Specie e soggetti
Species and subjects
No animal is the object of study in this work, and it must be said at once so the field does not mislead. It is a review: camelids appear in it in two different guises, and this card keeps them separate because they are two very different things for anyone who works with these animals.
First guise — the camelid as a source of sequences. The authors discuss repertoire differences between species and report a comparative-biology datum that is specific and verified in the full text: non-canonical cysteines — those that form the additional disulfide bridges that rigidify CDR3 — are enriched at different positions depending on the species. In camel and dromedary the prevailing position is IMGT 38, in CDR1; in llama and alpaca the prevailing position is IMGT 55, in FR2. This is not a detail for crystallographers: it is a measurable molecular difference between Old World and New World camelids, i.e. between two groups that the word "camelids" tends to flatten together.
Second guise — the camelid as an immunized animal. The full text describes the immunization protocol of the authors' own group. The species the authors state as used in the field are «llamas, alpacas, camels and dromedaries»; the species used in their own protocol is not stated → [TO BE CHECKED AGAINST THE FULL TEXT] — and the full text has been read, so here the annotation means exactly what it says: the datum is not in the article, not "we did not look for it."
Also not stated: how many animals, of what age, of what weight, at which facility, under which project authorization, and with which severity category assigned.
Domanda clinica (PICO)
Clinical question (PICO)
Not applicable, and by construction: a narrative review does not have a PICO question, it has a scope. The scope stated by the authors is: VHH structure, sequence diversity in the repertoires of the different camelid species, isolation and humanization platforms, the five VHH-based drugs on the market, and an in silico analysis of 49 sequences that have reached the clinic.
Dichiarato dagli autori: no — nessuna griglia è nominata nel testo pieno.
Study type and level of evidence
Narrative review with an original in silico analysis appended. PubMed classifies it as review-article · Review · Journal Article. It is not a systematic review: the full text shows no bibliographic search strategy — no database, no search string, no date, no inclusion criteria, no flow diagram. The authors do not claim otherwise, and this card does not attribute it to them.
One part of the work, however, is not a review: the analysis of the 49 sequences is original research, and it is the only part with a stated method and a versioned data source (TheraSAbDab, version 19.02.2025, plus three domains added by hand through literature curation). A reader who evaluates the article as a block is evaluating two genres at once; this card distinguishes them.
Level of evidence: narrative review — in the hierarchy, below the systematic review and without a claim to exhaustiveness. Consequence for the reader: the card states what was written and with what reasoning, not that this is all the literature contains. What is not in this article is not, for that reason, non-existent.
Relevant reporting standard: none. EQUATOR has no reporting guideline for the traditional narrative review, and — a documented trap noted at §4.1 of chapter 14 — the EQUATOR record that appears when searching «narrative review», namely RAMESES, concerns meta-narrative reviews, which are a systematic synthesis method of their own: it is not the grid for this article, and using it as such would be a misattribution. It is therefore assessed with SANRA, which is a quality scale and not a reporting guideline, and the raw score is published below item by item. Stated by the authors: no — no grid is named in the full text.
In breve
In brief
Thirty-two years after the Letter of card G-01, the VHH domain has reached the pharmacy. This review, written by four people who do exactly this job at a pharmaceutical company, lays out what happened: how the domain is built and why it stands on its own, how it is obtained (by immunizing a camelid, or from naive, synthetic or semi-synthetic libraries, or from transgenic animals that produce single domains, or — the latest arrival — by designing it de novo), which five molecules have obtained marketing authorization, and what the 49 single domains that have entered clinical trials look like as sequences.
For this collection, however, the piece that matters most sits in a twenty-line section titled Camelid immunization, and it is the main reason this article was catalogued.
Why this study matters
Because it closes a gap that chapter 08 of this KT had to declare open.
That chapter, in the section on the flip side of the coin for the animals (§11.3), deleted a figure that the previous version reported twice — "a final venous (jugular) blood draw on the order of 100–250 mL" — because none of the protocols open in that session contained it. The deletion was correct: a number without a source is not written.
In the full text of this article the figure is there, and it has a source. The Methods / Protocol field below discusses it. It does not resolve the question — because the denominator is still missing — but it moves it from nothing to a precise point, and shows anyone who wants to carry it further exactly what to ask.
Obiettivo
Objective
Stated by the authors in the abstract: to summarize the structural characteristics of VHHs; to discuss the sequence diversity of the repertoires in the different camelid species used; to describe the isolation and humanization platforms; to review the composition and mechanism of action of the VHH-based drugs on the market; to present novel biological concepts that use single domains as targeting modules; and to provide an in silico analysis of the properties of 49 VHH sequences that have reached clinical trials.
Metodi / Protocollo
«our standard immunization protocol comprises four injections in total (d0, d14, d28, and d35) using 300 µg of antigen per immunization (subcutaneous injection) and animal with blood collection (100 mL) seven days post final administration»
- la specie. Gli autori scrivono che nel campo si immunizzano «llamas, alpacas, camels and dromedaries», e non dicono quale sia la loro. Un lama adulto e un'alpaca adulta non hanno la stessa taglia, e un dromedario è un altro ordine di grandezza;
- il peso corporeo dell'animale. Senza peso, 100 mL non è una quantità giudicabile;
- il numero di animali, il numero di siti di iniezione per somministrazione, e quale dei due adiuvanti sia usato nel loro protocollo;
Methods / Protocol
For the review part: no method stated. No database, no search string, no time window, no inclusion or exclusion criteria, no count of works found and discarded. This is a characteristic of the genre, not a hidden omission; but it must be written, because it is the reason axis A of this card does not reach five stars and could not reach it even if the article were perfect (chapter 14, §11, stated ceiling).
For the in silico analysis: a stated and repeatable method. Source TheraSAbDab version 19.02.2025, from which 46 single-domain variable domains were extracted; manual literature curation added 3 more (two from ciltacabtagene autoleucel and one from netakimab), for a total of 49 domains incorporated into therapeutics in clinical phase or already on the market. For each: clustering by sequence identity (CDR3 at 85%, whole domain at 100%), similarity to the nearest human germline (IGHV/IGHJ), search for non-canonical cysteines and N-glycosylation sites, calculation of the isoelectric point. IMGT numbering.
The immunization protocol — reported in full, and with what it lacks
⚠️ What follows is a research protocol on a healthy animal, not a clinical dosage. It does not apply to a patient and must not be read as a therapeutic dose.
The authors describe their own standard protocol in a single sentence, which this card reports verbatim because every piece of it matters:
«our standard immunization protocol comprises four injections in total (d0, d14, d28, and d35) using 300 µg of antigen per immunization (subcutaneous injection) and animal with blood collection (100 mL) seven days post final administration»
Unpacked: four administrations on days 0, 14, 28 and 35; 300 µg of antigen per administration and per animal, subcutaneously; blood draw of 100 mL at seven days from the last administration. The general framework stated around it: immunizations typically last 6–12 weeks; the adjuvants used in the field are Freund's adjuvant or Gerbu fama; and — in the authors' own words — «immunized camelids usually remain alive after the procedure, a clear benefit from an animal welfare perspective».
What this protocol lacks in order to be assessable, and this is the part this card highlights:
- the species. The authors write that the field immunizes «llamas, alpacas, camels and dromedaries», and do not say which is theirs. An adult llama and an adult alpaca are not the same size, and a dromedary is another order of magnitude altogether;
- the body weight of the animal. Without weight, 100 mL is not a quantity that can be judged;
- the number of animals, the number of injection sites per administration, and which of the two adjuvants is used in their own protocol;
- the project authorization and the severity category assigned.
Why the denominator is everything. Annex VIII of Directive 2010/63/EU — read and reported in chapter 08 §11.3 of this KT — does not anchor the severity classification to milliliters, but to the percentage of circulating blood volume: among the examples of mild severity are blood samplings «totalling < 10% of circulating volume», and among those of moderate severity, samplings «> 10% of circulating volume» in the conscious animal. Until species and weight are known, the 100 mL of this protocol cannot be compared against that threshold, and anyone who claims that draw is "mild" or "moderate" is guessing.
Risultati (dati)
Dal testo pieno, e solo numeri:
I farmaci approvati.
I 49 domini in clinica.
- 46 domini estratti da TheraSAbDab v. 19.02.2025, +3 da curazione manuale = 49;
- somiglianza con il germinale umano più vicino: da 58,7 % a 89,1 % di identità;
- 6 domini su 49 portano un ponte disolfuro aggiuntivo che lega la CDR3 a una cisteina in CDR1 (IMGT 38) o in FR2 (IMGT 50 o 55);
- 0 domini su 49 portano un motivo canonico di N-glicosilazione (N-X-S/T) né nelle CDR né nei framework;
- 34 domini su 49 (≈ 69 %) hanno un punto isoelettrico del VHH superiore a 7,5;
- motivi hallmark in FR2: FERF in 15 su 49 (30,6 %), VGLW in 12 su 49 (24,5 %), i restanti 22 distribuiti fra motivi diversi (FERG, FERR, FGLG, YQRL/YQRW, YERL, VGPW).
Results (data)
From the full text, and only numbers:
The approved drugs. Five therapeutics with a VHH-based paratope have obtained marketing authorization from various health authorities, as of October 2025, in five different molecular architectures: caplacizumab (tandem VHH, bivalent monospecific); netakimab (an IgG-like antibody in which a VHH replaces the VH and pairs with a VL); envafolimab (VHH–Fc homodimer); ozoralizumab (a trivalent "beads on a string" construct, with two anti-TNF VHHs flanking a VHH that binds human serum albumin to extend half-life); ciltacabtagene autoleucel (a CAR whose ectodomain carries two tandem biparatopic anti-BCMA VHHs).
The 49 domains in the clinic.
- 46 domains extracted from TheraSAbDab v. 19.02.2025, +3 from manual curation = 49;
- similarity to the nearest human germline: from 58.7% to 89.1% identity;
- 6 of 49 domains carry an additional disulfide bridge linking CDR3 to a cysteine in CDR1 (IMGT 38) or FR2 (IMGT 50 or 55);
- 0 of 49 domains carry a canonical N-glycosylation motif (N-X-S/T) either in the CDRs or in the frameworks;
- 34 of 49 (≈69%) have a VHH isoelectric point above 7.5;
- FR2 hallmark motifs: FERF in 15 of 49 (30.6%), VGLW in 12 of 49 (24.5%), the remaining 22 distributed among different motifs (FERG, FERR, FGLG, YQRL/YQRW, YERL, VGPW).
The comparative camelid biology datum, already reported in the Species and subjects field: non-canonical cysteines are enriched in CDR1, position IMGT 38, in camel and dromedary, and in FR2, position IMGT 55, in llama and alpaca.
A contextual field datum, reported by the authors citing the 2022 review by Garces and colleagues: about 10% of multispecific antibodies in phase 1–3 clinical trials used VHHs as a building block. The denominator of that 10% is not reported in this review → [TO BE CHECKED] against the primary source, which is a different article.
Authors' interpretation (attributed, not merged with the data)
The authors argue that camelids are ideal hosts for immunization against human antigens for two complementary reasons: phylogenetic distance from humans, which makes human proteins more "foreign" and therefore more antigenic; and, at the same time, the high similarity of the variable-region genes — particularly between camelid VHH domains and the human VH3 family — which makes VHHs easy to humanize. This is an observation, not an experiment of this work.
The authors state that the fact that immunized camelids normally remain alive after the procedure is «a clear benefit from an animal welfare perspective», compared to classical murine protocols. The factual claim is true (the procedural endpoint is the blood draw, not sacrifice) and the judgment is the authors'. Chapter 08 §11.4 of this KT notes that this is a comparison internal to the field, not a third-party judgment, and that no independent comparative welfare study measuring the impact of repeated adjuvanted immunizations on llamas and alpacas was identified in that session. This card aligns with that annotation: surviving is not the same thing as doing well.
The authors report — they do not demonstrate this here, they take it from Valdés-Tresanco and colleagues — that synthetic libraries allow binders to be isolated without immunizing any animal, with practical advantages (toxic or non-immunogenic targets, reuse across programs, shorter timelines) and a stated cost: often poorer biophysical behavior, huge libraries, and typically lower affinity and specificity than those obtained from immune libraries. For an association concerned with welfare, this is the operative sentence of the entire article, and it sits in a review paragraph, not in a result.
The authors conclude that the modularity of VHHs — demonstrated by the reuse of the same albumin-binding sequence in eight different molecules across ozoralizumab, sonelokimab, vobarilizumab, brivekimig, lunsekimig, isecarosmab, gocatamig and podentamig — is the reason these domains function as interchangeable building blocks.
Limiti dichiarati dagli autori
Limitations stated by the authors
No limitations section in the full text as read. The authors state the scope («we focus here on recent developments in this field» in the section on transgenic animals) but do not discuss the limitations of the review as a review: there is no sentence warning the reader that the literature selection is not reproducible. This is the most significant gap in this article, and it is the one that weighs in the SANRA score below.
Limitations added by the editors
- The literature selection is not reproducible, and is not stated as such. The reader has no way of knowing what was left out.
- The perspective is that of a drug developer, and the camelid appears in it as a platform. This is not a flaw — the article is what it claims to be — but a veterinary reader must know it: they will not find here an assessment of welfare, they will find a sentence about welfare.
- The immunization protocol is given without species and without weight, that is, in a form that does not allow it to be assessed against the very European standard the authors themselves operate under (they are in Germany).
- The Crossref typo on the surname Hamers is copied at entry 16 of the bibliography (see card G-01). It is a small detail, and we flag it for one reason only: it shows that the bibliography was compiled from deposited metadata, not rechecked entry by entry — which is entirely normal, and should be kept in mind by anyone using it as a source of references.
Finanziamento
Dichiarato, ed è una dichiarazione di assenza. Sezione Funding Statement, riportata alla lettera: «The author(s) reported there is no funding associated with the work featured in this article.»
Funding
Stated, and it is a declaration of absence. Funding Statement section, reported verbatim: «The author(s) reported there is no funding associated with the work featured in this article.»
Conflitti di interesse
Dichiarati. Sezione Disclosure statement, riportata alla lettera: «All authors are affiliated with Merck Healthcare KGaA, Darmstadt, Germany. Besides, this work was conducted in the absence of any further commercial interest.»
Come va letta, senza sottintesi.
Conflicts of interest
Stated. Disclosure statement section, reported verbatim: «All authors are affiliated with Merck Healthcare KGaA, Darmstadt, Germany. Besides, this work was conducted in the absence of any further commercial interest.»
How it should be read, without innuendo. The declaration is transparent: it says what there is to know, namely that all four authors work for a pharmaceutical company active in antibody discovery. It should be noted, however, that the review describes the field in which that company competes, that one of the platforms discussed in the section on semi-synthetic libraries is explicitly attributed to the authors' own group («Our group described…»), and that the industrial affiliation is not a residual conflict against which "no further commercial interest" acts as a mitigation: it is the conflict. The phrase «in absence of any further commercial interest» is therefore correct to the letter and optimistic in tone. It does not change the value of the data, and it is not an accusation: it is what chapter 14 §5.4 asks to be written instead of left unsaid.
- strategia di ricerca bibliografica: assente per intero (banche dati, stringhe, date, lingue, criteri di inclusione ed esclusione);
- dichiarazione che la selezione non è sistematica: assente;
- sezione dei limiti: assente;
- per il protocollo di immunizzazione: specie, peso, numero di animali, adiuvante effettivamente usato, autorizzazione di progetto e categoria di gravità;
| # | Voce SANRA | Punteggio | Motivo |
|---|---|---|---|
◆ COMMENTO DELL'ASSOCIAZIONE
Camelid Research Studies APS — redazione CRL · 31/08/2026
Valutazione a stelle
What is missing relative to the reporting standard
Since no reporting guideline exists for the narrative review, the list is built on SANRA's six items and on what the reader looks for:
- bibliographic search strategy: entirely absent (databases, strings, dates, languages, inclusion and exclusion criteria);
- statement that the selection is not systematic: absent;
- limitations section: absent;
- for the immunization protocol: species, weight, number of animals, adjuvant actually used, project authorization and severity category;
- denominator of the "about 10% of phase 1–3 multispecifics" taken from a third party.
SANRA score, item by item — published raw
Chapter 14 (§11) requires the raw SANRA score to be published, item by item, so that anyone who does not share the CRL conversion to stars still has the original figure. Six items, from 0 to 2:
| # | SANRA item | Score | Reason |
|---|---|---|---|
| 1 | Justification of the topic's importance for the reader | 2 | Explicit and dated: five approved drugs as of October 2025, and relevance for building multispecifics |
| 2 | Statement of concrete objectives or questions | 2 | Objectives are listed one by one at the end of the abstract, and the text follows them in order |
| 3 | Description of the literature search | 0 | Absent. No database, no string, no date. The only versioned source is TheraSAbDab, and it concerns only the in silico analysis, not the review |
| 4 | Referencing | 2 | Dense and precise; key claims carry specific references (with the note on entry 16 made above) |
| 5 | Scientific reasoning | 2 | The link between structure, properties and therapeutic format is argued, not asserted |
| 6 | Appropriate presentation of data | 2 | The analysis of the 49 sequences carries method, source version, sample sizes and denominators |
| Total | 10 / 12 |
CRL conversion (this collection's convention, stated at §11 of chapter 14 and not proposed by the authors of SANRA): 9–12 → ★★★★☆ on axis A. Stated ceiling: a narrative review does not reach five stars on A, because the fifth requires a reproducible literature selection, which is precisely what this genre lacks.
◆ ASSOCIATION COMMENTARY
Camelid Research Studies APS — CRL editorial staff · 31/08/2026
The Camelid immunization section, twenty lines long, closes a gap that chapter 08 of this KT had to declare open.
The review is what it claims to be: four people who develop antibodies at a pharmaceutical company recount the field they work in, well, with a final part of original research that is the best thing in the piece. For an Italian veterinarian, nine-tenths of the content is good-level general culture and nothing more: knowing that ozoralizumab is trivalent and binds albumin changes no decision in the barn.
But the Camelid immunization section contains the number that chapter 08 of this KT had had to delete. The previous version of that chapter wrote "a final venous blood draw on the order of 100–250 mL" without knowing where it came from, and the revision removed it — rightly: a number without a source is not written, and that was a number used to judge a procedure, so it was the worst kind of sourceless number. As of today the figure has a primary, open, citable source: 100 mL, seven days after the last of four subcutaneous administrations of 300 µg, in the Darmstadt group's standard protocol.
And now comes the part that matters to us most. With that number in hand, we still cannot say whether that draw is mild or moderate, because the authors do not state the species and do not state the weight. Annex VIII of Directive 2010/63/EU does not reason in milliliters: it reasons in percentage of blood volume, with the 10% threshold. A hundred milliliters from an adult llama and a hundred milliliters from a young alpaca are two different fractions of the same animal — and one hundred twenty kilograms against thirty-five is not a nuance.
This is the question the association can ask, and it is a precise one. Not "are nanobodies ethical?", which is not a question one can answer; but: what species is the animal in your protocol, how much does it weigh, and what percentage of its blood volume are the hundred milliliters? That is three lines of answer, and anyone who wrote that protocol knows it. Until those are known, the severity classification cannot be inferred — and whoever infers it, in either direction, is doing advocacy with numbers they do not have.
Where we disagree, and we say it gently. The sentence on welfare — immunized camelids normally remain alive, «a clear benefit from an animal welfare perspective» — is true in fact and optimistic in inference. The point of comparison is the mouse that is euthanized to take its spleen, and against that the comparison holds. But the right comparison today is no longer that one: it is the synthetic library, which uses no animal at all, and it is in the same article, two sections above, with its stated cost (lower starting affinity). The authors put both things on the table; it is the reader who tends to read only the first. The position this association can write and defend remains that of chapter 08 §11.4: a growing share of nanobodies can be obtained without touching an animal, and when the immune route is chosen, that choice must be justified, not taken for granted.
One last thing, concerning this collection's method. Entry 16 of this article's bibliography reports "Hammers C." and "Hammers R.", i.e. the Crossref typo on the name of the two Hamers. We flag it without any reproach — the bibliography of a review with over one hundred seventy entries is compiled from metadata, not retyped — but it is the proof we were looking for: a record's typo propagates through bibliographies, and in thirty-two years it has crossed the entire field. If this collection insists on copying from the correct record, it is so as not to be the fourth link in that chain.
Star rating
Axis Score Rationale A · Scientific quality ★★★★☆ SANRA 10/12 (item-by-item detail above), which in the CRL conversion is worth four stars. Solid argumentation, dense referencing, and an original in silico analysis with a versioned source, sample sizes and explicit denominators. It does not rise to five because it cannot: the grid's stated ceiling forbids the fifth star to a narrative review, and here the reason is concrete, not formal — SANRA item 3 scores zero, because there is no line on the search strategy. B · Clinical utility ★★☆☆☆ For a European veterinarian or breeder the result is not actionable in the clinic: there is no diagnostic, therapeutic or management procedure to apply to an animal. It is actionable in one sense only, and for that it does not stay at one star: it supplies the exact questions to ask when facing an immunization protocol (species, weight, percentage of blood volume, adjuvant, authorization, severity category), which is the business of a welfare-focused association. A hypothesis to check, not a procedure to use. C · Educational value ★★★★★ It is today the first-choice reference for learning what a VHH is and what has been made of it: updated to October 2025, open access, illustrated, and with the cross-species comparative part that almost no other review reports — the non-canonical cysteines in CDR1 in the camel and in FR2 in the llama and alpaca. Anyone who needs to learn this starts here, and can do so for free, which in this field is not a given (see G-01). D · Species relevance ★★☆☆☆ Here the camelid is not a patient and is barely a subject: it is a production platform. Only two pieces of information are specifically camelid, and they are good ones — the difference in the position of non-canonical cysteines between Old and New World, and the immunization protocol. But the protocol arrives without the species, which is precisely the flaw axis D exists to penalize: relevance to a species cannot be assessed when the article does not name it. No overall score, by construction. A = 4 and B = 2 are not a contradiction: it is an excellent article written for a reader who is not ours, and this card exists to extract the two things our reader needs from it.
Bottom line
It can be concluded that as of October 2025 five VHH-paratope therapeutics have obtained marketing authorization from various health authorities, in five distinct molecular architectures; that of the 49 single domains that have reached clinical trials or the market, 6 carry an additional disulfide bridge to CDR3, none carry a canonical N-glycosylation motif, 34 have an isoelectric point above 7.5, and the most frequent FR2 hallmark motifs are FERF (15/49) and VGLW (12/49); that non-canonical cysteines are enriched in CDR1 (IMGT 38) in camel and dromedary and in FR2 (IMGT 55) in llama and alpaca; and that the authors' group's standard immunization protocol calls for four subcutaneous administrations of 300 µg on days 0, 14, 28 and 35 and a draw of 100 mL seven days after the last one.
It cannot be concluded which species of camelid that protocol refers to, nor what body weight: the authors do not say, the full text has been read in its entirety, and without those two data points the 100 mL cannot be converted into a percentage of circulating blood volume and cannot be compared against the 10% threshold of Annex VIII of Directive 2010/63/EU →
[TO BE CHECKED AGAINST THE FULL TEXT]. It cannot therefore be concluded which severity category that draw falls under, and the category is not to be inferred: it is read in the non-technical summary of the authorized project. It cannot be concluded that this review exhausts the literature on any of its topics: it has no stated search strategy and does not claim to have one. And nothing clinical can be concluded for a camelid: this article contains no sick animal, no diagnosis and no veterinary therapy.
Take-home messages
- Five VHH-based drugs are on the market, as of the October 2025 data point. The field born from card G-01 has reached the pharmacy, and not veterinary medicine: none of the five is a veterinary medicinal product.
- Immunizing a camelid to produce nanobodies does not kill it — the endpoint is the blood draw. But "stays alive" and "is doing well" are two different claims, and the second has not been measured independently by anyone.
- The published protocol states 300 µg per time, four times, subcutaneously, and 100 mL of blood seven days after the last one. It is the first volume-drawn figure with an open primary source in this KT.
- That number is still not enough to judge the procedure, because species and weight are missing: the European standard reasons in percentage of blood volume, with the 10% threshold. These are the three questions to ask anyone who manages a herd used for immunization.
- The animal-free alternative exists — synthetic and semi-synthetic libraries — with a stated cost in starting affinity. When the animal route is chosen, the choice must be justified.
- Llamas and alpacas are not camels even at the sequence level: non-canonical cysteines sit in CDR1 in Old World camelids and in FR2 in New World ones.
