È la conclusione letterale dell'articolo: «treatment with BZ should not be recommended without confirmed efficacy».

Camelid Research Studies APS — CRL editorial team · 31/08/2026

When fenbendazole failed one could always say the dose was too low, because no one has ever titrated it in camelids. Here that explanation no longer holds. Until now, whoever saw a fenbendazole failure in an alpaca herd could tell themselves two stories: either the worms are resistant, or the dose was too low — and since no one has ever titrated the right dose in camelids, the second story was always available and always reassuring. Codon 200 closes off that way out. Between 90 % and 100 % of the H. contortus genotypes, in seven herds out of seven, carry the benzimidazole-resistance allele. This is not an ambiguous phenotype that depends on how many milligrams were given: it is a gene, measured with a method that does not depend on treatment. In those herds benzimidazole does not fail because it is dosed wrong. It fails because the worms are resistant.

And the resistance is not camelid-specific: it is the same as in sheep. Codon 200 is the allele that dominates in H. contortus of sheep in central, western and northern Europe. This says two things at once: that alpacas have inherited a problem built elsewhere, and that the illusion of a separate camelid parasitology — with its own worms, its own epidemiology, its own rules — does not hold. The worm is the same, the genetics are the same. What changes is the patient (→ CRL-L-02), not the parasite.

The number to take back to the farm, however, is not 90 %: it is 15. The medians of the seven herds lie between 5 and 60 eggs per gram, with minimums below 5. This is a population with very low counts, and this overturns the entire ovine diagnostic logic. With those counts: (a) a FECRT on a single animal has no statistical meaning — it is a herd-level test; (b) methods with a low detection limit are needed, and the mini-FLOTAC at 5 EpG has here done what McMaster could not have done; (c) a negative count does not rule out haemonchosis. The European paradox of camelid haemonchosis lies entirely here: the most dangerous parasite lives in a population that sheds few eggs, and therefore is seen poorly exactly where it kills.

Overdispersion is today's good news, and it is actionable. 10 % of the animals produce most of the eggs, in all seven herds. This means that targeted selective treatment — treating that 10 % and leaving the rest as a refuge of worms still susceptible — is not a textbook ideal: it is the natural structure of these herds. And herd B, the one with the inconclusive outcome, is the case worth studying: the owner strictly adhered to a selective program, and in two years resistance to moxidectin has not advanced. One herd is not proof. But it is the only longitudinal observation this article offers, and it points in the direction the theory predicts.

What changes for a veterinarian in Italy, today. Four things, and none requires a new laboratory. (a) Stop treating on a calendar and start measuring. «Efficacy monitored after each treatment» is the authors' literal recommendation, and the WAAVP 2023 clinical protocol exists precisely to make the measurement feasible with few animals and few eggs. (b) Benzimidazole is not used without confirmed efficacy. This is the literal conclusion of the article: «treatment with BZ should not be recommended without confirmed efficacy». (c) The endectocide given for mange is an anthelmintic treatment that no one decided on. In herd C ivermectin was injected for a Psoroptes infestation, and it selected nematodes. The authors' recommendation — egg count after every ectoparasitic treatment with an endectocide, and if there are eggs, change the molecule — is perhaps the single easiest thing to introduce tomorrow into an Italian practice, and almost no one does it. (d) Monepantel is the last reserve, and must be treated as such. It held up in two herds out of two here, and disproved a previous suspicion. But it is the same molecule on which sheep have already burned through the illusion of invulnerability. Using it as a matter of routine is the way to lose it.

What this card cannot tell you. Whether the situation in Italy is the same. This was not searched for in this card, and it cannot be deduced from the article: seven German herds preselected for suspected resistance do not describe the peninsula. What can be said is that no one has measured it, and that an Italian survey with this exact protocol — mini-FLOTAC, WAAVP clinical protocol, dPCR on codon 200 — would cost little and would fill a real gap. It is, in the editors' judgment, the research project with the highest ratio of usefulness to cost that this issue suggests.

3 · Autori

Hinney Barbara · Wiedermann Sandra · Vaneev Xenia · Muhm Katharina · Joachim Anja · Wittek Thomas

Affiliazioni per singolo autore, come registrate nel intestazione dell'articolo dell'XML JATS (Europe PMC, 31/08/2026):

  • Department of Pathobiology, Institute of Parasitology, University of Veterinary Medicine Vienna, Vienna, Austria: Hinney, Wiedermann, Vaneev, Muhm, Joachim (cinque autori su sei).
  • Department for Farm Animals and Veterinary Public Health, University Clinic for Ruminants, University of Veterinary Medicine Vienna, Vienna, Austria: Wittek.

Hinney Barbara · Wiedermann Sandra · Vaneev Xenia · Muhm Katharina · Joachim Anja · Wittek Thomas

Affiliations per individual author, as recorded in the article header of the JATS XML (Europe PMC, 31/08/2026):

  • Department for Farm Animals and Veterinary Public Health, University Clinic for Ruminants, University of Veterinary Medicine Vienna, Vienna, Austria: Wittek.

A detail to be read, not skipped: the authors are from Vienna, the herds are German. This is no one's mistake — it is an Austrian group working on a German population — but whoever cites the article as "the Austrian study" gets the population wrong, and whoever cites it as "the German group" gets the authors wrong. It is the herds that are in Germany.

No ORCID is registered for any of the six authors, either in Crossref or in the PubMed Central XML. This is not a gap in the card: it is what the record contains.

9 · Specie e soggetti

9 · Species and subjects

Alpaca (Vicugna pacos) — no llama, no Old World camelid.

  • 7 herds in Germany, labeled A-G, with 9-46 animals each, that volunteered for the study.
  • 224 individual fecal samples, collected by the breeders between May and September 2020 and sent by courier, examined within 36 hours of arrival.
  • Sample size per herd (Table 1 of the article): A n=23 · B n=35 · C n=30 · D n=46 · E n=40 · F n=9 · G n=41.
  • Treatment groups: 8-12 animals per herd (Table 2).
  • ⚠️ Age, sex and weight of individual animals are not reported → to be checked against the full text — in the strict sense: the article does not report them at all, and the gap weighs on the ivermectin dosage (field 15).
  • The herds were not a random sample: they are the ones in which a previous study by the same group (Kultscher et al. 2019) had suspected resistance. The data should be read as an in-depth investigation into suspected cases, not as a prevalence figure.
  • Declared management context: feces removed daily, pastures not overstocked, no herd practiced co-grazing with sheep.

10 · Domanda clinica (PICO)

10 · Clinical question (PICO)

In European alpaca herds with suspected anthelmintic resistance [P], does the administration of moxidectin 0.4 mg/kg per os, monepantel 7.5 mg/kg per os or subcutaneous ivermectin [I], compared with the 99 % expected efficacy assumed by the WAAVP guidelines [C], produce a fecal egg count reduction compatible with susceptibility [O]? And is the resistance suspected by phenotype [secondary O] confirmed by the frequency of resistance alleles at codon 200 of H. contortus ß-tubulin?

11 · Disegno dello studio

11 · Study design

Prospective field follow-up study, with no untreated control group, with two independent components: (a) fecal egg count reduction tests (FECRT) conducted according to the clinical protocol of the WAAVP 2023 guideline, within a targeted selective treatment approach; (b) molecular analysis (digital PCR) of the benzimidazole-resistance SNPs on cultured larvae, independent of treatment.

12 · Level of evidence

Prospective observational study on a convenience sample, without randomization or control. For the reader this means: the efficacy values are estimates for individual herds, not population measures; each row of Table 2 concerns one herd only; and because the herds were preselected for suspected resistance, these percentages do not estimate the frequency of resistance in Germany. The molecular component is of a different and stronger level: it does not depend on treatment, does not depend on dose, and for this reason it is the only part of the article that stands on its own.

13 · Relevant reporting standard

STROBE would be the relevant standard for the observational component, and the authors do not declare it. They declare instead — and explicitly apply — the WAAVP 2023 guideline for the diagnosis of resistance by FECRT (Kaplan RM, Denwood MJ, Nielsen MK, Thamsborg SM, Torgerson PR, Gilleard JS et al., Vet Parasitol 2023;318:109936), with the clinical protocol and an expected efficacy of 99 %, and the fecrt.com software for the choice of protocol, the aggregation factor and the confidence intervals. For the animal component, ARRIVE 2.0 is not declared, and it was not needed: there was no experimental procedure.

14 · Metodo

Coproscopia.

Coproscopy. Mini-FLOTAC according to the protocol for herbivores, with a lower limit of detection of 5 eggs per gram (EpG). Strongyle eggs counted; other eggs and coccidian oocysts documented qualitatively. Sedimentation examination for trematode eggs.

Larval cultures and molecular analysis. Samples positive for strongyle eggs pooled per herd and subjected to coproculture for larval differentiation. Larvae examined with a digital PCR assay for the benzimidazole-resistance SNPs of H. contortus at codons 200, 167 and 198 (method of Hinney et al. 2023, Int J Parasitol Drugs Drug Resist 22:88-95). ⚠️ Post-treatment larvae not available or insufficient in herds A, E and G, because efficacy was nearly or exactly 100 %; in herd F the FECRT was not performed.

Treatment criterion. Minimum threshold 200 EpG; also included were animals with lower counts that the owner and/or attending veterinarian considered could benefit (poor body condition, anemia, signs of indigestion). This resulted in 8-12 animals per treatment group.

The molecular finding, in one line: the frequency of benzimidazole-resistance alleles at codon 200 of H. contortus ranges from 90.27 % to 100 % in all seven herds. Alleles at codon 167 are rare (the highest point value is 5.34 %; the others are declared below background level). No mutation at codon 198 was detected.

Comparison with the previous study (Kultscher et al. 2019, reported in the same Table 2): in the seven herds fenbendazole had given FECR between 7 % and 74 %, all classified as resistant; moxidectin had been susceptible in A and C and resistant in B; monepantel had given 99 % with suspected resistance in E and G. In the present study, the resistance suspected for monepantel in E is not confirmed, and that suspected for moxidectin in B is not confirmed but remains inconclusive.

16 · Authors' interpretation

The authors write that the high frequency of alleles at codon 200, "frequently observed in H. contortus of sheep in central, western and northern Europe," presented itself identically in the alpaca H. contortus: from this they assume that the treatment failures with benzimidazoles observed in those herds in 2018 were attributable "probably to resistance and not (or not exclusively) to an insufficient dose."

The authors conclude that monepantel remains effective in the herds examined; that with respect to moxidectin selection "might be ongoing" in one of the herds; and that in the ivermectin herd resistance "is very likely already well advanced."

The authors hypothesize that the low efficacy of ivermectin might also depend on the route of administration, observing that injectable products generally give lower efficacy than oral ones — while acknowledging that in camelid studies the results on plasma concentrations of moxidectin and ivermectin given orally or subcutaneously are contradictory, and that "the ideal route of administration of ivermectin in these hosts is not clear."

The authors recommend — and it is an operational recommendation, not a finding — that after each treatment of an ectoparasite with an endectocide an egg count be performed, and that if nematode eggs are counted, treatment with a different molecule be given to remove the resistant worms.

The authors note that overdispersion and the low counts make a targeted selective treatment approach feasible in almost all alpaca herds, and that selection of the animals should not be based on counts alone but also on clinical examination — body condition and, in herds predominant for H. contortus, FAMACHA© score.

17 · Limiti dichiarati dagli autori

17 · Limitations declared by the authors

Quoted, not summarized:

  • «The results of the FECRT should be interpreted with caution, since low counts together with a sample size below that required for the research protocol may compromise the validity of the FECRT.» The authors add that the clinical protocol is declared by the guidelines to be «as robust as the research one but more often leads to inconclusive outcomes», which is precisely what happened in one herd.
  • The authors openly discuss whether an expected efficacy of 99 % is applicable to alpacas: they note that the moxidectin titration reached a maximum efficacy of 98 % in alpacas, against 100 % reached in some llama herds, «so that the initially effective dose in alpacas remains unclear, potentially leading to an overestimation of the expected efficacy of moxidectin if the upper margin is set at 99 %.» They kept the margin because the WAAVP guidelines set 99 % also for goats, «a species in which a similar lack of knowledge about the originally effective dose exists.»
  • Declared taxonomic limitation: there are limited keys to differentiate the third-stage larvae of the genus Camelostrongylus, which "appear difficult to distinguish from Teladorsagia." In the present study the L3 were classified as Teladorsagia spp., "since Camelostrongylus spp. had not yet been described in central Europe."

18 · Limiti aggiunti dalla redazione

Campo distinto dal 17.

18 · Limitations added by the editors

A field distinct from 17.

  1. An internal discrepancy over the aggregation factor. The text writes «ĸ-values between 0.12 and 0.59»; Table 1 lists 0.42 · 0.25 · 0.15 · 0.50 · 0.39 · 0.43 · 0.59, whose minimum is 0.15. The value 0.12 does not appear in any row of the table. The card reports the interval from the table and marks the origin of the 0.12 as to be checked against the full text (probably in the supplementary file, not retrieved). This is a minor discrepancy, and it must be stated.
  2. The ivermectin dose is not a dose per kilogram. The article states 20 mg per animal, and the "0.2-0.4 mg/kg" is a conversion by the authors based on weights that the article does not publish. It is therefore not possible to know whether the resistant alpacas were under-dosed. Whoever cites this study as "ivermectin resistance at 0.2 mg/kg in alpacas" will be citing a conversion, not an administered dose.
  3. The ivermectin herd is confounded by two additional variables. It is the only one treated subcutaneously, the only one treated on the same day as sampling (the others 2-22 days later), and the only one in which the drug was chosen for a Psoroptes infestation rather than for nematodes. Three differences in a single herd: resistance is plausible, but the design does not allow it to be separated from route and timing.
  4. No untreated control group, as in almost all field FECRTs: the reduction is measured against the pre-treatment count of the same animal, not against untreated animals.
  5. One herd per molecule, in three cases out of three. Ivermectin: 1 herd. Monepantel: 2. Moxidectin: 3. The per-molecule conclusions rest on denominators that must be kept in mind when reading "monepantel is still effective."
  6. The sample is not random, and this goes beyond what the authors declare: the herds are the ones in which resistance was already suspected, and in addition they volunteered. The double selection makes the paper useful as an in-depth investigation and unusable as a prevalence estimate, in either direction.
  7. A link between the molecular finding and a contemporaneous benzimidazole treatment is missing. The SNPs were measured in 2020; the fenbendazole failures date to 2018 (Kultscher 2019). The authors' inference is reasonable but remains retrospective: no animal was treated with benzimidazole in this study.

19 · Finanziamento

«Not applicable (self-funded study)» — dichiarazione testuale nella sezione Funding dell'articolo.

19 · Funding

«Not applicable (self-funded study)» — textual statement in the Funding section of the article. No sponsor, no third-party role. The authors thank the Genomics Core Facility, VetCore of the University of Veterinary Medicine Vienna for the collaboration, and the owners of the participating alpacas.

20 · Conflitti di interesse

«The authors declare that they have no competing interests» — dichiarazione testuale.

20 · Conflicts of interest

«The authors declare that they have no competing interests» — textual statement. No author is affiliated with a manufacturer of anthelmintics (both affiliations are academic, §3). Given that the article makes efficacy judgments about four commercial molecules, the absence of industry ties is, here, an element that reinforces the finding.

21 · What is missing relative to the reporting standard (STROBE)

Bulleted list of what STROBE would require and is not present:

  • No declaration of adherence to STROBE (item 22 of the standard not applicable: the authors do not invoke it).
  • Characteristics of participants (item 14): age, sex, weight and productive status of the alpacas are not described in any form, not even aggregated.
  • Diagram or flow of participants (item 13): it is known that 224 animals were sampled and that 8-12 per herd were treated, but it is not possible to reconstruct how many animals were excluded, for what reason, and how many completed the second sampling.
  • Sample size calculation (item 10): absent in the prospective sense — the authors state a posteriori that the sample was sufficient for the clinical protocol but not for the research one.
  • Confounding factors (item 7): no confounding variable is defined or measured; herd management is described qualitatively in the discussion.
  • Sensitivity analysis (item 12e): absent.

What, on the other hand, is not missing, and must be credited: explicit inclusion criteria for treatment, diagnostic method with a stated detection limit, confidence intervals on every efficacy estimate, formal and cited definition of resistance, funding and conflicts declarations, and an ethics statement.

23 · Valutazione a stelle

23 · Star rating

Four independent axes, criteria of chapter 14 §11. The full text has been read in its entirety, so the rating does not carry the warning for a partial record.

AxisRatingRationale, with one fact inside
A — Scientific quality★★★☆☆Observational study with n declared per herd, explicit treatment selection criteria, diagnostic method with a stated detection limit, confidence intervals on every efficacy estimate, formal and cited definition of resistance, ethics, funding and conflicts statements all present. It does not rise to four because there is no randomization or control group, sample size is not justified prospectively, and the characteristics of the subjects (age, sex, weight) are entirely absent.
B — Clinical usefulness★★★★☆Result applicable with reasonable confidence in similar European herds, with explicit conditions of use: do not use benzimidazoles without confirmed efficacy, count eggs after an endectocide given for mange, adopt targeted selective treatment. Every dose cited has route, species and source. It does not reach five because it does not change a recommended practice with proportionate evidence: one herd per molecule in three cases out of three cannot bear that weight. (Constraint of chapter 14 §11 respected: B exceeds A by only one star.)
C — Educational value★★★★★Teaches, on real data, the hardest thing to teach in parasitology: how to distinguish resistance from under-dosing. It shows an ambiguous phenotypic test (FECRT on low counts, with an "inconclusive" outcome honestly labeled as such) alongside a genotypic test that does not depend on treatment, and shows that they answer different questions. It also contains a textbook case of the unintentional use of an anthelmintic (the endectocide for mange).
D — Relevance to the species★★★★★Study on alpacas, in continental Europe, in herds of 9 to 46 head with management — daily removal of feces, no co-grazing with sheep — comparable to that of Italian herds. The molecules are those available in Italy. Transferability is direct and requires no contextual adaptation.

24 · Bottom line

24 · Bottom line

It can be concluded that: in seven German alpaca herds preselected for suspected resistance, over 90 % of the Haemonchus contortus genotypes carry the benzimidazole-resistance allele at codon 200 in all seven, which demonstrates true resistance and not a under-dose; that ivermectin at 20 mg per head subcutaneously gave FECR 74.6 % (90 % CI 34.9-96.8; n=10 alpacas) in one herd, an outcome classified as resistance; and that moxidectin 0.4 mg/kg per os and monepantel 7.5 mg/kg per os maintained sustained efficacy in the herds where they were tested, with only one inconclusive outcome for moxidectin.

It cannot be concluded that: these values describe the frequency of resistance in alpaca herds in Germany, in Europe or in Italy — the sample is doubly selected (herds already suspected, and volunteers); nor that ivermectin is resistant "at 0.2-0.4 mg/kg," because that is not the administered dose but a conversion by the authors from a fixed dose per head, based on weights that the article does not publish; nor that monepantel is safe in general, having been tested in two herds.

  • Lavoro precedente dello stesso gruppo, verificato ma non schedato: Kultscher L, Hinney B, Schmäschke R, Joachim A, Wittek T. Current anthelmintic treatment is not always effective at controlling strongylid infections in German alpaca herds.

  • CRL-L-02 (this issue) — Wagener et al. 2024, anemia in South American camelids. It is the other half of this card: L-01 says the worm is resistant, L-02 says what that worm does to a host whose hematological thresholds are not those of sheep. They should be read in order.
  • CRL-L-05 (this issue) — Wagener et al. 2021, FAMACHA and BCS in European clinical practice. Hinney et al. recommend FAMACHA© for selecting the animals to treat in herds predominant for Haemonchus; L-05 measures how well that score actually holds up in Europe, and in alpacas the answer is less reassuring than the recommendation suggests. The two cards partly contradict each other, and it is useful to see this.
  • Previous work by the same group, verified but not filed as a card: Kultscher L, Hinney B, Schmäschke R, Joachim A, Wittek T. Current anthelmintic treatment is not always effective at controlling strongylid infections in German alpaca herds. Parasit Vectors 2019;12(1):330, DOI 10.1186/s13071-019-3588-3 (record copied from the bibliography of the JATS XML of Hinney 2024, entry 14, and cross-checked in Crossref on 31/08/2026: authors Kultscher Luise, Hinney Barbara, Schmäschke Ronald, Joachim Anja, Wittek Thomas, five, in order). ⚠️ The data from the two articles must not be mixed: the herds of Hinney 2024 are a subset of those of Kultscher 2019, and this card reports the 2024 values. Where Table 2 recalls the 2019 values, the card states so.
  • Verified but not filed context, for the low-count issue: Dittmer KE, Hinkson JA, Dwyer C, Adlington B, van Andel M. Prevalence of Candidatus Mycoplasma haemolamae, bovine viral diarrhoea virus, and gastrointestinal parasitism in a sample of adult New Zealand alpaca (Vicugna pacos). N Z Vet J 2018;66(1):9-15, DOI 10.1080/00480169.2017.1369912, PMID 28826356 (Crossref and Europe PMC, 31/08/2026; full text not accessible). In the abstract: 175 adult alpacas from 15 New Zealand herds, median FEC 50 eggs/g (min 0, max 4,700), 55/143 samples (38.5 %) at zero eggs/g and 33/143 (23.1 %) ≥ 250 eggs/g; Haemonchus spp. the most frequent nematode in the North Island larval cultures. Different hemisphere and climate, same phenomenon: the low count is not a German peculiarity.
