Che cosa misurano i quattro assi
- A · Qualità scientifica
- La domanda è ben posta? I metodi sono dichiarati? La statistica è appropriata? La conclusione sta dentro i dati?
- B · Utilità clinica
- Serve a chi ha l’animale davanti, oggi.
- C · Valore didattico
What the four axes measure
- A · Scientific quality
- Is the question well posed? Are the methods stated? Is the statistics appropriate? Does the conclusion stay within the data?
- B · Clinical usefulness
- Useful to whoever has the animal in front of them, today.
- C · Teaching value
- Does it teach something to those studying the subject?
- D · Relevance to the species
- Is it really about camelids, or a result borrowed from another species?
Each axis is worth 1 to 5 stars and carries its written reason. The stars in the index are the rounded average of the four.

Titolo originale
«Ultrasonography of kidney and spleen in clinically healthy llamas and alpacas»
ultrasonography of kidney…).
Original title
«Ultrasonography of kidney and spleen in clinically healthy llamas and alpacas»
Formal note, useful to whoever copies this: in the PubMed record and in Europe PMC the title appears with a lowercase initial (
ultrasonography of kidney…). The Crossref record — which chapter 14 §10 indicates as the source for bibliographic details — reports it with a capital letter. Here Crossref is followed, and the discrepancy is flagged rather than passed over in silence.

The contextual fact that matters for the Italian reader: this is a European group, from a ruminant university clinic that has a dedicated camelid center, and the study was carried out on private Austrian farms, not at an Andean experimental station. It is the main reason this work is transferable here.

| Alpaca (Vicugna pacos) | Lama (Lama glama) | Totale | |
|---|---|---|---|
| Animali | 103 | 32 | 135 |
| Femmine | 76 | 23 | 99 |
| Maschi | 27 | 9 | 36 |
| < 1 anno | 7 | 0 | 7 |
| ≥ 1 anno | 96 | 32 | 128 |
Species and subjects

| < 1 year | 7 | 0 | 7 |
| ≥ 1 year | 96 | 32 | 128 |
29 alpacas were pregnant (pregnancy of less than six months: it was an inclusion criterion). Age: the youngest 4 months, the oldest 17 years. Mean ages as stated by the authors: alpaca < 1 year 5.3 months (SD ± 1.2); alpaca > 1 year 5.9 years (± 3.6); llama > 1 year 6.6 years (± 4.2).
Inclusion criteria, as stated: no pathological findings on clinical examination; body condition score between 2.5 and 3.5; females, if pregnant, within the sixth month of gestation. Animals were chosen «at random» within the herds that took part.
The number that must be read first, and it is not 135. The 7 alpacas younger than one year were too few for a separate analysis: they are excluded from the value tables and appear only in the age comparison. This study does not provide reference intervals for the cria. Whoever scans a two-month-old cria has, to date, no normal value to compare against.
Study type and level of evidence
Design: descriptive cross-sectional observational study on healthy animals, conducted in the field with a standardized protocol — in the taxonomy of chapter 14 §1.3 it is a normative study (of reference values). It is not a trial, there is no pathological comparison group, there is no blinding: none of the three could have been present, because the question is not "does this technique work better than that one" but "what does normal look like."
Level of evidence: low in hierarchical terms — it sits at the level of descriptive observational studies. Consequence for the reader, which is the field that matters: the values reported here can be used as a term of comparison in a report, stating the source, the species and the plane; a deviation from these values cannot be used as a diagnosis, because no one has yet measured how much a diseased kidney departs from these intervals. They are the first term of the comparison; the second does not yet exist in the literature.
Relevant reporting standard: STROBE (observational studies). The authors do not declare it and the journal did not require it; STROBE is used here retroactively as a lens to say what is missing, not as a reproach (chapter 14 §3.3 and rule 3 of the summary).
In breve
In brief
Before this work, a veterinarian who placed the probe against an alpaca's flank had no normal value to compare against what they saw: the literature described the technique on few animals, often sedated, and without dimensions. Eibl and Franz ultrasonographed 135 healthy, unsedated camelids on private Austrian farms, with a 6.6 MHz convex probe and 70% alcohol as the sole contact medium, without shearing, and published means, minima, maxima and standard deviations for the right kidney, left kidney and spleen, in two scanning planes and for each of the two species, with the sample size next to every single measurement.
Three things about this work change practice more than the numbers do: the left kidney can only be seen from the left and the right kidney only from the right (this is not so in ruminants, where both are seen from the right); shearing is not necessary; the spleen was seen in one animal out of four, and not seeing it is not a finding.
Why this study matters
Because it is the only existing normative ultrasonographic study of the abdomen of South American camelids with a sample size sufficient to produce intervals, and it was done in Europe, on privately owned animals, with an ambulatory-grade scanner.
And because it corrects, without saying so, an error that anyone coming from ruminants makes almost by reflex. In cattle, sheep and goats, the rumen occupies almost the entire left half of the abdomen and pushes the left kidney medially: both kidneys are seen from the right flank. In the camelid, compartment C1 is the functional analogue of the rumen, but it does not displace the left kidney: whoever looks for the alpaca's left kidney from the right will not find it, and concludes — wrongly — that something is wrong. The authors themselves note that the result is surprising.
Obiettivo
Objective
To determine position, shape, sonographic appearance and dimensions of the right kidney, left kidney and spleen in clinically healthy llamas and alpacas, and to establish whether differences exist by species, by age and by sex.
In PICO form, as far as a normative design allows it: P — clinically healthy llamas and alpacas, from European private farms, unsedated; I — percutaneous transcutaneous ultrasonography with a 6.6 MHz convex probe from the paralumbar regions; C — internal comparison across species, sexes and age classes (not across techniques); O — organ dimensions in centimeters and description of sonographic appearance.
Metodi / Protocollo
Strumento.
Gli autori registrano che «the wool only seriously interfered with interpretation of the sonogram in five animals» e motivano la scelta con una ragione economica esplicita: «the fleece is of great economic value to the owner and shearing for routine ultrasonography can be undesirable».
Contenzione.
Piani.
Statistica. Coefficiente di variazione dello strumento verificato a inizio studio (nessun effetto rilevante sulle misure). SPSS (versione dichiarata come «ver. 2018»).
Methods / Protocol
Instrument. MyLab™One VET ultrasound scanner (Esaote, Genoa, Italy), 6.6 MHz convex probe. The authors note that this is a frequency «similar» to that of many devices already present in clinics: it is an observation, not a measurement, but it is the reason the protocol is replicable without buying anything.
Preparation — and this is the most operationally important point of the work. The animals were not sedated and were not sheared: the fibers in the area of interest were parted with the fingers, and the sole contact medium between probe and skin used was alcohol (70% ethanol). The authors record that «the wool only seriously interfered with interpretation of the sonogram in five animals» and justify the choice with an explicit economic reason: «the fleece is of great economic value to the owner and shearing for routine ultrasonography can be undesirable».
Restraint. The animals were held by the owners with a halter only.
Acoustic windows.
- Left kidney: left paralumbar region, probe at the level of the transverse processes from L4 to L7, immediately ventral to the bony structures.
- Right kidney: right paralumbar fossa, immediately caudal to the last rib, ventral to the lumbar vertebrae — and more cranial than the position used on the left.
- Spleen: left paralumbar fossa, probe cranial to the left kidney, at the level of the transverse processes from L3 to L7.
Planes. Every organ scanned in both sagittal and transverse plane, in the same anatomical regions.
What was measured, and where exactly. Kidneys: length (distance between cranial pole and caudal pole) and width (between medial and lateral margin) in sagittal plane, through the region of the medullary pyramids; width in transverse plane at the level of the renal hilus; thickness of cortex and medullary pyramids in both planes. Spleen: width in sagittal plane, between lateral and medial margin, before its visible physiological flexure, at the level of the caudal pole of the kidney. Measurements performed on frozen and archived images, with electronic calipers and MyLab_Desk software.
Statistics. Instrument coefficient of variation checked at the start of the study (no relevant effect on measurements). SPSS (version stated as «ver. 2018»). Normality with Kolmogorov-Smirnov. Comparisons across species, sexes and age classes with Student's t-test, significance level 5% (P < 0.05), 95% confidence intervals reported for every comparison.
Ethics. Procedures discussed in advance with the owners, who signed a consent letter, and approved by the institutional ethics committee of the University of Veterinary Medicine Vienna, in accordance with Good Scientific Practice guidelines and national legislation. The protocol approval number is not reported in the article.

| Milza (qualunque misura) | 35 | 25,9 % |

| Misura | P | Significativa? |
|---|---|---|
| Rene sx, sagittale — lunghezza | 0,001 | sì |
| Rene sx, trasversale — corticale | 0,045 | sì |
| Rene dx, sagittale — larghezza | 0,007 | sì |
| Rene sx, sagittale — larghezza | 0,061 | no |
| Rene sx, trasversale — larghezza | 0,282 | no |
| Rene dx, sagittale — lunghezza | 0,380 | no |
| Rene dx, trasversale — larghezza | 0,254 | no |
| Milza, sagittale — larghezza | 0,123 | no |
| Piramidi midollari (tutte le sedi e piani) | 0,193 – 0,994 | no |
- la ragione della maggiore dimensione degli organi del lama sia la differenza di taglia corporea fra le due specie — e dichiarano esplicitamente che non hanno raccolto i pesi, trattandosi di uno studio sul campo, e che quindi non hanno potuto testare la correlazione fra dimensione dell'organo e peso;
- la forma triangolare del rene sinistro nel 27,3 % dei casi possa dipendere dalla vicinanza embriologica alla milza «or even due to undetected pathological change of the kidney» — cioè gli autori stessi non escludono che una parte di quei reni triangolari fosse patologica;
Main results (data)
How many animals gave each measurement — to be read before the tables
Out of 135 animals, the percentage in which each measurement was obtained (denominator 135, both age classes, both species):
| Measurement | n | % of 135 |
|---|---|---|
| Left kidney, sagittal — length | 92 | 68.1% |
| Left kidney, sagittal — width | 90 | 66.7% |
| Left kidney, sagittal — cortex and pyramids | 45 | 33.3% |
| Left kidney, transverse — width | 38 | 28.1% |
| Left kidney, transverse — cortex and pyramids | 17 | 12.6% |
| Right kidney, sagittal — length | 83 | 61.5% |
| Right kidney, sagittal — width | 82 | 60.7% |
| Right kidney, sagittal — cortex | 71 | 52.6% |
| Right kidney, sagittal — pyramids | 70 | 51.9% |
| Right kidney, transverse — width | 48 | 35.6% |
| Right kidney, transverse — cortex | 36 | 26.7% |
| Right kidney, transverse — pyramids | 33 | 24.4% |
| Spleen (any measurement) | 35 | 25.9% |
Stated reason for missed measurements: defensive reactions and adverse behavior by the animal; five animals excluded for poor image quality (three males and two females, all alpacas).
Sonographic appearance
- Kidneys: smooth margins, surrounded by a thin hyperechogenic line (capsule). Uniformly echogenic cortex, slightly hypoechogenic relative to splenic parenchyma; hypoechogenic, circular medullary pyramids; renal sinus as a thin hyperechogenic band; hyperechogenic hilus. Arteries, veins and ureters not differentiable without Doppler; the normal, non-dilated ureter was not visualized in any animal, in either of the two planes.
- Shape of the left kidney in sagittal: oval in the majority, but triangular in 30 of 92 animals (27.3%), regardless of species. In transverse: from oval to circular in all animals; at the level of the hilus the medullary pyramids take on the shape the authors describe as an «inverted Euro sign».
- Spleen: L-shaped in sagittal plane in all camelids in which it was seen; homogeneous and echogenic parenchyma; anechogenic vessels; capsule as a thin hyperechogenic line. Position: caudal to compartment 1 and craniolateral to the left kidney, with the flexure in a medial direction at the level of the caudal pole of the left kidney.

Watch three rows of these tables. The cortex and pyramid measurements of the llama in transverse plane rest on 5 animals (left) and on 10-12 (right). An interval built on five subjects is not a reference interval: it is an indication of order of magnitude, and should be written as such in the report. The same applies to the spleen of the alpaca (n = 20) and of the llama (n = 13).
What turned out to be significant, and what did not
Between the two species (animals ≥ 1 year; Student's t-test, 95% CI reported in the article for every comparison). Only three measurements out of fifteen were significant:
| Measurement | P | Significant? |
|---|---|---|
| Left kidney, sagittal — length | 0.001 | yes |
| Left kidney, transverse — cortex | 0.045 | yes |
| Right kidney, sagittal — width | 0.007 | yes |
| Left kidney, sagittal — width | 0.061 | no |
| Left kidney, transverse — width | 0.282 | no |
| Right kidney, sagittal — length | 0.380 | no |
| Right kidney, transverse — width | 0.254 | no |
| Spleen, sagittal — width | 0.123 | no |
| Medullary pyramids (all sites and planes) | 0.193 – 0.994 | no |
By sex. In the llama, no measurement was significantly different between males and females. In the alpaca, three: width (P = 0.022) and length of the right kidney in sagittal (P = 0.000), and cortex of the right kidney in transverse (P = 0.036).
By age (alpaca only). Despite the only 7 alpacas younger than 1 year, the age comparison gives seven significant measurements out of fifteen: left kidney sagittal width (P = 0.021) and length (P = 0.000), left kidney transverse width (P = 0.004), right kidney sagittal width (P = 0.001) and length (P = 0.001), right kidney transverse width (P = 0.001) and cortex (P = 0.014). The spleen does not differ by age (P = 0.281).
Authors' interpretation (attributed, not merged with the data)
The authors conclude that percutaneous ultrasonography of kidney and spleen in llamas and alpacas is «a feasible technique», performable without shearing and with alcohol as the sole contact medium, and that the study provides reference values usable for comparing sick animals.
The authors hypothesize, without having measured it, that:
- the reason for the larger organ size in the llama is the difference in body size between the two species — and they explicitly state that they did not collect the weights, since it was a field study, and therefore could not test the correlation between organ size and weight;
- the triangular shape of the left kidney in 27.3% of cases might depend on the embryological proximity to the spleen «or even due to undetected pathological change of the kidney» — that is, the authors themselves do not exclude that some of those triangular kidneys were pathological;
- the more caudal position of the left kidney might depend on its contiguity with the spleen.
The authors observe, comparing with small ruminants, that the renal dimensions of llama and alpaca are similar to those of goat and ram despite the much greater body weight, and draw from this the consideration — stated as such — that «larger animals do not necessarily have correspondingly larger internal organs».
The authors recommend further studies on the influence of breed and conformation.
What is not a finding of this study. The claim circulating that «alpaca intervals are not applicable to the llama without a scale correction» does not come from this article: here twelve measurements out of fifteen do not differ significantly between the two species, and the intervals overlap widely (left kidney length: llama 6.46-12.04 cm versus alpaca 5.61-11.00 cm). The correct conclusion is more modest and more useful: the species must be stated on the report and its table used, but a single measurement at the limit discriminates neither species nor pathology.
Implicazioni cliniche
1. Da che lato ci si mette. «In camelids the left kidney only could be viewed via the left abdominal approach and the right kidney via the right abdominal approach.»
Clinical implications
1. Which side to stand on. «In camelids the left kidney only could be viewed via the left abdominal approach and the right kidney via the right abdominal approach.» In ruminants, scanning is done almost entirely from the right. Whoever applies the cattle habit to the camelid does not find the left kidney and risks reporting an agenesis or a nephrectomy that does not exist.
2. No shearing. Fiber is parted with the fingers and 70% alcohol is used. Out of 135 animals, the fiber seriously compromised the image in 5. It is a fact worth an entire conversation with the owner: abdominal ultrasonography does not cost the fleece.
3. No sedation. All 135 animals were examined unsedated, held by the owner with a halter. Missed measurements are due to behavior, not to the technique: the cost is paid in lost images, not in anesthetic risk.
4. The plane must be stated and compared with the plane. This is the easiest implication to get wrong. In the alpaca the transverse width of the left kidney (4.98 cm) exceeds the sagittal width (4.19 cm) by 0.79 cm; in the llama the gap is 0.63 cm (5.79 versus 5.16). Measuring in transverse and comparing with the sagittal interval produces a non-existent nephromegaly. The plane must be written on the report.
5. A spleen not seen is not an absent spleen. It was visualized in 35 of 135 animals (25.9%): three times out of four it is not seen in a healthy animal. Failure to locate the spleen is not a finding — and this single line prevents a recurring diagnostic alarm.
6. The normal ureter is not seen. In none of the 135 animals, in either of the two planes. Seeing a ureter in a camelid means it is dilated.
7. The cria has no values. Seven animals under one year, excluded from the tables. And the age comparison, even on that tiny sample, is significant on seven of fifteen measurements: that is, the young animal's kidney is measurably different, and using the adult's table on a cria is methodologically wrong.
Limiti
Dichiarati dagli autori (citati, non riassunti)
- Nessun esame di laboratorio a corredo. «By selecting preferentially for numbers, it was not feasible simultaneously to perform laboratory tests such as haematology and serum biochemical analyses.» Conseguenza che gli autori scrivono da sé: «It cannot be excluded that some animals may have been experiencing relevant sub-clinical diseases that might have influenced the appearance and sonographic measurements of the scanned organs.» La loro difesa — «this possibility is unlikely as the variability of the measured values show, unless all animals were affected» — è un argomento, non una misura.
- Cautela esplicita sulle differenze per sesso ed età nell'alpaca: «measurement results must be interpreted with caution due to the relatively low number of animals represented.»
Limitations
Stated by the authors (quoted, not summarized)
- No accompanying laboratory testing. «By selecting preferentially for numbers, it was not feasible simultaneously to perform laboratory tests such as haematology and serum biochemical analyses.» Consequence the authors write themselves: «It cannot be excluded that some animals may have been experiencing relevant sub-clinical diseases that might have influenced the appearance and sonographic measurements of the scanned organs.» Their defense — «this possibility is unlikely as the variability of the measured values show, unless all animals were affected» — is an argument, not a measurement.
- Body weights not collected, so it is impossible to correlate organ size and body size: the explanation for the difference between species remains a hypothesis.
- Low sample size for the llama, indicated by the authors as responsible for the lack of statistical significance in the llama group.
- Explicit caution on sex and age differences in the alpaca: «measurement results must be interpreted with caution due to the relatively low number of animals represented.»
- Failed scans due to lack of cooperation or excessive procedure duration.
- Comparisons with goat and ram are not directly comparable because the measurement sites vary between species.
Added by the editors
- The alpaca's age comparison rests on 7 animals against 96 and nonetheless produces seven significant results out of fifteen. A P of 0.000 on a group of seven is not a reliable estimate of the difference: it is a signal that a difference exists. The magnitude of that difference, in this study, is not estimable, and indeed the authors do not publish separate values for the young.
- The "clinically healthy" criterion is a physical examination, not a diagnostic workup. For a renal reference-value study this is a relevant design limitation: subclinical nephropathy does not necessarily alter the physical examination, and renal function was not measured in any animal. The authors say so; we add that it is the limitation that most affects the use of the numbers, because it shifts the intervals in the wrong direction (widening them) precisely in the organ in question.
- The 29 pregnant alpacas are not analyzed separately. Pregnancy within the sixth month was an inclusion criterion, but it does not appear as a variable in any of the three comparisons (species, sex, age). Out of 76 female alpacas, 29 pregnant is 38%: it is not a negligible subgroup, and abdominal topography changes in pregnancy.
- The significance tables report the 95% CI of the difference but not the mean differences, which makes some rows difficult to read: for the left kidney transverse cortex, P = 0.045 (significant) with a CI of the difference from −0.53 to +0.14, i.e. an interval that contains zero. The two pieces of information are not consistent with each other; the article does not comment on this. This cannot be resolved from here →
[TO BE CHECKED AGAINST THE FULL TEXT]— specifically, the authors should be asked whether in Table 3 the intervals reported are those of the difference between means or another interval. - The statistical software version is given as "IBM SPSS ver. 2018", which is not an existing version designation. Irrelevant for the results, relevant for reproducibility.
- No ethics protocol number. Approval is stated, its identification is not — unlike, for example, the study of card CRL-F-02, from the same group, which reports
ETK-018/01/2022. - No inter-operator repeatability measure. Measurements were performed by a single sonographer; there is no ICC, no comparison between observers. The study of card CRL-F-02, four years later and partly by the same authors, does this: it is a within-group improvement in method, and signals that it was missing here.
- Voce 9 (bias): nessuna sezione dedicata.
- Voce 19 (limiti): presenti e discussi, bene.
- Voce 22 (finanziamento): dichiarato come «Not applicable» — vedi sotto.
What is missing relative to the reporting standard (STROBE, applied retroactively as a lens)
STROBE 2007 is the checklist for observational studies (chapter 14 §3.5). Item by item, what cannot be found in the article:
- Item 1 (title/abstract): the design is not named in the title or in the abstract. It can be inferred, not stated.
- Item 5 (setting): neither the number of farms visited nor the data collection period are stated. For a field study both matter: it is not known whether the 135 animals come from three herds or from thirty, nor in which season. It is the most serious gap in the list, because clustering by farm cannot be assessed.
- Item 6 (participants): the inclusion criteria are present; the exclusion criteria are incomplete (the five animals excluded for image quality appear in the results, not in the methods). A participant flow diagram is missing.
- Item 9 (bias): no dedicated section. Selection bias — herds that took part, animals «chosen at random» without a description of the randomization — is not discussed.
- Item 10 (study size): no sample size calculation, no justification. The authors state they aimed at the highest possible number: it is honest, it is not a calculation.
- Item 12 (statistical methods): it is not stated whether and how missing data were handled, which here are the rule rather than the exception (up to 87% for some measurements). No correction for multiple comparisons is stated, even though 45 tests were performed (15 measurements × 3 comparisons). With α = 0.05 and 45 tests, two to three significant results are expected by chance: and the species comparisons give exactly three.
- Item 13 (participants): there is no breakdown of animals by farm nor a description of non-participants.
- Item 16 (main results): the tables give means, SD, minima and maxima, but not the reference intervals properly so called (for example 2.5-97.5 percentiles, as the ASVCP guideline for reference values would construct them). Calling them "reference values" is the authors'; they are descriptive statistics of a convenience sample. The difference is practical: a minimum and maximum observed on 26 llamas say less than a percentile built on a sample sized for the purpose.
- Item 19 (limitations): present and well discussed.
- Item 22 (funding): stated as «Not applicable» — see below.
Not missing, and it must be said: the sample size of every single measurement is published. It is exactly what almost no veterinary normative study does, and it is the reason these numbers can be used with judgment rather than blindly.
Finanziamento e conflitti di interesse
- Finanziamento: «Funding — Not applicable.» Nessuna fonte esterna dichiarata.
- Conflitti di interesse: «The authors declare that they have no competing interests.»
Commento dell'associazione — Camelid Research Studies APS · redazione CRL · 31/08/2026
Dove dissentiamo dagli autori.
Valutazione a stelle
Funding and conflicts of interest
- Funding: «Funding — Not applicable.» No external source declared. The authors thank E. Haslauer (ESAOTE Austria) for the technical assistance provided during the investigation, and A. Tichy (bioinformatics and biostatistics platform of the same university) for statistical supervision.
- Conflicts of interest: «The authors declare that they have no competing interests.»
Editorial observation, and it must be kept separate from the fact. The instrument used is an Esaote MyLab™One VET, and in the acknowledgments the technical assistance of Esaote Austria appears. It is not a declared conflict and there is no reason to believe the results are influenced by it: technical assistance on a device is not funding, and anatomical values do not depend on the brand. It is written down because chapter 14 §5.4 requires that the reader have this information together with the result and not afterward, and because the ICMJE reminds that «perceptions of conflict of interest are as important as actual conflicts of interest».
Association's comment — Camelid Research Studies APS · CRL editorial team · 31/08/2026
This is the most useful work in issue F, and not because it is the most rigorous. It is not: it is a descriptive study on a convenience sample, without a sample size calculation, without correction for multiple comparisons and with a criterion of "health" that is a physical examination. It is the most useful because before it, in this species, there was nothing, and because it does something almost no veterinary normative study does: it publishes the sample size of every single measurement. Thanks to that column, a veterinarian can decide for themselves which row to trust. Rows of 60 animals can be used; those of 5 are cited as an order of magnitude. Without that column we would have had to treat the entire table with the distrust owed to its worst row.
Where we disagree with the authors. On the phrase "reference values", used in the title of the conclusions and in the abstract. What Tables 1 and 2 contain are mean, SD, minimum and maximum of a convenience sample, not reference intervals built according to a stated method. It is a difference that veterinary hematology has learned at its own expense, and that ultrasonography is repeating. The practical consequence is not theoretical: the observed maximum is not an upper limit of normality. A llama's left kidney of 12.1 cm is not pathological because it exceeds by six hundredths the maximum observed in 26 animals.
Where the work surprises, and should be looked at twice. The triangular shape of the left kidney in 30 of 92 animals is presented as a physiological variant, but the authors themselves admit it could be «undetected pathological change». In a study that defines the normal, this is a crack: 27.3% of left kidneys have a shape the authors cannot explain and do not exclude as pathological. Whoever uses these intervals must know this. It is not a reason to discard them — it is a reason not to treat them as definitive.
What this study makes possible and no one has yet done. The term of comparison for the healthy now exists. The term of comparison for the sick is entirely missing: no one has published how much the kidney of an alpaca with obstructive urolithiasis, or with renal insufficiency, measures, or how much the spleen enlarges in a torsion. Until that second study exists, these numbers serve to recognize the normal, not to diagnose the abnormal, and the difference is everything. It is the card we would like to write first, and it could be done at a single clinic on current caseload: measure, note the final diagnosis, look at where the two groups separate.
Our operational position, for members. On a camelid's ultrasound report, four lines are always needed: species · scanning plane · side · value with unit. And, for whoever wants to be fully correct, the source: "ref. Eibl & Franz 2021, alpaca ≥ 1 year, n = 60." It takes twenty seconds to write and turns a number into a datum that can be compared by the colleague who will read that report three years from now.
Star rating
Axis Score Rationale A · Scientific quality ★★★☆☆ Observational study with n stated for every measurement, explicit inclusion criteria, stated ethics approval, statistics appropriate to the design and 95% CI reported: it is fully level three of the grid. It does not rise to four because a sample size calculation, the period and the number of sites, correction for 45 multiple comparisons and any inter-observer reproducibility measure are missing; and because an inconsistency between P and CI in Table 3 remains unexplained. B · Clinical utility ★★★★☆ The result is applicable with reasonable confidence in similar cases and the conditions of use are explicit: species, plane, side, sample size. The technique requires neither sedation nor shearing nor a different scanner from the one the clinic already has. It is not five because it does not change a recommended practice — it establishes one where there was none — and because no one has measured whether these values separate a healthy animal from a sick one. C · Educational value ★★★★☆ It teaches three things that are taken away and used immediately: the left kidney is only seen from the left (and why, against the ruminant intuition); the fiber is parted, not cut; the plane changes the measurement by eight tenths of a centimeter. Course material, not just a reference. It is not five because it does not explain any pathophysiological mechanism: it describes an anatomy. D · Relevance to the species ★★★★☆ Study on camelids, on both species kept in Europe, on European private farms, with a population described enough to assess transferability (sex, age, classes, pregnancy, body condition). It is not five because uptake by the subsequent literature is so far essentially internal to the Vienna group, which cites it as a technical reference in card CRL-F-02, and because the sample is a convenience sample from a single country. No overall score, by design. A = 3 and B = 4 are not a contradiction: a descriptive study with reporting gaps still delivers the only numbers that exist, and it delivers them with their sample size alongside.
Bottom line
It can be concluded that percutaneous ultrasonography of kidney and spleen in llama and alpaca is feasible on-farm, without sedation and without shearing, with a 6.6 MHz convex probe and 70% alcohol as the sole contact medium; that the left kidney is visualized only from the left paralumbar region and the right only from the right, unlike in ruminants; that the normal ureter is not visualizable; that the spleen is seen in about one animal in four (35 of 135) and that failure to locate it is not in itself a finding; and that, for the first time, there now exist means, minima, maxima and standard deviations by species, by side and by scanning plane, each with its own sample size.
It cannot be concluded that these are reference intervals in the metrological sense of the term: they are descriptive statistics of a convenience sample of 135 animals, without a sample size calculation and without percentiles, and the observed maximum is not an upper limit of normality. Nothing can be concluded about the cria: the 7 animals under one year are excluded from the tables, and the age comparison shows that the young animal's kidney is measurably different. It cannot be concluded that a deviation from these values indicates disease, because no study has yet measured kidney and spleen in sick camelids: the second term of the comparison is missing. It cannot be concluded that the alpaca's values should be transferred to the llama or vice versa, but neither that the two species are clearly separable: twelve of fifteen comparisons were not significant and the intervals overlap widely. Nothing can be concluded about pregnant alpacas as a subgroup: there were 29 of 76 females and they were not analyzed separately.
- Uretere visibile = uretere dilatato.
Take-home messages
- The left kidney from the left, the right from the right. It is not like in cattle. Whoever looks for both kidneys from the right finds one and reports the other poorly.
- No shearing and no sedation. Fiber parted with the fingers, 70% alcohol, halter held by the owner: 135 of 135 animals, with 5 images lost to fiber.
- On the report: species, side, plane, unit. The width of the alpaca's left kidney is 4.19 cm in sagittal and 4.98 cm in transverse (n = 60 and n = 26): they are two different numbers of the same structure.
- Spleen not seen ≠ spleen absent: visualized in 35 of 135 animals (25.9%) of healthy subjects.
- Visible ureter = dilated ureter. In the 135 healthy animals, none was seen.
- In the cria, do not use the adult's table: a cria table does not yet exist.
- Look at the n column before the range. Some rows rest on 5 animals.
