◆ ASSOCIATION'S COMMENT
Camelid Research Studies APS — CRL editorial team · 20/09/2026
This is the only card in this issue that an Italian veterinarian must learn to read backwards. All the other four describe something that can walk into their clinic. This one describes a biological cycle that does not close in Europe: Parelaphostrongylus tenuis has the white-tailed deer as its definitive host, which is North American, and terrestrial gastropods as intermediate hosts. Without the first link, the chain never starts. The 9.91% figure for llamas is not an Italian risk, and does not become one by citing it.
So why include it. For three reasons, and none of them is curiosity. (a) Because the clinical literature on camelids is North American, and meningoencephalitis from meningeal worm occupies an enormous space in it. A veterinarian who trains on the available manuals and articles builds a differential list in which, faced with an ataxic llama, P. tenuis ranks high. In Italy it ranks outside, and knowing why it ranks outside is what prevents putting it back in the moment someone names it with confidence. (b) Because the gap it leaves must be filled with something. Chapter 10 §4.1 of the KT states it with the precision needed: in Europe the niche of neurotropic cervid nematodes is occupied by a different genus, Elaphostrongylus; in the 223 German camelid necropsies no case of P. tenuis appears, while encephalitis and meningitis of other kinds do; and no study has ever searched for migrating neural larvae in European camelids. The question is open, not resolved in the negative — and this distinction is the only honest thing that can be said today to a breeder who asks whether their llama is at risk. (c) Because it is a textbook case in how to read a denominator, and this is best learned on a figure that is not frightening.
The number to remember is not the 9.91%: it is the ratio between the species. On the same pool of necropsies, with the same pathologists and the same criteria, camelids stand at 5-10% and small ruminants below 1%. A factor of ten to sixty times, in a table where everything else is held constant. That ratio does not depend on the denominator, because the denominator is the same for all four species; and it coincides with the order of susceptibility observed in experimental infections — llama, alpaca, goat, sheep. This is the kind of result that survives the criticism just leveled at the rest of the article, and it is the reason this card assigns a full three stars on axis A.
The methodological lesson that holds beyond meningeal worm. On one point the authors do something almost no one does: they refuse to call the season of deaths "the season of contagion." Their model finds the peak between October and December; they subtract the known incubation period and conclude that contagion occurred in late summer, and therefore that prevention must be done in July and August, that is, when no one is dying and no one is thinking about the problem. This reasoning — the peak of events is not the peak of the cause — applies equally well to cria coccidiosis, gastrointestinal parasitoses and any disease with a long incubation period. It is transferable to Europe even though the parasite is not.
What changes for a veterinarian in Italy, today. Three things. (a) Faced with a camelid with progressive neurological signs, the question to ask the owner is not "has it eaten snails": it is where the animal comes from, and how long it has been here. The reservoir is North American; an animal imported as an adult is the only way that history could make sense, and it remains remote. (b) The gap left by P. tenuis in the differential list must be filled with what is documented in Europe: listeriosis, polioencephalomalacia, tetanus, copper-deficiency ataxia, trauma, congenital malformations — all covered in chapter 10 §4 with their sources — and with the awareness that Elaphostrongylus in European camelids has never been looked for. (c) When reading a rate, look for the denominator before the number. Here the denominator is «the animals someone paid to have necropsied», which is a population selected for severity, for the animal's value and for the owner's financial means.
What this card cannot tell you. What the prevalence of P. tenuis is in camelids — not even in Minnesota: the authors themselves write that they found no prevalence study in alpacas; — whether a treatment works, because this study evaluated none; and whether an analogous risk exists in Europe, because the question has never been asked of a European camelid. Anyone wishing to ask it, the design is exactly that of this article, applied to an Italian diagnostic laboratory.

3 · Autori
Keane Charlena · Marchetto Katherine M. · Oliveira-Santos Luiz Gustavo R. · Wünschmann Arno · Wolf Tiffany M.
- Department of Veterinary Population Medicine, University of Minnesota, St. Paul, MN, Stati Uniti: tutti e cinque;
- Department of Ecology, BioScience Institute, Federal University of Mato Grosso do Sul, Campo Grande, Brasile: Oliveira-Santos (seconda affiliazione).
3 · Authors
Complete list, five authors, in the order and form of the record:
Keane Charlena · Marchetto Katherine M. · Oliveira-Santos Luiz Gustavo R. · Wünschmann Arno · Wolf Tiffany M.
No ORCID is registered for any of the five authors, neither in Crossref nor in PubMed. The spelling «Wünschmann» with the umlaut is the one in the record: PubMed encodes it as an entity (Wünschmann) and Crossref prints it directly; it is the same name, and the card uses the correct form.
Affiliations for each author, as recorded in PubMed and printed in the full text:
- Department of Veterinary Population Medicine, University of Minnesota, St. Paul, MN, United States: all five;
- Department of Ecology, BioScience Institute, Federal University of Mato Grosso do Sul, Campo Grande, Brazil: Oliveira-Santos (second affiliation).
Wünschmann is a pathologist at the diagnostic laboratory the data comes from: the full text states that «AW, KM and CK collected the data» and thanks «all the diagnosticians and pathologists at the VDL who performed necropsies over the years». This is not a courtesy detail: it means the authors read reports written, largely, by colleagues at their own institution.
4 · Affiliazione dell'autore corrispondente
Marchetto Katherine M. — Department of Veterinary Population Medicine, University of Minnesota, St. Paul, MN, Stati Uniti — posta elettronica [email protected].
4 · Corresponding author affiliation
Marchetto Katherine M. — Department of Veterinary Population Medicine, University of Minnesota, St. Paul, MN, United States — email [email protected].
Stated in full in the full text («*\Correspondence: Katherine M. Marchetto»): the corresponding author is second on the list, not first, and the card records this because a reader who infers correspondence from author order here would be wrong. The correspondence address is personal, not institutional, and is the same one the full text refers to for data access requests (field 21).

9 · Specie e soggetti
|---|---|---|
| Lama (Lama glama) | 212 | 21 |
| Alpaca (Vicugna pacos) | 563 | 30 |
| Capra (Capra aegagrus hircus) | 2.214 | 9 |
| Pecora (Ovis aries) | 2.628 | 4 |
| Totale | 5.617 | 64 |
Sesso (denominatore 64 casi): femmine 53 % (34/64, IC 95 % 41-65 %) — 20 intere, 2 gravide, 12 senza indicazione dello stato riproduttivo; maschi 33 % (21/64, IC 95 % 23-45 %) — 6 interi, 8 castrati, 7 senza indicazione; 9 casi senza sesso registrato. (34 + 21 + 9 = 64.)
9 · Species and subjects
The study covers four species; the card covers the two of interest to this series, and does not hide the other two because they are the term of comparison that gives the result its meaning.
| Species | Total necropsies in the period | Cases attributed to P. tenuis |
|---|---|---|
| Llama (Lama glama) | 212 | 21 |
| Alpaca (Vicugna pacos) | 563 | 30 |
| Goat (Capra aegagrus hircus) | 2,214 | 9 |
| Sheep (Ovis aries) | 2,628 | 4 |
| Total | 5,617 | 64 |
(The sums check out: 212 + 563 + 2,214 + 2,628 = 5,617 and 21 + 30 + 9 + 4 = 64 — editorial verification, not a check stated by the authors.)
What these animals are. They are carcasses voluntarily submitted to the University of Minnesota Veterinary Diagnostic Laboratory between January 2001 and December 2019. They are not a population, not a random sample, and not even all the animals that died: they are the ones someone decided to have necropsied, and paid to have done.
Geographic origin of the 64 cases: Minnesota 51, Wisconsin 11, North Dakota 2 (51 + 11 + 2 = 64, editorial verification).
Sex (denominator 64 cases): females 53% (34/64, 95% CI 41-65%) — 20 intact, 2 pregnant, 12 with no indication of reproductive status; males 33% (21/64, 95% CI 23-45%) — 6 intact, 8 castrated, 7 with no indication; 9 cases with sex not recorded. (34 + 21 + 9 = 64.) No male goat received a diagnosis of P. tenuis in the period.
Age (denominator 51 cases with age reported; 13 without): range 6 months to 18 years; sexually immature 18% (9/51, CI 10-30%), mature 71% (36/51, CI 57-81%), geriatric 10% (5/51, CI 4-21%).
Camelids: 30 of the 39 camelid cases with age reported (77%, CI 62-87%) were sexually mature at the time of the necropsy diagnosis.
Two editorial calculations on the authors' numbers. (a) Camelids account for 51 of the 64 cases (21 llamas + 30 alpacas) and those with age reported are 39: 12 of 51 camelids have no age recorded, against just 1 of the thirteen small ruminants — the 13 cases without age minus the 12 camelids. Age data is therefore missing almost only in camelids, which is consistent with companion animals purchased as adults and without a birth record. (b) ⚠️ The three age classes do not cover the 51 animals. 9 + 36 + 5 = 50, not 51. One animal with age reported falls into no class. The most likely explanation lies in the authors' own definitions, which for camelids set «immature from birth to 1 year» and «mature from 2 to 12 years»: there is no class between 1 and 2 years, and for sheep and goats the same definition creates an overlap at 8 years (mature «2 to 8», geriatric «at 8 years»). This is an editorial hypothesis, not an authors' explanation → [TO BE CHECKED AGAINST THE FULL TEXT]: the per-animal data is in Supplementary Tables 1 and 2, which were not read.
10 · Domanda clinica (PICO)
- C — confronto fra specie (camelidi contro piccoli ruminanti) e fra mesi e anni.
10 · Clinical question (PICO)
This is not a clinical study, and it is one of the cases in which chapter 14 §2.2 allows the PICO not to be forced. It is reported in the corresponding epidemiological form:
- P — llamas, alpacas, goats and sheep submitted for necropsy to a Midwestern U.S. university diagnostic laboratory, 2001-2019.
- E (exposure, in place of intervention) — species membership; month of the year; average summer temperature and cumulative summer precipitation for the year.
- C — comparison between species (camelids versus small ruminants) and between months and years.
- O — mortality attributed to P. tenuis over the pool of necropsy submissions, seasonality within the year, variability between years and temporal trend.
Objective stated by the authors: «to examine patterns of P. tenuis-induced mortality over time and describe variations in the epidemiology of P. tenuis in small ruminants and camelids in Minnesota and the surrounding area» (editorial translation).

11 · Study design
Retrospective observational study on passive surveillance — review of necropsy archives, with Bayesian modeling of seasonality (generalized multilevel mixed circular model). It is not a prevalence study, and the authors do not call it one.
12 · Level of evidence
Low as a design, high as a sample size — and it is precisely this combination that makes this work easy to cite incorrectly.
Consequence for the reader: 5,617 necropsies over 19 years give stable estimates (the confidence intervals are narrow: 6.57-14.67% for the llama), but stable for the wrong thing if the denominator is misread. All this study measures is: among the animals brought for necropsy, what share died of meningeal worm. It does not measure how many become infected, does not measure how many die in the population, and does not measure anything in Europe.
13 · Standard di reporting pertinente
STROBE — studio osservazionale.
13 · Relevant reporting standard
STROBE — observational study. The authors do not state this, and the structure of the article does not follow its order, although it contains most of the substantive items: data source, case classification criteria, handling of missing data (the substitution of the death date), measures of precision (confidence intervals on every proportion), limitations, funding, conflicts.

- confermato = nematode ritrovato oppure tragitti di migrazione osservati istologicamente;
- presunto = nessuna prova diretta, ma lesioni eosinofiliche a distribuzione casuale descritte come encefalite, meningoencefalite, mielite, meningomielite, encefalomielite o meningoencefalomielite;
pdftools, poi verificata manualmente.

- Source: necropsy reports from the University of Minnesota Veterinary Diagnostic Laboratory (MNVDL), January 2001 – December 2019.
- Selection: search of the pathology database with the terms «Parelaphostrongylus tenuis», «P. tenuis» and «meningeal worm» → 93 reports retrieved out of 5,617 total necropsies of alpacas, llamas, goats and sheep.
- Classification of the 93 reports, done manually by reading them one by one:
- confirmed = nematode found or migration tracts observed histologically;
- presumptive = no direct evidence, but randomly distributed eosinophilic lesions described as encephalitis, meningoencephalitis, myelitis, meningomyelitis, encephalomyelitis or meningoencephalomyelitis;
- excluded = clinical suspicion but infection excluded due to absence of compatible histological lesions.
- Data extraction: PDF scraping in an R environment with the
pdftoolspackage, then manually verified. Variables extracted: accession number, year received, date of death, date received, date of sampling, species, breed, sex, age, weight, body condition score, owner's location, reproductive status, clinical signs, and presence of the search terms in the diagnosis and comments sections. - Clinical signs searched for: neurological signs, ataxia, recumbency, tremors, opisthotonus, altered mental status.
- Missing data handled, and stated: when the date of death was missing, the date of carcass submission was used. The authors justify the substitution with a number: in cases where both were available, the mean difference was 1.2 days (standard deviation 1.41).
- Age classes: camelids — immature from birth to 1 year, mature from 2 to 12 years, geriatric at 13 years for llamas and 12 for alpacas; sheep and goats — immature up to 1 year, mature from 2 to 8 years, geriatric at 8 years. (On the gaps in these definitions, see field 9 and field 18b.)
- Descriptive statistics: exact Wilson confidence intervals for proportions, calculated with Epitools; the authors justify the choice by writing that they are appropriate «when p is extreme», and that in this study many proportions were close to zero. Comparison between species: z-test for two proportions. Temporal trend: linear regression model on annual rates.
- Seasonality model: non-linear trigonometric equation f(x) = K + E·cos(xRπ/12), where K is the intercept (annual mean), E the amplitude (strength of seasonality) and R the number of peaks in the year; number of deaths modeled as binomial over the necropsies performed each month; year entered as a random intercept; seasonal environmental covariates (mean temperature and cumulative precipitation) to explain variability between years.
- Estimation:
brmspackage, three Monte Carlo chains, 3,000 iterations each, 1,500 warm-up, convergence verified (R-hat = 1 for all parameters). Non-informative priors for K₀, E, β₁ and β₂; for R a Normal(1, 0.5) prior with a minimum of 0, meaning the model was constrained to expect few peaks. Comparison of the three seasonal models (spring, summer, autumn) using WAIC. - Weather data: National Weather Service Forecast Office, for the State of Minnesota; seasons defined by solstices and equinoxes — spring April-June, summer July-September, autumn October-December.
- Ethical approval: stated as not required, because the data are necropsy reports already collected; owner informed consent not obtained, as this is a retrospective analysis.
15 · Risultati (dati)
15.1 · Mortalità per specie (Tabella 1 dell'articolo, tradotta)
| Specie | Casi / necroscopie | Tasso | IC 95 % |
|---|---|---|---|
| Lama | 21/212 | 9,91 % | 6,57-14,67 % |
| Alpaca | 30/563 | 5,33 % | 3,76-7,50 % |
| Capra | 9/2.214 | 0,41 % | 0,21-0,77 % |
| Pecora | 4/2.628 | 0,15 % | 0,06-0,39 % |
| Totale | 64/5.617 | 1,14 % | 0,89-1,45 % |
- Tendenza in aumento, statisticamente significativa: p = 0,043, β = 0,0008 (regressione lineare sui tassi annui).

- in un anno di alta incidenza, fino al 22 % di un gregge di capre può infettarsi, con 44 % di mortalità fra gli infetti non trattati;
- mandrie di lama hanno avuto 36 % di infezione in un anno di alta incidenza, con 75 % di mortalità fra gli infetti;
- prevalenza di mandria riportata in letteratura: lama 36 %, pecore dal 2 % al 59 %, capre dallo 0,5 al 22 %; negli alpaca gli autori scrivono di non aver trovato alcuno studio di prevalenza;
- incubazione: 28-60 giorni secondo la letteratura, 4-71 giorni negli animali infettati sperimentalmente;
- suscettibilità sperimentale in ordine decrescente: lama, alpaca, capra, pecora;
15 · Results (data)
15.1 · Mortality by species (Table 1 of the article, translated)
| Species | Cases / necropsies | Rate | 95% CI |
|---|---|---|---|
| Llama | 21/212 | 9.91% | 6.57-14.67% |
| Alpaca | 30/563 | 5.33% | 3.76-7.50% |
| Goat | 9/2,214 | 0.41% | 0.21-0.77% |
| Sheep | 4/2,628 | 0.15% | 0.06-0.39% |
| Total | 64/5,617 | 1.14% | 0.89-1.45% |
Llama and alpaca rates are significantly higher than those of sheep and goats (p < 0.001, z-test for two proportions).
Reading of the intervals alone, done by the editors. The llama (6.57-14.67%) and alpaca (3.76-7.50%) intervals do not overlap: between the two camelid species the difference is not an impression. But the authors did not test llama against alpaca — they tested camelids against small ruminants — and the non-overlap of two intervals is not a hypothesis test. The defensible formulation is: among the animals submitted, llamas were affected in a greater proportion than alpacas, and the difference is compatible with the greater experimental susceptibility of the llama reported in the literature (field 16).
15.2 · Cases, selection and trend
- 93 reports retrieved by the search; 64 confirmed on a histological basis.
- Mean of 3.4 animals diagnosed per year, for a mean annual rate of 1.21% over the period. (3.4 × 19 years = 64.6 ≈ 64: editorial verification.)
- The annual rate ranged from 0% (2002 and 2013) to 3.9% (2016).
- Increasing trend, statistically significant: p = 0.043, β = 0.0008 (linear regression on annual rates).
- On 5 occasions the same owner submitted 2 positive cases in the same year; otherwise the cases were treated as independent events.
15.3 · Seasonality
- Observed monthly rates: from 0 to 3%.
- Lowest: March-April (mean 0.59%) and late summer, August-September (mean 0.23%).
- Highest: autumn-winter, October-February (mean 2.03%), followed by July (1.37%).
- 66% of the observed deaths were diagnosed between October and February.
Bayesian model results (model with summer covariates, the most plausible: ΔWAIC = 4.9 compared with the spring and autumn ones):
| Parameter | Estimate | 95% credible interval |
|---|---|---|
| Mean annual mortality rate (K₀) | 1.15% | 0.09-2.35% |
| Number of peaks in the year (R) | 1.08 | 0.92-1.23 |
| Strength of seasonality (E) | 0.75 | 0.32-1.17 |
| Ratio between the peak (October-December, ~1.62%) and the trough (May-June, 0.048%) | 3.95 times | 3.34-4.95 |
| Share of monthly variability attributable to variability between years (ICC) | 0.10 | 0.003-0.31 |
| Effect of mean summer temperature (β₁) | 0.17 | 0.05-0.30 |
| Effect of cumulative summer precipitation (β₂) | 0.04 | −0.13 – 0.20 |
Only one seasonal peak, then, and rainfall explains nothing: the interval for β₂ crosses zero, that for β₁ does not. The abstract reports the extremes of the two related variables: summer temperature from 20.4 to 22.4 °C, mortality rates from 0 to 3.9%. Correlation between temperature and rainfall within each season: summer r = 0.23, spring r = −0.30, autumn r = 0.04 — that is, the two covariates are not collinear.
15.4 · Contextual figures cited by the authors, with their stated source
These are not results of this study, and the card keeps them separate for that reason:
- in a high-incidence year, up to 22% of a herd of goats can become infected, with 44% mortality among untreated infected animals;
- llama herds have had 36% infection in a high-incidence year, with 75% mortality among infected animals;
- herd prevalence reported in the literature: llama 36%, sheep from 2% to 59%, goats from 0.5 to 22%; for alpacas the authors write that they found no prevalence study;
- incubation: 28-60 days according to the literature, 4-71 days in experimentally infected animals;
- experimental susceptibility in decreasing order: llama, alpaca, goat, sheep;
- summer temperatures in the study area increased by 1 °C between 1951 and 2012.
16 · Interpretazione degli autori
Attribuita, separata dai numeri.
16 · Authors' interpretation
Attributed, kept separate from the figures.
- On seasonality and what it means. The authors are explicit on a point that almost all summaries miss: the model describes the time of death, not the time of infection. «We are modeling temporal changes in the probability of attributing a diagnosis of P. tenuis to mortality», they write, and from there, applying the known incubation period, they estimate that the animals became infected approximately two months before dying, that is between late summer and early autumn. The practical consequence they draw is that preventive measures should be taken in mid-to-late summer, not in autumn when deaths begin.
- On temperature. They find a positive association with summer temperature and none with precipitation, and present this as a result against the current expectation, which favors humidity because of the role of gastropods. They propose three alternative explanations, without choosing one: that heat accelerates larval development inside the gastropod (observed in P. odocoilei); that mollusk estivation slows but does not prevent larval development; and that heat changes husbandry management — different pastures, different feeding, different contact with intermediate hosts.
- On climate change. «A positive association between summer temperature and P. tenuis-induced mortality in livestock is a concerning trend in the era of climate change»: if the observed trends continue, the authors expect an increase in deaths.
- On differences between species. They attribute the higher mortality of llamas and alpacas to the greater susceptibility documented experimentally, but immediately list the factors that complicate the comparison: different likelihood that a species is submitted for necropsy, different grazing behavior, different management. And they write, verbatim, that observational herd prevalence has never been compared across the four species in a single study.
- On the denominator's limitation. They say it themselves first: the necropsy data do not include animals that were treated and survived, nor those that died and were not submitted; and at least four reports described multiple animals from the same herd with clinical signs, of which only one was submitted.
- On prevention. They mention taller fencing to keep white-tailed deer out and co-grazing with poultry as a possible control of gastropods; and they note that research has begun to sequence P. tenuis, which could lead to a vaccine or a better diagnostic test.
17 · Limiti dichiarati dagli autori
Citati, non riassunti:
- «La sorveglianza passiva e la dimensione campionaria relativamente piccola sono un limite di questo studio»;
- il modello descrive la probabilità di attribuire la diagnosi alla morte, non il momento dell'infezione (campo 16);
- i dati necroscopici escludono per costruzione gli animali trattati e sopravvissuti e quelli non conferiti;
- il confronto fra prevalenza di mandria e proporzione necroscopica è difficile per più ragioni, che elencano;
17 · Limitations stated by the authors
Quoted, not summarized:
- «Passive surveillance and the relatively small sample size are a limitation of this study»;
- the model describes the probability of attributing the diagnosis to death, not the time of infection (field 16);
- the necropsy data by construction exclude animals that were treated and survived and those not submitted;
- the comparison between herd prevalence and necropsy proportion is difficult for several reasons, which they list;
- the relationship between mortality and summer temperature could reflect management changes driven by climate, not a direct biological effect.
This is an honest and complete list. It does not contain the point raised in field 18a, which concerns an internal discrepancy.
18 · Limitations added by the editors
(a) The methods state one denominator, the results use another. The methods section states: «The rate of P. tenuis-induced mortality was estimated by analyzing the total number of confirmed and presumptive cases over the total number of submissions for each species». The results, however, report 64, which is the number of confirmed cases only («of the 93 cases … 64 were confirmed infections based on histological evidence»), and Table 1 sums exactly to
- The 29 presumptive cases enter no published rate, and their fate is no longer mentioned in the article. This is not a calculation error: it is a methods sentence describing a different analysis from the one reported. The consequence is measurable: if presumptive cases were included, the overall rate would go from 64/5,617 (1.14%) to 93/5,617 (1.66%) (editorial calculation, and only an upper bound: some of the 93 may have been classified as "no evidence" rather than "presumptive," because the article does not give the breakdown between the two categories) → [TO BE CHECKED AGAINST THE FULL TEXT]: the number of presumptive cases does not appear anywhere in the text read.
(b) The age classes have a gap and an overlap, and one animal falls into it. As in field 9, the three stated classes cover 50 of the 51 animals with age reported, and the definitions leave the 1-2 year interval uncovered for all species, while for sheep and goats 8 years is simultaneously "mature" and "geriatric". In a work that is otherwise precise, this is a definitional oversight, not a measurement one.
(c) "Confirmed" and "presumptive" do not have the same probability of being found over time. A confirmed diagnosis requires finding the nematode or the migration tracts, which depends on how many histological sections are cut and how much is looked for. Over nineteen years, different pathologists, different workloads and different protocols may have shifted that threshold. The significant increasing trend (p = 0.043, β = 0.0008) is compatible both with a real increase and with an increase in diagnostic accuracy or in the propensity to submit, and the article has no way to separate them. The authors do not state this; the editors flag it because the p-value is very close to the conventional threshold and the climate conclusion also rests on it.
(d) The denominator's composition changes over time, and no one controls for it. The number of alpacas raised in the United States grew enormously between 2001 and 2019, and that of llamas declined. A rate calculated over the pool of necropsies is affected by this shift: more alpacas submitted in recent years means a different denominator, not just a larger one. The article does not report the species composition year by year → [TO BE CHECKED AGAINST THE FULL TEXT].
(e) Five double submissions from the same owner are not five independent events. The authors state this and then treat them as independent, which is a defensible choice over 64 cases, but one that should be weighed: herd infections are not spatially random, and the model has no herd term.
(f) In Europe this article measures nothing, and this is not a flaw of the article. This is the most important point of this card and is developed in field 22. P. tenuis requires the white-tailed deer (Odocoileus virginianus), a North American species, as its definitive host: the cycle this study describes does not exist in the European range. The card records this fact here because an Italian reader who took the 9.91% as "llama risk" would make two overlapping errors — a wrong denominator and a wrong continent.
(g) The two supplementary tables and the supplementary figure were not read. The breakdown by sex, by age class and the posterior distributions of the parameters are there → [TO BE CHECKED AGAINST THE FULL TEXT].
19 · Finanziamento

Stated, and of a nature relevant to the reading. Full text: «Funding for this project was provided by the Minnesota Invasive Terrestrial Plants and Pests Center through the Environment and Natural Resources Trust Fund, as recommended by the Legislative-Citizen Commission on Minnesota Resources (LCCMR)». Crossref records the same funder, with no grant number.
The editors note, without attributing any bias, that the funder is a center for invasive terrestrial species and pests: it is consistent that the research question was framed in terms of intermediate hosts, climate and environmental risk rather than in clinical terms. This is not a conflict: it is the reason this work looks at gastropods and temperature rather than at therapy.

21 · Cosa manca rispetto allo standard di reporting
Rispetto a STROBE, letto il testo pieno:
- coerenza fra metodi e risultati sulla definizione del caso (voci 7 e 12): violata — è il rilievo del campo 18a, ed è l'unica mancanza sostanziale del lavoro;
- numero dei casi presunti: mai riportato, benché la categoria sia definita nei metodi;
- flusso dei partecipanti (voce 13): parziale. Si sa che 93 referti sono stati recuperati e 64 confermati; non si sa quanti siano stati classificati «presunti» e quanti «nessuna evidenza»;
- variabili raccolte ma mai riportate (voce 7): peso, punteggio di condizione corporea, razza, segni clinici sono elencati fra i dati estratti e non compaiono in nessun risultato;
- dati mancanti (voce 12c): gestiti bene — la sostituzione della data di morte è dichiarata e quantificata (1,2 ± 1,41 giorni);
- misure di precisione (voce 16): presenti ovunque, con intervalli di confidenza su ogni proporzione e intervalli di credibilità su ogni parametro del modello;
- disponibilità dei dati: dichiarata limitata — «i dati riguardano informazioni riservate dei proprietari», con richieste da indirizzare all'autore corrispondente;
- finanziamento e conflitti (voce 22): presenti.
21 · What is missing relative to the reporting standard
Against STROBE, having read the full text:
- consistency between methods and results on the case definition (items 7 and 12): violated — this is the point raised in field 18a, and it is the only substantial shortcoming of the work;
- number of presumptive cases: never reported, although the category is defined in the methods;
- participant flow (item 13): partial. It is known that 93 reports were retrieved and 64 confirmed; it is not known how many were classified "presumptive" and how many "no evidence";
- variables collected but never reported (item 7): weight, body condition score, breed, clinical signs are listed among the extracted data and do not appear in any result;
- missing data (item 12c): handled well — the substitution of the death date is stated and quantified (1.2 ± 1.41 days);
- measures of precision (item 16): present everywhere, with confidence intervals on every proportion and credible intervals on every model parameter;
- data availability: stated as limited — «the data concern confidential owner information», with requests to be directed to the corresponding author;
- funding and conflicts (item 22): present.
Overall this is one of the best-reported works in this issue. The discrepancy in field 18a remains, and should be flagged precisely because everything else holds up.
23 · Valutazione a stelle
23 · Star rating
Four independent axes, criteria from chapter 14 §11. Full text read; supplementary material not read.
| Axis | Rating | Rationale, with one fact inside |
|---|---|---|
| A — Scientific quality | ★★★☆☆ | Observational study with a stated and verifiable sample size (the Table 1 sums check out to 5,617 and 64), explicit three-level case classification criteria, confidence intervals on every proportion, missing data handling stated and quantified (1.2 ± 1.41 days), Bayesian modeling with verified convergence (R-hat = 1) and model comparison via WAIC. It does not reach four because it is neither a controlled nor an experimental study, and because the methods state that rates are calculated on «confirmed and presumptive» cases while the results use only confirmed cases (field 18a): a discrepancy concerning the very definition of the primary outcome. |
| B — Clinical utility | ★★☆☆☆ | In Europe there is nothing to do with this result, because the cycle does not close: direct actionability is nil. What remains is the value of a hypothesis to verify and of a precise history-taking gesture (the animal's origin and length of residence), plus the transferable principle "the peak of deaths is not the peak of contagion." For a North American reader this same axis would be worth four stars: the card is valued for the reader this series has, and that is a European veterinarian. |
| C — Educational value | ★★★★☆ | It teaches three things that stick: that a rate calculated over the pool of necropsies is not a prevalence; that the peak of events is not the peak of the cause, with the incubation calculation done in front of the reader; and that a gradient of susceptibility between species holds even when the absolute denominator does not, because it is the same for all. It does not reach five because it is not the reference work on camelid neurology: it is the reference on one disease that is not present here. |
| D — Relevance to the species | ★★☆☆☆ | Data on real camelids, both species, with a respectable sample size — but in an epidemiological context that does not exist in Europe, because the definitive host is missing. It is exactly the level that the grid in chapter 14 §11 describes as "data on camelids in a context very distant from the European one." The two stars are not a judgment on the work, which is good: they are the measure of the distance between where it was done and where it is being read. |
24 · Bottom line
24 · Bottom line
It can be concluded that: among the 5,617 alpacas, llamas, goats and sheep submitted to a Minnesota diagnostic laboratory over nineteen years, 64 died from histologically confirmed P. tenuis infection (1.14%); that the share was much higher in camelids — llama 21/212 (9.91%), alpaca 30/563 (5.33%) — than in small ruminants (both below 1%, p < 0.001), in an order consistent with the susceptibility observed in experimental infections; that deaths show a single seasonal peak, between October and December, with probability 3.95 times higher than the minimum in May-June; and that year-to-year variability is associated with mean summer temperature (β = 0.17, CI 0.05-0.30) and not with precipitation (CI from −0.13 to 0.20).
It cannot be concluded that: one in ten llamas dies of meningeal worm — the denominator is the pool of necropsies, not a population, and the authors state this themselves; that the prevalence of infection is known, because no prevalence study exists for alpacas; that the increase over time is certainly real, because it is compatible also with an increase in diagnostic accuracy or in the propensity to submit (p = 0.043); and above all that these figures describe a European risk: P. tenuis requires the white-tailed deer, which does not exist in Europe. That the European risk is zero cannot be concluded either, however: no study has ever searched for migrating neural larvae in European camelids, and the question remains open, not resolved in the negative.
