◆ ASSOCIATION'S COMMENTARY

Camelid Research Studies APS — CRL editorial team · 31/08/2026

A single herd, but a sequence of entirely lawful steps: purchases from the United Kingdom with paperwork in order, and the official test failing to find the infected animals. Not for its numbers, which are from a single herd, but for the sequence of facts it describes, which is made up entirely of lawful steps: a Polish breeder buys alpacas from the United Kingdom for years, with health documentation in order; the tuberculin skin test — the official test — does not find the infected animals before they leave; two months after the last arrival (May 2018) two alpacas are hospitalized and then die of tuberculosis; in November the remaining twenty are culled and it is discovered that twenty out of twenty were infected. No step in this chain is unlawful, and the result is M. bovis in a herd that did not know it had it.

The most valuable data point is the negative one. A commercial assay — Enferplex Camelid TB — missed all twenty infected alpacas. A zero out of twenty needs no confidence interval to be read: that test, in that herd, saw nothing. And since a negative result is typically the one that least benefits a test vendor, it should be kept separate from the rest — it is the part of the work on which the authors' affiliation weighs least, not most.

The data point that will be cited most is the one that is worth least. «The DPP tests found 14 infected out of 20» will become, in second-hand citations, «the DPP assays have 70% sensitivity». It is not the same thing. The editorial team calculated the interval the article does not give — editorial calculation, Wilson method, z = 1.96, not a result from the authors: 14 out of 20 corresponds to a proportion between 48.1% and 85.5%. That is: in that herd, those assays may have seen a little more than half of the infected animals or nearly nine out of ten, and the data do not allow one to distinguish. A number with that spread does not found a purchasing protocol on its own; it founds the idea that one tests in more than one way and waits. (For completeness, with the same method: the Enferplex 0 out of 20 has an upper limit of 16.1%. Even under the most favourable assumption for the test, one infected alpaca in six would have been its best catch.)

What changes for a veterinarian in Italy, today. Three things, and none requires a new laboratory. (a) At the time of purchase, a certificate with a negative skin test result must be treated as a regulatory requirement fulfilled, not as proof of absence of disease: these are two different functions of the same piece of paper, and confusing them is precisely the error that produced this outbreak. (b) Quarantine on entry is not a bureaucratic leftover: in a disease with a long subclinical course, time does what the test does not do. Here about two months passed between the last arrival (May 2018) and the hospitalization of the two sick alpacas (July 2018), who died a few weeks later (article abstract on Europe PMC). (c) The combination to fear is not «infected animal», it is repeated multiple sourcing over time + absence of prolonged observation. The Polish centre had been buying from the United Kingdom «for several years»: each batch was an independent draw, and the herd was the sum of all of them.

What this record cannot tell you. Whether the DPP assay is available in Italy, at what cost, under what regulatory framework, and whether a positive serological result in a camelid currently has defined official consequences. This has not been looked into, and cannot be deduced from the article. Whoever must decide should ask the competent authority, not a bibliographic record.

Europe PMC reports an empty licence field. Read in Crossref on 31/08/2026.

This record may reproduce: factual data and short quoted excerpts from the abstract, which is the only accessible part of the text. No figures, no tables, no excerpts of the article body.

9 · Specie e soggetti

Alpaca (Vicugna pacos).

9 · Species and subjects

Alpaca (Vicugna pacos). The study population is 40 animals: 20 alpacas infected with Mycobacterium bovis and 20 uninfected. Of the infected ones, only 15 were tested with all the serological assays — the figure is in the abstract and recurs in field 15, where it creates an arithmetic ambiguity that this record does not resolve.

Husbandry context, from the abstract: a breeding centre in Poland that had been purchasing alpacas from the United Kingdom for several years; the last group arrived in May 2018. In July 2018 two sick alpacas from the centre were admitted to a veterinary clinic and both died of tuberculosis a few weeks later. In November 2018 the remaining 20 alpacas of the herd affected by M. bovis were culled and sampled.

The group also included three orphans and two cria-dam pairs, which according to the authors offered the opportunity to analyse immune aspects of the cria-dam relationship within the herd. Outcomes of that analysis: [TO BE CHECKED AGAINST THE FULL TEXT].

⚠️ Age, sex and origin of the 20 uninfected alpacas are not in the abstract → [TO BE CHECKED AGAINST THE FULL TEXT]. This is not a minor detail: where the comparison group comes from determines whether the specificities reported hold any value outside this herd.

10 · Domanda clinica (PICO)

  • I — quattro saggi sierologici: Enferplex Camelid TB, DPP VetTB, DPP BovidTB, MAPIA (multi-antigen print immunoassay).

10 · Clinical question (PICO)

  • P — alpacas from a naturally infected herd with M. bovis, imported from the United Kingdom and negative on the tuberculin skin test prior to movement.
  • I — four serological assays: Enferplex Camelid TB, DPP VetTB, DPP BovidTB, MAPIA (multi-antigen print immunoassay).
  • C — the tuberculin skin test (TST), which is the official test, and the four assays against each other; plus a group of 20 uninfected alpacas.
  • O — antibody detection in animals with confirmed M. bovis infection, and absence of reactivity in the uninfected ones.

As a question: in alpacas infected with M. bovis but negative on the skin test, which serological assays identify them, and how many are missed?

11 · Disegno dello studio

11 · Study design

Observational study on a naturally infected herd, with comparative evaluation of four diagnostic assays on two groups defined post mortem (20 infected, 20 uninfected). It is not a trial, it is not randomised, it is not prospective: it is the description of an outbreak plus a laboratory test on the samples that outbreak made available.

⚠️ How infected/uninfected status was ascertained — culture, histopathology, spoligotyping, or which combination — is not in the abstract → [TO BE CHECKED AGAINST THE FULL TEXT]. It is the pivot of the entire study, and it is not readable without the article.

12 · Level of evidence

Evidence from an observational study on a single herd, with no blinding declared and no reference standard described in the accessible record.

Consequence for the reader: the numbers in this work say what happened in that herd, and they are sufficient to demonstrate a failure (a test that misses all infected animals is wrong, and nothing more is needed to know that). They are not sufficient to establish the sensitivity of an assay: that requires multiple herds, a declared reference standard and confidence intervals. Whoever reads «the DPP tests found 14 of 20 infected» and derives «the DPP assays have 70% sensitivity» is extrapolating beyond the design.

13 · Relevant reporting standard

STROBE (observational studies), from the closed list in chapter 14 §3. The authors do not declare any reporting checklist in the abstract → [TO BE CHECKED AGAINST THE FULL TEXT].

⚠️ It must be stated precisely: the closed list in chapter 14 does not contain a checklist for diagnostic accuracy studies, which is what half of this article is. This record uses STROBE because it is the tool provided by the house method for the observational part, and it declares that it is using a tool that covers only half of the work. It is not the job of a single record to invent another one: if the gap matters, it is fixed in the chapter 14 checklist, for all records (§11), not here.

14 · Metodo

14 · Method

Reconstructed from the abstract alone, which is all the APIs return.

  • Cohort origin: Polish breeding centre, repeated purchases from the United Kingdom over several years, last group in May 2018.
  • Index event: July 2018, two sick alpacas admitted to a veterinary clinic; both died of tuberculosis a few weeks later.
  • Sampling: November 2018, culling and sampling of the remaining 20 alpacas of the affected herd.
  • Groups: 20 alpacas infected with M. bovis, 20 uninfected; only 15 of the infected ones were tested with all the serological assays.
  • Tests performed: tuberculin skin test (before movement, in the United Kingdom); Enferplex Camelid TB; DPP VetTB; DPP BovidTB; MAPIA as confirmation.
  • ⚠️ Not recoverable from the abstract → [TO BE CHECKED AGAINST THE FULL TEXT]: inclusion and exclusion criteria; origin and health status of the 20 uninfected animals; how infection status was ascertained; site and reading of the skin test; interval between the skin test and blood draw for serology (this is the variable that most shifts serological results in alpacas); assay positivity thresholds; operator blinding; number of sites; ethical approval.

Testualmente, dall'abstract: «The DPP VetTB and DPP BovidTB assays detected antibodies in 14 of the 20 infected alpacas, with results confirmed by MAPIA, and in none (MAPIA and DPP BovidTB) or one (DPP VetTB) of the 20 uninfected animals» e «None of the infected alpacas tested positive using the Enferplex assay».

Verbatim, from the abstract: «The DPP VetTB and DPP BovidTB assays detected antibodies in 14 of the 20 infected alpacas, with results confirmed by MAPIA, and in none (MAPIA and DPP BovidTB) or one (DPP VetTB) of the 20 uninfected animals» and «None of the infected alpacas tested positive using the Enferplex assay».

⚠️ Arithmetic ambiguity that this record flags and does not resolve. The abstract says that only 15 of the 20 infected animals were tested with all the assays, and then reports 14 of 20 for the two DPP tests. The two statements coexist only if the DPP tests were performed on all 20 and the full panel (MAPIA included) on 15. The abstract does not say so → [TO BE CHECKED AGAINST THE FULL TEXT]. Until this is verified, the DPP denominator is what the authors write — 20 — and this record does not fabricate another one.

⚠️ No confidence interval is reported in the abstract, for any of the values. See field 22 for the calculation the editorial team made, and why they made it.

Clinical outcomes, from the abstract: the two alpacas admitted in July 2018 both died of tuberculosis within a few weeks.

16 · Authors' interpretation

Explicitly attributed, from the abstract, and kept separate from field 15.

The authors write that the results «suggest a high susceptibility of this host species to infection with M. bovis and rapid progression to disease»; that the serological assays used «offer useful tools for detection of M. bovis infection in alpacas non-reactive to the skin test and to the Enferplex assay»; and that these assays «should be further evaluated for implementation in tuberculosis management and control strategies for camelid species». They open the work stating that it «highlights the transboundary nature of tuberculosis in alpacas and the failure of current ante mortem testing protocols to identify herds and individuals free of tuberculosis for export purposes».

Worth noting what the authors do not claim: they do not claim a sensitivity for the DPP assays, do not propose a threshold, do not declare the assays ready for official use. They say «should be further evaluated». Whoever cites this article to argue that «a reliable serological test for camelids exists» is making it say more than it says.

17 · Limiti dichiarati dagli autori

17 · Limitations declared by the authors

⚠️ Not recoverable: the limitations section is not in the abstract and the full text is behind a paywall → [TO BE CHECKED AGAINST THE FULL TEXT]. This record does not state that the authors did not declare them: it states that they could not be read. These are two different statements, and only the second one is true.

18 · Limiti aggiunti dalla redazione

18 · Limitations added by the editorial team

  1. Forty animals, one herd, one event. Every figure in this work describes a single Polish outbreak from 2018. There is no replication, no multicentricity, no between-herd variability from which to estimate how much the numbers would shift elsewhere.
  2. No confidence interval, for any value. With n = 20 the intervals are wide enough to change the meaning of a sentence (field 22).
  3. The comparison group is a closed box. Origin, health history and selection criteria of the 20 «uninfected» alpacas are not readable. A specificity of 100% measured on a group about which nothing is known is a number with no support.
  4. The reference standard is not described in the accessible record. Without knowing how «infected» was defined, one cannot say what the assays detected or missed.
  5. Blinding is not declared. Did whoever performed and read the assays know which group a sample belonged to? There is no way to know.
  6. The 15-versus-20 question remains open (field 15).
  7. ⚠️ Who conducted the study, and on which products. Four of the eleven authors — Sridhara, Elahi, Johnathan-Lee, Lyashchenko — declare affiliation with Chembio Diagnostic Systems, Inc. The assays that perform well in the study carry the abbreviation DPP in their name, which in the abstract is spelled out by the authors themselves as dual-path platform; the assay that misses all twenty infected animals, Enferplex Camelid TB, is a commercial product of a different manufacturer. No conflict-of-interest statement is recoverable from the PubMed record (CoiStatement field absent) → field 20. This is not an accusation and does not detract from the negative result, which is the most solid finding of the work: 0 out of 20 is 0 out of 20 no matter who measures it. It is information the reader must have before reading the comparison between competing products, and one that chapter 14 §5.4 requires be written.

19 · Finanziamento

19 · Funding

Small Business Innovation Research Program of the USDA National Institute of Food and Agriculture, grant no. 2016-33610-25688 — as recorded in PubMed's GrantList field and in Europe PMC's grantsList field (queried 31/08/2026). Crossref records no funder for this DOI (funder field empty).

It must be read for what it is: Small Business Innovation Research is, by the programme's definition, company R&D funding. The sponsor's role in study design, data collection and manuscript drafting is not declared in the accessible records → [TO BE CHECKED AGAINST THE FULL TEXT].

20 · Conflicts of interest

⚠️ No conflict-of-interest statement is present in the PubMed record (CoiStatement field absent at efetch query on 31/08/2026) → [TO BE CHECKED AGAINST THE FULL TEXT].

What is in the record, and what this record reports as fact and not as inference: four of eleven authors are affiliated with Chembio Diagnostic Systems, Inc., and the funding is an SBIR grant (field 19). If a conflict-of-interest statement exists in the article, it must be read: until it is, these two facts are what the reader has.

21 · What is missing relative to the reporting standard (STROBE)

Assessment conducted on the accessible record alone. Every item may be present in the full text; the list states what is not verifiable without it, not what is missing from the article.

  • Eligibility criteria and sources of participants (STROBE 6) — not verifiable, in particular for the uninfected group.
  • Definition of variables, including the reference standard for «infected» (STROBE 7) — not verifiable.
  • Data sources and measurement methods, with comparability across groups (STROBE 8) — not verifiable; in particular the interval between the skin test and serological sampling.
  • Efforts to address sources of bias, including blinding (STROBE 9) — not verifiable.
  • Study size: how 20 + 20 was arrived at (STROBE 10) — not verifiable; plausibly dictated by the outbreak and not by a calculation, which is legitimate and should be stated.
  • Missing data (STROBE 12c) — partially visible and unresolved: 15 of 20 tested with all the assays.
  • Precision of estimates, confidence intervals (STROBE 16a) — absent from the abstract.
  • Conflict-of-interest statement — absent from the PubMed record (field 20).

23 · Valutazione a stelle

23 · Star rating

Four independent axes, criteria from chapter 14 §11. A caveat weighing on all four lines: the score was assigned on the accessible record alone, because the full text is behind a paywall. If someone reads it, this assessment should be redone — and it might rise.

AxisScoreRationale, with one fact inside
A — Scientific quality★★☆☆☆Stated sample size (20 + 20) and readable design, but no confidence interval on any value, selection criteria for the comparison group not recoverable, reference standard not described in the record, and no conflict-of-interest statement in the PubMed record against four authors affiliated with the manufacturer of two of the assays evaluated.
B — Clinical usefulness★★★☆☆Offers a replicable management approach, immediately applicable in Europe — not trusting a single ante mortem test, extending quarantine observation, treating multiple sourcing as a risk factor — with the caution the level of evidence requires. It does not reach four because the quantitative part (which assays, at what threshold) is not usable from a single herd. (Constraint from chapter 14 §11 respected: B does not exceed A by more than two stars.)
C — Educational value★★★★☆Teaches, on a real, documented case, the difference between «negative test» and «absence of disease», and shows two commercial assays that give opposite answers on the same animal. It is training material for anyone who buys or certifies camelids. It does not reach five because the full diagnostic reasoning (how infection status was ascertained) is not readable.
D — Species relevance★★★★☆Study on alpacas, in Europe, on animals moved between two member states under procedures that also apply to Italy: transferability is direct. It does not reach five because it is a single herd and the population is not described enough (age, sex, origin of the controls) to assess its representativeness.

24 · Bottom line

24 · Bottom line

One can conclude that: in a herd of alpacas naturally infected with M. bovis, the tuberculin skin test did not prevent the international movement of infected animals, and a commercial serological assay (Enferplex Camelid TB) detected none of the twenty infected alpacas — while two assays on the DPP platform detected fourteen out of twenty, with MAPIA confirmation and at most one false positive out of twenty uninfected animals.

One cannot conclude that: the DPP assays have a known sensitivity, nor that they are ready to officially replace or supplement the skin test — the authors themselves write that they «should be further evaluated». And nothing can be concluded about the frequency of tuberculosis in alpacas in general: this study sampled one outbreak, not a population.
