Camelid Research Studies APS — redazione CRL · 20/09/2026

Ogni prescrizione è in deroga.

Quel numero è calcolato in milligrammi.

Quattro cose.

Association commentary

◆ ASSOCIATION COMMENTARY

Camelid Research Studies APS — CRL editorial team · 20/09/2026

This fact sheet closes the dossier because it is the only one that answers the question an Italian veterinarian asks first: what will come into my clinic. The other four each describe one anatomical district. This one lines up fifteen years and 634 animals in a university clinic four hundred kilometers from the Italian border, with European husbandry, climate and regulatory framework. The ranking that emerges — digestive system, castrations, foot and limbs, crias, skin — is not a population prevalence (it is a referral clinic, and the authors say so first), but it is the closest thing to a map that exists in Europe today.

The first surprising thing is where dental pathology sits. It is the leading cause of antibiotic use in the entire caseload, and it arrives disguised: 38 of the 71 digestive-system patients were oral cases, with root abscesses and osteomyelitis, and the cardinal symptom was local swelling — not pain, not weight loss, not difficulty chewing. The authors say it in one line that is worth the whole paper: pain was not generally the main reason for the visit. An alpaca with a dental root abscess does not behave like a dog with an abscess: it compensates, and arrives when the mandible is already deformed. In 23 of 71 patients, extraction was necessary.

The second thing is a number one does not forget: 28 of 44 crias died or were euthanized, 63.6%. In a European university clinic, with broad-spectrum antibiotics available. It is the same world as fact sheet CRL-J-02, where the septic cria with arthritis is the only one of the five animals not to leave alive. The operational conclusion is not therapeutic, it is temporal: in the camelid cria, what decides the outcome happens before admission — colostrum, immunocrit, umbilicus, the first twelve hours. By the time the cria reaches the clinic, the game is largely already played.

The third thing is the reason this fact sheet was chosen, and it concerns antibiotics. The article states something many in Italy do not know, or prefer not to say: no antibiotic is authorized for South American camelids in the European Union. Every prescription is off-label. On that ground, the EMA classification is not a formal requirement, it is the only ordering criterion available — and this paper applies it to itself with rare honesty: it writes that retrospectively the use of a fourth-generation cephalosporin may not have been justified in all cases of suspected enterotoxemia. A group that publishes its own caseload and then judges it severely deserves to be read in full.

And here the editorial team adds a calculation the article does not make, because it changes the conversation. The authors report that Category B weighed at most 11.1% of annual use. That figure is calculated in milligrams. Counting therapeutic acts instead, Category B applications are 465 of 1,466: 31.7%, nearly one in three; and by number of treatment courses the most-used molecule in the entire caseload is cefquinome, a fourth-generation cephalosporin, with 83 of 405 courses. The two figures do not contradict each other: cephalosporins are dosed in milligrams much lower than penicillins and tetracyclines, so the same prescribing behavior appears marginal when weighed and central when counted. Which of the two measures is the right one depends on what one wants to monitor, and the honest answer is that both are needed: mass for environmental pressure, acts for clinical habit. Anyone citing the 11.1% without saying "in milligrams" is giving a reassurance the data does not warrant.

What changes for a veterinarian in Italy, today. Four things. (a) Look at the mouth at every visit. The leading cause of antibiotic use in a European camelid clinic is dental disease, and it presents as swelling, not pain. (b) An anemic camelid should be worked up for Candidatus Mycoplasma haemolamae and for parasites, together. In this caseload 19 of 27 patients in the cardiovascular group were treated for suspected CMhl, 13 of 19 confirmed in the laboratory, and every patient with CMhl had a concurrent internal parasitosis. The same holds for enterotoxemia: all suspected cases were positive for endoparasites. Fecal examination is not an accessory: it is part of the differential. (c) Perform culture, and do so especially when about to use a Category B agent. This is the recommendation the authors repeat twice, and it is the one that makes an off-label prescription defensible. (d) Choose the route with the species in mind. Of 1,466 applications, only one was oral: the oral bioavailability of antibiotics in camelids is not known, and the same argument is used by the authors of CRL-J-02 to refuse to interpret the breakpoints of oral drugs. It is not a matter of school preference: it is an absence of data.

What this fact sheet cannot tell you. Which antibiotic works, for which disease, at what dose. This study measures the efficacy of nothing, and the table of molecules is an inventory of what was used, not a ranking of what worked. Nor can it tell you how common these diseases are in Italian alpacas: the denominator is an Austrian referral clinic, and for referred cases severity is selected upward. For doses, the reference remains chapter 4 of the KT (pharmacology) and the primary pharmacokinetic sources for the species; for responsibility for off-label use, the current Italian regulation and the prescriber.

  • Unit of Veterinary Public Health & Epidemiology, University of Veterinary Medicine, Vienna, Austria: Käsbohrer, Firth.

3 · Authors

Complete list, five authors, in the order and form of the record:

Hund Alexandra · Wittek Thomas · Selan Ursa · Käsbohrer Annemarie · Firth Clair L.

No ORCID is registered for any of the five authors, either in Crossref or in PubMed.

  • Unit of Veterinary Public Health & Epidemiology, University of Veterinary Medicine, Vienna, Austria: Käsbohrer, Firth.

The composition of the group explains the structure of the article: three ruminant clinicians who bring the cases, two veterinary public health epidemiologists who bring the reading grid — the EMA classification of antibiotics by importance in human medicine. This is not a therapy paper: it is an antimicrobial use paper that uses the cases as material.

9 · Specie e soggetti

Sesso (denominatore: i casi): lama — 89 maschi interi, 15 castrati, 34 femmine (1 caso senza sesso registrato); alpaca — 78 maschi interi, 25 castrati, 94 femmine (7 casi senza sesso). (Somme della redazione: 89 + 15 + 34 + 1 = 139 casi di lama; 78 + 25 + 94 + 7 = 204 casi di alpaca. Tornano entrambe.)

9 · Species and subjects

634 South American camelids admitted to the University Clinic for Ruminants (UCR) of Vetmeduni Vienna between January 1, 2005 and December 31, 2019. Of these, the study analyzes only those treated with antibiotics:

PatientsClinical cases
Alpaca202204
Llama136139
Total treated with antibiotics338343

(Editorial sums. The difference between patients and cases is explained by the authors: five animals were patients twice over the fifteen years — two alpacas and three llamas, per the same arithmetic.)

Editorial calculation: 338 of 634 patients (53.3%) received antibiotics. This figure is not written in the article, which reports the two numbers in two different sentences, and must be read with caution: the 634 is the total number of camelids admitted, the 338 those with diagnoses linked to an antibiotic treatment, which is the stated inclusion criterion. It is therefore not a prescription rate in the strict sense, and is an upper bound: an animal may have been admitted for one reason and received the antibiotic for another.

Trend over time: from 2 to 21 llamas treated per year; the first alpaca requiring antibiotics was treated in 2008, and the maximum number of alpacas — 33 — in 2019. The caseload thus shifts, over the fifteen years, from a llama clinic to an alpaca clinic.

Sex (denominator: the cases): llama — 89 intact males, 15 castrated, 34 females (1 case with sex not recorded); alpaca — 78 intact males, 25 castrated, 94 females (7 cases without sex). (Editorial sums: 89 + 15 + 34 + 1 = 139 llama cases; 78 + 25 + 94 + 7 = 204 alpaca cases. Both check out.)

Editorial reading, which the article does not make. Among llamas, 104 of 138 cases with known sex are males (75.4%); among alpacas, 103 of 197 (52.3%). A difference of this magnitude is not explained in the article, and the two simplest explanations are both managerial: castration — the second leading cause of antibiotic use in the whole caseload — and the different sex composition of the two types of herds in Austria. This is not a risk finding.

Age: alpaca from 1 day to 16 years, mean 38.9 months (± 35.7); llama from 2 days to 18 years, mean 59.8 months (± 51.5). Standard deviations nearly equal to the means indicate highly skewed distributions: a caseload with many young animals and a tail of elderly ones.

The authors are explicit about their own genre: «our retrospective data analysis cannot — and does not aim to — provide definitive evidence in the form of a randomized controlled clinical trial; however, it does demonstrate the practices of the specialists at the UCR over an extended period».

12 · Level of evidence

Low as a design, but the question it answers does not require a higher one.

Consequence for the reader: this study does not demonstrate that an antibiotic works, and does not claim to. It documents what was used, for what, how much and by which route, in a European referral clinic, for fifteen years. For a species in which no antibiotic is registered in the European Union — the article itself says so — this is a kind of information that has no substitutes: the only alternative is opinion.

13 · Standard di reporting pertinente

STROBE — studio osservazionale.

13 · Relevant reporting standard

STROBE — observational study. The authors do not declare it. However, the article contains the substantive items: data source, definitions, period, inclusion criterion, declared missing data (cases with sex not recorded), limitations discussed candidly, ethical, funding, conflict-of-interest and data-availability statements.

    • somministrazione (application): «la somministrazione di una singola dose a un animale»;
    • ciclo di terapia (treatment course): un animale può essere stato trattato più volte, e ciascuna volta è un ciclo distinto;
    • paziente: un animale può essere contato più volte in categorie diverse se ha ricevuto più di un antibiotico.

  • Source: UCR's animal hospital information system (AHIS); information on camelids extracted and exported to a spreadsheet for the period 01/01/2005 – 31/12/2019 (fifteen years).
  • Inclusion criterion: only animals with diagnoses related to antibiotic treatments. Camelids admitted for other reasons are not included in the analysis.
  • Diagnosis coding: GMON system (Gesundheitsmonitoring), part of the Austrian national health monitoring system for cattle, with additional categories created for diseases common in camelids. Each animal has a primary diagnosis and, in some cases, a secondary one. The "cria diseases" category includes all animals up to 6 months of age.
  • Antibiotic classification: EMA categories A to D, reported in full in Table 1 of the article (field 15.1).
  • Accounting definitions, and there are three different ones — this is the key to not getting the denominators wrong:
    • application: «the administration of a single dose to an animal»;
    • treatment course: an animal may have been treated more than once, and each time is a separate course;
    • patient: an animal may be counted more than once in different categories if it received more than one antibiotic.
  • Analysis: descriptive, in R, with the packages dplyr, ggplot2 and reshape2. No inferential statistics, no tests, no confidence intervals — consistent with a paper that describes a practice and does not compare hypotheses.
  • Ethical approval: declared not required, because the analysis was conducted in 2023 on data from already-existing patients (2005-2019), with owner consent obtained at the time of admission for treatment.

Ogni animale ricevette in media 2,1 (± 1,0) somministrazioni.

Piede e arti — 52 cicli in 33 pazienti.

Altre diagnosi: sistema nervoso centrale n = 5, organi di senso n = 3, infezioni ombelicali n = 3, apparato digerente n = 4, apparato respiratorio n = 3, cuore/circolo/sangue n = 3. ⚠️ Di tutti i 44 cria, 28 (63,6 %) furono soppressi o morirono.

Apparato tegumentario — 32 cicli in 25 animali.

Cuore, circolo e sangue — 31 cicli in 27 pazienti.

Sistema nervoso centrale — 29 cicli in 21 pazienti.

⚠️ Table 1 does not list two molecules that Table 2 records as used: ofloxacin (1 application) and chlortetracycline (2 applications). Ofloxacin is a fluoroquinolone and would fall into Category B; chlortetracycline is a tetracycline and would fall into Category D. This is a completeness oversight in the table, not a substantive error, and the editorial team flags it because it affects — very slightly — the Category B tallies (field 15.3).

(The editorial team summed the column: exactly 405. The applications column of Table 2 also sums exactly to 1,466. Both tables balance, which in a thirteen-column table is not a given.)

Detail of the main categories, as reported in the text:

Digestive system — 82 courses, the leading cause. About half of the patients treated in this category were treated for oral disease: 38 of 71 patients (53.5%). Of these, one had a cleft palate and all the others were cases of dental disease. The cardinal symptom was local swelling, and among the most common lesions dental root abscesses and osteomyelitis. In 23 patients (32.3%), surgical extraction of the affected teeth was performed; in 8 further dental patients the reason for the antibiotic was fistulated abscesses. In addition: 5 animals for esophageal diseases and 3 for other diseases of the abdominal cavity (2 rectal prolapses, 1 diaphragmatic hernia). Enterotoxemia from Clostridium perfringens: 25 patients treated on a presumptive or confirmed diagnosis. The presumptive diagnosis was based on rapid onset with gastrointestinal symptoms (colic, diarrhea, gaseous bloat) or neurological ones (seizures, depressed mentation, recumbency). 5 patients tested positive for C. perfringens type A toxin. In all of these, a fecal sample was collected for parasitological examination, and all samples tested positive for endoparasites, of varying type and burden.

Castrations — 79 courses, the second leading cause. Age of castrated animals: from 12 to 144 months, mean 31.41 months. Each animal received on average 2.1 (± 1.0) applications. Three animals received the combined product benzylpenicillin + streptomycin plus a second antibiotic.

Foot and limbs — 52 courses in 33 patients. One animal recumbent for traumatic gonitis; 32 patients with "fractures, wounds and other limb lesions." Of these, 11 (34.4%) had foot or limb wounds that were infected or had caused septic arthritis of various joints. In 2 patients, digits were amputated proximal to the middle phalanges due to purulent inflammation and osteomyelitis; 1 was treated for traumatic exungulation. In cases with joint involvement (n = 8), ampicillin was often used; gentamicin was used in infections with suspected Gram-negatives, for example in open fractures (n = 8). Ten animals received more than one antibiotic, and in 4 the combination was gentamicin + a penicillin. Five patients in this group were euthanized due to poor prognosis.

Cria diseases — 48 courses in 44 crias up to 6 months (4 animals treated with two molecules). The most frequent diagnosis was "other cria diseases," 16 animals (36.4%), of which 12 (75.0%) neonates under one month with weakness and/or generalized infections. Other diagnoses: central nervous system n = 5, sense organs n = 3, umbilical infections n = 3, digestive system n = 4, respiratory system n = 3, heart/circulation/blood n = 3. ⚠️ Of all 44 crias, 28 (63.6%) were euthanized or died.

Integumentary system — 32 courses in 25 animals. 8 patients (32.0%) for wounds or trauma; 9 (36.0%) for bacterial infections secondary to parasitosis or other skin infections; 8 (32.0%) classified as "other skin diseases," of which 5 abscesses and 2 infections secondary to zinc-responsive dermatosis. Among the cases with cutaneous parasitosis, 4 animals also had a secondary diagnosis of zinc-responsive dermatosis. Six animals received more than one molecule.

Heart, circulation and blood — 31 courses in 27 patients. 15 (55.6%) with septicemia or anemia; 1 with endocarditis, euthanized. 19 of 27 (70.4%) were treated for suspected Candidatus Mycoplasma haemolamae infection, as primary or secondary diagnosis; confirmed in the laboratory in 68.4% (13/19). Ten of the 19 were treated with oxytetracycline, seven with an aminopenicillin. Thirteen of the 19 improved and were discharged to continue therapy at home: 7 of these had been treated with oxytetracycline and 3 with aminopenicillins.

Central nervous system — 29 courses in 21 patients. 5 (23.8%) for CNS symptoms and 17 (81.0%) for diseases of the sense organs: eyes affected in 12 animals, ears in 5, with diagnoses of keratoconjunctivitis, corneal ulcers and otitis. Five patients with eye disease underwent enucleation. (The sum 5 + 17 = 22 exceeds the 21 patients, and the two percentages sum to 104.8%: the categories overlap by one patient, because the count is by primary and secondary diagnosis. Editorial observation.)

The calculation the article does not make, and it is the one that changes the reading (editorial calculation). Summing Category B applications — cefquinome 347, ceftiofur 73, enrofloxacin 34, marbofloxacin 10, ofloxacin 1 — gives 465 applications out of 1,466, i.e. 31.7%. Nearly one application in three, over fifteen years, was of an antibiotic that European guidelines classify as critical for human medicine. The article instead reports that Category B ranged from 0% (2005 and 2006) to a maximum of 11.1% (2015). The two figures do not contradict each other: they measure different things. The article's percentage is calculated on the milligrams administered each year; the editorial team's is calculated on the number of applications over the whole period. Because cephalosporins are dosed in milligrams per kilogram much lower than penicillins and tetracyclines, the same usage appears small when weighted in milligrams and large when counted in therapeutic acts. Which of the two measures matters depends on what one wants to monitor: selective pressure on the environment has more to do with mass, selective pressure on the individual patient and prescribing habit with the number of acts. Anyone citing this article should say which of the two they are citing.

Two further findings over time, as the article states them: in the early years the most-used molecule by number of applications was the combined product benzylpenicillin + dihydrostreptomycin (Category C), mainly as perioperative prophylaxis for castration and in orthopedic cases; 3rd and 4th generation cephalosporins (Category B) come into use from 2010, mainly for digestive system and cria diseases; and from 2017 onward Category D antibiotics exceed 50% of annual use in milligrams. By number of treatment courses, the single most-used molecule overall is cefquinome (83 of 405 courses).

⚠️ Two printing errors in Table 3, identified by the editorial team and verifiable by anyone. The absolute frequencies sum exactly to 1,466; the printed percentages sum to 104.7%, which is impossible. The two misplaced values are those for the intraocular route (7/1,466 = 0.5%, printed as 4.5%) and the oral route (1/1,466 = 0.07%, printed as 0.7%): in both cases the figure appears to be shifted by one decimal place. The substantive finding does not change — nearly 70% of applications are intramuscular — but anyone copying the intraocular row would carry forward a number nine times larger than the true one.

The clinical reading of the table, which the article does not spell out: of 1,466 applications, only one was by the oral route. In a species with a three-compartment stomach and with unknown oral bioavailability of antibiotics — the same reason the authors of CRL-J-02 give for not interpreting the breakpoints of oral antibiotics — the parenteral route is not a preference: it is almost the only one practiced.

16 · Interpretazione degli autori

Attribuita, e separata dai numeri.

16 · Authors' interpretation

Attributed, and kept separate from the numbers. The authors say six things, and at least three are uncomfortable:

  1. The regulatory context, first: «to our knowledge, no antibiotic substance has been officially authorized for use in South American camelids in the European Union», and the published material on the subject «is considered only a basic guideline». Hence: «it is often very difficult for veterinarians in the field to choose and administer the correct antibiotic».
  2. Some of the choices would not be defensible today, and they write it: the analysis covers treatments from almost twenty years ago, and the WHO published its first list of critically important antimicrobials precisely in 2005, the OIE its own in 2007, and the Austrian guidelines only in 2013. «For these historical reasons, the choice of antibiotics used between 2005 and 2019 may not always be justifiable today.»
  3. On enterotoxemia, the judgment is explicit: most animals with suspected enterotoxemia were treated with cefquinome, and «retrospectively, this use of a fourth-generation cephalosporin may not have been justified in all cases». They cite a Swiss recommendation indicating benzylpenicillin administered subcutaneously as the first choice in these cases (field 21).
  4. Why Category B has not decreased: due to the severity of the cases, the fact that the clinic is a referral one and receives camelids when disease is already chronic, and because «despite treatment, 64% of crias died or were euthanized». They add that with critical patients a decision must be made immediately, and broad-spectrum coverage «often seems the best option until bacteriological culture results are available».
  5. On dental disease: the etiology «has not yet been established», and common observations suggest a combination of genetic predisposition, diet and management factors. They note — citing previous work — that camelids with dental disease hide symptoms relatively well and that pain was not generally the main reason for the visit; and that culture in lesions with severe osteolytic changes has «questionable validity».
  6. On parasitic comorbidity: every patient diagnosed with Candidatus M. haemolamae had one or more concurrent internal parasitoses, which «appears to have an impact on the severity of symptoms»; and all patients with suspected enterotoxemia were positive for endoparasites. Hence the recommendation to include parasitological examination in diagnostic considerations.

The operational conclusion they draw is a single one and they repeat it twice: the use of Category B antibiotics cannot be entirely avoided with this type of case, but stewardship guidelines should be followed wherever possible, and the priority is to perform culture and antibiogram, «especially when using Category B substances».

17 · Limiti dichiarati dagli autori

Citati, non riassunti. Sono cinque, e vanno riconosciuti come insolitamente franchi:

  1. «I nostri risultati riguardo alla frequenza delle malattie possono non riflettere del tutto la situazione dei pazienti camelidi in Austria, perché la UCR non è esclusivamente una struttura di primo livello e alcuni pazienti erano riferiti da veterinari pratici sul campo»;
  2. l'analisi non può fornire evidenza nel senso di uno studio randomizzato controllato, e non lo pretende;
  3. l'analisi copre trattamenti di quasi vent'anni fa, in un contesto normativo che nel frattempo è cambiato (campo 16, punto 2);
  4. «l'influenza del numero di certe malattie sulla proporzione delle sostanze antibiotiche usate deve essere tenuta presente» nel loro insieme di dati limitato — cioè: la composizione della casistica determina la composizione del consumo;

17 · Limitations declared by the authors

Quoted, not summarized. There are five, and they should be recognized as unusually candid:

  1. «Our results regarding disease frequency may not fully reflect the situation of camelid patients in Austria, because the UCR is not exclusively a primary-care facility and some patients were referred by field practitioners»;
  2. the analysis cannot provide evidence in the sense of a randomized controlled trial, and does not claim to;
  3. the analysis covers treatments from almost twenty years ago, in a regulatory context that has since changed (field 16, point 2);
  4. «the influence of the number of certain diseases on the proportion of antibiotic substances used must be kept in mind» within their limited dataset — that is: the composition of the caseload determines the composition of consumption;
  5. «the clinician's decision regarding the drug used to treat individual cases cannot be fully reflected in the clinical records, because many undocumented factors may influence this decision».

(g) Nessun esito, salvo tre eccezioni.

18 · Limitations added by the editorial team

(a) Two percentages in Table 3 are wrong, and the sum to 104.7% makes this verifiable: field 15.4. This is the only numerical error found in the whole article, and it is a printing one.

(b) Table 1 does not list all the molecules that Table 2 records: ofloxacin and chlortetracycline are missing from the EMA classification, despite appearing among the applications (field 15.1).

(c) The percentages in the text have denominators that change from paragraph to paragraph, without notice. "38 of 71" are digestive-system patients; "23 (32.3%)" is calculated on the same 71; "11 (34.4%)" is calculated on 32 and not on the 33 orthopedic patients; the "23.8%" and "81.0%" for the nervous system are calculated on 21 patients and sum to 104.8% because one patient has two diagnoses. None of these is an error — they are all arithmetically correct — but the reader must reconstruct the denominator every time, and anyone quoting an extracted sentence will quote a percentage without its denominator.

(d) The coding system was designed for cattle. GMON is the Austrian health monitoring system for cattle, adapted with additional categories. The consequences are visible: eye diseases end up under "central nervous system" together with otitis, because the category is "sense organs"; and a diagnosis frequent in camelids such as dental disease falls under "digestive system," which is anatomically correct but makes the leading cause of antibiotic use a composite category. The grid distorts the picture, and the article does not discuss it.

(e) Possible, undeclared overlap with CRL-J-01. The 12 camelids with eye disease in this caseload, 5 of which underwent enucleation, come from the Clinic for Ruminants of Vetmeduni Vienna between 2005 and 2019; the 68 camelids in CRL-J-01 come from the Department for Companion Animals and Horses of the same university, between 2009 and 2025. Eleven years overlap. Neither article cites the other. The editorial team cannot establish whether any animal appears in both caseloads, and notes that they should not be summed.

(f) The inclusion criterion cuts out the most interesting information. Only cases that received an antibiotic are included: so it is not known how many of the diagnoses frequent in the species do not require an antibiotic, nor what share of admitted camelids is discharged without one. The denominator 634 appears only once and is never used again. For a stewardship question — "how much do we prescribe?" — this is the missing figure.

(g) No outcomes, except three exceptions. The article reports cria mortality (28/44), the five orthopedic euthanasias, and the improvement of 13 of the 19 Candidatus M. haemolamae patients. For everything else it is not known what happened to the animals: this is a drug-use study, not an efficacy one, and the authors state as much. It remains true that the clinical reader will tend to read a table of antibiotics as though it contained an indication of efficacy. It does not.

(h) No stratification by species in the main tables. Tables 2 and 3 combine alpacas and llamas. The breakdown of diagnoses by species exists, but is in Figure 1, not read → [TO BE CHECKED AGAINST THE FULL TEXT].

19 · Finanziamento

Dichiarato, e nullo.

19 · Funding

Declared, and none. Full text: «The authors declare that no financial support was received for the research, authorship, and/or publication of this article». Crossref registers no funders. The open-access publication fees were covered by the University of Veterinary Medicine Vienna, a statement also explicit in the text.

20 · Conflitti di interesse

Dichiarati, e assenti.

20 · Conflicts of interest

Declared, and absent. Full text and PubMed record: «The authors declare that the research was conducted in the absence of any commercial or financial relationships that could be construed as a potential conflict of interest».

For a paper that names molecules and commercial categories, the statement carries weight: none of the five authors has industry affiliations, and all three institutions are public.

21 · Cosa manca rispetto allo standard di reporting

Rispetto a STROBE, letto il testo pieno:

  • percentuali sbagliate nella Tabella 3 (voce 16): due valori, campo 15.4 — l'unico difetto numerico;
  • classificazione incompleta nella Tabella 1: due molecole usate e non classificate;
  • denominatori non esplicitati riga per riga nel testo dei risultati (voce 14): campo 18c;
  • flusso dei partecipanti (voce 13): parziale — si sa che i camelidi ricoverati furono 634 e che 338 ricevettero antibiotici, ma non quanti furono esclusi e per quali diagnosi;
  • misure di precisione (voce 16): assenti, coerentemente con un lavoro descrittivo senza inferenza; nessun intervallo di confidenza compare;
  • dati mancanti (voce 12c): dichiarati per il sesso (1 lama, 7 alpaca) e per le vie di somministrazione (82 non registrate); non dichiarati altrove;
  • limiti (voce 19): presenti e ampi;
  • finanziamento (voce 22): presente;

21 · What is missing relative to the reporting standard

Relative to STROBE, having read the full text:

  • wrong percentages in Table 3 (item 16): two values, field 15.4 — the only numerical flaw;
  • incomplete classification in Table 1: two molecules used and not classified;
  • denominators not made explicit line by line in the results text (item 14): field 18c;
  • participant flow (item 13): partial — it is known that 634 camelids were admitted and that 338 received antibiotics, but not how many were excluded and for which diagnoses;
  • precision measures (item 16): absent, consistent with a descriptive paper without inference; no confidence interval appears;
  • missing data (item 12c): declared for sex (1 llama, 7 alpacas) and for routes of administration (82 not recorded); not declared elsewhere;
  • limitations (item 19): present and extensive;
  • funding (item 22): present;
  • data availability: declared limited — owners did not consent to publication of the data, available anonymized upon reasoned request.

⚠️ A dosage this fact sheet does not transcribe, and why. Discussing enterotoxemia, the authors cite a Swiss recommendation for benzylpenicillin, and report it in international units per day, subcutaneously, for 5-7 days or more, without stating the denominator per kilogram of body weight. A dose without reference to weight is not a dose: applied to an 8 kg cria and a 150 kg llama it would give two therapies differing by nearly twentyfold. House rules require dose, route, species and source together: here the complete unit is missing, and the number is not written down. Anyone needing to use that recommendation should go to the Swiss source cited by the article, not to this fact sheet and not to the article.

23 · Valutazione a stelle

23 · Star rating

Four independent axes, criteria from chapter 14 §11. Full text read; Supplementary Table S1 and the four figures not read.

AxisScoreRationale, with one fact inside
A — Scientific quality★★★☆☆Observational study with numbers declared at several levels (634 patients, 338 treated, 405 courses, 1,466 applications), explicit inclusion criterion, a named and citable diagnosis coding system (GMON), classification of molecules according to an external, verifiable standard (EMA categories), and two tables out of three that balance exactly when the sums are checked. Limitations are discussed extensively and unusually candidly. Does not rise to four because there is no inference, no comparison, and no measure of precision, and because a printing error remains in two percentages (field 15.4) and an incomplete classification (field 15.1).
B — Clinical utility★★★☆☆Does not offer a therapy, and does not claim to; it does offer a diagnostic priority order immediately usable in Europe (mouth, castration, limbs, cria, skin), three actionable clinical associations (CMhl and parasites, enterotoxemia and parasites, zinc-responsive dermatosis and cutaneous parasitosis) and a grid for auditing one's own prescribing — EMA categories cross-referenced with diagnosis — that a practice can apply to itself tomorrow. (The constraint of chapter 14 §11 is respected: no part of this score rests on doses, because the fact sheet reports none.)
C — Educational value★★★★☆Teaches what actually comes into a European camelid clinic, and teaches it with denominators in view; teaches that camelid dental disease does not present with pain; and teaches — unintentionally, but well — that the same reality measured in milligrams or in therapeutic acts tells two different stories (field 15.3). Does not reach five because it is not a reference text on any of the diseases it lists: it is a map, not a guide.
D — Relevance to the species★★★★☆Study on South American camelids, both species, in Central Europe, with 634 patients over fifteen years — it is the largest caseload in the dossier and the population is described enough (age, sex, annual trend) to assess its transferability to the Italian context, which is very close in climate, management and regulatory framework. Does not reach five because it is single-center and a referral clinic, and because alpacas and llamas are not distinguished in the main tables (field 18h).

24 · Bottom line

24 · Bottom line

It can be concluded that: in an Austrian university clinic, between 2005 and 2019, 338 of 634 admitted South American camelids received antibiotics, for a total of 405 treatment courses and 1,466 applications; that the two leading causes of antibiotic treatment were diseases of the digestive system (82 courses), more than half of which were dental diseases — 38 of 71 patients, with root abscesses and osteomyelitis and swelling as the cardinal symptom — and perioperative prophylaxis for castration (79 courses); that 28 of the 44 treated crias (63.6%) died or were euthanized; that every patient with Candidatus Mycoplasma haemolamae and every suspected case of clostridial enterotoxemia had a concurrent internal parasitosis; that 69.3% of applications were intramuscular and only one of 1,466 was oral; and that no antibiotic is authorized for these species in the European Union.

It cannot be concluded that: an antibiotic is effective for a given disease — this study measures no therapeutic outcome, except three declared exceptions; that these are the disease frequencies in Austrian camelids, because the clinic is a referral one and receives cases already selected for severity and chronicity; that the use of critical antibiotics has been contained, because the declared maximum annual 11.1% is calculated in milligrams while by number of applications Category B accounts for 31.7% of the entire period (editorial calculation); and that the prescribing choices described are a model to follow — the authors themselves write that some «may not always be justifiable today».

In particolare:

What this dossier did not seek

Chapter 14 §5.5 requires declaring what is missing. This dossier sought five articles, one per anatomical district, and did not seek everything else. In particular:

  • The foot has no fact sheet of its own. Chapter 10 §1.3 declares pododermatitis as a gap: no dedicated caseload on camelids exists. The biomechanics of the footpad is already covered in dossier D, and nail trimming has no schedulable primary literature. This is a literature gap, not a choice of this dossier — but the choice not to force it is this dossier's.
  • Teeth have no fact sheet here because they are dossier N, already published.
  • None of the five articles was sought in order to "find a result": they were chosen from the already-verified sources of chapter 10, and then queried one by one via the APIs. No systematic literature search was conducted for this dossier, and it is not excluded that better works exist on each of the five districts.
  • No literature in a language other than English was sought for this dossier. The Andean literature in Spanish is dossier M.
  • No manual, no monograph, no conference proceedings were consulted. Only peer-reviewed literature with a verifiable record.
  • No publisher website was opened, not even for the two subscription articles: the only sources are the programmatic interfaces of Crossref, Europe PMC, PubMed and DOAJ.

The markings, counted

41 [TO BE CHECKED AGAINST THE FULL TEXT] markings across five fact sheets — plus one citation of the convention in the reading note, which is not a marking on a data point.

Fact sheetFull textMarkings
CRL-J-01 — eye, Viennanot read14
CRL-J-02 — septic arthritisread, with supplementary material4
CRL-J-03 — meningeal wormread4
CRL-J-04 — chorioptic mange, CATnot read17
CRL-J-05 — diagnoses and antibiotics, Viennaread2

The two unread articles carry 31 of the 41 markings; the three read ones carry 10. The ratio does not measure the quality of the articles: it measures access. Whoever had the full texts of J-01 and J-04 would resolve most of those thirty-one in an afternoon, and the fact sheets would need to be rewritten — particularly fields 13, 14, 20 and 23 of CRL-J-04, and fields 9, 14, 17 and 18 of CRL-J-01.

  • CRL-J-01, CRL-J-03: gli articoli non ne contengono nel materiale letto;
  • CRL-J-04: i protocolli acaricidi esistono nell'articolo ma sono nel testo pieno non letto — per le dosi effettivamente pubblicate sui camelidi, con l'animale a cui sono state somministrate, l'indirizzo è il capitolo 10 §5.2 della KT;

The doses in this dossier, in one place

Only one fact sheet contains administration figures: CRL-J-02, and it gives them with molecule, dose per kilogram, route, frequency, duration and the individual animal — species and age — to which each was given, reconstructed from the article's supplementary tables. All uses are declared off-label by the authors themselves, molecule by molecule. They are not a recommended posology.

In the other four fact sheets no administration figure appears:

  • CRL-J-01, CRL-J-03: the articles contain none in the material read;
  • CRL-J-04: acaricide protocols exist in the article but are in the full text not read — for the doses actually published on camelids, with the animal to which they were administered, the reference is chapter 10 §5.2 of the KT;
  • CRL-J-05: the article reports a Swiss recommendation without the denominator per kilogram of body weight, and a dose without body weight is not a dose: the number was not transcribed, and the absence is explained in field 21 of that fact sheet.

The calculations redone by the editorial team

Every editorial calculation present in this dossier is labeled as such at the point where it appears, and is never attributed to the authors. There are nine, and they are listed here so that anyone disputing them can find them all together:

The three things dossier J adds to the collection

  1. A European figure where before there was only an American one — on the eye (CRL-J-01) and on overall clinical practice (CRL-J-05).
  2. A declared limitation where before there was a table of protocols — on chorioptic mange (CRL-J-04), with the sentence that precedes every dose table: no effective protocol has been verified twice.
  3. A geographic boundary where before there was an imported differential diagnosis — on the meningeal worm (CRL-J-03): the cycle does not close in Europe, the question of what would replace it is open and not resolved in the negative, and no one has yet asked it of a European camelid.
