Che cosa misurano i quattro assi
A · Qualità scientifica
La domanda è ben posta? I metodi sono dichiarati? La statistica è appropriata? La conclusione sta dentro i dati?
B · Utilità clinica
Serve a chi ha l’animale davanti, oggi.
C · Valore didattico

What the four axes measure
A · Scientific quality
Is the question well posed? Are the methods stated? Is the statistics appropriate? Does the conclusion stay within the data?
B · Clinical usefulness
Useful to whoever has the animal in front of them, today.
C · Teaching value
Does it teach something to those studying the subject?
D · Relevance to the species
Is it really about camelids, or a result borrowed from another species?

Each axis is worth 1 to 5 stars and carries its written reason. The stars in the index are the rounded average of the four.

(d) L'esito precoce è buono, l'esito tardivo è documentato in due animali e uno era ancora zoppo a sei mesi.

◆ ASSOCIATION'S COMMENTARY

Camelid Research Studies APS — CRL editorial staff · 20/09/2026

Five animals, and they are worth more than many series of fifty: why this article publishes its tables. Chapter 10 of the KT uses this work to remove a claim that was there before — the one stating that in camelids septic arthritis comes «mainly from penetrating wounds or direct trauma, unlike the foal». Here the two neonates arrive via the hematogenous route from sepsis with failure of transfer of passive immunity, and the three juveniles arrive with a single joint and no wound, to the point that the authors hypothesize the hematogenous route for them too. Five cases do not establish a proportion; they are enough, however, to demolish the opposite claim, which was the one published. This is how a small series does its job: it does not prove, it disproves.

The most useful data point in this card is not the 80 %: it is fibrinogen. Four animals out of five had normal WBC counts, three out of five had normal neutrophil counts, and the most severe neonate — the one that later died — was leukopenic and neutropenic, that is, had a CBC that a hasty clinician reads as «this is not an infection». Fibrinogen was elevated in five out of five. With five animals one cannot calculate a sensitivity and this card does not calculate one; but the practical message does not require statistics: in a young camelid that is lame, a normal CBC rules out nothing, and fibrinogen is cheap.

The second thing that changes practice is ultrasound, and not for the reason one imagines. Severe synovial thickening was present in every joint examined with ultrasound, and ultrasound saw cortical irregularities and gas that radiography did not always show — the authors note that in the first 7-10 days radiography does not rule out osteomyelitis. But its most concrete function is another: it guided the needle in four cases out of five. In a small joint, in a four-day-old cria, with little fluid, blind arthrocentesis is precisely the maneuver that in case 1 yielded a volume insufficient for cytology.

The one line the editors urge readers to be strict about is the dose table. It is the only dose table in this issue, and it can be published because that article declares, molecule by molecule, the off-label use and reports for each animal the species, age, dose, route and frequency. But four cases out of five received ceftiofur, which in Europe is a third-generation cephalosporin in EMA category B — the molecules that the CRL-J-05 card in this same issue documents as those whose use should be reduced, and which in a European university clinic ended up precisely on crias and on dental pathology. The two cards must be read against each other: one tells what worked in California, the other what Europe asks not to use first. Whoever reads only one gets half the problem.

What changes for a veterinarian in Italy, today. Four things. (a) A cria that is lame and has elevated fibrinogen deserves arthrocentesis even if the CBC is normal and even if there is no fever: case 2 was normothermic and without appreciable swelling, and had a septic hip. (b) Enough fluid is drawn, and culture is done regardless: in two cases out of five the synovial culture yielded the same organism as the blood culture, meaning a single tube answered two questions. In three cases out of five the culture was negative in joints with 165,000 and 208,770 cells/µL: a negative culture does not clear a joint. (c) Isolated lameness in a four-month-old or one-year-old animal, without fever and without a wound, can be septic. Three cases out of five were like this, and one was lame only at the trot. (d) Early outcome is good, late outcome is documented in two animals and one was still lame at six months. This is what should be told to the owner, not the 80 %.

What this card cannot tell you. How frequent septic arthritis is in camelids — five cases in twenty-three years at one hospital say it is rare there, not how rare it is overall —; whether one protocol is better than another, because there is no comparison; and whether these antimicrobials, at these doses, are appropriate for a European camelid in 2026. On this last point the answer does not lie in this article and does not lie in this card: it lies in the prudent-use regulations in force and in the responsibility of the prescriber.

3 · Autori

Miklavcic Mitja (ORCID 0000-0001-8630-9452) · Kilcoyne Isabelle · Dechant Julie E. · Ratliff Bridget · Magdesian K. Gary

  • William R. Prichard Veterinary Medical Teaching Hospital, UC Davis School of Veterinary Medicine, Davis, CA, Stati Uniti: Miklavcic;
  • Department of Surgical and Radiological Sciences, UC Davis School of Veterinary Medicine, Davis, CA, Stati Uniti: Kilcoyne, Dechant;
  • Comstock Equine Hospital, Reno, NV, Stati Uniti: Ratliff;
  • Department of Medicine and Epidemiology, UC Davis School of Veterinary Medicine, Davis, CA, Stati Uniti: Magdesian.

3 · Authors

Complete list, five authors, in the order and form of the record — Crossref and PubMed agree on number, order and spelling:

Miklavcic Mitja (ORCID 0000-0001-8630-9452) · Kilcoyne Isabelle · Dechant Julie E. · Ratliff Bridget · Magdesian K. Gary

The first author's ORCID is the only one registered, and it appears in both sources (Crossref, PubMed and also the full text). No ORCID is registered for the other four authors.

Affiliations for each author, as printed in the full text and recorded in PubMed:

  • William R. Prichard Veterinary Medical Teaching Hospital, UC Davis School of Veterinary Medicine, Davis, CA, United States: Miklavcic;
  • Department of Surgical and Radiological Sciences, UC Davis School of Veterinary Medicine, Davis, CA, United States: Kilcoyne, Dechant;
  • Comstock Equine Hospital, Reno, NV, United States: Ratliff;
  • Department of Medicine and Epidemiology, UC Davis School of Veterinary Medicine, Davis, CA, United States: Magdesian.

Three departments out of four are equine or surgical, and one author works at an equine hospital. This is not a polemical point: it is the key to reading the entire article, which applies to camelids a diagnostic and therapeutic framework built on the foal, and states this explicitly (field 16).

⚠️ Internal error in the article, flagged because anyone citing it will copy one of the two forms. The printed affiliation reads «Prichard Veterinary Medical Teaching Hospital»; the Methods section of the same article reads «Pritchard». The correct name of the UC Davis hospital is the one with the t. This card reports both spellings as they stand in the text, without correcting either.

EventoData

Crossref registra published-print 02/03/2026 e published-online 25/03/2026; PubMed esummary registra pubdate 2026 Mar 2.

5 · Journal · year · volume · issue · pages

Journal of Veterinary Internal Medicine, 2026, volume 40, issue 2, article aalag056 (the journal numbers by article; there is no page range, and the PubMed and Europe PMC records in fact leave the pages field empty or report the article identifier).

This article has five dates, all read in the full text, and they must be distinguished:

EventDate

Crossref registers published-print 02/03/2026 and published-online 25/03/2026; PubMed esummary registers pubdate 2026 Mar 2. The date of 2 March is the acceptance date registered as the print publication date: it is a metadata discrepancy, not one in the article, and this card flags it because anyone citing «2026 Mar 2» is citing a date that in the full text means something else.

7 · Stato della rivista

Verificato il 20/09/2026. Journal of Veterinary Internal Medicine: ISSN 0891-6640 (stampa) e 1939-1676 (elettronico); NLM ID 8708660; editore Oxford University Press per conto dell'American College of Veterinary Internal Medicine.

7 · Journal status

Verified on 20/09/2026. Journal of Veterinary Internal Medicine: ISSN 0891-6640 (print) and 1939-1676 (electronic); NLM ID 8708660; publisher Oxford University Press on behalf of the American College of Veterinary Internal Medicine. PubMed: yes, with subset IM — Index Medicus. DOAJ: yes — the DOAJ API queried on issn:1939-1676 on 20/09/2026: 1 result, with license recorded as CC BY. It is therefore a journal that is entirely open access, not a subscription journal with individual open articles. It is the reference journal for North American veterinary internal medicine, and for a case series on a minor species it is a venue of good standing.

8 · Article license

This card may reproduce: everything, including tables and figures, provided attribution is given. The dose tables in field 15 are editorial reworkings of the data in supplementary tables S1 and S3, not graphic reproductions, and are presented with the attribution required.

9 · Specie e soggetti

1Alpaca4 giornifemminaradiocarpica sinistraneonato (≤ 1 mese)
2Alpaca6 giornimaschiocoxofemorale sinistraneonato (≤ 1 mese)
3Alpaca4 mesifemminametacarpofalangea laterale sinistra, anterioregiovane (1-12 mesi)
4Lama5 settimanefemminagomito sinistrogiovane (1-12 mesi)
5Lama12 mesimaschiogomito sinistrogiovane (1-12 mesi)

9 · Species and subjects

Five New World camelids: three alpacas and two llamas. The body of the article and supplementary table S1 make it possible to reconstruct every single animal — a rare thing in this issue, and the reason this card can carry a dose table.

CaseSpeciesAgeSexJoint affectedAge group (authors' definition)
1Alpaca4 daysfemaleleft radiocarpalneonate (≤ 1 month)
2Alpaca6 daysmaleleft coxofemoralneonate (≤ 1 month)
3Alpaca4 monthsfemaleleft lateral metacarpophalangeal, frontjuvenile (1-12 months)
4Llama5 weeksfemaleleft elbowjuvenile (1-12 months)
5Llama12 monthsmaleleft elbowjuvenile (1-12 months)

Authors' definitions: neonate = up to one month; juvenile = 1-12 months. The five-week-old llama therefore falls among the juveniles by a hair's breadth of the calendar, and is classified as such by the authors.

Each animal had only one joint affected, and none was polyarticular.

⚠️ Editorial observation, to be taken for what it is. All five affected joints are on the left. In a series of five animals, the probability that this happens by chance, assuming right and left are equally likely and independent, is (1/2)⁵ = 3.1 % (editorial calculation, not the authors'). This is not a finding: with five cases one could easily be looking at a coincidence. But it is unexpected enough to deserve a line, and the two non-clinical explanations should be considered before the clinical ones: a recording artifact in the chart, or a side convention in which «left» is the default value. The authors do not comment on this or flag it.

Husbandry context: absent. The article says nothing about herd of origin, management, colostrum intake (except through the diagnosis of FPT), distance, reason for referral → [TO BE CHECKED AGAINST THE FULL TEXT] — and here the flag must be read literally: the full text has been read, and the data point is simply not there.

10 · Domanda clinica (PICO)

  • I — antimicrobici sistemici e locali (intrarticolari) in tutti; lavaggio articolare chirurgico in 4 casi su 5; perfusione regionale endovenosa dell'arto in 1 caso su 5.

10 · Clinical question (PICO)

  • P — neonatal (≤ 1 month) and juvenile (1-12 months) New World camelids with a diagnosis of septic arthritis, presented to a university teaching hospital between 2000 and 2022.
  • I — systemic and local (intra-articular) antimicrobials in all; surgical joint lavage in 4 of 5 cases; intravenous regional limb perfusion in 1 of 5 cases.
  • C — no internal comparison group. The comparison is external and narrative, with the literature on the foal and calf, taken up by the authors in the discussion.
  • O — survival to discharge (short term) and survival to ≥ 6 months (long term), plus absence of lameness at discharge.

Objective stated by the authors: «to describe the clinical presentation, etiologies, diagnostics, treatment, and short- and long-term outcome in neonatal and juvenile New World camelids with septic arthritis» (editorial translation).

11 · Disegno dello studio

11 · Study design

Retrospective case series, single-center, with stated numerical inclusion criteria — which places it a notch above the ordinary case series, and makes verifiable something that almost never is (field 18, finding a).

12 · Level of evidence

Very low — retrospective case series, n = 5, without a comparison group.

Consequence for the reader: from five animals no transferable proportion can be derived. The «80 % survival to discharge» is four animals out of five, and its 95 % confidence interval, which the article does not calculate, ranges from 28.4 % to 99.5 % (editorial calculation, exact Clopper-Pearson binomial interval). That is: those data are compatible with a world in which fewer than a third of treated animals survive. What this article delivers is not rates, but five clinical pathways documented down to the dose — and that is a great deal, because before there was none.

13 · Relevant reporting standard

CARE (case report) as the minimum framework; for a series with explicit inclusion criteria, the STROBE criteria for observational studies are the stricter and more appropriate reference.

The authors declare neither one. The structure of the article, however, satisfies most of CARE: dated chronology per case, diagnostics reported, interventions described, outcomes with stated follow-up, ethical, funding and conflict-of-interest declarations, and — a rare and appreciable item — a declaration of off-label drug use.

  • Fonte: cartelle cliniche del William R. Pritchard Veterinary Medical Teaching Hospital, University of California-Davis.

  • Source: medical records of the William R. Pritchard Veterinary Medical Teaching Hospital, University of California-Davis.
  • Period: 2000-2022, twenty-three years.
  • Number of sites: one.
  • Inclusion criteria, verbatim (editorial translation): diagnosis of septic arthritis in neonatal or juvenile New World camelids, based on clinical signs and diagnostic imaging, together with cytological analysis of synovial fluid confirming a total nucleated cell count ≥ 25,000 cells/µL, with ≥ 80 % neutrophils, total protein ≥ 4.5 g/dL, and the presence of intracellular bacteria on cytology, or a positive bacterial culture of synovial fluid.
  • Exclusion criteria: no synovial fluid analysis performed, or incomplete medical record.
  • FPT definition (failure of passive transfer of immunity): serum IgG < 900 mg/dL or immunocrit < 11 %.
  • Outcome definition: short term = survival to discharge; long term = survival ≥ 6 months from discharge. Long-term follow-up obtained from the record or by telephone questionnaire to owners.
  • Antimicrobial susceptibility thresholds: the authors state that all breakpoints were extrapolated from CLSI guidelines for horses or from published pharmacokinetic studies in alpacas, and that they apply only to parenteral antibiotics. For oral antibiotics, interpretation was not included, «because the bioavailability and disposition of these drugs when administered in camelids are not known». This is a well-made statement of ignorance, and it deserves to be cited more often than it is.
  • Techniques described: through and through joint lavage with 14 or 16 G needles and sterile polyionic fluids, under sedation or general anesthesia, with ultrasound guidance in selected cases; arthrotomy with two incisions, removal of purulent material and lavage; intravenous regional limb perfusion with a proximal tourniquet, perfusate slowly injected into the cephalic vein and the tourniquet maintained for 20 minutes.
  • Ethical approval: declared not required by the authors, both for the animals and for the human component (owner questionnaire).
  • Statistical analysis: none, and correctly so — with five animals there is nothing to test. The authors report medians and ranges, not comparisons.

15.1 · Presentazione clinica

    • Caso 2: tachicardico 160 bpm, normotermico, zoppo sull'arto posteriore sinistro, senza tumefazione apprezzabile.

Casi 1 e 5: nessun intervento chirurgico, solo medicazione intrarticolare.

15.6 · Esito

  • Follow-up a lungo termine: disponibile per 2 casi su 5 (casi 2 e 5).

Numbers only. Where a number comes from the supplementary tables, this is stated.

15.1 · Clinical presentation

  • Two neonates (cases 1 and 2, both alpacas) presented for lethargy and fever in case 1, for left hindlimb lameness in case 2. Both had received procaine penicillin (PPG) before admission.
    • Case 1: obtunded, tachycardic at 140 bpm (reference interval cited by the authors 70-100 bpm), febrile at 39.4 °C (reference < 38.9 °C); heat and swelling of the left carpus.
    • Case 2: tachycardic at 160 bpm, normothermic, lame on the left hindlimb, without appreciable swelling.
  • Three juveniles (cases 3, 4, 5) presented with normal vital parameters and lameness as the sole reason for consultation: non-weight-bearing (case 3), evident at the walk (case 4), evident only at the trot (case 5). None had a history of trauma and none had wounds found at the initial examination. Heat and swelling over the affected joints in cases 3, 4 and 5.
  • Case 4 had received intra-articular amikacin before admission.

Reference intervals used by the authors: WBC 7,890-20,270 cell/µL; neutrophils 3,670-10,960 cell/µL; fibrinogen 100-300 mg/dL.

The data point that matters more than any other (editorial reading of the authors' numbers): fibrinogen was elevated in 5 of 5 cases, while total WBC was within the reference interval in 4 of 5 cases and neutrophils in 3 of 5 cases. In a series of five this is not proof; it is, however, the only parameter that did not miss a single animal, and the septic neonates are exactly the ones in which the CBC was misleading (case 1: leukopenia and profound neutropenia, 56 neutrophils/µL).

Immunocrit: measured only in case 2, 4 % (FPT threshold: < 11 %). In case 1 the diagnosis of FPT had been made before admission and the article does not report the value → [TO BE CHECKED AGAINST THE FULL TEXT] — the full text has been read and the value is not there.

15.5 · Antimicrobial treatment — the dose table

⛔ How this table must be read. These are doses administered to five individual animals at a US university hospital between 2000 and 2022, reconstructed from supplementary table S3 and matched here to the species and age of each animal (table S1). All off-label: the authors state by name that «amikacin, ampicillin, cefazolin, ceftiofur, florfenicol, oxytetracycline, and ticarcillin-clavulanate were used off-label». This is not a recommended dosage, it is not a protocol, and it has not been validated by any efficacy study in this species. In Italy, off-label use of these molecules in camelids falls under the responsibility of the prescribing veterinarian.

Intravenous regional limb perfusion: performed only once, in case 3 — a 4-month-old female alpaca — with 50 mg of ceftiofur sodium slowly injected into the cephalic vein, a proximal tourniquet maintained for 20 minutes, as adjunctive therapy for septic physitis. The authors do not report adverse effects.

Interventions: through and through lavage under general anesthesia in cases 2 and 4, under sedation in case 3; repeated in case 3 6 days later; in case 4 arthrotomy 4 days after the first lavage. Cases 1 and 5: no surgical intervention, only intra-articular medication. Number of intra-articular treatments: median 2, range 1-4.

Supportive therapy (supplementary material): cases 1 and 2 were also treated for FPT and septicemia with plasma transfusion (1-2 units, 300 mL, of llama plasma), intravenous fluid therapy, pantoprazole 1 mg/kg IV every 24 h, non-steroidal anti-inflammatory drugs (flunixin 1 mg/kg IV or IM every 12 h, or meloxicam 0.5 mg/kg IV every 48 h), dextrose 4 mg/kg/min as a continuous infusion, insulin 0.01 IU/kg/h as a continuous infusion, and thiamine 10 mg/kg IV diluted every 12 h. Cases 3-5 received flunixin meglumine 0.5 mg/kg IV or meloxicam 1 mg/kg orally, and cases 3 and 4 also pantoprazole 1 mg/kg IV every 24 h. Same caveats as the table above: five animals, off-label use, no efficacy validation.

15.6 · Outcome

  • Survival to discharge: 4 of 5 (80 %).
  • Cases free of lameness at discharge: 3 of 5. Case 2 was discharged with mild lameness, markedly improved compared with admission.
  • Case 1 (alpaca, 4 days): euthanized on the eighth day of hospitalization due to lack of response to therapy, after developing skin necrosis over the affected joint, which prevented further arthrocenteses and intra-articular treatments. Necropsy not performed.
  • Long-term follow-up: available for 2 of 5 cases (cases 2 and 5). Case 2 was still mildly lame at 6 months; in case 5 the lameness had resolved completely.
  • Hospitalization: from 4 to 10 days.

Editorial calculation, to convey the extent of the uncertainty the article does not quantify. 4 of 5 corresponds to a proportion ranging from 28.4 % to 99.5 % (exact Clopper-Pearson binomial interval). And the «long term» rests on two animals: one recovered, one still lame. Only one animal out of five in this series is documented as fully recovered at six months. The rest is unknown — not negative, unknown.

16 · Interpretazione degli autori

Attribuita, e separata dai numeri.

16 · Authors' interpretation

Attributed, and kept separate from the numbers.

  • Abstract conclusion: «Septic arthritis in neonatal and juvenile New World camelids appears to have a favorable prognosis for discharge and a guarded prognosis for long-term outcome, with appropriate treatment» (editorial translation).
  • Body conclusion: «the results of this study indicate that septic arthritis in neonatal and juvenile New World camelids does occur and most likely arises from hematogenous bacterial spread. Treatment strategies extrapolated from other large animal species may be effective».
  • On the origin: in the two neonates the route is considered hematogenous, arising from sepsis secondary to FPT; in the three juveniles, which presented with a single infected joint and no systemic signs, hematogenous infection is nonetheless considered the most probable, even though no prior infectious process was documented.
  • On the cross-species comparison, with the authors' own numbers: in a study of 256 crias and 356 foals in critical condition, septic arthritis was documented in 4/256 crias (1.6 %) versus 42/356 foals (11.5 %); the authors write that the reason is unknown and might reflect differences in the immune response to sepsis or in synovial vascular microanatomy.
  • On bacteriology: Klebsiella spp. and E. coli have already been reported in the septic cria, whereas — «to the authors' knowledge» — Enterobacter spp. and P. aeruginosa had not been. The authors note that their blood cultures yielded only Gram-negative organisms, against previous studies reporting 45-80 % Gram-positive isolates in critical crias, and propose three alternative explanations: prior treatment with penicillin, the small number of cases, geographically different bacterial populations.
  • On imaging: radiography «may be of limited value in diagnosing bone infection within the first 7-10 days», and a negative radiographic finding within that window does not rule out bone involvement; ultrasound is indicated as more sensitive early on for bone irregularities, as well as useful for needle guidance.
  • On regional perfusion: no adverse effect observed, and «in appropriate cases IVRLP should be considered as a possible adjunctive therapy».

17 · Limiti dichiarati dagli autori

  1. «Il numero limitato di casi su un periodo lungo potrebbe aver contribuito alla variabilità del modo in cui i casi sono stati diagnosticati e trattati»;
  2. «la dipendenza dall'accuratezza delle cartelle cliniche potrebbe aver limitato il recupero dei dati»;

17 · Limitations stated by the authors

Quoted, not summarized. The authors write that the main limitations are those inherent to the retrospective design, and specify three:

  1. «The limited number of cases over a long period may have contributed to variability in how cases were diagnosed and treated»;
  2. «reliance on the accuracy of medical records may have limited data retrieval»;
  3. in the discussion, they acknowledge that the observed bacteriological differences might depend on the small number of cases.

This is a sober and correct statement. It does not contain the most significant limitation, which is addressed in the next field.

18 · Limitations added by the editors

(a) The stated inclusion criteria are not met as written, in any of the five cases. This is a significant finding and must be justified line by line, because it is verifiable only thanks to the fact that the authors published the tables. The written criterion is: nucleated count ≥ 25,000/µL and neutrophils ≥ 80 % and protein ≥ 4.5 g/dL, and intracellular bacteria on cytology, or positive culture.

  • In cases 1 and 2 the cytological triad could not be measured: the volume of fluid obtained was insufficient (case 1: smear and culture only; case 2: culture only). Case 1 moreover has protein 4.1 g/dL, below the stated threshold. Both qualify only through the positive culture.
  • In cases 3, 4 and 5 the cytological triad is amply satisfied, but culture is negative and no intracellular bacteria were seen: they qualify only through cytology. No case satisfies both conditions simultaneously. The reading that makes the series coherent is the one in which the comma before «or» separates two alternative pathways — cytological triad or microbiological evidence — and this is almost certainly what the authors intended. But as written, the sentence says something else, and a reader who applied the criterion literally would exclude three cases out of five. This card does not accuse: it flags that this article's inclusion criterion needs to be rewritten before being reused.

(b) A contradiction between the body and the table, on a therapeutic point. The abstract states: «all animals were treated with systemic and intra-articular antimicrobials», and the body repeats: «all cases received intra-articular antimicrobials in the affected joint at the end of the procedure». Supplementary table S3, however, records, for case 4, «Not given» — no intra-articular antimicrobial, in an animal that received both lavage and arthrotomy. The two statements cannot both be true. In the same line of text the authors write that intra-articular treatments were median 2, range 1-4: if case 4 received none, the lower end of the range should be 0. The editors do not know which of the two versions is correct → [TO BE CHECKED AGAINST THE FULL TEXT], here in the precise sense of: the full text has been read, and it contradicts itself.

(c) A number in the body does not match the table. The body writes that the neutrophil count was «high in case 4 (17,580; reference 3,670-10,960 cells/µL)», with a median of 9,846 and a range 56-11,033. Table S1 assigns to case 4 17,580 total WBC and 11,030 neutrophils. The 17,580 in the body is the WBC value, not the neutrophil value; and the upper end of the range, 11,033, does not correspond to any value in the table (11,030). The clinical conclusion — case 4 had neutrophilia — does not change; the number that will be cited does.

(d) The «long term» is two animals. The article defines long-term survival as ≥ 6 months and obtains it for 2 of 5 cases. The conclusion «guarded long-term prognosis» is therefore built on one animal that recovered and one still lame. This is a prudent and therefore acceptable wording, but whoever cites it must also cite the denominator.

(e) External comparison with non-comparable populations. The survival figures used in the discussion — foals 78-93 %, calves 59 % of 54 — come from different species, hospitalization systems and protocols, and serve to provide context, not to compare. The authors do not present them as a formal comparison; this card notes it because the typographic proximity of the numbers invites one.

(f) Twenty-three years, five cases, one hospital. Between 2000 and 2022 the antibiotics available, prudent-use regulations, ultrasound equipment, and probably referral criteria all changed. The case treated in 2000 and the one treated in 2022 are not comparable, and the article does not date the individual cases → [TO BE CHECKED AGAINST THE FULL TEXT]: the individual dates do not appear even in the supplementary tables.

(g) European transferability not discussed. Ceftiofur crystalline free acid, ticarcillin- clavulanate and florfenicol have, in Europe, an availability, a regulatory status and a place in the EMA classification very different from those in the United States. Among the molecules in the table, ceftiofur is a third-generation cephalosporin, EMA category B — «to be restricted», critically important for human medicine (CRL-J-05, field 15). A European veterinarian reading this table as a menu would find themself choosing, in four cases out of five, a molecule that European guidelines ask them not to use first. The article does not say so, because it is not written for Europe.

19 · Finanziamento

Dichiarato, e nullo. Testo pieno: «The authors received no specific funding for this work» — «gli autori non hanno ricevuto alcun finanziamento specifico per questo lavoro».

19 · Funding

Declared, and none. Full text: «The authors received no specific funding for this work». Crossref registers no funders. This is not a silence: it is an explicit statement.

20 · Conflitti di interesse

Dichiarati, e assenti. Testo pieno e record PubMed: «The authors declare no conflicts of interest».

20 · Conflicts of interest

Declared, and absent. Full text and PubMed record: «The authors declare no conflicts of interest».

To this must be added a statement that almost no veterinary article makes, and which should be acknowledged: the declaration of off-label use — «Amikacin, ampicillin, cefazolin, ceftiofur, florfenicol, oxytetracycline, and ticarcillin–clavulanate used off-label». Seven molecules named one by one. This is the reason the dose table in field 15 can be published with its warning without this card having to infer anything.

  • criteri di inclusione: presenti ma internamente incoerenti con i casi inclusi (campo 18a) — è il difetto principale;
  • coerenza fra corpo e tabelle: violata due volte (campo 18b sul trattamento intrarticolare, campo 18c sui neutrofili del caso 4);
  • cronologia individuale: assente. CARE chiede una linea del tempo per caso: qui si hanno durate di degenza e intervalli fra procedure, ma nessuna data, e nessuna indicazione di quale anno appartenga a quale caso;
  • diagnosi differenziale: non discussa caso per caso. Per i tre giovani senza segni sistemici e senza ferita, le alternative — trauma, osteocondrosi, fisite non settica — non sono argomentate;
  • esito riferito dal proprietario: il follow-up telefonico non è descritto (quando, con quale domanda, quanti proprietari non raggiunti);
  • numerosità: non giustificata, come è inevitabile, e dichiarata;

21 · What is missing relative to the reporting standard

Against CARE and the STROBE observational criteria, having read the full text:

  • inclusion criteria: present but internally inconsistent with the cases included (field 18a) — this is the main defect;
  • consistency between body and tables: violated twice (field 18b on intra-articular treatment, field 18c on case 4's neutrophils);
  • individual chronology: absent. CARE calls for a timeline per case: here there are hospitalization durations and intervals between procedures, but no dates, and no indication of which year belongs to which case;
  • differential diagnosis: not discussed case by case. For the three juveniles without systemic signs and without a wound, the alternatives — trauma, osteochondrosis, non-septic physitis — are not argued;
  • owner-reported outcome: the telephone follow-up is not described (when, with what question, how many owners not reached);
  • sample size: not justified, as is unavoidable, and stated as such;
  • ethical, funding, conflict-of-interest, and off-label use declarations: all present. On this front the article is above the average of the camelid literature.

23 · Valutazione a stelle

23 · Star rating

Four independent axes, criteria from chapter 14 §11. Full text and supplementary material read: unlike CRL-J-01 and CRL-J-04, this judgment carries no access caveats.

AxisRatingRationale, with one fact inside
A — Scientific quality★★☆☆☆Case series with clear chronology, complete individual data, ethical, funding, conflict-of-interest and off-label-use declarations all present, and — rarely — the raw tables published. It remains at two because there is no comparison group or statistical analysis (correctly, with n = 5), and because the inclusion criteria are not met as written in any of the five cases (field 18a) and the body contradicts its own tables twice (fields 18b, 18c). The third star would require explicit and consistent selection criteria: here they are explicit and inconsistent.
B — Clinical utility★★★★☆Offers a replicable procedure described down to the dose: cytological thresholds for diagnosis, ultrasound-guided arthrocentesis, through and through lavage technique with needle gauge, arthrotomy, regional perfusion with tourniquet duration, and each animal's therapy with molecule, dose per kilogram, route, frequency and duration. The constraint of chapter 14 §11 is respected: B = A + 2, no more. It does not rise to five because it does not change a recommended practice — there is no recommended practice to change — and because in Europe four of the five protocols rest on an EMA category B molecule.
C — Educational value★★★★☆Teaches three things worth remembering: that the CBC can be normal or even leukopenic in overt septic arthritis while fibrinogen is not; that radiography does not rule out bone involvement in the first 7-10 days; and that a negative synovial culture does not clear a joint with 200,000 cells/µL. In addition, for those learning to read articles, it is a textbook case of what you discover when an author publishes their own tables. It does not reach five because it lacks a case-by-case differential diagnosis, which is the educational core of a series.
D — Species relevance★★★☆☆Study entirely on New World camelids, with both species identified animal by animal — alpacas and llamas are not lumped together, and this earns the full third star. It does not rise to four because the grid reserves that level for large or multicenter studies: here there are five animals in a single hospital, in a North American regulatory and pharmacological context, with molecules whose European placement is different (field 18g).

24 · Bottom line

24 · Bottom line

It can be concluded that: septic arthritis exists in neonatal and juvenile New World camelids, and presents via at least two documented routes — neonatal sepsis with failure of transfer of passive immunity (2 cases, both alpacas a few days old) and infection of a single joint in an older animal, without systemic signs and without a wound (3 cases); that in these five animals fibrinogen was elevated in all five while WBC was within the normal range in four out of five; that synovial fluid culture was positive in 2 of 5 cases and in both matched the blood culture; and that 4 animals out of 5 were discharged alive, 3 of whom without lameness.

It cannot be concluded that: 80 % is a transferable survival rate — five animals, with an exact confidence interval calculated by the editors at between 28.4 % and 99.5 % —; that the long-term prognosis is known, because it is documented in two animals and one was still lame at six months; that a negative culture rules out joint sepsis, because in three cases out of five it was negative in clearly septic joints; and that the antimicrobial protocols reported constitute a recommendation — they are off-label uses in five individual animals, declared as such by the authors, and four out of five rest on a molecule that in Europe is classified among those to be restricted.
