- B · Utilità clinica
- Serve a chi ha l’animale davanti, oggi.
- C · Valore didattico
What the four axes measure
- A · Scientific quality
- Is the question well posed? Are the methods stated? Is the statistics appropriate? Does the conclusion stay within the data?
- B · Clinical usefulness
- Useful to whoever has the animal in front of them, today.
- C · Teaching value
- Does it teach something to those studying the subject?
- D · Relevance to the species
- Is it really about camelids, or a result borrowed from another species?
Each axis is worth 1 to 5 stars and carries its written reason. The stars in the index are the rounded average of the four.

Two contextual elements that explain the form of the work: it is the same group as record CRL-F-01 (Eibl and Franz appear in both), and there is an anatomist from another university among the authors — which is exactly what a study starting from dissection needs in order to validate what is seen on the screen.
Rivista, anno, volume, fascicolo, pagine, identificatori
Animals (Basel), 2025, volume 15, fascicolo 12, numero d'articolo 1762.

DOAJ: sì — record unico per l'ISSN, editore MDPI AG, licenza dichiarata CC BY.

A reservation, stated and not withheld. Animals passes all five questions of chapter 14 §7.1 — it exists, it is indexed in PubMed, it is in DOAJ, it has an identifiable publisher, it has a stated review process. But it is a title with over twenty-five thousand articles, from a publisher whose publication speed is the subject of documented discussion within the scientific community. This is not an argument against this article, which has anatomical dissection, validation with methylene blue, numbered ethical approval, and inter-observer reproducibility analysis: it is a well-made article. It is, however, the reason why this collection looks at the single work and not the journal's name, in both directions. Issue F contains two articles in Animals (this one and CRL-F-04) and they are both among the best of the group.

Specie e soggetti
Parte prima — cadaveri (n = 6 teste).
| Alpaca (Vicugna pacos) | Lama (Lama glama) | Totale | |
|---|---|---|---|
| Animali | 50 | 21 | 71 |
| Femmine | 29 | 13 | 42 |
| Maschi (interi e castrati) | 21 | 8 | 29 |
| Specie | Sesso | n | Media (anni) | DS | Min | Max |
|---|---|---|---|---|---|---|
| Lama | maschi | 8 | 10,4 | 6,36 | 2 | 19 |
| Lama | femmine | 12 \* | 8,8 | 8,75 | 3 | 15 |
| Alpaca | maschi | 21 | 7,4 | 3,62 | 1 | 17 |
| Alpaca | femmine | 29 | 7,5 | 4,21 | 1 | 13 |
Attenzione a chi riassume in fretta.
Species and subjects
The study has two distinct parts, and the subjects are not to be summed.
Part one — cadavers (n = 6 heads).
- 2 heads dissected ventrally to the base of the left ear to identify landmarks: 1 adult alpaca, 1 adult llama. No evidence of ear disease.
- 4 further heads (left side) to describe the normal ultrasonographic anatomy and set up the technique: 2 adult alpacas, 2 adult llamas. No evidence of ear disease.
Part two — living animals (n = 71), convenience sample from six farms.
| Alpaca (Vicugna pacos) | Llama (Lama glama) | Total | |
|---|---|---|---|
| Animals | 50 | 21 | 71 |
| Females | 29 | 13 | 42 |
| Males (intact and castrated) | 21 | 8 | 29 |
Age (from Tables 1 and 2 of the article, which give it separated by sex — and that is how it should be read):
| Species | Sex | n | Mean (years) | SD | Min | Max |
|---|---|---|---|---|---|---|
| Llama | males | 8 | 10.4 | 6.36 | 2 | 19 |
| Llama | females | 12 \* | 8.8 | 8.75 | 3 | 15 |
| Alpaca | males | 21 | 7.4 | 3.62 | 1 | 17 |
| Alpaca | females | 29 | 7.5 | 4.21 | 1 | 13 |
\* Age was not available for one female llama (footnote to the table in the article): the female llamas examined are 13, but the age is known for 12.
A caution for anyone summarizing in a hurry. In the text the authors give only one overall mean age — «older than 1 year (mean: 8 years)» — and the two extremes (the oldest llama 19 years, the oldest alpaca 17, the youngest 1 year). The species means are not 8.8 and 7.5: 8.8 is the mean of the female llamas, 7.5 that of the female alpacas. Merging the table rows produces numbers that are not in the article.
Body condition score (1-5 scale): alpaca 2.0 to 3.5; llama 2.5 to 3.5.
Inclusion criteria, as stated: age over 1 year; habituated to handling; no history nor clinical evidence of ear disease (aural discharge, head shaking, abnormal pinna position, absent aural reflex, flaccid facial musculature, nystagmus, ptosis, ataxia, circling). Exclusion criterion: females in the last third of gestation.
Study type and level of evidence
Design, in two stages: (a) cadaveric methodological feasibility study, with anatomical validation by ultrasound-guided dye injection and confirmation by dissection; (b) cross-sectional descriptive observational field study on healthy living animals, with reproducibility analysis between two observers. The authors themselves define it as «this pilot study».
What this study is not, and it matters more than what it is: it is not a diagnostic accuracy study. There is not a single sick animal, there is no reference standard, there are no sensitivity and specificity figures. For this reason the STARD checklist does not apply to it — and the reason it is worth saying is that the hurried reader reads "ultrasonography of the tympanic bulla" and concludes "ultrasonography can diagnose otitis media in the alpaca." This study does not demonstrate that, and the authors write the exact opposite in the conclusions.
Level of evidence: low in hierarchical terms (descriptive study on healthy animals + ex vivo anatomical validation). Consequence for the reader: the described technique is replicable and can start to be used tomorrow to look; the measured value cannot yet be used to decide. It is a distinction that holds up the entire record.
Relevant reporting standard: STROBE for the observational field part. The authors do not state it. Applied retroactively as a lens (chapter 14 §3.3), not as a rebuke.
In breve
E poi la riga che rende questo articolo onesto, scritta dagli autori stessi: «our results dealing with the length of the tympanic bulla wall showed a wide range, which questions this parameter as a reliable indicator for detecting diseased animals.»
In brief
In South American camelids otitis media is a serious disease — head tilt, facial and trigeminal deficits, and in advanced stages abscess and bone sequestration within the bulla, up to death — and its imaging diagnosis today requires radiography or CT, i.e. sedation or general anaesthesia. The authors asked whether the bulla can be seen with the ultrasound scanner.
Answer: yes. With a 10 MHz linear probe (59 × 10 mm), by the lateral approach, in two probe positions, standing, without sedation, without shearing, the lateral wall of the bulla was seen in all 71 animals, and in all 71 it was measurable. The measurement is reproducible between two operators (ICC 0.86 and 0.85). The lateral wall of the bulla is significantly longer in the llama than in the alpaca (p = 0.002).
And then the line that makes this article honest, written by the authors themselves: «our results dealing with the length of the tympanic bulla wall showed a wide range, which questions this parameter as a reliable indicator for detecting diseased animals.» They built a reproducible measurement and published that it probably does not serve to distinguish the sick.
Gli autori chiudono con: «Further studies are required to evaluate the diagnostic potential of ultrasonography in diseased camelids before implementation in clinical practice.»
Why this study matters
For one clinical reason and one economic one, and for one danger.
The clinical reason. Otitis media in camelids has a prognosis that depends on how early treatment is started — the authors write this citing the species' clinical literature. But the examination that diagnoses it (CT or radiography) requires anaesthetizing an already compromised animal, and to do so it must be brought to a facility that has the scanner. An examination performed on the farm, standing, with a portable ultrasound scanner would change the threshold at which one looks inside an ear.
The economic reason. Camelids are not shorn to perform an examination: the fibre has value, and the authors note that «camelids are very sensitive to stress caused by the loud noise of shearing machines». Here, as in record CRL-F-01 and citing it expressly (reference [28] in the article's bibliography), the fibre is parted with the fingers and 70% alcohol is used.
The danger, which is why this record insists. A work like this is exactly the type of work that gets cited badly. «Ultrasonography of the tympanic bulla in llamas and alpacas» becomes, in three citation steps, «ultrasonography diagnoses otitis media in the alpaca.» That is not what was demonstrated. The authors close with: «Further studies are required to evaluate the diagnostic potential of ultrasonography in diseased camelids before implementation in clinical practice.»
Obiettivo
Objective
To describe the method and the normal ultrasonographic appearance of the tympanic bulla and surrounding structures in cadavers and in healthy, non-sedated llamas and alpacas, for the first time in these species; and to measure the length of the ultrasonographically visible portion of the lateral wall of the bulla, assessing differences by species, sex, age and body condition, and inter-observer reproducibility.
Hypothesis stated by the authors: that ultrasonography of the tympanic bulla is feasible even in South American camelids, but that technique and physiological findings would differ from those of other species for specific anatomical reasons — the external ear canal divided into a cartilaginous vertical portion and a short, narrow horizontal bony portion, and the bulla with a honeycomb bony structure, nearly rectangular in transverse section, extending from the skin to the base of the skull.
Metodi / Protocollo
Strumento.
Statistica. IBM SPSS v29. Dati come medie, deviazioni standard e intervalli.
Methods / Protocol
Instrument. Portable device DP-50 Expert® (Mindray Medical Germany GmbH, Darmstadt, Germany) with a 10 MHz linear probe, 59 mm × 10 mm.
Why that probe, and why not another. The authors argue that in the region of interest the acoustic window is small, and that therefore the probe's shape and bulk matter as much as the frequency: they hypothesize — stating it as their own assumption — that rectal probes are not usable in camelids because too large to manoeuvre in that space, referencing a study in the calf in which the rectal probe gave worse handling and worse image quality.
On the cadavers. Fleece shorn, 70% alcohol as coupling medium. Probe positioned at multiple points, in the rostrocaudal and dorsoventral directions, caudal to the mandibular ramus and ventral to the base of the ear, by the lateral approach. The anatomical validation, which is the strongest part of the method: a needle mounted on a syringe loaded with methylene blue was inserted under ultrasound guidance, in the same direction as the beam, up to the tympanic bulla, the parotid gland, the zygomatic bone and the paracondylar process; the needle tip was recognizable as a hyperechoic echo with reverberation artefacts; the dye was injected at the topographic location of the structure to be identified, and correct placement was then confirmed by dissecting the head.
On the living animals. Standing, non-sedated, without a restraint chute: the head held by the owner. Same device, same probe, same positions. Fleece not shorn, parted with the fingers; 70% alcohol. Always the left ear, always from the left side of the animal. Preliminary palpation of the landmarks (vertical mandibular ramus with condylar and subcondylar process; tympanic bulla directly ventral to the base of the ear).
The two probe positions, in full — this is the part to take into the clinic.
- Position 1 — rostrocaudal. Probe in line with the lateral canthus of the eye, caudal to the vertical mandibular ramus, ventral to the base of the ear. The zygomatic bone (rostral), the paracondylar process (caudal), the tympanic bulla are visualized, all as a smooth echogenic line with distal acoustic shadow; between the zygomatic bone and the paracondylar process, the cartilaginous conchal eminence and part of the external ear canal as a hypoechoic structure.
- Position 2 — dorsoventral. Probe directly ventral to the base of the ear. The lateral bony wall of the bulla is visualized as a smooth echogenic line with acoustic shadow and reverberation artefacts; the ventral end has a convex shape (ventromedial portion of the bulla). The wall is covered, from outside inward, by echogenic skin, hypoechoic muscle (dorsal portion) and the homogeneous, hypoechoic parotid gland. This is the position in which the measurement is taken, with calipers, of the length of the straight echogenic line on the lateral margin of the bulla.
The order recommended by the authors (stated as their own view, not as a result): rostrocaudal first, because it better identifies the bulla and surrounding structures; then dorsoventral, which serves to assess the bony surface and to measure. And before anything, palpation of the landmarks.
Statistics. IBM SPSS v29. Data as means, standard deviations and ranges. Comparison of length between the two species and analysis by species, sex, age and body condition with a linear model controlling for age and BCS as covariates. Significance p < 0.05. Reproducibility: intraclass correlation coefficient (ICC) between two experienced examiners, each of whom measured the same animal three times within two hours, independently of the other, on 16 animals (4 llamas and 12 alpacas) of the 71 total; correlation between the two data sets with Pearson's coefficient (r).
Ethics. Study discussed and approved by the institutional Ethics and Animal Welfare Committee according to Good Scientific Practice guidelines and national legislation. Approval by the Ethics Committee of the University of Veterinary Medicine Vienna, ETK-018/01/2022, 3 March 2022. Informed consent obtained for all owned animals.
Risultati principali (dati)
«All animals tolerated the ultrasonographic examination well. The examination was discontinued in none of the animals.»
| Specie | Sesso | n | Media | DS | Min | Max |
|---|---|---|---|---|---|---|
| Lama | maschi | 8 | 1,53 | 0,36 | 1,15 | 2,24 |
| Lama | femmine | 13 | 1,80 | 0,51 | 1,16 | 3,28 |
| Alpaca | maschi | 21 | 1,43 | 0,46 | 0,63 | 2,44 |
| Alpaca | femmine | 29 | 1,36 | 0,30 | 0,64 | 1,88 |
- ICC intra-osservatore: esaminatore 1 = 0,86; esaminatore 2 = 0,85.
Gli autori dichiarano espressamente che il passo successivo non è stato fatto: «Further studies are required to evaluate the diagnostic potential of ultrasonography in diseased camelids before implementation in clinical practice.»
Gli autori ipotizzano, senza averlo misurato:
- che la maggiore lunghezza della bolla nel lama dipenda dalla maggiore taglia corporea («likely due to their larger body size»);
- che nell'otite media cronica del camelide, la lisi delle lamelle ossee interne a nido d'ape possa, in fase avanzata, assottigliare la parete della bolla e facilitare la penetrazione del fascio, aiutando la diagnosi — e che lisi e sequestri possano apparire come discontinuità o irregolarità della parete. È un'ipotesi di lavoro, esplicitamente attribuita («the authors believe»), non un reperto di questo studio, che non ha esaminato alcun animale malato;
To the authors' knowledge, no information about the thickness of the osseous tympanic bulla wall in healthy camelids is available.»
E, soprattutto, gli autori mettono in dubbio la propria misura: «our results dealing with the length of the tympanic bulla wall showed a wide range, which questions this parameter as a reliable indicator for detecting diseased animals.»
Main results (data)
Feasibility — and these are the numbers that really matter
- All animals tolerated the examination well. «All animals tolerated the ultrasonographic examination well. The examination was discontinued in none of the animals.» — 71 of 71, no interruption.
- The bulla and surrounding structures were identified in all llamas and alpacas examined, by the lateral approach, with the two probe positions.
- Measurement of the length of the lateral wall was possible in all animals.
- The ultrasonographic appearance in living animals did not differ from that observed in cadavers.
- Extending the head toward the contralateral side did not enlarge the acoustic window (observation reported because it does in the dog, but not here).
The measurement: length of the lateral wall of the tympanic bulla (L-TB), in centimetres
| Species | Sex | n | Mean | SD | Min | Max |
|---|---|---|---|---|---|---|
| Llama | males | 8 | 1.53 | 0.36 | 1.15 | 2.24 |
| Llama | females | 13 | 1.80 | 0.51 | 1.16 | 3.28 |
| Alpaca | males | 21 | 1.43 | 0.46 | 0.63 | 2.44 |
| Alpaca | females | 29 | 1.36 | 0.30 | 0.64 | 1.88 |
The lateral wall of the bulla was found to be significantly longer in the llama than in the alpaca: p = 0.002.
The other associations tested:
- Body condition: no correlation with length, in either species.
- Alpaca, older females: significantly shorter length — r = −0.380; p = 0.042.
- Llama: no significant correlation between length, sex and age.
Reproducibility (16 of the 71 animals: 4 llamas, 12 alpacas)
- Intra-observer ICC: examiner 1 = 0.86; examiner 2 = 0.85.
- Correlation between the two examiners: llama r = 0.73; alpaca r = 0.69; p < 0.001 in both species.
A negative finding that is a finding
In none of the 71 animals was fluid found in the bulla. The authors compare this with the calf, in which fluid in the bulla has been described even in healthy animals (term fetal cadavers and live calves aged 2-10 weeks), attributed by the authors of that work to a low degree of ossification. In camelids, in animals all over one year of age, zero fluid.
A physical limitation of the technique, measured and not circumvented
It was not possible to perforate the tympanic membrane and experimentally fill the bulla with fluid in the camelid cadavers — a manoeuvre that in the dog and small animal serves to make the deep wall and trabeculae visible. Stated reason: the specific shape of the external ear canal (perpendicular ventral portion) and the honeycomb internal structure of the bulla. Direct consequence: the lateral wall of the bulla behaves like a gas interface — echogenic line with a "dirty" acoustic shadow and reverberations — and masks everything deeper. With this technique, in a healthy animal, the contents of the bulla cannot be seen.
Authors' interpretation (attributed, not merged with the data)
The authors conclude that ultrasonography is a feasible technique for visualizing the tympanic bulla and surrounding structures in healthy camelids, by the lateral approach, with high-frequency linear probes; and that, since otitis media can severely compromise the animal up to death, a non-invasive technique performable in the field with portable devices and without sedation would be highly advantageous for diagnosis and prognosis.
The authors expressly state that the next step has not been taken: «Further studies are required to evaluate the diagnostic potential of ultrasonography in diseased camelids before implementation in clinical practice.»
The authors hypothesize, without having measured it:
- that the greater bulla length in the llama depends on greater body size («likely due to their larger body size»);
- that in chronic otitis media of camelids, lysis of the internal honeycomb bony lamellae might, in an advanced stage, thin the bulla wall and facilitate beam penetration, aiding diagnosis — and that lysis and sequestra might appear as discontinuities or irregularities of the wall. This is a working hypothesis, explicitly attributed («the authors believe»), not a finding of this study, which did not examine any sick animal;
- that the shorter length in older female alpacas has no clear explanation («though the reason for this was unclear»).
The authors state that they do not know something that the technique needs: «To the authors' knowledge, no information about the thickness of the osseous tympanic bulla wall in healthy camelids is available.» Wall thickness determines how far the beam penetrates: it is the parameter on which whether this technique will ever be able to see inside the bulla depends, and it is not known.
And, above all, the authors cast doubt on their own measurement: «our results dealing with the length of the tympanic bulla wall showed a wide range, which questions this parameter as a reliable indicator for detecting diseased animals.»
Implicazioni cliniche
Clinical implications
1. It can be examined, and it can be examined on the farm. A 10 MHz linear probe — the one used for tendons and superficial tissues, not new equipment — lateral approach, animal standing, head held by the owner, no sedation, no shearing, 70% alcohol. 71 of 71 animals examined without interruptions.
2. The order of the two positions is not indifferent. Rostrocaudal first (to orient: zygomatic rostral, paracondylar process caudal, bulla in between), then dorsoventral (to assess the bony surface and measure). Before that even, palpate the landmarks.
3. What is seen is the surface, not the contents. The wall behaves like a gas interface: acoustic shadow and reverberations, and everything behind it is masked. In a healthy animal, with this technique, the cavity of the bulla is not inspectable. Anyone expecting to "see the pus" in a camelid is applying a small-animal expectation, where the bulla is filled with fluid through the tympanic membrane — a manoeuvre that in this species did not succeed even on the cadaver.
4. What can reasonably be looked for, with caution. Not the contents, but the bony surface: irregularities, discontinuities, deformations of the wall. This is the authors' hypothesis, consistent with what is known of the pathology (lysis and sequestration), and it has not yet been verified in a single sick animal.
5. The measurement, for now, is not a test. The observed ranges are wide and overlap between species and sexes: alpaca 0.63 to 2.44 cm; llama 1.15 to 3.28 cm. The difference between the two species is statistically significant (p = 0.002) and clinically insufficient to separate one individual from another. The measurement is reproducible (ICC 0.85-0.86), which is a different thing from being discriminating: a reproducible ruler measuring the wrong thing remains reproducible.
6. If it is ever used, it will be used as a first-level examination. The comparison the authors report from other species gives the order of magnitude to expect: in the calf with otitis media verified by histology, ultrasonography showed sensitivity from 32% to 63% and specificity from 84% to 100%; in the sick dog, compared with CT, sensitivity 37-74% and specificity 55-74%. These are figures from other species, cited by the authors and not measured here. But they suggest the probable shape of the tool: a negative will not rule out anything; a positive will be worth more.
7. The animal that did not tolerate the examination did not exist. Zero interruptions out of 71 — and these were animals selected as "habituated to handling." This is an inclusion criterion, not a generalizable result: in an unhabituated alpaca, that number would be different.
Limiti
Motivazione dichiarata: «animals younger than one year were excluded to gather insights into the feasibility of ultrasonographic imaging to visualize the tympanic bulla in older camelids before examining younger animals.»
- Pochi lama. «Second, only a small number of llamas were examined.»
- La misura potrebbe non servire. «Our results dealing with the length of the tympanic bulla wall showed a wide range, which questions this parameter as a reliable indicator for detecting diseased animals.»
Limitations
Stated by the authors (quoted, not summarized)
- Animals under one year of age excluded. Stated rationale: «animals younger than one year were excluded to gather insights into the feasibility of ultrasonographic imaging to visualize the tympanic bulla in older camelids before examining younger animals.»
- Few llamas. «Second, only a small number of llamas were examined.»
- The measurement might not be useful. «Our results dealing with the length of the tympanic bulla wall showed a wide range, which questions this parameter as a reliable indicator for detecting diseased animals.»
- The number of animals observed must be kept in mind when interpreting the results by species, sex and age.
- The thickness of the bony wall of the bulla in the healthy camelid is not known to anyone, by the authors' own admission.
- Impossible to fill the bulla with fluid in camelid cadavers, for anatomical reasons.
Added by the editorial team
- No sick animal. This is the limitation that absorbs all the others, and the authors say so: the diagnostic potential was not evaluated. The record exists because the technique is usable immediately; the test does not yet exist.
- Only the left ear, only from the left side. All 71 animals. It is not demonstrated that the right acoustic window is equivalent, and symmetry is a reasonable but unverified assumption. In a unilateral disease like otitis media, this matters.
- The ICC is called "intra-observer reliability" but measures two examiners. The article writes «the intra-observer reliability was determined between two examiners»: the triple repetition by each examiner is intra-observer, but the comparison between the two is inter-observer, and it is that which is needed to know whether a colleague would measure the same as we do. The published coefficients are two ICCs (one per examiner) and two Pearson's r (per species). The proper inter-observer ICC is missing, and the confidence intervals of the ICCs are not reported, which for a sample of 16 animals would be wide.
- The reproducibility subsample is 16 of 71 animals, of which 4 llamas: four animals do not establish reproducibility in that species.
- An arithmetic inconsistency in Table 1. For the female llamas, mean 8.8 years, SD 8.75, range 3-15 are stated for 12 animals. With values between 3 and 15, the sample standard deviation of 12 observations cannot exceed about 6.3: 8.75 is arithmetically impossible. One of the four numbers in that row is wrong, and the article does not allow one to know which →
[TO BE CHECKED AGAINST THE FULL TEXT], that is: the raw data must be requested from the authors. It does not affect the main result (bulla length), but it is a row of a published table that does not add up, and whoever copies it propagates the error. - The between-species significance is not accompanied by an effect size. The linear model gives p = 0.002, but neither the estimated mean difference between llama and alpaca nor its confidence interval is published, which is what is needed to understand whether the difference is clinically useful. The raw means suggest a gap on the order of two to four tenths of a centimetre, in the presence of individual ranges spanning more than a centimetre and a half: the answer, in effect, is no.
- No analysis by side, by breed, by Huacaya versus Suri. Fleece type is not recorded, and the fibre is the medium through which the beam passes.
- Six farms, a single country, convenience sample: no sample size calculation, and no description of how the six farms were chosen.
- Voce 9 (bias): nessuna sezione dedicata.
- Voci 20-22 (interpretazione, generalizzabilità, finanziamento): coperte, e la generalizzabilità è dichiarata in modo esemplare («further studies are required… before implementation in clinical practice»).
What is missing relative to the reporting standard (STROBE, as a retroactive lens)
- Item 1: design not named in the title or abstract (in the abstract «pilot study» appears, in the body).
- Item 5 (setting): the six farms are named but not described, and the data collection period is not stated. The ethical approval is from March 2022 and the article is published in June 2025: the interval is not accounted for.
- Item 6 (participants): inclusion and exclusion criteria explicit and well written — this is the best-covered item of the article. Missing, however, is the number of candidate animals not included: we do not know how many alpacas were excluded because they were not habituated to handling, and that number is the real measure of field feasibility.
- Item 9 (bias): no dedicated section. The sample is selected for docility: this is the bias with the greatest impact on the feasibility result, and it is discussed only in passing.
- Item 10 (study size): no calculation, no justification, either for the 71 or for the 16 of the reproducibility subsample.
- Item 12 (statistical methods): the linear model is named but not specified (dependent variable, terms included, handling of the farm-level structure). Clustering by farm is not modelled: 71 animals from 6 herds are not 71 independent observations.
- Item 16 (main results): point estimates without confidence intervals for the between-species comparison and for the ICCs. This is the gap that most limits practical use.
- Item 19 (limitations): present, brief but substantial, and include doubt about their own measurement: this should be recognized as a merit.
- Items 20-22 (interpretation, generalizability, funding): covered, and generalizability is stated in exemplary fashion («further studies are required… before implementation in clinical practice»).

Commento dell'associazione — Camelid Research Studies APS · redazione CRL · 31/08/2026
Valutazione a stelle
Funding and conflicts of interest
- Funding: «This research received Open Access Funding by the University of Veterinary Medicine Vienna.» Crossref records University of Veterinary Medicine Vienna as funder. This is therefore the funding of the open-access publication fee, not the funding of the research: no external source is stated for the conduct of the study. No role of a sponsor in the design or analysis is stated — nor is its absence stated.
- Conflicts of interest: «The authors declare no conflicts of interest.»

Association's comment — Camelid Research Studies APS · CRL editorial team · 31/08/2026
This is an article that deserves to be read for how it is built, even before what it finds. It starts from dissection, validates what is seen on the screen by injecting a dye under ultrasound guidance and then opening the head to check, sets up the technique on the cadaver, brings it into the field on 71 animals, and measures whether a second operator obtains the same number. This is the correct sequence, and it is rare. Between this work and that of record CRL-F-01 — same group, four years earlier — there is a visible methodological improvement: inter-observer reproducibility has been added, which was missing in 2021.
And then the authors do the hardest thing: they publish doubt about their own measurement. «Which questions this parameter as a reliable indicator» is a costly sentence in an article whose only quantitative result is that measurement. This is acknowledged with respect, and it is written here that it is the reason this record trusts the rest.
Where we insist more than the authors. On the distance between «the bulla can be seen» and «otitis is diagnosed.» In the healthy camelid the bulla wall behaves like a gas interface and blinds the beam: what is seen is a bony profile, not a cavity. The authors' hypothesis — that in advanced chronic otitis the wall thins enough to let the beam through — is biologically sensible and has an uncomfortable consequence that the article does not draw: if the beam passes through only when lysis is advanced, then ultrasonography would see inside the bulla exactly in the cases where the diagnosis is already late, i.e. those in which the authors say the prognosis is poor. The value of the technique, if there is one, will probably lie in the bony surface — irregularities and discontinuities of the profile — not in the contents. This is the hypothesis that seems to us to need testing first, and it differs from the one the article emphasizes.
A caution about reproducibility, because it is the most misunderstood word in diagnostics. ICC 0.86 and 0.85 mean that two operators measure the same thing the same way. They do not mean that that thing distinguishes a sick animal from a healthy one. The two properties are independent, and this article has demonstrated the first and explicitly cast doubt on the second. Anyone citing the ICC to support diagnostic usefulness is using the right number for the wrong claim.
What the literature lacks, and whose task it is. What is missing is the study on sick animals: twenty camelids with otitis media confirmed by CT or necropsy, scanned first, with blind reading. This is a study that a European clinic with camelid caseload could carry out in two or three years without sacrificing anything and without a single extra animal, because those animals arrive anyway and the CT is done anyway. It is among the records we would like to write and cannot. Also missing, by the authors' own admission, is the measurement of the thickness of the bony wall of the bulla in the healthy camelid: this is data that can be obtained from a series of already archived CT scans, without touching an animal.
Our operational position, for members. Faced with a camelid with a head tilt or facial deficit: ultrasonography of the bulla is an examination that can be done immediately, on the farm, and that replaces nothing. If the bony profile appears irregular or discontinuous, one has an additional element to convince the owner to take the animal for a CT. If it appears normal, nothing has been excluded — and this is the sentence that must be said to the owner at the same moment, because otherwise a negative examination becomes a reassurance no one has authorized.
Star rating
Axis Score Rationale A · Scientific quality ★★★☆☆ Observational study with stated n, explicit inclusion and exclusion criteria, numbered and dated ethical approval ( ETK-018/01/2022, 3 March 2022), anatomical validation of the ultrasonographic finding with dye and dissection, reproducibility analysis: full level three. It does not rise to four because the sample size calculation, confidence intervals for all estimates, specification of the linear model, and handling of farm-level clustering are missing; and because a row of Table 1 is arithmetically impossible (SD 8.75 over a range of 3-15 with n = 12).B · Clinical usefulness ★★★☆☆ Offers a replicable procedure described in sufficient detail — probe, frequency, two positions, order, coupling medium, landmarks — to be used with the caution imposed by the level of evidence: this is exactly level three. It does not rise to four because the conditions for diagnostic use are not explicit, for the most radical reason possible: not a single sick animal was examined, and the authors write that the diagnostic potential remains to be evaluated. C · Educational value ★★★★☆ Teaches a validation method (needle under guidance, dye, confirmatory dissection) that is valuable well beyond the ear; teaches why the bulla wall blinds the beam, and therefore what can and cannot be hoped to be seen; and shows the difference between a reproducible measurement and a discriminating measurement. Teaching material. It is not five because it is not yet the first-choice reference on camelid otitis media: that reference remains CT. D · Relevance to the species ★★★★☆ Study on the two species kept in Europe, on European farms, with a population described by sex, age and body condition sufficiently to assess its transferability; the first work ever on the subject in these species. It is not five because the llamas number 21 and the measured female llamas 13, because there is not yet subsequent literature building on it, and because the sample is selected for docility in six farms in a single country. No overall score, by design. A = 3 and B = 3 say one single, coherent thing: it is a good study that has demonstrated that one can look, not that one can decide.
Bottom line
It can be concluded that the tympanic bulla and surrounding structures are visualized ultrasonographically in llamas and alpacas by the lateral approach, with a 10 MHz linear probe (59 × 10 mm), in two probe positions (rostrocaudal and dorsoventral), standing, without sedation, without shearing, with 70% alcohol as coupling medium: 71 of 71 animals visualized and measured, no examination discontinued; that the measurement of the lateral wall is reproducible (ICC 0.86 and 0.85; r between examiners 0.73 in the llama and 0.69 in the alpaca, p < 0.001); and that the wall is significantly longer in the llama than in the alpaca (p = 0.002), with means from 1.36 cm (female alpacas, n = 29) to 1.80 cm (female llamas, n = 13).
It cannot be concluded that ultrasonography diagnoses otitis media in the camelid. No sick animal was examined, no sensitivity, specificity or predictive value exists in this study, and the authors expressly write that the diagnostic potential must be evaluated before introducing the technique into clinical practice. The wall length cannot be used as an indicator of disease: the individual ranges are wide and overlapping (alpaca 0.63-2.44 cm; llama 1.15-3.28 cm) and it is the authors themselves who cast doubt on its usefulness. Nothing can be concluded about the contents of the bulla: the wall behaves like a gas interface and masks what lies behind it, and in camelid cadavers it was not possible to fill it with fluid for anatomical reasons. Nothing can be concluded about animals under one year of age (excluded by criterion) nor about the right ear (never examined). The sensitivity of 32-63% measured in the calf or of 37-74% measured in the dog cannot be transferred to the camelid: these are data from other species, cited by the authors as a comparison and not measured here.
Messaggi da portare a casa
- Sonda lineare 10 MHz, via laterale, in stazione, senza sedazione, senza tosare.
Take-home messages
- 10 MHz linear probe, lateral approach, standing, without sedation, without shearing. This is an examination that can be done on the farm with what is already in the clinic.
- First palpate, then rostrocaudal, then dorsoventral. In the first one orients oneself, in the second one assesses the bony surface and measures.
- The profile is seen, not the contents. The bulla wall blinds the beam: in a healthy animal the cavity is not inspected.
- A normal ultrasound does not exclude otitis media. This must be said to the owner at the same moment as saying the examination is fine.
- Reproducible ≠ discriminating. ICC 0.85-0.86 says that two operators agree, not that the measurement separates the sick.
- Means are not interchangeable between species or sexes, and the individual range is much wider than the difference between the means.
- It does not apply in the cria: animals under one year were excluded by inclusion criterion.
