpus also includes 0-5, 1-5, 1-9, 1-10, and several sources propose two of them at once.
Weighing versus scoring. 18 of 35 sources (51.4%) express a position on weighing animals. Eight (22.9%) consider both methods adequate; two indicate that in crias body weight is more meaningful than BCS; two state that BCS should be done in addition to weighing; only two (5.7%, both British textbooks) consider weighing more suitable.
Recommended frequency: at every handling of the animal, and in any case at least monthly.
Inter-observer agreement — the point where this card corrects its own premise
It is true that none of the 35 instructions carries agreement data with it. It is not true that agreement has never been measured: it has been measured once, by the same first author, the year before.
DOI 10.3389/fvets.2023.1126399 · PMID 36816196 · PMCID PMC9936059 (free full text: yes, CC BY). Verified on Europe PMC on 31/08/2026.
Twenty llamas and nine alpacas (n = 29 animals), six examiners, a single herd visit. A reference BCS ("gold standard") calculated from the results of the two most experienced examiners. The other examiners deviated by no more than 0.5 points from the reference in over 80% of the animals. Inter-observer agreement statistics: r = 0.52-0.89 · τ = 0.43-0.80 · weighted κ = 0.50-0.79, values that the authors describe as comparable to those of the BCS in sheep and cattle. Agreement is higher among the more experienced examiners.
And the gap that remains, declared by the review's authors in the discussion:
Intra-observer reliability has never been studied in alpacas or llamas. «It is unknown how exactly single examiners can reproduce their results when assessing the BCS.»
How the sentence should be written, then. Not "no evidence of inter-observer agreement", which is false. But: of 35 instructions, none carries its own agreement data; inter-observer agreement has been measured only once, on a single site (palpation of the lumbar vertebrae), with a single scale (1-5), on 29 animals of a single herd, by the group that recommends that scale; and the repeatability of the same examiner has never been measured.
Authors' interpretation (attributed)
The authors conclude that most of the information available on BCS in South American camelids is based on practical experience rather than on scientific evidence, and that some of the assumptions contained in the instructions may need to be verified or modified. They also argue that, unlike body weight, BCS reflects nutritional status even after a single examination — a claim that is argued, not measured, in this work.
Implicazioni cliniche
Clinical implications
- The camelid's BCS is palpated. The fleece hides emaciation: visual assessment alone is not an assessment. This also applies to the freshly shorn animal, where the risk is the opposite — appearing thinner than it actually is.
- The site to use is palpation of the lumbar vertebrae, with the ribs as the second site. It is the only one on which the majority of the 35 sources converge and it is the only one for which an inter-examiner agreement measure exists.
- The pelvis is the site not to use without stating it. There is one source that explicitly withdrew it between two editions.
- Scale 1-5, optimum 3, unless a stated reason for otherwise — and the scale must be written next to the number, every time.
BCS 3is not a datum;BCS 3/5, lumbar palpationis. - At least monthly, and in any case at every handling: it is the only examination that costs nothing and requires no instruments.
- In the cria, weight. Two sources out of 35 say so explicitly, and they are a minority only because the majority does not address the point.
- Agreement increases with experience. In a facility with several operators, the BCS recorded by untrained staff is not comparable to that of the veterinarian: if a historical series is needed, it must always be taken by the same hand, or it is the operator being measured rather than the animal.
Limiti
Dichiarati dagli autori.
Dichiarazione presente.
Approvazione etica: «Not applicable» — corretto, non ci sono stati animali coinvolti.
Cosa manca rispetto allo standard di reporting (PRISMA-ScR). Nessuna lacuna rilevante rilevata.
◆ COMMENTO DELL'ASSOCIAZIONE
Camelid Research Studies APS — redazione CRL · 31/08/2026
Seconda cosa, e vale come avvertimento a noi stessi.
Valutazione a stelle
Bottom line
Si può concludere che esistono, in lingua inglese, 35 istruzioni per il BCS di alpaca e lama — 10 libri di testo (28,6%), 11 pubblicazioni scientifiche o tesi (31,4%), 7 documenti di associazioni o enti (20,0%), 7 materiali di servizi veterinari (20,0%) — e che nessuna di esse è uno studio originale sul BCS: «An original study in which the BCS of SACs was the subject could not be found at the time of the literature search», e delle undici fonti «scientifiche» otto sono rassegne e tre sono lavori originali su altro argomento.
Limitations
Declared by the authors.
- Intra-observer reliability for the BCS of alpacas and llamas has never been studied.
- There is no agreement on which body sites are essential for a reliable BCS: besides the spine, additional sites are presented by different sources, but those sources give only superficial information on the interpretation and significance of findings obtained from the ribs or paralumbar fossa.
- The BCS, however standardized, will always remain a subjective assessment.
Added by the editors.
- English only. Criterion (d) excludes German and Italian sources that the authors themselves name — including one of their own previous works in German. For a European collection this is a real limitation: it means that the picture of the 35 instructions is not the picture of the instructions in use in continental Europe, but of those written in English.
- A quarter of the sources (7 of 35) come from breeder associations and another fifth from commercial veterinary services, neither category retrievable from a scientific database. The review correctly includes them and labels them — but whoever cites "the camelid BCS" without distinguishing is often citing promotional material from a farm.
- The search dates from 29-30 August 2022. As of the date of this card, four years have passed: the review does not contain its own first author's agreement study, published in February 2023, which is cited only in the discussion.
- The methodological conflict of interest is structural and must be named: the group that conducted the review is the same group that published the only existing agreement measure and that recommends lumbar palpation. There is nothing irregular about this — it is a small field — but an independent verification of the 35 instructions does not yet exist.
- The review does not establish which instruction is correct, and could not have. A scoping review does not assess the risk of bias of the sources. Anyone who cites it as though it had arbitrated the dispute between the 35 instructions is using it for something it does not do.
Funding: open-access publication funded by Projekt DEAL, by the Deutsche Forschungsgemeinschaft (programme LE 824/10-1, Open Access Publication Costs) and by the Stiftung Tierärztliche Hochschule Hannover. Declared.
Conflicts of interest: the authors declare that the research was conducted in the absence of any commercial or financial relationship that could be interpreted as a potential conflict. Statement present.
Ethical approval: «Not applicable» — correct, no animals were involved.
What is missing relative to the reporting standard (PRISMA-ScR). No relevant gap detected. Present and verifiable: question in PICOS form, eligibility criteria, databases with exact dates, full search string, handling of duplicates, PRISMA 2020 flow diagram, source categories, extraction fields, list of excluded sources with names, funding statement, conflict statement, data availability. It is the best-reported work in this issue, and in the camelid field it is an exception.
◆ COMMENTO DELL'ASSOCIAZIONE
Camelid Research Studies APS — redazione CRL · 31/08/2026
Thirty-five instructions, zero original studies, and a number that circulates throughout the entire industry as if it were a measurement. This is the snapshot, and it does not concern a niche curiosity: it concerns the most collected clinical datum in the world for our species.
First thing to do, and it costs nothing. From today, in every medical record, every protocol, every report to the owner, the association writes the BCS with the scale and the site:
BCS 3/5 — palpation of the lumbar vertebrae. NeverBCS 3. This is not pedantry: it is the only difference between a datum and a number, and this review shows that without those two additions the number is undecidable. Even if we did nothing else for a year, this one line would make us comparable with ourselves, which today we are not.Second thing, and it stands as a warning to ourselves. In chapter 02 of the KT there is a finding that this card explains: Budzyńska M, Kapustka J, Podobińska M (2026), BMC Veterinary Research 22(1):129, DOI 10.1186/s12917-026-05324-y, PMID 41618336 — 30 alpacas — finds that spitting during physical examination correlates negatively with BCS. It is an interesting and plausible finding: the animal in worse condition reacts worse. But that correlation is calculated on a BCS taken with one of the 35 instructions, on one farm, by one operator. The correlation is real, the metric is local. It is not a reference to build on: it is a hypothesis that someone needs to replicate with the scale stated.
Third thing, and it is our criticism of the work. The authors conclude that "most of the information is based on practical experience rather than on scientific evidence" and propose, correctly, lumbar palpation. We add what they do not write: that the only existing agreement evidence on camelid BCS was produced by the same authors, on 29 animals, in a single herd, on a single day, with a "reference" built from the two most experienced examiners of the same group. It is good work and it should be said that it exists — but a reference calculated by the most experienced examiners is not a known state: it is an internal consensus. The agreement measured is, in part, the agreement of a group with itself.
What would truly be missing, and is within our reach. No European project is needed. What is needed is a study that no one has done and that a single association can carry out in one season: the same herd, scored by multiple operators with declared experience levels, repeated a few days apart by the same operator blinded to their own previous score — that is, intra-observer reliability, which the authors state is unknown — and checked against an independent measure: weight, ultrasound fat thickness, or body composition. Without an independent state, we will keep measuring agreement between people instead of the accuracy of the instrument. It is exactly the mistake that card D-01 documents in the donkey, and here we have the chance not to repeat it.
And a note of reasoned pride. In the camelid field this is a well-made review: stated protocol, published string, exclusions named one by one, conflicts and funding disclosed. When the CRL cites the BCS, it cites this work. When someone cites "the standard alpaca BCS," ask them which of the thirty-five.
Valutazione a stelle
Asse Voto Motivazione A · Qualità scientifica ★★★★☆ Exemplary reporting according to PRISMA-ScR: PICOS, eligibility criteria, four databases with exact dates, full string, flow diagram, exclusions named, conflicts and funding disclosed. The fifth star is unreachable by construction: the CRL grid reserves it for those who report a risk-of-bias assessment of the included studies, and a scoping review by definition does not perform one. B · Utilità clinica ★★★★☆ Delivers a procedure applicable tomorrow morning with reasonable confidence and with explicit conditions of use: lumbar palpation, 1-5 scale with optimum 3, at least monthly frequency, obligation to state scale and site. It does not reach five because it does not change a recommended practice: it gives it, for the first time, a documented foundation. C · Valore didattico ★★★★★ It is the first-choice reference on the topic in the species: whoever needs to learn camelid BCS starts here, and moreover learns, along the way, how to read a literature made up of textbooks and association pamphlets. D · Rilevanza per la specie ★★★★★ Study on both species of interest to us, conducted in Europe, on a parameter of daily use, with data directly transferable to the European context. It is already taken up by subsequent literature. SANRA score, reported for transparency. The CRL grid uses SANRA (Baethge C, Goldbeck-Wood S, Mertens S, 2019, Research Integrity and Peer Review 4:5, DOI 10.1186/s41073-019-0064-8, PMID 30962953, PMCID PMC6434870) for narrative reviews. This is not one: it has a reproducible search strategy, so it falls under the main scale and is assessed with PRISMA-ScR. Scored anyway as a counter-check, it gives 12/12 (importance 2 · aims 2 · description of the search 2 · referencing 2 · reasoning 2 · data presentation 2). And the full score is, in itself, the lesson of this card: SANRA measures how the review is written, not how solid what the review finds is. A 12/12 on a corpus in which not a single original study exists is proof that the two things are independent.
Bottom line
It can be concluded that there exist, in English, 35 instructions for the BCS of alpacas and llamas — 10 textbooks (28.6%), 11 scientific publications or theses (31.4%), 7 documents from associations or bodies (20.0%), 7 materials from veterinary services (20.0%) — and that none of them is an original study on the BCS: «An original study in which the BCS of SACs was the subject could not be found at the time of the literature search», and of the eleven "scientific" sources eight are reviews and three are original works on other topics. It can be concluded that the camelid's BCS is palpated, that the two sites on which the majority converge are the spine — in particular the lumbar vertebrae — and the ribs, that the area between the forelimbs is named by 24 of 35 sources (68.6%), that between the hindlimbs by 21 of 35 (60.0%), both by 20 of 35 (57.1%), and that the pelvis is treated in a contradictory way, to the point of a source that advises against it after having recommended it in the previous edition. It can be concluded that the instructions are imprecise to a documented extent — only 17 of 35 (48.6%) indicate an examination order, 15 of 35 (42.9%) do not describe every score, and the corpus includes the scales 0-5, 1-5, 1-9 and 1-10 with different optima, with 1-5 with optimum 3 being the most cited; that 18 sources of 35 (51.4%) take a position on weighing and the recommended frequency is at least monthly; and that inter-observer agreement has been measured only once (29 animals, six examiners, weighted κ 0.50-0.79), while intra-observer reliability has never been studied, as the authors state.
It cannot be concluded which of the 35 instructions is the correct one. A scoping review does not assess the risk of bias of the included sources, and here there would have been nothing to arbitrate: the original study is missing. It cannot be concluded that this is the picture of the instructions in use in continental Europe: criterion (d) admits only English and excludes Italian and German sources that the authors themselves name, including one of their own previous works. It cannot be concluded that the picture is up to date: the search dates from 29-30 August 2022 and does not even contain its own first author's agreement study, from February 2023, cited only in the discussion. It cannot be concluded that that agreement is accuracy: the "gold standard" is calculated from the two most experienced examiners of the same group — an internal consensus, not a known state — on a single herd, in a single visit, on a single site and with a single scale, and no independent measure (weight, ultrasound fat thickness, body composition) was used as a term of comparison. It cannot be concluded that the BCS reflects nutritional status after a single examination: this is a claim argued by the authors and not measured in this work, and the card states so. And it does not come from this review, either the claim that BCS is the most widely used clinical score in the world in the species, or the negative correlation between spitting and BCS in 30 alpacas: the first is an unquantified editorial observation, the second is from another source, taken with one of the 35 instructions, on one farm, by one operator.
Scala 1-5 e scala 1-10 danno all'optimum due numeri diversi:
Take-home messages
- The camelid's BCS is palpated, not looked at, and the site on which the literature converges — and the only one with an agreement measure — is palpation of the lumbar vertebrae.
- A BCS without the scale and the site is not a datum. Scale 1-5 and scale 1-10 give the optimum two different numbers:
3/5is an animal in order,3/10is a thin animal. - None of the 35 instructions is an original study on the BCS. Eight of the eleven "scientific" sources are reviews; the other three are about something else.
- Inter-observer agreement has been measured only once — 29 animals, one herd, six examiners, weighted κ 0.50-0.79 — and the repeatability of the same observer never.
- The observer's experience changes the number. If a historical series is needed, it must always be taken by the same hand.
